The current study examined the roles of collective self-esteem and personal self-esteem in the relationship between national identity and subjective well-being.Participants were 583 Chinese college students(females=49...The current study examined the roles of collective self-esteem and personal self-esteem in the relationship between national identity and subjective well-being.Participants were 583 Chinese college students(females=49%;mean age=19.25±1.85 years).They completed measures of national identity,collective self-esteem,personal self-esteem,and subjective well-being.Path analysis findings result indicated national identity to influence the students’subjective wellbeing through three pathways:(1)national identity→collective self-esteem→subjective well-being,meaning higher subjective wellbeing with collective self-esteem.(2)national identity→personal self-esteem→subjective well-being,to suggest higher personal self-esteem was associated with subjective wellbeing;(3)national identity→collective selfesteem→personal self-esteem→subjective well-being.Compared to simple mediation models constructed with only personal self-esteem or collective self-esteem as a single mediating variable,the chain mediation model better explains the mediating mechanism of national identity on subjective well-being(the variance explained by the mediating variables increased by 65.38%and 59.26%,respectively).The collective self-esteem and personal self-esteem mediation is consistent with social identity theory,whereby national identity enhances collective self-evaluation,which in turn bolsters personal self-worth and subjective well-being.These findings of the current study offer new insights into how national identity affects subjective well-being in collectivistic culture.展开更多
In order to ascertain prevalence rate of premarital sexual intercourse,unintended pregnancy and abortion,and evaluate associated factors of unintended pregnancy among undergraduates from all over China,the representat...In order to ascertain prevalence rate of premarital sexual intercourse,unintended pregnancy and abortion,and evaluate associated factors of unintended pregnancy among undergraduates from all over China,the representative sample of unmarried undergraduates was obtained by using a multi-stage,stratified,probability cluster design,and data were collected by using a survey questionnaire.62 326 available responders were gained.11.6% of them acknowledged having experiences of premarital sexual intercourse(standardized prevalence rate of sexual intercourse was 13.8%).31.5% of students active in premarital sex acknowledged undergoing unintended pregnancy.76.2% of pregnant students selected abortion to end it.Of students active in premarital sex,46.2% used contraception at the first sexual intercourse,28.2% replied "always" using contraception in sexual intercourse.The rate of using condoms,oral contraceptives(OCs),and withdrawal among students who had used contraception was 52.0%,31.0%,and 27.2% respectively."No preparation for sex"(40.3%),"pleasure decrement"(32.1%),"won't-be-pregnancy in occasional sexual intercourse"(30.2%) were their common excuses for using no contraception.The identified risk factors for unintended pregnancy among students active in premarital sex by multivariate analysis were as follows:having no steady lover [having no steady lover vs having a steady lover:odds ratio(OR),1.875;95% confidence interval(CI),1.629-2.158],unaware of the course of conception(unaware vs aware:OR,2.023;95% CI,1.811-2.260),considering abortion not endanger women's physical and mental health(no endangerment vs endangerment:OR,2.659;95% CI,2.265-3.121),nonuse of contraception(never use vs always use:OR,1.682;95% CI,1.295-2.185).Medical students were not less likely to experience an unintended pregnancy than nonmedical students(OR,1.111;95% CI,0.906-1.287).The substantial proportion of unintended pregnancy among undergraduates indicates a need for convenient and targeted contraceptive education and services.展开更多
Objectives:The aim of this study was to verify the causal relationship between trait aggressiveness(TA)and moral disengagement(MD),know more about the growth trajectory of MD,and explore the effects of gender and TA o...Objectives:The aim of this study was to verify the causal relationship between trait aggressiveness(TA)and moral disengagement(MD),know more about the growth trajectory of MD,and explore the effects of gender and TA on the growth trajectory.Methods:We used the Buss-Perry Aggression Questionnaire and Moral Disengagement Scale to survey 433 Chinese junior high school students longitudinally three times.Results:The results of the random intercept cross-lagged panel model(RI-CLPM)analysis indicated that TA positively predicted MD,while MD did not predict TA at the within-person level.Thus,TA could be considered an antecedent variable of MD.Furthermore,the unconditional latent growth linear model analysis revealed that MD among junior high school students exhibited an upward trend,characterized by an increasing rate of growth over time.In the conditional latent growth linear model analysis,we found that gender influenced only the initial level of MD but had no effect on its growth rate.Conversely,TA demonstrated both delayed and immediate positive effects on the growth of MD.Conclusions:Our findings suggested a one-way cross-lag effect between TA and MD.Additionally,the growth trajectory of MD among junior high school students was found to be ascending.This growth trajectory was influenced by gender and TA,offering valuable insights for the prevention and intervention of behavioral problems in junior high school students.展开更多
This study investigated the relationship between social anxiety and problematic Internet use in college students,and how it is moderated by attitudes toward seeking professional psychological help.Participants were 14...This study investigated the relationship between social anxiety and problematic Internet use in college students,and how it is moderated by attitudes toward seeking professional psychological help.Participants were 1451 Chinese college students(female=60.2%;mean age=19.85 years,SD=1.89 years).They completed the Interaction Anxiousness Scale,the Attitudes Toward Seeking Professional Psychological Help Scale-Short Form,and the Problematic Internet Use Scale.The results revealed that college students with higher social anxiety reported greater severity of problematic Internet use.Moreover,students with negative attitudes toward seeking professional psychological help also reported greater severity of problematic Internet use.Notably,attitudes toward seeking professional psychological help moderated the relationship between social anxiety and problematic Internet use in college student,such that the relationship was weakened when attitudes toward seeking professional psychological help was positive.These findings suggest a need for student development and support programs for promoting openness to seeking professional psychological help if with problematic Internet use from social anxiety.展开更多
BACKGROUND Depression,anxiety,and insomnia were found out that were significant relevance to the mental health impact of the coronavirus disease 2019(COVID-19)lock-down.AIM To examine the interrelationships among perc...BACKGROUND Depression,anxiety,and insomnia were found out that were significant relevance to the mental health impact of the coronavirus disease 2019(COVID-19)lock-down.AIM To examine the interrelationships among perceived severity,anxiety,depression,insomnia,and sense of security in Chinese community residents during the COVID-19 lockdown period.METHODS Participants were selected using simple random sampling from four large gated communities in Chengdu,China.All participants were invited to complete a survey that included the Perceived Severity Questionnaire,Security Question-naire,Patient Health Questionnaire-9,Generalized Anxiety Disorder 7-item,and Insomnia Severity Index-7.In total,568 valid questionnaires were gathered.Co-rrelation analysis and structural equation models were used to explore the rela-RESULTS The observed prevalence rates of anxiety,depression,and insomnia among residents during lockdown were 27.5%,17.6%,and 16.0%,respectively.Correlation analysis showed that both perceived severity and sense of security were positively correlated with anxiety,depression,and insomnia[Pearson’s r was perceived severity and anxiety r=0.44(P<0.01);with depression r=0.48(P<0.01);with insomnia r=0.43(P<0.01);security with anxiety r=-0.65(P<0.01);with depression r=-0.65(P<0.01);with insomnia r=-0.53(P<0.01)].Structural equation modeling and bootstrap tests revealed that sense of security acted as a significant mediator in the relationship between perceived severity and emotional and sleep disorders(anxiety,depression,and insomnia).CONCLUSION This study demonstrates that sense of security is a significant predictor of emotional and sleep disorders(namely,depression,anxiety,and insomnia)among residents during the COVID-19 lockdown,with sense of security acting as a mediating factor.These findings suggest that mental health interventions for Chinese community residents during lockdowns may benefit from developing community-based educational programs to reduce perceived severity and ensuring the stable supply of essential resources and promoting social support networks to enhance the sense of security.展开更多
BACKGROUND Peer bullying victimization(PBV)is a significant public health issue that adolescents often face,with approximately one-third having experienced PBV.Understanding its interrelationships with mental health p...BACKGROUND Peer bullying victimization(PBV)is a significant public health issue that adolescents often face,with approximately one-third having experienced PBV.Understanding its interrelationships with mental health problems is crucial for effective intervention.This study aims to examine the longitudinal relationships between internalizing problems(depression and anxiety),externalizing problems(aggression),and PBV among middle school students using cross-lagged panel network analysis.AIM To examine the longitudinal relationships between internalizing problems(depression and anxiety),externalizing problems(aggression),and PBV among middle school students using cross-lagged panel network analysis.METHODS A total of 1260 middle school students(54.6%male)participated in this study.Data were collected at two time points(time 1 and time 2)using self-report questionnaires to assess PBV,depression,anxiety,and aggression.Cross-lagged panel network analysis was applied to examine the bi-directional relationships between these variables over time.RESULTS Depression,particularly a“sad mood,”was the most significant predictor of both PBV and aggression.Cyberbullying victimization also emerged as a key factor influencing depression and anxiety.While anxiety had weaker and less consistent effects on PBV,aggression was predominantly influenced by depressive symptoms and sleep disturbances.The analysis also identified key symptoms,such as a“sad mood”and sleep disturbances,as crucial targets for intervention to mitigate the cycle of PBV,depression,anxiety,and aggression.CONCLUSION This study provides important insights for bullying victimization prevention and intervention strategies:(1)Early identification and intervention targeting depression,particularly a“sad mood,”and sleep disturbances;(2)The importance of addressing cyberbullying as a distinct factor influencing mental health;and(3)The need for comprehensive,longitudinal approaches to understanding and intervening in the interconnected mental health issues among adolescents.展开更多

With the rapid advancement of wearable devices,Human Activities Recognition(HAR)based on these devices has emerged as a prominent research field.The objective of this study is to enhance the recognition performance of...With the rapid advancement of wearable devices,Human Activities Recognition(HAR)based on these devices has emerged as a prominent research field.The objective of this study is to enhance the recognition performance of HAR by proposing an LSTM-1DCNN recognition algorithm that utilizes a single triaxial accelerometer.This algorithm comprises two branches:one branch consists of a Long and Short-Term Memory Network(LSTM),while the other parallel branch incorporates a one-dimensional Convolutional Neural Network(1DCNN).The parallel architecture of LSTM-1DCNN initially extracts spatial and temporal features from the accelerometer data separately,which are then concatenated and fed into a fully connected neural network for information fusion.In the LSTM-1DCNN architecture,the 1DCNN branch primarily focuses on extracting spatial features during convolution operations,whereas the LSTM branch mainly captures temporal features.Nine sets of accelerometer data from five publicly available HAR datasets are employed for training and evaluation purposes.The performance of the proposed LSTM-1DCNN model is compared with five other HAR algorithms including Decision Tree,Random Forest,Support Vector Machine,1DCNN,and LSTM on these five public datasets.Experimental results demonstrate that the F1-score achieved by the proposed LSTM-1DCNN ranges from 90.36%to 99.68%,with a mean value of 96.22%and standard deviation of 0.03 across all evaluated metrics on these five public datasets-outperforming other existing HAR algorithms significantly in terms of evaluation metrics used in this study.Finally the proposed LSTM-1DCNN is validated in real-world applications by collecting acceleration data of seven human activities for training and testing purposes.Subsequently,the trained HAR algorithm is deployed on Android phones to evaluate its performance.Experimental results demonstrate that the proposed LSTM-1DCNN algorithm achieves an impressive F1-score of 97.67%on our self-built dataset.In conclusion,the fusion of temporal and spatial information in the measured data contributes to the excellent HAR performance and robustness exhibited by the proposed 1DCNN-LSTM architecture.展开更多

The results showed that, regarding brain function, some dimensions of social adjustment were associated with the insula, and some regions of the frontal and occipital lobes. Self-adjustment and satisfaction required activation of the middle frontal gyrus, while career adjustment and academic adjustment required inhibition of the inferior frontal gyrus and lingual gyrus, respectively. Decreased metabolic activity of the lingual gyrus was beneficial for obtaining satisfaction. Regarding brain structure, the total score and some dimensions of social adaptation were associated with the gray matter of portions of the temporal and parietal lobes. The superior temporal gyrus was associated with the total social adjustment and satisfaction score, the middle temporal gyrus with campus-life adjustment and satisfaction, and the post central gyrus and the inferior parietal lobule with emotional adjustment. The changes in the gray matter volume of these brain regions to a certain extent reflected socially adaptive behaviors. The results suggest that social adaptability is associated with various brain regions dispersed among both hemispheres of the brain, and requires synergistic inter-actions between multiple brain regions and both brain hemispheres.展开更多

METHODS: Randomized, double blind, plac...OBJECTIVE: To estimate the effect of outpatient administration of a single d os e of vaginal misoprostol at term on the interval to delivery in women with unfav orable cervices. METHODS: Randomized, double blind, placebo-controlled trial co mparing a single 25-μg outpatient intravaginal dose of misoprostol to placebo in pregnant women with Bishop scores less than 9 at 40 weeks or greater. After p lacement of the study medication, subjects were permitted to go into spontaneous labor unless an indication for induction developed. Analysis was by intent to t reat. The interval to delivery, defined as the time from medication placement to delivery, was compared by Student t test and by survival analysis with the log -rank test. RESULTS: Thirty-three women were randomly assigned to receive miso prostol, and 35 were assigned to receive placebo. The mean interval to delivery was significantly less in the misoprostol group, 4.2 ±.4.1 compared with 6.1 ± .3.6 days, P = .04. The interval to delivery for only the nulliparous patients w as significantly less in the misoprostol group, 4.2 ±.4.0 compared with 7.2 ±. 3.7 days, P = .02. The survival curves for the interval to delivery were signifi cantly different (P = .04 by log-rank test) with 4.1 days median interval to de livery for misoprostol compared with 9.2 days for placebo. There were no adverse outcomes in either group. CONCLUSION: A single 25-μg outpatient intravaginal dose of misoprostol is effective in decreasing the interval to delivery in women with unfavorable cervices at term.展开更多

In patients with recurrent gynecologic cancer, liver and intrathoracic lesions should undergo a biopsy to rule in metastatic malignancy, as clinical findings and CAT scan results may represent other disease processes. Case. A 67 year old woman had a total abdominal hysterectomy, bilateral salpingo-oophorectomy, pelvic and periaortic lymphadenectomy, and peritoneal cytology in 2001 for Stage I B grade 1 adenocarcinoma of the endometrium. She developed a vaginal recurrence in 2005. A CT scan of lungs, abdomen, and pelvis revealed extensive mediastinal adenopathy and multiple space occupying hepatic lesions worrisome for metastatic disease. A needle biopsy of the largest liver lesion revealed sarcoidosis. Conclusion. Sarcoid lesions may mimic metastatic disease in patients with malignancy, potentially leading to delayed and/or inappropriate therapy.展开更多
To the Editor:Contemporary guidelines recommend primary percutaneous coronary intervention(PPCI)as the gold-standard reperfusion modality in patients with ST-segment elevation myocardial infarction(STEMI)within 12 h o...To the Editor:Contemporary guidelines recommend primary percutaneous coronary intervention(PPCI)as the gold-standard reperfusion modality in patients with ST-segment elevation myocardial infarction(STEMI)within 12 h of symptom onset.However,in realworld clinical practice,the implementation of PPCI faces substantial operational challenges,particularly in resource-limited settings,such as inadequate availability of specialized cardiac catheterization teams with requisite procedural expertise or the complete absence of coronary intervention capabilities in numerous rural healthcare facilities across China.Consequently,fibrinolytic therapy remains the predominant reperfusion strategy for STEMI management in county-level hospitals,with particularly high utilization rates observed in geographically remote regions.[3,4]Furthermore,our previous study revealed that 42.5%of STEMI patients fail to receive any form of evidence-based reperfusion therapy,representing a critical gap in optimal care delivery.To elucidate the prognostic implications of these treatment pathways within China’s unique healthcare system,this study aimed to compare the long-term outcomes among STEMI patients receiving PPCI,fibrinolysis,and no reperfusion therapy using data from the China Acute Myocardial Infarction Registry(CAMI Registry,clinicaltrials.gov,NCT01874691).展开更多

文摘Objectives:The aim of this study was to verify the causal relationship between trait aggressiveness(TA)and moral disengagement(MD),know more about the growth trajectory of MD,and explore the effects of gender and TA on the growth trajectory.Methods:We used the Buss-Perry Aggression Questionnaire and Moral Disengagement Scale to survey 433 Chinese junior high school students longitudinally three times.Results:The results of the random intercept cross-lagged panel model(RI-CLPM)analysis indicated that TA positively predicted MD,while MD did not predict TA at the within-person level.Thus,TA could be considered an antecedent variable of MD.Furthermore,the unconditional latent growth linear model analysis revealed that MD among junior high school students exhibited an upward trend,characterized by an increasing rate of growth over time.In the conditional latent growth linear model analysis,we found that gender influenced only the initial level of MD but had no effect on its growth rate.Conversely,TA demonstrated both delayed and immediate positive effects on the growth of MD.Conclusions:Our findings suggested a one-way cross-lag effect between TA and MD.Additionally,the growth trajectory of MD among junior high school students was found to be ascending.This growth trajectory was influenced by gender and TA,offering valuable insights for the prevention and intervention of behavioral problems in junior high school students.

文摘This study investigated the relationship between social anxiety and problematic Internet use in college students,and how it is moderated by attitudes toward seeking professional psychological help.Participants were 1451 Chinese college students(female=60.2%;mean age=19.85 years,SD=1.89 years).They completed the Interaction Anxiousness Scale,the Attitudes Toward Seeking Professional Psychological Help Scale-Short Form,and the Problematic Internet Use Scale.The results revealed that college students with higher social anxiety reported greater severity of problematic Internet use.Moreover,students with negative attitudes toward seeking professional psychological help also reported greater severity of problematic Internet use.Notably,attitudes toward seeking professional psychological help moderated the relationship between social anxiety and problematic Internet use in college student,such that the relationship was weakened when attitudes toward seeking professional psychological help was positive.These findings suggest a need for student development and support programs for promoting openness to seeking professional psychological help if with problematic Internet use from social anxiety.

文摘BACKGROUND Depression,anxiety,and insomnia were found out that were significant relevance to the mental health impact of the coronavirus disease 2019(COVID-19)lock-down.AIM To examine the interrelationships among perceived severity,anxiety,depression,insomnia,and sense of security in Chinese community residents during the COVID-19 lockdown period.METHODS Participants were selected using simple random sampling from four large gated communities in Chengdu,China.All participants were invited to complete a survey that included the Perceived Severity Questionnaire,Security Question-naire,Patient Health Questionnaire-9,Generalized Anxiety Disorder 7-item,and Insomnia Severity Index-7.In total,568 valid questionnaires were gathered.Co-rrelation analysis and structural equation models were used to explore the rela-RESULTS The observed prevalence rates of anxiety,depression,and insomnia among residents during lockdown were 27.5%,17.6%,and 16.0%,respectively.Correlation analysis showed that both perceived severity and sense of security were positively correlated with anxiety,depression,and insomnia[Pearson’s r was perceived severity and anxiety r=0.44(P<0.01);with depression r=0.48(P<0.01);with insomnia r=0.43(P<0.01);security with anxiety r=-0.65(P<0.01);with depression r=-0.65(P<0.01);with insomnia r=-0.53(P<0.01)].Structural equation modeling and bootstrap tests revealed that sense of security acted as a significant mediator in the relationship between perceived severity and emotional and sleep disorders(anxiety,depression,and insomnia).CONCLUSION This study demonstrates that sense of security is a significant predictor of emotional and sleep disorders(namely,depression,anxiety,and insomnia)among residents during the COVID-19 lockdown,with sense of security acting as a mediating factor.These findings suggest that mental health interventions for Chinese community residents during lockdowns may benefit from developing community-based educational programs to reduce perceived severity and ensuring the stable supply of essential resources and promoting social support networks to enhance the sense of security.

文摘BACKGROUND Peer bullying victimization(PBV)is a significant public health issue that adolescents often face,with approximately one-third having experienced PBV.Understanding its interrelationships with mental health problems is crucial for effective intervention.This study aims to examine the longitudinal relationships between internalizing problems(depression and anxiety),externalizing problems(aggression),and PBV among middle school students using cross-lagged panel network analysis.AIM To examine the longitudinal relationships between internalizing problems(depression and anxiety),externalizing problems(aggression),and PBV among middle school students using cross-lagged panel network analysis.METHODS A total of 1260 middle school students(54.6%male)participated in this study.Data were collected at two time points(time 1 and time 2)using self-report questionnaires to assess PBV,depression,anxiety,and aggression.Cross-lagged panel network analysis was applied to examine the bi-directional relationships between these variables over time.RESULTS Depression,particularly a“sad mood,”was the most significant predictor of both PBV and aggression.Cyberbullying victimization also emerged as a key factor influencing depression and anxiety.While anxiety had weaker and less consistent effects on PBV,aggression was predominantly influenced by depressive symptoms and sleep disturbances.The analysis also identified key symptoms,such as a“sad mood”and sleep disturbances,as crucial targets for intervention to mitigate the cycle of PBV,depression,anxiety,and aggression.CONCLUSION This study provides important insights for bullying victimization prevention and intervention strategies:(1)Early identification and intervention targeting depression,particularly a“sad mood,”and sleep disturbances;(2)The importance of addressing cyberbullying as a distinct factor influencing mental health;and(3)The need for comprehensive,longitudinal approaches to understanding and intervening in the interconnected mental health issues among adolescents.

文摘This paper reviews the related formulas of Sugemule,introduces the advances in research of clinical application of these formulas in treating Heyi type insomnia,cardiac diseases,and renal diseases.Besides,it summarizes pharmacological research advances regarding these formulas,including their anti-insomnia effects,cardioprotective properties,and ovarian function preservation capabilities.This study is expected to provide a reference and insight for in-depth and systematic research on classical Mongolian medicinal formulas.

文摘With the rapid advancement of wearable devices,Human Activities Recognition(HAR)based on these devices has emerged as a prominent research field.The objective of this study is to enhance the recognition performance of HAR by proposing an LSTM-1DCNN recognition algorithm that utilizes a single triaxial accelerometer.This algorithm comprises two branches:one branch consists of a Long and Short-Term Memory Network(LSTM),while the other parallel branch incorporates a one-dimensional Convolutional Neural Network(1DCNN).The parallel architecture of LSTM-1DCNN initially extracts spatial and temporal features from the accelerometer data separately,which are then concatenated and fed into a fully connected neural network for information fusion.In the LSTM-1DCNN architecture,the 1DCNN branch primarily focuses on extracting spatial features during convolution operations,whereas the LSTM branch mainly captures temporal features.Nine sets of accelerometer data from five publicly available HAR datasets are employed for training and evaluation purposes.The performance of the proposed LSTM-1DCNN model is compared with five other HAR algorithms including Decision Tree,Random Forest,Support Vector Machine,1DCNN,and LSTM on these five public datasets.Experimental results demonstrate that the F1-score achieved by the proposed LSTM-1DCNN ranges from 90.36%to 99.68%,with a mean value of 96.22%and standard deviation of 0.03 across all evaluated metrics on these five public datasets-outperforming other existing HAR algorithms significantly in terms of evaluation metrics used in this study.Finally the proposed LSTM-1DCNN is validated in real-world applications by collecting acceleration data of seven human activities for training and testing purposes.Subsequently,the trained HAR algorithm is deployed on Android phones to evaluate its performance.Experimental results demonstrate that the proposed LSTM-1DCNN algorithm achieves an impressive F1-score of 97.67%on our self-built dataset.In conclusion,the fusion of temporal and spatial information in the measured data contributes to the excellent HAR performance and robustness exhibited by the proposed 1DCNN-LSTM architecture.
文摘KEY POINTSTraditional stretching routines performed during warm-up procedures before exercise can increase flexibility for a short time,but there is little scientific evidence that such routines can improve exercise performance,reduce delayed-onset muscular soreness,or prevent injuries.Stretching on a regular basis,e.g.,3-5 days/week,away from the exercise environment may be effective in improving flexibility and some types of exercise performance,and it may reduce injury risk,but further research is required to validate this concept.Passive stretching for 15-30 seconds is more effective for increasing flexibility than stretching for shorter durations and is equally effective as stretching for longer periods.
文摘This study investigated the neural basis of social adjustment using multimodal brain imaging and social-adjustment measurements to analyze functional and structural brain features during social adjustment in college students. The results showed that, regarding brain function, some dimensions of social adjustment were associated with the insula, and some regions of the frontal and occipital lobes. Self-adjustment and satisfaction required activation of the middle frontal gyrus, while career adjustment and academic adjustment required inhibition of the inferior frontal gyrus and lingual gyrus, respectively. Decreased metabolic activity of the lingual gyrus was beneficial for obtaining satisfaction. Regarding brain structure, the total score and some dimensions of social adaptation were associated with the gray matter of portions of the temporal and parietal lobes. The superior temporal gyrus was associated with the total social adjustment and satisfaction score, the middle temporal gyrus with campus-life adjustment and satisfaction, and the post central gyrus and the inferior parietal lobule with emotional adjustment. The changes in the gray matter volume of these brain regions to a certain extent reflected socially adaptive behaviors. The results suggest that social adaptability is associated with various brain regions dispersed among both hemispheres of the brain, and requires synergistic inter-actions between multiple brain regions and both brain hemispheres.
文摘OBJECTIVE: To estimate the effect of outpatient administration of a single d os e of vaginal misoprostol at term on the interval to delivery in women with unfav orable cervices. METHODS: Randomized, double blind, placebo-controlled trial co mparing a single 25-μg outpatient intravaginal dose of misoprostol to placebo in pregnant women with Bishop scores less than 9 at 40 weeks or greater. After p lacement of the study medication, subjects were permitted to go into spontaneous labor unless an indication for induction developed. Analysis was by intent to t reat. The interval to delivery, defined as the time from medication placement to delivery, was compared by Student t test and by survival analysis with the log -rank test. RESULTS: Thirty-three women were randomly assigned to receive miso prostol, and 35 were assigned to receive placebo. The mean interval to delivery was significantly less in the misoprostol group, 4.2 ±.4.1 compared with 6.1 ± .3.6 days, P = .04. The interval to delivery for only the nulliparous patients w as significantly less in the misoprostol group, 4.2 ±.4.0 compared with 7.2 ±. 3.7 days, P = .02. The survival curves for the interval to delivery were signifi cantly different (P = .04 by log-rank test) with 4.1 days median interval to de livery for misoprostol compared with 9.2 days for placebo. There were no adverse outcomes in either group. CONCLUSION: A single 25-μg outpatient intravaginal dose of misoprostol is effective in decreasing the interval to delivery in women with unfavorable cervices at term.
文摘Background. Sarcoidosis is a multisystem disease and can be confused with benign or malignant tumors. In patients with recurrent gynecologic cancer, liver and intrathoracic lesions should undergo a biopsy to rule in metastatic malignancy, as clinical findings and CAT scan results may represent other disease processes. Case. A 67 year old woman had a total abdominal hysterectomy, bilateral salpingo-oophorectomy, pelvic and periaortic lymphadenectomy, and peritoneal cytology in 2001 for Stage I B grade 1 adenocarcinoma of the endometrium. She developed a vaginal recurrence in 2005. A CT scan of lungs, abdomen, and pelvis revealed extensive mediastinal adenopathy and multiple space occupying hepatic lesions worrisome for metastatic disease. A needle biopsy of the largest liver lesion revealed sarcoidosis. Conclusion. Sarcoid lesions may mimic metastatic disease in patients with malignancy, potentially leading to delayed and/or inappropriate therapy.

文摘To the Editor:Contemporary guidelines recommend primary percutaneous coronary intervention(PPCI)as the gold-standard reperfusion modality in patients with ST-segment elevation myocardial infarction(STEMI)within 12 h of symptom onset.However,in realworld clinical practice,the implementation of PPCI faces substantial operational challenges,particularly in resource-limited settings,such as inadequate availability of specialized cardiac catheterization teams with requisite procedural expertise or the complete absence of coronary intervention capabilities in numerous rural healthcare facilities across China.Consequently,fibrinolytic therapy remains the predominant reperfusion strategy for STEMI management in county-level hospitals,with particularly high utilization rates observed in geographically remote regions.[3,4]Furthermore,our previous study revealed that 42.5%of STEMI patients fail to receive any form of evidence-based reperfusion therapy,representing a critical gap in optimal care delivery.To elucidate the prognostic implications of these treatment pathways within China’s unique healthcare system,this study aimed to compare the long-term outcomes among STEMI patients receiving PPCI,fibrinolysis,and no reperfusion therapy using data from the China Acute Myocardial Infarction Registry(CAMI Registry,clinicaltrials.gov,NCT01874691).
