Since it first appeared in 2022,the phenomenon referred to as Colony Collapse Disorder(CCD)has affected several regions of Morocco to varying degrees.In order to assess the possible impact of pesticides on the appeara...Since it first appeared in 2022,the phenomenon referred to as Colony Collapse Disorder(CCD)has affected several regions of Morocco to varying degrees.In order to assess the possible impact of pesticides on the appearance of this syndrome,we conducted a study aimed at evaluating the impact of pesticide use on the emergence of this syndrome through a year-long survey involving 160 beekeepers in the Beni Mellal–Khenifra Region(BKR)who also experienced an unprecedented desertion of hives during the same period.The majority of surveyed beekeepers practice mixed(45%)or migratory beekeeping(42%)and provide supplementary feeding(83.75%)to support their bees.Nearly 37.5%of the hives are located near crops treated with pesticides,exposing the bees to these chemicals.The results showed that the majority of beekeepers reported a cessation of queen laying(74.38%),high mortality rates among worker bees(81.25%),drones(65.63%),and queens(61.88%).Abnormal behaviors such as immobility with trembling(42.50%),reduced flights(47.50%),and disoriented navigation(28.75%)were also observed.Correlation analyses indicate that proximity to treated crops significantly increases the risk of queen laying cessation(Odds Ratio 6.0)and a reduction in waggle dances(Odds Ratio 2.41).Extended foraging flights show a borderline statistical significance(Odds Ratio 2.33),suggesting a disruption of natural food sources.These results highlight the potential impact of pesticides on colony health and bee behavior,pointing out the need to adapt beekeeping practices and implement protective measures against pesticides.展开更多

Until recently, the functions of border-associated macrophages have been poorly understood and largely overlooked. However, a recent study reported that border-associated macrophages participate in stroke-induced inflammation, although many details and the underlying mechanisms remain unclear. In this study, we performed a comprehensive single-cell analysis of mouse border-associated macrophages using sequencing data obtained from the Gene Expression Omnibus(GEO) database(GSE174574 and GSE225948). Differentially expressed genes were identified, and enrichment analysis was performed to identify the transcription profile of border-associated macrophages. CellChat analysis was conducted to determine the cell communication network of border-associated macrophages. Transcription factors were predicted using the ‘pySCENIC' tool. We found that, in response to hypoxia, borderassociated macrophages underwent dynamic transcriptional changes and participated in the regulation of inflammatory-related pathways. Notably, the tumor necrosis factor pathway was activated by border-associated macrophages following ischemic stroke. The pySCENIC analysis indicated that the activity of signal transducer and activator of transcription 3(Stat3) was obviously upregulated in stroke, suggesting that Stat3 inhibition may be a promising strategy for treating border-associated macrophages-induced neuroinflammation. Finally, we constructed an animal model to investigate the effects of border-associated macrophages depletion following a stroke. Treatment with liposomes containing clodronate significantly reduced infarct volume in the animals and improved neurological scores compared with untreated animals. Taken together, our results demonstrate comprehensive changes in border-associated macrophages following a stroke, providing a theoretical basis for targeting border-associated macrophages-induced neuroinflammation in stroke treatment.展开更多
Light plays an essential role in psychobiological and psychophysiological processes,such as alertness.The alerting effect is influenced by light characteristics and the timing of interventions.This meta-analysis is th...Light plays an essential role in psychobiological and psychophysiological processes,such as alertness.The alerting effect is influenced by light characteristics and the timing of interventions.This meta-analysis is the first to systematically review the effect of light intervention on alertness and to discuss the optimal protocol for light intervention.In this meta-analysis,registered at PROSPERO(Registration ID:CRD42020181485),we conducted a systematic search of the Web of Science,PubMed,and PsycINFO databases for studies published in English prior to August 2021.The outcomes included both subjective and objective alertness.Subgroup analyses considered a variety of factors,such as wavelength,correlated color temperature(CCT),light illuminance,and timing of interventions(daytime,night-time,or all day).Twenty-seven crossover studies and two parallel-group studies were included in this meta-analysis,with a total of 1210 healthy participants(636(52%)male,mean age 25.62 years).The results revealed that light intervention had a positive effect on both subjective alertness(standardized mean difference(SMD)=-0.28,95%confidence interval(CI):-0.49 to-0.06,P=0.01)and objective alertness in healthy subjects(SMD=-0.34,95%CI:-0.68 to-0.01,P=0.04).The subgroup analysis revealed that cold light was better than warm light in improving subjective alertness(SMD=-0.37,95%CI:-0.65 to-0.10,P=0.007,I2=26%)and objective alertness(SMD=-0.36,95%CI:-0.66 to-0.07,P=0.02,I2=0).Both daytime(SMD=-0.22,95%CI:-0.37 to-0.07,P=0.005,I2=74%)and night-time(SMD=-0.32,95%CI:-0.61 to-0.02,P=0.04,I2=0)light exposure improved subjective alertness.The results of this meta-analysis and systematic review indicate that light exposure is associated with significant improvement in subjective and objective alertness.In addition,light exposure with a higher CCT was more effective in improving alertness than light exposure with a lower CCT.Our results also suggest that both daytime and night-time light exposure can improve subjective alertness.展开更多
Detection of SARS-CoV-2 RNA in wastewater is a promising tool for informing public health decisions during the COVID-19 pandemic.However,approaches for its analysis by use of reverse transcription quantitative polymer...Detection of SARS-CoV-2 RNA in wastewater is a promising tool for informing public health decisions during the COVID-19 pandemic.However,approaches for its analysis by use of reverse transcription quantitative polymerase chain reaction(RT-q PCR)are still far from standardized globally.To characterize inter-and intra-laboratory variability among results when using various methods deployed across Canada,aliquots from a real wastewater sample were spiked with surrogates of SARS-CoV-2(gamma-radiation inactivated SARS-CoV-2 and human coronavirus strain 229E[HCoV-229E])at low and high levels then provided"blind"to eight laboratories.Concentration estimates reported by individual laboratories were consistently within a 1.0-log_(10) range for aliquots of the same spiked condition.All laboratories distinguished between low-and high-spikes for both surrogates.As expected,greater variability was observed in the results amongst laboratories than within individual laboratories,but SARS-CoV-2 RNA concentration estimates for each spiked condition remained mostly within 1.0-log_(10) ranges.The no-spike wastewater aliquots provided yielded non-detects or trace levels(<20 gene copies/mL)of SARS-CoV-2 RNA.Detections appear linked to methods that included or focused on the solids fraction of the wastewater matrix and might represent in-situ SARS-CoV-2 to the wastewater sample.HCoV-229E RNA was not detected in the no-spike aliquots.Overall,all methods yielded comparable results at the conditions tested.Partitioning behavior of SARS-CoV-2 and spiked surrogates in wastewater should be considered to evaluate method effectiveness.A consistent method and laboratory to explore wastewater SARS-CoV-2 temporal trends for a given system,with appropriate quality control protocols and documented in adequate detail should succeed.展开更多

This overuse use does not contribute to the quality of medical care, does not shorten hospital stay, nor reduce mortality. The utilization of diagnostic laboratories has increased over the last decade around the world. This increased laboratory use is appropriate if it allows accurate diagnoses to be made, ideal treatment to be identified and monitored, accurate prognoses to be established, and patients’ hospital stays to be shortened. Thus, improving the appropriateness of testing behavior and reducing the number of laboratory tests have been recognized as essential parts of quality improvement program. In this study, the effectiveness of a computer-based system in improving the laboratory test-ordering in a general hospital was investigated. The study was conducted through four stages, the preparation stage, the pre-intervention stage, the post-intervention 1) stage and post-intervention 2) stage. Guideline and computer system were developed during preparation stage. Medical records were reviewed against guideline recommendations before any intervention during the pre-intervention stage, after guideline dissemination through educational workshops during the post intervention 1) stage, and after implementation of the computer system with the new requesting form during the post intervention 2) stage. The study revealed that the computer-based system achieved a statistically significant increase in the percentage of appropriate use from 44.6% in the post-intervention 1) stage to 55.6%, and a statistically significant increase in the compliance with guideline by prescriber as well as increased in guideline conformity rate from 16.7% in the post-intervention 1) stage to 32.5% in the post-intervention 2) stage, and decreased in the percentage of prescribers whose level was unsatisfactory from 85.4% the post-intervention 1) stage to 66.7% in the post-intervention 2) stage.展开更多

We conducted a seroprevalence study to evaluate the magnitude of SARS-CoV-2 exposure among healthcare workers, school children, and people who attended general markets in Zanzibar. The objectives of the study were to analyse the total antibodies from selected higher-risk population groups in order to determine magnitude in SARS CoV-2 exposure. Blood samples were collected from eligible and consented participants (adults and children), and their serum was analyzed for total antibodies against SARS-CoV-2 using ELISA. A questionnaire was used to collect participants’ demographic and clinical data. The overall SARS-CoV-2 seroprevalence across all age groups was 33%, and a higher seroprevalence was observed in the 40 - 49 years’ age group relative to other ages as well as in those who attended markets. A runny nose (18.8% of participants) was the most frequently reported SARS-CoV-2 infection-related symptom. Multivariable analysis showed significantly higher odds of infection in people living in urban districts. The findings provide insight into SARS-CoV-2 infection among school children, health workers, and people who attended markets in Zanzibar in the early stages of the pandemic. Exposure in these groups might have been influenced by infection and prevention strategies taken by the government, as well as shopping behavior, school overcrowding, and population density in urban settings. The study had methodological limitations, including cross-sectional design. Further, well-designed, longitudinal studies are recommended to understand exposure and transmission at a population level.展开更多
Background:China’s accelerating development and increasingly important role in global health engagement create a great demand for global health professionals including international consulting experts.This study repo...Background:China’s accelerating development and increasingly important role in global health engagement create a great demand for global health professionals including international consulting experts.This study reported the detailed development and evaluation of an international consulting training for global health workforce.Methods:Based on Kirkpatrick’s model,a mixed-methods approach was used to evaluate the effectiveness of the training.Quantitative and qualitative data on participants’reaction,learning,and application of the learned knowledge and skills were collected by a training evaluation survey at the ending of training and a follow-up interview in three months after the training.Results:Thirty-six participants attended the training and 34 of them completed quantitative investigation.The training satisfaction evaluations were positive,for which participants rated the training program highly and over 90%of them agreed with the usefulness of the training.About knowledge and skills change,participants showed improved consulting knowledge and skills from pre-to post-training(P<0.001).A total of 23 participants accepted follow-up interview,and most participants applied knowledge and skills learned from the training in their daily work or study.However,only 30.4%of participants applied their learning in the consulting program.The largest barrier of application was the lack of consulting opportunities.In addition,almost all the participants reported that they would be glad to attend more training courses in the future.Conclusion:The international consulting training program was well-received,and was feasible to improve the consulting service competence of global health professionals.According to participants’feedback,it is essential to develop and expand consulting training in the field of global health.展开更多

In order to tackle the emergence of resistant Neisseria gonorrhoeae, this study aimed to determine the prevalence and risk factors of penicillinase type β-lactamase-producing Neisseria gonorrheae among patients consulting for genital infectious disorders in two health-facilities in Yaounde, Cameroon. Materials and Method: A cross-sectional descriptive and analytical study was conducted over a 3-month period, from July 2<sup>nd</sup> to October 2<sup>nd</sup>, 2022. Vaginal and urethral secretions were collected. Biochemical identification tests were performed on colonies grown on chocolate agar + polyvitex using the Api NH gallery. The detection of penicillinases was equally performed using the API NH gallery and confirmed using the antimicrobial susceptibility testing. The Minimum Inhibitory Concentrations of some antibiotics were determined using the E-Test. Results: The results showed that out of the 198 patients sampled, 16 (8.08%) were positive for Neisseria gonorrhoeae, among which 13/16 (81.25%) were penicillinase-type β-lactamase producers. Antimicrobial susceptibility testing results showed high co-resistances to antibiotics, mainly ciprofloxacin (100%), nalidixic acid (92.31%) and azithromycin (84.62%). Moreover, high Minimum Inhibitory Concentrations of ceftriaxone (ranging from 6 to 24 mg/L) was observed toward Neisseria gonorrhoeae isolates. The risk factors of the carriage of penicillinase-type β-lactamase producing Neisseria gonorrhoeae identified were: a history of Sexually Transmitted infections (p = 0.01) and unprotected sexual intercourse (p = 0.01). Conclusion: The emergence of penicillinase-type β-lactamase producing Neisseria gonorrhoeae is increasing and the situation is becoming worrisome. The identified risk factors can constitute a basic outlook to tackle resistant Neisseria gonorrhoeae, and therefore sustain antibiotic stewardship.展开更多

Methods: Due to the temporary effectiveness (increase of the temperature of water in boilers and shock hyperchlorination), the high costs (point-of-use water filters) or the ineffectiveness (hydrogen peroxide, H202) of the previous control procedures, 3 devices (one for each hot water loop) continuously injecting monochloramine have been installed in two different Italian hospitals heavily contaminated by Legionellapneumophila SG3 and SG6. Aim: To evaluate the efficacy of continuous disinfection by monochloramine for control of Legionella on domestic hot water (DHW) distribution system of health-care facilities. Findings: One month after the disinfection of DHW with monochloramine, the load ofL. pneumophila SG3 and SG6 (previous mean count ranging from 103 to 105 CFU/L), as well as the Heterotrophic Plate Count (HPC) (previous mean count ranging from 〉 10^2 to 〉 10^4 CFU/mL), decreased at undetectable levels in 100% of the sampling points in the two hospitals. Conclusion: The results suggest that continuous injection of monochloramine on DHW systems can fully control L. pneumophila and HPC in contaminated hospitals since the beginning of its application.展开更多

In this study, we determined the constituents of unleaded and diesel fuel sold in the District of Abidjan, notably sulfur and lead, and assessed the impact on mechanics, pump attendants and painters. Twenty tow fuel samples collected in the district of Abidjan, including 14 from legal sites distribution and 8 from illegal sites, were analyzed to determine the sulfur and lead content. Blood was collected among 45 volunteers, including 15 mechanics, 15 painters and 15 pump attendants to hematological and biochemical analysis. The results show that sulfur levels ranged from 1530 ppm to 1900 ppm and from 94.9 ppm to 9.4 ppm in diesel and unleaded fuel respectively. Sulfur values below Côte d’Ivoire standard of 1500 - 3500 ppm for diesel and 500 - 1500 ppm for unleaded fuel but remain above international standard of 10 pp. Fuel characterization did not show the presence of lead in any of the tested fuel samples. Blood results showed MCHC values above of reference values for all professionals, while MCH values were below reference values. Neutrophil values were low compared with reference values in all occupational groups of mechanics and pump attendants, while lymphocyte results were 48.20% and 52.50% for mechanics and fuel attendants respectively were well above reference values. GGT values ranged from 42 ± 14.98 IU/l to 87 ± 3.45 were well above reference values. However, the GGT levels of pump attendants were significantly (P < 0.05) higher than those of other professionals.展开更多

Method: A cross-sec...Objective: This study aims to analyze the relationship between active transportation, screen time and sleep quality among metabolically health versus unhealthy Congolese schoolboys and girls obese. Method: A cross-sectional study was conducted among 58 obese aged 15.36 ± 1.22 years in Brazzaville (Republic of Congo). They were divided into metabolically healthy obese (MHO, n = 29) and metabolically unhealthy obese (MUHO, n = 29). Data collection consisted of anthropometric measurements, lipids profile parameters, Pittsburgh Sleep Quality Index and the screen time measurements. The relationship was analyzed by using the logistic regression for healthy and unhealthy schoolboys and obese girls. Results: Compared to MUH obese subjects, MUHO obese subjects were significantly less engaged in active transportation (p = 0.03), TV and smartphone times significantly increased (p = 0.000 and p = 0.003), sleep quality significantly poor (p = 0.001). They were 1.85 (95% CI: 0.85 - 3.88) lower odds to engage in active transportation, had 1.82 (95% CI: 1.11 - 3.10) and 2.04 (95% CI: 1.11 - 3.10) higher odds of TV time respectively, had 1.87 (95% CI: 1.24 - 2.84) and 2.04 (95% CI: 1.47 - 2.85) higher odds of smartphone time respectively and have 2.35 (95% CI: 1.62 - 3.41) higher odds of poor sleep. Conclusion: MUHO subjects underwent high screen time and poor sleep quality. Higher TV-viewing/smartphone time and poorer sleeping quality were found to be associated with less time spent in active transportation. This bad habit on screen and sleep negatively affects the cardiometabolic parameters.展开更多
Background:The One Health approach involves collaboration across several sectors,including public health,veterinary and environmental sectors in an integrated manner.These sectors may be disparate and unrelated,howeve...Background:The One Health approach involves collaboration across several sectors,including public health,veterinary and environmental sectors in an integrated manner.These sectors may be disparate and unrelated,however to succeed,all stakeholders need to understand what the other stakeholders are communicating.Likewise,it is important that there is public acceptance and support of One Health approaches,which requires effective communication between professional and institutional organisations and the public.To help aid and facilitate such communication,written materials need to be readable by all stakeholders,in order to communicate effectively.There has been an exponential increase in the publication of papers involving One Health,with<5 per year,in the 2000s,to nearly 500 published in 2023.To date,readability of One Health information has not been scrutinised,nor has it been considered as an integral intervention of One Health policy communication.The aim of this study was therefore to examine readability of public-facing One Health information prepared by 24 global organisations.Methods:Readability was calculated using Readable software,to obtain four readability scores[(i)Flesch Reading Ease(FRE),(ii)Flesch-Kincaid Grade Level(FKGL),(iii)Gunning Fog Index and(iv)SMOG Index]and two text metrics[words/sentence,syllables/word]for 100 sources of One Health information,from four categories[One Health public information;PubMed abstracts;Science in One Health(SOH)abstracts(articles);SOH abstracts(reviews)].Results:Readability of One Health information for the public is poor,not reaching readability reference standards.No information was found that had a readability of less than 9th grade(around 14 years old).Mean values for the FRE and FKGL were(19.4±1.4)(target>60)and(15.6±0.3)(target<8),respectively,with mean words per sentence and syllables per word of 20.5 and 2.0,respectively.Abstracts with“One Health”in the title were more difficult to read than those without“One Health”in the title(FRE:P=0.0337;FKGL:P=0.0087).Comparison of FRE and FKGL readability scores for the four categories of One Health information[One Health public information;PubMed abstracts;SOH abstracts(articles);SOH abstracts(reviews)]showed that SOH abstracts from articles were easier to read than those from SOH reviews.No One Health public-facing information from the 100 sources examined met the FKGL target of8.The most easily read One Health information required a Grade Level of 9th grade(14-15 years old),with a mean Grade Level of 15.5(university/college level).Conclusion:Considerable work is required in making One Health written materials more readable,particularly for children and adolescents(<14 years of age).It is important that any interventions or mitigations taken to support better public understanding of the One Health approach are not ephemeral,but have longer lasting and legacy value.Authors of One Health information should consider using readability calculators when preparing One Health information for their stakeholders,to check the readability of their work,so that the final material is within recommended readability reference parameters,to support the health literacy and stakeholder-directed knowledge of their readers.展开更多
BACKGROUND Middle meningeal artery embolization(MMAE)is emerging as a promising treatment for chronic subdural hematoma(CSDH),serving both as an adjunct to surgery and as a primary therapeutic option depending on pati...BACKGROUND Middle meningeal artery embolization(MMAE)is emerging as a promising treatment for chronic subdural hematoma(CSDH),serving both as an adjunct to surgery and as a primary therapeutic option depending on patient presentation.Due to its low recurrence rate and minimal complications,MMAE has gained increasing acceptance among clinicians in recent years.This report presents a case of diplopia following MMAE due to the presence of a potential anastomotic artery,aiming to enhance awareness of this complication.CASE SUMMARY A 60-year-old male patient presented with a headache following head trauma,and cranial computed tomography revealed a left-sided CSDH.The patient underwent left MMAE;however,polyvinyl alcohol particles inadvertently flowed into the lacrimal artery through an anastomotic artery,resulting in diplopia due to impaired abduction of the left eye.The diplopia resolved by postoperative day 40.The patient’s headache resolved by postoperative day 7,and the hematoma completely resolved by postoperative day 108.CONCLUSION Potential anastomotic arteries in the middle meningeal artery(MMA)can lead to serious complications.Superselective angiography of the MMA or its branches prior to embolization is essential.Performing embolization distal to potential anastomotic sites can reduce risks,and the presence of an anastomosis may warrant coil embolization or termination of the procedure.展开更多
Male infertility can result from impaired sperm motility caused by multiple morphological abnormalities of the flagella(MMAF).Distinct projections encircling the central microtubules of the spermatozoal axoneme play p...Male infertility can result from impaired sperm motility caused by multiple morphological abnormalities of the flagella(MMAF).Distinct projections encircling the central microtubules of the spermatozoal axoneme play pivotal roles in flagellar bending and spermatozoal movement.Mammalian sperm-associated antigen 17(SPAG17)encodes a conserved axonemal protein of cilia and flagella,forming part of the C1a projection of the central apparatus,with functions related to ciliary/flagellar motility,skeletal growth,and male fertility.This study investigated two novel homozygous SPAG17 mutations(M1:NM_206996.2,c.829+1G>T,p.Asp212_Glu276del;and M2:c.2120del,p.Leu707*)identified in four infertile patients from two consanguineous Pakistani families.These patients displayed the MMAF phenotype confirmed by Papanicolaou staining and scanning electron microscopy assays of spermatozoa.Quantitative real-time polymerase chain reaction(PCR)of patients’spermatozoa also revealed a significant decrease in SPAG17 mRNA expression,and immunofluorescence staining showed the absence of SPAG17 protein signals along the flagella.However,no apparent ciliary-related symptoms or skeletal malformations were observed in the chest X-rays of any of the patients.Transmission electron microscopy of axoneme cross-sections from the patients showed incomplete C1a projection and a higher frequency of missing microtubule doublets 1 and 9 compared with those from fertile controls.Immunofluorescence staining and Western blot analyses of spermatogenesis-associated protein 17(SPATA17),a component of the C1a projection,and sperm-associated antigen 6(SPAG6),a marker of the spring layer,revealed disrupted expression of both proteins in the patients’spermatozoa.Altogether,these findings demonstrated that SPAG17 maintains the integrity of spermatozoal flagellar axoneme,expanding the phenotypic spectrum of SPAG17 mutations in humans.展开更多

Objective:To evaluate the effectiveness of two doses of coenzyme Q10(CoQ10)on semen parameters,sperm DNA damage,and the partner pregnancy rate in men with idiopathic oligoasthenozoospermia.Methods:250 patients with id...Objective:To evaluate the effectiveness of two doses of coenzyme Q10(CoQ10)on semen parameters,sperm DNA damage,and the partner pregnancy rate in men with idiopathic oligoasthenozoospermia.Methods:250 patients with idiopathic oligoasthenozoospermia were examined.The first group(n=125)received 100 mg/day of CoQ10 and the second group(n=125)received 200 mg/day of CoQ10 orally for 6 months.Semen parameters,DNA fragmentation index(DFI)and the partner pregnancy rate were analyzed at baseline and after 6 months of treatment.Results:Comparing with baseline data,treatment with CoQ10(100 mg/day or 200 mg/day)resulted in a significant increase in sperm concentration(both P<0.001),a significant improvement in progressive motility and total motile sperm count(P=0.05,P=0.001,respectively).The mean DFI was significantly improved after treatment with CoQ10 at 100 mg/day and at 200 mg/day,after 6 months of treatment(P<0.01).Moreover,CoQ10 significantly improved the partner pregnancy rate.A strongest correlation was found between seminal fluid parameters and DFI(P<0.001).Conclusions:CoQ10 is effective in improving semen parameters,DFI and on the partner pregnancy rate after 6 months with CoQ10 at two doses,with a greater improvement shown in men who took 200 mg/day than in those who took 100 mg/day.展开更多

However, Antipalu could present potential health effects on patients, and few toxi...Antipalu is a phytomedicinal medicinal beverage that is popular in the District of Abidjan, particularly for the treatment of malaria. However, Antipalu could present potential health effects on patients, and few toxicological studies have been conducted before its use. In order to determine the cytotoxicity of Antipalu, two complementary tests, LDH activity and the MTT cell proliferation assay, were used using Vero cells. Vero cells were exposed to increasing concentrations of Antipalu and incubated for 24, 48 and 72 hours. In addition, forty (40) rats distributed randomly into 4 groups, including 10 animals per group (5 males and 5 females) were used for the potential hepatoxic effects. Animals in group 1 received distilled water and were used as a control group. On the other hand, Lot I, II and III received by gavage a volume of the Antipalu extract corresponding to 1 ml/100 g of body weight at 200 mg/kg, 400 mg/kg, 800 mg/kg, respectively. The extract was administered daily at the same time for 28 days and serum was collected once a week to evaluate hepatic biochemical markers. After 28 days of study, all rats were euthanized by an overdose of ether and the liver of the rats was removed for gross morphological and histopathological analysis. The results of the cell supernatant assay showed an increasing extracellular LDH enzyme activity with lethal concentrations at 10% and 50% (LC10 = 111 µg/mL and LC50 = 555 µg/mL, respectively). In addition, the MTT assay showed a decrease in mitochondrial activity and thus cell proliferation after 24, 48 and 72 H of incubation. Our study showed that Antipalu caused alterations in the plasma membranes of the cells, resulting in the release of lactase dehydrogenase (LDH) into the external environment and a decrease in the mitochondrial activity of the Vero cells. The biochemical parameters ALT, ASAT, ALPs, and GGT showed no significant change (P > 0.05) in the group of treated rats compared to the controls. However, these variations were moderate and transient, with values remaining almost within their standard limits. Microscopic observations of liver tissue sections from rats treated with the Antipalu showed no lesions, edema and necrosis. These results suggest that the Antipalu did not interfere with the functioning or alter the integrity of the liver.展开更多
文摘Since it first appeared in 2022,the phenomenon referred to as Colony Collapse Disorder(CCD)has affected several regions of Morocco to varying degrees.In order to assess the possible impact of pesticides on the appearance of this syndrome,we conducted a study aimed at evaluating the impact of pesticide use on the emergence of this syndrome through a year-long survey involving 160 beekeepers in the Beni Mellal–Khenifra Region(BKR)who also experienced an unprecedented desertion of hives during the same period.The majority of surveyed beekeepers practice mixed(45%)or migratory beekeeping(42%)and provide supplementary feeding(83.75%)to support their bees.Nearly 37.5%of the hives are located near crops treated with pesticides,exposing the bees to these chemicals.The results showed that the majority of beekeepers reported a cessation of queen laying(74.38%),high mortality rates among worker bees(81.25%),drones(65.63%),and queens(61.88%).Abnormal behaviors such as immobility with trembling(42.50%),reduced flights(47.50%),and disoriented navigation(28.75%)were also observed.Correlation analyses indicate that proximity to treated crops significantly increases the risk of queen laying cessation(Odds Ratio 6.0)and a reduction in waggle dances(Odds Ratio 2.41).Extended foraging flights show a borderline statistical significance(Odds Ratio 2.33),suggesting a disruption of natural food sources.These results highlight the potential impact of pesticides on colony health and bee behavior,pointing out the need to adapt beekeeping practices and implement protective measures against pesticides.
文摘As the population of older Asian Americans continues to grow rapidly,understanding their mental health needs has become increasingly critical.This literature review summarizes current issues,developments,and future directions in addressing the mental health challenges faced by older Asian Americans in the last five years.We briefly touch on the comparative prevalence of mental health disorders experienced by ethnic subgroups.Additionally,we review recent studies highlighting the role of the coronavirus disease 2019,racism,social support,cultural stigma,and self-rated health as significant factors influencing mental well-being of this population.We discuss the utilization of health services among older Asian Americans.We conclude with thoughts for future research,emphasizing the importance of longitudinal studies,consideration of diverse Asian American ethnic subgroups,and culturally sensitive diagnostic and treatment approaches.

文摘Border-associated macrophages are located at the interface between the brain and the periphery, including the perivascular spaces, choroid plexus, and meninges. Until recently, the functions of border-associated macrophages have been poorly understood and largely overlooked. However, a recent study reported that border-associated macrophages participate in stroke-induced inflammation, although many details and the underlying mechanisms remain unclear. In this study, we performed a comprehensive single-cell analysis of mouse border-associated macrophages using sequencing data obtained from the Gene Expression Omnibus(GEO) database(GSE174574 and GSE225948). Differentially expressed genes were identified, and enrichment analysis was performed to identify the transcription profile of border-associated macrophages. CellChat analysis was conducted to determine the cell communication network of border-associated macrophages. Transcription factors were predicted using the ‘pySCENIC' tool. We found that, in response to hypoxia, borderassociated macrophages underwent dynamic transcriptional changes and participated in the regulation of inflammatory-related pathways. Notably, the tumor necrosis factor pathway was activated by border-associated macrophages following ischemic stroke. The pySCENIC analysis indicated that the activity of signal transducer and activator of transcription 3(Stat3) was obviously upregulated in stroke, suggesting that Stat3 inhibition may be a promising strategy for treating border-associated macrophages-induced neuroinflammation. Finally, we constructed an animal model to investigate the effects of border-associated macrophages depletion following a stroke. Treatment with liposomes containing clodronate significantly reduced infarct volume in the animals and improved neurological scores compared with untreated animals. Taken together, our results demonstrate comprehensive changes in border-associated macrophages following a stroke, providing a theoretical basis for targeting border-associated macrophages-induced neuroinflammation in stroke treatment.

文摘Light plays an essential role in psychobiological and psychophysiological processes,such as alertness.The alerting effect is influenced by light characteristics and the timing of interventions.This meta-analysis is the first to systematically review the effect of light intervention on alertness and to discuss the optimal protocol for light intervention.In this meta-analysis,registered at PROSPERO(Registration ID:CRD42020181485),we conducted a systematic search of the Web of Science,PubMed,and PsycINFO databases for studies published in English prior to August 2021.The outcomes included both subjective and objective alertness.Subgroup analyses considered a variety of factors,such as wavelength,correlated color temperature(CCT),light illuminance,and timing of interventions(daytime,night-time,or all day).Twenty-seven crossover studies and two parallel-group studies were included in this meta-analysis,with a total of 1210 healthy participants(636(52%)male,mean age 25.62 years).The results revealed that light intervention had a positive effect on both subjective alertness(standardized mean difference(SMD)=-0.28,95%confidence interval(CI):-0.49 to-0.06,P=0.01)and objective alertness in healthy subjects(SMD=-0.34,95%CI:-0.68 to-0.01,P=0.04).The subgroup analysis revealed that cold light was better than warm light in improving subjective alertness(SMD=-0.37,95%CI:-0.65 to-0.10,P=0.007,I2=26%)and objective alertness(SMD=-0.36,95%CI:-0.66 to-0.07,P=0.02,I2=0).Both daytime(SMD=-0.22,95%CI:-0.37 to-0.07,P=0.005,I2=74%)and night-time(SMD=-0.32,95%CI:-0.61 to-0.02,P=0.04,I2=0)light exposure improved subjective alertness.The results of this meta-analysis and systematic review indicate that light exposure is associated with significant improvement in subjective and objective alertness.In addition,light exposure with a higher CCT was more effective in improving alertness than light exposure with a lower CCT.Our results also suggest that both daytime and night-time light exposure can improve subjective alertness.

文摘Detection of SARS-CoV-2 RNA in wastewater is a promising tool for informing public health decisions during the COVID-19 pandemic.However,approaches for its analysis by use of reverse transcription quantitative polymerase chain reaction(RT-q PCR)are still far from standardized globally.To characterize inter-and intra-laboratory variability among results when using various methods deployed across Canada,aliquots from a real wastewater sample were spiked with surrogates of SARS-CoV-2(gamma-radiation inactivated SARS-CoV-2 and human coronavirus strain 229E[HCoV-229E])at low and high levels then provided"blind"to eight laboratories.Concentration estimates reported by individual laboratories were consistently within a 1.0-log_(10) range for aliquots of the same spiked condition.All laboratories distinguished between low-and high-spikes for both surrogates.As expected,greater variability was observed in the results amongst laboratories than within individual laboratories,but SARS-CoV-2 RNA concentration estimates for each spiked condition remained mostly within 1.0-log_(10) ranges.The no-spike wastewater aliquots provided yielded non-detects or trace levels(<20 gene copies/mL)of SARS-CoV-2 RNA.Detections appear linked to methods that included or focused on the solids fraction of the wastewater matrix and might represent in-situ SARS-CoV-2 to the wastewater sample.HCoV-229E RNA was not detected in the no-spike aliquots.Overall,all methods yielded comparable results at the conditions tested.Partitioning behavior of SARS-CoV-2 and spiked surrogates in wastewater should be considered to evaluate method effectiveness.A consistent method and laboratory to explore wastewater SARS-CoV-2 temporal trends for a given system,with appropriate quality control protocols and documented in adequate detail should succeed.
文摘The overuse of clinical laboratory services has been documented for many years. This overuse use does not contribute to the quality of medical care, does not shorten hospital stay, nor reduce mortality. The utilization of diagnostic laboratories has increased over the last decade around the world. This increased laboratory use is appropriate if it allows accurate diagnoses to be made, ideal treatment to be identified and monitored, accurate prognoses to be established, and patients’ hospital stays to be shortened. Thus, improving the appropriateness of testing behavior and reducing the number of laboratory tests have been recognized as essential parts of quality improvement program. In this study, the effectiveness of a computer-based system in improving the laboratory test-ordering in a general hospital was investigated. The study was conducted through four stages, the preparation stage, the pre-intervention stage, the post-intervention 1) stage and post-intervention 2) stage. Guideline and computer system were developed during preparation stage. Medical records were reviewed against guideline recommendations before any intervention during the pre-intervention stage, after guideline dissemination through educational workshops during the post intervention 1) stage, and after implementation of the computer system with the new requesting form during the post intervention 2) stage. The study revealed that the computer-based system achieved a statistically significant increase in the percentage of appropriate use from 44.6% in the post-intervention 1) stage to 55.6%, and a statistically significant increase in the compliance with guideline by prescriber as well as increased in guideline conformity rate from 16.7% in the post-intervention 1) stage to 32.5% in the post-intervention 2) stage, and decreased in the percentage of prescribers whose level was unsatisfactory from 85.4% the post-intervention 1) stage to 66.7% in the post-intervention 2) stage.
文摘One Health is an integrative approach that emphasizes the interconnectedness of human,animal,and environmental health,advocating for collaborative,multidisciplinary efforts to address health challenges,particularly amid globalization and emerging threats.This paper examines the integration of One Health principles into global health education,highlighting the importance of interdisciplinary collaboration and innovative pedagogical approaches.It evaluates various teaching methods,including problem-based learning(PBL),team-based learning(TBL),simulation-based education(SBE),case-based learning(CBL),interdisciplinary workshops and seminars(IWS),and service-learning(SL),analyzing their strengths and weaknesses in fostering interdisciplinary understanding and practical application of One Health concepts.While these methods enhance learning by promoting critical thinking,collaboration,and real-world application,they also face challenges such as resource constraints,variability in group dynamics,and the complexity of assessing long-term learning outcomes.The paper also discusses the role of global partnerships,such as the Global One Health Research Partnership(GOHRP),in advancing One Health education through collaborative research and educational initiatives.Addressing challenges in curriculum integration and interdisciplinary collaboration is crucial for the effective implementation of One Health education,ensuring that future health professionals are equipped to tackle complex global health challenges.
文摘In Zanzibar, from the start of the pandemic in March 2020 to the time of sampling in December 2020, SARS-CoV-2 seroprevalence data was limited. We conducted a seroprevalence study to evaluate the magnitude of SARS-CoV-2 exposure among healthcare workers, school children, and people who attended general markets in Zanzibar. The objectives of the study were to analyse the total antibodies from selected higher-risk population groups in order to determine magnitude in SARS CoV-2 exposure. Blood samples were collected from eligible and consented participants (adults and children), and their serum was analyzed for total antibodies against SARS-CoV-2 using ELISA. A questionnaire was used to collect participants’ demographic and clinical data. The overall SARS-CoV-2 seroprevalence across all age groups was 33%, and a higher seroprevalence was observed in the 40 - 49 years’ age group relative to other ages as well as in those who attended markets. A runny nose (18.8% of participants) was the most frequently reported SARS-CoV-2 infection-related symptom. Multivariable analysis showed significantly higher odds of infection in people living in urban districts. The findings provide insight into SARS-CoV-2 infection among school children, health workers, and people who attended markets in Zanzibar in the early stages of the pandemic. Exposure in these groups might have been influenced by infection and prevention strategies taken by the government, as well as shopping behavior, school overcrowding, and population density in urban settings. The study had methodological limitations, including cross-sectional design. Further, well-designed, longitudinal studies are recommended to understand exposure and transmission at a population level.

文摘Background:China’s accelerating development and increasingly important role in global health engagement create a great demand for global health professionals including international consulting experts.This study reported the detailed development and evaluation of an international consulting training for global health workforce.Methods:Based on Kirkpatrick’s model,a mixed-methods approach was used to evaluate the effectiveness of the training.Quantitative and qualitative data on participants’reaction,learning,and application of the learned knowledge and skills were collected by a training evaluation survey at the ending of training and a follow-up interview in three months after the training.Results:Thirty-six participants attended the training and 34 of them completed quantitative investigation.The training satisfaction evaluations were positive,for which participants rated the training program highly and over 90%of them agreed with the usefulness of the training.About knowledge and skills change,participants showed improved consulting knowledge and skills from pre-to post-training(P<0.001).A total of 23 participants accepted follow-up interview,and most participants applied knowledge and skills learned from the training in their daily work or study.However,only 30.4%of participants applied their learning in the consulting program.The largest barrier of application was the lack of consulting opportunities.In addition,almost all the participants reported that they would be glad to attend more training courses in the future.Conclusion:The international consulting training program was well-received,and was feasible to improve the consulting service competence of global health professionals.According to participants’feedback,it is essential to develop and expand consulting training in the field of global health.
文摘Background and Objectives: Mitigation of antibiotic resistant Neisseria gonorrhoeae has become a priority due to considerable health and economical disabilities it generates. In order to tackle the emergence of resistant Neisseria gonorrhoeae, this study aimed to determine the prevalence and risk factors of penicillinase type β-lactamase-producing Neisseria gonorrheae among patients consulting for genital infectious disorders in two health-facilities in Yaounde, Cameroon. Materials and Method: A cross-sectional descriptive and analytical study was conducted over a 3-month period, from July 2<sup>nd</sup> to October 2<sup>nd</sup>, 2022. Vaginal and urethral secretions were collected. Biochemical identification tests were performed on colonies grown on chocolate agar + polyvitex using the Api NH gallery. The detection of penicillinases was equally performed using the API NH gallery and confirmed using the antimicrobial susceptibility testing. The Minimum Inhibitory Concentrations of some antibiotics were determined using the E-Test. Results: The results showed that out of the 198 patients sampled, 16 (8.08%) were positive for Neisseria gonorrhoeae, among which 13/16 (81.25%) were penicillinase-type β-lactamase producers. Antimicrobial susceptibility testing results showed high co-resistances to antibiotics, mainly ciprofloxacin (100%), nalidixic acid (92.31%) and azithromycin (84.62%). Moreover, high Minimum Inhibitory Concentrations of ceftriaxone (ranging from 6 to 24 mg/L) was observed toward Neisseria gonorrhoeae isolates. The risk factors of the carriage of penicillinase-type β-lactamase producing Neisseria gonorrhoeae identified were: a history of Sexually Transmitted infections (p = 0.01) and unprotected sexual intercourse (p = 0.01). Conclusion: The emergence of penicillinase-type β-lactamase producing Neisseria gonorrhoeae is increasing and the situation is becoming worrisome. The identified risk factors can constitute a basic outlook to tackle resistant Neisseria gonorrhoeae, and therefore sustain antibiotic stewardship.
文摘Backgroud: The prevention of Legionella spp. colonization of water distribution systems is a critical issue in healthcare settings and only an effective disinfection of water systems and appropriate environmental surveillance strategies allow to prevent nosocomial legionellosis. Methods: Due to the temporary effectiveness (increase of the temperature of water in boilers and shock hyperchlorination), the high costs (point-of-use water filters) or the ineffectiveness (hydrogen peroxide, H202) of the previous control procedures, 3 devices (one for each hot water loop) continuously injecting monochloramine have been installed in two different Italian hospitals heavily contaminated by Legionellapneumophila SG3 and SG6. Aim: To evaluate the efficacy of continuous disinfection by monochloramine for control of Legionella on domestic hot water (DHW) distribution system of health-care facilities. Findings: One month after the disinfection of DHW with monochloramine, the load ofL. pneumophila SG3 and SG6 (previous mean count ranging from 103 to 105 CFU/L), as well as the Heterotrophic Plate Count (HPC) (previous mean count ranging from 〉 10^2 to 〉 10^4 CFU/mL), decreased at undetectable levels in 100% of the sampling points in the two hospitals. Conclusion: The results suggest that continuous injection of monochloramine on DHW systems can fully control L. pneumophila and HPC in contaminated hospitals since the beginning of its application.
文摘The presence of chemical constituents from fuels in the work atmosphere can reduce workplace and have a serious impact on human health. In this study, we determined the constituents of unleaded and diesel fuel sold in the District of Abidjan, notably sulfur and lead, and assessed the impact on mechanics, pump attendants and painters. Twenty tow fuel samples collected in the district of Abidjan, including 14 from legal sites distribution and 8 from illegal sites, were analyzed to determine the sulfur and lead content. Blood was collected among 45 volunteers, including 15 mechanics, 15 painters and 15 pump attendants to hematological and biochemical analysis. The results show that sulfur levels ranged from 1530 ppm to 1900 ppm and from 94.9 ppm to 9.4 ppm in diesel and unleaded fuel respectively. Sulfur values below Côte d’Ivoire standard of 1500 - 3500 ppm for diesel and 500 - 1500 ppm for unleaded fuel but remain above international standard of 10 pp. Fuel characterization did not show the presence of lead in any of the tested fuel samples. Blood results showed MCHC values above of reference values for all professionals, while MCH values were below reference values. Neutrophil values were low compared with reference values in all occupational groups of mechanics and pump attendants, while lymphocyte results were 48.20% and 52.50% for mechanics and fuel attendants respectively were well above reference values. GGT values ranged from 42 ± 14.98 IU/l to 87 ± 3.45 were well above reference values. However, the GGT levels of pump attendants were significantly (P < 0.05) higher than those of other professionals.
文摘Objective: This study aims to analyze the relationship between active transportation, screen time and sleep quality among metabolically health versus unhealthy Congolese schoolboys and girls obese. Method: A cross-sectional study was conducted among 58 obese aged 15.36 ± 1.22 years in Brazzaville (Republic of Congo). They were divided into metabolically healthy obese (MHO, n = 29) and metabolically unhealthy obese (MUHO, n = 29). Data collection consisted of anthropometric measurements, lipids profile parameters, Pittsburgh Sleep Quality Index and the screen time measurements. The relationship was analyzed by using the logistic regression for healthy and unhealthy schoolboys and obese girls. Results: Compared to MUH obese subjects, MUHO obese subjects were significantly less engaged in active transportation (p = 0.03), TV and smartphone times significantly increased (p = 0.000 and p = 0.003), sleep quality significantly poor (p = 0.001). They were 1.85 (95% CI: 0.85 - 3.88) lower odds to engage in active transportation, had 1.82 (95% CI: 1.11 - 3.10) and 2.04 (95% CI: 1.11 - 3.10) higher odds of TV time respectively, had 1.87 (95% CI: 1.24 - 2.84) and 2.04 (95% CI: 1.47 - 2.85) higher odds of smartphone time respectively and have 2.35 (95% CI: 1.62 - 3.41) higher odds of poor sleep. Conclusion: MUHO subjects underwent high screen time and poor sleep quality. Higher TV-viewing/smartphone time and poorer sleeping quality were found to be associated with less time spent in active transportation. This bad habit on screen and sleep negatively affects the cardiometabolic parameters.
文摘Background:The One Health approach involves collaboration across several sectors,including public health,veterinary and environmental sectors in an integrated manner.These sectors may be disparate and unrelated,however to succeed,all stakeholders need to understand what the other stakeholders are communicating.Likewise,it is important that there is public acceptance and support of One Health approaches,which requires effective communication between professional and institutional organisations and the public.To help aid and facilitate such communication,written materials need to be readable by all stakeholders,in order to communicate effectively.There has been an exponential increase in the publication of papers involving One Health,with<5 per year,in the 2000s,to nearly 500 published in 2023.To date,readability of One Health information has not been scrutinised,nor has it been considered as an integral intervention of One Health policy communication.The aim of this study was therefore to examine readability of public-facing One Health information prepared by 24 global organisations.Methods:Readability was calculated using Readable software,to obtain four readability scores[(i)Flesch Reading Ease(FRE),(ii)Flesch-Kincaid Grade Level(FKGL),(iii)Gunning Fog Index and(iv)SMOG Index]and two text metrics[words/sentence,syllables/word]for 100 sources of One Health information,from four categories[One Health public information;PubMed abstracts;Science in One Health(SOH)abstracts(articles);SOH abstracts(reviews)].Results:Readability of One Health information for the public is poor,not reaching readability reference standards.No information was found that had a readability of less than 9th grade(around 14 years old).Mean values for the FRE and FKGL were(19.4±1.4)(target>60)and(15.6±0.3)(target<8),respectively,with mean words per sentence and syllables per word of 20.5 and 2.0,respectively.Abstracts with“One Health”in the title were more difficult to read than those without“One Health”in the title(FRE:P=0.0337;FKGL:P=0.0087).Comparison of FRE and FKGL readability scores for the four categories of One Health information[One Health public information;PubMed abstracts;SOH abstracts(articles);SOH abstracts(reviews)]showed that SOH abstracts from articles were easier to read than those from SOH reviews.No One Health public-facing information from the 100 sources examined met the FKGL target of8.The most easily read One Health information required a Grade Level of 9th grade(14-15 years old),with a mean Grade Level of 15.5(university/college level).Conclusion:Considerable work is required in making One Health written materials more readable,particularly for children and adolescents(<14 years of age).It is important that any interventions or mitigations taken to support better public understanding of the One Health approach are not ephemeral,but have longer lasting and legacy value.Authors of One Health information should consider using readability calculators when preparing One Health information for their stakeholders,to check the readability of their work,so that the final material is within recommended readability reference parameters,to support the health literacy and stakeholder-directed knowledge of their readers.
文摘BACKGROUND Middle meningeal artery embolization(MMAE)is emerging as a promising treatment for chronic subdural hematoma(CSDH),serving both as an adjunct to surgery and as a primary therapeutic option depending on patient presentation.Due to its low recurrence rate and minimal complications,MMAE has gained increasing acceptance among clinicians in recent years.This report presents a case of diplopia following MMAE due to the presence of a potential anastomotic artery,aiming to enhance awareness of this complication.CASE SUMMARY A 60-year-old male patient presented with a headache following head trauma,and cranial computed tomography revealed a left-sided CSDH.The patient underwent left MMAE;however,polyvinyl alcohol particles inadvertently flowed into the lacrimal artery through an anastomotic artery,resulting in diplopia due to impaired abduction of the left eye.The diplopia resolved by postoperative day 40.The patient’s headache resolved by postoperative day 7,and the hematoma completely resolved by postoperative day 108.CONCLUSION Potential anastomotic arteries in the middle meningeal artery(MMA)can lead to serious complications.Superselective angiography of the MMA or its branches prior to embolization is essential.Performing embolization distal to potential anastomotic sites can reduce risks,and the presence of an anastomosis may warrant coil embolization or termination of the procedure.

文摘Male infertility can result from impaired sperm motility caused by multiple morphological abnormalities of the flagella(MMAF).Distinct projections encircling the central microtubules of the spermatozoal axoneme play pivotal roles in flagellar bending and spermatozoal movement.Mammalian sperm-associated antigen 17(SPAG17)encodes a conserved axonemal protein of cilia and flagella,forming part of the C1a projection of the central apparatus,with functions related to ciliary/flagellar motility,skeletal growth,and male fertility.This study investigated two novel homozygous SPAG17 mutations(M1:NM_206996.2,c.829+1G>T,p.Asp212_Glu276del;and M2:c.2120del,p.Leu707*)identified in four infertile patients from two consanguineous Pakistani families.These patients displayed the MMAF phenotype confirmed by Papanicolaou staining and scanning electron microscopy assays of spermatozoa.Quantitative real-time polymerase chain reaction(PCR)of patients’spermatozoa also revealed a significant decrease in SPAG17 mRNA expression,and immunofluorescence staining showed the absence of SPAG17 protein signals along the flagella.However,no apparent ciliary-related symptoms or skeletal malformations were observed in the chest X-rays of any of the patients.Transmission electron microscopy of axoneme cross-sections from the patients showed incomplete C1a projection and a higher frequency of missing microtubule doublets 1 and 9 compared with those from fertile controls.Immunofluorescence staining and Western blot analyses of spermatogenesis-associated protein 17(SPATA17),a component of the C1a projection,and sperm-associated antigen 6(SPAG6),a marker of the spring layer,revealed disrupted expression of both proteins in the patients’spermatozoa.Altogether,these findings demonstrated that SPAG17 maintains the integrity of spermatozoal flagellar axoneme,expanding the phenotypic spectrum of SPAG17 mutations in humans.

文摘This letter provides a critical appraisal of the comprehensive meta-analysis by Hou et al,which synthesizes the incidence and risk factors for postoperative delirium(POD)in organ transplant recipients.Their work establishes a pooled POD incidence of 20%,with significant variability across organ types(lung 34%,liver 22%,kidney 6%),and identifies key risk factors including primary graft dysfunction,hepatic encephalopathy,and high model for end-stage liver disease/acute physiology and chronic health evaluation Ⅱ scores.This commentary acknowledges the study's strength in providing a robust,trans-organ synthesis of current evidence.However,it critically discusses the substantial heterogeneity,the counterintuitive non-significance of age as a risk factor,and the unavoidable limitation of unmeasured confounders inherent in meta-analyses,such as preoperative cognitive/psychiatric status and anesthetic protocols.While the findings provide an essential evidence base for risk stratification and prevention,this letter argues that the high heterogeneity underscores the need for organ-specific analysis and calls for large-scale,prospective studies with standardized protocols to translate these findings into reliable clinical prediction tools and targeted interventions.
文摘Objective:To evaluate the effectiveness of two doses of coenzyme Q10(CoQ10)on semen parameters,sperm DNA damage,and the partner pregnancy rate in men with idiopathic oligoasthenozoospermia.Methods:250 patients with idiopathic oligoasthenozoospermia were examined.The first group(n=125)received 100 mg/day of CoQ10 and the second group(n=125)received 200 mg/day of CoQ10 orally for 6 months.Semen parameters,DNA fragmentation index(DFI)and the partner pregnancy rate were analyzed at baseline and after 6 months of treatment.Results:Comparing with baseline data,treatment with CoQ10(100 mg/day or 200 mg/day)resulted in a significant increase in sperm concentration(both P<0.001),a significant improvement in progressive motility and total motile sperm count(P=0.05,P=0.001,respectively).The mean DFI was significantly improved after treatment with CoQ10 at 100 mg/day and at 200 mg/day,after 6 months of treatment(P<0.01).Moreover,CoQ10 significantly improved the partner pregnancy rate.A strongest correlation was found between seminal fluid parameters and DFI(P<0.001).Conclusions:CoQ10 is effective in improving semen parameters,DFI and on the partner pregnancy rate after 6 months with CoQ10 at two doses,with a greater improvement shown in men who took 200 mg/day than in those who took 100 mg/day.
文摘Antipalu is a phytomedicinal medicinal beverage that is popular in the District of Abidjan, particularly for the treatment of malaria. However, Antipalu could present potential health effects on patients, and few toxicological studies have been conducted before its use. In order to determine the cytotoxicity of Antipalu, two complementary tests, LDH activity and the MTT cell proliferation assay, were used using Vero cells. Vero cells were exposed to increasing concentrations of Antipalu and incubated for 24, 48 and 72 hours. In addition, forty (40) rats distributed randomly into 4 groups, including 10 animals per group (5 males and 5 females) were used for the potential hepatoxic effects. Animals in group 1 received distilled water and were used as a control group. On the other hand, Lot I, II and III received by gavage a volume of the Antipalu extract corresponding to 1 ml/100 g of body weight at 200 mg/kg, 400 mg/kg, 800 mg/kg, respectively. The extract was administered daily at the same time for 28 days and serum was collected once a week to evaluate hepatic biochemical markers. After 28 days of study, all rats were euthanized by an overdose of ether and the liver of the rats was removed for gross morphological and histopathological analysis. The results of the cell supernatant assay showed an increasing extracellular LDH enzyme activity with lethal concentrations at 10% and 50% (LC10 = 111 µg/mL and LC50 = 555 µg/mL, respectively). In addition, the MTT assay showed a decrease in mitochondrial activity and thus cell proliferation after 24, 48 and 72 H of incubation. Our study showed that Antipalu caused alterations in the plasma membranes of the cells, resulting in the release of lactase dehydrogenase (LDH) into the external environment and a decrease in the mitochondrial activity of the Vero cells. The biochemical parameters ALT, ASAT, ALPs, and GGT showed no significant change (P > 0.05) in the group of treated rats compared to the controls. However, these variations were moderate and transient, with values remaining almost within their standard limits. Microscopic observations of liver tissue sections from rats treated with the Antipalu showed no lesions, edema and necrosis. These results suggest that the Antipalu did not interfere with the functioning or alter the integrity of the liver.
