However, investigating the underlying effect of orthodontic forces shows various molecular and cellular changes. Also, orthodontics is in close relation with dentofacial orthopedics which involves bone regeneration. In this review current and future applications of stem cells(SCs) in orthodontics and dentofacial orthopedics have been discussed. For craniofacial anomalies, SCs have been applied to regenerate hard tissue(such as treatment of alveolar cleft) and soft tissue(such as treatment of hemifacial macrosomia). Several attempts have been done to reconstruct impaired temporomandibular joint. Also, SCs with or without bone scaffolds and growth factors have been used to regenerate bone following distraction osteogenesis of mandibular bone or maxillary expansion. Current evidence shows that SCs also have potential to be used to regenerate infrabony alveolar defects and move the teeth into regenerated areas. Future application of SCs in orthodontics could involve accelerating tooth movement, regenerating resorbed roots and expanding tooth movement limitations. However, evidence supporting these roles is weak and further studies are required to evaluate the possibility of these ideas.展开更多

The populations from Southeast Asian countries (Chinese and Malaysian) showed the highest prevalence rate of 15.8%, which can seriously affect oral function, facial appearance, and mental health. As anterior crossbite tends to worsen with growth, early orthodontic treatment can harness growth potential to normalize maxillofacial development or reduce skeletal malformation severity, thereby reducing the difficulty and shortening the treatment cycle of later-stage treatment. This is beneficial for the physical and mental growth of children. Therefore, early orthodontic treatment for Class Ⅲ malocclusion is particularly important. Determining the optimal timing for early orthodontic treatment requires a comprehensive assessment of clinical manifestations, dental age, and skeletal age, and can lead to better results with less effort. Methods: A finite element model was constructed after extracting the mandibular first premolar, and a long-arm bracket with a traction height of 6 mm was placed on the labial side of the mandibular canine. Three working conditions of 50 g, 100 g, and 150 g were simulated, and the magnitude and distribution of von Mises stress in the periodontal ligament were compared for each condition. Results: The maximum von Mises stress in the periodontal ligament was 0.013281 MPa in the 50 g condition, 0.02536 MPa in the 100 g condition, and 0.035549 MPa in the 150 g condition. As the orthodontic force increased, the stress distribution area in the periodontal ligament also expanded. Conclusion: A 100 g orthodontic force is the most suitable when using long-arm brackets, providing a relatively uniform stress distribution in the periodontal ligament and keeping the stress within a reasonable range.展开更多

文摘Protrusive facial deformities, characterized by the forward displacement of the teeth and/or jaws beyond the normal range, affect a considerable portion of the population. The manifestations and morphological mechanisms of protrusive facial deformities are complex and diverse, requiring orthodontists to possess a high level of theoretical knowledge and practical experience in the relevant orthodontic field. To further optimize the correction of protrusive facial deformities, this consensus proposes that the morphological mechanisms and diagnosis of protrusive facial deformities should be analyzed and judged from multiple dimensions and factors to accurately formulate treatment plans. It emphasizes the use of orthodontic strategies, including jaw growth modification, tooth extraction or non-extraction for anterior teeth retraction, and maxillofacial vertical control. These strategies aim to reduce anterior teeth and lip protrusion, increase chin prominence, harmonize nasolabial and chin-lip relationships, and improve the facial profile of patients with protrusive facial deformities. For severe skeletal protrusive facial deformities, orthodonticorthognathic combined treatment may be suggested. This consensus summarizes the theoretical knowledge and clinical experience of numerous renowned oral experts nationwide, offering reference strategies for the correction of protrusive facial deformities.

文摘Stem cells play a crucial role in maintaining tissue regenerative capacity and homeostasis. However, mechanisms associated with stem cell senescence require further investigation. In this study,we conducted a proteomic analysis of human dental pulp stem cells(HDPSCs)obtained from individuals of various ages. Our findings showed that the expression of NUP62 was decreased in aged HDPSCs.We discovered that NUP62 alleviated senescence-associated phenotypes and enhanced differentiation potential both in vitro and in vivo.Conversely, the knocking down of NUP62 expression aggravated the senescence-associated phenotypes and impaired the proliferation and migration capacity of HDPSCs.Through RNA-sequence and decoding the epigenomic landscapes remodeled induced by NUP62 overexpression,we found that NUP62 helps alleviate senescence in HDPSCs by enhancing the nuclear transport of the transcription factor E2F1.This,in turn, stimulates the transcription of the epigenetic enzyme NSD2.Finally,the overexpression of NUP62 influences the H3K36me2 and H3K36me3 modifications of anti-aging genes(HMGA1,HMGA2,and SIRT6).Our results demonstrated that NUP62 regulates the fate of HDPSCs via NSD2-dependent epigenetic reprogramming.
文摘BACKGROUND This case report presents an innovative approach to managing a complex class II division 1 malocclusion with ectopic maxillary canines using a modified twin block(TB)appliance. The modification facilitated simultaneous skeletal correction and canine eruption, reducing treatment time and improving patient satisfaction.CASE SUMMARY A 14-year-old male presented with concerns about a retruded chin and spacing between teeth.Clinical and radiographic evaluation revealed a class II division 1 malocclusion on a moderate class II skeletal base, with palatally impacted maxillary canines.Treatment involved a modified Clark TB appliance designed with hooks to allow active traction of the canines, followed by comprehensive fixed orthodontics.Over 18 months, the patient achieved improved occlusion, corrected overjet and overbite, successful eruption of the impacted canines, and enhanced facial aesthetics. The final results showed significant dental and skeletal improvement without requiring surgical intervention.CONCLUSION A modified TB appliance can effectively manage class II malocclusion with impacted canines in growing patients. The modified approach combining functional mandibular advancement and active canine traction represents a lessdocumented clinical adaptation, offering a valuable, efficient alternative to traditional two-phase interventions.
文摘Objective: To compare the stress distribution in the periodontal ligament under different orthodontic forces during canine distalization using long-arm brackets, and to determine the optimal force value for this device in orthodontic treatment. Conclusion: A 100 g orthodontic force is the most suitable when using long-arm brackets, providing a relatively uniform stress distribution in the periodontal ligament and keeping the stress within a reasonable range.

文摘Promotion of angiogenesis is crucial for bone tissue repair, and the poor activity of angiogenic cells and growth factors is the main problem in angiogenesis.New proangiogenic nanomaterials are urgently needed to be a promising strategy for this issue.Nb promotes bone formation and fracture healing, possibly by increasing vascular endothelial growth factor(VEGF)production.Nanoniobium particles(nNb)may promote angiogenesis. However, the effect of nNb on angiogenesis is unclear, limiting its application. This study confirmed that nNb significantly promoted angiogenesis.nNb increased and Ras-related C3 botulinum toxin substrate(Rac)family small guanosine triphosphatase(GTPase)1(Rac1)expression,inducing F-actin aggregation at the front edge of cells and the formation of pseudopodia to mediate cell migration, further promoting angiogenesis. We discovered that cyclin-dependent kinase-like 5(CDKL5)is a new signaling molecule that activates Rac1.V-ets erythroblastosis virus E26 oncogene homolog(ETS)domain-containing protein(ELK1),regulating CDKL5 and Rac1,plays an upstream regulatory role. When ELK1 was inhibited,CDKL5 and Rac1 levels were decreased.ELK1,CDKL5 or Rac1 are effective regulatory targets of angiogenesis.Inhibiting expression of ELK1,CDKL5 or Rac1 decreased angiogenesis. Thus,nNb has good angiogenic effects. The ELK1-CDKL5-Rac1 signaling pathway regulates the migration of endothelial cells to promote angiogenesis.nNb can be used in bone tissue engineering as a new nanomaterial, and it will promote the development of a new strategy for tissue engineering.

文摘Decorin(DCN)is primarily found in the connective tissues of various parts of the body, including the lungs, kidneys, bone tissue, aorta, and tendons. It is an important component of the extracellular matrix(ECM)and belongs to the class I small leucine-rich proteoglycans family.DCN is increasingly attracting attention due to its significant role in tumors, fibrotic diseases, and the regulation of vascular formation. Moreover, its anti-tumor properties have positioned it as a promising biomarker in the fight against cancer.Numerous studies have confirmed that DCN can exert inhibitory effects in various solid tumors, particularly in oral squamous cell carcinoma(OSCC),by activating its downstream pathways through binding with the epidermal growth factor receptor(EGFR)and mesenchymal-epithelial transition(MET)receptor,or by stabilizing and enhancing the expression of the tumor suppressor gene p53 to mediate apoptosis in cancer cells that have undergone mutation. The occurrence of OSCC is a continuous and dynamic process, encompassing the transition from normal mucosa to oral potentially malignant disorders(OPMDs),and further progressing from OPMDs to the malignant transformation into OSCC.We have found that DCN may exhibit a bidirectional effect in the progression of oral mucosal carcinogenesis, showing a trend of initial elevation followed by a decline, which decreases with the differentiation of OSCC.In OPMDs,DCN exhibits high expression and may be associated with malignant transformation, possibly linked to the increased expression of P53 in OPMDs.In OSCC,the expression of DCN is reduced, which can impact OSCC angiogenesis, and inhibit tumor cell proliferation, migration, and invasion capabilities, serving as a potential marker for predicting adverse prognosis in OSCC patients. This article reviews the current research status of DCN,covering its molecular structure, properties, and involvement in the onset and progression of oral mucosal carcinogenesis. It elucidates DCN’s role in this process and aims to offer insights for future investigations into its mechanism of action in oral mucosal carcinogenesis and its potential application in the early diagnosis and treatment of OSCC.

文摘Osteoarthritis(OA)is a degenerative joint disease accompanied with the loss of cartilage and consequent nociceptive symptoms.Normal articular cartilage maintains at aneural state.Neuron guidance factor Semaphorin 3A(Sema3A)is a membrane-associated secreted protein with chemorepulsive properties for axons. However, the role of Sema3A in articular cartilage is still not clear. In the present studies,we investigated the functions of Sema3A in OA development in mice,non-human primates, and patients with OA.Sema3A has a protective effect on cartilage degradation, validated by the organoid culture in vitro and confirmed in chondrocyte-specific Sema3A conditional knockout mice.We demonstrated that Sema3A is a key molecule in maintaining cartilage homeostasis from chondrocyte hypertrophy via activating the PI3K pathway. The potential usage of Sema3A for OA treatment was validated in mouse and Rhesus macaque OA models through intra-articular injection of Sema3A,and also in patients by administering Sema3A containing platelet-rich plasma into the knee joints. Our studies demonstrated that Sema3A exerts a critical role in inhibiting neurite ingrowth and preventing chondrocyte hypertrophy in cartilage, and could be potentially used for OA treatment.
