Dear Editor,Fungal keratitis is a serious and potentially sightthreatening infection that poses a global health concern,with higher incidence rates reported in low-income countries and regions with tropical climates[1-3].Risk factors for fungal keratitis include trauma with vegetative matter,contact lens wear,history of ocular surgery,chronic topical steroid use,low socioeconomic status and tropical climate[1-2].Fungi have the propensity to penetrate deep into the corneal stroma,making the infection difficult to treat with topical antifungal agents alone.Complications such as corneal perforation and endophthalmitis can arise in severe cases,making early identification of the causative fungal pathogen of utmost importance[4-5].Moreover,obtaining susceptibilities to the available antifungal agents can be sight and eye saving[5]. 展开更多

The retrospective study by Edwar et al reinforces the role of therapeutic penetrating keratoplasty(PK)as a vital intervention in severe,treatment-resistant infectious keratitis.In advanced cases—often complicated by trauma,delayed presentation,and corneal perforation—PK restores globe integrity and provides limited visual recovery.However,its application is constrained by graft-related complications and donor shortages,particularly in low-resource settings.These limitations highlight the need for earlier,globe-sparing strategies to prevent progression and reduce surgical demand.Photoactivated chromophore for infectious keratitis-corneal collagen cross-linking(PACK-CXL)has emerged as a promising adjunct or alternative.With both antimicrobial and tissue-stabilizing effects,PACK-CXL may control infection and preserve corneal structure in earlier stages.A layered treatment framework that incorporates PACK-CXL as an initial intervention and reserves PK for refractory cases may help improve clinical outcomes.Further studies are needed to define their best use in practice. 展开更多

Impact of corneal parameters on intraocular pressure measurements in different tonometry methods 被引量:6

AIM: To evaluate the impact of central corneal thickness(CCT) and corneal curvature on intraocular pressure(IOP) measurements performed by three different tonometers. METHODS: IOP in 132 healthy eyes of 66 participants was measured using three different tonometry techniques: Goldmann applanation tonometer(GAT), Pascal dynamic contour tonometer(DCT), and ICare rebound tonometer(RT). CCT and corneal curvature were assessed. RESULTS: In healthy eyes, DCT presents significantly higher values of IOP than GAT(17.34±3.69 and 15.27±4.06 mm Hg, P<0.0001). RT measurements are significantly lower than GAT(13.56±4.33 mm Hg, P<0.0001). Compared with GAT, DCT presented on average 2.51 mm Hg higher values in eyes with CCT<600 μm and 0.99 mm Hg higher results in eyes with CCT≥600 μm. The RT results were lower on average by 1.61 and 1.95 mm Hg than those obtained by GAT, respectively. Positive correlations between CCT in eyes with CCT<600 μm were detected for all IOP measurement techniques, whereas a similar relationship was not observed in eyes with thicker corneas. A correlation between IOP values and keratometry in the group with CCT<600 μm was not detected with any of the tonometry methods. In thicker corneas, a positive correlation was found for GAT and mean keratometry values(R=0.369, P=0.005). CONCLUSION: The same method should always be chosen for routine IOP control, and measurements obtained by different methods cannot be compared. All analysed tonometry methods are dependent on CCT;thus, CCT should be taken into consideration for both diagnostics and monitoring. 展开更多

The aim of this article is to examine the effect of omega-3(ω-3) long-chain polyunsaturated fatty acids(LCPUFAs) intake on retinopathy of prematurity(ROP) by reviewing the experimental and clinical trials conducted on animal models and infants.LCPUFAs demonstrate cytoprotective and cytotherapeutic actions contributing to a number of anti-angiogenic and neuroprotective mechanisms within the retina.Their intake appears to have a beneficial effect on ischemia,oxidative stress,inflammation and cellular signaling mechanisms,influencing retinal cell gene expression and cellular differentiation.ω-3 LCPUFAs may modulate metabolic processes that activate molecules implicated in the pathogenesis of vasoproliferative and neurodegenerative retinal diseases such as ROP. 展开更多

AIM:To evaluate the incidence of increased intraocular pressure(IOP)and glaucomatous changes in systemic lupus erythematosus(SLE)patients in comparison with systemic steroids and immunosuppressive treatment.METHODS:Sixty-two women with SLE were divided into two groups:treated(n=47,94 eyes)and not treated(n=15,30 eyes)with systemic glucocorticosteroids(GC;GC-free).Twenty-one individuals in GC group were treated with immunosuppressive agents(immunomodulating and biologic).The visual acuity and IOP with ocular pulsatile amplitude(OPA)measurements,as well as scanning laser polarimetry(GDx)with nerve fiber index(NFI)measurement,spectral domain optical coherence tomography(SD-OCT)of the optic disk with retinal nerve fiber layer(RNFL)analysis and the macular region with ganglion cell analysis(GCA)were performed.RESULTS:Mean IOP values in group with combined GC and immunosuppressive therapy was 15.8±2.56 mm Hg and was significantly lower than in individuals with exclusive GC treatment(17.63±4.38 mm Hg,P=0.043).Contrary,no dif ferences in mean IOP values between GC-free group and individuals treated with combined GC and immunosuppressive therapy were detected(P=0.563).Similarly,mean IOP in GC was 17.14±3.94 mm Hg and in GC-free patients was equal to 16.67±3.45 mm Hg(P=0.671).According to treatment regimen no statistical differences in optic disk SD-OCT for RNFL thickness,RNFL symmetry,cupping volume and the C/D ratio were observed.Similarly,no statistical differences for the mean and minimal ganglion cell layer(GCL)thickness measured in macular SD-OCT or NFI in GDx were detected.CONCLUSION:Combined immunosuppressive and systemic GC therapy in SLE patients may lower the risk of iatrogenic ocular hypertension.No relationship between treatment regimen and glaucomatous damage of optic nerve fibers in analyzed groups with SLE is detected. 展开更多

Objective: to study the clinical effect of modified double eyelid plasty in the treatment of senile upper eyelid skin relaxation with entropion and trichiasis. Methods: the clinical data of 100 elderly patients with upper eyelid skin relaxation and entropion trichiasis treated in our hospital were retrospectively analyzed. All patients were treated with modified double eyelid plasty, and the clinical effect was analyzed. Results: the suture was removed 7 days after operation. The skin incision healed well, the height and radian of double eyelid were natural, the position of eyelid margin was normal, the eyelashes were far away from the cornea, and the clinical symptoms alleviated or disappeared;10 cases of slight postoperative eyelid insufficiency were improved by applying ointment after operation;There were no serious complications in 100 patients;After 12 months of follow-up, there was no recurrence of trichiasis and the double eyelids were symmetrical on both sides. Compared with before operation, the width, inclination, angle and inner canthus distance of blepharoplasty were better (P < 0.05). Conclusion: the treatment of senile upper eyelid skin relaxation with entropion trichiasis with modified double eyelid plasty can improve the state of upper eyelid skin relaxation, restore the position of eyelid to normal, and correct entropion trichiasis. Compared with before and after operation, the width, inclination, angle and distance of eyelid fissure are better, which can be used for reference. 展开更多

AIM:To evaluate the alterations of the retinal microvasculature and foveal avascular zone in patients with Parkinson’s disease(PD)using optical coherence tomography angiography(OCT-A).METHODS:A retrospective study of PD patients examined in the Ophthalmology Department of the General Hospital of Athens,“Georgios Gennimatas”from March 2021 to March 2022 was conducted.Totally 44 patients with PD were included and 18 healthy controls were examined,hence a total of 124 eyes were enrolled in the study.The foveal and parafoveal superficial and deep capillary plexus vascular density(fSCP-VD,fDCP-VD,pSCP-VD,pDCP-CD)and foveal avascular zone(FAZ)were quantified with OCTA.Optical coherence tomography(OCT)was used to measure macular thickness.Our statistical analysis was conducted by using a mixed effect linear regression model.RESULTS:After adjustment for age and gender,the mean parafoveal superficial capillary plexus vascular density(pSCP-VD)and mean parafoveal deep capillary plexus vascular density(pDCP-VD)were significantly decreased in individuals with PD(P<0.001 in both)by-2.35(95%CI-3.3,-1.45)and-7.5(95%CI-10.4,-4.6)respectively.fSCP-VD and fDCP-VD didn’t approach statistical significance.The FAZ area and perimeter were significantly decreased(P<0.001 in both)by-0.1 mm^(2)(95%CI-0.13,-0.07)and-0.49 mm^(2)(95%CI-0.66,-0.32)respectively.Circularity didn’t approach statistical significance.Central retinal thickness(CRT)was significantly decreased in individuals with PD(P<0.001)by-23.1μm(95%CI-30.2,-16)and temporal retinal thickness(TRT)was decreased(P=0.025)by-11μm(95%CI-22,-1.5)while nasal retinal thickness(NRT)only approached statistical significance(P=0.066).CONCLUSION:The mean pSCP-VD,pDCP-VD,CRT and TRT are significantly decreased and FAZ is altered in individuals with PD.These findings can be potentially used as biomarkers for the diagnosis and evaluation of early PD. 展开更多

Background:Diabetic retinopathy is a major cause of visual impairment and blindness among working-age people worldwide.The aim of our study was to investigate the effects of a carotenoid supplementation on retinal thickness and macular function of patients with diabetes using optical coherence tomography(OCT)and multifocal electroretinography(mfERG).Methods:A retrospective study of one hundred and twenty eyes of sixty patients age between 40 and 60 years with non-insulin dependent type 2 diabetes mellitus without diabetic retinopathy who underwent OCT and mfERG and took vitamin supplements for a period of two years.Patients received a carotenoid supplement containing lutein(10 mg),zeaxanthin(2 mg)and meso-zeaxanthin(10 mg)once a day for two years.The thickness of the fovea was evaluated using OCT and the macular function was tested by mfERG.Results:OCT showed an increase in the central foveal thickness and mfERG revealed increased retinal response density within the central 13°surrounding the fovea(rings 1 to 3)at two years after the onset of carotenoids supplement intake.Conclusion:The use of carotenoid supplements may be of benefit for improving visual function of type 2 diabetes patients.However,further study is needed to assess the treatment’s long-term efficacy. 展开更多
