Background Examining the health outcomes of veterans who have completed the United States Veterans Health Administration’s(VHA’s)Traumatic Brain Injury(TBI)Screening and Evaluation Program may aid in the refinement ...Background Examining the health outcomes of veterans who have completed the United States Veterans Health Administration’s(VHA’s)Traumatic Brain Injury(TBI)Screening and Evaluation Program may aid in the refinement and improvement of clinical care initiatives within the VHA.This study compared self-reported physical functioning,cardiometabolic health conditions,and health care utilization patterns in Million Veteran Program enrollees with TBI Screening and Evaluation Program data(collected between 2007 and 2019),with the goal of enhancing understanding of potentially modifiable health conditions in this population.Methods In this observational cohort study,veterans(n=16,452)were grouped based on the diagnostic outcome of the TBI Screening and Evaluation Program:1)negative TBI screen(Screen^(-));2)positive TBI screen but no confirmed TBI diagnosis[Screen^(+)/Comprehensive TBI Evaluation(CTBIE)^(-)];or 3)positive TBI screen and confirmed TBI diagnosis(Screen^(+)/CTBIE^(+)).Chi-square tests and analysis of covariance were used to explore group differences in physical functioning,cardiometabolic health conditions,and health care utilization patterns,and logistic regressions were used to examine predictors of Screen^(+/-)and CTBIE^(+/-)group status.Results The results showed that veterans in the Screen^(+)/CTBIE^(-)and Screen^(+)/CTBIE^(+)groups generally reported poorer levels of physical functioning(P’s<0.001,np2=0.02 to 0.03),higher rates of cardiometabolic health conditions(P’s<0.001,φ=0.14 to 0.52),and increased health care utilization(P’s<0.001,φ=0.14 to>0.5)compared with the Screen-group;however,health outcomes were generally comparable between the Screen^(+)/CTBIE^(-)and Screen^(+)/CTBIE^(+)groups.Follow-up analyses confirmed that while physical functioning,hypertension,stroke,healthcare utilization,and prescription medication use reliably distinguished between the Screen-and Screen^(+)groups(P’s<0.02,OR’s 0.78 to 3.38),only physical functioning distinguished between the Screen^(+)/CTBIE^(-)and Screen^(+)/CTBIE^(+)groups(P<0.001,OR 0.99).Conclusions The findings suggest that veterans who screen positive for TBI,regardless of whether they are ultimately diagnosed with TBI,are at greater risk for negative health outcomes,signifying that these veterans represent a vulnerable group that may benefit from increased clinical care and prevention efforts.展开更多

Objectives:To evaluate the occurrence of depression and its related factors among public health nurses(PHNs)engaged in child neglect-related abuse cases.Methods:A cross-sectional study of PHNs involved in maternal and...Objectives:To evaluate the occurrence of depression and its related factors among public health nurses(PHNs)engaged in child neglect-related abuse cases.Methods:A cross-sectional study of PHNs involved in maternal and child health or child and family welfare,and engaged in cases of child neglect-related abuse was conducted from January to March 2023 in all municipalities of 13 prefectures selected from Japan's seven regions,considering geographic diversity and major urban areas.The Patient Health Questionnaire-2 was used to screen for depression.Statistical analyses included the Chi-square test,Mann-Whitney U test,and logistic regression analysis.Results:A total of 359 provided valid responses.Among them,11.4%screened positive for depression.Significant individual factors related depression were management-stage nurses(OR=6.27,95%CI:1.63-24.18),no disability welfare experience(OR=3.49,95%CI:1.05-11.55),being assignment of highrisk cases within the past year(OR=8.28,95%CI:2.07-33.08),and lower work control(OR=0.53,95%CI:0.32-0.86).Workplace environment factors were lower perceptions and relationships within the workplace(OR=0.43,95%CI:0.29-0.65)and perceiving manpower as not secured(OR=9.07,95%CI:1.17-70.40),Conclusions:Mental health measures for PHNs should include workload management and reduced burdens through workforce enhancement,attention to burdens on managerial PHNs,promoting communication and mutual support,and opportunities to engage in disability welfare work.展开更多

BACKGROUND Insulin resistance,lipotoxicity,and mitochondrial dysfunction contribute to the pathogenesis of metabolic dysfunction-associated steatotic liver disease(MASLD).Mitochondrial dysfunction impairs oxidative ph...BACKGROUND Insulin resistance,lipotoxicity,and mitochondrial dysfunction contribute to the pathogenesis of metabolic dysfunction-associated steatotic liver disease(MASLD).Mitochondrial dysfunction impairs oxidative phosphorylation and increases reactive oxygen species production,leading to steatohepatitis and hepatic fibrosis.Artificial intelligence(AI)is a potent tool for disease diagnosis and risk stratification.AIM To investigate mitochondrial DNA polymorphisms in susceptibility to MASLD and establish an AI model for MASLD screening.METHODS Multiplex polymerase chain reaction was performed to comprehensively genotype 82 mitochondrial DNA variants in the screening dataset(n=264).The significant mitochondrial single nucleotide polymorphism was validated in an independent cohort(n=1046)using the Taqman®allelic discrimination assay.Random forest,eXtreme gradient boosting,Naive Bayes,and logistic regression algorithms were employed to construct an AI model for MASLD.RESULTS In the screening dataset,only mt12361A>G was significantly associated with MASLD.mt12361A>G showed borderline significance in MASLD patients with 2-3 cardiometabolic traits compared with controls in the validation dataset(P=0.055).Multivariate regression analysis confirmed that mt12361A>G was an independent risk factor of MASLD[odds ratio(OR)=2.54,95%confidence interval(CI):1.19-5.43,P=0.016].The genetic effect of mt12361A>G was significant in the non-diabetic group but not in the diabetic group.mt12361G carriers had a 2.8-fold higher risk than A carriers in the non-diabetic group(OR=2.80,95%CI:1.22-6.41,P=0.015).By integrating clinical features and mt12361A>G,random forest outperformed other algorithms in detecting MASLD[training area under the receiver operating characteristic curve(AUROC)=1.000,validation AUROC=0.876].CONCLUSION The mt12361A>G variant increased the severity of MASLD in non-diabetic patients.AI supports the screening and management of MASLD in primary care settings.展开更多
Objective:This study aimed to evaluate the effectiveness of an integrative educational intervention-combining hospital-based schooling programs,child-centered communication,and therapeutic play-on clinical,psychosocia...Objective:This study aimed to evaluate the effectiveness of an integrative educational intervention-combining hospital-based schooling programs,child-centered communication,and therapeutic play-on clinical,psychosocial,and educational outcomes among school-aged children with cancer.Materials and Methods:A quasi-experimental pre-and post-test control group design was conducted at the pediatric oncology unit of Rumah Sakit Islam Jemursari,Surabaya,Indonesia.Fifty-two hospitalized children aged 6-18 years were randomly assigned to an intervention group(n=26)or a control group(n=26).The intervention lasted 1 month and integrated hospital-based lessons aligned with the national curriculum,therapeutic play,and developmentally appropriate communication by trained staff.Outcomes included pain intensity(Wong-Baker FACES Scale),treatment adherence,quality of life(Pediatric Cancer Quality of Life-32[PCQL-32]),anxiety(Revised Children’s Manifest Anxiety Scale),and literacy-numeracy performance.Data were analyzed using paired t-tests,ANCOVA,and Cohen’s d,with significance set at P<0.05.Results:Compared with controls,the intervention group showed significant improvements in pain reduction(P<0.001),treatment adherence(P<0.001),and quality of life across all PCQL-32 domains(P<0.001).Anxiety levels decreased significantly(P<0.001),and academic performance in literacy and numeracy improved markedly(P<0.001).Conclusion:The integrative educational intervention effectively enhanced clinical recovery,psychosocial well-being,and academic continuity among hospitalized children with cancer,emphasizing the need to integrate hospital-based education and play-based communication into pediatric oncology care.展开更多
Objective:To predict parents'willingness to pay for dengue fever vaccination through the perception of dengue vaccine based on the Health Belief Model(HBM).Methods:A multicenter crosssectional study was conducted ...Objective:To predict parents'willingness to pay for dengue fever vaccination through the perception of dengue vaccine based on the Health Belief Model(HBM).Methods:A multicenter crosssectional study was conducted on parents who had children aged≤5 years and were participating in integrated health service post activities in 3 districts/cities with the highest incidence of dengue fever in Southeast Sulawesi.The instruments used to collect data in this study consisted of a participant characteristics questionnaire,a dengue vaccine perception questionnaire based on the HBM,and a willingness to pay for dengue vaccine questionnaire.Chi-square test and multivariate logistic regression analysis were used to determine the correlation between the perceived HBM subscales and willingness to pay for dengue vaccination.Results:A total of 310 people participated in this study with an average age of(29.9±5.6)years.Most of them were females(284/310,91.7%),80.9%(251/310)had a high education status and 87.4%(271/310)had direct experience with DF.The results showed good perceptions including Perceived Susceptibility(adjusted OR 3.7,95%CI 2.7-6.3,P<0.001),Perceived Severity(adjusted OR 2.8,95%CI 1.7-5.5,P<0.001),Perceived Benefits(adjusted OR 2.8,95%CI 1.7-4.5,P<0.001),and Cues to Action(adjusted OR 3.1,95%CI 1.9-5.1,P<0.001)were positively correlated with willingness to pay for the dengue vaccine comparing with poor perceptions,whereas Perceived Barriers showed no significant effect on willingness to pay for the dengue vaccine.Conclusions:These results suggest that a person's willingness to pay for dengue vaccine increases with their understanding of the risks and advantages of immunization as well as encouraging outside incentives.As a result,these elements are crucial in raising community involvement in the immunization campaign.展开更多
As the world’s largest palm oil producer,Indonesia significantly benefits from its palm oil industry but also faces serious environmental challenges from palm oil mill effluent(POME)-a high-strength wastewater contai...As the world’s largest palm oil producer,Indonesia significantly benefits from its palm oil industry but also faces serious environmental challenges from palm oil mill effluent(POME)-a high-strength wastewater containing substantial organic matter,nutrients,suspended solids,and various chemical compounds.Sustainable and effective wastewater treatment strategies are urgently needed to address this issue.This review presents a comprehensive analysis of existing POME treatment technologies,including anaerobic digestion(AD),advanced oxidation processes(AOPs),membrane filtration,adsorption,phytoremediation,and microalgae-based systems.Each method is examined in terms of treatment efficiency,operational feasibility,and potential for imple-mentation under Indonesian conditions.While advanced processes,such as AOPs and membrane filtration,achieve high pollutant removal,they are often limited by operational costs.In contrast,biological approaches,such as AD and phytoremediation,offer both environmental benefits and economic value through the recovery of biogas,biofertilizers,and biomass.This review highlights the potential for integrating wastewater purification with resource recovery and valorization,supporting a shift toward more circular and sustainable management of POME.The insights provided are intended to guide future research,inform policy decisions,and facilitate the industrial adoption of optimized treatment systems.展开更多
Aceh in Indonesia is rich inmarine resources and abundant fishery products such as oyster.Traditionally,fishermen only harvest oysters and discard the shells,which can cause pollution and environmental contamination.W...Aceh in Indonesia is rich inmarine resources and abundant fishery products such as oyster.Traditionally,fishermen only harvest oysters and discard the shells,which can cause pollution and environmental contamination.Waste Oyster Shells(WOS)contain a high percentage of calcium carbonate(CaCO_(3))that experiences thermal decomposition at high temperature,following the reaction CaCO_(3)→CaO+CO_(2)(ΔT=825℃).At temperature>900℃,dead-burned lime is formed,which severely influences CaO reactivity.However,the optimum temperature for producing high CaO content is still uncertain.Therefore,this study aimed to determine the optimum calcination temperature to produce high CaO content,assess initial setting time of WOS paste,and identify the best compressive strength of paste.For the experiment,WOS was used as a partial cement replacement(with a size of 0.075 mm)in paste at a proportion of 5%and calcined at temperature of 700℃,800℃,900℃,and 1000℃.The specimens used were an ebonite ring(dimensions:70 mm bottom diameter,60 mm top diameter,and 40 mm height)and a cube(dimensions:5 cm×5 cm×5 cm).The experiment was conducted following the ASTM(American Society for Testing andMaterials)standards and optimumcompressive strength values were analyzed using ANOVA(Analysis of Variance)and Response Surface Methodology(RSM)through the Design Expert software.The results showed that WOS calcined at 1000℃ increased CaO content by approximately 57.40%.Furthermore,the initial setting time test of 5%WOS paste at 1000℃ showed a more uniform binding performance compared to conventional cement paste,with an initial setting time of 75 min and a penetration depth of 15 mm.In line with the analysis,optimum compressive strength of 71.028 MPa with a desirability value of 0.986 was achieved at 5%cement replacement and calcination temperature of 786.44℃.展开更多
Objective:This study aimed to describe the implementation of advance care planning(ACP)in patients with stroke.Methods:A scoping review was conducted according to the Arksey and O'Malley framework.Accordingly,we s...Objective:This study aimed to describe the implementation of advance care planning(ACP)in patients with stroke.Methods:A scoping review was conducted according to the Arksey and O'Malley framework.Accordingly,we searched for articles published in ScienceDirect,PubMed,Scopus,ProQuest,and Medline.The inclusion criteria were original research on ACP among adult or elderly stroke patients,hospital-or community-based studies,and those published in English or Indonesian.Qualitative analysis was then used to identify keywords,categories,and themes.Results:Among the 1,050 articles identified,only 8 satisfied the inclusion criteria,indicating that limited studies were available for ACP interventions among stroke patients.Four themes were ultimately identified:the strategy implementation of AC,the challenges of ACP implementation,the benefits of ACP,and the factors influencing ACP.Strategy ACP implementation included the ACP method,the communication style,participation role,media used,and ACP documentation.Lack of information about ACP,facilities,incapacitated patients,and reluctance to implement have become challenges to implementation in stroke.The benefits of ACP,particularly the outcomes of the ACP program,included awareness of ACP engagement,patients can express their wishes,and increased advance directive(AD)documentation.The factors influencing ACP included patient condition,knowledge,and appropriate ACP methods.Conclusion:ACP has not been widely used when providing interventions for stroke patients.The implementation of ACP in stroke patients is full of challenges due to various patient conditions.Effective strategies are needed to improve ACP in stroke patients.It is necessary to develop an ACP for stroke patients'method that involves interprofessional collaboration,and studies on AcP in stroke patients need to be further conducted through interprofessional collaboration.展开更多
Background and Objective Patients with glioma experience a high symptom burden and have diverse palliative care needs.However,the assessment scales used in palliative care remain non-standardized and highly heterogene...Background and Objective Patients with glioma experience a high symptom burden and have diverse palliative care needs.However,the assessment scales used in palliative care remain non-standardized and highly heterogeneous.To evaluate the application patterns of the current scales used in palliative care for glioma,we aim to identify gaps and assess the need for disease-specific scales in glioma palliative care.Methods We conducted a systematic search of five databases including PubMed,Web of Science,Medline,EMBASE,and CINAHL for quantitative studies that reported scale-based assessments in glioma palliative care.We extracted data on scale characteristics,domains,frequency,and psychometric properties.Quality assessments were performed using the Cochrane ROB 2.0 and ROBINS-I tools.Results Of the 3,405 records initially identified,72 studies were included.These studies contained 75 distinct scales that were used 193 times.Mood(21.7%),quality of life(24.4%),and supportive care needs(5.2%)assessments were the most frequently assessed items,exceeding half of all scale applications.Among the various assessment dimensions,the Distress Thermometer(DT)was the most frequently used tool for assessing mood,while the Short Form-36 Health Survey Questionnaire(SF-36)was the most frequently used tool for assessing quality of life.The Mini Mental Status Examination(MMSE)was the most common tool for cognitive assessment.Performance status(5.2%)and social support(6.8%)were underrepresented.Only three brain tumor-specific scales were identified.Caregiver-focused scales were limited and predominantly burden-oriented.Conclusions There are significant heterogeneity,domain imbalances,and validation gaps in the current use of assessment scales for patients with glioma receiving palliative care.The scale selected for use should be comprehensive and user-friendly.展开更多
Rubberized concrete is one of the most studied applications of discarded tires and offers a promising approach to developing materials with enhanced properties.The rubberized concrete mixture results in a reduced modu...Rubberized concrete is one of the most studied applications of discarded tires and offers a promising approach to developing materials with enhanced properties.The rubberized concrete mixture results in a reduced modulus of elasticity and a reduced compressive and tensile strength compared to traditional concrete.This study employs finite element simulations to investigate the elastic properties of rubberized mortar(RuM),considering the influence of inclusion stiffness and interfacial debonding.Different homogenization schemes,including Voigt,Reuss,and mean-field approaches,are implemented using DIGIMAT and ANSYS.Furthermore,the influence of the interfacial transition zone(ITZ)between mortar and rubber is analyzed by periodic homogenization.Subsequently,the influence of the ITZ is examined through a linear fracture analysis with the stress intensity factor as a key parameter,using the ANSYS SMART crack growth tool.Finally,a non-linear study in FEniCS is carried out to predict the strength of the composite material through a compression test.Comparisons with high density polyethylene(HDPE)and gravel inclusions show that increasing inclusion stiffness enhances compressive strength far more effectively than simply improving the mortar/rubber bond.Indeed,when the inclusions are much softer than the surrounding matrix,any benefit gained on the elastic modulus or strength from stronger interfacial adhesion becomes almost negligible.This study provide numerical evidence that tailoring the rubber’s intrinsic stiffness—not merely strengthening the rubber/mortar interface—is a decisive factor for improving the mechanical performance of RuM.展开更多
BACKGROUND Unintended pregnancy occurs when an individual or couple conceives without planning or desire,which can potentially affect a child’s physical,mental,and social well-being.This can then lead to long-term so...BACKGROUND Unintended pregnancy occurs when an individual or couple conceives without planning or desire,which can potentially affect a child’s physical,mental,and social well-being.This can then lead to long-term socioeconomic challenges for families and communities.Although its impact on child growth and development is a pressing concern,research remains limited particularly in multicenter settings.AIM To examine the long-term consequences of unintended pregnancy on the critical years of early childhood growth and development.METHODS This analytical observational study employed a case-control design and was conducted in research centers across Indonesia,encompassing those located in Central Java,Lampung,Bali,and West Nusa Tenggara.A total of 700 children aged≤5 years with histories of intended or unintended pregnancies participated.Data collection involved structured interviews and direct anthropometric and developmental assessments.Data analyses were conducted using multivariate statistics and partial least squares structural equation modeling.RESULTS Unintended pregnancy was found to have a statistically significant effect on both child growth(t=8.178;P<0.001)and child development(t=25.688;P<0.001).Key growth problems identified included underweight,undernutrition,abnormal head circumference,and stunting.Developmental challenges prominently associated with unintended pregnancy included behavioral and emotional disorders,autism spectrum disorder,attentiondeficit/hyperactivity disorder,social and motor skill deficits,as well as visual and hearing impairments.CONCLUSION Unintended pregnancy significantly affects child growth and development,underscoring the need for early intervention,quality prenatal care,and strengthened family planning policies.展开更多
Objectives:Professional antigen-presenting cells known as dendritic cells(DCs)assist as a connection between the innate and adaptive components of the immune response.DCs are attractive targets for immunomodulatory dr...Objectives:Professional antigen-presenting cells known as dendritic cells(DCs)assist as a connection between the innate and adaptive components of the immune response.DCs are attractive targets for immunomodulatory drugs because of their crucial function in triggering immunity.This study set out to examine,for the first time,how hibifolin affected mouse bone-marrow derived(BMDCs)dendritic cells,triggered by lipopolysaccharide(LPS)in vitro.Additionally,a mouse model of contact hypersensitivity(CHS)was used to assess its possible therapeutic effects in vivo.Methods:LPS was administered to BMDCs with or without hibifolin.Major Histocompatibility Complex(MHC)class II,cytokine production,and co-stimulatory molecule(CD80,CD86)expression levels were assessed.To evaluate the functional effects on T-cell activation,mixed lymphocyte responses using OVA specific T-cells were conducted.In vivo,immunoregulatory potential of hibifolin was examined using a CHS mouse model sensitized with 2,4-dinitrofluorobenzene(DNFB).Results:Hibifolin significantly reduced the expression of the proinflammatory cytokines TNF-αand IL-6,as well as the costimulatory molecules CD80,CD86,and MHC II induced by LPS,by about 40-50%.These effects were associated with reduced NF-κB and p38-MAPK pathway activity.JNK and ERK phosphorylation levels did not alter significantly.In vitro,BMDCs treated with hibifolin showed a 40%-45%down-regulated capacity to stimulate T-cell propagation and IFN-γrelease.Oral hibifolin treatment in vivo modestly decreased DNFB-induced CHS responses by 30%-40%.Conclusion:Overall,our results offer new insights that by inhibiting NF-κB and p38-MAPK signaling,hibifolin decreases the expression of costimulatory molecules and cytokines,thereby limiting BMDC activation,suppressing T-cell responses,and exerting immunomodulatory effects in the CHS mouse model.展开更多
AIM:To assess expression of matrix metalloproteinases 2(MMP2)and MMP9 in gastric cancer,superficial gastritis and normal mucosa,and to measure metalloproteinase activity.METHODS:MMP2 and MMP9 mRNA expression was deter...AIM:To assess expression of matrix metalloproteinases 2(MMP2)and MMP9 in gastric cancer,superficial gastritis and normal mucosa,and to measure metalloproteinase activity.METHODS:MMP2 and MMP9 mRNA expression was determined by quantitative real-time polymerase chain reaction.Normalization was carried out using three different factors.Proteins were analyzed by quantitative gelatin zymography(qGZ).RESULTS:18S ribosomal RNA(18SRNA)was very highly expressed,while hypoxanthine ribosyltransferase-1(HPRT-1)was moderately expressed.MMP2 was highly expressed,while MMP9 was not detected or lowly expressed in normal tissues,moderately or highly expressed in gastritis and highly expressed in cancer.Relative expression of 18SRNA and HPRT-1 showed no significant differences.Significant differences in MMP2 and MMP9 were found between cancer and normal tissue,but not between gastritis and normal tissue.Absolute quantification of MMP9 echoed this pattern,but differential expression of MMP2 proved conflictive.Analysis by qGZ indicated significant differences between cancer and normal tissue in MMP-2,total MMP-9,250 and 110 kDa bands.CONCLUSION:MMP9 expression is enhanced in gastric cancer compared to normal mucosa;interpretation of differential expression of MMP2 is difficult to establish.展开更多
Previous studies have demonstrated that doublecortin-positive immature neurons exist pre- dominantly in the superficial layer of the cerebral cortex of adult mammals such as guinea pigs, and these neurons exhibit very...Previous studies have demonstrated that doublecortin-positive immature neurons exist pre- dominantly in the superficial layer of the cerebral cortex of adult mammals such as guinea pigs, and these neurons exhibit very weak properties of self-proliferation during adulthood under physiological conditions. To verify whether environmental enrichment has an impact on the proliferation and maturation of these immature neurons in the prefrontal cortex of adult guinea pigs, healthy adult guinea pigs were subjected to short-term environmental enrichment. Animals were allowed to play with various cognitive and physical stimulating objects over a period of 2 weeks, twice per day, for 60 minutes each. Immunofluorescence staining results indicated that the number of doublecortin-positive cells in layer II of the prefrontal cortex was significantly increased after short-term environmental enrichment exposure. In addition, these doublecortin-positive cells co-expressed 5-bromo-2-deoxyuridine (a marker of cell prolifera- tion), c-Fos (a marker of cell viability) and NeuN (a marker of mature neurons). Experimental findings showed that short-term environmental enrichment can induce proliferation, activation and maturation of doublecortin-positive cells in layer II of the prefrontal cortex of adult guinea pigs.展开更多
Objective:To identify the bioactive compounds in catechins isolation and its components from green tea GMB-4 clone.Methods:Green tea GMB-4 clones were extracted with distilled water at 90C.Samples were eluted into the...Objective:To identify the bioactive compounds in catechins isolation and its components from green tea GMB-4 clone.Methods:Green tea GMB-4 clones were extracted with distilled water at 90C.Samples were eluted into the column with 10%ethanol.Subsequently,the column was eluted with95%ethanol and evaporated separately.Green tea extract was identified by thin layer chromatography.Catechins were separated by the stationary phase in column chromatography using polyamide with 10%ethanol eluent and 95%ethanol.The results of isolations were analyzed by high performance liquid chromatographic(HPLC)and LCMS/MS.Analysis of catechins by HPLC was done by external standard.Results:Fraction from 10%ethanol showed that four major peaks at retention time of1.663,2.367,2.950 and 4.890,indicated the presence of four catechins components including catechin,epicatechins,gallocatechin and epigallocatechin.Whereas,fraction from 95%ethanol showed two main peaks at retention time of 5.167 and 9.82,which indicated the presence of epigallocatechin gallate(EGCG)and epicatechin gallate(ECG).EGCG(m/z 459),epigallocatechin(m/z 307),ECG(m/z 443),and epicatechin(m/z 291)were isolated and separated successfully using HPLC and LC-MS/MS.Conclusions:The HPLC and LC-MS/MS methods were successfully tuned for the qualitative analysis of green tea extract with EGCG and ECG.Four major catechins were separated and identified by LC-MS/MS,such as EGCG,epigallocatechin,ECG and epicatechin.The result of HPLC analysis showed that EGCG and ECG were main components from catechins isolation of green tea GMB-4 clone.展开更多

After the administration of each agent, we subjected the rat livers to total ischemia followed by reperfusion. After reperfusion, we performed the following investigations: alanine aminotransferase(ALT), histological findings, Td T-mediated d UTP-biotin nick end labeling(TUNEL) staining, and transmission electron microscopy(TEM). We also investigated the expressionof proteins associated with apoptosis, hepatoprotection, and S1 P accumulation. RESULTS: S1 P accumulated in the HA-S1 P group livers more than S1 P group livers. Serum ALT levels, TUNEL-positive hepatocytes, and expression of cleaved caspase-3 expression, were significantly decreased in the HA-S1 P group. TEM revealed that the liver sinusoidal endothelial cell(LSEC) lining was preserved in the HA-S1 P group. Moreover, the HA-S1 P group showed a greater increase in the HO-1 protein levels compared to the S1 P group.CONCLUSION: Our results suggest that HA-S1 P exhibits cytoprotective effects in the liver through the inhibition of LSEC apoptosis. HA-S1 P is an effective agent for hepatic ischemia/reperfusion injury.展开更多
AIM:To develop a classifier for differentiating between healthy and early stage glaucoma eyes based on peripapillary retinal nerve fiber layer(RNFL)thicknesses measured with optical coherence tomography(OCT),using mac...AIM:To develop a classifier for differentiating between healthy and early stage glaucoma eyes based on peripapillary retinal nerve fiber layer(RNFL)thicknesses measured with optical coherence tomography(OCT),using machine learning algorithms with a high interpretability.METHODS:Ninety patients with early glaucoma and 85 healthy eyes were included.Early glaucoma eyes showed a visual field(VF)defect with mean deviation>-6.00 d B and characteristic glaucomatous morphology.RNFL thickness in every quadrant,clock-hour and average thickness were used to feed machine learning algorithms.Cluster analysis was conducted to detect and exclude outliers.Tree gradient boosting algorithms were used to calculate the importance of parameters on the classifier and to check the relation between their values and its impact on the classifier.Parameters with the lowest importance were excluded and a weighted decision tree analysis was applied to obtain an interpretable classifier.Area under the ROC curve(AUC),accuracy and generalization ability of the model were estimated using cross validation techniques.RESULTS:Average and 7 clock-hour RNFL thicknesses were the parameters with the highest impor tance.Correlation between parameter values and impact on classification displayed a stepped pattern for average thickness.Decision tree model revealed that average thickness lower than 82μm was a high predictor for early glaucoma.Model scores had AUC of 0.953(95%CI:0.903-0998),with an accuracy of 89%.CONCLUSION:Gradient boosting methods provide accurate and highly interpretable classifiers to discriminate between early glaucoma and healthy eyes.Average and 7-hour RNFL thicknesses have the best discriminant power.展开更多
Objective:Gemcitabine plus nab-paclitaxel(GnP)is the standard first-line therapy for advanced pancreatic ductal adenocarcinoma(PDAC).S-1,an oral fluoropyrimidine derivative,as compared with gemcitabine,is non-inferior...Objective:Gemcitabine plus nab-paclitaxel(GnP)is the standard first-line therapy for advanced pancreatic ductal adenocarcinoma(PDAC).S-1,an oral fluoropyrimidine derivative,as compared with gemcitabine,is non-inferior in terms of overall survival(OS)and is associated with lower hematologic toxicity.Accordingly,S-1 is a convenient oral alternative treatment for advanced PDAC.This study was aimed at comparing the efficacy and safety of gemcitabine plus S-1(GS)vs.GnP as first-line chemotherapy for advanced PDAC.Methods:Patients with advanced PDAC who received first-line GS or GnP at the Peking Union Medical College Hospital between March 2011 and November 2022 were evaluated.Results:A total of 300 patients were assessed,of whom 84 received GS and 216 received GnP.The chemotherapy completion rate was higher with GS than GnP(50.0%vs.30.3%,P=0.0028).The objective response rate(ORR)was slightly higher(14.3%vs.9.7%,P=0.35),and the median OS was significantly longer(17.9 months vs.13.3 months,P=0.0078),in the GS group than the GnP group.However,the median progression-free survival(PFS)did not significantly differ between groups.Leukopenia risk was significantly lower in the GS group than the GnP group(14.9%vs.28.1%,P=0.049).Conclusions:As first-line chemotherapy for advanced PDAC,the GS regimen led to a significantly longer OS than the GnP regimen.The PFS,ORR,and incidence of severe adverse events were comparable between the GS and GnP groups.展开更多

文摘Background Examining the health outcomes of veterans who have completed the United States Veterans Health Administration’s(VHA’s)Traumatic Brain Injury(TBI)Screening and Evaluation Program may aid in the refinement and improvement of clinical care initiatives within the VHA.This study compared self-reported physical functioning,cardiometabolic health conditions,and health care utilization patterns in Million Veteran Program enrollees with TBI Screening and Evaluation Program data(collected between 2007 and 2019),with the goal of enhancing understanding of potentially modifiable health conditions in this population.Methods In this observational cohort study,veterans(n=16,452)were grouped based on the diagnostic outcome of the TBI Screening and Evaluation Program:1)negative TBI screen(Screen^(-));2)positive TBI screen but no confirmed TBI diagnosis[Screen^(+)/Comprehensive TBI Evaluation(CTBIE)^(-)];or 3)positive TBI screen and confirmed TBI diagnosis(Screen^(+)/CTBIE^(+)).Chi-square tests and analysis of covariance were used to explore group differences in physical functioning,cardiometabolic health conditions,and health care utilization patterns,and logistic regressions were used to examine predictors of Screen^(+/-)and CTBIE^(+/-)group status.Results The results showed that veterans in the Screen^(+)/CTBIE^(-)and Screen^(+)/CTBIE^(+)groups generally reported poorer levels of physical functioning(P’s<0.001,np2=0.02 to 0.03),higher rates of cardiometabolic health conditions(P’s<0.001,φ=0.14 to 0.52),and increased health care utilization(P’s<0.001,φ=0.14 to>0.5)compared with the Screen-group;however,health outcomes were generally comparable between the Screen^(+)/CTBIE^(-)and Screen^(+)/CTBIE^(+)groups.Follow-up analyses confirmed that while physical functioning,hypertension,stroke,healthcare utilization,and prescription medication use reliably distinguished between the Screen-and Screen^(+)groups(P’s<0.02,OR’s 0.78 to 3.38),only physical functioning distinguished between the Screen^(+)/CTBIE^(-)and Screen^(+)/CTBIE^(+)groups(P<0.001,OR 0.99).Conclusions The findings suggest that veterans who screen positive for TBI,regardless of whether they are ultimately diagnosed with TBI,are at greater risk for negative health outcomes,signifying that these veterans represent a vulnerable group that may benefit from increased clinical care and prevention efforts.

文摘The unfolded protein response pathway is an evolutionarily conserved cytoprotective signaling cascade,essential for cell function and survival.Unfolded protein response signaling is tightly integrated with bone cell differentiation and function,and chronic unfolded protein response activation has been identified in bone disease.The unfolded protein response has been found to promote oncogenesis and drug resistance,raising the possibility that unfolded protein response modulators may have activity as anti-cancer agents.Cancer-associated bone disease remains a major cause of morbidity for patients with multiple myeloma or bone-metastatic disease.Understanding the critical role of unfolded protein response signaling in cancer development and metastasis,as well as its role in bone homeostasis,may lead to novel mechanisms by which to target cancer-associated bone disease.In this review,we summarize the current research delineating the roles of the unfolded protein response in bone biology and pathophysiology,and furthermore,review unfolded protein response modulating agents in the contexts of cancer and cancer-associated bone disease.

文摘Objectives:To evaluate the occurrence of depression and its related factors among public health nurses(PHNs)engaged in child neglect-related abuse cases.Methods:A cross-sectional study of PHNs involved in maternal and child health or child and family welfare,and engaged in cases of child neglect-related abuse was conducted from January to March 2023 in all municipalities of 13 prefectures selected from Japan's seven regions,considering geographic diversity and major urban areas.The Patient Health Questionnaire-2 was used to screen for depression.Statistical analyses included the Chi-square test,Mann-Whitney U test,and logistic regression analysis.Results:A total of 359 provided valid responses.Among them,11.4%screened positive for depression.Significant individual factors related depression were management-stage nurses(OR=6.27,95%CI:1.63-24.18),no disability welfare experience(OR=3.49,95%CI:1.05-11.55),being assignment of highrisk cases within the past year(OR=8.28,95%CI:2.07-33.08),and lower work control(OR=0.53,95%CI:0.32-0.86).Workplace environment factors were lower perceptions and relationships within the workplace(OR=0.43,95%CI:0.29-0.65)and perceiving manpower as not secured(OR=9.07,95%CI:1.17-70.40),Conclusions:Mental health measures for PHNs should include workload management and reduced burdens through workforce enhancement,attention to burdens on managerial PHNs,promoting communication and mutual support,and opportunities to engage in disability welfare work.
文摘The rapidly growing seaweed farming contributes to food,feed,biofuel,and biochemicals,with Indonesia being the largest exporter of raw seaweed(53.35%)but low in carrageenan derivative products(4.49%).Sustainable farming supports SDG 13 and SDG 14 through emission reduction,marine ecosystem conservation,and increased biodiversity.However,its supply chain faces challenges in raw material availability,such as fluctuating harvests,inconsistent quality,limited logistics infrastructure,and market price volatility.Therefore,good environmental management is needed to optimize raw materials in carrageenan production through resource efficiency,environmentally friendly products,environmental regulations,and market and stakeholder awareness.This study identifies factors in the environment-based supply chain that influence the development of carrageenan production centers and analyzes their ecological impacts using quantitative SEM analysis.SEM analysis reveals that resource efficiency,waste management,environmental regulations,and stakeholder awareness and commitment are the main influencing factors.shows significant factors including raw material efficiency,waste management,and environmental regulations.Environmentally based supply chain integration for the development of carrageenan production center areas can be done by;focusing on sustainability and minimal environmental impact,support for achieving SDG 12,13,and 14,strategies for sustainable raw material management,circular economy,sustainability certification,renewable energy and environmentally friendly technology,and stakeholder collaboration and awareness.

文摘BACKGROUND Insulin resistance,lipotoxicity,and mitochondrial dysfunction contribute to the pathogenesis of metabolic dysfunction-associated steatotic liver disease(MASLD).Mitochondrial dysfunction impairs oxidative phosphorylation and increases reactive oxygen species production,leading to steatohepatitis and hepatic fibrosis.Artificial intelligence(AI)is a potent tool for disease diagnosis and risk stratification.AIM To investigate mitochondrial DNA polymorphisms in susceptibility to MASLD and establish an AI model for MASLD screening.METHODS Multiplex polymerase chain reaction was performed to comprehensively genotype 82 mitochondrial DNA variants in the screening dataset(n=264).The significant mitochondrial single nucleotide polymorphism was validated in an independent cohort(n=1046)using the Taqman®allelic discrimination assay.Random forest,eXtreme gradient boosting,Naive Bayes,and logistic regression algorithms were employed to construct an AI model for MASLD.RESULTS In the screening dataset,only mt12361A>G was significantly associated with MASLD.mt12361A>G showed borderline significance in MASLD patients with 2-3 cardiometabolic traits compared with controls in the validation dataset(P=0.055).Multivariate regression analysis confirmed that mt12361A>G was an independent risk factor of MASLD[odds ratio(OR)=2.54,95%confidence interval(CI):1.19-5.43,P=0.016].The genetic effect of mt12361A>G was significant in the non-diabetic group but not in the diabetic group.mt12361G carriers had a 2.8-fold higher risk than A carriers in the non-diabetic group(OR=2.80,95%CI:1.22-6.41,P=0.015).By integrating clinical features and mt12361A>G,random forest outperformed other algorithms in detecting MASLD[training area under the receiver operating characteristic curve(AUROC)=1.000,validation AUROC=0.876].CONCLUSION The mt12361A>G variant increased the severity of MASLD in non-diabetic patients.AI supports the screening and management of MASLD in primary care settings.
文摘Objective:This study aimed to evaluate the effectiveness of an integrative educational intervention-combining hospital-based schooling programs,child-centered communication,and therapeutic play-on clinical,psychosocial,and educational outcomes among school-aged children with cancer.Materials and Methods:A quasi-experimental pre-and post-test control group design was conducted at the pediatric oncology unit of Rumah Sakit Islam Jemursari,Surabaya,Indonesia.Fifty-two hospitalized children aged 6-18 years were randomly assigned to an intervention group(n=26)or a control group(n=26).The intervention lasted 1 month and integrated hospital-based lessons aligned with the national curriculum,therapeutic play,and developmentally appropriate communication by trained staff.Outcomes included pain intensity(Wong-Baker FACES Scale),treatment adherence,quality of life(Pediatric Cancer Quality of Life-32[PCQL-32]),anxiety(Revised Children’s Manifest Anxiety Scale),and literacy-numeracy performance.Data were analyzed using paired t-tests,ANCOVA,and Cohen’s d,with significance set at P<0.05.Results:Compared with controls,the intervention group showed significant improvements in pain reduction(P<0.001),treatment adherence(P<0.001),and quality of life across all PCQL-32 domains(P<0.001).Anxiety levels decreased significantly(P<0.001),and academic performance in literacy and numeracy improved markedly(P<0.001).Conclusion:The integrative educational intervention effectively enhanced clinical recovery,psychosocial well-being,and academic continuity among hospitalized children with cancer,emphasizing the need to integrate hospital-based education and play-based communication into pediatric oncology care.

文摘Objective:To predict parents'willingness to pay for dengue fever vaccination through the perception of dengue vaccine based on the Health Belief Model(HBM).Methods:A multicenter crosssectional study was conducted on parents who had children aged≤5 years and were participating in integrated health service post activities in 3 districts/cities with the highest incidence of dengue fever in Southeast Sulawesi.The instruments used to collect data in this study consisted of a participant characteristics questionnaire,a dengue vaccine perception questionnaire based on the HBM,and a willingness to pay for dengue vaccine questionnaire.Chi-square test and multivariate logistic regression analysis were used to determine the correlation between the perceived HBM subscales and willingness to pay for dengue vaccination.Results:A total of 310 people participated in this study with an average age of(29.9±5.6)years.Most of them were females(284/310,91.7%),80.9%(251/310)had a high education status and 87.4%(271/310)had direct experience with DF.The results showed good perceptions including Perceived Susceptibility(adjusted OR 3.7,95%CI 2.7-6.3,P<0.001),Perceived Severity(adjusted OR 2.8,95%CI 1.7-5.5,P<0.001),Perceived Benefits(adjusted OR 2.8,95%CI 1.7-4.5,P<0.001),and Cues to Action(adjusted OR 3.1,95%CI 1.9-5.1,P<0.001)were positively correlated with willingness to pay for the dengue vaccine comparing with poor perceptions,whereas Perceived Barriers showed no significant effect on willingness to pay for the dengue vaccine.Conclusions:These results suggest that a person's willingness to pay for dengue vaccine increases with their understanding of the risks and advantages of immunization as well as encouraging outside incentives.As a result,these elements are crucial in raising community involvement in the immunization campaign.

文摘As the world’s largest palm oil producer,Indonesia significantly benefits from its palm oil industry but also faces serious environmental challenges from palm oil mill effluent(POME)-a high-strength wastewater containing substantial organic matter,nutrients,suspended solids,and various chemical compounds.Sustainable and effective wastewater treatment strategies are urgently needed to address this issue.This review presents a comprehensive analysis of existing POME treatment technologies,including anaerobic digestion(AD),advanced oxidation processes(AOPs),membrane filtration,adsorption,phytoremediation,and microalgae-based systems.Each method is examined in terms of treatment efficiency,operational feasibility,and potential for imple-mentation under Indonesian conditions.While advanced processes,such as AOPs and membrane filtration,achieve high pollutant removal,they are often limited by operational costs.In contrast,biological approaches,such as AD and phytoremediation,offer both environmental benefits and economic value through the recovery of biogas,biofertilizers,and biomass.This review highlights the potential for integrating wastewater purification with resource recovery and valorization,supporting a shift toward more circular and sustainable management of POME.The insights provided are intended to guide future research,inform policy decisions,and facilitate the industrial adoption of optimized treatment systems.
文摘Aceh in Indonesia is rich inmarine resources and abundant fishery products such as oyster.Traditionally,fishermen only harvest oysters and discard the shells,which can cause pollution and environmental contamination.Waste Oyster Shells(WOS)contain a high percentage of calcium carbonate(CaCO_(3))that experiences thermal decomposition at high temperature,following the reaction CaCO_(3)→CaO+CO_(2)(ΔT=825℃).At temperature>900℃,dead-burned lime is formed,which severely influences CaO reactivity.However,the optimum temperature for producing high CaO content is still uncertain.Therefore,this study aimed to determine the optimum calcination temperature to produce high CaO content,assess initial setting time of WOS paste,and identify the best compressive strength of paste.For the experiment,WOS was used as a partial cement replacement(with a size of 0.075 mm)in paste at a proportion of 5%and calcined at temperature of 700℃,800℃,900℃,and 1000℃.The specimens used were an ebonite ring(dimensions:70 mm bottom diameter,60 mm top diameter,and 40 mm height)and a cube(dimensions:5 cm×5 cm×5 cm).The experiment was conducted following the ASTM(American Society for Testing andMaterials)standards and optimumcompressive strength values were analyzed using ANOVA(Analysis of Variance)and Response Surface Methodology(RSM)through the Design Expert software.The results showed that WOS calcined at 1000℃ increased CaO content by approximately 57.40%.Furthermore,the initial setting time test of 5%WOS paste at 1000℃ showed a more uniform binding performance compared to conventional cement paste,with an initial setting time of 75 min and a penetration depth of 15 mm.In line with the analysis,optimum compressive strength of 71.028 MPa with a desirability value of 0.986 was achieved at 5%cement replacement and calcination temperature of 786.44℃.

文摘Objective:This study aimed to describe the implementation of advance care planning(ACP)in patients with stroke.Methods:A scoping review was conducted according to the Arksey and O'Malley framework.Accordingly,we searched for articles published in ScienceDirect,PubMed,Scopus,ProQuest,and Medline.The inclusion criteria were original research on ACP among adult or elderly stroke patients,hospital-or community-based studies,and those published in English or Indonesian.Qualitative analysis was then used to identify keywords,categories,and themes.Results:Among the 1,050 articles identified,only 8 satisfied the inclusion criteria,indicating that limited studies were available for ACP interventions among stroke patients.Four themes were ultimately identified:the strategy implementation of AC,the challenges of ACP implementation,the benefits of ACP,and the factors influencing ACP.Strategy ACP implementation included the ACP method,the communication style,participation role,media used,and ACP documentation.Lack of information about ACP,facilities,incapacitated patients,and reluctance to implement have become challenges to implementation in stroke.The benefits of ACP,particularly the outcomes of the ACP program,included awareness of ACP engagement,patients can express their wishes,and increased advance directive(AD)documentation.The factors influencing ACP included patient condition,knowledge,and appropriate ACP methods.Conclusion:ACP has not been widely used when providing interventions for stroke patients.The implementation of ACP in stroke patients is full of challenges due to various patient conditions.Effective strategies are needed to improve ACP in stroke patients.It is necessary to develop an ACP for stroke patients'method that involves interprofessional collaboration,and studies on AcP in stroke patients need to be further conducted through interprofessional collaboration.
文摘Background and Objective Patients with glioma experience a high symptom burden and have diverse palliative care needs.However,the assessment scales used in palliative care remain non-standardized and highly heterogeneous.To evaluate the application patterns of the current scales used in palliative care for glioma,we aim to identify gaps and assess the need for disease-specific scales in glioma palliative care.Methods We conducted a systematic search of five databases including PubMed,Web of Science,Medline,EMBASE,and CINAHL for quantitative studies that reported scale-based assessments in glioma palliative care.We extracted data on scale characteristics,domains,frequency,and psychometric properties.Quality assessments were performed using the Cochrane ROB 2.0 and ROBINS-I tools.Results Of the 3,405 records initially identified,72 studies were included.These studies contained 75 distinct scales that were used 193 times.Mood(21.7%),quality of life(24.4%),and supportive care needs(5.2%)assessments were the most frequently assessed items,exceeding half of all scale applications.Among the various assessment dimensions,the Distress Thermometer(DT)was the most frequently used tool for assessing mood,while the Short Form-36 Health Survey Questionnaire(SF-36)was the most frequently used tool for assessing quality of life.The Mini Mental Status Examination(MMSE)was the most common tool for cognitive assessment.Performance status(5.2%)and social support(6.8%)were underrepresented.Only three brain tumor-specific scales were identified.Caregiver-focused scales were limited and predominantly burden-oriented.Conclusions There are significant heterogeneity,domain imbalances,and validation gaps in the current use of assessment scales for patients with glioma receiving palliative care.The scale selected for use should be comprehensive and user-friendly.

文摘Rubberized concrete is one of the most studied applications of discarded tires and offers a promising approach to developing materials with enhanced properties.The rubberized concrete mixture results in a reduced modulus of elasticity and a reduced compressive and tensile strength compared to traditional concrete.This study employs finite element simulations to investigate the elastic properties of rubberized mortar(RuM),considering the influence of inclusion stiffness and interfacial debonding.Different homogenization schemes,including Voigt,Reuss,and mean-field approaches,are implemented using DIGIMAT and ANSYS.Furthermore,the influence of the interfacial transition zone(ITZ)between mortar and rubber is analyzed by periodic homogenization.Subsequently,the influence of the ITZ is examined through a linear fracture analysis with the stress intensity factor as a key parameter,using the ANSYS SMART crack growth tool.Finally,a non-linear study in FEniCS is carried out to predict the strength of the composite material through a compression test.Comparisons with high density polyethylene(HDPE)and gravel inclusions show that increasing inclusion stiffness enhances compressive strength far more effectively than simply improving the mortar/rubber bond.Indeed,when the inclusions are much softer than the surrounding matrix,any benefit gained on the elastic modulus or strength from stronger interfacial adhesion becomes almost negligible.This study provide numerical evidence that tailoring the rubber’s intrinsic stiffness—not merely strengthening the rubber/mortar interface—is a decisive factor for improving the mechanical performance of RuM.
文摘BACKGROUND Unintended pregnancy occurs when an individual or couple conceives without planning or desire,which can potentially affect a child’s physical,mental,and social well-being.This can then lead to long-term socioeconomic challenges for families and communities.Although its impact on child growth and development is a pressing concern,research remains limited particularly in multicenter settings.AIM To examine the long-term consequences of unintended pregnancy on the critical years of early childhood growth and development.METHODS This analytical observational study employed a case-control design and was conducted in research centers across Indonesia,encompassing those located in Central Java,Lampung,Bali,and West Nusa Tenggara.A total of 700 children aged≤5 years with histories of intended or unintended pregnancies participated.Data collection involved structured interviews and direct anthropometric and developmental assessments.Data analyses were conducted using multivariate statistics and partial least squares structural equation modeling.RESULTS Unintended pregnancy was found to have a statistically significant effect on both child growth(t=8.178;P<0.001)and child development(t=25.688;P<0.001).Key growth problems identified included underweight,undernutrition,abnormal head circumference,and stunting.Developmental challenges prominently associated with unintended pregnancy included behavioral and emotional disorders,autism spectrum disorder,attentiondeficit/hyperactivity disorder,social and motor skill deficits,as well as visual and hearing impairments.CONCLUSION Unintended pregnancy significantly affects child growth and development,underscoring the need for early intervention,quality prenatal care,and strengthened family planning policies.

文摘Objectives:Professional antigen-presenting cells known as dendritic cells(DCs)assist as a connection between the innate and adaptive components of the immune response.DCs are attractive targets for immunomodulatory drugs because of their crucial function in triggering immunity.This study set out to examine,for the first time,how hibifolin affected mouse bone-marrow derived(BMDCs)dendritic cells,triggered by lipopolysaccharide(LPS)in vitro.Additionally,a mouse model of contact hypersensitivity(CHS)was used to assess its possible therapeutic effects in vivo.Methods:LPS was administered to BMDCs with or without hibifolin.Major Histocompatibility Complex(MHC)class II,cytokine production,and co-stimulatory molecule(CD80,CD86)expression levels were assessed.To evaluate the functional effects on T-cell activation,mixed lymphocyte responses using OVA specific T-cells were conducted.In vivo,immunoregulatory potential of hibifolin was examined using a CHS mouse model sensitized with 2,4-dinitrofluorobenzene(DNFB).Results:Hibifolin significantly reduced the expression of the proinflammatory cytokines TNF-αand IL-6,as well as the costimulatory molecules CD80,CD86,and MHC II induced by LPS,by about 40-50%.These effects were associated with reduced NF-κB and p38-MAPK pathway activity.JNK and ERK phosphorylation levels did not alter significantly.In vitro,BMDCs treated with hibifolin showed a 40%-45%down-regulated capacity to stimulate T-cell propagation and IFN-γrelease.Oral hibifolin treatment in vivo modestly decreased DNFB-induced CHS responses by 30%-40%.Conclusion:Overall,our results offer new insights that by inhibiting NF-κB and p38-MAPK signaling,hibifolin decreases the expression of costimulatory molecules and cytokines,thereby limiting BMDC activation,suppressing T-cell responses,and exerting immunomodulatory effects in the CHS mouse model.

文摘AIM:To assess expression of matrix metalloproteinases 2(MMP2)and MMP9 in gastric cancer,superficial gastritis and normal mucosa,and to measure metalloproteinase activity.METHODS:MMP2 and MMP9 mRNA expression was determined by quantitative real-time polymerase chain reaction.Normalization was carried out using three different factors.Proteins were analyzed by quantitative gelatin zymography(qGZ).RESULTS:18S ribosomal RNA(18SRNA)was very highly expressed,while hypoxanthine ribosyltransferase-1(HPRT-1)was moderately expressed.MMP2 was highly expressed,while MMP9 was not detected or lowly expressed in normal tissues,moderately or highly expressed in gastritis and highly expressed in cancer.Relative expression of 18SRNA and HPRT-1 showed no significant differences.Significant differences in MMP2 and MMP9 were found between cancer and normal tissue,but not between gastritis and normal tissue.Absolute quantification of MMP9 echoed this pattern,but differential expression of MMP2 proved conflictive.Analysis by qGZ indicated significant differences between cancer and normal tissue in MMP-2,total MMP-9,250 and 110 kDa bands.CONCLUSION:MMP9 expression is enhanced in gastric cancer compared to normal mucosa;interpretation of differential expression of MMP2 is difficult to establish.

文摘Previous studies have demonstrated that doublecortin-positive immature neurons exist pre- dominantly in the superficial layer of the cerebral cortex of adult mammals such as guinea pigs, and these neurons exhibit very weak properties of self-proliferation during adulthood under physiological conditions. To verify whether environmental enrichment has an impact on the proliferation and maturation of these immature neurons in the prefrontal cortex of adult guinea pigs, healthy adult guinea pigs were subjected to short-term environmental enrichment. Animals were allowed to play with various cognitive and physical stimulating objects over a period of 2 weeks, twice per day, for 60 minutes each. Immunofluorescence staining results indicated that the number of doublecortin-positive cells in layer II of the prefrontal cortex was significantly increased after short-term environmental enrichment exposure. In addition, these doublecortin-positive cells co-expressed 5-bromo-2-deoxyuridine (a marker of cell prolifera- tion), c-Fos (a marker of cell viability) and NeuN (a marker of mature neurons). Experimental findings showed that short-term environmental enrichment can induce proliferation, activation and maturation of doublecortin-positive cells in layer II of the prefrontal cortex of adult guinea pigs.

文摘Objective:To identify the bioactive compounds in catechins isolation and its components from green tea GMB-4 clone.Methods:Green tea GMB-4 clones were extracted with distilled water at 90C.Samples were eluted into the column with 10%ethanol.Subsequently,the column was eluted with95%ethanol and evaporated separately.Green tea extract was identified by thin layer chromatography.Catechins were separated by the stationary phase in column chromatography using polyamide with 10%ethanol eluent and 95%ethanol.The results of isolations were analyzed by high performance liquid chromatographic(HPLC)and LCMS/MS.Analysis of catechins by HPLC was done by external standard.Results:Fraction from 10%ethanol showed that four major peaks at retention time of1.663,2.367,2.950 and 4.890,indicated the presence of four catechins components including catechin,epicatechins,gallocatechin and epigallocatechin.Whereas,fraction from 95%ethanol showed two main peaks at retention time of 5.167 and 9.82,which indicated the presence of epigallocatechin gallate(EGCG)and epicatechin gallate(ECG).EGCG(m/z 459),epigallocatechin(m/z 307),ECG(m/z 443),and epicatechin(m/z 291)were isolated and separated successfully using HPLC and LC-MS/MS.Conclusions:The HPLC and LC-MS/MS methods were successfully tuned for the qualitative analysis of green tea extract with EGCG and ECG.Four major catechins were separated and identified by LC-MS/MS,such as EGCG,epigallocatechin,ECG and epicatechin.The result of HPLC analysis showed that EGCG and ECG were main components from catechins isolation of green tea GMB-4 clone.

文摘AIM: To investigate the cytoprotective effects in hepatic ischemia-reperfusion injury, we developed a new formulation of hyaluronic acid(HA) and sphingosine 1-phophate.METHODS: We divided Sprague-Dawley rats into 4 groups: control, HA, sphingosine 1-phosphate(S1P), and HA-S1 P. After the administration of each agent, we subjected the rat livers to total ischemia followed by reperfusion. After reperfusion, we performed the following investigations: alanine aminotransferase(ALT), histological findings, Td T-mediated d UTP-biotin nick end labeling(TUNEL) staining, and transmission electron microscopy(TEM). We also investigated the expressionof proteins associated with apoptosis, hepatoprotection, and S1 P accumulation. RESULTS: S1 P accumulated in the HA-S1 P group livers more than S1 P group livers. Serum ALT levels, TUNEL-positive hepatocytes, and expression of cleaved caspase-3 expression, were significantly decreased in the HA-S1 P group. TEM revealed that the liver sinusoidal endothelial cell(LSEC) lining was preserved in the HA-S1 P group. Moreover, the HA-S1 P group showed a greater increase in the HO-1 protein levels compared to the S1 P group.CONCLUSION: Our results suggest that HA-S1 P exhibits cytoprotective effects in the liver through the inhibition of LSEC apoptosis. HA-S1 P is an effective agent for hepatic ischemia/reperfusion injury.
文摘AIM:To develop a classifier for differentiating between healthy and early stage glaucoma eyes based on peripapillary retinal nerve fiber layer(RNFL)thicknesses measured with optical coherence tomography(OCT),using machine learning algorithms with a high interpretability.METHODS:Ninety patients with early glaucoma and 85 healthy eyes were included.Early glaucoma eyes showed a visual field(VF)defect with mean deviation>-6.00 d B and characteristic glaucomatous morphology.RNFL thickness in every quadrant,clock-hour and average thickness were used to feed machine learning algorithms.Cluster analysis was conducted to detect and exclude outliers.Tree gradient boosting algorithms were used to calculate the importance of parameters on the classifier and to check the relation between their values and its impact on the classifier.Parameters with the lowest importance were excluded and a weighted decision tree analysis was applied to obtain an interpretable classifier.Area under the ROC curve(AUC),accuracy and generalization ability of the model were estimated using cross validation techniques.RESULTS:Average and 7 clock-hour RNFL thicknesses were the parameters with the highest impor tance.Correlation between parameter values and impact on classification displayed a stepped pattern for average thickness.Decision tree model revealed that average thickness lower than 82μm was a high predictor for early glaucoma.Model scores had AUC of 0.953(95%CI:0.903-0998),with an accuracy of 89%.CONCLUSION:Gradient boosting methods provide accurate and highly interpretable classifiers to discriminate between early glaucoma and healthy eyes.Average and 7-hour RNFL thicknesses have the best discriminant power.

文摘Objective:Gemcitabine plus nab-paclitaxel(GnP)is the standard first-line therapy for advanced pancreatic ductal adenocarcinoma(PDAC).S-1,an oral fluoropyrimidine derivative,as compared with gemcitabine,is non-inferior in terms of overall survival(OS)and is associated with lower hematologic toxicity.Accordingly,S-1 is a convenient oral alternative treatment for advanced PDAC.This study was aimed at comparing the efficacy and safety of gemcitabine plus S-1(GS)vs.GnP as first-line chemotherapy for advanced PDAC.Methods:Patients with advanced PDAC who received first-line GS or GnP at the Peking Union Medical College Hospital between March 2011 and November 2022 were evaluated.Results:A total of 300 patients were assessed,of whom 84 received GS and 216 received GnP.The chemotherapy completion rate was higher with GS than GnP(50.0%vs.30.3%,P=0.0028).The objective response rate(ORR)was slightly higher(14.3%vs.9.7%,P=0.35),and the median OS was significantly longer(17.9 months vs.13.3 months,P=0.0078),in the GS group than the GnP group.However,the median progression-free survival(PFS)did not significantly differ between groups.Leukopenia risk was significantly lower in the GS group than the GnP group(14.9%vs.28.1%,P=0.049).Conclusions:As first-line chemotherapy for advanced PDAC,the GS regimen led to a significantly longer OS than the GnP regimen.The PFS,ORR,and incidence of severe adverse events were comparable between the GS and GnP groups.
