The objective of this work was to help identify the determinants this morbidity and mortality. Materials and Methods: A prospective study, over a 4-month period (January to April 2020). All patients aged 0 to 15 years, hospitalized for emergency reasons in the Albert Royer emergency department, were included. Mortality was analyzed according to sociodemographic data, the patient’s itinerary, transfer procedures, availability of emergency medications, diagnosis made during hospitalization and causes of death. The data were collected based on a protocol in an established file and analyzed by SPSS (Statistical Package for Science Social) software. Version 18. Results: 225 children in emergency situations were included. The age of our patients is between 0 and 15 years (minimum 0.23 months, maximum 191 months, average 47 months and standard deviation 54). Thus, emergencies represented 57.98% of the 388 hospitalizations. Seventy percent of patients were less than 60 months old with a male predominance (sex ratio 1.16), 2.22% were newborns and 27.11% were aged between 60 and 191 months). A total of 79 (35.1%) were transferred and only 57.4% received care before transfer. A total of 12 deaths were reported, representing a lethality of 5.3%. Only young age (less than 59 months, a mortality rate of 91.7%), cardiac decompensations, severe sepsis, and neonatal conditions were more associated with mortality. The majority of our deceased patients came from families with a low socio-economic level (83.3%). Conclusion: Pediatric emergencies are frequent and responsible for lethality and require the necessary efforts, particularly through parent education, the creation of emergency outpatient care units and, above all, the strengthening of the technical platform and therapeutic means.展开更多

It is characterized by an oedematous, multifocal and bilateral degeneration of the nasosinus mucosa, which originates in the lateral masses of the ethmoid, where it causes the formation of smooth, gelatinous, translucent and pyriform polyp lesions. The objective of this study was to review epidemiological, clinical, paraclinical data and evaluate the results of endoscopic surgical treatment. Patients and Methods: This is a retrospective study on 60 patients followed at the ENT department of the Fann National University Hospital Center, from January 2010 to December 2015. All patients with sinonasal polyposis were included in the study. Results: The average age of our patients was 38 years and the sex ratio (M/F) was 0.8. In the patients’ histories, we found 18% asthma and 10% Widal’s disease. The average consultation time was 8.5 years. All patients had consulted for nasal obstruction;rhinorrhea was bilateral and found in 67.7% of cases, with olfactory disorders accounting for 50%. The CT scan performed in 58% of cases made it possible to specify the extent of the lesions;the involvement of the ethmoidal sinus was constant and extended to the other sinuses except in 2 cases. All patients had received medical treatment with local corticosteroids. Endoscopic surgical treatment was initiated in 43% of cases after failure of corticosteroid-based medical treatment. The evolution under treatment marked by the reappearance of symptoms that increased each month. At one month postoperatively, all clinical symptomatology had improved with the exception of olfactory disorders, which persisted in 3 patients. At 12 months we noted 12 cases of reappearance of nasal obstruction. Conclusion: SNP is a disease of little known etiology. The diagnosis is almost always clinical. Endoscopic surgery remains a recourse to medical treatment. For good local control, patients should be more respectful of good compliance with corticosteroid therapy.展开更多

Objective: This work aimed to describe the epidemiological profile of AD in children aged 0 to 15 years in Libreville. Materials and Methods: All children aged 0 to 15 years who were victims of unintentional trauma occurring at home or in its immediate surroundings were included. We studied the mother’s age, family situation, socioeconomic level, type of housing, age and sex of the child, characteristics of AD and their management. Results: The majority of mothers lived in an intermediate dwelling (80.6%). They were married (37.1%), middle managers (58.2%) and of average socioeconomic level (60.5%). The average age of the mothers was 39.9 ± 11.4 years. Families with more than three children were most exposed (39.2%) to the occurrence of AD. The average age of the children was 6.5 ± 3.3 years with a male predominance. The sex ratio was 1.8. The most common ADs were falls (34.7%), followed by cuts (22.3%) and burns (17.7%). Wounds (54.4%), followed by burns (33%) and fractures (5.1%) were the main types of injuries. The upper limbs were the most affected body part (33.9%) followed by the lower limbs (30.1%) and the head (27.3%). The yard was the preferred location for ADs to occur (51.1%), and particularly during the holiday period (48.4%). The risk factors related to the occurrence of AD were age, socioeconomic level, number of children and type of housing. Care was provided at home in 51.9% of cases. Conclusion: The occurrence of AD in children is not negligible;hence the need to implement preventive measures to minimize their frequency.展开更多

Objective Size distribution is an important biophysical property of extracellular vesicles(EVs).EVs include small EVs(s-EVs)and large EVs(l-EVs)by size.Differential ultracentrifugation(dUC)is widely used to separate E...Objective Size distribution is an important biophysical property of extracellular vesicles(EVs).EVs include small EVs(s-EVs)and large EVs(l-EVs)by size.Differential ultracentrifugation(dUC)is widely used to separate EVs from biofluids,but it can precipitate large impurity particles.Dynamic light scattering(DLS)is a simple and fast method for analyzing the size distribution of EVs.However,this approach is nonideal for heterogeneous and polydisperse samples since a small quantity of large impurity particles can markedly disturb the DLS results.Here,we developed a simple method to improve the reliability of DLS measurements.Methods Plasma was obtained from 13 volunteers.The plasma was first processed by dUC to obtain crude l-EVs.The crude l-EVs were filtered with syringe filters(pore size of 1μm and membrane material of hydrophilic polyvinylidene fluoride(PVDF))to remove large impurity particles from l-EVs.The size distributions of the crude l-EVs and filtered l-EVs were measured via DLS.Results After the samples were filtered,the coefficients of variation of the hydrodynamic radius and Peak 1 intensity of the filtered l-EVs decreased from 20.39%(12.76–28.96%)and 20.44%(14.58–28.32%)to 3.05%(1.79–4.72%)and 3.43%(1.76–5.88%),respectively,compared with those of the crude l-EVs.Conclusion These findings suggest that filtration can effectively separate circulating l-EVs in plasma to remove large impurity particles and make samples suitable for characterization by DLS.Our findings provide a simple method to improve precision via DLS to measure the size distribution of EVs.展开更多

As such, conventional methods for DNA extraction of somatic cells may not be suitable for obtaining sperm DNA. In this study, we evaluated and compared three sperm DNA extraction techniques, namely, modified guanidinium thiocyanate method (method A), traditional phenol-chloroform method (method B), and TianGen kit method (method C). Spectrophotometry and agarose gel electrophoresis analyses showed that method A produced DNA with higher quantity and purity than those of methods B and C (P〈0.01). PCR results revealed that method A was more reliable in amplifying DEAD-box polypeptide 4 (DDX4) and copy number variations (CNVs) than methods B and C, which generated false-positive errors. The results of sperm DNA methylation assay further indicated that methods A and B were effective, and the former yielded higher quantitative accuracy. In conclusion, the modified guanidinium thiocyanate method provided high quality and reli- able results and could be an optimal technique for extracting sperm DNA for methylation assay.展开更多

This study established a pregnant rat model of hyperhomocysteinemia, in which blood plasma homocysteine concentrations were twice or three times greater than that of normal pregnant rats. TUNEL revealed an increase in the number of apoptotic cells in the frontal cortex of pregnant rats with hyperhomocysteinemia. In addition, immunohistochemical staining detected activated nuclear factor-KB-positve cells in the frontal cortex. Reverse transcription-PCR detected that mRNA expression of the anti-apoptotic gene bcl-2 diminished in the frontal cortex. In situ hybridization and western blotting revealed that N-methyi-D- aspartate receptor 1 mRNA and protein expression was upregulated in the frontal cortex and hippocampus. These results indicate that hyperhomocysteinemia can induce brain cell apoptosis, increase nerve excitability, and promote the occurrence of preeclampsia in pregnant rats.展开更多

In the present prospective study we investigated the clinical characteristics and emergency management of children with severe encephalitis associated with NPE caused by enterovirus 71.METHODS: The study was conducted in 2 pediatric intensive care units (PICUs) over a 2-month period. Clinical records were reviewed of critically ill children with severe encephalitis associated with NPE caused by EV71 who were admitted to PICUs during the period of May to June 2008 in Fuyang.RESULTS: We reviewed the complete records of 36 children, of whom 23 (63.9%) were male and 13 (36.1%) female. Their age ranged from 4 to 48 months, with an average of 15.8 months. All children except one were under 3 years of age. The overall mortality in these children was 19.4%. The average duration of critical life threatening signs and symptoms was 2.1 days (12 hours-5 days). Nervous system diseases included brainstem encephalitis in 27 children (75%), brainstem encephalitis associated with myelitis in 6 children (16.7%), and general encephalitis in 3 chidren (8.3%), respectively. In 12 patients of NPE (33.3%) pink or bloody bubble sputum and asymmetric pulmonary edema or hemorrhage was the primary manifestation but no typical exanthema was observed. Five children died of acute onset of NPE and / or pulmonary hemorrhage with rapid progression of cardiopulmonary failure within hours after admission. Therapeutic management consisted of mechanical ventilation and administration of mannitol, methylprednisolone, intravenous immunoglobulin (IVIG) and vasoactive drugs, associated with the need of fluid volume resuscitation in 9 (25%) of the 36 children.CONCLUSION: In children less than 3 years of age found to be affected by severe EV71 encephalitis associated with NPE, one fifth may die. The major organ systems infected by severe EV71 include the central nervous system, the respiratory system, and the cardiovascular system. Early diagnosis and evaluation, respiratory support, treatment of intracranial hypertension, and mainttenance of function of the cardiovascular system are the most important therapeutic measures.展开更多
Low birth weight(LBW) and preterm birth(PB) are associated with newborn mortality and diseases in adulthood.We explored factors related to LBW and PB by conducting a population-based case-control study from Januar...Low birth weight(LBW) and preterm birth(PB) are associated with newborn mortality and diseases in adulthood.We explored factors related to LBW and PB by conducting a population-based case-control study from January 2011 to December 2013 in Wuhan,China.A total of 337 LBW newborn babies,472 PB babies,and 708 babies with normal birth weights and born from term pregnancies were included in this study.Information of newborns and their parents was collected by trained investigators using questionnaires and referring to medical records.Univariate and logistic regression analyses with the stepwise selection method were used to determine the associations of related factors with LBW and PB.Results showed that maternal hypertension(OR=6.78,95% CI:2.27–20.29,P=0.001),maternal high-risk pregnancy(OR=1.53,95% CI:1.06–2.21,P=0.022),and maternal fruit intake ≥300 g per day during the first trimester(OR=1.70,95% CI:1.17–2.45,P=0.005) were associated with LBW.BMI ≥24 kg/m^2 of mother prior to delivery(OR=0.48,95% CI:0.32–0.74,P=0.001) and gestation ≥37 weeks(OR=0.01,95% CI:0.00–0.02,P〈0.034) were protective factors for LBW.Maternal hypertension(OR=3.36,95% CI:1.26–8.98,P=0.016),maternal high-risk pregnancy(OR=4.38,95% CI:3.26–5.88,P〈0.001),maternal meal intake of only twice per day(OR=1.88,95% CI:1.10–3.20,P=0.021),and mother liking food with lots of aginomoto and salt(OR=1.60,95% CI:1.02–2.51,P=0.040) were risk factors for PB.BMI ≥24 kg/m^2 of mother prior to delivery(OR=0.66,95% CI:0.47–0.93,P=0.018),distance of house from road ≥36 meters(OR=0.72,95% CI:0.53– 0.97,P=0.028),and living in rural area(OR= 0.60,95% CI:0.37–0.99,P=0.047) were protective factors for PB.Our study demonstrated some risk factors and protective factors for LBW and PB,and provided valuable information for the prevention of the conditions among newborns.展开更多
AIM:To study the change of torsion in both eyes after unilateral inferior oblique(IO)weakening on children with congenital superior oblique palsy(SOP).METHODS:This retrospective study enrolled all patients diagnosed w...AIM:To study the change of torsion in both eyes after unilateral inferior oblique(IO)weakening on children with congenital superior oblique palsy(SOP).METHODS:This retrospective study enrolled all patients diagnosed with unilateral congenital superior oblique palsy(UCSOP)accompanied by inferior oblique overaction(IOOA).A total of 120 eyes of 60 patients were divided into group 1(more extorted paretic eye)and group 2(more extorted nonparetic eye).The degree of fundus torsion was evaluated before and 1 mo after the IO weakening procedure.The torsion of the fundus was recorded by measuring the diskfoveal angle(DFA)using fundus photography.RESULTS:Group 1 included 26 cases and group 2 included 34 cases,thus the rate of extorsion was insignificantly higher in the nonparetic eye(P=0.10).The preoperative DFA in the paretic and nonparetic eyes was 13.21±5.95,7.97±4.25 in group 1,and 4.65±3.79,13.16±5.35 in group 2(both P<0.001).The postoperative DFA in the paretic and nonparetic eyes was 8.57±4.87,7.32±4.27 in group 1(P=0.24),and 3.85±6.00 and 9.94±5.45 in group 2(P<0.001).The amount of postoperative reduction of the DFA in the paretic and nonparetic eyes was 4.64±3.90,0.65±0.76 in group 1(P=0.002),and 0.80±0.81,3.21±5.50 in group 2(P=0.01).The difference in the amount of reduction of DFA in the more extorted eye in group 1(paretic eye)vs group 2(nonparetic eye)was insignificant(P=0.30).CONCLUSION:Excyclotorsion in the nonparetic eye has a similar probability in the paretic eye in UCSOP children,and weakening of the ipsilateral IO has a more obvious effect on the decrement of extorsion in the more extorted eye regardless of which eye is paretic.展开更多

As the essential downstream effector of Notch signaling pathway, Hes1, and its dynamic expression, plays an essential role on maintaining neural progenitor/stem cells(NPCs) cell fate and fetal brain development. In the present study, we reported the first observation of Hes1 oscillatory expression in human NPCs, with an approximately1.5 hour periodicity and a Hes1 protein half-life of about 17(17.6 ± 0.2) minutes. HCMV infection disrupts the Hes1 rhythm and down-regulates its expression. Furthermore, we discovered that depleting Hes1 protein disturbed NPCs cell fate by suppressing NPCs proliferation and neurosphere formation, and driving NPCs abnormal differentiation. These results suggested a novel mechanism linking disruption of Hes1 rhythm and down-regulation of Hes1 expression to neurodevelopmental disorders caused by congenital HCMV infection.展开更多

Of great concern are the data pointing to the recent increase in the prevalence of obesity irregardless of age group and country. Overweight and obesity in adolescence are markers of overweight and obesity in adults, respectively. Very little data are currently available on the prevalence of childhood obesity in Iran, and more research on the risk factors is required before preventive public health programs can be for mulated and put into practice. Objective: The objective of this study was to quantify the prevalence of overweight and obesity and their associated factors in adolescent children living in Tehran.Materials and methods: During a multistage stratified cluster sampling, 2900 students (1200 males and 1700 females) aged 11-17 years were selected from 20 secondary schools in the school year of 2004-2005. A questionnaire was filled, and weight and height were measured. Discussions and conclusions:The body mass index (BMI) was calculated and adjusted for age and sex. Prevalences of overweight and obesity were 17.9 and 7.1%, respectively. BMI increased with age, and it was higher in those who had lower levels of physical activity.Age at menarche was negatively associated with BMI. There was no relationship between macro-and micronutrient intake and overweight and obesity. This study highlights the high prevalence of overweight and obesity in adolescent children in Tehran.展开更多

A neonatal rat model of hypoxic-ischemic white matter damage was established via bilateral carotid artery ligation in 4-day-old Sprague-Dawley rats. The rats were subsequently treated with recombinant human erythropoietin to observe pathological changes in the brain and long-term neurobehavioral functions before and after intervention. Results showed that the number of myelin basic protein-positive cells, which reflected myelin/oligodendrocyte damage, significantly increased, although the number of amyloid precursor protein-positive cells, which reflected axonaf injury, significantly decreased in periventricular white matter at 72 hours and 7 days following erythropoietin intervention. The number of glial fibrillary acidic protein-positive cells, indicating astrocytic damage, significantly decreased in periventricular white matter of erythropoietin-treated rats at 48 hours, 72 hours, 7 days and 26 days. Following erythropoietin intervention in the 30-day-old rats, head-turning time in the slope test was shortened and open-field test scores increased. These results suggested that erythropoietJn promoted repair of white matter damage, as well as improved neurobehavioral functions in a rat model of hypoxic-ischemic injury.展开更多

Capital flexion initiates the straightening of the cervical spine which causes the connection of the head on the C1-C2 suboccipital part to the thoracic and lumbar parts. With this, the spine starts to move and postural tone increases. Without construction of the neck or alteration of the axis, as seen with cases of prematurity, postural tone becomes low. Typical features of children with prematurity include low postural tone, altered axis of the head and neck which generates incorrect or ineffective vestibular information and poor cortical movement caused by poor development of capital flexion. OBJECTIVE The Ca2+-activated Cl-channel(Ca CC)plays a crucial role in various physiological functions.Recent evidences suggest TMEM16A encodes CaC C in various cells,including endothelial cells.However,the role of TME...OBJECTIVE The Ca2+-activated Cl-channel(Ca CC)plays a crucial role in various physiological functions.Recent evidences suggest TMEM16A encodes CaC C in various cells,including endothelial cells.However,the role of TMEM16A in the vascular endothelial dysfunction in hypertension is unclear.METHODS In the study,RT-PCR,Western blotting,co-immunopricipitation,confocal imaging,patch-clamp,and endothelial-specific TMEM16A transgenic and knockout mice were employed.RESULTS We found that TMEM16A was expressed abundantly and functioned as Ca CC in endothelial cells.AngiotensinⅡ(AngⅡ)induced endothelial dysfunction with an increase in TMEM16A expression,which was alleviated by TMEM16A inhibitor.Further studies revealed that TMEM16A endothelial-specific knockout significantly lowered the blood pressure and ameliorated endothelial dysfunction in AngⅡ-induced hypertension,whereas,TMEM16A endothelial-specific overexpression showed the opposite effects.These results were related to the increased reactive oxygen species(ROS)generation,NADPH oxidase activation,and Nox2,p22phox expression facilitated by TMEM16A upon AngⅡ-induced hypertensive challenges.Moreover,TMEM16A directly interacted with Nox2 monomer and reduced the degradation of Nox2 through the proteasome-dependent endoplasmic recticulum-associated degradation pathway.TMEM16A also potentiated the translocation of p47phox and p67phox from cytosol to cell membrane and the subsequent interaction with Nox2.CONCLUSION Our results demonstrated that TMEM16A,as Ca CC,is a positive regulator of ROS generation via upregulating the activation of Nox2 NADPH oxidase in the vascular endothelium,and therefore facilitates endothelial dysfunction and hypertension.Modification of TMEM16A may be a novel therapeutic strategy for endothelial dysfunction-associated cardiovascular diseases.展开更多
Objective:To explore the Clinical observation on the treatment of severe hand-foot-mouth disease with antelope horn powder combined with auricular point application.Methods:From July 2016 to June 2018,90 children were...Objective:To explore the Clinical observation on the treatment of severe hand-foot-mouth disease with antelope horn powder combined with auricular point application.Methods:From July 2016 to June 2018,90 children were randomly divided into control group(n=30),treatment group(n=30)and control group(n=30).The control group was treated with routine western medicine,the treatment group 1 was treated with oral antelope horn powder on the basis of control group,the second group was treated with auricular point application on the basis of treatment group 1,and the time of symptom relief and clinical cure were observed in each group.Related immune function and related inflammatory factors,serum neuron-specific enolase(NSE),safety index.Results:In treatment group 1,the time of herpes regression,antipyretic,antispasmodic time and clinical cure time were shorter than those of control group,and the time of treatment 2 group was shorter than that of group 1(P<0.05).The levels of CD3,CD4 and CD8 were increased after treatment in the three groups,especially in the treatment group(P<0.05),and the levels of TNF-毩αand IL-2,IL-6,IL-10 were decreased after treatment in the three groups,especially in the treatment group(P<0.05).NSE decreased after treatment in three groups,especially in treatment group 2(P<0.05).Conclusion:The application of antelope horn powder combined with auricular point application can obviously improve the severe hand,foot and mouth disease,and the clinical curative effect is definite.It is worth popularizing and applying in clinical practice.展开更多
文摘Introduction: Infant and child morbidity and mortality constitute a public health problem in Africa, particularly in Senegal. The objective of this work was to help identify the determinants this morbidity and mortality. Materials and Methods: A prospective study, over a 4-month period (January to April 2020). All patients aged 0 to 15 years, hospitalized for emergency reasons in the Albert Royer emergency department, were included. Mortality was analyzed according to sociodemographic data, the patient’s itinerary, transfer procedures, availability of emergency medications, diagnosis made during hospitalization and causes of death. The data were collected based on a protocol in an established file and analyzed by SPSS (Statistical Package for Science Social) software. Version 18. Results: 225 children in emergency situations were included. The age of our patients is between 0 and 15 years (minimum 0.23 months, maximum 191 months, average 47 months and standard deviation 54). Thus, emergencies represented 57.98% of the 388 hospitalizations. Seventy percent of patients were less than 60 months old with a male predominance (sex ratio 1.16), 2.22% were newborns and 27.11% were aged between 60 and 191 months). A total of 79 (35.1%) were transferred and only 57.4% received care before transfer. A total of 12 deaths were reported, representing a lethality of 5.3%. Only young age (less than 59 months, a mortality rate of 91.7%), cardiac decompensations, severe sepsis, and neonatal conditions were more associated with mortality. The majority of our deceased patients came from families with a low socio-economic level (83.3%). Conclusion: Pediatric emergencies are frequent and responsible for lethality and require the necessary efforts, particularly through parent education, the creation of emergency outpatient care units and, above all, the strengthening of the technical platform and therapeutic means.
文摘Introduction: Sinonasal polyposis (SNP) is a chronic inflammatory disease of the mucosa of the nasal cavities and facial sinuses. It is characterized by an oedematous, multifocal and bilateral degeneration of the nasosinus mucosa, which originates in the lateral masses of the ethmoid, where it causes the formation of smooth, gelatinous, translucent and pyriform polyp lesions. The objective of this study was to review epidemiological, clinical, paraclinical data and evaluate the results of endoscopic surgical treatment. Patients and Methods: This is a retrospective study on 60 patients followed at the ENT department of the Fann National University Hospital Center, from January 2010 to December 2015. All patients with sinonasal polyposis were included in the study. Results: The average age of our patients was 38 years and the sex ratio (M/F) was 0.8. In the patients’ histories, we found 18% asthma and 10% Widal’s disease. The average consultation time was 8.5 years. All patients had consulted for nasal obstruction;rhinorrhea was bilateral and found in 67.7% of cases, with olfactory disorders accounting for 50%. The CT scan performed in 58% of cases made it possible to specify the extent of the lesions;the involvement of the ethmoidal sinus was constant and extended to the other sinuses except in 2 cases. All patients had received medical treatment with local corticosteroids. Endoscopic surgical treatment was initiated in 43% of cases after failure of corticosteroid-based medical treatment. The evolution under treatment marked by the reappearance of symptoms that increased each month. At one month postoperatively, all clinical symptomatology had improved with the exception of olfactory disorders, which persisted in 3 patients. At 12 months we noted 12 cases of reappearance of nasal obstruction. Conclusion: SNP is a disease of little known etiology. The diagnosis is almost always clinical. Endoscopic surgery remains a recourse to medical treatment. For good local control, patients should be more respectful of good compliance with corticosteroid therapy.
文摘Domestic accidents (DA) are common in children and responsible for high morbidity and mortality in developed countries. Objective: This work aimed to describe the epidemiological profile of AD in children aged 0 to 15 years in Libreville. Materials and Methods: All children aged 0 to 15 years who were victims of unintentional trauma occurring at home or in its immediate surroundings were included. We studied the mother’s age, family situation, socioeconomic level, type of housing, age and sex of the child, characteristics of AD and their management. Results: The majority of mothers lived in an intermediate dwelling (80.6%). They were married (37.1%), middle managers (58.2%) and of average socioeconomic level (60.5%). The average age of the mothers was 39.9 ± 11.4 years. Families with more than three children were most exposed (39.2%) to the occurrence of AD. The average age of the children was 6.5 ± 3.3 years with a male predominance. The sex ratio was 1.8. The most common ADs were falls (34.7%), followed by cuts (22.3%) and burns (17.7%). Wounds (54.4%), followed by burns (33%) and fractures (5.1%) were the main types of injuries. The upper limbs were the most affected body part (33.9%) followed by the lower limbs (30.1%) and the head (27.3%). The yard was the preferred location for ADs to occur (51.1%), and particularly during the holiday period (48.4%). The risk factors related to the occurrence of AD were age, socioeconomic level, number of children and type of housing. Care was provided at home in 51.9% of cases. Conclusion: The occurrence of AD in children is not negligible;hence the need to implement preventive measures to minimize their frequency.

文摘Hepatocellular carcinoma(HCC)is the predominant form of primary liver cancer,accounting for 90%of all cases.Currently,early diagnosis of HCC can be achieved through serum alpha-fetoprotein detection,B-ultrasound,and computed tomography scanning;however,their specificity and sensitivity are suboptimal.Despite significant advancements in HCC biomarker detection,the prognosis for patients with HCC remains unfavorable due to tumor heterogeneity and limited understanding of its pathogenesis.Therefore,it is crucial to explore more sensitive HCC biomarkers for improved diagnosis,monitoring,and management of the disease.Long non-coding RNA(lncRNA)serves as an auxiliary carrier of genetic information and also plays diverse intricate regulatory roles that greatly contribute to genome complexity.Moreover,investigating gene expression regulation networks from the perspective of lncRNA may provide insights into the diagnosis and prognosis of HCC.We searched the PubMed database for literature,comprehensively classified regulated cell death mechanisms and systematically reviewed research progress on lncRNA-mediated cell death pathways in HCC cells.Furthermore,we prospectively summarize its potential implications in diagnosing and treating HCC.

文摘Objective Size distribution is an important biophysical property of extracellular vesicles(EVs).EVs include small EVs(s-EVs)and large EVs(l-EVs)by size.Differential ultracentrifugation(dUC)is widely used to separate EVs from biofluids,but it can precipitate large impurity particles.Dynamic light scattering(DLS)is a simple and fast method for analyzing the size distribution of EVs.However,this approach is nonideal for heterogeneous and polydisperse samples since a small quantity of large impurity particles can markedly disturb the DLS results.Here,we developed a simple method to improve the reliability of DLS measurements.Methods Plasma was obtained from 13 volunteers.The plasma was first processed by dUC to obtain crude l-EVs.The crude l-EVs were filtered with syringe filters(pore size of 1μm and membrane material of hydrophilic polyvinylidene fluoride(PVDF))to remove large impurity particles from l-EVs.The size distributions of the crude l-EVs and filtered l-EVs were measured via DLS.Results After the samples were filtered,the coefficients of variation of the hydrodynamic radius and Peak 1 intensity of the filtered l-EVs decreased from 20.39%(12.76–28.96%)and 20.44%(14.58–28.32%)to 3.05%(1.79–4.72%)and 3.43%(1.76–5.88%),respectively,compared with those of the crude l-EVs.Conclusion These findings suggest that filtration can effectively separate circulating l-EVs in plasma to remove large impurity particles and make samples suitable for characterization by DLS.Our findings provide a simple method to improve precision via DLS to measure the size distribution of EVs.

文摘Summary: Human sperm DNA is an important genetic and epigenetic material, whose chromatin structure differs from that of somatic cells. As such, conventional methods for DNA extraction of somatic cells may not be suitable for obtaining sperm DNA. In this study, we evaluated and compared three sperm DNA extraction techniques, namely, modified guanidinium thiocyanate method (method A), traditional phenol-chloroform method (method B), and TianGen kit method (method C). Spectrophotometry and agarose gel electrophoresis analyses showed that method A produced DNA with higher quantity and purity than those of methods B and C (P〈0.01). PCR results revealed that method A was more reliable in amplifying DEAD-box polypeptide 4 (DDX4) and copy number variations (CNVs) than methods B and C, which generated false-positive errors. The results of sperm DNA methylation assay further indicated that methods A and B were effective, and the former yielded higher quantitative accuracy. In conclusion, the modified guanidinium thiocyanate method provided high quality and reli- able results and could be an optimal technique for extracting sperm DNA for methylation assay.

文摘Hyperhomocysteinemia is an important risk factor for preeclampsia-eclampsia. This study established a pregnant rat model of hyperhomocysteinemia, in which blood plasma homocysteine concentrations were twice or three times greater than that of normal pregnant rats. TUNEL revealed an increase in the number of apoptotic cells in the frontal cortex of pregnant rats with hyperhomocysteinemia. In addition, immunohistochemical staining detected activated nuclear factor-KB-positve cells in the frontal cortex. Reverse transcription-PCR detected that mRNA expression of the anti-apoptotic gene bcl-2 diminished in the frontal cortex. In situ hybridization and western blotting revealed that N-methyi-D- aspartate receptor 1 mRNA and protein expression was upregulated in the frontal cortex and hippocampus. These results indicate that hyperhomocysteinemia can induce brain cell apoptosis, increase nerve excitability, and promote the occurrence of preeclampsia in pregnant rats.
文摘BACKGROUND: Hand-foot-mouth disease has become a major public health issue in children in China. In the present prospective study we investigated the clinical characteristics and emergency management of children with severe encephalitis associated with NPE caused by enterovirus 71.METHODS: The study was conducted in 2 pediatric intensive care units (PICUs) over a 2-month period. Clinical records were reviewed of critically ill children with severe encephalitis associated with NPE caused by EV71 who were admitted to PICUs during the period of May to June 2008 in Fuyang.RESULTS: We reviewed the complete records of 36 children, of whom 23 (63.9%) were male and 13 (36.1%) female. Their age ranged from 4 to 48 months, with an average of 15.8 months. All children except one were under 3 years of age. The overall mortality in these children was 19.4%. The average duration of critical life threatening signs and symptoms was 2.1 days (12 hours-5 days). Nervous system diseases included brainstem encephalitis in 27 children (75%), brainstem encephalitis associated with myelitis in 6 children (16.7%), and general encephalitis in 3 chidren (8.3%), respectively. In 12 patients of NPE (33.3%) pink or bloody bubble sputum and asymmetric pulmonary edema or hemorrhage was the primary manifestation but no typical exanthema was observed. Five children died of acute onset of NPE and / or pulmonary hemorrhage with rapid progression of cardiopulmonary failure within hours after admission. Therapeutic management consisted of mechanical ventilation and administration of mannitol, methylprednisolone, intravenous immunoglobulin (IVIG) and vasoactive drugs, associated with the need of fluid volume resuscitation in 9 (25%) of the 36 children.CONCLUSION: In children less than 3 years of age found to be affected by severe EV71 encephalitis associated with NPE, one fifth may die. The major organ systems infected by severe EV71 include the central nervous system, the respiratory system, and the cardiovascular system. Early diagnosis and evaluation, respiratory support, treatment of intracranial hypertension, and mainttenance of function of the cardiovascular system are the most important therapeutic measures.

文摘Low birth weight(LBW) and preterm birth(PB) are associated with newborn mortality and diseases in adulthood.We explored factors related to LBW and PB by conducting a population-based case-control study from January 2011 to December 2013 in Wuhan,China.A total of 337 LBW newborn babies,472 PB babies,and 708 babies with normal birth weights and born from term pregnancies were included in this study.Information of newborns and their parents was collected by trained investigators using questionnaires and referring to medical records.Univariate and logistic regression analyses with the stepwise selection method were used to determine the associations of related factors with LBW and PB.Results showed that maternal hypertension(OR=6.78,95% CI:2.27–20.29,P=0.001),maternal high-risk pregnancy(OR=1.53,95% CI:1.06–2.21,P=0.022),and maternal fruit intake ≥300 g per day during the first trimester(OR=1.70,95% CI:1.17–2.45,P=0.005) were associated with LBW.BMI ≥24 kg/m^2 of mother prior to delivery(OR=0.48,95% CI:0.32–0.74,P=0.001) and gestation ≥37 weeks(OR=0.01,95% CI:0.00–0.02,P〈0.034) were protective factors for LBW.Maternal hypertension(OR=3.36,95% CI:1.26–8.98,P=0.016),maternal high-risk pregnancy(OR=4.38,95% CI:3.26–5.88,P〈0.001),maternal meal intake of only twice per day(OR=1.88,95% CI:1.10–3.20,P=0.021),and mother liking food with lots of aginomoto and salt(OR=1.60,95% CI:1.02–2.51,P=0.040) were risk factors for PB.BMI ≥24 kg/m^2 of mother prior to delivery(OR=0.66,95% CI:0.47–0.93,P=0.018),distance of house from road ≥36 meters(OR=0.72,95% CI:0.53– 0.97,P=0.028),and living in rural area(OR= 0.60,95% CI:0.37–0.99,P=0.047) were protective factors for PB.Our study demonstrated some risk factors and protective factors for LBW and PB,and provided valuable information for the prevention of the conditions among newborns.
文摘AIM:To study the change of torsion in both eyes after unilateral inferior oblique(IO)weakening on children with congenital superior oblique palsy(SOP).METHODS:This retrospective study enrolled all patients diagnosed with unilateral congenital superior oblique palsy(UCSOP)accompanied by inferior oblique overaction(IOOA).A total of 120 eyes of 60 patients were divided into group 1(more extorted paretic eye)and group 2(more extorted nonparetic eye).The degree of fundus torsion was evaluated before and 1 mo after the IO weakening procedure.The torsion of the fundus was recorded by measuring the diskfoveal angle(DFA)using fundus photography.RESULTS:Group 1 included 26 cases and group 2 included 34 cases,thus the rate of extorsion was insignificantly higher in the nonparetic eye(P=0.10).The preoperative DFA in the paretic and nonparetic eyes was 13.21±5.95,7.97±4.25 in group 1,and 4.65±3.79,13.16±5.35 in group 2(both P<0.001).The postoperative DFA in the paretic and nonparetic eyes was 8.57±4.87,7.32±4.27 in group 1(P=0.24),and 3.85±6.00 and 9.94±5.45 in group 2(P<0.001).The amount of postoperative reduction of the DFA in the paretic and nonparetic eyes was 4.64±3.90,0.65±0.76 in group 1(P=0.002),and 0.80±0.81,3.21±5.50 in group 2(P=0.01).The difference in the amount of reduction of DFA in the more extorted eye in group 1(paretic eye)vs group 2(nonparetic eye)was insignificant(P=0.30).CONCLUSION:Excyclotorsion in the nonparetic eye has a similar probability in the paretic eye in UCSOP children,and weakening of the ipsilateral IO has a more obvious effect on the decrement of extorsion in the more extorted eye regardless of which eye is paretic.

文摘Human cytomegalovirus(HCMV) infection is a leading cause of birth defects, primarily affecting the central nervous system and causing its maldevelopment. As the essential downstream effector of Notch signaling pathway, Hes1, and its dynamic expression, plays an essential role on maintaining neural progenitor/stem cells(NPCs) cell fate and fetal brain development. In the present study, we reported the first observation of Hes1 oscillatory expression in human NPCs, with an approximately1.5 hour periodicity and a Hes1 protein half-life of about 17(17.6 ± 0.2) minutes. HCMV infection disrupts the Hes1 rhythm and down-regulates its expression. Furthermore, we discovered that depleting Hes1 protein disturbed NPCs cell fate by suppressing NPCs proliferation and neurosphere formation, and driving NPCs abnormal differentiation. These results suggested a novel mechanism linking disruption of Hes1 rhythm and down-regulation of Hes1 expression to neurodevelopmental disorders caused by congenital HCMV infection.
文摘BACKGROUND Sensory integration intervention is highly related to the child's effective interaction with the environment and the child's development.Currently,various sensory integration interventions are being applied,but research methodological problems are arising due to unsystematic protocols.This study aims to present the optimal intervention protocol by presenting scientific standards for sensory integration intervention through meta-analysis.AIM To prove the effectiveness of sensory integration therapy,examine the latest trend of sensory integration studies in Korea,and provide clinical evidence for sensory integration therapies.METHODS The database of Korean search engines,including RISS,KISS,and DBpia,was used to search for related literature published from 2001 to October 2020.The keywords,“Children”,“Sensory integration”,“Integrated sensory”,“Sensorymotor”,and“Sensory stimulation”were used in this search.Then,a meta-analysis was conducted on 24 selected studiesRISS,KISS,and DBpia,was used to search for related literature published from 2001 to October 2020.The keywords,“Children”,“Sensory integration”,“Integrated sensory”,“Sensorymotor”,and“Sensory stimulation”were used in this search.Then,a meta-analysis was conducted on 24 selected studies.RESULTS Sensory integration intervention has been proven effective in children with cerebral palsy,autism spectrum disorder,attention deficit/hyperactivity disorder,developmental disorder,and intellectual disability in relation to the diagnosis of children.Regarding sensory integration therapies,1:1 individual treatment with a therapist or a therapy session lasting for 40 min was most effective.In terms of dependent variables,sensory integration therapy effectively promoted social skills,adaptive behavior,sensory processing,and gross motor and fine motor skills.CONCLUSION The results of this study may be used as therapeutic evidence for sensory integration intervention in the clinical field of occupational therapy for children,and can help to present standards for sensory integration intervention protocols.
文摘Overview: Obesity is a significant health crisis around the world. Of great concern are the data pointing to the recent increase in the prevalence of obesity irregardless of age group and country. Overweight and obesity in adolescence are markers of overweight and obesity in adults, respectively. Very little data are currently available on the prevalence of childhood obesity in Iran, and more research on the risk factors is required before preventive public health programs can be for mulated and put into practice. Objective: The objective of this study was to quantify the prevalence of overweight and obesity and their associated factors in adolescent children living in Tehran.Materials and methods: During a multistage stratified cluster sampling, 2900 students (1200 males and 1700 females) aged 11-17 years were selected from 20 secondary schools in the school year of 2004-2005. A questionnaire was filled, and weight and height were measured. Discussions and conclusions:The body mass index (BMI) was calculated and adjusted for age and sex. Prevalences of overweight and obesity were 17.9 and 7.1%, respectively. BMI increased with age, and it was higher in those who had lower levels of physical activity.Age at menarche was negatively associated with BMI. There was no relationship between macro-and micronutrient intake and overweight and obesity. This study highlights the high prevalence of overweight and obesity in adolescent children in Tehran.

文摘Erythropoietin has been shown to exhibit neuroprotective effects in animal models. A neonatal rat model of hypoxic-ischemic white matter damage was established via bilateral carotid artery ligation in 4-day-old Sprague-Dawley rats. The rats were subsequently treated with recombinant human erythropoietin to observe pathological changes in the brain and long-term neurobehavioral functions before and after intervention. Results showed that the number of myelin basic protein-positive cells, which reflected myelin/oligodendrocyte damage, significantly increased, although the number of amyloid precursor protein-positive cells, which reflected axonaf injury, significantly decreased in periventricular white matter at 72 hours and 7 days following erythropoietin intervention. The number of glial fibrillary acidic protein-positive cells, indicating astrocytic damage, significantly decreased in periventricular white matter of erythropoietin-treated rats at 48 hours, 72 hours, 7 days and 26 days. Following erythropoietin intervention in the 30-day-old rats, head-turning time in the slope test was shortened and open-field test scores increased. These results suggested that erythropoietJn promoted repair of white matter damage, as well as improved neurobehavioral functions in a rat model of hypoxic-ischemic injury.

文摘In a human, the head and spine work together in any kind of posture and movement. Any movement starts from the head through neck flexion and specifically capital flexion. Capital flexion initiates the straightening of the cervical spine which causes the connection of the head on the C1-C2 suboccipital part to the thoracic and lumbar parts. With this, the spine starts to move and postural tone increases. Without construction of the neck or alteration of the axis, as seen with cases of prematurity, postural tone becomes low. Typical features of children with prematurity include low postural tone, altered axis of the head and neck which generates incorrect or ineffective vestibular information and poor cortical movement caused by poor development of capital flexion. 文摘Objective:To investigate the causes,pathogenesis and prognosis of shunt displacement in arachnoid cyst.Methods:Retrospective analysis was made on the case of the left frontal top arachnoid cyst peritoneal shunt self-extraneous and cured in our hospital in November 2019.Combined with literature review,the treatment and prognosis of shunt displacement after arachnoid cyst peritoneal shunt were discussed.Results:Arachnoid cyst disappeared children heal,clinical symptoms improved,wipe out the shift shunt,literature review arachnoid cyst peritoneal shunt tube shift and recover rare,mainly for the shunt tube distal displacement,falling in the scrotum,emergence anus,and shift to the chest,or even a heart,required reoperation processing,timely deal with good prognosis.Conclusion:Arachnoid cyst peritoneal shunt may be an effective method for the treatment of cerebral convex arachnoid cyst in children,and it also suggests that the shunt tube can be removed after the disappearance of cerebral convex arachnoid cyst.Shunt tube displacement is rare in clinic and needs timely surgical treatment.

文摘OBJECTIVE The Ca2+-activated Cl-channel(Ca CC)plays a crucial role in various physiological functions.Recent evidences suggest TMEM16A encodes CaC C in various cells,including endothelial cells.However,the role of TMEM16A in the vascular endothelial dysfunction in hypertension is unclear.METHODS In the study,RT-PCR,Western blotting,co-immunopricipitation,confocal imaging,patch-clamp,and endothelial-specific TMEM16A transgenic and knockout mice were employed.RESULTS We found that TMEM16A was expressed abundantly and functioned as Ca CC in endothelial cells.AngiotensinⅡ(AngⅡ)induced endothelial dysfunction with an increase in TMEM16A expression,which was alleviated by TMEM16A inhibitor.Further studies revealed that TMEM16A endothelial-specific knockout significantly lowered the blood pressure and ameliorated endothelial dysfunction in AngⅡ-induced hypertension,whereas,TMEM16A endothelial-specific overexpression showed the opposite effects.These results were related to the increased reactive oxygen species(ROS)generation,NADPH oxidase activation,and Nox2,p22phox expression facilitated by TMEM16A upon AngⅡ-induced hypertensive challenges.Moreover,TMEM16A directly interacted with Nox2 monomer and reduced the degradation of Nox2 through the proteasome-dependent endoplasmic recticulum-associated degradation pathway.TMEM16A also potentiated the translocation of p47phox and p67phox from cytosol to cell membrane and the subsequent interaction with Nox2.CONCLUSION Our results demonstrated that TMEM16A,as Ca CC,is a positive regulator of ROS generation via upregulating the activation of Nox2 NADPH oxidase in the vascular endothelium,and therefore facilitates endothelial dysfunction and hypertension.Modification of TMEM16A may be a novel therapeutic strategy for endothelial dysfunction-associated cardiovascular diseases.
文摘Objective:To explore the Clinical observation on the treatment of severe hand-foot-mouth disease with antelope horn powder combined with auricular point application.Methods:From July 2016 to June 2018,90 children were randomly divided into control group(n=30),treatment group(n=30)and control group(n=30).The control group was treated with routine western medicine,the treatment group 1 was treated with oral antelope horn powder on the basis of control group,the second group was treated with auricular point application on the basis of treatment group 1,and the time of symptom relief and clinical cure were observed in each group.Related immune function and related inflammatory factors,serum neuron-specific enolase(NSE),safety index.Results:In treatment group 1,the time of herpes regression,antipyretic,antispasmodic time and clinical cure time were shorter than those of control group,and the time of treatment 2 group was shorter than that of group 1(P<0.05).The levels of CD3,CD4 and CD8 were increased after treatment in the three groups,especially in the treatment group(P<0.05),and the levels of TNF-毩αand IL-2,IL-6,IL-10 were decreased after treatment in the three groups,especially in the treatment group(P<0.05).NSE decreased after treatment in three groups,especially in treatment group 2(P<0.05).Conclusion:The application of antelope horn powder combined with auricular point application can obviously improve the severe hand,foot and mouth disease,and the clinical curative effect is definite.It is worth popularizing and applying in clinical practice.
