Objective This study aimed to compare the upgrade rate and cancer detection rate between the 18F-DCFPyL PET/MRI-guided ultrasound fusion targeted biopsy(TB)and systematic biopsy in selected patients with suspected pro...Objective This study aimed to compare the upgrade rate and cancer detection rate between the 18F-DCFPyL PET/MRI-guided ultrasound fusion targeted biopsy(TB)and systematic biopsy in selected patients with suspected prostate cancer(the molecular imaging prostate-specific membrane antigen score of≥2 and multiparametric MRI Prostate Imaging Reporting and Data System score of≥4).Methods Eighty-seven selected biopsy-naive patients were randomized into two groups:TB(n=41)and systematic biopsy(control;n=46).Patients diagnosed with clinically significant prostate cancer proceeded to radical prostatectomy.The primary outcome was the pathological upgrade rate.Secondary outcomes,including the cancer detection rate,incidence of repeat biopsy,positive surgical margin,complications,and prostate-specific antigen level at 6 weeks postoperatively,were compared between the groups using the Pearson or Fisher's exact test,as appropriate.Results In the study,prostate cancer was ultimately detected in all patients.The TB group successfully identified all tumors,whereas five patients in the control group initially missed diagnosis.The pathological upgrade rates for the TB and control groups were 31.7%and 56.5%,respectively.Overall,the detection rate for clinically significant prostate cancer(the International Society of Urological Pathology grade of≥2)was significantly higher in the TB group(92.7%)compared with the control group(76.1%,p=0.035).However,no significant difference was found in the detection rate of all prostate cancer.Complications(Clavien–Dindo grade of≤2)occurred in both the TB group(n=11)and control group(n=13).No statistically significant difference was observed between the groups in terms of the positive surgical margin,complications,or 6-week postoperative prostate-specific antigen level.Conclusion The 18F-DCFPyL PET/MRI-guided ultrasound fusion TB alone was an efficient modality in diagnosing selected patients with prostate cancer.展开更多

Developing synergistic therapy can provide a highly promising strategy for anti-cancer treatment through combining the benefits of various mechanisms.Methods In this study, we developed a synergistic strategy for chemo-photothermal therapy by constructing nanocomposites using gold nanorods(GNRs) and tetrahedral framework nucleic acids(tFNA) loaded with the anti-tumor drug doxorubicin(DOX).Results Our in vitro studies have systematically clarified the anti-cancer behaviors of tFNA-DOX@GNR nanocomposites, characterized by their enhanced cellular uptake and proficient lysosomal escape capabilities. It was found that the key role of tFNA-DOX@GNR nanocomposites in tumor ablation is primarily due to their capacity to induce cytotoxicity in tumor cells via a photothermal effect, which generates instantaneous high temperatures. This mechanism introduces various responses in tumor cells, facilitated by the thermal effect and the integrated chemotherapeutic action of DOX. These reactions include the induction of endoplasmic reticulum stress, characterized by elevated reactive oxygen species levels, the promotion of apoptotic cell death, and the suppression of tumor cell proliferation.Conclusion This work exhibits the potential of synergistic therapy utilizing nanocomposites for cancer treatment and offers a promising avenue for future therapeutic strategies.展开更多

Objective:Both ligament-advanced reinforcement system(LARS)and hamstring tendon autograft can serve as grafts for posterior cruciate ligament(PCL)reconstruction.However,few studies have compared the effectiveness of t...Objective:Both ligament-advanced reinforcement system(LARS)and hamstring tendon autograft can serve as grafts for posterior cruciate ligament(PCL)reconstruction.However,few studies have compared the effectiveness of these two approaches.This study therefore aimed to compare the clinical efficacy of arthroscopic reconstruction of the PCL using either the LARS or hamstring tendon autograft.Methods:A total of 36 patients who underwent PCL reconstruction were retrospectively analyzed.Within this cohort,15 patients received a reconstruction using the LARS(LARS group)and 21 using the hamstring tendon autograft(HT group).Results:The pre-and post-operative subjective scores and knee stability were evaluated and the patients were followed up for a period of 2 to 10.5 years(4.11±2.0 years on average).The last follow-up showed that functional scores and knee stability were significantly improved in both groups(P<0.05).Six months after operation,Lysholm scores and IKDC subjective scores were higher in the LARS group than in the HT group(P<0.05).Nonetheless,the last follow-up showed no significant differences in the functional scores or the posterior drawer test between the two groups(P>0.05).In the LARS and HT groups,12 and 9 patients,respectively exhibited KT1000 values<3 mm,with the difference being statistically significant(P<0.05).In the HT group,the diameter of the four-strand hamstring tendon was positively correlated with height(P<0.05),which was 7.37±0.52 mm in males and 6.50±0.77 mm in females(P<0.05).Conclusion:Both LARS and hamstring tendon approaches achieved good efficacy for PCL reconstruction,but patients in the LARS group exhibited faster functional recovery and better knee stability in the long term.LARS is especially suitable for those who hope to resume activities as early as possible.展开更多
Background:Heat stroke(HS)is a serious,life-threatening disease.However,there is no scoring system for HS so far.This research is to establish a scoring system that can quantitatively assess the severity of exertional...Background:Heat stroke(HS)is a serious,life-threatening disease.However,there is no scoring system for HS so far.This research is to establish a scoring system that can quantitatively assess the severity of exertional heat stroke(EHS).Methods:Data were collected from a total of 170 exertional heat stroke(EHS)patients between 2005 and 2016 from 52 hospitals in China.Univariate statistical methods and comparison of the area under the receiver operating characteristic(ROC)curve(AUC)were used to screen exertional heat stroke score(EHSS)parameters,including but not limited body temperature(T),Glasgow Coma Scale(GCS)and others.By comparing the sizes of the AUCs of the APACHE II,SOFA and EHSS assessments,the effectiveness of EHSS in evaluating the prognosis of EHS patients was verified.Results:Through screening with a series of methods,as described above,the present study determined 12 parameters–body temperature(T),GCS,p H,lactate(Lac),platelet count(PLT),prothrombin time(PT),fibrinogen(Fib),troponin I(Tn I),aspartate aminotransferase(AST),total bilirubin(TBIL),creatinine(Cr)and acute gastrointestinal injury(AGI)classification–as EHSS parameters.It is a 0–47 point system designed to reflect increasing severity of heat stroke.Low(EHSS<20)and high scores(EHSS>35)showed 100%survival and 100%mortality,respectively.We found that AUCEHSS>AUCSOFA>AUCAPACHE II.Conclusions:A total of 12 parameters–T,GCS,p H,Lac,PLT,PT,Fib,Tn I,AST,TBIL,Cr and gastrointestinal AGI classification–are the EHSS parameters with the best effectiveness in evaluating the prognosis of EHS patients.As EHSS score increases,the mortality rate of EHS patients gradually increases.展开更多

BACKGROUND A significant relationship between gastric cancer(GC)and depression has been found in the last 20 years.However,there is no comprehensive information that helps researchers find popular and potential resear...BACKGROUND A significant relationship between gastric cancer(GC)and depression has been found in the last 20 years.However,there is no comprehensive information that helps researchers find popular and potential research directions on GC and depression.AIM To determine the research status and hotspots by bibliometric analysis of relevant publications on the relationship between GC and depression.METHODS We used the Web of Science Core Collection to search and collate the literature on GC and depression from 2000 to 2022 on 31 May,2023.Then,visualization analysis was performed using VOSviewer software(version 1.6.19)and the Bibliometrix package in R software.RESULTS We retrieved 153 pertinent publications from 2000 to 2022.The annual publication count showed an overall upward trend.China had the most prominent publications and significant contributions to this field(n=64,41.83%).Before 2020,most studies focused on“the effect of GC on the development and progression of depression in patients.”The latest research trends indicate that“the effect of depression on the occurrence and development of GC and its mechanism”will receive more attention in the future.CONCLUSION The study of“the effect of depression on the occurrence and development of GC and its mechanism”has emerged as a novel research theme over the past two years,which may become a research hotspot in this field.This study provides new insights into the hotpots and frontiers of the relationship between GC and depression,potentially guiding researchers toward hot research topics in the future.展开更多
BACKGROUND Hypertension is the most modifiable factor associated with cardiovascular events and complications.The conventional blood pressure(BP) meter method is simple but is limited in terms of real-time monitoring ...BACKGROUND Hypertension is the most modifiable factor associated with cardiovascular events and complications.The conventional blood pressure(BP) meter method is simple but is limited in terms of real-time monitoring abnormal BP.Therefore,the development of a multifunction smartwatch(HUAWEI WATCH D) sphygmomanometer could significantly improve integrated BP monitoring.METHODS We enrolled 361 subjects from Chinese PL A General Hospital,Beijing,China to validate the accuracy of the smartwatch versatile sphygmomanometer using ISO 81060-2:2018.Resting and ambulatory BP accuracy of the smartwatch were compared with gold standard clinical sphygmomanometers using ISO 81060-2:2018 guidelines,the accuracy of 24 h systolic blood pressure(SBP) circadian rhythm monitoring,and diurnal high SBP alert for this smartwatch were assessed using a confusion matrix approach.Additionally,we analyzed online users of different ages for compliance.RESULTS Eighty-five subjects underwent resting BP measurements;the mean resting BP differences between two devices were-0.683±6.203 mmHg(SBP)(P=0.723) and 1.628±5.028 mmHg(diastolic blood pressure,DBP)(P=0.183).In 35 subjects’ ambulatory BP measurements,the mean differences of ambulatory BP were-1.943±5.475 mmHg(SBP)(P=0.923) and 3.195±5.862 mmHg(DBP)(P=0.065).All data complied with ISO 81060-2:2018 guidelines(mean ≤±5 mmHg and standard deviation ≤±8 mmHg) with no significant differences.Positive predictive values(PPV) of resting SBP and DBP were 0.635 and 0.671,respectively.The PPV of ambulatory SBP and DBP were 0.686.Also,24 h SBP circadian rhythm monitoring was performed in 107 subjects:accuracy=0.850,specificity=0.864,precision/PPV=0.833,sensitivity=0.833,and F1-measure(F1)=0.833.The accuracy,specificity,precision,sensitivity,and F1 values in 85 subjects undergoing diurnal high SBP alerting were 0.858,0.876,0.706,0.809,and 0.754,respectively.CONCLUSIONS When compared with the gold standard clinical sphygmomanometer,smartwatch results were consistent and accurate.Online user feedback showed that elderly individuals cared more about BP monitoring accuracy,with better compliance.展开更多
BACKGROUND Irreversible electroporation(IRE)is a local non-thermal ablative technique which has been suggested as a potential cancer therapy.However,the specific anatomic characteristics of the pancreatic head make it...BACKGROUND Irreversible electroporation(IRE)is a local non-thermal ablative technique which has been suggested as a potential cancer therapy.However,the specific anatomic characteristics of the pancreatic head make it challenging to perform any local ablation in this region.Therefore,the safety and feasibility of IRE in the pancreatic head region should be further explored.AIM To evaluate the safety of IRE in pancreatic head region including its effects on pancreatic ducts,vessels,and adjacent gastrointestinal organs.METHODS Eight landrace miniature pigs underwent IRE of pancreatic head tissue successfully,with a total of 16 lesions created.Laboratory testing including white blood cell(WBC)count and serum amylase before IRE with follow-up laboratory analysis and pathological examination at 1,7,14,and 28 d postablation were performed.RESULTS All pigs tolerated the ablation procedure without serious perioperative complications.Transiently elevated WBC count and amylase were observed at 24 h post-IRE,suggesting an acute pancreatic tissue damage which was confirmed by pathological observations.Vascular endothelial cells and pancreatic duct epithelial cells in ablation zone were also positive in terminal deoxynucleotidyl transferase dUTP nick end labeling staining.There was extensive duodenum mucosa damage with local hemorrhage 24 h after ablation,while regeneration of new villous structures were observed at 7 and 28 d post-IRE.Masson’s trichromatic staining showed that the extracellular matrix was still intact in vessels and pancreatic ducts,and even in the duodenum.CONCLUSION IRE ablation to the pancreatic head may be safe and feasible without long-term damage to the surrounding vital structures.However,risks of stress injuries in acute phase should be taken into consideration to prevent severe perioperative complications.展开更多
BACKGROUND N-terminal pro-B-type natriuretic peptide(NT-proBNP)is often viewed as an indicator for heart failure.However,the prognostic association and the predictive utility of NT-proBNP for postoperative major adver...BACKGROUND N-terminal pro-B-type natriuretic peptide(NT-proBNP)is often viewed as an indicator for heart failure.However,the prognostic association and the predictive utility of NT-proBNP for postoperative major adverse cardiovascular events(MACEs)and myocardial injury after noncardiac surgery(MINS)among older patients are unclear.METHODS In this study,we included 5033 patients aged 65 years or older who underwent noncardiac surgery with preoperative NT-proBNP recorded.Logistic regression was adopted to model the associations between preoperative NT-proBNP and the risk of MACEs and MINS.The receiver operating characteristic curve was used to determine the predictive value of NT-proBNP.RESULTS A total of 5033 patients were enrolled,63 patients(1.25%)and 525 patients(10.43%)had incident postoperative MACEs and MINS,respectively.Analysis of the receiver operating characteristic curve indicated that the cutoff values of ln(NT-proBNP)for MACEs and MINS were 5.16(174 pg/mL)and 5.30(200 pg/mL),respectively.Adding preoperative ln(NT-proBNP)to the Revised Cardiac Risk Index score and the Cardiac and Stroke Risk Model boosted the area under the receiver operating characteristic curves from 0.682 to 0.726 and 0.787 to 0.804,respectively.The inclusion of preoperative NT-proBNP in the prediction models significantly increased the reclassification and discrimination.CONCLUSIONS Increased preoperative NT-proBNP was associated with a higher risk of postoperative MACEs and MINS.The inclusion of NT-proBNP enhances the predictive ability of the preexisting models.展开更多

The coupling of charge carrier and phonon transport limits the application of Ag_(2)Se as a low-toxic near-room-temperature thermoelectric material.Strategies that reduce the thermal conductivity via enhanc-ing the ph...The coupling of charge carrier and phonon transport limits the application of Ag_(2)Se as a low-toxic near-room-temperature thermoelectric material.Strategies that reduce the thermal conductivity via enhanc-ing the phonon scattering usually lead to reduced carrier mobility due to high grain boundary potential barrier.In this study,we developed a cell-membrane-mimic grain boundary engineering strategy for de-coupling the charge carrier and phonon scattering through decorating high-dielectric-constant rutile TiO_(2) at Ag_(2)Se grain boundaries to enable the charge carrier/phonon selective permeability.The nano-sized TiO_(2) with high dielectric permittivity can secure the charge carrier transport by shielding the interfacial Coulomb potential to lower the energy barrier of grain boundaries,rendering an enhanced power factor.Additionally,benefited from the enhanced phonon scattering by TiO_(2) nanoparticles,a significantly de-creased lattice thermal conductivity of~0.20 W m^(-1) K^(-1) and a high zT of~0.97 at 390 K are obtained in the Ag_(2)Se-based nanocomposites.This work demonstrates that such cell-membrane-mimic grain bound-ary engineering strategy may shed light on developing high-performance thermoelectric materials.展开更多
Photobiomodulation(PBM)promoting wound healing has been demonstrated by many studies.Currently,630 nm and 810 nm light-emitting diodes(LEDs),as light sources,are frequently used in the treatment of diabetic foot ulcer...Photobiomodulation(PBM)promoting wound healing has been demonstrated by many studies.Currently,630 nm and 810 nm light-emitting diodes(LEDs),as light sources,are frequently used in the treatment of diabetic foot ulcers(DFUs)in clinics.However,the dose-effect relationship of LED-mediated PBM is not fully understood.Furthermore,among the 630 nm and 810 nm LEDs,which one gets a better effect on accelerating the wound healing of diabetic ulcers is not clear.The aim of this study is to evaluate and compare the effects of 630 nm and 810 nm LED-mediated PBM in wound healing both in vitro and in vivo.Our results showed that both 630 nm and 810 nm LED irradiation significantly promoted the proliferation of mouse fibroblast cells(L929)at different light irradiances(1,5,and 10 mW/cm^(2)).The cell proliferation rate increased with the extension of irradiation time(100,200,and 500 s),but it decreased when the irradiation time was over 500 s.Both 630 nm and 810nm LED irradiation(5 mW/cm^(2))significantly improved the migration capability of L929 cells.No difference between 630 nm and 810 nm LED-mediated PBM in promoting cell proliferation and migration was detected.In vivo results presented that both 630 nm and 810 nm LED irradiation promoted the wound healing and the expression of the vascular endothelial growth factor(VEGF)and transforming growth factor(TGF)in the wounded skin of type 2 diabetic mice.Overall,these results suggested that LED-mediated PBM promotes wound healing of diabetic mice through promoting fibroblast cell proliferation,migration,and the expression of growth factors in the wounded skin.LEDs(630 nm and 810 nm)have a similar outcome in promoting wound healing of type 2 diabetic mice.展开更多
BACKGROUND Utilizing the large jumbo cup in revision total hip arthroplasty is an effective approach to cure many lacunar and segmental peripheral bone defects.However,with the use of the jumbo cup,the center of the h...BACKGROUND Utilizing the large jumbo cup in revision total hip arthroplasty is an effective approach to cure many lacunar and segmental peripheral bone defects.However,with the use of the jumbo cup,the center of the hip joint may become elevated relative to the primary acetabulum,and the diameter of the large cup is greater.AIM To study the height and the significance of the elevation of the hip joint center.METHODS Eighty-eight patients matched the criteria for this condition and were included in the study.The center height of the hip joint was measured relative to the opposite normal hip joint.The diameter of the jumbo cup was measured and checked according to operation notes,and the diameter of the jumbo cup was measured with a prosthesis label.Then,the horizontal and vertical centers of rotation were measured on the surgical side and opposite side.The average center height of the hip joint on the renovated side and the opposite side and the position of the hip cup relative to the teardrop were compared using a paired t-test.RESULTS Radiometric analysis showed that the average hip joint center was elevated by 7.6 mm.The rotational center height delta of the renovated hip was 7.6±5.6 mm,and there was an obvious difference between the two groups(P=0.00).The difference in horizontal distance was 0.5±5.1 mm(-11.5-14.0 mm),and there was no obvious difference between the two groups(P=0.38).According to the foreign standard,the rotational center height delta of the renovated hip was 7.5±6.2 mm,and there was a significant difference between the two groups(P=0.00).There was no obvious difference between the domestic and foreign standards(P>0.05)between the two groups.CONCLUSION The application of the jumbo cup elevates the rotational center of the hip joint,but it is feasible and effective to use the jumbo cup.展开更多

Objective:To compare the efficacy of transoral robotic surgery(TORS)and non-robotic surgery(NRS)in the treatment of tongue base tumors.Methods:A total of 45 patients with tongue base tumors treated in our hospital wer...Objective:To compare the efficacy of transoral robotic surgery(TORS)and non-robotic surgery(NRS)in the treatment of tongue base tumors.Methods:A total of 45 patients with tongue base tumors treated in our hospital were selected,and they were divided into the TORS group and NRS group according to different surgical methods.The surgical indicators and postoperative complications of patients in the two groups were compared and analyzed.Results:Compared with the NRS group,the operative time,bleeding volume and length of hospital stay were less in the TORS group,and the postoperative recurrence rate was less in the TORS group than that in the NRS group.The incidence rate of dysphagia and restricted mouth opening in the TORS group was lower than that in the NRS group within 30 d after surgery,and the difference was statistically significant(P<0.05).Conclusion:TORS has better minimally invasive advantages in the treatment of tongue base tumors,including less intraoperative bleeding,smaller trauma,shorter length of hospital stay and faster recovery.展开更多
Background:The healing of bone defects can be challenging for clinicians to manage,especially after exposure to ionizing radiation.In this regard,radiation therapy and accidental exposure to gamma(o inhibit bone forma...Background:The healing of bone defects can be challenging for clinicians to manage,especially after exposure to ionizing radiation.In this regard,radiation therapy and accidental exposure to gamma(o inhibit bone formation and increase the risk of fractures.Cortical bone-derivγ)-ray radiation have been shown ted stem cells(CBSCs)are reportedly essential for osteogenic lineages,bone maintenance and repair.This study aimed to investigate the effects of melatonin on postradiation CBSCs and bone defect healing.Methods:CBSCs were extracted from C57BL/6 mice and were identified by flow cytometry.Then CBSCs were subjected to 6 Gyγ-ray radiation followed by treatment with various concentrations of melatonin.The effects of exogenous melatonin on the self-renewal and osteogenic capacity of postradiation CBSCs in vitro were analyzed.The underlying mechanisms involved in genomic stability,apoptosis and oxidative stress-related signaling were further analyzed by Western blotting,flow cytometry and immunofluorescence assays.Moreover,postradiation femoral defect models were established and treated with Matrigel and melatonin.The effects of melatonin on postradiation bone healing in vivo were evaluated by micro-CT and pathological analysis.Results:The decrease in radiation-induced self-renewal and osteogenic capacity were partially reversed in postradiation CBSCs treated with melatonin(P<0.05).Melatonin maintained genomic stability,reduced postradiation CBSC apoptosis and intracellular oxidative stress,and enhanced expression of antioxidant-related enzymes(P<0.05).Western blotting validated the anti-inflammatory effects of melatonin by downregulating interleukin-6(IL-6)and tumor necrosis factor alpha(TNF-α)levels via the extracellular regulated kinase(ERK)/nuclear factor erythroid 2-related factor 2(NRF2)/heme oxygenase-1(HO-1)signaling pathway.Melatonin was also found to exhibit antioxidant effects via NRF2 signaling.In vivo experiments demonstrated that the newly formed bone in the melatonin plus Matrigel group had higher trabecular bone volume per tissue volume(BV/TV)and bone mineral density values with lower IL-6 and TNF-αlevels than in the irradiation and the Matrigel groups(P<0.05).Conclusions:This study suggested that melatonin could protect CBSCs against e defects.γ-ray radiation and assist in the healing of postradiation bon.展开更多
Gastric cancer(GC)was referred to a malignant tumor of the digestive tract originating from the epithelium of gastric mucosa.Transcription factor DLX5 was verified as an oncogene in various types of tumors,while miR-3...Gastric cancer(GC)was referred to a malignant tumor of the digestive tract originating from the epithelium of gastric mucosa.Transcription factor DLX5 was verified as an oncogene in various types of tumors,while miR-376a-3p was speculated as a tumor suppressor.Based on the bioinformatics database,we hypothesized that miR-376a participated in the regulation of GC development by targeting DLX5.Compared with adjacent tissue,a significant increase of DLX5 expression was determined in GC tissues,but the expression level is significantly reduced in miR-376a.Similar expression signature of DLX5 and miR-376a was also determined between 4 GC cells(HGC,SGC,MGC,and AGS cell lines)and GES cell line.The level of DLX5 was notably reduced in HGC and MGC cell lines after miR-376a-3p overexpression,and increased after miR-376a-3p inhibition.Then,the inhibition role of miR-376a-3p on DLX5 was further proved by dualluciferase reporter assay.Gain-of-function experiments showed that upregulation of miR-376a-3p in GC cells could inhibit the ability of epithelial-mesenchymal transition,proliferation,and invasion,and enhance the GC cell apoptosis level.However,these roles of miR-376a-3p could be abolished by DLX5 overexpression.This study confirmed that reduction of miR-376a-3p expression level in GC cells would lead to the increase in cell growth and invasion,indicating that upregulation of miR-376a-3p might have a potential therapeutic role on GC.展开更多
文摘Colorectal cancer(CRC)is a prevalent malignancy worldwide,posing a significant public health concern.Mounting evidence has confirmed that timely early screening facilitates the detection of incipient CRC,thereby enhancing patient prognosis.Obviously,non-participation of asymptomatic individuals in screening programs hampers early diagnosis and may adversely affect long-term outcomes for CRC patients.In this letter,we provide a comprehensive overview of the current status of early screening practices,while also thoroughly examine the dilemmas and potential solutions associated with early screening for CRC.In response to these issues,we proffer a set of recommendations directed at governmental authorities and the general public,which focus on augmenting financial investment,establishing standardized screening protocols,advancing technological capabilities,and bolstering public awareness campaigns.The importance of collaborative efforts from various stakeholders cannot be overstated in the quest to enhance early detection rates and alleviate the societal burden of CRC.

文摘Objective This study aimed to compare the upgrade rate and cancer detection rate between the 18F-DCFPyL PET/MRI-guided ultrasound fusion targeted biopsy(TB)and systematic biopsy in selected patients with suspected prostate cancer(the molecular imaging prostate-specific membrane antigen score of≥2 and multiparametric MRI Prostate Imaging Reporting and Data System score of≥4).Methods Eighty-seven selected biopsy-naive patients were randomized into two groups:TB(n=41)and systematic biopsy(control;n=46).Patients diagnosed with clinically significant prostate cancer proceeded to radical prostatectomy.The primary outcome was the pathological upgrade rate.Secondary outcomes,including the cancer detection rate,incidence of repeat biopsy,positive surgical margin,complications,and prostate-specific antigen level at 6 weeks postoperatively,were compared between the groups using the Pearson or Fisher's exact test,as appropriate.Results In the study,prostate cancer was ultimately detected in all patients.The TB group successfully identified all tumors,whereas five patients in the control group initially missed diagnosis.The pathological upgrade rates for the TB and control groups were 31.7%and 56.5%,respectively.Overall,the detection rate for clinically significant prostate cancer(the International Society of Urological Pathology grade of≥2)was significantly higher in the TB group(92.7%)compared with the control group(76.1%,p=0.035).However,no significant difference was found in the detection rate of all prostate cancer.Complications(Clavien–Dindo grade of≤2)occurred in both the TB group(n=11)and control group(n=13).No statistically significant difference was observed between the groups in terms of the positive surgical margin,complications,or 6-week postoperative prostate-specific antigen level.Conclusion The 18F-DCFPyL PET/MRI-guided ultrasound fusion TB alone was an efficient modality in diagnosing selected patients with prostate cancer.

文摘Objective Cancer remains a significant global health challenge, necessitating the development of effective treatment approaches. Developing synergistic therapy can provide a highly promising strategy for anti-cancer treatment through combining the benefits of various mechanisms.Methods In this study, we developed a synergistic strategy for chemo-photothermal therapy by constructing nanocomposites using gold nanorods(GNRs) and tetrahedral framework nucleic acids(tFNA) loaded with the anti-tumor drug doxorubicin(DOX).Results Our in vitro studies have systematically clarified the anti-cancer behaviors of tFNA-DOX@GNR nanocomposites, characterized by their enhanced cellular uptake and proficient lysosomal escape capabilities. It was found that the key role of tFNA-DOX@GNR nanocomposites in tumor ablation is primarily due to their capacity to induce cytotoxicity in tumor cells via a photothermal effect, which generates instantaneous high temperatures. This mechanism introduces various responses in tumor cells, facilitated by the thermal effect and the integrated chemotherapeutic action of DOX. These reactions include the induction of endoplasmic reticulum stress, characterized by elevated reactive oxygen species levels, the promotion of apoptotic cell death, and the suppression of tumor cell proliferation.Conclusion This work exhibits the potential of synergistic therapy utilizing nanocomposites for cancer treatment and offers a promising avenue for future therapeutic strategies.

文摘Objective:Both ligament-advanced reinforcement system(LARS)and hamstring tendon autograft can serve as grafts for posterior cruciate ligament(PCL)reconstruction.However,few studies have compared the effectiveness of these two approaches.This study therefore aimed to compare the clinical efficacy of arthroscopic reconstruction of the PCL using either the LARS or hamstring tendon autograft.Methods:A total of 36 patients who underwent PCL reconstruction were retrospectively analyzed.Within this cohort,15 patients received a reconstruction using the LARS(LARS group)and 21 using the hamstring tendon autograft(HT group).Results:The pre-and post-operative subjective scores and knee stability were evaluated and the patients were followed up for a period of 2 to 10.5 years(4.11±2.0 years on average).The last follow-up showed that functional scores and knee stability were significantly improved in both groups(P<0.05).Six months after operation,Lysholm scores and IKDC subjective scores were higher in the LARS group than in the HT group(P<0.05).Nonetheless,the last follow-up showed no significant differences in the functional scores or the posterior drawer test between the two groups(P>0.05).In the LARS and HT groups,12 and 9 patients,respectively exhibited KT1000 values<3 mm,with the difference being statistically significant(P<0.05).In the HT group,the diameter of the four-strand hamstring tendon was positively correlated with height(P<0.05),which was 7.37±0.52 mm in males and 6.50±0.77 mm in females(P<0.05).Conclusion:Both LARS and hamstring tendon approaches achieved good efficacy for PCL reconstruction,but patients in the LARS group exhibited faster functional recovery and better knee stability in the long term.LARS is especially suitable for those who hope to resume activities as early as possible.

文摘Background:Heat stroke(HS)is a serious,life-threatening disease.However,there is no scoring system for HS so far.This research is to establish a scoring system that can quantitatively assess the severity of exertional heat stroke(EHS).Methods:Data were collected from a total of 170 exertional heat stroke(EHS)patients between 2005 and 2016 from 52 hospitals in China.Univariate statistical methods and comparison of the area under the receiver operating characteristic(ROC)curve(AUC)were used to screen exertional heat stroke score(EHSS)parameters,including but not limited body temperature(T),Glasgow Coma Scale(GCS)and others.By comparing the sizes of the AUCs of the APACHE II,SOFA and EHSS assessments,the effectiveness of EHSS in evaluating the prognosis of EHS patients was verified.Results:Through screening with a series of methods,as described above,the present study determined 12 parameters–body temperature(T),GCS,p H,lactate(Lac),platelet count(PLT),prothrombin time(PT),fibrinogen(Fib),troponin I(Tn I),aspartate aminotransferase(AST),total bilirubin(TBIL),creatinine(Cr)and acute gastrointestinal injury(AGI)classification–as EHSS parameters.It is a 0–47 point system designed to reflect increasing severity of heat stroke.Low(EHSS<20)and high scores(EHSS>35)showed 100%survival and 100%mortality,respectively.We found that AUCEHSS>AUCSOFA>AUCAPACHE II.Conclusions:A total of 12 parameters–T,GCS,p H,Lac,PLT,PT,Fib,Tn I,AST,TBIL,Cr and gastrointestinal AGI classification–are the EHSS parameters with the best effectiveness in evaluating the prognosis of EHS patients.As EHSS score increases,the mortality rate of EHS patients gradually increases.
文摘Aging and age-related ailments have emerged as critical challenges and great burdens within the global contemporary society.Addressing these concerns is an imperative task,with the aims of postponing the aging process and finding effective treatments for age-related degenerative diseases.Recent investigations have highlighted the significant roles of nicotinamide adenine dinucleotide(NAD+)in the realm of anti-aging.It has been empirically evidenced that supplementation with nicotinamide mononucleotide(NMN)can elevate NAD+levels in the body,thereby ameliorating certain age-related degenerative diseases.The principal anti-aging mechanisms of NMN essentially lie in its impact on cellular energy metabolism,inhibition of cell apoptosis,modulation of immune function,and preservation of genomic stability,which collectively contribute to the deferral of the aging process.This paper critically reviews and evaluates existing research on the anti-aging mechanisms of NMN,elucidates the inherent limitations of current research,and proposes novel avenues for anti-aging investigations.
文摘BACKGROUND A significant relationship between gastric cancer(GC)and depression has been found in the last 20 years.However,there is no comprehensive information that helps researchers find popular and potential research directions on GC and depression.AIM To determine the research status and hotspots by bibliometric analysis of relevant publications on the relationship between GC and depression.METHODS We used the Web of Science Core Collection to search and collate the literature on GC and depression from 2000 to 2022 on 31 May,2023.Then,visualization analysis was performed using VOSviewer software(version 1.6.19)and the Bibliometrix package in R software.RESULTS We retrieved 153 pertinent publications from 2000 to 2022.The annual publication count showed an overall upward trend.China had the most prominent publications and significant contributions to this field(n=64,41.83%).Before 2020,most studies focused on“the effect of GC on the development and progression of depression in patients.”The latest research trends indicate that“the effect of depression on the occurrence and development of GC and its mechanism”will receive more attention in the future.CONCLUSION The study of“the effect of depression on the occurrence and development of GC and its mechanism”has emerged as a novel research theme over the past two years,which may become a research hotspot in this field.This study provides new insights into the hotpots and frontiers of the relationship between GC and depression,potentially guiding researchers toward hot research topics in the future.

文摘BACKGROUND Hypertension is the most modifiable factor associated with cardiovascular events and complications.The conventional blood pressure(BP) meter method is simple but is limited in terms of real-time monitoring abnormal BP.Therefore,the development of a multifunction smartwatch(HUAWEI WATCH D) sphygmomanometer could significantly improve integrated BP monitoring.METHODS We enrolled 361 subjects from Chinese PL A General Hospital,Beijing,China to validate the accuracy of the smartwatch versatile sphygmomanometer using ISO 81060-2:2018.Resting and ambulatory BP accuracy of the smartwatch were compared with gold standard clinical sphygmomanometers using ISO 81060-2:2018 guidelines,the accuracy of 24 h systolic blood pressure(SBP) circadian rhythm monitoring,and diurnal high SBP alert for this smartwatch were assessed using a confusion matrix approach.Additionally,we analyzed online users of different ages for compliance.RESULTS Eighty-five subjects underwent resting BP measurements;the mean resting BP differences between two devices were-0.683±6.203 mmHg(SBP)(P=0.723) and 1.628±5.028 mmHg(diastolic blood pressure,DBP)(P=0.183).In 35 subjects’ ambulatory BP measurements,the mean differences of ambulatory BP were-1.943±5.475 mmHg(SBP)(P=0.923) and 3.195±5.862 mmHg(DBP)(P=0.065).All data complied with ISO 81060-2:2018 guidelines(mean ≤±5 mmHg and standard deviation ≤±8 mmHg) with no significant differences.Positive predictive values(PPV) of resting SBP and DBP were 0.635 and 0.671,respectively.The PPV of ambulatory SBP and DBP were 0.686.Also,24 h SBP circadian rhythm monitoring was performed in 107 subjects:accuracy=0.850,specificity=0.864,precision/PPV=0.833,sensitivity=0.833,and F1-measure(F1)=0.833.The accuracy,specificity,precision,sensitivity,and F1 values in 85 subjects undergoing diurnal high SBP alerting were 0.858,0.876,0.706,0.809,and 0.754,respectively.CONCLUSIONS When compared with the gold standard clinical sphygmomanometer,smartwatch results were consistent and accurate.Online user feedback showed that elderly individuals cared more about BP monitoring accuracy,with better compliance.
文摘BACKGROUND Irreversible electroporation(IRE)is a local non-thermal ablative technique which has been suggested as a potential cancer therapy.However,the specific anatomic characteristics of the pancreatic head make it challenging to perform any local ablation in this region.Therefore,the safety and feasibility of IRE in the pancreatic head region should be further explored.AIM To evaluate the safety of IRE in pancreatic head region including its effects on pancreatic ducts,vessels,and adjacent gastrointestinal organs.METHODS Eight landrace miniature pigs underwent IRE of pancreatic head tissue successfully,with a total of 16 lesions created.Laboratory testing including white blood cell(WBC)count and serum amylase before IRE with follow-up laboratory analysis and pathological examination at 1,7,14,and 28 d postablation were performed.RESULTS All pigs tolerated the ablation procedure without serious perioperative complications.Transiently elevated WBC count and amylase were observed at 24 h post-IRE,suggesting an acute pancreatic tissue damage which was confirmed by pathological observations.Vascular endothelial cells and pancreatic duct epithelial cells in ablation zone were also positive in terminal deoxynucleotidyl transferase dUTP nick end labeling staining.There was extensive duodenum mucosa damage with local hemorrhage 24 h after ablation,while regeneration of new villous structures were observed at 7 and 28 d post-IRE.Masson’s trichromatic staining showed that the extracellular matrix was still intact in vessels and pancreatic ducts,and even in the duodenum.CONCLUSION IRE ablation to the pancreatic head may be safe and feasible without long-term damage to the surrounding vital structures.However,risks of stress injuries in acute phase should be taken into consideration to prevent severe perioperative complications.

文摘BACKGROUND N-terminal pro-B-type natriuretic peptide(NT-proBNP)is often viewed as an indicator for heart failure.However,the prognostic association and the predictive utility of NT-proBNP for postoperative major adverse cardiovascular events(MACEs)and myocardial injury after noncardiac surgery(MINS)among older patients are unclear.METHODS In this study,we included 5033 patients aged 65 years or older who underwent noncardiac surgery with preoperative NT-proBNP recorded.Logistic regression was adopted to model the associations between preoperative NT-proBNP and the risk of MACEs and MINS.The receiver operating characteristic curve was used to determine the predictive value of NT-proBNP.RESULTS A total of 5033 patients were enrolled,63 patients(1.25%)and 525 patients(10.43%)had incident postoperative MACEs and MINS,respectively.Analysis of the receiver operating characteristic curve indicated that the cutoff values of ln(NT-proBNP)for MACEs and MINS were 5.16(174 pg/mL)and 5.30(200 pg/mL),respectively.Adding preoperative ln(NT-proBNP)to the Revised Cardiac Risk Index score and the Cardiac and Stroke Risk Model boosted the area under the receiver operating characteristic curves from 0.682 to 0.726 and 0.787 to 0.804,respectively.The inclusion of preoperative NT-proBNP in the prediction models significantly increased the reclassification and discrimination.CONCLUSIONS Increased preoperative NT-proBNP was associated with a higher risk of postoperative MACEs and MINS.The inclusion of NT-proBNP enhances the predictive ability of the preexisting models.
文摘Liver cancer,one of the most common malignancies worldwide,ranks sixth in incidence and third in mortality.Liver cancer treatment options are diverse,inclu-ding surgical resection,liver transplantation,percutaneous ablation,transarterial chemoembolization,radiotherapy,chemotherapy,targeted therapy,immuno-therapy,and traditional Chinese medicine(TCM).A multidisciplinary team(MDT)is essential to customize treatment plans based on tumor staging,liver function,and performance status(PS),ensuring individualized patient care.Treatment decisions require a MDT to tailor strategies based on tumor staging,liver function,and PS,ensuring personalized care.The approval of new first-line and second-line drugs and the establishment of standard treatments based on immune checkpoint inhibitors have significantly expanded treatment options for advanced liver cancer,improving overall prognosis.However,many patients do not respond effectively to these treatments and ultimately succumb to the disease.Modern oncology treatments,while extending patient survival,often come with severe side effects,resistance,and damage to the body,negatively impacting quality of life.Huang et al's study published at World Journal of Gastroenterology rigorously validates the anticancer properties of Calculus bovis,enhancing our understanding of TCM and contributing to new liver cancer treatment strategies.For over 5000 years,TCM has been used in East Asian countries like China to treat various diseases,including liver conditions.Analysis of real-world clinical data suggests that for patients with advanced-stage tumors lacking effective treatments,integrated TCM therapies could provide significant breakthroughs.

文摘Bacterial resistance and excessive inflammation are common issues that hinder wound healing.Antimicrobial peptides(AMPs)offer a promising and versatile antibacterial option compared to traditional antibiotics,with additional anti-inflammatory properties.However,the applications of AMPs are limited by their antimicrobial effects and stability against bacterial degradation.TFNAs are regarded as a promising drug delivery platform that could enhance the antibacterial properties and stability of nanodrugs.Therefore,in this study,a composite hydrogel(HAMA/t-GL13K)was prepared via the photocross-linking method,in which tFNAs carry GL13K.The hydrogel was injectable,biocompatible,and could be instantly photocured.It exhibited broad-spectrum antibacterial and anti-inflammatory properties by inhibiting the expression of inflammatory factors and scavenging ROS.Thereby,the hydrogel inhibited bacterial infection,shortened the wound healing time of skin defects in infected skin full-thickness defect wound models and reduced scarring.The constructed HAMA/tFNA-AMPs hydrogels exhibit the potential for clinical use in treating microbial infections and promoting wound healing.

文摘The coupling of charge carrier and phonon transport limits the application of Ag_(2)Se as a low-toxic near-room-temperature thermoelectric material.Strategies that reduce the thermal conductivity via enhanc-ing the phonon scattering usually lead to reduced carrier mobility due to high grain boundary potential barrier.In this study,we developed a cell-membrane-mimic grain boundary engineering strategy for de-coupling the charge carrier and phonon scattering through decorating high-dielectric-constant rutile TiO_(2) at Ag_(2)Se grain boundaries to enable the charge carrier/phonon selective permeability.The nano-sized TiO_(2) with high dielectric permittivity can secure the charge carrier transport by shielding the interfacial Coulomb potential to lower the energy barrier of grain boundaries,rendering an enhanced power factor.Additionally,benefited from the enhanced phonon scattering by TiO_(2) nanoparticles,a significantly de-creased lattice thermal conductivity of~0.20 W m^(-1) K^(-1) and a high zT of~0.97 at 390 K are obtained in the Ag_(2)Se-based nanocomposites.This work demonstrates that such cell-membrane-mimic grain bound-ary engineering strategy may shed light on developing high-performance thermoelectric materials.

文摘Photobiomodulation(PBM)promoting wound healing has been demonstrated by many studies.Currently,630 nm and 810 nm light-emitting diodes(LEDs),as light sources,are frequently used in the treatment of diabetic foot ulcers(DFUs)in clinics.However,the dose-effect relationship of LED-mediated PBM is not fully understood.Furthermore,among the 630 nm and 810 nm LEDs,which one gets a better effect on accelerating the wound healing of diabetic ulcers is not clear.The aim of this study is to evaluate and compare the effects of 630 nm and 810 nm LED-mediated PBM in wound healing both in vitro and in vivo.Our results showed that both 630 nm and 810 nm LED irradiation significantly promoted the proliferation of mouse fibroblast cells(L929)at different light irradiances(1,5,and 10 mW/cm^(2)).The cell proliferation rate increased with the extension of irradiation time(100,200,and 500 s),but it decreased when the irradiation time was over 500 s.Both 630 nm and 810nm LED irradiation(5 mW/cm^(2))significantly improved the migration capability of L929 cells.No difference between 630 nm and 810 nm LED-mediated PBM in promoting cell proliferation and migration was detected.In vivo results presented that both 630 nm and 810 nm LED irradiation promoted the wound healing and the expression of the vascular endothelial growth factor(VEGF)and transforming growth factor(TGF)in the wounded skin of type 2 diabetic mice.Overall,these results suggested that LED-mediated PBM promotes wound healing of diabetic mice through promoting fibroblast cell proliferation,migration,and the expression of growth factors in the wounded skin.LEDs(630 nm and 810 nm)have a similar outcome in promoting wound healing of type 2 diabetic mice.
文摘BACKGROUND Utilizing the large jumbo cup in revision total hip arthroplasty is an effective approach to cure many lacunar and segmental peripheral bone defects.However,with the use of the jumbo cup,the center of the hip joint may become elevated relative to the primary acetabulum,and the diameter of the large cup is greater.AIM To study the height and the significance of the elevation of the hip joint center.METHODS Eighty-eight patients matched the criteria for this condition and were included in the study.The center height of the hip joint was measured relative to the opposite normal hip joint.The diameter of the jumbo cup was measured and checked according to operation notes,and the diameter of the jumbo cup was measured with a prosthesis label.Then,the horizontal and vertical centers of rotation were measured on the surgical side and opposite side.The average center height of the hip joint on the renovated side and the opposite side and the position of the hip cup relative to the teardrop were compared using a paired t-test.RESULTS Radiometric analysis showed that the average hip joint center was elevated by 7.6 mm.The rotational center height delta of the renovated hip was 7.6±5.6 mm,and there was an obvious difference between the two groups(P=0.00).The difference in horizontal distance was 0.5±5.1 mm(-11.5-14.0 mm),and there was no obvious difference between the two groups(P=0.38).According to the foreign standard,the rotational center height delta of the renovated hip was 7.5±6.2 mm,and there was a significant difference between the two groups(P=0.00).There was no obvious difference between the domestic and foreign standards(P>0.05)between the two groups.CONCLUSION The application of the jumbo cup elevates the rotational center of the hip joint,but it is feasible and effective to use the jumbo cup.

文摘Dental Caries is a kind of chronic oral disease that greatly threaten human being’s health.Though dentists and researchers struggled for decades to combat this oral disease,the incidence and prevalence of dental caries remain quite high.Therefore,improving the disease management is a key issue for the whole population and life cycle management of dental caries.So clinical difficulty assessment system of caries prevention and management is established based on dental caries diagnosis and classification.Dentists should perform oral examination and establish dental records at each visit.When treatment plan is made on the base of caries risk assessment and carious lesion activity,we need to work out patient-centered and personalized treatment planning to regain oral microecological balance,to control caries progression and to restore the structure and function of the carious teeth.And the follow-up visits are made based on personalized caries management.This expert consensus mainly discusses caries risk assessment,caries treatment difficulty assessment and dental caries treatment plan,which are the most important parts of caries management in the whole life cycle.
文摘Objective:To compare the efficacy of transoral robotic surgery(TORS)and non-robotic surgery(NRS)in the treatment of tongue base tumors.Methods:A total of 45 patients with tongue base tumors treated in our hospital were selected,and they were divided into the TORS group and NRS group according to different surgical methods.The surgical indicators and postoperative complications of patients in the two groups were compared and analyzed.Results:Compared with the NRS group,the operative time,bleeding volume and length of hospital stay were less in the TORS group,and the postoperative recurrence rate was less in the TORS group than that in the NRS group.The incidence rate of dysphagia and restricted mouth opening in the TORS group was lower than that in the NRS group within 30 d after surgery,and the difference was statistically significant(P<0.05).Conclusion:TORS has better minimally invasive advantages in the treatment of tongue base tumors,including less intraoperative bleeding,smaller trauma,shorter length of hospital stay and faster recovery.

文摘Background:The healing of bone defects can be challenging for clinicians to manage,especially after exposure to ionizing radiation.In this regard,radiation therapy and accidental exposure to gamma(o inhibit bone formation and increase the risk of fractures.Cortical bone-derivγ)-ray radiation have been shown ted stem cells(CBSCs)are reportedly essential for osteogenic lineages,bone maintenance and repair.This study aimed to investigate the effects of melatonin on postradiation CBSCs and bone defect healing.Methods:CBSCs were extracted from C57BL/6 mice and were identified by flow cytometry.Then CBSCs were subjected to 6 Gyγ-ray radiation followed by treatment with various concentrations of melatonin.The effects of exogenous melatonin on the self-renewal and osteogenic capacity of postradiation CBSCs in vitro were analyzed.The underlying mechanisms involved in genomic stability,apoptosis and oxidative stress-related signaling were further analyzed by Western blotting,flow cytometry and immunofluorescence assays.Moreover,postradiation femoral defect models were established and treated with Matrigel and melatonin.The effects of melatonin on postradiation bone healing in vivo were evaluated by micro-CT and pathological analysis.Results:The decrease in radiation-induced self-renewal and osteogenic capacity were partially reversed in postradiation CBSCs treated with melatonin(P<0.05).Melatonin maintained genomic stability,reduced postradiation CBSC apoptosis and intracellular oxidative stress,and enhanced expression of antioxidant-related enzymes(P<0.05).Western blotting validated the anti-inflammatory effects of melatonin by downregulating interleukin-6(IL-6)and tumor necrosis factor alpha(TNF-α)levels via the extracellular regulated kinase(ERK)/nuclear factor erythroid 2-related factor 2(NRF2)/heme oxygenase-1(HO-1)signaling pathway.Melatonin was also found to exhibit antioxidant effects via NRF2 signaling.In vivo experiments demonstrated that the newly formed bone in the melatonin plus Matrigel group had higher trabecular bone volume per tissue volume(BV/TV)and bone mineral density values with lower IL-6 and TNF-αlevels than in the irradiation and the Matrigel groups(P<0.05).Conclusions:This study suggested that melatonin could protect CBSCs against e defects.γ-ray radiation and assist in the healing of postradiation bon.

文摘Gastric cancer(GC)was referred to a malignant tumor of the digestive tract originating from the epithelium of gastric mucosa.Transcription factor DLX5 was verified as an oncogene in various types of tumors,while miR-376a-3p was speculated as a tumor suppressor.Based on the bioinformatics database,we hypothesized that miR-376a participated in the regulation of GC development by targeting DLX5.Compared with adjacent tissue,a significant increase of DLX5 expression was determined in GC tissues,but the expression level is significantly reduced in miR-376a.Similar expression signature of DLX5 and miR-376a was also determined between 4 GC cells(HGC,SGC,MGC,and AGS cell lines)and GES cell line.The level of DLX5 was notably reduced in HGC and MGC cell lines after miR-376a-3p overexpression,and increased after miR-376a-3p inhibition.Then,the inhibition role of miR-376a-3p on DLX5 was further proved by dualluciferase reporter assay.Gain-of-function experiments showed that upregulation of miR-376a-3p in GC cells could inhibit the ability of epithelial-mesenchymal transition,proliferation,and invasion,and enhance the GC cell apoptosis level.However,these roles of miR-376a-3p could be abolished by DLX5 overexpression.This study confirmed that reduction of miR-376a-3p expression level in GC cells would lead to the increase in cell growth and invasion,indicating that upregulation of miR-376a-3p might have a potential therapeutic role on GC.
