Bipolar disorder(BD)is a severe mood disorder characterized by recurrent episodes of mania and depression, and it is prone to delayed diagnosis, which can lead to worsened outcomes, including more frequent mood episodes, greater functional impairment, and comorbidities.Early diagnosis of BD remains a significant challenge, although recent advances offer promising insights, such as research in molecular biomarkers, neuroimaging, exosomes, genetics, and epigenetics. This mini-review highlights their potential for providing earlier, more accurate identification of BD and discusses the underlying reasons why current research has not yet succeeded. For instance, the high heterogeneity of symptomatic presentations leads to low consistency in study participants;delayed BD diagnosis results in the inclusion of potential BD patients in the depression group;low specificity of biomarkers stems from limited understanding of BD pathophysiology;and there is a possibility that BD is not innate but develops over the course of the disease.Deepening our understanding of BD pathology, identifying more specific biomarkers, and integrating multiomics approaches for validation studies in well-defined homogeneous cohorts hold promise for significant breakthroughs. 展开更多

BACKGROUND Hepatocellular carcinoma(HCC)is one of the most common malignancies, with high recurrence rates after treatment.Identifying reliable biomarkers for predicting recurrence is essential for improving patient outcomes.Hepatitis B corerelated antigen(HBcrAg)has shown potential as a predictive marker for HCC recurrence.AIM To evaluate the association between HBcrAg levels and the risk of HCC recurrence.METHODS A systematic review was conducted following PRISMA guidelines.PubMed,EMBASE,Web of Science, and the Cochrane Library were searched without restrictions on date or language.Observational studies reporting hazard ratios(HRs)for HBcrAg as a predictor of HCC recurrence were included.Data extraction and quality assessment were performed independently by two reviewers.Statistical analyses used a random-effects model to account for heterogeneity(I²≥50%),and sensitivity analysis was performed to ensure the robustness of the results.RESULTS A total of 1339 articles were initially identified, and 17 studies were included in the final meta-analysis after screening. The pooled analysis showed a significant association between elevated HBcrAg levels and HCC recurrence(HR=4.42,95%confidence interval:3.43-5.41)with substantial heterogeneity(I²=92.6%).Subgroup analysis revealed higher pooled HRs in studies with≥500 participants(HR=4.18)and HBcrAg cut-offs≥4.0 LogU/mL(HR=5.29).Studies with≥10 years of follow-up showed a lower HR(2.89)compared to those with<10 years(3.27).Patients treated with nucleos(t)ide analogs had a pooled HR of 1.98,while those without nucleos(t)ide analog had a higher HR of 3.87.Sensitivity analysis confirmed the robustness of the results, with no significant publication bias detected.CONCLUSION This meta-analysis provides strong evidence that elevated HBcrAg levels are associated with an increased risk of HCC recurrence.HBcrAg may serve as a valuable biomarker for predicting recurrence, aiding personalized management and surveillance strategies for HCC patients. 展开更多

[Objectives] To analyze the influencing factors of fixed defects in patients with catheter fixation in clinical nursing work, in order to provide the best catheter fixation nursing plan for patients.[Methods] 176 inpatients with indwelling catheter from surgical system of Taihe Hospital in Shiyan City from August 2022 to March 2023 were selected. Using a retrospective analysis method, the influencing factors of catheter fixation defects in the study subjects were divided into two categories based on objective characteristics: type I non modifiable influencing factors and type II modifiable influencing factors. Using the standard for catheter fixation defects, whether the patient had catheter fixation defects was determined. After classified and statistically analyzed item by item, binary Logistic multiple regression analysis was used to identify the influencing factors.[Results] The occurrence of catheter fixation defects in patients with catheter fixation was related to factors such as whether the patient was evaluated before fixation, whether the fixation method was standardized and systematic, whether there was sufficient communication between nurses and patients, and the patient s knowledge of catheter fixation. It was also influenced by factors such as the patient s age, catheterization site, catheterization number, catheterization duration, where there was a consciousness disorder, educational level, and external environmental temperature.[Conclusions] Early attention to the key factors affecting patients with catheter fixation defects can effectively prevent adverse factors and provide patients with the best catheter fixation nursing plan to improve nursing quality. 展开更多

AIM: To investigate the role of subjective global assessment (SGA) in nutritional assessment and outcome prediction of Chinese patients with gastrointestinal cancer. METHODS: A total of 751 patients diagnosed with gastrointestinal cancer between August 2004 and August 2006 were enrolled in this study. Within 72 h after admission, SGA, anthropometric parameters, and laboratory tests were used to assess the nutritional status of each patient. The outcome variables including hospital stay, complications, and in-hospital medical expenditure were also obtained.RESULTS: Based on the results of SGA, 389 (51.8%), 332 (44.2%), and 30 (4.0%) patients were classified into well nourished group (SGA-A), mildly to moderately malnourished group (SGA-B), and severely malnourished group (SGA-C), respectively. The prevalence of malnutrition classified by SGA, triceps skinfold thickness (TSF), mid-upper arm muscle circumference (MAMC), albumin (ALB), prealbumin (PA), and body mass index (BMI) was 48.2%, 39.4%, 37.7%, 31.3%, 21.7%, and 9.6%, respectively. In addition, ANOVA tests revealed significant differences in body mass index (BMI), TSF, PA, and ALB of patients in different SGA groups. The more severely malnourished the patient was, the lower the levels of BMI, TSF, PA, and ALB were (P 〈 0.05). x^2 tests showed a significant difference in SGA classification between patients receiving different types of treatment (surgery vs chemotherapy/radiotherapy). As the nutritional status classified by SGA deteriorated, the patients stayed longer in hospital and their medical expenditures increased significantly. Furthermore, multiple regression analysis showed that SGA and serum ALB could help predict the medical expenditures and hospital stay of patients undergoing surgery. The occurrence of complications increased in parallel with the increasing grade of SGA, and was the highest in the SGA-C group (23.3%) and the lowest in the SGA-A group (16.8%). CONCLUSION: SGA is a reliable assessment too and helps to predict the hospital stay and medica expenditures of Chinese surgical gastrointestina cancer patients. 展开更多

Background:Current lung cancer screening guidelines recommend annual low-dose computed tomography(LDCT)for high-risk individuals. However, the effectiveness of LDCT in non-high-risk individuals remains inadequately explored. With the incidence of lung cancer steadily increasing among non-high-risk individuals, this study aims to assess the risk of lung cancer in non-high-risk individuals and evaluate the potential of thin-section LDCT reconstruction combined with artificial intelligence(LDCT-TRAI)as a screening tool. Methods:A real-world cohort study on lung cancer screening was conducted at the West China Hospital of Sichuan University from January 2010 to July 2021.Participants were screened using either LDCT-TRAI or traditional thick-section LDCT without AI(traditional LDCT).The AI system employed was the uAI-ChestCare software.Lung cancer diagnoses were confirmed through pathological examination. Results:Among the 259121 enrolled non-high-risk participants,87260(33.7%)had positive screening results.Within 1 year,728(0.3%)participants were diagnosed with lung cancer,of whom 87.1%(634/728)were never-smokers,and 92.7%(675/728)presented with stage I disease.Compared with traditional LDCT,LDCT-TRAI demonstrated a higher lung cancer detection rate(0.3%vs.0.2%,P<0.001),particularly for stage I cancers(94.4%vs.83.2%,P<0.001),and was associated with improved survival outcomes(5-year overall survival rate:95.4%vs.81.3%,P<0.0001).Conclusion:These findings highlight the importance of expanding lung cancer screening to non-high-risk populations, especially never-smokers.LDCT-TRAI outperformed traditional LDCT in detecting early-stage cancers and improving survival outcomes, underscoring its potential as a more effective screening tool for early lung cancer detection in this population. 展开更多

Magnesium(Mg)deficiency is associated with increased risk and malignancy in colorectal cancer(CRC),yet the underlying mechanisms remain elusive. Here,we used genomic, proteomic, and phosphoproteomic data to elucidate the impact of Mg deficiency on CRC.Genomic analysis identified 160 genes with higher mutation frequencies in Low-Mg tumors, including key driver genes such as KMT2C and ERBB3.Unexpectedly,initiation driver genes of CRC,such as TP53 and APC,displayed higher mutation frequencies in High-Mg tumors.Additionally, proteomic and phosphoproteomic data indicated that low Mg content in tumors may activate epithelial–mesenchymal transition(EMT)by modulating inflammation or remodeling the phosphoproteome of cancer cells.Notably,we observed a negative correlation between the phosphorylation of DBN1 at S142(DBN1S142p)and Mg content.A mutation in S142 to D(DBN1S142D)mimicking DBN1S142p up-regulated MMP2 and enhanced cell migration, while treatment with MgCl2 reduced DBN1S142p,thereby reversing this phenotype.Mechanistically,Mg2þattenuated the DBN1–ACTN4 interaction by decreasing DBN1S142p,which in turn enhanced the binding of ACTN4 to F-actin and promoted F-actin polymerization, ultimately reducing MMP2 expression. These findings shed new light on the crucial role of Mg deficiency in CRC progression and suggest that Mg supplementation may be a promising preventive and therapeutic strategy for CRC. 展开更多

Background Observational studies have indicated a potential link between gut microbiota and sarcopenia. However, the underlying mechanisms and a causal relationship have not been established. Thus, the objective of this study is to examine the possible causal association between gut microbiota and sarcopenia-related traits, including low hand-grip strength and appendicular lean mass(ALM),to shed light on the gut–muscle axis. Methods To investigate the potential impact of gut microbiota on low hand-grip strength and ALM,we utilized a two-sample Mendelian randomization(MR)approach.Summary statistics were obtained from genome-wide association studies of gut microbiota, low hand-grip strength, and ALM.The primary MR analysis employed the random-effects inverse-variance weighted(IVW)method. To assess the robustness,we conducted sensitivity analyses using the MR pleiotropy residual sum and outlier(MR-PRESSO)test to detect and correct for horizontal pleiotropy,as well as the MR-Egger intercept test and leave-one-out analysis. Results Alcaligenaceae,Family XIII,and Paraprevotella were positively associated with the risk of low hand-grip strength(P-values<0.05).Streptococcaceae were negatively associated with low hand-grip strength(P-values<0.05).Eight bacterial taxa(Actinomycetales,Actinomycetaceae,Bacteroidaceae,Porphyromonadaceae,Prevotellaceae,Bacteroides,Marvinbryantia, and Phascolarctobacterium)were associated with a higher risk of ALM(P-values<0.05).Eubacterium fissicatena group was negatively associated with ALM(P-values<0.05).Conclusion We found several gut microbiota components causally associated with sarcopenia-related traits. Our findings provided insights into novel strategies for the prevention and treatment of sarcopenia through the regulation of the gut microbiota, contributing to a better understanding of the gut–muscle axis. 展开更多

Dear Editor,Recently, the World Health Organization(WHO)declared monkeypox as a Public Health Emergency of International Concern(PHEIC),another one after the last PHEIC of COVID-19 announced in early 2020.1 Unlike the new SARS-CoV-2 virus that has multiple variants, monkeypox virus is a DNA virus first identified in 1958 with no evidence of significant mutation of its sequence. 展开更多
