Psychoeducation is a structured but straightforward and effective treatment,which consists of educating patients and family members about psychiatric disorders,teaching them to cope with the illness,and supporting the...Psychoeducation is a structured but straightforward and effective treatment,which consists of educating patients and family members about psychiatric disorders,teaching them to cope with the illness,and supporting them during treatment.Psychoeducation as an independent treatment is effective in schizophrenia and bipolar disorder.Psychoeducation is a core component of cognitivebehavioural therapy(CBT)or exposure and response prevention(ERP)treatment,which are the standard psychotherapeutic treatments for obsessive-compulsive disorder(OCD).However,research on the benefits of psychoeducational treatments in OCD is limited compared to schizophrenia and bipolar disorders.Very few studies have examined psychoeducation as an independent treatment for OCD,and its efficacy is not proven.The effectiveness of CBT or ERP for OCD is well established,but the contribution of psychoeducation to the efficacy of these treatments is unclear.Recently,there has been a greater understanding of maladaptive family responses,such as accommodation and antagonism,that worsen the outcome and treatment response in OCD.Family-based psychoeducational treatments targeting inappropriate family responses are effective in reducing obsessional symptoms,attenuating the negative family impact of OCD,and improving patient and family functioning.Family-based psychoeducation is a promising treatment for OCD,but there are methodological issues with the evidence,and trials among adult patients are limited compared to children and adolescents.The other obstacles to the implementation of psychoeducational treatments of OCD are the large treatment gap,lack of research from low-and middle-income countries,and the limited attempts to develop culturally adapted treatments in these countries.Thus,further research should focus on optimising psychoeducational treatments for OCD and efficiently delivering these treatments in routine clinical settings.展开更多
BACKGROUND Tuberculosis(TB)is a common infection causing huge morbidity and mortality to mankind.The analytical methods used in diagnosing TB are not sensitive in paucibacillary infections and also require trained tec...BACKGROUND Tuberculosis(TB)is a common infection causing huge morbidity and mortality to mankind.The analytical methods used in diagnosing TB are not sensitive in paucibacillary infections and also require trained technical personnel.MicroRNAs are stable in serum and other body fluids,and hold great potential in the diagnosis of TB.AIM To analyze the dysregulated microRNA profiles among patients with cavitatory and non-cavitatory pulmonary TB.METHODS The prospective study will be conducted in a tertiary care center in India.Adult patients with newly diagnosed pulmonary TB will be included.There will be two groups:Patients with sputum positive pulmonary TB with cavity and without cavity(group1),and apparently healthy individuals(group 2).The participants will undergo sputum examination,Xpert Mycobacterium TB complex/resistance to rifampin(Mtb/RIF)assay,chest X-ray,and blood investigations and serum microRNA detection.Ethics approval has been obtained.Written informed consent will be obtained.Appropriate statistical analyses will be used.RESULTS MicroRNAs will be correlated with sputum positivity,Xpert Mtb/RIF assay,radiological involvement,inflammatory markers,and course of the disease among cases and controls.CONCLUSION MicroRNAs could serve as potential diagnostic biomarkers in diagnostically challenging TB patients.展开更多
BACKGROUND Orthopedic training,one of the most useful but under-represented specialties in undergraduate medical curricula,has some difficulties in clinical teaching.AIM To determine if simulation-based learning(SBL)w...BACKGROUND Orthopedic training,one of the most useful but under-represented specialties in undergraduate medical curricula,has some difficulties in clinical teaching.AIM To determine if simulation-based learning(SBL)was effective in enhancing procedural accuracy,skill confidence,and knowledge recall in final-year medical students.METHODS This was a cross-sectional observational study performed in the Department of Orthopaedics in a tertiary care teaching hospital with simulation training facilities.The trial was conducted over 2 months(January 2025-February 2025)and was designed to determine the effect of SBL on procedural skills,knowledge retention,and self-perceived confidence in a group of final-year medical students.The inclusion criterion was undergraduate medical students in their final year who had clinical postings in orthopedics.Convenience sampling was used to recruit 106 students.The sample size was estimated to achieve a post-training 50%improvement ratio in procedural accuracy,a confidence interval of 95%,and an absolute precision of 10%.The simulation sessions included exercises with synthetic bone models,cast equipment,and procedural packs[orthopedic skills:(1)Closed fracture reduction;(2)Application of plaster;and(3)Traction].Each session consisted of an instructor-led demonstration,practice under supervision,and immediate feedback.RESULTS In our study involving 106 final-year medical students,SBL significantly improved procedural accuracy with scores rising from 62.5%to 84.9%(P<0.001).Knowledge retention also improved markedly with post-test scores increasing from 63.4%to 78.2%(P<0.001).Self-reported confidence levels showed a substantial gain,increasing from 4.6 to 8.2 on a 10-point scale.Prior simulation exposure and academic performance≥75%were significantly associated with higher post-training accuracy.Gender had no significant influence on outcomes.CONCLUSION The current study attested to the value of SBL in the enhancement of procedural skills,knowledge retention,and self-confidence of final-year medical students in orthopedics.展开更多
BACKGROUND Drug utilization research has an important role in assisting the healthcare administration to know,compute,and refine the prescription whose principal objective is to enable the rational use of drugs.Resear...BACKGROUND Drug utilization research has an important role in assisting the healthcare administration to know,compute,and refine the prescription whose principal objective is to enable the rational use of drugs.Research in developing nations relating to the cost of treatment is scarce when compared with developed countries.Thus,the drug utilization research studies from developing nations are most needed,and their number has been growing.AIM To evaluate patterns of utilization of antipsychotic drugs and direct medical cost analysis in patients newly diagnosed with schizophrenia.METHODS The present study was observational in type and based on a retrospective cohort to evaluate patterns of utilization of antipsychotic drugs using World Health Organization(WHO)core prescribing indicators and anatomical therapeutic chemical/defined daily dose indicators.We also calculated direct medical costs for a period of 6 months.RESULTS This study has found that atypical antipsychotics are the mainstay of treatment for schizophrenia in every age group and subcategories of schizophrenia.The evaluation based on WHO prescribing indicators showed a low average number of drugs per prescription and low prescribing frequency of antipsychotics from the National List of Essential Medicines 2015 and the WHO Essential Medicines List 2019.The total mean drug cost of our study was 1396 Indian rupees.The total mean cost due to the investigation in our study was 1017.34 Indian rupees.Therefore,the total mean direct medical cost incurred on patients in our study was 4337.28 Indian rupees.CONCLUSION The information from the present study can be used for reviewing and updating treatment policy at the institutional level.展开更多
Gastric cancer(GC)is the fifth most prevalent malignancy worldwide and remains a leading cause of cancer-related mortality.Major risk factors for GC include Helicobacter pylori infection,increasing age,high dietary sa...Gastric cancer(GC)is the fifth most prevalent malignancy worldwide and remains a leading cause of cancer-related mortality.Major risk factors for GC include Helicobacter pylori infection,increasing age,high dietary salt intake,and diets deficient in vegetables and fruits.Due to the often subtle and nonspecific early symptoms,coupled with the lack of routine screening programs,a significant proportion of GC cases are diagnosed at advanced stages.The etiology of GC is multifactorial,and diagnosis is confirmed histologically through endoscopic biopsy,followed by staging via computed tomography,positron emission tomography,staging laparoscopy,and endoscopic ultrasound.Treatment strategies typically involve a multidisciplinary approach including chemotherapy,surgical resection,radiotherapy,and emerging immunotherapeutic options.Despite advances in diagnostic and therapeutic modalities,the prognosis of advanced GC remains poor,with high rates of recurrence and metastasis.In recent years,increasing attention has been given to the role of tight junction(TJ)proteins in the pathogenesis and progression of GC.TJ proteins,critical components of epithelial barrier function,have been implicated in various stages of gastric carcinogenesis,from intestinal metaplasia to invasion and metastasis.Infection and inflammation,particularly due to Helicobacter pylori,disrupt TJ integrity,compromising the gastric mucosal barrier and facilitating neoplastic transformation.This review synthesizes current evidence from PubMed,EMBASE,Google Scholar,ScienceDirect,SpringerLink,and other reputable databases to provide a comprehensive overview of the involvement of TJ proteins in GC.By elucidating the molecular interplay between TJ dysregulation and gastric tumorigenesis,this work aims to highlight the potential of TJ proteins as novel diagnostic biomarkers and therapeutic targets in GC management.展开更多
Rationale:This case report describes a couple with recurrent fertilization failure despite undergoing multiple cycles of intracytoplasmic sperm injection(ICSI).The principal clinical concern was suspected oocyte activ...Rationale:This case report describes a couple with recurrent fertilization failure despite undergoing multiple cycles of intracytoplasmic sperm injection(ICSI).The principal clinical concern was suspected oocyte activation deficiency(OAD),in which fertilization is impeded due to the oocyte’s inability to initiate embryogenesis,commonly attributed to inadequate intracellular calcium(Ca^(2+))release following sperm injection.Patient concerns:The couple repeatedly experienced complete or near-complete fertilization failure in previous ICSI cycles,raising suspicion of an underlying oocyte activation defect.Diagnosis:Based on the repeated absence of fertilization post-ICSI and clinical history,a diagnosis of suspected OAD leading to recurrent ICSI fertilization failure was considered.Interventions:Artificial oocyte activation(AOA)using the calcium ionophore A23187 was performed.After ICSI,unfertilized oocytes were exposed to the ionophore to induce Ca^(2+)influx,simulating physiological calcium oscillations essential for oocyte activation.The efficacy of intervention was evaluated through subsequent embryonic development,morphological grading,and chromosomal integrity.Outcomes:Following AOA treatment,successful oocyte activation occurred,resulting in the formation of high-grade embryos with normal developmental progression.Chromosomal analysis revealed no detectable abnormalities,indicating genomic stability.Lessons:Calcium ionophore–mediated AOA may serve as an effective adjunct in cases of recurrent ICSI failure attributed to OAD.This case highlights the importance of individualized therapeutic strategies in assisted reproduction;however,further research is needed to refine protocols,validate broader clinical efficacy,and assess long-term safety,including potential epigenetic risks.展开更多
BACKGROUND Orthobiologics-biological substances like platelet-rich plasma(PRP),bone marrow aspirate concentrate,and stem cells-are increasingly used in musculoskeletal care to promote tissue repair and reduce reliance...BACKGROUND Orthobiologics-biological substances like platelet-rich plasma(PRP),bone marrow aspirate concentrate,and stem cells-are increasingly used in musculoskeletal care to promote tissue repair and reduce reliance on invasive surgery.Despite global momentum,India's clinical adoption remains underexplored.AIM To inform education,policy,and resource allocation for the safe and effective adoption of orthobiologics in musculoskeletal care.METHODS A cross-sectional electronic survey was conducted from January to March 2025 among orthopaedic surgeons,academicians,and trainees across India.The questionnaire assessed demographics,knowledge of orthobiologics,attitudes toward training and subspecialization,usage trends,regulatory awareness,and perceived barriers.Data were analyzed using descriptive statistics andχ2/Fisher’s exact tests,with P<0.05 considered significant.RESULTS A total of 1280 valid responses were collected.Awareness of orthobiologics was high(97%),with PRP being the most familiar and widely used(80%).Formal training was reported by only 31%,though 85%showed interest in structured education,and 68%supported orthobiologics as a subspecialty.Satisfaction with clinical outcomes averaged 6.5±2.3 out of 10 points.Barriers included high treatment cost(64%),poor patient awareness(90%),and limited access to biologics labs(18%).Regulatory understanding was moderate,with academic-affiliated clinicians more informed about stem cell guidelines.CONCLUSION Indian orthopaedic professionals demonstrate strong awareness and optimism toward orthobiologics,but widespread gaps in training,infrastructure,and regulation hinder broader adoption.Strategic investments in education,standardized protocols,and accessible facilities are essential to support safe and evidence-driven integration of regenerative therapies into clinical practice.展开更多
Molecular hydrogen(H2)demonstrates selective antioxidant and anti-inflammatory properties with therapeutic potential across musculoskeletal conditions including osteoarthritis,rheumatoid arthritis,exercise-induced mus...Molecular hydrogen(H2)demonstrates selective antioxidant and anti-inflammatory properties with therapeutic potential across musculoskeletal conditions including osteoarthritis,rheumatoid arthritis,exercise-induced muscle damage,chronic pain syndromes,tendinopathies,and muscle atrophy.This review critically evaluates preclinical and clinical evidence for H2 therapy and identifies research gaps.A comprehensive search of PubMed,EMBASE,and Cochrane Library(up to April 2025)yielded 45 eligible studies:25 preclinical and 20 clinical trials.Preclinical models consistently showed reductions in reactive oxygen species,inflammatory cytokines,and improved cell viability.Clinical trials reported symptomatic relief in osteoarthritis,decreased Disease Activity Score 28 in rheumatoid arthritis,and accelerated clearance of muscle damage markers.Delivery methods varied-hydrogen-rich water,gas inhalation,and saline infusion-hindering direct comparison.Mechanistic biomarkers were inconsistently reported,limiting understanding of target engagement.Common limitations included small sample sizes,short durations,and protocol heterogeneity.Despite these constraints,findings suggest H2 may serve as a promising adjunctive therapy via antioxidant,anti-inflammatory,and cytoprotective mechanisms.Future research should prioritize standardized delivery protocols,robust mechanistic endpoints,and longer-term randomized trials to validate clinical efficacy and optimize therapeutic strategies.展开更多

In the competitive retail industry of the digital era,data-driven insights into gender-specific customer behavior are essential.They support the optimization of store performance,layout design,product placement,and ta...In the competitive retail industry of the digital era,data-driven insights into gender-specific customer behavior are essential.They support the optimization of store performance,layout design,product placement,and targeted marketing.However,existing computer vision solutions often rely on facial recognition to gather such insights,raising significant privacy and ethical concerns.To address these issues,this paper presents a privacypreserving customer analytics system through two key strategies.First,we deploy a deep learning framework using YOLOv9s,trained on the RCA-TVGender dataset.Cameras are positioned perpendicular to observation areas to reduce facial visibility while maintaining accurate gender classification.Second,we apply AES-128 encryption to customer position data,ensuring secure access and regulatory compliance.Our system achieved overall performance,with 81.5%mAP@50,77.7%precision,and 75.7%recall.Moreover,a 90-min observational study confirmed the system’s ability to generate privacy-protected heatmaps revealing distinct behavioral patterns between male and female customers.For instance,women spent more time in certain areas and showed interest in different products.These results confirm the system’s effectiveness in enabling personalized layout and marketing strategies without compromising privacy.展开更多
文摘Artificial intelligence(AI)tools,like OpenAI's Chat Generative Pre-trained Transformer(ChatGPT),hold considerable potential in healthcare,academia,and diverse industries.Evidence demonstrates its capability at a medical student level in standardized tests,suggesting utility in medical education,radiology reporting,genetics research,data optimization,and drafting repetitive texts such as discharge summaries.Nevertheless,these tools should augment,not supplant,human expertise.Despite promising applications,ChatGPT confronts limitations,including critical thinking tasks and generating false references,necessitating stringent cross-verification.Ensuing concerns,such as potential misuse,bias,blind trust,and privacy,underscore the need for transparency,accountability,and clear policies.Evaluations of AI-generated content and preservation of academic integrity are critical.With responsible use,AI can significantly improve healthcare,academia,and industry without compromising integrity and research quality.For effective and ethical AI deployment,collaboration amongst AI developers,researchers,educators,and policymakers is vital.The development of domain-specific tools,guidelines,regulations,and the facilitation of public dialogue must underpin these endeavors to responsibly harness AI's potential.

文摘The China–Sri Lanka Joint Center for Education and Research for the 21 st Century Maritime Silk Road(CSL-CER) was established in August 2015. The Joint Center is an overseas science and education platform established by Chinese Academy of Sciences. CSL-CER has conducted a series of research activities, including an annual scientific expedition to the eastern Indian Ocean, with many kinds of instruments installed for monsoon and ocean observation. The ‘Numerical Forecast System for the Surrounding Area of Sri Lanka,' which was developed by the Joint Center, can provide rapid mapping on multiple scales and at multiple resolutions for numerical forecast products serving local society. The annual China–Sri Lanka Joint Workshop on Monsoon Climate and Ocean Environment was successfully held in 2015 and 2016. According to the Memorandum of Understanding(MOU) on joint Master's degrees, 15 new Master's students were accepted by University of Chinese Academy of Sciences(UCAS) in 2015.

文摘Objective of the survey: to understand the status quo of special education students' independent learning ability, in order to train their professional independent learning ability to take measures to provide practical basis;Respondents: 310 students in grade 1-4 majoring in special education in Xinjiang and abroad;Questionnaire survey design: option takes the form of Likert 5 subscales, classification, research contents include the learner's goal setting and self-efficacy, goal orientation, prepare to review, knowledge, understanding, difficult point to grasp, learning arrangements, supervision, learning initiative, environmental management, time management, cooperative learning, the resources use 13 dimensions. The results show that the students' autonomous learning ability of special education major is moderately positive on the whole. There was no difference in the independent learning ability of the special education students of different genders. There are significant differences in student origin place, family environment, class leader or not, location of university and grade.

文摘Pump valve pipeline vibration brings serious safety hazards to the operation of the equipment,for the pump valve system in the process of variable flow,variable speed,variable openings lead to excessive pipeline vibration.An active damping device(ADD)is used to the vibration of the pump valve pipeline system to apply the control force,to achieve the active control of the pipeline vibration.A pump-valve pipeline vibration test bench was built to compare the control effect of active damping device on pipeline vibration under different pump valve working conditions,and the results show that applying ADD control could effectively suppress the vibration of the pump valve pipeline and enhance the stability of the equipment during operation.At different pump operating rotation frequencies,the vibration amplitude of the pump valve pipeline in working frequency and its multiple frequencies are also effectively suppressed,with the maximum amplitude reduction of more than 60%.For the valve vibration caused by different operating openings,the vibration of the highest reduction of 68%,and the centrifugal pump drive shaft vi-bration reduced by up to 73%,which provides a new idea for vibration control of pump valve pipeline system.
文摘Psychoeducation is a structured but straightforward and effective treatment,which consists of educating patients and family members about psychiatric disorders,teaching them to cope with the illness,and supporting them during treatment.Psychoeducation as an independent treatment is effective in schizophrenia and bipolar disorder.Psychoeducation is a core component of cognitivebehavioural therapy(CBT)or exposure and response prevention(ERP)treatment,which are the standard psychotherapeutic treatments for obsessive-compulsive disorder(OCD).However,research on the benefits of psychoeducational treatments in OCD is limited compared to schizophrenia and bipolar disorders.Very few studies have examined psychoeducation as an independent treatment for OCD,and its efficacy is not proven.The effectiveness of CBT or ERP for OCD is well established,but the contribution of psychoeducation to the efficacy of these treatments is unclear.Recently,there has been a greater understanding of maladaptive family responses,such as accommodation and antagonism,that worsen the outcome and treatment response in OCD.Family-based psychoeducational treatments targeting inappropriate family responses are effective in reducing obsessional symptoms,attenuating the negative family impact of OCD,and improving patient and family functioning.Family-based psychoeducation is a promising treatment for OCD,but there are methodological issues with the evidence,and trials among adult patients are limited compared to children and adolescents.The other obstacles to the implementation of psychoeducational treatments of OCD are the large treatment gap,lack of research from low-and middle-income countries,and the limited attempts to develop culturally adapted treatments in these countries.Thus,further research should focus on optimising psychoeducational treatments for OCD and efficiently delivering these treatments in routine clinical settings.
文摘BACKGROUND Tuberculosis(TB)is a common infection causing huge morbidity and mortality to mankind.The analytical methods used in diagnosing TB are not sensitive in paucibacillary infections and also require trained technical personnel.MicroRNAs are stable in serum and other body fluids,and hold great potential in the diagnosis of TB.AIM To analyze the dysregulated microRNA profiles among patients with cavitatory and non-cavitatory pulmonary TB.METHODS The prospective study will be conducted in a tertiary care center in India.Adult patients with newly diagnosed pulmonary TB will be included.There will be two groups:Patients with sputum positive pulmonary TB with cavity and without cavity(group1),and apparently healthy individuals(group 2).The participants will undergo sputum examination,Xpert Mycobacterium TB complex/resistance to rifampin(Mtb/RIF)assay,chest X-ray,and blood investigations and serum microRNA detection.Ethics approval has been obtained.Written informed consent will be obtained.Appropriate statistical analyses will be used.RESULTS MicroRNAs will be correlated with sputum positivity,Xpert Mtb/RIF assay,radiological involvement,inflammatory markers,and course of the disease among cases and controls.CONCLUSION MicroRNAs could serve as potential diagnostic biomarkers in diagnostically challenging TB patients.
文摘BACKGROUND Orthopedic training,one of the most useful but under-represented specialties in undergraduate medical curricula,has some difficulties in clinical teaching.AIM To determine if simulation-based learning(SBL)was effective in enhancing procedural accuracy,skill confidence,and knowledge recall in final-year medical students.METHODS This was a cross-sectional observational study performed in the Department of Orthopaedics in a tertiary care teaching hospital with simulation training facilities.The trial was conducted over 2 months(January 2025-February 2025)and was designed to determine the effect of SBL on procedural skills,knowledge retention,and self-perceived confidence in a group of final-year medical students.The inclusion criterion was undergraduate medical students in their final year who had clinical postings in orthopedics.Convenience sampling was used to recruit 106 students.The sample size was estimated to achieve a post-training 50%improvement ratio in procedural accuracy,a confidence interval of 95%,and an absolute precision of 10%.The simulation sessions included exercises with synthetic bone models,cast equipment,and procedural packs[orthopedic skills:(1)Closed fracture reduction;(2)Application of plaster;and(3)Traction].Each session consisted of an instructor-led demonstration,practice under supervision,and immediate feedback.RESULTS In our study involving 106 final-year medical students,SBL significantly improved procedural accuracy with scores rising from 62.5%to 84.9%(P<0.001).Knowledge retention also improved markedly with post-test scores increasing from 63.4%to 78.2%(P<0.001).Self-reported confidence levels showed a substantial gain,increasing from 4.6 to 8.2 on a 10-point scale.Prior simulation exposure and academic performance≥75%were significantly associated with higher post-training accuracy.Gender had no significant influence on outcomes.CONCLUSION The current study attested to the value of SBL in the enhancement of procedural skills,knowledge retention,and self-confidence of final-year medical students in orthopedics.
文摘BACKGROUND Drug utilization research has an important role in assisting the healthcare administration to know,compute,and refine the prescription whose principal objective is to enable the rational use of drugs.Research in developing nations relating to the cost of treatment is scarce when compared with developed countries.Thus,the drug utilization research studies from developing nations are most needed,and their number has been growing.AIM To evaluate patterns of utilization of antipsychotic drugs and direct medical cost analysis in patients newly diagnosed with schizophrenia.METHODS The present study was observational in type and based on a retrospective cohort to evaluate patterns of utilization of antipsychotic drugs using World Health Organization(WHO)core prescribing indicators and anatomical therapeutic chemical/defined daily dose indicators.We also calculated direct medical costs for a period of 6 months.RESULTS This study has found that atypical antipsychotics are the mainstay of treatment for schizophrenia in every age group and subcategories of schizophrenia.The evaluation based on WHO prescribing indicators showed a low average number of drugs per prescription and low prescribing frequency of antipsychotics from the National List of Essential Medicines 2015 and the WHO Essential Medicines List 2019.The total mean drug cost of our study was 1396 Indian rupees.The total mean cost due to the investigation in our study was 1017.34 Indian rupees.Therefore,the total mean direct medical cost incurred on patients in our study was 4337.28 Indian rupees.CONCLUSION The information from the present study can be used for reviewing and updating treatment policy at the institutional level.
文摘Gastric cancer(GC)is the fifth most prevalent malignancy worldwide and remains a leading cause of cancer-related mortality.Major risk factors for GC include Helicobacter pylori infection,increasing age,high dietary salt intake,and diets deficient in vegetables and fruits.Due to the often subtle and nonspecific early symptoms,coupled with the lack of routine screening programs,a significant proportion of GC cases are diagnosed at advanced stages.The etiology of GC is multifactorial,and diagnosis is confirmed histologically through endoscopic biopsy,followed by staging via computed tomography,positron emission tomography,staging laparoscopy,and endoscopic ultrasound.Treatment strategies typically involve a multidisciplinary approach including chemotherapy,surgical resection,radiotherapy,and emerging immunotherapeutic options.Despite advances in diagnostic and therapeutic modalities,the prognosis of advanced GC remains poor,with high rates of recurrence and metastasis.In recent years,increasing attention has been given to the role of tight junction(TJ)proteins in the pathogenesis and progression of GC.TJ proteins,critical components of epithelial barrier function,have been implicated in various stages of gastric carcinogenesis,from intestinal metaplasia to invasion and metastasis.Infection and inflammation,particularly due to Helicobacter pylori,disrupt TJ integrity,compromising the gastric mucosal barrier and facilitating neoplastic transformation.This review synthesizes current evidence from PubMed,EMBASE,Google Scholar,ScienceDirect,SpringerLink,and other reputable databases to provide a comprehensive overview of the involvement of TJ proteins in GC.By elucidating the molecular interplay between TJ dysregulation and gastric tumorigenesis,this work aims to highlight the potential of TJ proteins as novel diagnostic biomarkers and therapeutic targets in GC management.
文摘Rationale:This case report describes a couple with recurrent fertilization failure despite undergoing multiple cycles of intracytoplasmic sperm injection(ICSI).The principal clinical concern was suspected oocyte activation deficiency(OAD),in which fertilization is impeded due to the oocyte’s inability to initiate embryogenesis,commonly attributed to inadequate intracellular calcium(Ca^(2+))release following sperm injection.Patient concerns:The couple repeatedly experienced complete or near-complete fertilization failure in previous ICSI cycles,raising suspicion of an underlying oocyte activation defect.Diagnosis:Based on the repeated absence of fertilization post-ICSI and clinical history,a diagnosis of suspected OAD leading to recurrent ICSI fertilization failure was considered.Interventions:Artificial oocyte activation(AOA)using the calcium ionophore A23187 was performed.After ICSI,unfertilized oocytes were exposed to the ionophore to induce Ca^(2+)influx,simulating physiological calcium oscillations essential for oocyte activation.The efficacy of intervention was evaluated through subsequent embryonic development,morphological grading,and chromosomal integrity.Outcomes:Following AOA treatment,successful oocyte activation occurred,resulting in the formation of high-grade embryos with normal developmental progression.Chromosomal analysis revealed no detectable abnormalities,indicating genomic stability.Lessons:Calcium ionophore–mediated AOA may serve as an effective adjunct in cases of recurrent ICSI failure attributed to OAD.This case highlights the importance of individualized therapeutic strategies in assisted reproduction;however,further research is needed to refine protocols,validate broader clinical efficacy,and assess long-term safety,including potential epigenetic risks.
文摘BACKGROUND Orthobiologics-biological substances like platelet-rich plasma(PRP),bone marrow aspirate concentrate,and stem cells-are increasingly used in musculoskeletal care to promote tissue repair and reduce reliance on invasive surgery.Despite global momentum,India's clinical adoption remains underexplored.AIM To inform education,policy,and resource allocation for the safe and effective adoption of orthobiologics in musculoskeletal care.METHODS A cross-sectional electronic survey was conducted from January to March 2025 among orthopaedic surgeons,academicians,and trainees across India.The questionnaire assessed demographics,knowledge of orthobiologics,attitudes toward training and subspecialization,usage trends,regulatory awareness,and perceived barriers.Data were analyzed using descriptive statistics andχ2/Fisher’s exact tests,with P<0.05 considered significant.RESULTS A total of 1280 valid responses were collected.Awareness of orthobiologics was high(97%),with PRP being the most familiar and widely used(80%).Formal training was reported by only 31%,though 85%showed interest in structured education,and 68%supported orthobiologics as a subspecialty.Satisfaction with clinical outcomes averaged 6.5±2.3 out of 10 points.Barriers included high treatment cost(64%),poor patient awareness(90%),and limited access to biologics labs(18%).Regulatory understanding was moderate,with academic-affiliated clinicians more informed about stem cell guidelines.CONCLUSION Indian orthopaedic professionals demonstrate strong awareness and optimism toward orthobiologics,but widespread gaps in training,infrastructure,and regulation hinder broader adoption.Strategic investments in education,standardized protocols,and accessible facilities are essential to support safe and evidence-driven integration of regenerative therapies into clinical practice.
文摘Molecular hydrogen(H2)demonstrates selective antioxidant and anti-inflammatory properties with therapeutic potential across musculoskeletal conditions including osteoarthritis,rheumatoid arthritis,exercise-induced muscle damage,chronic pain syndromes,tendinopathies,and muscle atrophy.This review critically evaluates preclinical and clinical evidence for H2 therapy and identifies research gaps.A comprehensive search of PubMed,EMBASE,and Cochrane Library(up to April 2025)yielded 45 eligible studies:25 preclinical and 20 clinical trials.Preclinical models consistently showed reductions in reactive oxygen species,inflammatory cytokines,and improved cell viability.Clinical trials reported symptomatic relief in osteoarthritis,decreased Disease Activity Score 28 in rheumatoid arthritis,and accelerated clearance of muscle damage markers.Delivery methods varied-hydrogen-rich water,gas inhalation,and saline infusion-hindering direct comparison.Mechanistic biomarkers were inconsistently reported,limiting understanding of target engagement.Common limitations included small sample sizes,short durations,and protocol heterogeneity.Despite these constraints,findings suggest H2 may serve as a promising adjunctive therapy via antioxidant,anti-inflammatory,and cytoprotective mechanisms.Future research should prioritize standardized delivery protocols,robust mechanistic endpoints,and longer-term randomized trials to validate clinical efficacy and optimize therapeutic strategies.
文摘Musculoskeletal injuries are among the most common causes of disability worldwide,with early detection and appropriate intervention critical to minimizing long-term complications.Infrared thermography(IRT)has emerged as a noninvasive,real-time imaging modality that captures superficial temperature changes reflecting underlying physiological processes such as inflammation and vascular alterations.This review explores the fundamental principles of medical thermography,differentiates between passive and active approaches,and outlines key technological advancements including artificial intelligence integration.The clinical utility of IRT is discussed in various contexts–ranging from acute soft tissue injuries and overuse syndromes to chronic pain and rehabilitation monitoring.Comparative insights with conventional imaging techniques such as ultrasound and magnetic resonance imaging are also presented.While IRT offers functional imaging capabilities with advantages in portability,safety,and speed,its limitations–such as lack of deep-tissue penetration and protocol standardization–remain significant barriers to broader adoption.Future directions include the integration of IRT with other imaging modalities and digital health platforms to enhance musculoskeletal assessment and injury prevention strategies.
文摘Nanotechnology has revolutionized drug delivery,particularly through nanoformulations of phytoconstituents,enhancing their therapeutic potential.Despite their broad bioactivities,plant-based compounds often suffer from poor bioavailability and stability.Nanoformulations address these limitations by improving solubility,targeted delivery,and controlled release.This approach opens new possibilities for treating chronic diseases like cancer,diabetes,and neurodegenerative disorders.This review aims to examine recent advancements in nanotechnology-based formulation strategies designed to enhance the delivery,stability,and therapeutic efficacy of phytochemicals and also discusses regulatory issues,safety concerns,scalability,and cost-effectiveness.Emphasis was placed on nanoformulation techniques employed for key phytoconstituents such as curcumin,resveratrol,epigallocatechin gallate,and quercetin.The most commonly employed nanocarriers included polymeric nanoparticles,solid lipid nanoparticles,and liposomes.These formulations significantly improved the solubility,stability,and controlled release profiles of phytochemicals.In vitro and in vivo studies demonstrated enhanced anti-inflammatory,anticancer,and antioxidant activities.Moreover,surface-modified and targeted nanoparticles showed promise in increasing site-specific targeting and enhancing bioavailability of the encapsulated compounds.Nanoformulations present a promising strategy for overcoming the pharmacokinetic limitations of phytochemicals.Despite encouraging preclinical results,further studies are needed to address issues related to long-term safety,clinical efficacy,and regulatory approval for successful clinical translation.
文摘In the competitive retail industry of the digital era,data-driven insights into gender-specific customer behavior are essential.They support the optimization of store performance,layout design,product placement,and targeted marketing.However,existing computer vision solutions often rely on facial recognition to gather such insights,raising significant privacy and ethical concerns.To address these issues,this paper presents a privacypreserving customer analytics system through two key strategies.First,we deploy a deep learning framework using YOLOv9s,trained on the RCA-TVGender dataset.Cameras are positioned perpendicular to observation areas to reduce facial visibility while maintaining accurate gender classification.Second,we apply AES-128 encryption to customer position data,ensuring secure access and regulatory compliance.Our system achieved overall performance,with 81.5%mAP@50,77.7%precision,and 75.7%recall.Moreover,a 90-min observational study confirmed the system’s ability to generate privacy-protected heatmaps revealing distinct behavioral patterns between male and female customers.For instance,women spent more time in certain areas and showed interest in different products.These results confirm the system’s effectiveness in enabling personalized layout and marketing strategies without compromising privacy.
