AIM: To investigate the change of anterior chamber angle morphology and intraocular pressure (lOP) reduction after cataract surgery in patients with normal-tension glaucoma (NTG) using swept-source optical cohere...AIM: To investigate the change of anterior chamber angle morphology and intraocular pressure (lOP) reduction after cataract surgery in patients with normal-tension glaucoma (NTG) using swept-source optical coherence tomography (SS-OCT). METHODS: This prospective, comparative, observational study recruited patients into two groups. Group 1 was the control group including normal subjects except those with cataracts (cataract group, n=67 eyes of 67 patients), and group 2 was NTG group including patients who were diagnosed with NTG and cataracts (n=43 eyes of 43 patients), which were treated with phacoemulsification and intraocular lens implantation. Before surgery, and at postoperative 1 and 6too, anterior chamber angles were evaluated by SS-OCT under dark conditions using three- dimensional angle analysis scan protocol. Angle opening distance (AOD), angle recess area (ARA), and trabecular- iris surface area (TISA) at four quadrants (temporal, nasal, superior, and inferior) were calculated automatically by SS-OGT, after the observer marked scleral spurs. RESULTS: A total of 106 patients (54 males and 52 females) were enrolled in the study. Angle parameters, AOD, ARA, and TISA were increased after surgery in both groups. However, changes of angle parameters were only significant in group 2. In group 2, preoperative lOP was 13.2+2.9 mm Hg, and postoperative lOP at 1 and 6mo were 10.5+3.0 and 10.7+2.8 mm Hg, respectively. In group 1, preoperative lOP was 12.42.8 mm Hg, and postoperative lOP at 1 and 6mo were 11.6+2.5 and 12.0+2.8 mm Hg, respectively. After cataract surgery, angle parameters changed significantly while IOP significantly reduced and was maintained in group 2 (P〈0.001). The changes in angle parameters (&AOD500, ATISAS00 at temporal; &AOD500, S, RA500 at nasal) were linearly correlated with postoperative IOP changes. CONCLUSION: Cataract surgery may have improved antedor chamber angle parameters and decreased lOP in NTG patients.展开更多

METHODS: This retrospective, interventional study included 41 patients with acute, unilateral BRVO without macular edema. All patients completed at least a 6-month follow-up period. The PCT was measured at eight locations(temporal, superotemporal, superior, superonasal, nasal, inferonasal, inferior, and inferotemporal). In addition to calculating the average of all locations, the peripapillary choroidal area was divided into four sectors: superior(average of superotemporal PCT, superior PCT, and superonasal PCT), temporal, inferior(average of inferotemporal PCT, inferior PCT, and inferonasal PCT), and nasal. RESULTS: In the BRVO-affected eyes, the mean PCT was 177.7±69.8 μm(range, 70.1-396.0 μm) at baseline and 127.8±54.8 μm(range, 56.4-312.1 μm) at 6 mo(P<0.001). In the non-affected contralateral eyes, the mean PCT was 192.5±60.6 μm(range, 61.4-365.0 μm) at baseline and 165.9±61.1 μm(range, 56.8-326.8 μm) at 6 mo(P<0.001). In sectoral analysis, the mean PCT in each sector was significantly reduced in over 6 mo in the BRVO-affected eyes(all P<0.001). In the non-affected contralateral eyes, the mean PCT was not significantly changed in any sector over the 6-month follow-up period(superior sector, P=0.143; temporal sector, P=0.825; inferior sector, P=0.192; and nasal sector, P=0.599).CONCLUSION: Sectoral analysis shows that the mean PCTs in all sectors are reduced significantly over 6 mo in the BRVO-affected eyes, but not in the non-affected contralateral eyes.展开更多
AIM:To evaluate the long-term effect and safety of focal laser photocoagulation treatment in eyes with polypoidal choroidal vasculopathy(PCV).METHODS:Medical records of 13 eyes of 13 patients with PCV were followed-up...AIM:To evaluate the long-term effect and safety of focal laser photocoagulation treatment in eyes with polypoidal choroidal vasculopathy(PCV).METHODS:Medical records of 13 eyes of 13 patients with PCV were followed-up for more than 2 y after focal laser photocoagulation treatment.The patients were diagnosed with PCV using indocyanine green angiography,and eyes with other comorbid ocular diseases were excluded.The measurement outcomes of the study were the post-treatment regression and recurrence of polyps,complications,and changes in visual acuities.Paired t-test was performed to compare visual outcome before and after the treatment.RESULTS:The mean age of the 13 patients was 70.2±5.5 y,and the follow-up period was 72.3±31.0(range,25-118)mo.Three eyes had juxtafoveal polyps and 10 eyes had extrafoveal polyps.Of the 13 eyes,9 eyes(69.2%)had regression of polyps 1.7±1.2(range,0.9-4)mo after focal laser photocoagulation.Five eyes(55.6%)showed recurrence of polyps during the follow-up periods,and the recurrence period was 12.8±18.9(range,1.9-48)mo.Mild subretinal hemorrhage occurred in two eyes(15.4%)27 and 72 d after laser treatment,respectively.There were no statistically significant differences in visual acuities at baseline;1,2,3 y post-treatment(all P>0.05);and last follow-up(0.63±0.5,0.73±0.70,0.67±0.57,0.75±0.7,and 0.95±0.8 log MAR,respectively).CONCLUSION:Focal laser photocoagulation is beneficial for early regression of polyps in eyes with PCV and does not result in significant submacular hemorrhage during the long-term follow-up.Furthermore,it can be primarily considered in eyes with PCV with extrafoveal or juxtafoveal polyps to regress risky polyps as well as to maintain visual acuity without serious hemorrhagic complications.展开更多
AIM:To investigate the clinical characteristics and prognosis of patients with malignant eyelid tumors.·METHODS:This was a retrospective,non-randomized,clinical reviews.Between January,2002 and December,2011,75 c...AIM:To investigate the clinical characteristics and prognosis of patients with malignant eyelid tumors.·METHODS:This was a retrospective,non-randomized,clinical reviews.Between January,2002 and December,2011,75 cases with histologically confirmed malignant eyelid tumors were evaluated.Patients’ charts were reviewed for clinical information,treatment procedure,and disease course.Survival analysis in terms of recurrence-free survival was performed using age,sex,location of tumor and histopathological type.The followup ranged from 1 to 78 months(mean=21 months).·RESULTS:The 75 eyelid tumors included 35 basal cell carcinoma(BCC,46.7%),22 sebaceous gland carcinoma(SGC,29.3%),7 squamous cell carcinoma(SCC,9.3%),10 malignant melanoma(MM,13.3%),and 1 Merkel cell carcinoma(MCC,1.3%).Recurrence developed in 17cases(22.7%).The recurrence rate of BCC(4/35,11.4%)was significant lower than MM(6/10,60.0%,P 【0.001).The mean interval of recurrence was 21 months(range 3-62) for all eyelid tumors.Tumor located at canthus had higher recurrence rate(50%) compared with those located at eyelid(19%,P 【0.05).Histological type was independent variable for recurrence by Cox regression analysis.·CONCLUSION:It is important to achieve a negative tumor margin in canthus located malignant eyelid tumor.Clinicians should have a high level of suspicion for recurrence according to histological type when treating patients with eyelid tumor.展开更多

AIM:To determine the prevalence of focal lamina cribrosa(LC)defect among patients with pachychoroid disease spectrum(PDS)in the absence of peripapillary retinoschisis.METHODS:This retrospective,cross-sectional study c...AIM:To determine the prevalence of focal lamina cribrosa(LC)defect among patients with pachychoroid disease spectrum(PDS)in the absence of peripapillary retinoschisis.METHODS:This retrospective,cross-sectional study comprised of 180 patients with PDS,including polypoidal choroidal vasculopathy(PCV),central serous chorioretinopathy,and pachychoroidal neovasculopathy.Medical records and optic ner ve head evaluations conducted using spectral-domain optical coherence tomography with enhanced depth imaging were reviewed.As a control group,236 patients who underwent ophthalmologic evaluation for vitreous floaters,without obvious ocular disease,were also included.RESULTS:The mean age of the PDS group,which included 118 male patients(65.6%),was 57.4±11.1 y.There was no significant difference between the two groups in age(P=0.710)or sex(P=0.248).Six patients(3.3%)in the PDS group and none in the control group showed focal LC defect(P=0.318).Among the six patients with focal LC defect in the PDS group,four eyes had PCV,one eye was the fellow eye of a PCV eye,and one eye had pachychoroidal neovasculopathy.CONCLUSION:Focal LC defect can be defected in patients with PDS in the absence of peripapillar y retinoschisis.However,the prevalence of focal LC defect was not different significantly between PDS patients and those who did not have PDS.展开更多
AIM:To investigate the association of axial length(AL)and ocular factors on AL elongation.METHODS:A retrospective chart review of patients who underwent two or more AL examinations for more than two years.Totally 4 gr...AIM:To investigate the association of axial length(AL)and ocular factors on AL elongation.METHODS:A retrospective chart review of patients who underwent two or more AL examinations for more than two years.Totally 4 groups were divided according to initial AL(<24 mm,24-26 mm,26-28 mm,≥28 mm).Initial fundus photograph was used to find risk factors associated AL elongation.RESULTS:The mean age of the patients was 47.21±7.79 y.AL remained almost unchanged in the groups with AL<24 mm and 24≤AL<26 mm.On the contrary,AL increased by 0.011 mm/y in the group with 26≤AL<28 mm and 0.035 mm/y in the group with AL≥28 mm(P<0.001).In high myopia,AL elongation increased in eye with longer AL(r=0.003,P=0.024),female gender(r=0.014,P=0.019),eye with larger peripapillary chorioretinal atrophic area(r=0.002,P=0.019),and smaller vascular arcade angle(r=-0.004,P=0.006).The risk of elongation 0.03 mm/y in high myopia was increased in female gender(P=0.040),and gradually increased in eye with large peripapillary chorioretinal atrophy area(P<0.01).CONCLUSION:AL elongate significantly in the eye with longer AL,female gender,and the eye with larger atrophic area and smaller arcade angle on fundus photography.展开更多

METHODS: Total 101 human eyes were included in this prospective observational case series. Delivery characteristics of 5 types of IOLs including i Sert250 NC60(NC60), En Vista MX60(MX60), Acry Sof IQ SN60 WF(SN60 WF), TECNIS ZCB00(ZCB00), and TECNIS PCB00(PCB00) were investigated. NC60 and PCB00 were injected via preloaded delivery systems and other IOLs were injected via nonpreloaded systems. In the human trial, time taken from IOL loading to completion of implantation was measured in all eyes undergoing conventional cataract surgery. Using 4 excised porcine eyes, dynamics of ophthalmic viscosurgical device(OVD) between an IOL injector and a porcine eye was analyzed using fluorescein sodiumstained OVD. RESULTS: The average time for IOL implantation was 22.0 s for NC60, 43.2 s for MX60, 32.3 s for SN60 WF, 41.4 s for ZCB00, and 14.6 s for PCB00 respectively. The number of cases with IOL manipulation with a second instrument was 6 for MX60, 2 for ZCB00, 0 for SN60 WF, NC60, and PCB00. Amount of OVD pushed into a porcine eye was smaller with a preloaded system than with non-preloaded systems. CONCLUSION: IOL delivery with preloaded systems is faster and more predictable. Moreover, a preloaded delivery system shows relatively less OVD pushed into a porcine eye than non-preloaded systems.展开更多

Background To evaluate the association between retinal artery/arteriole occlusion(RAO)and unruptured intracranial aneurysm(UIA).Methods Incident UIA patients from a nationwide cohort(n=253240)were categorised into thr...Background To evaluate the association between retinal artery/arteriole occlusion(RAO)and unruptured intracranial aneurysm(UIA).Methods Incident UIA patients from a nationwide cohort(n=253240)were categorised into three groups based on subsequent treatment:observation(n=208993),microsurgical clipping(n=14168)and endovascular treatment(EVT)groups(n=30079).The incidence and the incident time of RAO were analysed.HRs of RAO and associated risk factors were evaluated.Additionally,a hospital cohort comprising 2569 consecutive UIA patients treated at a tertiary hospital was analysed with detailed clinical information of UIAs.Results In the nationwide cohort analysis,the incidence of RAO was significantly higher in EVT group than in observation and clipping groups,especially within 60 days(early RAO(within 60 days):HR=4.00,95%CI:2.44 to 6.56);delayed RAO(after 60 days:HR=1.74,95%CI:1.13 to 2.68).Multivariable analysis showed that the presence of chronic kidney disease(p=0.009)and use of a balloon microcatheter during the procedure(p=0.013)were associated with a higher risk of RAO.In hospital cohort analysis,11(0.8%)cases of RAO occurred after EVT,whereas none occurred after microsurgical clipping(p<0.001).Patients with RAO were younger and received balloon microcatheters more frequently than their counterparts.Ten cases of RAO(90.9%)occurred in paraclinoid aneurysms,where EVT was preferred over microsurgical clipping.Conclusions Performing EVT for UIA may increase the risk of subsequent RAO.Care should be taken when treating paraclinoid aneurysms with balloon microcatheters.展开更多
Bioresponsive hydrogels are smart materials that respond to various external stimuli and exhibit great potential as biosensors owing to their capability of real-time and label-free detection.Here,we propose a sensing ...Bioresponsive hydrogels are smart materials that respond to various external stimuli and exhibit great potential as biosensors owing to their capability of real-time and label-free detection.Here,we propose a sensing platform based on bioresponsive hydrogels,employing the concept of moiré patterns.Two sets of line patterns with different pitch sizes are prepared;a hydrogel grating whose pitch size changes according to external stimuli and a reference grating with constant pitch size.The volume changes of the hydrogel caused by external stimuli changes the pitch size of the hydrogel grating,and subsequently,the pitch sizes of the moiré patterns(moiré signal),whose values can be obtained in a real-time and label-free manner through customized moiré microscopy and signal processing.After confirming that the pH-induced swelling of hydrogel could be monitored using moiré patterns,we performed moiré pattern-based detection of specific proteins using protein-responsive hydrogel that underwent shrinking via interaction with target proteins.Brain-derived neurotrophic factor and platelet-derived growth factor were selected as the model proteins,and our proposed system successfully detected both proteins at nanomolar levels.In both cases,the pitch size change of hydrogel grating was monitored much more sensitively using moiré patterns than through direct measurements.The changes in the moiré signals caused by target proteins were detected in ex-vivo environments using a custom-made intraocular lens incorporating the hydrogel grating,demonstrating the capability of the proposed system to detect various markers in intraocular aqueous humor,when implanted in the eye.展开更多

文摘AIM:To investigate the clinical characteristics and prognosis of patients with malignant eyelid tumors.·METHODS:This was a retrospective,non-randomized,clinical reviews.Between January,2002 and December,2011,75 cases with histologically confirmed malignant eyelid tumors were evaluated.Patients’ charts were reviewed for clinical information,treatment procedure,and disease course.Survival analysis in terms of recurrence-free survival was performed using age,sex,location of tumor and histopathological type.The followup ranged from 1 to 78 months(mean=21 months).·RESULTS:The 75 eyelid tumors included 35 basal cell carcinoma(BCC,46.7%),22 sebaceous gland carcinoma(SGC,29.3%),7 squamous cell carcinoma(SCC,9.3%),10 malignant melanoma(MM,13.3%),and 1 Merkel cell carcinoma(MCC,1.3%).Recurrence developed in 17cases(22.7%).The recurrence rate of BCC(4/35,11.4%)was significant lower than MM(6/10,60.0%,P 【0.001).The mean interval of recurrence was 21 months(range 3-62) for all eyelid tumors.Tumor located at canthus had higher recurrence rate(50%) compared with those located at eyelid(19%,P 【0.05).Histological type was independent variable for recurrence by Cox regression analysis.·CONCLUSION:It is important to achieve a negative tumor margin in canthus located malignant eyelid tumor.Clinicians should have a high level of suspicion for recurrence according to histological type when treating patients with eyelid tumor.

文摘Dear Editor,We would like to introduce a simple technique for the efficient rescue and refixation of a subluxated retropupillary iris claw intraocular lens(RPICIOL;Artisan■Aphakia model 205,Ophtec BV,Groningen,the Netherlands).The RPICIOL is a suture-less,posterior iris-fixated,polyinethyl methacrylate intraocular lens(IOL)that has gained widespread recognition and expanding use because of its efficiency in management of aphakia,combined with long-term intraocular safety and excellent sustained visual outcomes11 Postoperative dislocation/disenclavation of the RPICIOL may occur spontaneously or following trauma,with a rate reportedly between 0 to 10%[4-6].

文摘AIM:To determine the prevalence of focal lamina cribrosa(LC)defect among patients with pachychoroid disease spectrum(PDS)in the absence of peripapillary retinoschisis.METHODS:This retrospective,cross-sectional study comprised of 180 patients with PDS,including polypoidal choroidal vasculopathy(PCV),central serous chorioretinopathy,and pachychoroidal neovasculopathy.Medical records and optic ner ve head evaluations conducted using spectral-domain optical coherence tomography with enhanced depth imaging were reviewed.As a control group,236 patients who underwent ophthalmologic evaluation for vitreous floaters,without obvious ocular disease,were also included.RESULTS:The mean age of the PDS group,which included 118 male patients(65.6%),was 57.4±11.1 y.There was no significant difference between the two groups in age(P=0.710)or sex(P=0.248).Six patients(3.3%)in the PDS group and none in the control group showed focal LC defect(P=0.318).Among the six patients with focal LC defect in the PDS group,four eyes had PCV,one eye was the fellow eye of a PCV eye,and one eye had pachychoroidal neovasculopathy.CONCLUSION:Focal LC defect can be defected in patients with PDS in the absence of peripapillar y retinoschisis.However,the prevalence of focal LC defect was not different significantly between PDS patients and those who did not have PDS.
文摘AIM:To investigate the association of axial length(AL)and ocular factors on AL elongation.METHODS:A retrospective chart review of patients who underwent two or more AL examinations for more than two years.Totally 4 groups were divided according to initial AL(<24 mm,24-26 mm,26-28 mm,≥28 mm).Initial fundus photograph was used to find risk factors associated AL elongation.RESULTS:The mean age of the patients was 47.21±7.79 y.AL remained almost unchanged in the groups with AL<24 mm and 24≤AL<26 mm.On the contrary,AL increased by 0.011 mm/y in the group with 26≤AL<28 mm and 0.035 mm/y in the group with AL≥28 mm(P<0.001).In high myopia,AL elongation increased in eye with longer AL(r=0.003,P=0.024),female gender(r=0.014,P=0.019),eye with larger peripapillary chorioretinal atrophic area(r=0.002,P=0.019),and smaller vascular arcade angle(r=-0.004,P=0.006).The risk of elongation 0.03 mm/y in high myopia was increased in female gender(P=0.040),and gradually increased in eye with large peripapillary chorioretinal atrophy area(P<0.01).CONCLUSION:AL elongate significantly in the eye with longer AL,female gender,and the eye with larger atrophic area and smaller arcade angle on fundus photography.

文摘AIM: To compare characteristics of preloaded and nonpreloaded intraocular lens(IOL) delivery systems during IOL delivery procedures. METHODS: Total 101 human eyes were included in this prospective observational case series. Delivery characteristics of 5 types of IOLs including i Sert250 NC60(NC60), En Vista MX60(MX60), Acry Sof IQ SN60 WF(SN60 WF), TECNIS ZCB00(ZCB00), and TECNIS PCB00(PCB00) were investigated. NC60 and PCB00 were injected via preloaded delivery systems and other IOLs were injected via nonpreloaded systems. In the human trial, time taken from IOL loading to completion of implantation was measured in all eyes undergoing conventional cataract surgery. Using 4 excised porcine eyes, dynamics of ophthalmic viscosurgical device(OVD) between an IOL injector and a porcine eye was analyzed using fluorescein sodiumstained OVD. RESULTS: The average time for IOL implantation was 22.0 s for NC60, 43.2 s for MX60, 32.3 s for SN60 WF, 41.4 s for ZCB00, and 14.6 s for PCB00 respectively. The number of cases with IOL manipulation with a second instrument was 6 for MX60, 2 for ZCB00, 0 for SN60 WF, NC60, and PCB00. Amount of OVD pushed into a porcine eye was smaller with a preloaded system than with non-preloaded systems. CONCLUSION: IOL delivery with preloaded systems is faster and more predictable. Moreover, a preloaded delivery system shows relatively less OVD pushed into a porcine eye than non-preloaded systems.

文摘Dear Editor,We report our experience using a continuous intraoperative optical coherence tomography(OCT)-enabled microscope system for real-time anatomical evaluation of the optic pit during surgery in an 8-year-old girl who presented with symptomatic optic disc pit(ODP) maculopathy. ODP is an extremely rare congenital anomaly secondary to colobomatous malformation of the optic nerve head, with a reported incidence of 0.00009%[1]. Although patients are frequently asymptomatic, approximately 40%-50% of patients experience visual symptoms because of subretinal fluid(SRF) accumulation beneath the macula.

文摘Background To evaluate the association between retinal artery/arteriole occlusion(RAO)and unruptured intracranial aneurysm(UIA).Methods Incident UIA patients from a nationwide cohort(n=253240)were categorised into three groups based on subsequent treatment:observation(n=208993),microsurgical clipping(n=14168)and endovascular treatment(EVT)groups(n=30079).The incidence and the incident time of RAO were analysed.HRs of RAO and associated risk factors were evaluated.Additionally,a hospital cohort comprising 2569 consecutive UIA patients treated at a tertiary hospital was analysed with detailed clinical information of UIAs.Results In the nationwide cohort analysis,the incidence of RAO was significantly higher in EVT group than in observation and clipping groups,especially within 60 days(early RAO(within 60 days):HR=4.00,95%CI:2.44 to 6.56);delayed RAO(after 60 days:HR=1.74,95%CI:1.13 to 2.68).Multivariable analysis showed that the presence of chronic kidney disease(p=0.009)and use of a balloon microcatheter during the procedure(p=0.013)were associated with a higher risk of RAO.In hospital cohort analysis,11(0.8%)cases of RAO occurred after EVT,whereas none occurred after microsurgical clipping(p<0.001).Patients with RAO were younger and received balloon microcatheters more frequently than their counterparts.Ten cases of RAO(90.9%)occurred in paraclinoid aneurysms,where EVT was preferred over microsurgical clipping.Conclusions Performing EVT for UIA may increase the risk of subsequent RAO.Care should be taken when treating paraclinoid aneurysms with balloon microcatheters.

文摘Bioresponsive hydrogels are smart materials that respond to various external stimuli and exhibit great potential as biosensors owing to their capability of real-time and label-free detection.Here,we propose a sensing platform based on bioresponsive hydrogels,employing the concept of moiré patterns.Two sets of line patterns with different pitch sizes are prepared;a hydrogel grating whose pitch size changes according to external stimuli and a reference grating with constant pitch size.The volume changes of the hydrogel caused by external stimuli changes the pitch size of the hydrogel grating,and subsequently,the pitch sizes of the moiré patterns(moiré signal),whose values can be obtained in a real-time and label-free manner through customized moiré microscopy and signal processing.After confirming that the pH-induced swelling of hydrogel could be monitored using moiré patterns,we performed moiré pattern-based detection of specific proteins using protein-responsive hydrogel that underwent shrinking via interaction with target proteins.Brain-derived neurotrophic factor and platelet-derived growth factor were selected as the model proteins,and our proposed system successfully detected both proteins at nanomolar levels.In both cases,the pitch size change of hydrogel grating was monitored much more sensitively using moiré patterns than through direct measurements.The changes in the moiré signals caused by target proteins were detected in ex-vivo environments using a custom-made intraocular lens incorporating the hydrogel grating,demonstrating the capability of the proposed system to detect various markers in intraocular aqueous humor,when implanted in the eye.
