BACKGROUND Antimicrobial resistance(AMR)is an escalating global health threat,disproportionately impacting low-income and middle-income countries like India.The rise of multidrug-resistant organisms(MDROs)complicates ...BACKGROUND Antimicrobial resistance(AMR)is an escalating global health threat,disproportionately impacting low-income and middle-income countries like India.The rise of multidrug-resistant organisms(MDROs)complicates treatment,increases healthcare costs,and contributes to higher morbidity and mortality.In India,factors such as antibiotic misuse,over-the-counter availability,and self-medication have exacerbated this crisis.Despite the urgency,regional data on MDRO prevalence remains limited.AIM To assess the burden and distribution of MDRO infections identified at a tertiary healthcare institute in North India.METHODS This cross-sectional study analyzed inpatient data from All India Institute of Medical Sciences(AIIMS)Rishikesh using the E-hospital database from May 2021 to February 2024,except 2022.All inpatients with culture-confirmed MDRO infections were included.Patient charts and discharge summaries were reviewed.Data analysis was performed using Jamovi software,with descriptive statistics summarizing demographics.Theχ²test was used to assess associations between MDROs and various factors.Ethical approval was obtained,and patient confidentiality was maintained.RESULTS This study included 820 participants having 1106 MDROs.The majority of patients were aged 35-64 years,with a male predominance(57.6%).Most cases were from Uttar Pradesh(49.1%)and Uttarakhand(47.6%),with Bijnor and Haridwar districts reporting the highest burden from their respective states.Klebsiella pneumoniae(K.pneumoniae)(30.6%),Acinetobacter baumannii(16.7%),and Escherichia coli(E.coli)(16.7%)were the most common MDROs.Urine was the most frequent sample type(51.0%),with K.pneumoniae as the leading pathogen.Infections varied significantly across departments(P<0.001),with General Medicine and Surgery reporting the highest burden.E.coli and Klebsiella spp.were most prevalent in these departments respectively.MDRO prevalence fluctuated over time,with significant variations by quarter(P<0.001).Following the second coronavirus disease 2019 wave,there was a rapid surge in MDROs,which stabilized after approximately three months.The overall mortality among patients harboring MDROs was 25.9%.CONCLUSION This study highlights the burden of MDROs among patients at AIIMS Rishikesh,with K.pneumoniae as the predominant pathogen.Strengthening antimicrobial stewardship and infection control measures is essential to combat rising AMR,with department-specific,and pathogen-specific stewardships.展开更多

It ...Introduction: Malignant sylvian infarction (MSI) is a type of ischemic stroke (ICS) usually affecting the entire territory of the middle cerebral artery (MCA) associated with significant cerebral edema and a mass. It represents about 10% of all AICs, with a mortality of up to 80%. The objectives of our study were to describe the sociodemographic profile and the main clinical manifestations and identify the prognostic factors of ISM. Material and Methods: We conducted a retrospective descriptive study over a 2-year period. It included patients hospitalized for cerebral infarction involving 2/3 of the ACM territory with a NIHSS score ≥ 17 and/or a Glasgow score Results: We collected 223 patients hospitalized for ischemic stroke, of whom 21 patients (9.4%) presented with ISM. The mean age was 57.43 ± 24.24 years with a male predominance (52.4%). The mean admission time was 47 ± 0.87 hours, and hemiplegia was the frequent neurological sign (85.7%). HBP was the common cardiovascular risk factor (76.2%). The mean NIHSS at admission was 18.38 ± 12.29. Respiratory distress (p-value = 0.00015), aspiration pneumonia (p-value = 0.015) and brain herniation (p-value = 0.014) were the main complications associated with mortality. Conclusion: ISM is associated with poor prognosis in the absence of surgical treatment. Respiratory distress, aspiration pneumonia and brain herniation are associated with high mortality.展开更多
The prompt fission neutron spectrum(PFNS)is a key nuclear data quantity that is of particular interest and plays a crucial role in understanding and modeling fission processes.An array comprising 48 liquid scintillati...The prompt fission neutron spectrum(PFNS)is a key nuclear data quantity that is of particular interest and plays a crucial role in understanding and modeling fission processes.An array comprising 48 liquid scintillation detectors and a parallelplate avalanche counter(PPAC)was developed at the China Institute of Atomic Energy(CIAE)to measure the PFNS of actinide nuclei.Efficiency and energy calibrations were performed for all the liquid scintillators,and their efficiencies were consistently found to be better than 5%.The time resolutions of the PPAC and liquid scintillators were measured to be 1.08 ns and 1.16 ns using~(252)Cf and~(207)Bi sources,respectively.The pulse shape discrimination of the liquid scintillator was utilized to identify neutron andγsignals on an event-by-event basis,and the figure of merit was deduced as 1.12 at a 200 ke Vee threshold.The contribution to the PFNS from multiple scattered neutrons was evaluated via Geant4 simulations,and those originating from the environment were found to be comparable to the crosstalk between the detectors.The neutron efficiency of the entire detection array was calibrated using a~(252)Cf spontaneous fission source and was demonstrated to be consistent with the Geant4 simulation results,which verified the reliability of the detection array.展开更多

It has been recognized as a feature of Parkinson’s disease (PD) since the first descriptions of the disease. Material and Methods: This was a prospective descriptive study lasting six (06) months from November 1, 2023 to April 30, 2024. We included all patients diagnosed with PD and who had pain. Sociodemographic, clinical, paraclinical and therapeutic data were evaluated for each patient. Results: We identified a sample of 62 Parkinson’s patients, of whom 52 patients or 85.2% had associated pain. We noted a male predominance (38M/14F) and a sex ratio of 2.71. Musculoskeletal pain was common in 80% of our respondents. WHO level I, antidepressants and background treatment for KD were the most prescribed molecules. Conclusion: Our study shows a frequency of pain in PD. However, musculoskeletal pain is the most frequently encountered type of pain in PD patients. WHO step I analgesics, antidepressants and background treatment of KD were the main prescriptions in our study.展开更多
BACKGROUND Bile spillage occurs more frequently in patients with incidental gallbladder carcinoma(iGBC)and may be associated with poor survival due to presumed high risk of peritoneal seeding.AIM To investigate the im...BACKGROUND Bile spillage occurs more frequently in patients with incidental gallbladder carcinoma(iGBC)and may be associated with poor survival due to presumed high risk of peritoneal seeding.AIM To investigate the impact of bile spillage during primary surgery on the survival of patients with iGBC.METHODS Medical records of patients with iGBC diagnosed between 2000 and 2019 in 27 Dutch secondary centers and 5 tertiary centers were retrospectively reviewed.Patient medical records were assessed.Predictors for overall survival(OS)were determined using multivariable Cox regression.RESULTS Of the 346 included patients with iGBC,138(39.9%)had bile spillage,which was associated with higher American Society of Anesthesiologists classification(P=0.020),cholecystitis(P<0.001),higher tumor stage(P=0.005),and non-radical resection(P<0.001).Bile spillage was associated with poor OS[hazard ratio=1.97,95%confidence interval(CI):1.48-2.63,P<0.001]with a median OS of 12 months(95%CI:7-18 months)vs 34 months(95%CI:14-55 months,P<0.001).In multivariable analysis,spillage was not an independent prognostic factor for survival(hazard ratio=1.21,95%CI:0.84-1.74,P=0.313).CONCLUSION Although bile spillage correlates with prognostic factors,it lacks independent prognostic significance for survival.Patients with an indication for additional treatment should be promptly referred to a specialized hepatopancreatobiliary center,irrespective of whether bile spillage has occurred.展开更多
Purpose:Interdisciplinary research has become a critical approach to addressing complex societal,economic,technological,and environmental challenges,driving innovation and integrating scientific knowledge.While interd...Purpose:Interdisciplinary research has become a critical approach to addressing complex societal,economic,technological,and environmental challenges,driving innovation and integrating scientific knowledge.While interdisciplinarity indicators are widely used to evaluate research performance,the impact of classification granularity on these assessments remains underexplored.Design/methodology/approach:This study investigates how different levels of classification granularity-macro,meso,and micro-affect the evaluation of interdisciplinarity in research institutes.Using a dataset of 262 institutes from four major German non-university organizations(FHG,HGF,MPG,WGL)from 2018 to 2022,we examine inconsistencies in interdisciplinarity across levels,analyze ranking changes,and explore the influence of institutional fields and research focus(applied vs.basic).Findings:Our findings reveal significant inconsistencies in interdisciplinarity across classification levels,with rankings varying substantially.Notably,the Fraunhofer Society(FHG),which performs well at the macro level,experiences significant ranking declines at meso and micro levels.Normalizing interdisciplinarity by research field confirmed that these declines persist.The research focus of institutes,whether applied,basic,or mixed,does not significantly explain the observed ranking dynamics.Research limitations:This study has only considered the publication-based dimension of institutional interdisciplinarity and has not explored other aspects.Practical implications:The findings provide insights for policymakers,research managers,and scholars to better interpret interdisciplinarity metrics and support interdisciplinary research effectively.Originality/value:This study underscores the critical role of classification granularity in interdisciplinarity assessment and emphasizes the need for standardized approaches to ensure robust and fair evaluations.展开更多

Systematic vitamin D supplementation has been implemented in most Western countries, which has drastically reduced the prevalence of rickets. In Senegal, a country with enough sunshine, no large-scale data exists on vitamin D deficiency in children. In addition, there are no guidelines from the Ministry of Health and Social Action (MoHSA) on vitamin D supplementation. Our objective was to determine the prevalence of vitamin D deficiency in children aged 6 to 59 months and to analyze the factors associated with it at the Institute of Social Pediatrics in Pikine-Guédiawaye, a suburb of Dakar. Patients and Methods: This was a prospective, descriptive and analytical cross-sectional study over a 3-month period (from January to March 2024) at the Institute of Social Pediatric (IPS) of Pikine-Guédiawaye. The study population consisted of all children aged 6 to 59 months free from any acute or chronic pathology, whose parents had agreed to answer the questionnaires after having consented and signed the consent and children who had had a sample taken for vitamin D dosage. Results: We included 102 children, 35 of whom had vitamin D deficiency, i.e. a prevalence of 34.31% with a slight male predominance (sex ratio 1.05). Infants aged 13 - 24 months were more affected (41.67%), but the difference was not statistically significant (p-value = 0.385). Patients with acute malnutrition had a significantly higher prevalence of vitamin D deficiency (54.17% of cases versus 28.38%) in those without acute malnutrition (p = 0.02). On the other hand, chronic malnutrition, underweight and diversification foods were not significantly associated with vitamin D deficiency (p of 0.60, 0.42 and 0.09 respectively). Conclusion: Vitamin deficiency affects one-third of apparently healthy children under 5 and this deficiency has no significant relationship with diversification foods. This suggests that the observed vitamin D deficiencies are less related to diet than to lack of sun exposure.展开更多

Nuclear astrophysics is a rapidly developing interdisciplinary feld of research that has received extensive attention from the scientifc community since the midtwentieth century.Broadly,it uses the laws of extremely s...Nuclear astrophysics is a rapidly developing interdisciplinary feld of research that has received extensive attention from the scientifc community since the midtwentieth century.Broadly,it uses the laws of extremely small atomic nuclei to explain the evolution of the universe.Owing to the complexity of nucleosynthesis processes and our limited understanding of nuclear physics in astrophysical environments,several critical astrophysical problems remain unsolved.To achieve a better understanding of astrophysics,it is necessary to measure the cross sections of key nuclear reactions with the precision required by astrophysical models.Direct measurement of nuclear reaction cross sections is an important method of investigating how nuclear reactions infuence stellar evolution.Given the challenges involved in measuring the extremely low crosssections of nuclear reactions in the Gamow peak and preparing radioactive targets,indirect methods,such as the transfer reaction,coulomb dissociation,and surrogate ratio methods,have been developed over the past several decades.These are powerful tools in the investigation of,for example,neutron-capture(n,r)reactions with short-lived radioactive isotopes.However,direct measurement is still preferable,such as in the case of reactions involving light and stable nuclei.As an essential part of stellar evolution,these low-energy stable nuclear reactions have been of particular interest in recent years.To overcome the diffculties in measurements near or deeply within the Gamow window,the combination of an underground laboratory and high-exposure accelerator/detector complex is currently the optimal solution.Therefore,underground experiments have emerged as a new and promising direction of research.In addition,to better simulate the stellar environment in the laboratory,research on nuclear physics under laser-driven plasma conditions has gradually become a frontier hotspot.In recent years,the CIAE team conducted a series of distinctive nuclear astrophysics studies,relying on the Jinping Underground Nuclear Astrophysics platform and accelerators in Earth’s surface laboratories,including the Beijing Radioactive Ion beam Facility,as well as other scientifc platforms at home and abroad.This research covered nuclear theories,numerical models,direct measurements,indirect measurements,and other novel approaches,achieving great interdisciplinary research results,with high-level academic publications and signifcant international impacts.This article reviews the above research and predicts future developments.展开更多
BACKGROUND:In clinical practice,some patients might not be able or unwilling to provide a thorough history of medication and poison exposure.The aim of this study was to use toxicological analysis to examine the clini...BACKGROUND:In clinical practice,some patients might not be able or unwilling to provide a thorough history of medication and poison exposure.The aim of this study was to use toxicological analysis to examine the clinical characteristics of patients with acute poisoning whose exposure history was uncertain from a toxicological analysis perspective.METHODS:This was a retrospective and descriptive study from an institute of poisoning.Patient registration information and test reports spanning the period from April 1,2020 to March 31,2022,were obtained.Patients with uncertain exposure histories and who underwent toxicological analysis were included.Clinical manifestations and categories of toxics were analyzed.RESULTS:Among the 195 patients with positive toxicological analysis results,the main causes of uncertain exposure history was disturbance of consciousness(62.6%),unawareness(23.6%)and unwillingness or lack of cooperation(13.8%).The predominant clinical manifestations were disturbed consciousness(62.6%),followed by vomiting and nausea(14.4%)and liver function abnormalities(8.7%).A comparison of clinical manifestations between patients with positive and negative(n=99)toxicological analyses results revealed significantly different proportions of disturbances in consciousness(63%vs.21%),dizziness(1.5%vs.5.1%),multi-organ failure(1.5%vs.7.1%),and local pain(0 vs 4%).The main categories of substances involved were psychiatric medications(23.1%),sedatives(20.5%),insecticides(13.8%),and herbicides(12.8%).CONCLUSION:The clinical manifestations of acute poisoning in patients with an uncertain exposure history are diverse and nonspecific,and toxicological analysis plays a pivotal role in the diagnosis and differential diagnosis of such patients.展开更多

It was the subject of a priority axis...Background: In the context of the fight against HIV, a lack of skills in monitoring and evaluating the personnel in charge of activities has been identified at the national level. It was the subject of a priority axis of the national plan for monitoring and evaluating the fight against HIV (2006-2010) that was aimed at strengthening the capacities of actors in this area. To increase the critical mass of competent human resources in the short term, the National Institute of Public Health (NIPH) of Côte d’Ivoire organized monitoring and evaluation training sessions for healthcare professionals from 2011 to 2016. Methods: A single case study with multiple levels of analysis was carried out, combining a qualitative survey and a literature review. An evaluation was carried out six months after each training session. In addition, the results of the pre- and post-tests and of the daily and final evaluations that accompanied the various training sessions were used to provide further information. The qualitative data collected were analyzed using INVIVO 15 software. Results: Some 89 health professionals (69% men and 31% women) working at the national level (51% at the central level, including 58% in health programs) and in development partner agencies (37%) participated in this capacity building program. Most participants were senior health managers (56%), data managers (23%), and statisticians and computer scientists (10%). Almost all the trainings were financed by 16 technical and financial partners (85%), mainly the MEASURE Evaluation project (27%). Conclusion: M&E training, despite all its imperfections, has made it possible to identify M&E training needs at the national level and to increase the critical mass of national skills and to have some culture in M&E.展开更多

The breakdown of this barrier is the origin of the synthesis of antisperm antibodies (ASA). The presence of sperm agglutinates in semen is characteristic of ASA. But is the presence of agglutinates in semen necessarily linked to the level of ASA in semen? The objective of this study was to assess the concentration of anti-sperm antibodies (ASA) in normozoosperms and infertile men with azoospermia. The biological material consisted of samples of human sperms: 30 samples with azoospermia and 32 with normozoospermia. The ASA assay was performed in seminal plasma using the DRG® Sperm Antibody ELISA (seminal plasma) kit (EIA-4249). The reading was carried out using a microplate reader at 450 nm. Data analysis was performed using Graph Pad Prism 7 software. The results obtained showed that the difference in ASA concentration between these two categories of sperm was not significant, with an average ASA level of 31.54 ± 2.45 U/mL in azoospermic ejaculate and 27.63 ± 1.51 U/mL in normozoosperms. Statistical analysis showed higher ASA concentrations in azoosperms with 6.67% of these declared positive. The ASA positivity rate made it possible to distinguish secretory azoospermias from obstructive ones. Also, the presence of ASA is not necessarily linked to the presence of agglutinates in the semen.展开更多

Neonatal mortality rates remain high in low-income countries. In Guinea, after 20 years, this rate has fallen from 34.2% in 1998 to 32% in 2018. Objective: To identify the main risk factors for neonatal mortality. Methods: This was an observational, analytical case-control study, lasting 6 months from January 1 to June 30, 2019, conducted at the Institut de Nutrition et de la Santé de l’Enfant (INSE) at Donka National Hospital. Results: We collected 242 cases and 242 controls, i.e. a total of 484 records. 748 patients were registered, with 32.35% deaths. 82.86% of deaths occurred in the early neonatal period. Statistical analysis revealed the main risk factors: prematurity (RQ 7.39 95% CI 3.27 - 16.61 p = 0.0000003), hypothermia (RQ 2.29 95% CI 1.51 - 3.46 p = 0.0001), acute fetal distress (RQ 2.13 95% CI 1.33 - 3.43 p = 0.0016), low birth weight (QR 1.91 95% CI 1.12 - 3.24 p = 0.016), home birth (QR 3.26 95% CI 1.25 - 8.46 p = 0.015). Conclusion: Neonatal mortality is a health problem in the INSE neonatology department. To reduce the mortality rate in this referral facility, it is essential to equip it and provide ongoing training for staff.展开更多
Background:Hodgkin lymphoma refers to a malignancy of the lymphatic tissue.Extra-nodal Hodgkin lymphoma(ENHL)is a rare variant consisting of Hodgkin lymphoma occurring outside the lymphatic system.Studies investigatin...Background:Hodgkin lymphoma refers to a malignancy of the lymphatic tissue.Extra-nodal Hodgkin lymphoma(ENHL)is a rare variant consisting of Hodgkin lymphoma occurring outside the lymphatic system.Studies investigating the epidemiology associated with ENHL are rare.Methods:618 ENHL cases were analyzed using the National Cancer Institute’s Surveillance,Epidemiology,and End Results(SEER)database(2000–2020).Demographics including sex,race/ethnicity,rural-urban continuum,age group(categorized as adolescent and young adult(AYA)if between years of 15 and 39,and adult if over the age of 40),and living status(alive/deceased)were examined,with survival status as the main outcome.Results:This study included 335 males(54.2%)and 283 females(45.8%).Of this total,47.9%were in the AYA group.In terms of ethnicity the distribution was 12.5%non-Hispanic Black,67.5%non-Hispanic White,5.2%non-Hispanic Asian/Pacific Islander,and 14.9%Hispanic.Bivariate analyses evidenced significant differences in survival by age group with 91.6%in AYA vs.72.4%in adults(P<0.001)alive by the end of the study period.Multivariable analyses identified age as a key predictor of survival,as the AYA patients had a lower odds ratio for death(odds ratio=0.25,P<0.001).In addition,survival outcomes were also impacted by race,with non-Hispanic Blacks showing higher survival probabilities.Regarding treatment,27.0%of patients underwent surgery,with 10.2%receiving post-surgery radiation,reducing odds of mortality(odds ratio=0.32,P=0.046).Conclusion:The background research as such,tends to affirm that these two factors–age and race are quite crucial in the prognosis as well as management of ENHL.Compared to adults,AYA patients had significantly lower odds of death,while non-Hispanic Black individuals exhibited reduced survival probabilities.It should be noted that 27.0%of patients underwent surgery with 10.2%receiving post-operative radiation which led to decrease in mortality rates.Thus,these results reiterate the necessity for tailor-made treatment methods according to demographic characteristics to boost patient outcomes effectively.For better ENHL care,future studies could shed light on these disparities and improve treatment regimens as needed.展开更多
文摘BACKGROUND Antimicrobial resistance(AMR)is an escalating global health threat,disproportionately impacting low-income and middle-income countries like India.The rise of multidrug-resistant organisms(MDROs)complicates treatment,increases healthcare costs,and contributes to higher morbidity and mortality.In India,factors such as antibiotic misuse,over-the-counter availability,and self-medication have exacerbated this crisis.Despite the urgency,regional data on MDRO prevalence remains limited.AIM To assess the burden and distribution of MDRO infections identified at a tertiary healthcare institute in North India.METHODS This cross-sectional study analyzed inpatient data from All India Institute of Medical Sciences(AIIMS)Rishikesh using the E-hospital database from May 2021 to February 2024,except 2022.All inpatients with culture-confirmed MDRO infections were included.Patient charts and discharge summaries were reviewed.Data analysis was performed using Jamovi software,with descriptive statistics summarizing demographics.Theχ²test was used to assess associations between MDROs and various factors.Ethical approval was obtained,and patient confidentiality was maintained.RESULTS This study included 820 participants having 1106 MDROs.The majority of patients were aged 35-64 years,with a male predominance(57.6%).Most cases were from Uttar Pradesh(49.1%)and Uttarakhand(47.6%),with Bijnor and Haridwar districts reporting the highest burden from their respective states.Klebsiella pneumoniae(K.pneumoniae)(30.6%),Acinetobacter baumannii(16.7%),and Escherichia coli(E.coli)(16.7%)were the most common MDROs.Urine was the most frequent sample type(51.0%),with K.pneumoniae as the leading pathogen.Infections varied significantly across departments(P<0.001),with General Medicine and Surgery reporting the highest burden.E.coli and Klebsiella spp.were most prevalent in these departments respectively.MDRO prevalence fluctuated over time,with significant variations by quarter(P<0.001).Following the second coronavirus disease 2019 wave,there was a rapid surge in MDROs,which stabilized after approximately three months.The overall mortality among patients harboring MDROs was 25.9%.CONCLUSION This study highlights the burden of MDROs among patients at AIIMS Rishikesh,with K.pneumoniae as the predominant pathogen.Strengthening antimicrobial stewardship and infection control measures is essential to combat rising AMR,with department-specific,and pathogen-specific stewardships.

文摘The software technology field is facing new talent demands brought by the Information Technology Application Innovation(ITAI)industry.This paper takes Shanwei Institute of Technology as an example to deeply explore the construction of a school-enterprise community education model driven by the ITAI industry.It establishes the Kirin Workshop training base to facilitate talent cultivation,integrates the ITAI Application Adaptation Center to enhance technical capabilities,cooperates with Liqi Technology to establish an industrial college for government talent training,adjusts the professional curriculum system,and arranges for students to participate in ITAI vocational skills competitions.The school-enterprise collaborative cultivation mechanism meets the talent needs of the ITAI field,with effective practical results.This paper also points out the shortcomings of the school-enterprise collaborative education model in the ITAI industry and provides optimization methods to explore new paths for industry-education integration and serve the development of regional and national ITAI industries^([1]).
文摘Introduction: Malignant sylvian infarction (MSI) is a type of ischemic stroke (ICS) usually affecting the entire territory of the middle cerebral artery (MCA) associated with significant cerebral edema and a mass. It represents about 10% of all AICs, with a mortality of up to 80%. The objectives of our study were to describe the sociodemographic profile and the main clinical manifestations and identify the prognostic factors of ISM. Material and Methods: We conducted a retrospective descriptive study over a 2-year period. It included patients hospitalized for cerebral infarction involving 2/3 of the ACM territory with a NIHSS score ≥ 17 and/or a Glasgow score Results: We collected 223 patients hospitalized for ischemic stroke, of whom 21 patients (9.4%) presented with ISM. The mean age was 57.43 ± 24.24 years with a male predominance (52.4%). The mean admission time was 47 ± 0.87 hours, and hemiplegia was the frequent neurological sign (85.7%). HBP was the common cardiovascular risk factor (76.2%). The mean NIHSS at admission was 18.38 ± 12.29. Respiratory distress (p-value = 0.00015), aspiration pneumonia (p-value = 0.015) and brain herniation (p-value = 0.014) were the main complications associated with mortality. Conclusion: ISM is associated with poor prognosis in the absence of surgical treatment. Respiratory distress, aspiration pneumonia and brain herniation are associated with high mortality.

文摘The prompt fission neutron spectrum(PFNS)is a key nuclear data quantity that is of particular interest and plays a crucial role in understanding and modeling fission processes.An array comprising 48 liquid scintillation detectors and a parallelplate avalanche counter(PPAC)was developed at the China Institute of Atomic Energy(CIAE)to measure the PFNS of actinide nuclei.Efficiency and energy calibrations were performed for all the liquid scintillators,and their efficiencies were consistently found to be better than 5%.The time resolutions of the PPAC and liquid scintillators were measured to be 1.08 ns and 1.16 ns using~(252)Cf and~(207)Bi sources,respectively.The pulse shape discrimination of the liquid scintillator was utilized to identify neutron andγsignals on an event-by-event basis,and the figure of merit was deduced as 1.12 at a 200 ke Vee threshold.The contribution to the PFNS from multiple scattered neutrons was evaluated via Geant4 simulations,and those originating from the environment were found to be comparable to the crosstalk between the detectors.The neutron efficiency of the entire detection array was calibrated using a~(252)Cf spontaneous fission source and was demonstrated to be consistent with the Geant4 simulation results,which verified the reliability of the detection array.

文摘In response to the current issues in the construction of software engineering(SE)degree granting program,such as insufficient resource integration,low level of internationalization,and inadequate quality control,we propose the Software Engineering Degree Granting Program Construction Practice Project at Harbin Institute of Technology(HIT).This project aims to explore new models for software talent cultivation,establish a superior SE degree granting program,and ultimately cultivate outstanding internationalized composite SE professionals to support the high-quality development of the national software industry.To this end,we design a distinctive overall construction idea and plan for the SE degree granting program,which are characterized by“3I3S:three highlights for specialized cultivation and strictness in three aspects to ensure quality control”.After years of practice and validation of the project at the School of Software at HIT,this project has proven effective in optimizing talent cultivation models,enhancing students’practical abilities,promoting international exchange and cooperation,advancing industry-education integration,and meeting industrial needs.

文摘Introduction: Pain has been defined for more than 20 years by the International Association for the Study of Pain (IASP) as an unpleasant sensory and emotional experience associated with actual or potential tissue damage, or described in terms of such damage. It has been recognized as a feature of Parkinson’s disease (PD) since the first descriptions of the disease. Material and Methods: This was a prospective descriptive study lasting six (06) months from November 1, 2023 to April 30, 2024. We included all patients diagnosed with PD and who had pain. Sociodemographic, clinical, paraclinical and therapeutic data were evaluated for each patient. Results: We identified a sample of 62 Parkinson’s patients, of whom 52 patients or 85.2% had associated pain. We noted a male predominance (38M/14F) and a sex ratio of 2.71. Musculoskeletal pain was common in 80% of our respondents. WHO level I, antidepressants and background treatment for KD were the most prescribed molecules. Conclusion: Our study shows a frequency of pain in PD. However, musculoskeletal pain is the most frequently encountered type of pain in PD patients. WHO step I analgesics, antidepressants and background treatment of KD were the main prescriptions in our study.
文摘BACKGROUND Bile spillage occurs more frequently in patients with incidental gallbladder carcinoma(iGBC)and may be associated with poor survival due to presumed high risk of peritoneal seeding.AIM To investigate the impact of bile spillage during primary surgery on the survival of patients with iGBC.METHODS Medical records of patients with iGBC diagnosed between 2000 and 2019 in 27 Dutch secondary centers and 5 tertiary centers were retrospectively reviewed.Patient medical records were assessed.Predictors for overall survival(OS)were determined using multivariable Cox regression.RESULTS Of the 346 included patients with iGBC,138(39.9%)had bile spillage,which was associated with higher American Society of Anesthesiologists classification(P=0.020),cholecystitis(P<0.001),higher tumor stage(P=0.005),and non-radical resection(P<0.001).Bile spillage was associated with poor OS[hazard ratio=1.97,95%confidence interval(CI):1.48-2.63,P<0.001]with a median OS of 12 months(95%CI:7-18 months)vs 34 months(95%CI:14-55 months,P<0.001).In multivariable analysis,spillage was not an independent prognostic factor for survival(hazard ratio=1.21,95%CI:0.84-1.74,P=0.313).CONCLUSION Although bile spillage correlates with prognostic factors,it lacks independent prognostic significance for survival.Patients with an indication for additional treatment should be promptly referred to a specialized hepatopancreatobiliary center,irrespective of whether bile spillage has occurred.
文摘Purpose:Interdisciplinary research has become a critical approach to addressing complex societal,economic,technological,and environmental challenges,driving innovation and integrating scientific knowledge.While interdisciplinarity indicators are widely used to evaluate research performance,the impact of classification granularity on these assessments remains underexplored.Design/methodology/approach:This study investigates how different levels of classification granularity-macro,meso,and micro-affect the evaluation of interdisciplinarity in research institutes.Using a dataset of 262 institutes from four major German non-university organizations(FHG,HGF,MPG,WGL)from 2018 to 2022,we examine inconsistencies in interdisciplinarity across levels,analyze ranking changes,and explore the influence of institutional fields and research focus(applied vs.basic).Findings:Our findings reveal significant inconsistencies in interdisciplinarity across classification levels,with rankings varying substantially.Notably,the Fraunhofer Society(FHG),which performs well at the macro level,experiences significant ranking declines at meso and micro levels.Normalizing interdisciplinarity by research field confirmed that these declines persist.The research focus of institutes,whether applied,basic,or mixed,does not significantly explain the observed ranking dynamics.Research limitations:This study has only considered the publication-based dimension of institutional interdisciplinarity and has not explored other aspects.Practical implications:The findings provide insights for policymakers,research managers,and scholars to better interpret interdisciplinarity metrics and support interdisciplinary research effectively.Originality/value:This study underscores the critical role of classification granularity in interdisciplinarity assessment and emphasizes the need for standardized approaches to ensure robust and fair evaluations.
文摘Introduction: Vitamin D plays a key role in phosphocalcic metabolism, in normal functioning of the immune system and in the prevention of certain forms of cancer. Systematic vitamin D supplementation has been implemented in most Western countries, which has drastically reduced the prevalence of rickets. In Senegal, a country with enough sunshine, no large-scale data exists on vitamin D deficiency in children. In addition, there are no guidelines from the Ministry of Health and Social Action (MoHSA) on vitamin D supplementation. Our objective was to determine the prevalence of vitamin D deficiency in children aged 6 to 59 months and to analyze the factors associated with it at the Institute of Social Pediatrics in Pikine-Guédiawaye, a suburb of Dakar. Patients and Methods: This was a prospective, descriptive and analytical cross-sectional study over a 3-month period (from January to March 2024) at the Institute of Social Pediatric (IPS) of Pikine-Guédiawaye. The study population consisted of all children aged 6 to 59 months free from any acute or chronic pathology, whose parents had agreed to answer the questionnaires after having consented and signed the consent and children who had had a sample taken for vitamin D dosage. Results: We included 102 children, 35 of whom had vitamin D deficiency, i.e. a prevalence of 34.31% with a slight male predominance (sex ratio 1.05). Infants aged 13 - 24 months were more affected (41.67%), but the difference was not statistically significant (p-value = 0.385). Patients with acute malnutrition had a significantly higher prevalence of vitamin D deficiency (54.17% of cases versus 28.38%) in those without acute malnutrition (p = 0.02). On the other hand, chronic malnutrition, underweight and diversification foods were not significantly associated with vitamin D deficiency (p of 0.60, 0.42 and 0.09 respectively). Conclusion: Vitamin deficiency affects one-third of apparently healthy children under 5 and this deficiency has no significant relationship with diversification foods. This suggests that the observed vitamin D deficiencies are less related to diet than to lack of sun exposure.

文摘Nuclear astrophysics is a rapidly developing interdisciplinary feld of research that has received extensive attention from the scientifc community since the midtwentieth century.Broadly,it uses the laws of extremely small atomic nuclei to explain the evolution of the universe.Owing to the complexity of nucleosynthesis processes and our limited understanding of nuclear physics in astrophysical environments,several critical astrophysical problems remain unsolved.To achieve a better understanding of astrophysics,it is necessary to measure the cross sections of key nuclear reactions with the precision required by astrophysical models.Direct measurement of nuclear reaction cross sections is an important method of investigating how nuclear reactions infuence stellar evolution.Given the challenges involved in measuring the extremely low crosssections of nuclear reactions in the Gamow peak and preparing radioactive targets,indirect methods,such as the transfer reaction,coulomb dissociation,and surrogate ratio methods,have been developed over the past several decades.These are powerful tools in the investigation of,for example,neutron-capture(n,r)reactions with short-lived radioactive isotopes.However,direct measurement is still preferable,such as in the case of reactions involving light and stable nuclei.As an essential part of stellar evolution,these low-energy stable nuclear reactions have been of particular interest in recent years.To overcome the diffculties in measurements near or deeply within the Gamow window,the combination of an underground laboratory and high-exposure accelerator/detector complex is currently the optimal solution.Therefore,underground experiments have emerged as a new and promising direction of research.In addition,to better simulate the stellar environment in the laboratory,research on nuclear physics under laser-driven plasma conditions has gradually become a frontier hotspot.In recent years,the CIAE team conducted a series of distinctive nuclear astrophysics studies,relying on the Jinping Underground Nuclear Astrophysics platform and accelerators in Earth’s surface laboratories,including the Beijing Radioactive Ion beam Facility,as well as other scientifc platforms at home and abroad.This research covered nuclear theories,numerical models,direct measurements,indirect measurements,and other novel approaches,achieving great interdisciplinary research results,with high-level academic publications and signifcant international impacts.This article reviews the above research and predicts future developments.

文摘BACKGROUND:In clinical practice,some patients might not be able or unwilling to provide a thorough history of medication and poison exposure.The aim of this study was to use toxicological analysis to examine the clinical characteristics of patients with acute poisoning whose exposure history was uncertain from a toxicological analysis perspective.METHODS:This was a retrospective and descriptive study from an institute of poisoning.Patient registration information and test reports spanning the period from April 1,2020 to March 31,2022,were obtained.Patients with uncertain exposure histories and who underwent toxicological analysis were included.Clinical manifestations and categories of toxics were analyzed.RESULTS:Among the 195 patients with positive toxicological analysis results,the main causes of uncertain exposure history was disturbance of consciousness(62.6%),unawareness(23.6%)and unwillingness or lack of cooperation(13.8%).The predominant clinical manifestations were disturbed consciousness(62.6%),followed by vomiting and nausea(14.4%)and liver function abnormalities(8.7%).A comparison of clinical manifestations between patients with positive and negative(n=99)toxicological analyses results revealed significantly different proportions of disturbances in consciousness(63%vs.21%),dizziness(1.5%vs.5.1%),multi-organ failure(1.5%vs.7.1%),and local pain(0 vs 4%).The main categories of substances involved were psychiatric medications(23.1%),sedatives(20.5%),insecticides(13.8%),and herbicides(12.8%).CONCLUSION:The clinical manifestations of acute poisoning in patients with an uncertain exposure history are diverse and nonspecific,and toxicological analysis plays a pivotal role in the diagnosis and differential diagnosis of such patients.

文摘Pneumoconiosis is a group of heterogeneous fibrotic lung diseases caused by inorganic mineral dust and includes coal workers’pneumoconiosis and silicosis.Silicosis involves diffuse or nodular interstitial pulmonary fibrosis caused by exposure to asbestos or silica dust.China is thought to have the highest number of silicosis cases,with 6,000 new cases reported annually^([1]).Currently,the clinical diagnosis and monitoring of silicosis relies mainly on a history of occupational exposure and radiological abnormalities^([2]).Therefore,determining further indicators is crucial to reflect the severity of silicosis.
文摘Background: In the context of the fight against HIV, a lack of skills in monitoring and evaluating the personnel in charge of activities has been identified at the national level. It was the subject of a priority axis of the national plan for monitoring and evaluating the fight against HIV (2006-2010) that was aimed at strengthening the capacities of actors in this area. To increase the critical mass of competent human resources in the short term, the National Institute of Public Health (NIPH) of Côte d’Ivoire organized monitoring and evaluation training sessions for healthcare professionals from 2011 to 2016. Methods: A single case study with multiple levels of analysis was carried out, combining a qualitative survey and a literature review. An evaluation was carried out six months after each training session. In addition, the results of the pre- and post-tests and of the daily and final evaluations that accompanied the various training sessions were used to provide further information. The qualitative data collected were analyzed using INVIVO 15 software. Results: Some 89 health professionals (69% men and 31% women) working at the national level (51% at the central level, including 58% in health programs) and in development partner agencies (37%) participated in this capacity building program. Most participants were senior health managers (56%), data managers (23%), and statisticians and computer scientists (10%). Almost all the trainings were financed by 16 technical and financial partners (85%), mainly the MEASURE Evaluation project (27%). Conclusion: M&E training, despite all its imperfections, has made it possible to identify M&E training needs at the national level and to increase the critical mass of national skills and to have some culture in M&E.
文摘Under normal circumstances, spermatozoa are protected from the immune system by the blood-testis barrier. The breakdown of this barrier is the origin of the synthesis of antisperm antibodies (ASA). The presence of sperm agglutinates in semen is characteristic of ASA. But is the presence of agglutinates in semen necessarily linked to the level of ASA in semen? The objective of this study was to assess the concentration of anti-sperm antibodies (ASA) in normozoosperms and infertile men with azoospermia. The biological material consisted of samples of human sperms: 30 samples with azoospermia and 32 with normozoospermia. The ASA assay was performed in seminal plasma using the DRG® Sperm Antibody ELISA (seminal plasma) kit (EIA-4249). The reading was carried out using a microplate reader at 450 nm. Data analysis was performed using Graph Pad Prism 7 software. The results obtained showed that the difference in ASA concentration between these two categories of sperm was not significant, with an average ASA level of 31.54 ± 2.45 U/mL in azoospermic ejaculate and 27.63 ± 1.51 U/mL in normozoosperms. Statistical analysis showed higher ASA concentrations in azoosperms with 6.67% of these declared positive. The ASA positivity rate made it possible to distinguish secretory azoospermias from obstructive ones. Also, the presence of ASA is not necessarily linked to the presence of agglutinates in the semen.
文摘Introduction: The birth of a baby is a moment of joy and celebration. However, the neonatal period is a very delicate phase of life. Neonatal mortality rates remain high in low-income countries. In Guinea, after 20 years, this rate has fallen from 34.2% in 1998 to 32% in 2018. Objective: To identify the main risk factors for neonatal mortality. Methods: This was an observational, analytical case-control study, lasting 6 months from January 1 to June 30, 2019, conducted at the Institut de Nutrition et de la Santé de l’Enfant (INSE) at Donka National Hospital. Results: We collected 242 cases and 242 controls, i.e. a total of 484 records. 748 patients were registered, with 32.35% deaths. 82.86% of deaths occurred in the early neonatal period. Statistical analysis revealed the main risk factors: prematurity (RQ 7.39 95% CI 3.27 - 16.61 p = 0.0000003), hypothermia (RQ 2.29 95% CI 1.51 - 3.46 p = 0.0001), acute fetal distress (RQ 2.13 95% CI 1.33 - 3.43 p = 0.0016), low birth weight (QR 1.91 95% CI 1.12 - 3.24 p = 0.016), home birth (QR 3.26 95% CI 1.25 - 8.46 p = 0.015). Conclusion: Neonatal mortality is a health problem in the INSE neonatology department. To reduce the mortality rate in this referral facility, it is essential to equip it and provide ongoing training for staff.
文摘Background:Hodgkin lymphoma refers to a malignancy of the lymphatic tissue.Extra-nodal Hodgkin lymphoma(ENHL)is a rare variant consisting of Hodgkin lymphoma occurring outside the lymphatic system.Studies investigating the epidemiology associated with ENHL are rare.Methods:618 ENHL cases were analyzed using the National Cancer Institute’s Surveillance,Epidemiology,and End Results(SEER)database(2000–2020).Demographics including sex,race/ethnicity,rural-urban continuum,age group(categorized as adolescent and young adult(AYA)if between years of 15 and 39,and adult if over the age of 40),and living status(alive/deceased)were examined,with survival status as the main outcome.Results:This study included 335 males(54.2%)and 283 females(45.8%).Of this total,47.9%were in the AYA group.In terms of ethnicity the distribution was 12.5%non-Hispanic Black,67.5%non-Hispanic White,5.2%non-Hispanic Asian/Pacific Islander,and 14.9%Hispanic.Bivariate analyses evidenced significant differences in survival by age group with 91.6%in AYA vs.72.4%in adults(P<0.001)alive by the end of the study period.Multivariable analyses identified age as a key predictor of survival,as the AYA patients had a lower odds ratio for death(odds ratio=0.25,P<0.001).In addition,survival outcomes were also impacted by race,with non-Hispanic Blacks showing higher survival probabilities.Regarding treatment,27.0%of patients underwent surgery,with 10.2%receiving post-surgery radiation,reducing odds of mortality(odds ratio=0.32,P=0.046).Conclusion:The background research as such,tends to affirm that these two factors–age and race are quite crucial in the prognosis as well as management of ENHL.Compared to adults,AYA patients had significantly lower odds of death,while non-Hispanic Black individuals exhibited reduced survival probabilities.It should be noted that 27.0%of patients underwent surgery with 10.2%receiving post-operative radiation which led to decrease in mortality rates.Thus,these results reiterate the necessity for tailor-made treatment methods according to demographic characteristics to boost patient outcomes effectively.For better ENHL care,future studies could shed light on these disparities and improve treatment regimens as needed.
