OBJECTIVE:To evaluate the therapeutic effect of a selfformulated Traditional Chinese Medicine(TCM),Shugan Jieyu San(疏肝解郁散),on improvement of liver function and depression alleviation in the patients with triple n...OBJECTIVE:To evaluate the therapeutic effect of a selfformulated Traditional Chinese Medicine(TCM),Shugan Jieyu San(疏肝解郁散),on improvement of liver function and depression alleviation in the patients with triple negative breast cancer(TNBC).METHODS:A total of 60 patients diagnosed with TNBC and depression were enrolled and randomly assigned to either the control group(receiving routine tumor treatment and duloxetine)or the treatment group(receiving the TCM in addition to routine treatment).RESULTS:Both treatment and control groups showed a reduction in depressive symptoms and improved daily living abilities after treatment.However,the treatment group demonstrated significantly better outcomes compared to the control group.Furthermore,the levels of dopamine and serotonin in the serum increased in both groups after 8 weeks of treatment,while the treatment group exhibited superior results.CONCLUSIONS:This TCM showed promising results in reducing depressive symptoms and improving daily abilities in patients with TNBC and comorbid depression,which was verified by the increase in serum levels of dopamine and serotonin,suggesting the potential involvement of these neurotransmitters in the therapeutic effects of this TCM.展开更多
AIM:To investigate the prognostic value of preoperative platelet count(PLT) in patients with primary gallbladder cancer(GBC).METHODS:The clinical data of 223 GBC patients after surgery was retrospectively reviewed.A r...AIM:To investigate the prognostic value of preoperative platelet count(PLT) in patients with primary gallbladder cancer(GBC).METHODS:The clinical data of 223 GBC patients after surgery was retrospectively reviewed.A receiver operating characteristic(ROC) curve was plotted to verify the optimum cutoff point for PLT.Univariate and multivariate survival analyses were performed to identify the factors associated with the prognosis.RESULTS:The ROC curve showed that the optimum cutoff point for PLT was 178 × 109/L,and the entire cohort was stratified into group A with PLT > 178 × 109/L and group B with PLT ≤ 178 × 109/L.Group A had a better survival than group B(P < 0.001).There was an obvious difference between the two groups in terms of the differentiation degree,advanced tumor stage,lymph node metastasis(P < 0.001) and pathological type(P < 0.05).The univariate analysis demonstrated that tumor location,differentiation degree,TNM stage,Nevin stage,lymph node metastasis and PLT were associated with overall survival(P < 0.001).In the multivariate analysis,PLT(P = 0.032),lymph node metastasis(P = 0.007),tumor location(P < 0.001) and TNM stage(P = 0.005) were independent prognostic factors.CONCLUSION:PLT is closely correlated with GBC prognosis and could be used to identify the population with a poorer prognosis after surgery.展开更多

Both the hardware composition and the software realization of the system are introduced in detail. The experiment shows that the distinguishing ability of the temperature and that of the space are 1℃ and 2m,respectively,and that the system response time is about 180 s with a sensing range of 5km and with a temperature measurement range of 0-100℃.展开更多

Many risk factors are well documented for in cholangiocarcinoma, but the impacts of advanced biliary interventions, like endoscopic sphincterotomy(ES), endoscopic papillary balloon dilatation(EPBD),and cholecystectomy, are inconsistent in the previous literature.AIM To clarify the risks of cholangiocarcinoma after ES/EPBD, cholecystectomy or no intervention for cholelithiasis using the National Health Insurance Research Database(NHIRD).METHODS From data of NHIRD 2004-2011 in Taiwan, we selected 7938 cholelithiasis cases as well as 23814 control group cases(matched by sex and age in a 1:3 ratio). We compared the previous risk factors of cholangiocarcinoma and cholangiocarcinoma rate in the cholelithiasis and control groups. The incidences of total and subsequent cholangiocarcinoma were calculated in ES/EPBD patients, cholecystectomy patients, cholelithiasis patients without intervention,and groups from the normal population.RESULTS In total, 537 cases underwent ES/EPBD, 1743 cases underwent cholecystectomy,and 5658 cholelithiasis cases had no intervention. Eleven(2.05%), 37(0.65%), and7(0.40%) subsequent cholangiocarcinoma cases were diagnosed in the ES/EPBD,no intervention, and cholecystectomy groups, respectively, and the odds ratio for subsequent cholangiocarcinoma was 3.13 in the ES/EPBD group and 0.61 in the cholecystectomy group when compared with the no intervention group.CONCLUSION In conclusion, symptomatic cholelithiasis patients who undergo cholecystectomy can reduce the incidence of subsequent cholangiocarcinoma, while cholelithiasis patients who undergo ES/EPBD are at a great risk of subsequent cholangiocarcinoma according to our findings.展开更多
Harris Hawks Optimizer (HHO) is a recent well-established optimizer based on the hunting characteristics of Harris hawks, which shows excellent efficiency in solving a variety of optimization issues. However, it under...Harris Hawks Optimizer (HHO) is a recent well-established optimizer based on the hunting characteristics of Harris hawks, which shows excellent efficiency in solving a variety of optimization issues. However, it undergoes weak global search capability because of the levy distribution in its optimization process. In this paper, a variant of HHO is proposed using Crisscross Optimization Algorithm (CSO) to compensate for the shortcomings of original HHO. The novel developed optimizer called Crisscross Harris Hawks Optimizer (CCHHO), which can effectively achieve high-quality solutions with accelerated convergence on a variety of optimization tasks. In the proposed algorithm, the vertical crossover strategy of CSO is used for adjusting the exploitative ability adaptively to alleviate the local optimum;the horizontal crossover strategy of CSO is considered as an operator for boosting explorative trend;and the competitive operator is adopted to accelerate the convergence rate. The effectiveness of the proposed optimizer is evaluated using 4 kinds of benchmark functions, 3 constrained engineering optimization issues and feature selection problems on 13 datasets from the UCI repository. Comparing with nine conventional intelligence algorithms and 9 state-of-the-art algorithms, the statistical results reveal that the proposed CCHHO is significantly more effective than HHO, CSO, CCNMHHO and other competitors, and its advantage is not influenced by the increase of problems’ dimensions. Additionally, experimental results also illustrate that the proposed CCHHO outperforms some existing optimizers in working out engineering design optimization;for feature selection problems, it is superior to other feature selection methods including CCNMHHO in terms of fitness, error rate and length of selected features.展开更多

BACKGROUND Barrett’s esophagus(BE),which has increased in prevalence worldwide,is a precursor for esophageal adenocarcinoma.Although there is a gap in the detection rates between endoscopic BE and histological BE in ...BACKGROUND Barrett’s esophagus(BE),which has increased in prevalence worldwide,is a precursor for esophageal adenocarcinoma.Although there is a gap in the detection rates between endoscopic BE and histological BE in current research,we trained our artificial intelligence(AI)system with images of endoscopic BE and tested the system with images of histological BE.AIM To assess whether an AI system can aid in the detection of BE in our setting.METHODS Endoscopic narrow-band imaging(NBI)was collected from Chung Shan Medical University Hospital and Changhua Christian Hospital,resulting in 724 cases,with 86 patients having pathological results.Three senior endoscopists,who were instructing physicians of the Digestive Endoscopy Society of Taiwan,independently annotated the images in the development set to determine whether each image was classified as an endoscopic BE.The test set consisted of 160 endoscopic images of 86 cases with histological results.RESULTS Six pre-trained models were compared,and EfficientNetV2B2(accuracy[ACC]:0.8)was selected as the backbone architecture for further evaluation due to better ACC results.In the final test,the AI system correctly identified 66 of 70 cases of BE and 85 of 90 cases without BE,resulting in an ACC of 94.37%.CONCLUSION Our AI system,which was trained by NBI of endoscopic BE,can adequately predict endoscopic images of histological BE.The ACC,sensitivity,and specificity are 94.37%,94.29%,and 94.44%,respectively.展开更多

This study explores how coordinated planning between two large public ownerships in Minnesota impact landscape-level...Background: Many forested landscapes throughout the world involve a mix of public forest ownerships. This study explores how coordinated planning between two large public ownerships in Minnesota impact landscape-level trade-offs between timber production and production of core area of older forest (COF) for the region. COF is an important metric for wildlife habitat. Emphasis is on better understanding potential gains from both coordinated planning at the site-level where ownerships share stand boundaries and from coordinated planning at a broader policy level involving assumed values of COF by the public. The study area involves over 300,000 ha, 150,000 analysis units and a 100-year planning horizon. Methods: The concept of influence zones in modeling spatial interdependencies is described and implemented. The estimated total area of COF is assumed an important landscape metric for forest wildlife habitat condition for each forest planning period. COF has a surrounding buffer protecting it from edge effects. Differences are recognized between COF condition requirements and condition requirements for its surrounding buffer. A spatially-explicit harvest scheduling model is applied in conjunction with moving-windows techniques of GIS to find near-optimal management schedules for the large landscape. Multiple model runs are examined to help better understand both potential gains from coordinated planning and the tradeoffs between timber and COF production. Results: Results demonstrate the ability to incorporate detailed site-level COF production into management scheduling models for broad, landscape-level planning. For the study area and the assumed COF definitions, substantially larger gains are possible by coordinating COF value assumptions across ownerships, as compared to possible gains from coordinating on-the-ground management activities in areas involving shared stand boundaries. Although a general map of the study area shows a definite intertwining mosaic of ownership by the two large public agencies, a detailed breakdown of influence zone information shows that a low percentage of the land is influenced by both of these ownerships for COF production. Conclusions: This research helps illuminate potential large gains from coordinated planning at a broad policy level by large public ownerships through coordination of assumed COF values. For the study area, these gains are substantially greater than gains from combined modeling efforts addressing spatial detail and shared stand boundaries or neighborhoods. From a practical standpoint, this is important, as spatial detail adds substantially to model size, making combined analysis a major undertaking. Detailed site-level coordination also presents operational challenges in schedule implementation.展开更多

Eleven(2.05%), 37(0.65%), and7(0.40%) subsequent cholangiocarcinoma cases were diagnosed in the ES/EPBD,no intervention, and cholecystectomy groups, respectively, and the odds ratio for subsequent cholangiocarcinoma was 3.13 in the ES/EPBD group and 0.61 in the cholecystectomy group when compared with the no intervention group.CONCLUSION In conclusion, symptomatic cholelithiasis patients who undergo cholecystectomy can reduce the incidence of subsequent cholangiocarcinoma, while cholelithiasis patients who undergo ES/EPBD are at a great risk of subsequent cholangiocarcinoma according to our findings.
文摘Harris Hawks Optimizer (HHO) is a recent well-established optimizer based on the hunting characteristics of Harris hawks, which shows excellent efficiency in solving a variety of optimization issues. However, it undergoes weak global search capability because of the levy distribution in its optimization process. In this paper, a variant of HHO is proposed using Crisscross Optimization Algorithm (CSO) to compensate for the shortcomings of original HHO. The novel developed optimizer called Crisscross Harris Hawks Optimizer (CCHHO), which can effectively achieve high-quality solutions with accelerated convergence on a variety of optimization tasks. In the proposed algorithm, the vertical crossover strategy of CSO is used for adjusting the exploitative ability adaptively to alleviate the local optimum;the horizontal crossover strategy of CSO is considered as an operator for boosting explorative trend;and the competitive operator is adopted to accelerate the convergence rate. The effectiveness of the proposed optimizer is evaluated using 4 kinds of benchmark functions, 3 constrained engineering optimization issues and feature selection problems on 13 datasets from the UCI repository. Comparing with nine conventional intelligence algorithms and 9 state-of-the-art algorithms, the statistical results reveal that the proposed CCHHO is significantly more effective than HHO, CSO, CCNMHHO and other competitors, and its advantage is not influenced by the increase of problems’ dimensions. Additionally, experimental results also illustrate that the proposed CCHHO outperforms some existing optimizers in working out engineering design optimization;for feature selection problems, it is superior to other feature selection methods including CCNMHHO in terms of fitness, error rate and length of selected features.

文摘BACKGROUND Barrett’s esophagus(BE),which has increased in prevalence worldwide,is a precursor for esophageal adenocarcinoma.Although there is a gap in the detection rates between endoscopic BE and histological BE in current research,we trained our artificial intelligence(AI)system with images of endoscopic BE and tested the system with images of histological BE.AIM To assess whether an AI system can aid in the detection of BE in our setting.METHODS Endoscopic narrow-band imaging(NBI)was collected from Chung Shan Medical University Hospital and Changhua Christian Hospital,resulting in 724 cases,with 86 patients having pathological results.Three senior endoscopists,who were instructing physicians of the Digestive Endoscopy Society of Taiwan,independently annotated the images in the development set to determine whether each image was classified as an endoscopic BE.The test set consisted of 160 endoscopic images of 86 cases with histological results.RESULTS Six pre-trained models were compared,and EfficientNetV2B2(accuracy[ACC]:0.8)was selected as the backbone architecture for further evaluation due to better ACC results.In the final test,the AI system correctly identified 66 of 70 cases of BE and 85 of 90 cases without BE,resulting in an ACC of 94.37%.CONCLUSION Our AI system,which was trained by NBI of endoscopic BE,can adequately predict endoscopic images of histological BE.The ACC,sensitivity,and specificity are 94.37%,94.29%,and 94.44%,respectively.

文摘Background: Many forested landscapes throughout the world involve a mix of public forest ownerships. This study explores how coordinated planning between two large public ownerships in Minnesota impact landscape-level trade-offs between timber production and production of core area of older forest (COF) for the region. COF is an important metric for wildlife habitat. Emphasis is on better understanding potential gains from both coordinated planning at the site-level where ownerships share stand boundaries and from coordinated planning at a broader policy level involving assumed values of COF by the public. The study area involves over 300,000 ha, 150,000 analysis units and a 100-year planning horizon. Methods: The concept of influence zones in modeling spatial interdependencies is described and implemented. The estimated total area of COF is assumed an important landscape metric for forest wildlife habitat condition for each forest planning period. COF has a surrounding buffer protecting it from edge effects. Differences are recognized between COF condition requirements and condition requirements for its surrounding buffer. A spatially-explicit harvest scheduling model is applied in conjunction with moving-windows techniques of GIS to find near-optimal management schedules for the large landscape. Multiple model runs are examined to help better understand both potential gains from coordinated planning and the tradeoffs between timber and COF production. Results: Results demonstrate the ability to incorporate detailed site-level COF production into management scheduling models for broad, landscape-level planning. For the study area and the assumed COF definitions, substantially larger gains are possible by coordinating COF value assumptions across ownerships, as compared to possible gains from coordinating on-the-ground management activities in areas involving shared stand boundaries. Although a general map of the study area shows a definite intertwining mosaic of ownership by the two large public agencies, a detailed breakdown of influence zone information shows that a low percentage of the land is influenced by both of these ownerships for COF production. Conclusions: This research helps illuminate potential large gains from coordinated planning at a broad policy level by large public ownerships through coordination of assumed COF values. For the study area, these gains are substantially greater than gains from combined modeling efforts addressing spatial detail and shared stand boundaries or neighborhoods. From a practical standpoint, this is important, as spatial detail adds substantially to model size, making combined analysis a major undertaking. Detailed site-level coordination also presents operational challenges in schedule implementation.
