Objective:This article employs a scoping review methodology,integrates knowledge and information about current pediatric nurses'practices and perceptions regarding family-centered care in pediatric settings.Method...Objective:This article employs a scoping review methodology,integrates knowledge and information about current pediatric nurses'practices and perceptions regarding family-centered care in pediatric settings.Methods:Published articles were retrieved from databases including EBSCO host,PubMed,Springer,Science Direct,Ovid,and CINAHL between 2013 and 2023.Results:The finding shows a better understanding of pediatric nurses'perceptions of family-centered care in association with their clinical settings.However,the evidence indicates that integrating family-centered care components into health care services is difficult and confusing to nurses and is often not implemented in a clinical setting.As evidenced by this review,studies have consistently reported similar results;family-centered care was a good perception and understood by pediatric nurses as a concept but inconsistently used in a daily practice setting.Conclusions:This scoping review is the first phase in promoting a strategic plan to provide educational interventions for pediatric nurses to implement family-centered care in their daily practice settings.It's necessary to recognize pediatric nurses'perceptions and practices concerning family-centered care to provide optimal healthcare services in pediatric settings.展开更多
Background:Luliconazole is an imidazole antifungal drug mainly used to treat dermatophytic infections including tinea pedis,tinea cruris,and tinea corporis.Objective:The purpose of this research was to synthesize a tr...Background:Luliconazole is an imidazole antifungal drug mainly used to treat dermatophytic infections including tinea pedis,tinea cruris,and tinea corporis.Objective:The purpose of this research was to synthesize a transferosomal gel incorporating luliconazole for external applications.Materials and methods:The preparation method employed thin film hydration to prepare transferosomes loaded with luliconazole with lecithin and tween 80 at different concentrations.The transferosomes formed were characterized in terms of particle size and entrapment efficiency.Finally,the prepared transferosomes were applied in a carbopol gel base and characterized for drug content,pH,spreadability,viscosity,in vitro release profile,and antifungal studies.Results:The synthesized luliconazole transferosomes had high entrapment efficiency of 74.45%,and 92.75%,with particle size ranging between 60 and 200 nm.The shape of the transferosomes was established using scanning electron microscopy;the results depicted spherical-shaped vesicles.The in vitro release study also suggested that the entrapment efficiency influences in vitro release where there is a negative relationship between the two.The gel formulation revealed a good antifungal effect.Conclusion:The luliconazole transferosomal gel exhibited a sustained release profile of the drug and thus may lessen the number of applications required,thereby enhancing patient compliance.展开更多

Theexpression levels of proteins including Bcl-2, Bax, Caspase3, SIRT1, and components of theMAPK pathway were analyzed via Western blot. Results: Resveratrol significantly inhibitedthe migration and proliferation of HepG2 cells at concentrations above 100 μmol/L(P<0.01). The expression of Bax, cleaved Caspase3 and SIRT1 was up-regulate (P<0.05)and Bcl-2, p-JNK、p-p38 MAPK was down-regulate (P<0.05) by resveratrol. The distributed fiber optic sensing system,known for its high sensitivity and wide-ranging measurement capabilities,has been widely used in monitoring underground gas pipelines.It primarily serves to perceive vibratio...The distributed fiber optic sensing system,known for its high sensitivity and wide-ranging measurement capabilities,has been widely used in monitoring underground gas pipelines.It primarily serves to perceive vibration signals induced by external events and to effectively provide early warnings of potential intrusion activities.Due to the complexity and diversity of external intrusion events,traditional deep learning methods can achieve event recognition with an average accuracy exceeding 90%.However,these methods rely on large-scale datasets,leading to significant time and labor costs during the data collection process.Additionally,traditional methods perform poorly when faced with the scarcity of low-frequency event samples,making it challenging to address these rare occurrences.To address this issue,this paper proposes a small-sample learning model based on triplet learning for intrusion event recognition.The model employs a 6-way 20-shot support set configuration and utilizes the KNN clustering algorithm to assess the model's performance.Experimental results indicate that the model achieves an average accuracy of 91.6%,further validating the superior performance of the triplet learning model in classifying external intrusion events.Compared to traditional methods,this approach not only effectively reduces the dependence on large-scale datasets but also better addresses the classification of low-frequency event samples,demonstrating significant application potential.展开更多

Wang et al identifiedseveral significant risk factors of SSIs, including age ≥ 60 years, diabetesmellitus, and surgical complications such as insufficient cystic duct stump closure,gallbladder perforation, empyema, and postoperative hematoma. Their findingssuggest that prophylactic antibiotics can serve as a protective factor against SSIs.However, other reported risk factors and preventive strategies warrant considerationto further reduce the incidence of SSIs, lower healthcare costs, and enhancepatient outcomes. Additionally, the judicious use of prophylactic antibioticsis crucial in light of the growing global challenge of antibiotic resistance caused bythe misuse and overuse of antibiotics. Objective:The objective of this study was to determine the level of methotrexate(MTX)toxicity in the intestines of mice and to evaluate the protective effect of probiotics composed of Streptococcus,Bifidobacterium,and...Objective:The objective of this study was to determine the level of methotrexate(MTX)toxicity in the intestines of mice and to evaluate the protective effect of probiotics composed of Streptococcus,Bifidobacterium,and Lactobacillus species on intestinal cells during MTX treatment.Methods:Mice were divided into three groups:control,MTX group(received MTX injections),and MTX+probiotics group(received MTX injections along with a diet containing probiotics).Morphological and histological changes,the level of mitochondrial DNA(mtDNA)damage,the level of lipid peroxidation products,and gene expression in the mice’s small intestine were assessed.Results:We demonstrated that intraperitoneal MTX injections significantly increased mtDNA damage in the liver(p<0.001),small intestine(p<0.001),and blood of mice(p<0.01).MTX elevated the quantity of lipid peroxidation products in the liver and small intestine,indicating its strong prooxidative properties.MTX induced structural changes in the mice’s intestines,characterized by leukocytic infiltration of tissues.Probiotic therapy in mice partially mitigated the morphological and histological changes in the small intestine induced by MTX,reduced oxidative stress,and promoted increased expression of quinone oxidoreductase 1(Nqo1),which participates in both cell protection against oxidative stress and drug/xenobiotic detoxification.Probiotics prevented the upregulation of the proinflammatory cytokine IL-1b in the small intestine and induced increased expression of genes associated with the Nuclear factor erythroid 2-related factor 2/Antioxidant response element(Nrf2/ARE)pathway,an important mechanism of cell protection.Conclusions:Probiotics can be considered an effective approach to reducing the toxicity of MTX during psoriasis or cancer treatment.展开更多
The rapid evolution of wireless technologies and the advent of 6G networks present new challenges and opportunities for Internet ofThings(IoT)applications,particularly in terms of ultra-reliable,secure,and energyeffic...The rapid evolution of wireless technologies and the advent of 6G networks present new challenges and opportunities for Internet ofThings(IoT)applications,particularly in terms of ultra-reliable,secure,and energyefficient communication.This study explores the integration of Reconfigurable Intelligent Surfaces(RIS)into IoT networks to enhance communication performance.Unlike traditional passive reflector-based approaches,RIS is leveraged as an active optimization tool to improve both backscatter and direct communication modes,addressing critical IoT challenges such as energy efficiency,limited communication range,and double-fading effects in backscatter communication.We propose a novel computational framework that combines RIS functionality with Physical Layer Security(PLS)mechanisms,optimized through the algorithm known as Deep Deterministic Policy Gradient(DDPG).This framework adaptively adapts RIS configurations and transmitter beamforming to reduce key challenges,including imperfect channel state information(CSI)and hardware limitations like quantized RIS phase shifts.By optimizing both RIS settings and beamforming in real-time,our approach outperforms traditional methods by significantly increasing secrecy rates,improving spectral efficiency,and enhancing energy efficiency.Notably,this framework adapts more effectively to the dynamic nature of wireless channels compared to conventional optimization techniques,providing scalable solutions for large-scale RIS deployments.Our results demonstrate substantial improvements in communication performance setting a new benchmark for secure,efficient and scalable 6G communication.This work offers valuable insights for the future of IoT networks,with a focus on computational optimization,high spectral efficiency and energy-aware operations.展开更多

A systematic review was performed of all randomized clinical trials (RCTs) tha...This study was a systematic review of the available evidence on quality of life in patients after laparoscopic or open colorectal surgery. A systematic review was performed of all randomized clinical trials (RCTs) that compared laparoscopic with open colorectal surgery. Study selection, quality assessment and data extraction were carried out independently by two reviewers. Primary endpoint was quality of life after laparoscopic and open colorectal surgery, as assessed by validated questionnaires. The search resulted in nine RCTs that included 2263 patients. Shortand long-term results of these RCTs were described in 13 articles. Postoperative follow-up ranged from 2 d to 6.7 years. Due to clinical heterogeneity, no meta-analysis could be conducted. Four RCTs did not show any difference in quality of life between laparoscopic or open colorectal surgery. The remaining five studies reported a better quality of life in favor of the laparoscopic group on a few quality of life scales at time points ranging from 1 wk to 2 years after surgery. In conclusion, based on presently available high-level evidence, this systematic review showed no clinically relevant differences in postoperative quality of life between laparoscopic and open colorectal surgery.展开更多
OBJECTIVE:To assess the efficacy and safety of extracorporeal shockwave therapy(ESWT)combined with sodium hyaluronate(HA)for the treatment of knee osteoarthritis(KOA).METHODS:PubM ed,Embase,the Cochrane Library,Web of...OBJECTIVE:To assess the efficacy and safety of extracorporeal shockwave therapy(ESWT)combined with sodium hyaluronate(HA)for the treatment of knee osteoarthritis(KOA).METHODS:PubM ed,Embase,the Cochrane Library,Web of Science,China National Knowledge Infrastructure,Wanfang database,China Science and Technology Journal Database,and SinoMed were searched from inception to July 2020.The quality of the randomized controlled trials was evaluated independently by two reviewers according to the criteria in the Cochrane Collaboration for Systematic Reviews.The identified articles were then screened individually using EndnoteX9 for eligibility in this Meta-analysis.The heterogeneity among the articles was evaluated using I2.RESULTS:A total of 17 studies,comprising 2000 individuals,were included in this Meta-analysis.The results showed that a significant improvement was observed in knee pain and function based on the clinical efficacy of ESWT combined with HA.Statistical analysis of clinical efficacy showed that[relative risk(RR)=1.21,95%confidence interval(CI)(1.12,1.30),P<0.01].Statistical analysis of visual analog scale showed that[standardized mean difference(SMD)=-2.84,95%CI(-4.01,-1.66),P<0.01].Western Ontario and McMaster University osteoarthritis index statistical analysis showed that[SMD=-1.57,95%CI(-2.52,-0.61),P<0.01].Lysholm score statistical analysis showed that[SMD=1.71,95%CI(0.98,2.44),P<0.01].In addition,only minor side effects,such as redness and swelling of the skin,were observed.CONCLUSIONS:Medium to low quality evidence showed that ESWT combined with HA offers an inexpensive,welltolerated,safe,and effective method to improve pain and functionality in patients with KOA.However,tightly controlled,randomized,large multicenter trials are warranted to validate the current findings.展开更多

Phytates in all samples were also determined using the Method of Fruhbeck et al., 1995. AIM To characterise venous thromboembolism(VTE) in gastrointestinal cancer and assess the clinical utility of risk stratification scoring. METHODS We performed a retrospective analysis using electronic patient records...AIM To characterise venous thromboembolism(VTE) in gastrointestinal cancer and assess the clinical utility of risk stratification scoring. METHODS We performed a retrospective analysis using electronic patient records of 910 gastro-oesophageal(GO) cancer and 1299 colorectal cancer(CRC) patients referred to a tertiary cancer centre to identify the incidence of VTE, its relationship to chemotherapy and impact on survival.VTE risk scores were calculated using the Khorana index. Patients were classified as low risk(0 points), intermediate risk(1 to 2 points) or high risk(3 points). Data was analysed to determine the sensitivity of the Khorana score to predict VTE. RESULTS The incidence of VTE was 8.9% for CRC patients and 9.7% for GO cancer patients. Pulmonary emboli(PE) were more common in advanced than in localised CRC(50% vs 21% of events respectively) and lower limb deep vein thrombosis(DVT) were more common in localised than in advanced CRC(62% vs 39% of events respectively). The median time to VTE from cancer diagnosis was 8.3 mo for CRC patients compared to 6.7 mo in GO cancer. In localised CRC median time to VTE was 7.1 mo compared with 10.1 mo in advanced CRC. In contrast in GO cancer, the median time to VTE was 12.5 mo in localised disease and 6.8 mo in advanced disease. No survival difference was seen between patients with and without VTE in this cohort. The majority of patients with CRC in whom VTE was diagnosed had low or intermediate Khorana risk score(94% for localised and 97% in advanced CRC). In GO cancer, all patients scored either intermediate or high risk due to the primary site demonstrating a limitation of the risk assessment score in discriminating high and low risk patients with GO cancers. Additional risk factors were identified in this cohort including surgery, chemotherapy or hospital admission. Overall, 81% of patients with CRC and 77% of patients with GO cancer had one or more of these factors within 4 wk prior to diagnosis VTE. These should be factored into clinical risk assessment scores. CONCLUSION The Khorana score has low sensitivity for thrombotic events in CRC and cannot discriminate low risk patients in high risk cancer sites such as GO cancer.展开更多

In this study,a convenient method of preparing the substrate is proposed with one-pot synthesis of silver colloid under body heat,and the SERS detection uses the fresh substrate to avoid the drawback of substrates’sh...In this study,a convenient method of preparing the substrate is proposed with one-pot synthesis of silver colloid under body heat,and the SERS detection uses the fresh substrate to avoid the drawback of substrates’short life of use.The synthesis of silver colloid is carried out in a 10 mL vial by using ascorbic acid as a reductant and trisodium citrate as a stabilizer.The vial is grasped with the palm of the experimenter for several minutes without shaking.The proposed method is simple,rapid,green energy and cost-effective.By adjusting the concentration of trisodium citrate,not only the particle size can be controlled from about 110 nm to 50 nm but also the homogeneity of nanoparticles can be improved.As a SERS substrate,the silver colloid has high batch reproducibility and showed good SERS activity.The relative standard deviation between different manufacturers is 5.51%when the substrate of silver colloid is used for the detection of rhodamine 6 G.Using the substrate,the lowest detection concentrations of rhodamine 6 G,crystal violet,enrofloxacin,melamine and leucomalachite green are 1.0×10-8,6.1×10-8,1.4×10-6,7.1×10-5 and 5.1×10-8 mol/L,respectively.Results demonstrate that the developed method has the advantage of convenience and high efficiency in the field preparation of reliable SERS substrate.展开更多
Gold nanoparticle arrays fabricated via layerby-layer technique were investigated using grazing-incidence small-angle X-ray scattering(GISAXS) method.Samples containing two gold nanoparticle layers that were separated...Gold nanoparticle arrays fabricated via layerby-layer technique were investigated using grazing-incidence small-angle X-ray scattering(GISAXS) method.Samples containing two gold nanoparticle layers that were separated by 5,11,15 and 21 poly electrolyte(PE) interlayers were studied.By using different X-ray incident angles,correlations of gold nanoparticles(GNPs) in the same layer and in two different layers were investigated.It is found that both sideway correlations between GNPs in the same layer and vertical correlation between two gold nanoparticle layers depend on the thickness of PE interlayers.According to sideway correlation,the size of GNPs is determined to be(13.0±0.5) nm in all of the four samples,which was also proved by scanning electron microscopy(SEM) and theoretical calculation of form factor of spherical particles.From vertical correlation,distance between two gold nanoparticle layers was determined for sample with 11,15 and 21 PE layers.These distances can be reasonably explained with the number of PE layers and the thickness of single PE layer.These results indicate that by repeated depositing of oppositely charged PE layers,a true three-dimensional(3 D) nanostructure can eventually be designed.展开更多

Results of the flow visualization show that the material flow is asymmetrical during the friction stir welding (FSW) process and there are also significant differences in the flow patterns observed on advancing side and retreating side. On advancing side, some material transport forward and some move backward, but on retreating side, material only transport backward. At the top surface of the weld, significant material transport forward due to the action of the rotating tool shoulder. Combining the data from all the markers, a three-dituensional flow visualization, similar to the 3D image reconstruction technique, was obtained. The three-dimensional plot gives the tendency chart of material flow in friction stir welding process and from the plot it can be seen that there is a vertical, circular motion around the longitudinal axis of the weld. On the advancing side of the weld, the material is pushed downward but on the retreating side, the material is pushed toward the crown of the weld. The net result of the two relative motions in both side of the advancing and the retreating is that a circular motion comes into being. Comparatively, the material flow around the longitudinal axis is a secondary motion.展开更多
A new set of descriptors, HSEHPCSV (component score vector of hydrophobic, steric, and electronic properties together with hydrogen bonding contributions), were derived from principal component analyses of 95 physic...A new set of descriptors, HSEHPCSV (component score vector of hydrophobic, steric, and electronic properties together with hydrogen bonding contributions), were derived from principal component analyses of 95 physicochemical variables of 20 natural amino acids separately according to different kinds of properties described, namely, hydrophobic, steric, and electronic properties as well as hydrogen bonding contributions. HSEHPCSV scales were then employed to express structures of angiotensin-converting enzyme inhibitors, bitter tasting thresholds and bactericidal 18 peptide, and to construct QSAR models based on partial least square (PLS). The results obtained are as follows: the multiple correlation coefficient (R2cum) of 0.846, 0.917 and 0.993, leave-one-out cross validated Q2cm of 0.835, 0.865 and 0.899, and root-mean-square error for estimated error (RMSEE) of 0.396, 0.187and 0.22, respectively. Satisfactory results showed that, as new amino acid scales, data of HSEHPCSV may be a useful structural expression methodology'for the studies on peptide QSAR (quantitative structure-activity relationship) due to many advantages such as plentiful structural information, definite physical and chemical meaning and easy interpretation.展开更多
文摘Objective:This article employs a scoping review methodology,integrates knowledge and information about current pediatric nurses'practices and perceptions regarding family-centered care in pediatric settings.Methods:Published articles were retrieved from databases including EBSCO host,PubMed,Springer,Science Direct,Ovid,and CINAHL between 2013 and 2023.Results:The finding shows a better understanding of pediatric nurses'perceptions of family-centered care in association with their clinical settings.However,the evidence indicates that integrating family-centered care components into health care services is difficult and confusing to nurses and is often not implemented in a clinical setting.As evidenced by this review,studies have consistently reported similar results;family-centered care was a good perception and understood by pediatric nurses as a concept but inconsistently used in a daily practice setting.Conclusions:This scoping review is the first phase in promoting a strategic plan to provide educational interventions for pediatric nurses to implement family-centered care in their daily practice settings.It's necessary to recognize pediatric nurses'perceptions and practices concerning family-centered care to provide optimal healthcare services in pediatric settings.
文摘Background:Luliconazole is an imidazole antifungal drug mainly used to treat dermatophytic infections including tinea pedis,tinea cruris,and tinea corporis.Objective:The purpose of this research was to synthesize a transferosomal gel incorporating luliconazole for external applications.Materials and methods:The preparation method employed thin film hydration to prepare transferosomes loaded with luliconazole with lecithin and tween 80 at different concentrations.The transferosomes formed were characterized in terms of particle size and entrapment efficiency.Finally,the prepared transferosomes were applied in a carbopol gel base and characterized for drug content,pH,spreadability,viscosity,in vitro release profile,and antifungal studies.Results:The synthesized luliconazole transferosomes had high entrapment efficiency of 74.45%,and 92.75%,with particle size ranging between 60 and 200 nm.The shape of the transferosomes was established using scanning electron microscopy;the results depicted spherical-shaped vesicles.The in vitro release study also suggested that the entrapment efficiency influences in vitro release where there is a negative relationship between the two.The gel formulation revealed a good antifungal effect.Conclusion:The luliconazole transferosomal gel exhibited a sustained release profile of the drug and thus may lessen the number of applications required,thereby enhancing patient compliance.

文摘Background: Resveratrol is a widely recognized anti-inflammatory and antioxidant agent,and it has been suggested to possess anti-tumor effects. But the effect of resveratrol onhepatocellular carcinoma and its molecular mechanisms are unknown. This study confirmedthe effects of resveratrol on HepG2 cell proliferation and migration, and the underlyingmechanism. Methods: Viability of resveratrol (0-200 μmol/L)-treated HepG2 cells wasdetected by CCK-8. Wound healing assay was employed to evaluate cell migration. Theexpression levels of proteins including Bcl-2, Bax, Caspase3, SIRT1, and components of theMAPK pathway were analyzed via Western blot. Results: Resveratrol significantly inhibitedthe migration and proliferation of HepG2 cells at concentrations above 100 μmol/L(P<0.01). The expression of Bax, cleaved Caspase3 and SIRT1 was up-regulate (P<0.05)and Bcl-2, p-JNK、p-p38 MAPK was down-regulate (P<0.05) by resveratrol. Conclusion:Resveratrol suppresses the proliferation and migration of HepG2 cells by activating theSIRT1 signaling pathway and inhibiting the JNK and p38 MAPK pathways.

文摘The distributed fiber optic sensing system,known for its high sensitivity and wide-ranging measurement capabilities,has been widely used in monitoring underground gas pipelines.It primarily serves to perceive vibration signals induced by external events and to effectively provide early warnings of potential intrusion activities.Due to the complexity and diversity of external intrusion events,traditional deep learning methods can achieve event recognition with an average accuracy exceeding 90%.However,these methods rely on large-scale datasets,leading to significant time and labor costs during the data collection process.Additionally,traditional methods perform poorly when faced with the scarcity of low-frequency event samples,making it challenging to address these rare occurrences.To address this issue,this paper proposes a small-sample learning model based on triplet learning for intrusion event recognition.The model employs a 6-way 20-shot support set configuration and utilizes the KNN clustering algorithm to assess the model's performance.Experimental results indicate that the model achieves an average accuracy of 91.6%,further validating the superior performance of the triplet learning model in classifying external intrusion events.Compared to traditional methods,this approach not only effectively reduces the dependence on large-scale datasets but also better addresses the classification of low-frequency event samples,demonstrating significant application potential.

文摘In this article, we have addressed the recent published article by Wang et al whichexamines risk factors associated with surgical site infections (SSIs) and evaluatesthe effectiveness of prophylactic antibiotics in their prevention. Wang et al identifiedseveral significant risk factors of SSIs, including age ≥ 60 years, diabetesmellitus, and surgical complications such as insufficient cystic duct stump closure,gallbladder perforation, empyema, and postoperative hematoma. Their findingssuggest that prophylactic antibiotics can serve as a protective factor against SSIs.However, other reported risk factors and preventive strategies warrant considerationto further reduce the incidence of SSIs, lower healthcare costs, and enhancepatient outcomes. Additionally, the judicious use of prophylactic antibioticsis crucial in light of the growing global challenge of antibiotic resistance caused bythe misuse and overuse of antibiotics. Effective management strategies forprophylactic antibiotic use should be prioritized to balance infection control withthe need to combat antimicrobial resistance.

文摘Objective:The objective of this study was to determine the level of methotrexate(MTX)toxicity in the intestines of mice and to evaluate the protective effect of probiotics composed of Streptococcus,Bifidobacterium,and Lactobacillus species on intestinal cells during MTX treatment.Methods:Mice were divided into three groups:control,MTX group(received MTX injections),and MTX+probiotics group(received MTX injections along with a diet containing probiotics).Morphological and histological changes,the level of mitochondrial DNA(mtDNA)damage,the level of lipid peroxidation products,and gene expression in the mice’s small intestine were assessed.Results:We demonstrated that intraperitoneal MTX injections significantly increased mtDNA damage in the liver(p<0.001),small intestine(p<0.001),and blood of mice(p<0.01).MTX elevated the quantity of lipid peroxidation products in the liver and small intestine,indicating its strong prooxidative properties.MTX induced structural changes in the mice’s intestines,characterized by leukocytic infiltration of tissues.Probiotic therapy in mice partially mitigated the morphological and histological changes in the small intestine induced by MTX,reduced oxidative stress,and promoted increased expression of quinone oxidoreductase 1(Nqo1),which participates in both cell protection against oxidative stress and drug/xenobiotic detoxification.Probiotics prevented the upregulation of the proinflammatory cytokine IL-1b in the small intestine and induced increased expression of genes associated with the Nuclear factor erythroid 2-related factor 2/Antioxidant response element(Nrf2/ARE)pathway,an important mechanism of cell protection.Conclusions:Probiotics can be considered an effective approach to reducing the toxicity of MTX during psoriasis or cancer treatment.

文摘The rapid evolution of wireless technologies and the advent of 6G networks present new challenges and opportunities for Internet ofThings(IoT)applications,particularly in terms of ultra-reliable,secure,and energyefficient communication.This study explores the integration of Reconfigurable Intelligent Surfaces(RIS)into IoT networks to enhance communication performance.Unlike traditional passive reflector-based approaches,RIS is leveraged as an active optimization tool to improve both backscatter and direct communication modes,addressing critical IoT challenges such as energy efficiency,limited communication range,and double-fading effects in backscatter communication.We propose a novel computational framework that combines RIS functionality with Physical Layer Security(PLS)mechanisms,optimized through the algorithm known as Deep Deterministic Policy Gradient(DDPG).This framework adaptively adapts RIS configurations and transmitter beamforming to reduce key challenges,including imperfect channel state information(CSI)and hardware limitations like quantized RIS phase shifts.By optimizing both RIS settings and beamforming in real-time,our approach outperforms traditional methods by significantly increasing secrecy rates,improving spectral efficiency,and enhancing energy efficiency.Notably,this framework adapts more effectively to the dynamic nature of wireless channels compared to conventional optimization techniques,providing scalable solutions for large-scale RIS deployments.Our results demonstrate substantial improvements in communication performance setting a new benchmark for secure,efficient and scalable 6G communication.This work offers valuable insights for the future of IoT networks,with a focus on computational optimization,high spectral efficiency and energy-aware operations.
文摘This study was a systematic review of the available evidence on quality of life in patients after laparoscopic or open colorectal surgery. A systematic review was performed of all randomized clinical trials (RCTs) that compared laparoscopic with open colorectal surgery. Study selection, quality assessment and data extraction were carried out independently by two reviewers. Primary endpoint was quality of life after laparoscopic and open colorectal surgery, as assessed by validated questionnaires. The search resulted in nine RCTs that included 2263 patients. Shortand long-term results of these RCTs were described in 13 articles. Postoperative follow-up ranged from 2 d to 6.7 years. Due to clinical heterogeneity, no meta-analysis could be conducted. Four RCTs did not show any difference in quality of life between laparoscopic or open colorectal surgery. The remaining five studies reported a better quality of life in favor of the laparoscopic group on a few quality of life scales at time points ranging from 1 wk to 2 years after surgery. In conclusion, based on presently available high-level evidence, this systematic review showed no clinically relevant differences in postoperative quality of life between laparoscopic and open colorectal surgery.

文摘OBJECTIVE:To assess the efficacy and safety of extracorporeal shockwave therapy(ESWT)combined with sodium hyaluronate(HA)for the treatment of knee osteoarthritis(KOA).METHODS:PubM ed,Embase,the Cochrane Library,Web of Science,China National Knowledge Infrastructure,Wanfang database,China Science and Technology Journal Database,and SinoMed were searched from inception to July 2020.The quality of the randomized controlled trials was evaluated independently by two reviewers according to the criteria in the Cochrane Collaboration for Systematic Reviews.The identified articles were then screened individually using EndnoteX9 for eligibility in this Meta-analysis.The heterogeneity among the articles was evaluated using I2.RESULTS:A total of 17 studies,comprising 2000 individuals,were included in this Meta-analysis.The results showed that a significant improvement was observed in knee pain and function based on the clinical efficacy of ESWT combined with HA.Statistical analysis of clinical efficacy showed that[relative risk(RR)=1.21,95%confidence interval(CI)(1.12,1.30),P<0.01].Statistical analysis of visual analog scale showed that[standardized mean difference(SMD)=-2.84,95%CI(-4.01,-1.66),P<0.01].Western Ontario and McMaster University osteoarthritis index statistical analysis showed that[SMD=-1.57,95%CI(-2.52,-0.61),P<0.01].Lysholm score statistical analysis showed that[SMD=1.71,95%CI(0.98,2.44),P<0.01].In addition,only minor side effects,such as redness and swelling of the skin,were observed.CONCLUSIONS:Medium to low quality evidence showed that ESWT combined with HA offers an inexpensive,welltolerated,safe,and effective method to improve pain and functionality in patients with KOA.However,tightly controlled,randomized,large multicenter trials are warranted to validate the current findings.

文摘Background: Minerals bioaccessibility of food products could be increased by enhancing the apparent absorption of most minerals with the reduction of anti-nutritional factors (phytates) through extrusion cooking. The aim of the study was to increase the mineral bioaccessibility in co-extruded millet flours enriched with Moringa and Baobab for vulnerable populations. Methods: Three extruded instant formulas were developed using pearl millet, Moringa and Baobab powders: FA (90% Millet + 10% Baobab);FB (90% Millet + 10% Moringa);FC (80% Millet + 10% Baobab + 10% Moringa). Non-extruded formulations of FA, FB and FC were used as controls. Then treatments and controls were analyzed to determine their percent mineral bioaccessibility using the in vitro equilibrium dialysability method (Miller et al., 1981) and their total amounts bioaccessible according to the Burgos et al., 2018 method. Phytates in all samples were also determined using the Method of Fruhbeck et al., 1995. Results: Extrusion cooking significantly improved iron bioaccessible percentages in co-extruded flours respectively in FB and FC (p . Extrusion significantly reduced the percentages and amounts of bioaccessible zinc and calcium in all extruded flours compared to their controls (p Conclusion: Extrusion cooking reduced the chelation effect of phytates by thermal degradation, which led to a modification in the bioavailability of minerals. Food-to-food fortification and extrusion cooking displayed a positive effect on the bioaccessibility of iron, magnesium and phosphorus. For calcium and zinc, extrusion has no positive effect on their bioaccessibilities. However, with daily consumption, co-extruded fortified flours could be used as a way to fight against malnutrition in vulnerable populations.
文摘Background:Identifying and managing patients'supportive care needs is an essential component of health care.The cancer patients'needs are multidimensional and it is important to identify these needs.This study was conducted to assess supportive care needs of women with gynecologic cancer and affecting factors.Methods:This cross-sectional study consisted of 98 women with gynecologic cancer.The data of the study was gathered using“Personal Information Form”and the“James Supportive Care Screening”.Results:The most frequently reported unmet needs of gynecologic cancer patients were emotional issues,including uncertainty(87.6%),fears(85.7%),feeling down(86.7%),worry(87.6%),loss of interest in usual activities(73.3%)and physical symptoms including fatigue/lack of energy(81.9%),weakness(81.9%),lack of appetite(76.2%).The most frequently reported social/practical problems was transportation problems(50%).The mean scores for JSCS was 29.72±15.57.There were a statistically significant difference was found only between family type,stage of the disease,and time of diagnosis,and the total mean scores of the JSCS(p<0.05).Conclusion:It has been determined that patients with gynecological cancer mostly experience emotional and physical problems.Health professionals working in the oncology unit should be aware of unmet needs.Certain programs and services to address the identified unmet needs should be provided.

文摘AIM To characterise venous thromboembolism(VTE) in gastrointestinal cancer and assess the clinical utility of risk stratification scoring. METHODS We performed a retrospective analysis using electronic patient records of 910 gastro-oesophageal(GO) cancer and 1299 colorectal cancer(CRC) patients referred to a tertiary cancer centre to identify the incidence of VTE, its relationship to chemotherapy and impact on survival.VTE risk scores were calculated using the Khorana index. Patients were classified as low risk(0 points), intermediate risk(1 to 2 points) or high risk(3 points). Data was analysed to determine the sensitivity of the Khorana score to predict VTE. RESULTS The incidence of VTE was 8.9% for CRC patients and 9.7% for GO cancer patients. Pulmonary emboli(PE) were more common in advanced than in localised CRC(50% vs 21% of events respectively) and lower limb deep vein thrombosis(DVT) were more common in localised than in advanced CRC(62% vs 39% of events respectively). The median time to VTE from cancer diagnosis was 8.3 mo for CRC patients compared to 6.7 mo in GO cancer. In localised CRC median time to VTE was 7.1 mo compared with 10.1 mo in advanced CRC. In contrast in GO cancer, the median time to VTE was 12.5 mo in localised disease and 6.8 mo in advanced disease. No survival difference was seen between patients with and without VTE in this cohort. The majority of patients with CRC in whom VTE was diagnosed had low or intermediate Khorana risk score(94% for localised and 97% in advanced CRC). In GO cancer, all patients scored either intermediate or high risk due to the primary site demonstrating a limitation of the risk assessment score in discriminating high and low risk patients with GO cancers. Additional risk factors were identified in this cohort including surgery, chemotherapy or hospital admission. Overall, 81% of patients with CRC and 77% of patients with GO cancer had one or more of these factors within 4 wk prior to diagnosis VTE. These should be factored into clinical risk assessment scores. CONCLUSION The Khorana score has low sensitivity for thrombotic events in CRC and cannot discriminate low risk patients in high risk cancer sites such as GO cancer.
文摘Objective:To study the isolation and characterization of the constituent responsible for the cytotoxic activity of the ethanolic extract of stem of Capparis decidua(C.decidua).Methods:The preliminary cytotoxic effect of isolated compound(β-Sitosterol triacontenate) was investigated by MTT assay on A549 solid tumor cells.Results:IC<sub>50</sub> value of theβ-Sitosterol triacontenate was found to be 1μM.The cytotoxic activity increased in a dose dependent manner in case ofβ-Sitosterol triacontenate.Conclusions:The data therefore provide direct evidence for the role ofβ-Sitosterol triacontenate as a potent antimetastatic agent,which can markedly inhibit the metastatic and invasive capacity of malignant cells.
文摘Two dynamic grey models DGM (1, 1) for the verification cycle and the lifecycle of measuring instrument based on time sequence and frequency sequence were set up, according to the statistical feature of examination data and weighting method. By a specific case, i.e. vernier caliper, it is proved that the fit precision and forecast precision of the models are much higher, the cycles are obviously different under different working conditions, and the forecast result of the frequency sequence model is better than that of the time sequence model. Combining dynamic grey model and auto-manufacturing case the controlling and information subsystems of verification cycle and the lifecycle based on information integration, multi-sensor controlling and management controlling were given. The models can be used in production process to help enterprise reduce error, cost and flaw.
文摘In this study,a convenient method of preparing the substrate is proposed with one-pot synthesis of silver colloid under body heat,and the SERS detection uses the fresh substrate to avoid the drawback of substrates’short life of use.The synthesis of silver colloid is carried out in a 10 mL vial by using ascorbic acid as a reductant and trisodium citrate as a stabilizer.The vial is grasped with the palm of the experimenter for several minutes without shaking.The proposed method is simple,rapid,green energy and cost-effective.By adjusting the concentration of trisodium citrate,not only the particle size can be controlled from about 110 nm to 50 nm but also the homogeneity of nanoparticles can be improved.As a SERS substrate,the silver colloid has high batch reproducibility and showed good SERS activity.The relative standard deviation between different manufacturers is 5.51%when the substrate of silver colloid is used for the detection of rhodamine 6 G.Using the substrate,the lowest detection concentrations of rhodamine 6 G,crystal violet,enrofloxacin,melamine and leucomalachite green are 1.0×10-8,6.1×10-8,1.4×10-6,7.1×10-5 and 5.1×10-8 mol/L,respectively.Results demonstrate that the developed method has the advantage of convenience and high efficiency in the field preparation of reliable SERS substrate.

文摘Gold nanoparticle arrays fabricated via layerby-layer technique were investigated using grazing-incidence small-angle X-ray scattering(GISAXS) method.Samples containing two gold nanoparticle layers that were separated by 5,11,15 and 21 poly electrolyte(PE) interlayers were studied.By using different X-ray incident angles,correlations of gold nanoparticles(GNPs) in the same layer and in two different layers were investigated.It is found that both sideway correlations between GNPs in the same layer and vertical correlation between two gold nanoparticle layers depend on the thickness of PE interlayers.According to sideway correlation,the size of GNPs is determined to be(13.0±0.5) nm in all of the four samples,which was also proved by scanning electron microscopy(SEM) and theoretical calculation of form factor of spherical particles.From vertical correlation,distance between two gold nanoparticle layers was determined for sample with 11,15 and 21 PE layers.These distances can be reasonably explained with the number of PE layers and the thickness of single PE layer.These results indicate that by repeated depositing of oppositely charged PE layers,a true three-dimensional(3 D) nanostructure can eventually be designed.
文摘The material flow in friction stir welded 2014 Al alloy has been investigated using a marker insert technique (MIT). Results of the flow visualization show that the material flow is asymmetrical during the friction stir welding (FSW) process and there are also significant differences in the flow patterns observed on advancing side and retreating side. On advancing side, some material transport forward and some move backward, but on retreating side, material only transport backward. At the top surface of the weld, significant material transport forward due to the action of the rotating tool shoulder. Combining the data from all the markers, a three-dituensional flow visualization, similar to the 3D image reconstruction technique, was obtained. The three-dimensional plot gives the tendency chart of material flow in friction stir welding process and from the plot it can be seen that there is a vertical, circular motion around the longitudinal axis of the weld. On the advancing side of the weld, the material is pushed downward but on the retreating side, the material is pushed toward the crown of the weld. The net result of the two relative motions in both side of the advancing and the retreating is that a circular motion comes into being. Comparatively, the material flow around the longitudinal axis is a secondary motion.

Satisfactory results showed that, as new amino acid scales, data of HSEHPCSV may be a useful structural expression methodology'for the studies on peptide QSAR (quantitative structure-activity relationship) due to many advantages such as plentiful structural information, definite physical and chemical meaning and easy interpretation.
