The Joint Center is an overseas science and education platform established by Chinese Academy of Sciences. CSL-CER has conducted a series of research activities, including an annual scientific expedition to the eastern Indian Ocean, with many kinds of instruments installed for monsoon and ocean observation. The ‘Numerical Forecast System for the Surrounding Area of Sri Lanka,' which was developed by the Joint Center, can provide rapid mapping on multiple scales and at multiple resolutions for numerical forecast products serving local society. The annual China–Sri Lanka Joint Workshop on Monsoon Climate and Ocean Environment was successfully held in 2015 and 2016. BACKGROUND Tuberculosis(TB)is a common infection causing huge morbidity and mortality to mankind.The analytical methods used in diagnosing TB are not sensitive in paucibacillary infections and also require trained tec...BACKGROUND Tuberculosis(TB)is a common infection causing huge morbidity and mortality to mankind.The analytical methods used in diagnosing TB are not sensitive in paucibacillary infections and also require trained technical personnel.MicroRNAs are stable in serum and other body fluids,and hold great potential in the diagnosis of TB.AIM To analyze the dysregulated microRNA profiles among patients with cavitatory and non-cavitatory pulmonary TB.METHODS The prospective study will be conducted in a tertiary care center in India.Adult patients with newly diagnosed pulmonary TB will be included.There will be two groups:Patients with sputum positive pulmonary TB with cavity and without cavity(group1),and apparently healthy individuals(group 2).The participants will undergo sputum examination,Xpert Mycobacterium TB complex/resistance to rifampin(Mtb/RIF)assay,chest X-ray,and blood investigations and serum microRNA detection.Ethics approval has been obtained.Written informed consent will be obtained.Appropriate statistical analyses will be used.RESULTS MicroRNAs will be correlated with sputum positivity,Xpert Mtb/RIF assay,radiological involvement,inflammatory markers,and course of the disease among cases and controls.CONCLUSION MicroRNAs could serve as potential diagnostic biomarkers in diagnostically challenging TB patients.展开更多
BACKGROUND Drug utilization research has an important role in assisting the healthcare administration to know,compute,and refine the prescription whose principal objective is to enable the rational use of drugs.Resear...BACKGROUND Drug utilization research has an important role in assisting the healthcare administration to know,compute,and refine the prescription whose principal objective is to enable the rational use of drugs.Research in developing nations relating to the cost of treatment is scarce when compared with developed countries.Thus,the drug utilization research studies from developing nations are most needed,and their number has been growing.AIM To evaluate patterns of utilization of antipsychotic drugs and direct medical cost analysis in patients newly diagnosed with schizophrenia.METHODS The present study was observational in type and based on a retrospective cohort to evaluate patterns of utilization of antipsychotic drugs using World Health Organization(WHO)core prescribing indicators and anatomical therapeutic chemical/defined daily dose indicators.We also calculated direct medical costs for a period of 6 months.RESULTS This study has found that atypical antipsychotics are the mainstay of treatment for schizophrenia in every age group and subcategories of schizophrenia.The evaluation based on WHO prescribing indicators showed a low average number of drugs per prescription and low prescribing frequency of antipsychotics from the National List of Essential Medicines 2015 and the WHO Essential Medicines List 2019.The total mean drug cost of our study was 1396 Indian rupees.The total mean cost due to the investigation in our study was 1017.34 Indian rupees.Therefore,the total mean direct medical cost incurred on patients in our study was 4337.28 Indian rupees.CONCLUSION The information from the present study can be used for reviewing and updating treatment policy at the institutional level.展开更多
BACKGROUND Orthobiologics-biological substances like platelet-rich plasma(PRP),bone marrow aspirate concentrate,and stem cells-are increasingly used in musculoskeletal care to promote tissue repair and reduce reliance...BACKGROUND Orthobiologics-biological substances like platelet-rich plasma(PRP),bone marrow aspirate concentrate,and stem cells-are increasingly used in musculoskeletal care to promote tissue repair and reduce reliance on invasive surgery.Despite global momentum,India's clinical adoption remains underexplored.AIM To inform education,policy,and resource allocation for the safe and effective adoption of orthobiologics in musculoskeletal care.METHODS A cross-sectional electronic survey was conducted from January to March 2025 among orthopaedic surgeons,academicians,and trainees across India.The questionnaire assessed demographics,knowledge of orthobiologics,attitudes toward training and subspecialization,usage trends,regulatory awareness,and perceived barriers.Data were analyzed using descriptive statistics andχ2/Fisher’s exact tests,with P<0.05 considered significant.RESULTS A total of 1280 valid responses were collected.Awareness of orthobiologics was high(97%),with PRP being the most familiar and widely used(80%).Formal training was reported by only 31%,though 85%showed interest in structured education,and 68%supported orthobiologics as a subspecialty.Satisfaction with clinical outcomes averaged 6.5±2.3 out of 10 points.Barriers included high treatment cost(64%),poor patient awareness(90%),and limited access to biologics labs(18%).Regulatory understanding was moderate,with academic-affiliated clinicians more informed about stem cell guidelines.CONCLUSION Indian orthopaedic professionals demonstrate strong awareness and optimism toward orthobiologics,but widespread gaps in training,infrastructure,and regulation hinder broader adoption.Strategic investments in education,standardized protocols,and accessible facilities are essential to support safe and evidence-driven integration of regenerative therapies into clinical practice.展开更多
Molecular hydrogen(H2)demonstrates selective antioxidant and anti-inflammatory properties with therapeutic potential across musculoskeletal conditions including osteoarthritis,rheumatoid arthritis,exercise-induced mus...Molecular hydrogen(H2)demonstrates selective antioxidant and anti-inflammatory properties with therapeutic potential across musculoskeletal conditions including osteoarthritis,rheumatoid arthritis,exercise-induced muscle damage,chronic pain syndromes,tendinopathies,and muscle atrophy.This review critically evaluates preclinical and clinical evidence for H2 therapy and identifies research gaps.A comprehensive search of PubMed,EMBASE,and Cochrane Library(up to April 2025)yielded 45 eligible studies:25 preclinical and 20 clinical trials.Preclinical models consistently showed reductions in reactive oxygen species,inflammatory cytokines,and improved cell viability.Clinical trials reported symptomatic relief in osteoarthritis,decreased Disease Activity Score 28 in rheumatoid arthritis,and accelerated clearance of muscle damage markers.Delivery methods varied-hydrogen-rich water,gas inhalation,and saline infusion-hindering direct comparison.Mechanistic biomarkers were inconsistently reported,limiting understanding of target engagement.Common limitations included small sample sizes,short durations,and protocol heterogeneity.Despite these constraints,findings suggest H2 may serve as a promising adjunctive therapy via antioxidant,anti-inflammatory,and cytoprotective mechanisms.Future research should prioritize standardized delivery protocols,robust mechanistic endpoints,and longer-term randomized trials to validate clinical efficacy and optimize therapeutic strategies.展开更多
Nanotechnology has revolutionized drug delivery,particularly through nanoformulations of phytoconstituents,enhancing their therapeutic potential.Despite their broad bioactivities,plant-based compounds often suffer fro...Nanotechnology has revolutionized drug delivery,particularly through nanoformulations of phytoconstituents,enhancing their therapeutic potential.Despite their broad bioactivities,plant-based compounds often suffer from poor bioavailability and stability.Nanoformulations address these limitations by improving solubility,targeted delivery,and controlled release.This approach opens new possibilities for treating chronic diseases like cancer,diabetes,and neurodegenerative disorders.This review aims to examine recent advancements in nanotechnology-based formulation strategies designed to enhance the delivery,stability,and therapeutic efficacy of phytochemicals and also discusses regulatory issues,safety concerns,scalability,and cost-effectiveness.Emphasis was placed on nanoformulation techniques employed for key phytoconstituents such as curcumin,resveratrol,epigallocatechin gallate,and quercetin.The most commonly employed nanocarriers included polymeric nanoparticles,solid lipid nanoparticles,and liposomes.These formulations significantly improved the solubility,stability,and controlled release profiles of phytochemicals.In vitro and in vivo studies demonstrated enhanced anti-inflammatory,anticancer,and antioxidant activities.Moreover,surface-modified and targeted nanoparticles showed promise in increasing site-specific targeting and enhancing bioavailability of the encapsulated compounds.Nanoformulations present a promising strategy for overcoming the pharmacokinetic limitations of phytochemicals.Despite encouraging preclinical results,further studies are needed to address issues related to long-term safety,clinical efficacy,and regulatory approval for successful clinical translation.展开更多
Over the last three decades burgeoning research has shown that anxiety disorder comorbidity is not only highly prevalent in bipolar disorder(BD),but it also adversely impacts the course,outcome,and treatment of BD.The...Over the last three decades burgeoning research has shown that anxiety disorder comorbidity is not only highly prevalent in bipolar disorder(BD),but it also adversely impacts the course,outcome,and treatment of BD.The present review provides an overview of the current trends in research on comorbid anxiety and BDs based on prior reviews and meta-analyses(n=103),epidemiological surveys,and large-scale clinical studies.The results reiterated the fact that at least half of those with BD are likely to develop an anxiety disorder in their lifetimes and a third of them will manifest an anxiety disorder at any point of time.All types of anxiety disorders were equally common in BD.However,there was a wide variation in rates across different sources,with most of this discrepancy being accounted for by methodological differences between reports.Comorbid anxiety disorders negatively impacted the presentation and course of BD.This unfavourable clinical profile led to poorer outcome and functioning and impeded treatment of BD.Despite the extensive body of research there was paucity of data on aetiology and treatment of anxiety disorder comorbidity in BD.Nevertheless,the substantial burden and unique characteristics of this comorbidity has important clinical and research implications.展开更多

This study aimed to prepare a disaster manual of a 1 900 bed tertiary care hospital, in consultation and involvement of all concerned stakeholders.METHODS: A committee of members from hospital administration, clinical, diagnostic and supportive departments worked on an initial document prepared according to the Act and gave their inputs to frame a final disaster manual.RESULTS: The prepared departmental standard operating procedures involved 116 people(doctors and paramedical staff), and were then synchronized, in 12 committee meetings, to produce the final hospital disaster manual.CONCLUSIONS: The present disaster manual is one of the few comprehensive plans prepared by the stakeholders of a government hospital in India, who themselves form a part of the disaster response team. It also helped in co-ordinated conduction of mock drills.展开更多

The endothelium modulates vascular homeostasis owing to a variety of vasoconstrictors and vasodilators.Endothelial dysfunction(ED),characterized by impaired vasodilation,inflammation,and thrombosis,triggers future car...The endothelium modulates vascular homeostasis owing to a variety of vasoconstrictors and vasodilators.Endothelial dysfunction(ED),characterized by impaired vasodilation,inflammation,and thrombosis,triggers future cardiovascular(CV)diseases.Chronic kidney disease,a state of chronic inflammation caused by oxidative stress,metabolic abnormalities,infection,and uremic toxins damages the endothelium.ED is also associated with a decline in estimated glomerular filtration rate.After kidney transplantation,endothelial functions undergo immediate but partial restoration,promising graft longevity and enhanced CV health.However,the anticipated CV outcomes do not happen due to various transplant-related and unrelated risk factors for ED,culminating in poor CV health and graft survival.ED in kidney transplant recipients is an underrecognized and poorly studied entity.CV diseases are the leading cause of death among kidney transplant candidates with functioning grafts.ED contributes to the pathogenesis of many of the CV diseases.Various biomarkers and vasoreactivity tests are available to study endothelial functions.With an increasing number of transplants happening every year,and improved graft rejection rates due to the availability of effective immunosuppressants,the focus has now shifted to endothelial protection for the prevention,early recognition,and treatment of CV diseases.展开更多

The purpose of this study was to evaluate the characteristic features of psychotropic drug prescriptions for patients with schizophrenia in Pakistan. Methods: Three centers in Pakistan participated in a large scale collaborative study known as Research on Asian Prescription Pattern (REAP). The 2016 REAP survey included centers from 15 countries in Asia and used a unified research protocol. The design of the study was quantitative and of descriptive epidemiology. Analysis was made on the data collected from three centers i.e., Lahore, Karachi and Islamabad. The data collected in Pakistan were compared with those from other Asian countries. The details of REAP were presented on the homepage of REAP (http://www.REAP.Asia). Results: From Pakistan, 298 patients were included. Patients with schizophrenia in Pakistan received higher rate of antipsychotic polypharmacy and a higher rate of co-prescription of mood stabilizers and anxiolytics when compared with participants from other South Asian countries (India and Bangladesh). Conclusion: The main findings of the study were that a majority of the patients were prescribed antipsychotic polypharmacy drugs in Pakistan. Insufficient training on the use of psychotropic drugs and a lack of treatment guideline are considered to be the major contributing factors. Further education and training on the proper use of psychotropic drugs are recommended to psychiatrists in Pakistan. The guidelines on pharmacotherapy for patients with schizophrenia should also be developed and promoted in Pakistan.展开更多

文摘Artificial intelligence(AI)tools,like OpenAI's Chat Generative Pre-trained Transformer(ChatGPT),hold considerable potential in healthcare,academia,and diverse industries.Evidence demonstrates its capability at a medical student level in standardized tests,suggesting utility in medical education,radiology reporting,genetics research,data optimization,and drafting repetitive texts such as discharge summaries.Nevertheless,these tools should augment,not supplant,human expertise.Despite promising applications,ChatGPT confronts limitations,including critical thinking tasks and generating false references,necessitating stringent cross-verification.Ensuing concerns,such as potential misuse,bias,blind trust,and privacy,underscore the need for transparency,accountability,and clear policies.Evaluations of AI-generated content and preservation of academic integrity are critical.With responsible use,AI can significantly improve healthcare,academia,and industry without compromising integrity and research quality.For effective and ethical AI deployment,collaboration amongst AI developers,researchers,educators,and policymakers is vital.The development of domain-specific tools,guidelines,regulations,and the facilitation of public dialogue must underpin these endeavors to responsibly harness AI's potential.
文摘BACKGROUND Tuberculosis(TB)is a common infection causing huge morbidity and mortality to mankind.The analytical methods used in diagnosing TB are not sensitive in paucibacillary infections and also require trained technical personnel.MicroRNAs are stable in serum and other body fluids,and hold great potential in the diagnosis of TB.AIM To analyze the dysregulated microRNA profiles among patients with cavitatory and non-cavitatory pulmonary TB.METHODS The prospective study will be conducted in a tertiary care center in India.Adult patients with newly diagnosed pulmonary TB will be included.There will be two groups:Patients with sputum positive pulmonary TB with cavity and without cavity(group1),and apparently healthy individuals(group 2).The participants will undergo sputum examination,Xpert Mycobacterium TB complex/resistance to rifampin(Mtb/RIF)assay,chest X-ray,and blood investigations and serum microRNA detection.Ethics approval has been obtained.Written informed consent will be obtained.Appropriate statistical analyses will be used.RESULTS MicroRNAs will be correlated with sputum positivity,Xpert Mtb/RIF assay,radiological involvement,inflammatory markers,and course of the disease among cases and controls.CONCLUSION MicroRNAs could serve as potential diagnostic biomarkers in diagnostically challenging TB patients.
文摘BACKGROUND Drug utilization research has an important role in assisting the healthcare administration to know,compute,and refine the prescription whose principal objective is to enable the rational use of drugs.Research in developing nations relating to the cost of treatment is scarce when compared with developed countries.Thus,the drug utilization research studies from developing nations are most needed,and their number has been growing.AIM To evaluate patterns of utilization of antipsychotic drugs and direct medical cost analysis in patients newly diagnosed with schizophrenia.METHODS The present study was observational in type and based on a retrospective cohort to evaluate patterns of utilization of antipsychotic drugs using World Health Organization(WHO)core prescribing indicators and anatomical therapeutic chemical/defined daily dose indicators.We also calculated direct medical costs for a period of 6 months.RESULTS This study has found that atypical antipsychotics are the mainstay of treatment for schizophrenia in every age group and subcategories of schizophrenia.The evaluation based on WHO prescribing indicators showed a low average number of drugs per prescription and low prescribing frequency of antipsychotics from the National List of Essential Medicines 2015 and the WHO Essential Medicines List 2019.The total mean drug cost of our study was 1396 Indian rupees.The total mean cost due to the investigation in our study was 1017.34 Indian rupees.Therefore,the total mean direct medical cost incurred on patients in our study was 4337.28 Indian rupees.CONCLUSION The information from the present study can be used for reviewing and updating treatment policy at the institutional level.
文摘BACKGROUND Orthobiologics-biological substances like platelet-rich plasma(PRP),bone marrow aspirate concentrate,and stem cells-are increasingly used in musculoskeletal care to promote tissue repair and reduce reliance on invasive surgery.Despite global momentum,India's clinical adoption remains underexplored.AIM To inform education,policy,and resource allocation for the safe and effective adoption of orthobiologics in musculoskeletal care.METHODS A cross-sectional electronic survey was conducted from January to March 2025 among orthopaedic surgeons,academicians,and trainees across India.The questionnaire assessed demographics,knowledge of orthobiologics,attitudes toward training and subspecialization,usage trends,regulatory awareness,and perceived barriers.Data were analyzed using descriptive statistics andχ2/Fisher’s exact tests,with P<0.05 considered significant.RESULTS A total of 1280 valid responses were collected.Awareness of orthobiologics was high(97%),with PRP being the most familiar and widely used(80%).Formal training was reported by only 31%,though 85%showed interest in structured education,and 68%supported orthobiologics as a subspecialty.Satisfaction with clinical outcomes averaged 6.5±2.3 out of 10 points.Barriers included high treatment cost(64%),poor patient awareness(90%),and limited access to biologics labs(18%).Regulatory understanding was moderate,with academic-affiliated clinicians more informed about stem cell guidelines.CONCLUSION Indian orthopaedic professionals demonstrate strong awareness and optimism toward orthobiologics,but widespread gaps in training,infrastructure,and regulation hinder broader adoption.Strategic investments in education,standardized protocols,and accessible facilities are essential to support safe and evidence-driven integration of regenerative therapies into clinical practice.
文摘Molecular hydrogen(H2)demonstrates selective antioxidant and anti-inflammatory properties with therapeutic potential across musculoskeletal conditions including osteoarthritis,rheumatoid arthritis,exercise-induced muscle damage,chronic pain syndromes,tendinopathies,and muscle atrophy.This review critically evaluates preclinical and clinical evidence for H2 therapy and identifies research gaps.A comprehensive search of PubMed,EMBASE,and Cochrane Library(up to April 2025)yielded 45 eligible studies:25 preclinical and 20 clinical trials.Preclinical models consistently showed reductions in reactive oxygen species,inflammatory cytokines,and improved cell viability.Clinical trials reported symptomatic relief in osteoarthritis,decreased Disease Activity Score 28 in rheumatoid arthritis,and accelerated clearance of muscle damage markers.Delivery methods varied-hydrogen-rich water,gas inhalation,and saline infusion-hindering direct comparison.Mechanistic biomarkers were inconsistently reported,limiting understanding of target engagement.Common limitations included small sample sizes,short durations,and protocol heterogeneity.Despite these constraints,findings suggest H2 may serve as a promising adjunctive therapy via antioxidant,anti-inflammatory,and cytoprotective mechanisms.Future research should prioritize standardized delivery protocols,robust mechanistic endpoints,and longer-term randomized trials to validate clinical efficacy and optimize therapeutic strategies.
文摘Nanotechnology has revolutionized drug delivery,particularly through nanoformulations of phytoconstituents,enhancing their therapeutic potential.Despite their broad bioactivities,plant-based compounds often suffer from poor bioavailability and stability.Nanoformulations address these limitations by improving solubility,targeted delivery,and controlled release.This approach opens new possibilities for treating chronic diseases like cancer,diabetes,and neurodegenerative disorders.This review aims to examine recent advancements in nanotechnology-based formulation strategies designed to enhance the delivery,stability,and therapeutic efficacy of phytochemicals and also discusses regulatory issues,safety concerns,scalability,and cost-effectiveness.Emphasis was placed on nanoformulation techniques employed for key phytoconstituents such as curcumin,resveratrol,epigallocatechin gallate,and quercetin.The most commonly employed nanocarriers included polymeric nanoparticles,solid lipid nanoparticles,and liposomes.These formulations significantly improved the solubility,stability,and controlled release profiles of phytochemicals.In vitro and in vivo studies demonstrated enhanced anti-inflammatory,anticancer,and antioxidant activities.Moreover,surface-modified and targeted nanoparticles showed promise in increasing site-specific targeting and enhancing bioavailability of the encapsulated compounds.Nanoformulations present a promising strategy for overcoming the pharmacokinetic limitations of phytochemicals.Despite encouraging preclinical results,further studies are needed to address issues related to long-term safety,clinical efficacy,and regulatory approval for successful clinical translation.
文摘Over the last three decades burgeoning research has shown that anxiety disorder comorbidity is not only highly prevalent in bipolar disorder(BD),but it also adversely impacts the course,outcome,and treatment of BD.The present review provides an overview of the current trends in research on comorbid anxiety and BDs based on prior reviews and meta-analyses(n=103),epidemiological surveys,and large-scale clinical studies.The results reiterated the fact that at least half of those with BD are likely to develop an anxiety disorder in their lifetimes and a third of them will manifest an anxiety disorder at any point of time.All types of anxiety disorders were equally common in BD.However,there was a wide variation in rates across different sources,with most of this discrepancy being accounted for by methodological differences between reports.Comorbid anxiety disorders negatively impacted the presentation and course of BD.This unfavourable clinical profile led to poorer outcome and functioning and impeded treatment of BD.Despite the extensive body of research there was paucity of data on aetiology and treatment of anxiety disorder comorbidity in BD.Nevertheless,the substantial burden and unique characteristics of this comorbidity has important clinical and research implications.
文摘BACKGROUND: As per the "Disaster Management Act, 2005" of India, it is mandatory for government hospitals in India to prepare a disaster plan. This study aimed to prepare a disaster manual of a 1 900 bed tertiary care hospital, in consultation and involvement of all concerned stakeholders.METHODS: A committee of members from hospital administration, clinical, diagnostic and supportive departments worked on an initial document prepared according to the Act and gave their inputs to frame a final disaster manual.RESULTS: The prepared departmental standard operating procedures involved 116 people(doctors and paramedical staff), and were then synchronized, in 12 committee meetings, to produce the final hospital disaster manual.CONCLUSIONS: The present disaster manual is one of the few comprehensive plans prepared by the stakeholders of a government hospital in India, who themselves form a part of the disaster response team. It also helped in co-ordinated conduction of mock drills.
文摘Recent studies have revealed that chemerin plays an essential role in the development of cardiovascular diseases.Autopsy studies found a strong correlation between the secretion of chemerin in peripheral tissues and aortic and coronary atherosclerosis.Plasma chemerin is a marker of systemic infl ammation and is associated with metabolic syndrome.Chemerin plays a vital role in vascular infl ammation and atherogenesis.Plasma chemerin levels are increased in patients with dilated cardiomyopathy,and chemerin is associated with left ventricular dysfunction.In this review,we focus on chemerin expression,chemerin processing,its biological function,and its role in the diagnosis of cardiovascular diseases.
文摘Recently,muscular function/dysfunction has gained importance in the maintenance of metabolic homeostasis in cardiovascular diseases.Skeletal muscle plays a vital role in coordinating the activity and metabolism of endocrine organs by secreting many myokines,especially irisin.Irisin is a polypeptide hormone consisting of 112 amino acids secreted into the blood from muscle and adipose tissues.Serum irisin levels are associated with cardiometabolic risk factors such as obesity and insulin resistance as defi ned by homeostatic model assessment.Irisin reduces endothelial damage by inhibiting infl ammation and oxidative stress,thus playing a key role in maintaining endothelial cell function.Unsurprisingly,low irisin levels cause endothelial dysfunction and increase the incidence of atherosclerosis.We aimed to summarize the studies on this issue since we have not found any review in the literature on the role of serum irisin levels in the process of atherosclerosis and other cardiovascular events in cardiovascular diseases.
文摘The endothelium modulates vascular homeostasis owing to a variety of vasoconstrictors and vasodilators.Endothelial dysfunction(ED),characterized by impaired vasodilation,inflammation,and thrombosis,triggers future cardiovascular(CV)diseases.Chronic kidney disease,a state of chronic inflammation caused by oxidative stress,metabolic abnormalities,infection,and uremic toxins damages the endothelium.ED is also associated with a decline in estimated glomerular filtration rate.After kidney transplantation,endothelial functions undergo immediate but partial restoration,promising graft longevity and enhanced CV health.However,the anticipated CV outcomes do not happen due to various transplant-related and unrelated risk factors for ED,culminating in poor CV health and graft survival.ED in kidney transplant recipients is an underrecognized and poorly studied entity.CV diseases are the leading cause of death among kidney transplant candidates with functioning grafts.ED contributes to the pathogenesis of many of the CV diseases.Various biomarkers and vasoreactivity tests are available to study endothelial functions.With an increasing number of transplants happening every year,and improved graft rejection rates due to the availability of effective immunosuppressants,the focus has now shifted to endothelial protection for the prevention,early recognition,and treatment of CV diseases.
文摘Knee osteoarthritis(OA)is a debilitating condition with limited long-term treatment options.The therapeutic potential of mesenchymal stem cells(MSCs),particularly those derived from bone marrow aspirate concentrate,has garnered attention for cartilage repair in OA.While the iliac crest is the traditional site for bone marrow harvesting(BMH),associated morbidity has prompted the exploration of alternative sites such as the proximal tibia,distal femur,and proximal humerus.This paper reviews the impact of different harvesting sites on mesenchymal stem cell(MSC)yield,viability,and regenerative potential,emphasizing their relevance in knee OA treatment.The iliac crest consistently offers the highest MSC yield,but alternative sites within the surgical field of knee procedures offer comparable MSC characteristics with reduced morbidity.The integration of harvesting techniques into existing knee surgeries,such as total knee arthroplasty,provides a less invasive approach while maintaining thera-peutic efficacy.However,variability in MSC yield from these alternative sites underscores the need for further research to standardize techniques and optimize clinical outcomes.Future directions include large-scale comparative studies,advanced characterization of MSCs,and the development of personalized harvesting strategies.Ultimately,the findings suggest that optimizing the site of BMH can significantly influence the quality of MSC-based therapies for knee OA,enhancing their clinical utility and patient outcomes.

文摘Objective: Little is known about the prescription pattern of psychotropic drugs for patients with schizophrenia in Pakistan. The purpose of this study was to evaluate the characteristic features of psychotropic drug prescriptions for patients with schizophrenia in Pakistan. Methods: Three centers in Pakistan participated in a large scale collaborative study known as Research on Asian Prescription Pattern (REAP). The 2016 REAP survey included centers from 15 countries in Asia and used a unified research protocol. The design of the study was quantitative and of descriptive epidemiology. Analysis was made on the data collected from three centers i.e., Lahore, Karachi and Islamabad. The data collected in Pakistan were compared with those from other Asian countries. The details of REAP were presented on the homepage of REAP (http://www.REAP.Asia). Results: From Pakistan, 298 patients were included. Patients with schizophrenia in Pakistan received higher rate of antipsychotic polypharmacy and a higher rate of co-prescription of mood stabilizers and anxiolytics when compared with participants from other South Asian countries (India and Bangladesh). Conclusion: The main findings of the study were that a majority of the patients were prescribed antipsychotic polypharmacy drugs in Pakistan. Insufficient training on the use of psychotropic drugs and a lack of treatment guideline are considered to be the major contributing factors. Further education and training on the proper use of psychotropic drugs are recommended to psychiatrists in Pakistan. The guidelines on pharmacotherapy for patients with schizophrenia should also be developed and promoted in Pakistan.

文摘Several articles on the mental health impact of the metaverse and the need to balance its potential benefits with the risks of metaverse use has recently published.The metaverse consists of a combination of immersive technologies and artificial intelligence algorithms.The metaverse differs from the preceding digital psychiatric interventions due to its complex structure and interactions between components.The diverse functions of the metaverse ensure that it may have a substantial impact on mental health.However,the evidence for its efficacy in treating mental health disorders is limited to a few trials.The mental health benefits of immersive technologies are well-documented and suggest that metaverse-based psychiatric treatment may be similarly efficacious.The mental health risks of the metaverse are largely unknown,and it is not clear whether they will be greater than other digital psychiatric interventions.Much more research is needed to determine whether metaverse-based psychiatric treatment will meet the standards of appropriate mental healthcare.
文摘The gut microbiome comprises a vast community of microbes inhabiting the human alimentary canal,playing a crucial role in various physiological functions.These microbes generally live in harmony with the host;however,when dysbiosis occurs,it can contribute to the pathogenesis of diseases,including osteoporosis.Osteoporosis,a systemic skeletal disease characterized by reduced bone mass and increased fracture risk,has attracted significant research attention concerning the role of gut microbes in its development.Advances in molecular biology have highlighted the influence of gut microbiota on osteoporosis through mechanisms involving immunoregulation,modulation of the gut-brain axis,and regulation of the intestinal barrier and nutrient absorption.These microbes can enhance bone mass by inhibiting osteoclast differentiation,inducing apoptosis,reducing bone resorption,and promoting osteoblast proliferation and maturation.Despite these promising findings,the therapeutic effectiveness of targeting gut microbes in osteoporosis requires further investigation.Notably,gut microbiota has been increasingly studied for their potential in early diagnosis,intervention,and as an adjunct therapy for osteoporosis,suggesting a growing utility in improving bone health.Further research is essential to fully elucidate the therapeutic potential and clinical application of gut microbiome modulation in the management of osteoporosis.
