This study aimed to investigate the relationship between the intention to leave the profession(ILP) and ERI and job satisfaction among the medical staff in Qom Province.Methods: A descriptive-correlative study was conducted on 202 medical staff in Qom Province based on random sampling in 2018. Demographics checklist, standard ILP, job satisfaction, and Siegrist’s ERI questionnaires were used for data collection. The chisquared test, independent t-test, and one-way Analyses of Variance(ANOVA) were used to analyze data.Results: The mean age of employees was 32.04 ± 7.9 years, and 165(87.1%) of the employees were women. The results showed that the medical staff was willing to leave their profession at a moderate level(40.7 ± 10.3). There was no significant relationship between demographics and ILP. Nevertheless, a significant and inverse relationship was observed between ERI(r:0.318, P < 0.01) and ILP(r: 0.197, P < 0.01). Leave the profession(LP) had a negative correlation with the dimensions of job descriptive index(JDI) such as job, manager, coworker and wage score(P < 0.01, r:-0.147, r:-0.262, r:-0.292, r:-0.271, r:-0.396).Conclusions: According to the results, managers need to make sure that their working staff is rewarded as they deserve. According to the results, managers need to ensure that the reward factor is observed for the staff, while an ERI imbalance may contribute to ILP of the staff. On the other hand, it leads to job satisfaction.展开更多

Methods: The present descriptive study was performed from March 2016 t...Objective: To determine the antibiotic resistance patterns of the Acinetobacter(A.) baumannii complex isolates that cause the confirmed infection. Methods: The present descriptive study was performed from March 2016 to March 2018 in three referral hospitals in Isfahan, Iran. All A. baumannii complex strains isolated from different clinical samples were identified by conventional phenotypic methods and antibiotic susceptibility pattern was detected. After the clinical investigation, contaminated samples were excluded and the source(hospital/community) and site of the infection were determined. Data on antibiotic susceptibility testing were extracted from WHONET software and analysis was done with SPSS.Results: From 254 patients who had confirmed A. baumannii complex infection, 158(62.20%) cases were male, 27(10.63%) were less than 20 years old, 172(67.72%) had healthcare-associated infections and 96(37.79%) were admitted in intensive care units. The most frequent infection was bloodstream infections(111, 43.70%). Our results showed that most of the isolates were resistant to most of the antibiotics(more than 75.00%) and a lower rate of non-susceptibility was observed against minocycline(20, 44.44%) and colistin(0%). The rate of multidrug-resistant isolates was 88.97%. There was no significant difference between resistance of A. baumannii complex isolates according to age. However, the resistance to amikacin and minocycline and the rate of multidrug resistance(MDR) were significantly different between males and females. In patients with healthcare associated infection(HAI), MDR isolates were significantly different regarding admission in ICU ward. Resistance to levofloxacin and ciprofloxacin were lower in isolates from patients with bloodstream infections in comparison to other diagnoses.Conclusions: In our study, a high level of antibiotic resistance was detected in both community-acquired and healthcare-associated A. baumannii complex infections. Appropriate antibiotic prescription in a clinical setting is an essential need for the control and prevention of A. baumannii resistant infections.展开更多

Chronic renal failure as a developed and irreversible failure is usually progressive. End Stage Renal Disease (ESRD) is a situation when renal function is not sufficient to preserve one’s life leading to acute uremia resulting in dialysis and/or kidney transplantation for the patients. Although dialysis methods bring about complications for the patients, peritoneal dialysis is relatively cost-effective and more convenient to survive. The present study aimed to investigate (the) Etiologic factor of renal failure and imposed complications of peritoneal dialysis in the patients of Isfahan Alzahra hospital. Materials and Methods: It was a cross-sectional descriptive study conducted on 67 patients in Isfahan Peritoneal Dialysis Center. The data from the patients were collected through a two-section questionnaire whose first section was allocated to demographic information and the second part was related to the disease and complications of peritoneal dialysis. The collected data were analyzed by SPSS version 18. Results: The findings showed that there were 39 (58%) males and 28 (42%) females. Mean age of the subjects was 48 (18.8) years old. Regarding marital status, 65% were married and 35% were single. Considering complications, 28 subjects (53.5%) had infectious complications while 24 subjects (46.2%) had non-infectious complications. The highest frequency in etiologic factor of renal failure was diabetes mellitus in 29 subjects (43.9%) and the lowest for polycystic kidney in 2 subjects (3%);in addition, hypertension was observed in 51 subjects (77.3%). Discussion: The most frequent and important complication of peritoneal dialysis catheters is infection, which may result in catheter loss and discontinuation of peritoneal dialysis (PD). Diabetes and hypertension were found as the most important etiologic factors for renal failure. They should also be considered as two major risk factors in prevention of renal diseases.展开更多

The etiology of MS includes both genetic and environmental factors. A single nucleotide polymorphism (SNP) linked with autoimmune disorders predisposition, identified by Genome-Wide Association Study (GWAS) among genes which immunologically related are considerably over signified. The goal of the current study is investigation of the association between rs1800795 (-174 G/C) polymorphism in the promoter of IL6 gene variant with the risk of RRMS in a subset of Iranian population. In this case-control study, 110 healthy subjects and 110 patients with RRMS were included. DNA was extracted from blood samples and polymerase chain reaction (PCR) was used to amplify the fragment of interest contain rs1800795 SNP, restriction fragment length polymorphism (RFLP) method was performed for genotyping of the DNA samples with a specific restriction enzyme (NlaIII). SPSS for Windows software (version 18.0;SPSS, Chicago, IL) was used for statistical analysis. No significant differences were found between RRMS patients and healthy controls with respect to the distribution of the cytokine gene polymorphism investigated. Odds ratio adjusted for age, sex, and blood groups (except A blood group) has displayed similar outcomes. These results indicate that the rs1800795 SNP is not a susceptibility gene variant for development of RRMS in the Isfahan population. Further studies using new data on complex transcriptional interactions between IL-6 polymorphic sites are necessary to determine IL-6 haplotype influence on susceptibility to RRMS.展开更多
Plasma electrolytic oxidation(PEO)coatings were prepared on Al−Mg laminated macro composites(LMCs)using both unipolar and bipolar waveforms in an appropriate electrolyte for both aluminum and magnesium alloys.The tech...Plasma electrolytic oxidation(PEO)coatings were prepared on Al−Mg laminated macro composites(LMCs)using both unipolar and bipolar waveforms in an appropriate electrolyte for both aluminum and magnesium alloys.The techniques of FESEM/EDS,grazing incident beam X-ray diffraction(GIXRD),and electrochemical methods of potentiodynamic polarization and electrochemical impedance spectroscopy(EIS)were used to characterize the coatings.The results revealed that the coatings produced using the bipolar waveform exhibited lower porosity and higher thickness than those produced using the unipolar one.The corrosion performance of the specimens’cut edge was investigated using EIS after 1,8,and 12 h of immersion in a 3.5 wt.%NaCl solution.It was observed that the coating produced using the bipolar waveform demonstrated the highest corrosion resistance after 12 h of immersion,with an estimated corrosion resistance of 5.64 kΩ·cm^(2),which was approximately 3 times higher than that of the unipolar coating.Notably,no signs of galvanic corrosion were observed in the LMCs,and only minor corrosion attacks were observed on the magnesium layer in some areas.展开更多
As we all are aware of the deleterious and ill effects of various chemical pesticides and inorganic fertilizers,the agricultural community has to move towards an alternative having a sustainable and eco-friendly appro...As we all are aware of the deleterious and ill effects of various chemical pesticides and inorganic fertilizers,the agricultural community has to move towards an alternative having a sustainable and eco-friendly approach.Thus in this study,we analyzed the efficiency of growth promoting microorganism products(GPMPs)using Tricuran-P(Trichoderma harzianum T-22);Nitroxin(Azospirilium and Azotobacter rhizobacteria in combination);Effective Micro-Organisms(EM)(photosynthetic bacteria,lactobacillus,Streptomyces,Actinomycetes,yeast,etc.);Phytohumic;Formycine Gold PX 20%;and Fenamiphos(Nemacur)on the tomato growth and physico-chemical properties of the plant and the soil at a greenhouse condition in interaction with root-knot nematodes(RKNs),Meloidogyne javanica pathosystem along with irrigation water at the interval of every three weeks for three times.The results showed that the lowest second-stage juveniles(J2)reproduction factor rate in the soil was related to Nemacur 18%and Tricuran-P 19%,followed by Formycine with 30%reduction in comparison to inoculated controls,respectively.Increasing the concentration of Formycine led to a significant decrease in the reproduction factor rate of J2 in the soil and the roots.Furthermore,in the biomass traits,the treatments showed a significant effect on the related traits,which were increased with the Formycine increase in the growth parameters of the biomass traits.The results of the effect of different treatments on physico-chemical traits of the soil indicated that the treatments had a significant difference on all the relevant traits except the concentration of phosphorus(P_(2)O_(5))and potassium(K_(2)O).Whereas,the content of elements in the tomato plants showed a significant difference on all the evaluated traits.At the same time,the correlation between the attributes in the tomato plant with the soil indicated that the plant’s N had a positive and significant correlation with Organic Carbon(OC),total N,P_(2)O_(5),K_(2)O and the soil moisture;and a negative and significant correlation with the soil’s EC(electrical conductivity).Moreover,the content of K,Ca,and Mg of the plant had a positive and significant correlation with the soil moisture,and a negative and significant correlation with the soil’s EC.Cumulatively,GPMPs not only are enable to shift over the soil environments in the favor of the plant growth,but also similarly modulate immunity against RKNs,by virtue of which Formycine and others can compete relatively compared to Fenamiphos(Nemacur)and other relevant GPMPs in this research.展开更多
Mimicking the hierarchical structure of the skin is one of the most important strategies in skin tissue engineering.Monolayer wound dressings are usually not able to provide several functions at the same time and cann...Mimicking the hierarchical structure of the skin is one of the most important strategies in skin tissue engineering.Monolayer wound dressings are usually not able to provide several functions at the same time and cannot meet all clinical needs.In order to maximize therapeutic efficiency,herein,we fabricated a Tri-layer wound dressing,where the middle layer was fabricated via 3D-printing and composed of alginate,tragacanth and zinc oxide nanoparticles(ZnO NPs).Both upper and bottom layers were constructed using electrospinning technique;the upper layer was made of hydrophobic polycaprolactone to mimic epidermis,while the bottom layer consisted of Soluplus■ and insulin-like growth factor-1(IGF-1)to promote cell behavior.Swelling,water vapor permeability and tensile properties of the dressings were evaluated and the Tri-layer dressing exhibited impressive antibacterial activity and cell stimulation following by the release of ZnO NPs and IGF-1.Additionally,the Tri-layer dressing led to faster healing of full-thicknesswound in ratmodel compared to monolayer and Bilayer dressings.Overall,the evidence confirmed that the Trilayer wound dressing is extremely effective for full-thickness wound healing.展开更多
Objective:To investigate the influence of age,body mass index(BMI),varicocele,diabetes,tobacco use,and environmental occupational risks on sperm DNA fragmentation index(DFI)and its association with semen parameters an...Objective:To investigate the influence of age,body mass index(BMI),varicocele,diabetes,tobacco use,and environmental occupational risks on sperm DNA fragmentation index(DFI)and its association with semen parameters and in vitro fertility(IVF)outcomes.Methods:This cross-sectional study was conducted on 116 infertile men.Conventional semen parameters were analyzed according to the World Health Organization criteria.Sperm DNA fragmentation was evaluated using sperm chromatin dispersion method.After visiting the infertility center,the men's height and weight were measured,and blood tests were performed to check for diabetes,and medical records were reviewed for varicocele,tobacco use,and type of occupation.The sperm was then examined for DFI.Then,the association between sperm DFI and IVF failure rate was investigated.Results:The study showed a significant association between DFI≥20%with BMI(OR 1.134,95%CI 1.04-1.24,P=0.006),varicocele(OR 4.330,95%CI 1.25-14.96,P=0.021),tobacco use(OR 3.066,95%CI 1.06-8.90,P=0.039)and environmental and occupational risks(OR 2.694,95%CI 1.08-6.75,P=0.034)as well as sperm motility(P<0.05).Although the amount of DNA damage increased in those aged≥40 years,there was no significant association between the amount of DFI≥≥20%and age,diabetes,sperm volume and concentration,morphology and progressive rate(P>0.05).The IVF failure rate was higher in people with a DFI≥20%.Conclusions:Factors such as BMI,varicocele,improper working conditions and environment cause damage to sperm DNA,and DFI≥20%damage can have adverse effects on IVF outcomes.展开更多

Unfavorablesystemic side-effects of chemotherapeutic agents and susceptibility to the degradation of small interfering RNAs(si RNAs), which can knock down a specific gene involved in the disease, have hampered their clinical application. So, it could be beneficial to develop an efficient carrier for the stabilization and specific delivery of drugs and si RNA to cells. Targeted nanoparticles have gained considerable attention as an efficient drug and gene delivery system, which is due to their capability in achieving the highest accumulation of cytotoxic agents in tumor tissue, modifiable drug pharmacokinetic- and bio-distribution, improved effectiveness of treatment, and limited sideeffects. Recent studies have shed more light on the advantages of novel drug loaded carrier systems vs free drugs. Most of the animal studies have reported improvement in treatment efficacy and survival rate using novel carrier systems. Targeted delivery may be achieved passively or actively. In passive targeting, no ligand as homing device is used, while targeting is achieved by incorporating the therapeutic agent into a macromolecule or nanoparticle that passively reaches the target organ. However, in active targeting, the therapeutic agent or carrier system is conjugated to a tissue or cell-specific receptor which is overexpressed in a special malignancy using a ligand called a homing device. This review covers a broad spectrum of targeted nanoparticles as therapeutic and nonviral si RNA delivery systems, which are developed for enhanced cellular uptake and targeted gene silencing in vitro and in vivo and their characteristics and opportunities for the clinical applications of drugs and therapeutic si RNA are discussed in this article. Asialoglycoprotein receptors, low-density lipoprotein, ganglioside GM1 cell surface ligand, epidermal growth factor receptor receptors, monoclonal antibodies, retinoic acid receptors, integrin receptors targeted by Arg-Gly-Asp peptide, folate, and transferrin receptors are the most widely studied cell surface receptors which are used for the site specific delivery of drugs and si RNA-based therapeutics in HCC and discussed in detail in this article.展开更多

In the present study, the effect of ginger (rhizome of Zingiber officinale Roscoe) volatile oil on a rat model ...OBJECTIVE: Ulcerative colitis is a chronically recurrent inflammatory bowel disease of unknown origin. In the present study, the effect of ginger (rhizome of Zingiber officinale Roscoe) volatile oil on a rat model of colitis was evaluated. METHODS: Volatile oil of ginger with doses of 100, 200, and 400 mg/kg, prednisolone (4 mg/kg), or vehicle were administered orally to groups of male Wistar rats (n = 6) for 5 d. Animals were randomly divided into 6 groups, each group consisting of 6 rats. Colitis was induced by intracolonic instillation of 2 mL of 4% (v/v) acetic acid solution. All rats were sacrificed 24 h later and the tissue injuries were assessed macroscopically and histopathologically. RESULTS: Ginger volatile oil with all doses reduced colon weight/length ratio (P 〈 0.01) and the effects were similar to the reference drugs. Higher oral doses of volatile oil (200 and 400 mg/kg) reduced ulcer severity (P 〈 0.05 and P 〈 0.01), ulcer area (P 〈 0.01) and ulcer index (P 〈 0.01). On the other hand, evaluation of microscopic scores showed that the dose of 400 mg/kg of volatile oil was effective to reduce inflammation severity (P 〈 0.01) and inflammation extent (P 〈 0.05) compared to the control group. CONCLUSION: It is concluded that ginger volatile oil could effectively reduce symptoms of experimental colitis in a dose-dependent manner.展开更多

The undifferentiated stem cells were seeded into mandibular bone through-andthrough defects of 4 dogs. Similar defects in control group were filled with cell-free collagen scaffold. After 12 wk, biopsies were taken and morphometric analysis was performed. The percentage of new bone formation and foreign body reaction were measured in each case. The data were subject to statistical analysis using the Mann-Whitney U and Kruskalwalis statistical tests. Differences at P < 0.05 was considered as significant level.RESULTS: There were no significant differences between control and SHED-seeded groups in connective tissue(P = 0.248), woven bone(P = 0.248) and compact bone(P = 0.082). There were not any side effects in transplanted SHED group such as teratoma or malignancy and abnormalities in this period.CONCLUSION: SHEDs which had been isolated and characterized 5 years ago and stored with cryopreservation banking were capable of proliferation and osteogenesis after 5 years, and no immune response was observed after three months of seeded SHEDs.展开更多
MXenes,transition metal carbides and nitrides with graphene-like structures,have received considerable attention since their first discovery.On the other hand,Graphene has been extensively used in biomedical and medic...MXenes,transition metal carbides and nitrides with graphene-like structures,have received considerable attention since their first discovery.On the other hand,Graphene has been extensively used in biomedical and medicinal applications.MXene and graphene,both as promising candidates of two-dimensional materials,have shown to possess high potential in future biomedical applications due to their unique physicochemical properties such as superior electrical conductivity,high biocompatibility,large surface area,optical and magnetic features,and extraordinary thermal and mechanical properties.These special structural,functional,and biological characteristics suggest that the hybrid/composite structure of MXene and graphene would be able to meet many unmet needs in different fields;particularly in medicine and biomedical engineering,where high-performance mechanical,electrical,thermal,magnetic,and optical requirements are necessary.However,the hybridization and surface functionalization should be further explored to obtain biocompatible composites/platforms with unique physicochemical properties,high stability,and multifunctionality.In addition,toxicological and long-term biosafety assessments and clinical translation evaluations should be given high priority in research.Although very limited studies have revealed the excellent potentials of MXene/graphene in biomedicine,the next steps should be toward the extensive research and detailed analysis in optimizing the properties and improving their functionality with a clinical and industrial outlook.Herein,different synthesis/fabrication methods and performances of MXene/graphene composites are discussed for potential biomedical applications.The potential toxicological effects of these composites on human cells and tissues are also covered,and future perspectives toward more successful translational applications are presented.The current state-of-the-art biotechnological advances in the use of MXene-Graphene composites,as well as their developmental challenges and future prospects are also deliberated.Due to the superior properties and multifunctionality of MXene-graphene composites,these hybrid structures can open up considerable new horizons in future of healthcare and medicine.展开更多

For data collection, a total of 4763 individuals were asked to complete a questionnaire on demographics, personality traits, life events, coping with stress, social support, and psychological outcomes such as anxiety and depression. To evaluate the comprehensive relation-ship between the variables, a path model was fitted.RESULTS: The standard electronic modules showed that personality traits and perceived stress are important determinants of psychological outcomes. Social support and coping strategies were demonstrated to reduce the increasing cumulative positive effects of neuroticism and perceived stress on the psychological outcomes and enhance the protective effect of extraversion through decreasing the positive effect of perceived stress on the psychological outcomes. CONCLUSION: Personal resources play an important role in reduction and prevention of anxiety and depression. In order to improve the psychological health, it is necessary to train and reinforce the adaptive coping strategies and social support, and thus, to moderate negative personality traits.展开更多
AIM:To explore the association between consumption of spicy foods and prevalence of irritable bowel syndrome(IBS)among Iranian adults.METHODS:In this cross-sectional study,data from4763 Iranian adult participants were...AIM:To explore the association between consumption of spicy foods and prevalence of irritable bowel syndrome(IBS)among Iranian adults.METHODS:In this cross-sectional study,data from4763 Iranian adult participants were used.Consumption of spicy foods was estimated using a dietary habits questionnaire that included a question on spicy foods consumption:"how frequently do you use spicy foods(pepper,curry,ginger,cinnamon and turmeric)during a week?"Participants could respond to the question by choosing one of these choices:never,1-3 times,4-6times,7-9 times,or more than 10 times per week.A modified Persian version of the RomeⅢquestionnaire was used to determine the prevalence of IBS.RESULTS:IBS was prevalent in 21.7%(18.6%of men and 24.1%of women)of the study population.After controlling for potential confounders including dietary behaviors,those consuming spicy foods≥10 times per week were 92%more likely to have IBS compared with those who never consumed spicy foods(OR=1.92;95%CI:1.23-3.01,Ptrend<0.01).The association remained significant even after taking lactose intolerance into account(OR=1.85;95%CI:1.18-2.90,Ptrend<0.01).Stratified analysis by gender revealed that the association between consumption of spicy foods and IBS was not significant in men;however,a significant association was found among women after taking potential cofounders,including meal regularity and lactose intolerance,into account.Women who consumed spicy foods≥10 times per week were two times more likely to have IBS compared with those who never consumed spicy foods(OR=2.03;95%CI:1.09-3.77,Ptrend=0.02).CONCLUSION:Consumption of spicy foods is directly associated with IBS,particularly in women.Further,prospective studies are warranted to(1)examine this association in other populations;and(2)evaluate whether dietary interventions,for example a reduction in spice consumption,would improve IBS symptoms.展开更多

METHODS In a population based cohort study, 1765 with mean ± SD age: 42.32 ± 6.18 healthy participants were followed up from 2003 till 2013(n = 960). Anthropometric and biochemical measures of participants were evaluated regularly during the follow up period. BMI and WC measures at baseline and diabetes and prediabetes status of participants at 2013 were determined. Multivariable logistic regression analysis was used for determining the risk of diabetes and prediabetes considering important potential confounding variables. Receiver operatingcharacteristic curve analysis was conducted to determine the best cut of values of BMI and WC for diabetes and prediabetes. RESULTS At 2013, among participants who had complete data, 45 and 307 people were diabetic and prediabetic, respectively. In final fully adjusted model, BMI value was a significant predictor of diabetes(RR = 1.39, 95%CI: 1.06-1.82 and AUC = 0.68, 95%CI: 0.59-0.75; P < 0.001) however not a significant risk factor for prediabetes. Also, WC was a significant predictor for diabetes(RR = 1.2, 95%CI: 1.05-1.38 and AUC = 0.67, 95%CI: 0.6-0.75) but not significant risk factor for prediabetes. Similar results were observed in both genders.CONCLUSION General and abdominal obesity are significant risk factors for diabetes in future.展开更多

The nBCP with composition of 63/37 (...In this paper, preparation of nano-biphasic calcium phosphate (nBCP), mechanical behavior and load-bearing of poly (lactide-co-glycolide) (PLGA) and PLGA/nBCP are presented. The nBCP with composition of 63/37 (w/w) HA/-TCP (hydroxyapatite/fl-tricalcium phosphate) was produced by heating of bovine bone at 700℃. Composite scaffolds were made by using PLGA matrix and 10-50 wt% nBCP powders as reinforcement material. All scaffolds were prepared by thermally induced solid-liquid phase separation (TIPS) at -60~C under 4 Pa (0.04 mbar) vacuum. The results of elastic modulus testing were adjusted with Ishai-Cohen and Narkis models for rigid polymeric matrix and compared to each other. PLGA/nBCP scaffolds with 30 wt% nBCP showed the highest value of yield strength among the scaffolds. In addition, it was found that by increasing the nBCP in scaffolds to 50 wt%, the modulus of elasticity was highly enhanced. However, the optimum value of yield strength was obtained at 30 wt% nBCP, and the agglomeration of reinforcing particles at higher percentages caused a reduction in yield strength. It is clear that the elastic modulus of matrix has the significant role in elastic modulus of scaffolds, as also the size of the filler particles in the matrix.展开更多

The sample collection was done in outpatient clinics in three university hospitals in Isfahan, Iran. We used a demographic questionnaire containingquestions that evaluated osteoporosis-related life habits, including exercise, smoking, intake of calcium and vitamin D supplements and so on. We also used the Osteoporosis Knowledge Assessment Tool to measure osteoporosis knowledge of women.RESULTS: The mean level of knowledge about awareness of osteoporosis, its risk factors and preventive factors were 56, 55 and 22, respectively. The relationship of education level and awareness of osteoporosis, its risk factors and preventive factors was significant, with R = 0.76, R = 0.73 and R = 0.83, respectively(P < 0.001). The relationship of education level and osteoporosis-related life habits was not significant(R = 0.03 and P = 0.56). The relationship of osteoporosis-related life habits and awareness of osteoporosis and its risk factors was significant, with R = 16%, P = 0.006 and R = 16%, P = 0.008, respectively, but the relationship of osteoporosis-related life habits and preventive factors was not significant(R = 0, P = 0.99).CONCLUSION: Iranian women with a higher education level have significantly better knowledge about osteoporosis than women with a lower educational level but they do not use this knowledge in their life.展开更多

The present study was conducted to evaluate the selected risk processes of emergency surgery department of a treatment-educational Qaem center in Mashhad by using analysis method of the conditions and failure effects in health care.METHODS: In this study, in combination(qualitative action research and quantitative crosssectional), failure modes and effects of 5 high-risk procedures of the emergency surgery department were identified and analyzed according to Healthcare Failure Mode and Effects Analysis(HFMEA). To classify the failure modes from the "nursing errors in clinical management model(NECM)", the classification of the effective causes of error from "Eindhoven model" and determination of the strategies to improve from the "theory of solving problem by an inventive method" were used. To analyze the quantitative data of descriptive statistics(total points) and to analyze the qualitative data, content analysis and agreement of comments of the members were used.RESULTS: In 5 selected processes by "voting method using rating", 23 steps, 61 sub-processes and 217 potential failure modes were identifi ed by HFMEA. 25(11.5%) failure modes as the high risk errors were detected and transferred to the decision tree. The most and the least failure modes were placed in the categories of care errors(54.7%) and knowledge and skill(9.5%), respectively. Also, 29.4% of preventive measures were in the category of human resource management strategy.CONCLUSION: "Revision and re-engineering of processes", "continuous monitoring of the works", "preparation and revision of operating procedures and policies", "developing the criteria for evaluating the performance of the personnel", "designing a suitable educational content for needs of employee", "training patients", "reducing the workload and power shortage", "improving team communication" and "preventive management of equipment's" were on the agenda as the guidelines.展开更多

Therefore, the present study was carried out with the aim to prioritize factors related to women's mental health during an earthquake. METHODS: This study was conducted using the Delphi method in 2017. Participants consisted of experts including psychologists, counselors and psychiatrists, social medicine specialists, and psychiatric-mental health nurses with experience in service and disaster awareness, especially earthquake. They were selected using purposive and snowball sampling methods. The Delphi method was used in 2 rounds with 21 components and the fi nal attendance of 19 participants and the collective agreement of 75%. RESULTS: Of the 21 factors related to women's mental health during the earthquake, the following components were, respectively, preferred by the participants: psychological well-being training, group relationships and support of women in emergency situations, fair treatment in the provision of rights and services to women, crisis awareness and management of behavior and diffi cult conditions, and social security during disasters with the total mean standard deviation (SD) of 13.4 (2.4), 13.0 (2.4), 12.7 (2.5), 12.5 (2.4), and 12.3 (2.2), respectively. CONCLUSION: Training in the fi eld of psychology and disasters, and social and cultural factors were prioritized among factors related to women's mental health during an earthquake. Therefore, the results of this study can be applied by the planners and executives of mental health, women and disasters, and the women's community itself.展开更多

文摘Objective: The effort–reward imbalance(ERI) model claims that work that has the characteristics of high effort and low reward has a mutual defect between input and output and this imbalance may result in sustained and long-lasting results. This study aimed to investigate the relationship between the intention to leave the profession(ILP) and ERI and job satisfaction among the medical staff in Qom Province.Methods: A descriptive-correlative study was conducted on 202 medical staff in Qom Province based on random sampling in 2018. Demographics checklist, standard ILP, job satisfaction, and Siegrist’s ERI questionnaires were used for data collection. The chisquared test, independent t-test, and one-way Analyses of Variance(ANOVA) were used to analyze data.Results: The mean age of employees was 32.04 ± 7.9 years, and 165(87.1%) of the employees were women. The results showed that the medical staff was willing to leave their profession at a moderate level(40.7 ± 10.3). There was no significant relationship between demographics and ILP. Nevertheless, a significant and inverse relationship was observed between ERI(r:0.318, P < 0.01) and ILP(r: 0.197, P < 0.01). Leave the profession(LP) had a negative correlation with the dimensions of job descriptive index(JDI) such as job, manager, coworker and wage score(P < 0.01, r:-0.147, r:-0.262, r:-0.292, r:-0.271, r:-0.396).Conclusions: According to the results, managers need to make sure that their working staff is rewarded as they deserve. According to the results, managers need to ensure that the reward factor is observed for the staff, while an ERI imbalance may contribute to ILP of the staff. On the other hand, it leads to job satisfaction.
文摘Objective: To determine the antibiotic resistance patterns of the Acinetobacter(A.) baumannii complex isolates that cause the confirmed infection. Methods: The present descriptive study was performed from March 2016 to March 2018 in three referral hospitals in Isfahan, Iran. All A. baumannii complex strains isolated from different clinical samples were identified by conventional phenotypic methods and antibiotic susceptibility pattern was detected. After the clinical investigation, contaminated samples were excluded and the source(hospital/community) and site of the infection were determined. Data on antibiotic susceptibility testing were extracted from WHONET software and analysis was done with SPSS.Results: From 254 patients who had confirmed A. baumannii complex infection, 158(62.20%) cases were male, 27(10.63%) were less than 20 years old, 172(67.72%) had healthcare-associated infections and 96(37.79%) were admitted in intensive care units. The most frequent infection was bloodstream infections(111, 43.70%). Our results showed that most of the isolates were resistant to most of the antibiotics(more than 75.00%) and a lower rate of non-susceptibility was observed against minocycline(20, 44.44%) and colistin(0%). The rate of multidrug-resistant isolates was 88.97%. There was no significant difference between resistance of A. baumannii complex isolates according to age. However, the resistance to amikacin and minocycline and the rate of multidrug resistance(MDR) were significantly different between males and females. In patients with healthcare associated infection(HAI), MDR isolates were significantly different regarding admission in ICU ward. Resistance to levofloxacin and ciprofloxacin were lower in isolates from patients with bloodstream infections in comparison to other diagnoses.Conclusions: In our study, a high level of antibiotic resistance was detected in both community-acquired and healthcare-associated A. baumannii complex infections. Appropriate antibiotic prescription in a clinical setting is an essential need for the control and prevention of A. baumannii resistant infections.
文摘Introduction: Worldwide specific diseases are jeopardizing people’s health in the world as well as in Iran. Chronic renal failure as a developed and irreversible failure is usually progressive. End Stage Renal Disease (ESRD) is a situation when renal function is not sufficient to preserve one’s life leading to acute uremia resulting in dialysis and/or kidney transplantation for the patients. Although dialysis methods bring about complications for the patients, peritoneal dialysis is relatively cost-effective and more convenient to survive. The present study aimed to investigate (the) Etiologic factor of renal failure and imposed complications of peritoneal dialysis in the patients of Isfahan Alzahra hospital. Materials and Methods: It was a cross-sectional descriptive study conducted on 67 patients in Isfahan Peritoneal Dialysis Center. The data from the patients were collected through a two-section questionnaire whose first section was allocated to demographic information and the second part was related to the disease and complications of peritoneal dialysis. The collected data were analyzed by SPSS version 18. Results: The findings showed that there were 39 (58%) males and 28 (42%) females. Mean age of the subjects was 48 (18.8) years old. Regarding marital status, 65% were married and 35% were single. Considering complications, 28 subjects (53.5%) had infectious complications while 24 subjects (46.2%) had non-infectious complications. The highest frequency in etiologic factor of renal failure was diabetes mellitus in 29 subjects (43.9%) and the lowest for polycystic kidney in 2 subjects (3%);in addition, hypertension was observed in 51 subjects (77.3%). Discussion: The most frequent and important complication of peritoneal dialysis catheters is infection, which may result in catheter loss and discontinuation of peritoneal dialysis (PD). Diabetes and hypertension were found as the most important etiologic factors for renal failure. They should also be considered as two major risk factors in prevention of renal diseases.
文摘Multiple sclerosis (MS) is an inflammatory demyelinating disease of central nervous system (CNS) that mostly affects young adults. The etiology of MS includes both genetic and environmental factors. A single nucleotide polymorphism (SNP) linked with autoimmune disorders predisposition, identified by Genome-Wide Association Study (GWAS) among genes which immunologically related are considerably over signified. The goal of the current study is investigation of the association between rs1800795 (-174 G/C) polymorphism in the promoter of IL6 gene variant with the risk of RRMS in a subset of Iranian population. In this case-control study, 110 healthy subjects and 110 patients with RRMS were included. DNA was extracted from blood samples and polymerase chain reaction (PCR) was used to amplify the fragment of interest contain rs1800795 SNP, restriction fragment length polymorphism (RFLP) method was performed for genotyping of the DNA samples with a specific restriction enzyme (NlaIII). SPSS for Windows software (version 18.0;SPSS, Chicago, IL) was used for statistical analysis. No significant differences were found between RRMS patients and healthy controls with respect to the distribution of the cytokine gene polymorphism investigated. Odds ratio adjusted for age, sex, and blood groups (except A blood group) has displayed similar outcomes. These results indicate that the rs1800795 SNP is not a susceptibility gene variant for development of RRMS in the Isfahan population. Further studies using new data on complex transcriptional interactions between IL-6 polymorphic sites are necessary to determine IL-6 haplotype influence on susceptibility to RRMS.
文摘Plasma electrolytic oxidation(PEO)coatings were prepared on Al−Mg laminated macro composites(LMCs)using both unipolar and bipolar waveforms in an appropriate electrolyte for both aluminum and magnesium alloys.The techniques of FESEM/EDS,grazing incident beam X-ray diffraction(GIXRD),and electrochemical methods of potentiodynamic polarization and electrochemical impedance spectroscopy(EIS)were used to characterize the coatings.The results revealed that the coatings produced using the bipolar waveform exhibited lower porosity and higher thickness than those produced using the unipolar one.The corrosion performance of the specimens’cut edge was investigated using EIS after 1,8,and 12 h of immersion in a 3.5 wt.%NaCl solution.It was observed that the coating produced using the bipolar waveform demonstrated the highest corrosion resistance after 12 h of immersion,with an estimated corrosion resistance of 5.64 kΩ·cm^(2),which was approximately 3 times higher than that of the unipolar coating.Notably,no signs of galvanic corrosion were observed in the LMCs,and only minor corrosion attacks were observed on the magnesium layer in some areas.
文摘As we all are aware of the deleterious and ill effects of various chemical pesticides and inorganic fertilizers,the agricultural community has to move towards an alternative having a sustainable and eco-friendly approach.Thus in this study,we analyzed the efficiency of growth promoting microorganism products(GPMPs)using Tricuran-P(Trichoderma harzianum T-22);Nitroxin(Azospirilium and Azotobacter rhizobacteria in combination);Effective Micro-Organisms(EM)(photosynthetic bacteria,lactobacillus,Streptomyces,Actinomycetes,yeast,etc.);Phytohumic;Formycine Gold PX 20%;and Fenamiphos(Nemacur)on the tomato growth and physico-chemical properties of the plant and the soil at a greenhouse condition in interaction with root-knot nematodes(RKNs),Meloidogyne javanica pathosystem along with irrigation water at the interval of every three weeks for three times.The results showed that the lowest second-stage juveniles(J2)reproduction factor rate in the soil was related to Nemacur 18%and Tricuran-P 19%,followed by Formycine with 30%reduction in comparison to inoculated controls,respectively.Increasing the concentration of Formycine led to a significant decrease in the reproduction factor rate of J2 in the soil and the roots.Furthermore,in the biomass traits,the treatments showed a significant effect on the related traits,which were increased with the Formycine increase in the growth parameters of the biomass traits.The results of the effect of different treatments on physico-chemical traits of the soil indicated that the treatments had a significant difference on all the relevant traits except the concentration of phosphorus(P_(2)O_(5))and potassium(K_(2)O).Whereas,the content of elements in the tomato plants showed a significant difference on all the evaluated traits.At the same time,the correlation between the attributes in the tomato plant with the soil indicated that the plant’s N had a positive and significant correlation with Organic Carbon(OC),total N,P_(2)O_(5),K_(2)O and the soil moisture;and a negative and significant correlation with the soil’s EC(electrical conductivity).Moreover,the content of K,Ca,and Mg of the plant had a positive and significant correlation with the soil moisture,and a negative and significant correlation with the soil’s EC.Cumulatively,GPMPs not only are enable to shift over the soil environments in the favor of the plant growth,but also similarly modulate immunity against RKNs,by virtue of which Formycine and others can compete relatively compared to Fenamiphos(Nemacur)and other relevant GPMPs in this research.

文摘Mimicking the hierarchical structure of the skin is one of the most important strategies in skin tissue engineering.Monolayer wound dressings are usually not able to provide several functions at the same time and cannot meet all clinical needs.In order to maximize therapeutic efficiency,herein,we fabricated a Tri-layer wound dressing,where the middle layer was fabricated via 3D-printing and composed of alginate,tragacanth and zinc oxide nanoparticles(ZnO NPs).Both upper and bottom layers were constructed using electrospinning technique;the upper layer was made of hydrophobic polycaprolactone to mimic epidermis,while the bottom layer consisted of Soluplus■ and insulin-like growth factor-1(IGF-1)to promote cell behavior.Swelling,water vapor permeability and tensile properties of the dressings were evaluated and the Tri-layer dressing exhibited impressive antibacterial activity and cell stimulation following by the release of ZnO NPs and IGF-1.Additionally,the Tri-layer dressing led to faster healing of full-thicknesswound in ratmodel compared to monolayer and Bilayer dressings.Overall,the evidence confirmed that the Trilayer wound dressing is extremely effective for full-thickness wound healing.
文摘Objective:To investigate the influence of age,body mass index(BMI),varicocele,diabetes,tobacco use,and environmental occupational risks on sperm DNA fragmentation index(DFI)and its association with semen parameters and in vitro fertility(IVF)outcomes.Methods:This cross-sectional study was conducted on 116 infertile men.Conventional semen parameters were analyzed according to the World Health Organization criteria.Sperm DNA fragmentation was evaluated using sperm chromatin dispersion method.After visiting the infertility center,the men's height and weight were measured,and blood tests were performed to check for diabetes,and medical records were reviewed for varicocele,tobacco use,and type of occupation.The sperm was then examined for DFI.Then,the association between sperm DFI and IVF failure rate was investigated.Results:The study showed a significant association between DFI≥20%with BMI(OR 1.134,95%CI 1.04-1.24,P=0.006),varicocele(OR 4.330,95%CI 1.25-14.96,P=0.021),tobacco use(OR 3.066,95%CI 1.06-8.90,P=0.039)and environmental and occupational risks(OR 2.694,95%CI 1.08-6.75,P=0.034)as well as sperm motility(P<0.05).Although the amount of DNA damage increased in those aged≥40 years,there was no significant association between the amount of DFI≥≥20%and age,diabetes,sperm volume and concentration,morphology and progressive rate(P>0.05).The IVF failure rate was higher in people with a DFI≥20%.Conclusions:Factors such as BMI,varicocele,improper working conditions and environment cause damage to sperm DNA,and DFI≥20%damage can have adverse effects on IVF outcomes.
文摘Overt and harmful diabetes mellitus(DM)has detrimental effects on individuals and,by extension,the community.Among the microvascular DM complications is diabetic retinopathy(DR).DR may cause irreversible vision deterioration in cases of poor blood glucose regulation.Changes in vascular permeability are key trigger points for diabetic macular edema(DME),a condition characterized by the accumulation of fluid in the macula.The development of vascular endothelial growth factor(VEGF)pathway inhibitors has provided a pathogenesis-based treatment approach for DME.Optical coherence tomography(OCT)provides highresolution imaging of the anatomy,including the aging of DME and its structural damage,in distinct morphologic subtypes of macular edema,thereby supporting the assessment of macular edema treatment.The availability of repeated OCT monitoring provides clinical reassurance through the treatment.OCT angiography(OCTA)provides retinal blood flow maps with high spatial resolution.The ability promotes an understanding of disease pathogenesis and facilitates the implementation of new therapeutic methods.This review compares the potential of OCT and OCTA in the diagnosis and treatment of DME,as well as their respective therapeutic applications.
文摘Hepatocellular carcinoma(HCC) is the 5th most common malignancy which is responsible for more than half million annual mortalities; also, it is the third leading cause of cancer related death. Unfavorablesystemic side-effects of chemotherapeutic agents and susceptibility to the degradation of small interfering RNAs(si RNAs), which can knock down a specific gene involved in the disease, have hampered their clinical application. So, it could be beneficial to develop an efficient carrier for the stabilization and specific delivery of drugs and si RNA to cells. Targeted nanoparticles have gained considerable attention as an efficient drug and gene delivery system, which is due to their capability in achieving the highest accumulation of cytotoxic agents in tumor tissue, modifiable drug pharmacokinetic- and bio-distribution, improved effectiveness of treatment, and limited sideeffects. Recent studies have shed more light on the advantages of novel drug loaded carrier systems vs free drugs. Most of the animal studies have reported improvement in treatment efficacy and survival rate using novel carrier systems. Targeted delivery may be achieved passively or actively. In passive targeting, no ligand as homing device is used, while targeting is achieved by incorporating the therapeutic agent into a macromolecule or nanoparticle that passively reaches the target organ. However, in active targeting, the therapeutic agent or carrier system is conjugated to a tissue or cell-specific receptor which is overexpressed in a special malignancy using a ligand called a homing device. This review covers a broad spectrum of targeted nanoparticles as therapeutic and nonviral si RNA delivery systems, which are developed for enhanced cellular uptake and targeted gene silencing in vitro and in vivo and their characteristics and opportunities for the clinical applications of drugs and therapeutic si RNA are discussed in this article. Asialoglycoprotein receptors, low-density lipoprotein, ganglioside GM1 cell surface ligand, epidermal growth factor receptor receptors, monoclonal antibodies, retinoic acid receptors, integrin receptors targeted by Arg-Gly-Asp peptide, folate, and transferrin receptors are the most widely studied cell surface receptors which are used for the site specific delivery of drugs and si RNA-based therapeutics in HCC and discussed in detail in this article.
文摘OBJECTIVE: Ulcerative colitis is a chronically recurrent inflammatory bowel disease of unknown origin. In the present study, the effect of ginger (rhizome of Zingiber officinale Roscoe) volatile oil on a rat model of colitis was evaluated. METHODS: Volatile oil of ginger with doses of 100, 200, and 400 mg/kg, prednisolone (4 mg/kg), or vehicle were administered orally to groups of male Wistar rats (n = 6) for 5 d. Animals were randomly divided into 6 groups, each group consisting of 6 rats. Colitis was induced by intracolonic instillation of 2 mL of 4% (v/v) acetic acid solution. All rats were sacrificed 24 h later and the tissue injuries were assessed macroscopically and histopathologically. RESULTS: Ginger volatile oil with all doses reduced colon weight/length ratio (P 〈 0.01) and the effects were similar to the reference drugs. Higher oral doses of volatile oil (200 and 400 mg/kg) reduced ulcer severity (P 〈 0.05 and P 〈 0.01), ulcer area (P 〈 0.01) and ulcer index (P 〈 0.01). On the other hand, evaluation of microscopic scores showed that the dose of 400 mg/kg of volatile oil was effective to reduce inflammation severity (P 〈 0.01) and inflammation extent (P 〈 0.05) compared to the control group. CONCLUSION: It is concluded that ginger volatile oil could effectively reduce symptoms of experimental colitis in a dose-dependent manner.
文摘AIM: To investigate the effect of stem cells from human exfoliated deciduous teeth(SHED) transplanted for bone regeneration in the dog mandibular defect.METHODS: In this prospective comparative study, SHEDs had been isolated 5 years ago from human exfoliated deciduous teeth. The undifferentiated stem cells were seeded into mandibular bone through-andthrough defects of 4 dogs. Similar defects in control group were filled with cell-free collagen scaffold. After 12 wk, biopsies were taken and morphometric analysis was performed. The percentage of new bone formation and foreign body reaction were measured in each case. The data were subject to statistical analysis using the Mann-Whitney U and Kruskalwalis statistical tests. Differences at P < 0.05 was considered as significant level.RESULTS: There were no significant differences between control and SHED-seeded groups in connective tissue(P = 0.248), woven bone(P = 0.248) and compact bone(P = 0.082). There were not any side effects in transplanted SHED group such as teratoma or malignancy and abnormalities in this period.CONCLUSION: SHEDs which had been isolated and characterized 5 years ago and stored with cryopreservation banking were capable of proliferation and osteogenesis after 5 years, and no immune response was observed after three months of seeded SHEDs.

文摘MXenes,transition metal carbides and nitrides with graphene-like structures,have received considerable attention since their first discovery.On the other hand,Graphene has been extensively used in biomedical and medicinal applications.MXene and graphene,both as promising candidates of two-dimensional materials,have shown to possess high potential in future biomedical applications due to their unique physicochemical properties such as superior electrical conductivity,high biocompatibility,large surface area,optical and magnetic features,and extraordinary thermal and mechanical properties.These special structural,functional,and biological characteristics suggest that the hybrid/composite structure of MXene and graphene would be able to meet many unmet needs in different fields;particularly in medicine and biomedical engineering,where high-performance mechanical,electrical,thermal,magnetic,and optical requirements are necessary.However,the hybridization and surface functionalization should be further explored to obtain biocompatible composites/platforms with unique physicochemical properties,high stability,and multifunctionality.In addition,toxicological and long-term biosafety assessments and clinical translation evaluations should be given high priority in research.Although very limited studies have revealed the excellent potentials of MXene/graphene in biomedicine,the next steps should be toward the extensive research and detailed analysis in optimizing the properties and improving their functionality with a clinical and industrial outlook.Herein,different synthesis/fabrication methods and performances of MXene/graphene composites are discussed for potential biomedical applications.The potential toxicological effects of these composites on human cells and tissues are also covered,and future perspectives toward more successful translational applications are presented.The current state-of-the-art biotechnological advances in the use of MXene-Graphene composites,as well as their developmental challenges and future prospects are also deliberated.Due to the superior properties and multifunctionality of MXene-graphene composites,these hybrid structures can open up considerable new horizons in future of healthcare and medicine.
文摘AIM: To provide a structural model of the relationship between personality traits, perceived stress, coping strategies, social support, and psychological outcomes in the general population.METHODS: This is a cross sectional study in which the study group was selected using multistage cluster and convenience sampling among a population of 4 million. For data collection, a total of 4763 individuals were asked to complete a questionnaire on demographics, personality traits, life events, coping with stress, social support, and psychological outcomes such as anxiety and depression. To evaluate the comprehensive relation-ship between the variables, a path model was fitted.RESULTS: The standard electronic modules showed that personality traits and perceived stress are important determinants of psychological outcomes. Social support and coping strategies were demonstrated to reduce the increasing cumulative positive effects of neuroticism and perceived stress on the psychological outcomes and enhance the protective effect of extraversion through decreasing the positive effect of perceived stress on the psychological outcomes. CONCLUSION: Personal resources play an important role in reduction and prevention of anxiety and depression. In order to improve the psychological health, it is necessary to train and reinforce the adaptive coping strategies and social support, and thus, to moderate negative personality traits.

文摘AIM:To explore the association between consumption of spicy foods and prevalence of irritable bowel syndrome(IBS)among Iranian adults.METHODS:In this cross-sectional study,data from4763 Iranian adult participants were used.Consumption of spicy foods was estimated using a dietary habits questionnaire that included a question on spicy foods consumption:"how frequently do you use spicy foods(pepper,curry,ginger,cinnamon and turmeric)during a week?"Participants could respond to the question by choosing one of these choices:never,1-3 times,4-6times,7-9 times,or more than 10 times per week.A modified Persian version of the RomeⅢquestionnaire was used to determine the prevalence of IBS.RESULTS:IBS was prevalent in 21.7%(18.6%of men and 24.1%of women)of the study population.After controlling for potential confounders including dietary behaviors,those consuming spicy foods≥10 times per week were 92%more likely to have IBS compared with those who never consumed spicy foods(OR=1.92;95%CI:1.23-3.01,Ptrend<0.01).The association remained significant even after taking lactose intolerance into account(OR=1.85;95%CI:1.18-2.90,Ptrend<0.01).Stratified analysis by gender revealed that the association between consumption of spicy foods and IBS was not significant in men;however,a significant association was found among women after taking potential cofounders,including meal regularity and lactose intolerance,into account.Women who consumed spicy foods≥10 times per week were two times more likely to have IBS compared with those who never consumed spicy foods(OR=2.03;95%CI:1.09-3.77,Ptrend=0.02).CONCLUSION:Consumption of spicy foods is directly associated with IBS,particularly in women.Further,prospective studies are warranted to(1)examine this association in other populations;and(2)evaluate whether dietary interventions,for example a reduction in spice consumption,would improve IBS symptoms.
文摘AIM To determine the predictive role of body mass index(BMI) and waist circumference(WC) for diabetes and prediabetes risk in future in total sample as well as in men and women separately. METHODS In a population based cohort study, 1765 with mean ± SD age: 42.32 ± 6.18 healthy participants were followed up from 2003 till 2013(n = 960). Anthropometric and biochemical measures of participants were evaluated regularly during the follow up period. BMI and WC measures at baseline and diabetes and prediabetes status of participants at 2013 were determined. Multivariable logistic regression analysis was used for determining the risk of diabetes and prediabetes considering important potential confounding variables. Receiver operatingcharacteristic curve analysis was conducted to determine the best cut of values of BMI and WC for diabetes and prediabetes. RESULTS At 2013, among participants who had complete data, 45 and 307 people were diabetic and prediabetic, respectively. In final fully adjusted model, BMI value was a significant predictor of diabetes(RR = 1.39, 95%CI: 1.06-1.82 and AUC = 0.68, 95%CI: 0.59-0.75; P < 0.001) however not a significant risk factor for prediabetes. Also, WC was a significant predictor for diabetes(RR = 1.2, 95%CI: 1.05-1.38 and AUC = 0.67, 95%CI: 0.6-0.75) but not significant risk factor for prediabetes. Similar results were observed in both genders.CONCLUSION General and abdominal obesity are significant risk factors for diabetes in future.

文摘In this paper, preparation of nano-biphasic calcium phosphate (nBCP), mechanical behavior and load-bearing of poly (lactide-co-glycolide) (PLGA) and PLGA/nBCP are presented. The nBCP with composition of 63/37 (w/w) HA/-TCP (hydroxyapatite/fl-tricalcium phosphate) was produced by heating of bovine bone at 700℃. Composite scaffolds were made by using PLGA matrix and 10-50 wt% nBCP powders as reinforcement material. All scaffolds were prepared by thermally induced solid-liquid phase separation (TIPS) at -60~C under 4 Pa (0.04 mbar) vacuum. The results of elastic modulus testing were adjusted with Ishai-Cohen and Narkis models for rigid polymeric matrix and compared to each other. PLGA/nBCP scaffolds with 30 wt% nBCP showed the highest value of yield strength among the scaffolds. In addition, it was found that by increasing the nBCP in scaffolds to 50 wt%, the modulus of elasticity was highly enhanced. However, the optimum value of yield strength was obtained at 30 wt% nBCP, and the agglomeration of reinforcing particles at higher percentages caused a reduction in yield strength. It is clear that the elastic modulus of matrix has the significant role in elastic modulus of scaffolds, as also the size of the filler particles in the matrix.

文摘AIM: To assess possible relationships of knowledge and related factors with educational level and osteoporosisrelated life habits.METHODS: This was a cross sectional study conducted on 268 women(≥ 35 years old) from June 2011 to August 2011. The sample collection was done in outpatient clinics in three university hospitals in Isfahan, Iran. We used a demographic questionnaire containingquestions that evaluated osteoporosis-related life habits, including exercise, smoking, intake of calcium and vitamin D supplements and so on. We also used the Osteoporosis Knowledge Assessment Tool to measure osteoporosis knowledge of women.RESULTS: The mean level of knowledge about awareness of osteoporosis, its risk factors and preventive factors were 56, 55 and 22, respectively. The relationship of education level and awareness of osteoporosis, its risk factors and preventive factors was significant, with R = 0.76, R = 0.73 and R = 0.83, respectively(P < 0.001). The relationship of education level and osteoporosis-related life habits was not significant(R = 0.03 and P = 0.56). The relationship of osteoporosis-related life habits and awareness of osteoporosis and its risk factors was significant, with R = 16%, P = 0.006 and R = 16%, P = 0.008, respectively, but the relationship of osteoporosis-related life habits and preventive factors was not significant(R = 0, P = 0.99).CONCLUSION: Iranian women with a higher education level have significantly better knowledge about osteoporosis than women with a lower educational level but they do not use this knowledge in their life.
文摘BACKGROUND: Ensuring about the patient's safety is the f irst vital step in improving the quality of care and the emergency ward is known as a high-risk area in treatment health care. The present study was conducted to evaluate the selected risk processes of emergency surgery department of a treatment-educational Qaem center in Mashhad by using analysis method of the conditions and failure effects in health care.METHODS: In this study, in combination(qualitative action research and quantitative crosssectional), failure modes and effects of 5 high-risk procedures of the emergency surgery department were identified and analyzed according to Healthcare Failure Mode and Effects Analysis(HFMEA). To classify the failure modes from the "nursing errors in clinical management model(NECM)", the classification of the effective causes of error from "Eindhoven model" and determination of the strategies to improve from the "theory of solving problem by an inventive method" were used. To analyze the quantitative data of descriptive statistics(total points) and to analyze the qualitative data, content analysis and agreement of comments of the members were used.RESULTS: In 5 selected processes by "voting method using rating", 23 steps, 61 sub-processes and 217 potential failure modes were identifi ed by HFMEA. 25(11.5%) failure modes as the high risk errors were detected and transferred to the decision tree. The most and the least failure modes were placed in the categories of care errors(54.7%) and knowledge and skill(9.5%), respectively. Also, 29.4% of preventive measures were in the category of human resource management strategy.CONCLUSION: "Revision and re-engineering of processes", "continuous monitoring of the works", "preparation and revision of operating procedures and policies", "developing the criteria for evaluating the performance of the personnel", "designing a suitable educational content for needs of employee", "training patients", "reducing the workload and power shortage", "improving team communication" and "preventive management of equipment's" were on the agenda as the guidelines.
文摘BACKGROUND: According to the fact that women account for the highest rate of victims of mental health loss during disasters, prioritization of their requirements is of remarkable signifi cance. Therefore, the present study was carried out with the aim to prioritize factors related to women's mental health during an earthquake. METHODS: This study was conducted using the Delphi method in 2017. Participants consisted of experts including psychologists, counselors and psychiatrists, social medicine specialists, and psychiatric-mental health nurses with experience in service and disaster awareness, especially earthquake. They were selected using purposive and snowball sampling methods. The Delphi method was used in 2 rounds with 21 components and the fi nal attendance of 19 participants and the collective agreement of 75%. RESULTS: Of the 21 factors related to women's mental health during the earthquake, the following components were, respectively, preferred by the participants: psychological well-being training, group relationships and support of women in emergency situations, fair treatment in the provision of rights and services to women, crisis awareness and management of behavior and diffi cult conditions, and social security during disasters with the total mean standard deviation (SD) of 13.4 (2.4), 13.0 (2.4), 12.7 (2.5), 12.5 (2.4), and 12.3 (2.2), respectively. CONCLUSION: Training in the fi eld of psychology and disasters, and social and cultural factors were prioritized among factors related to women's mental health during an earthquake. Therefore, the results of this study can be applied by the planners and executives of mental health, women and disasters, and the women's community itself.
