Physical activity in primary and secondary prevention of cardiovascular disease:Overview updated 被引量:8

Although the observed progress in the cardiovascular disease treatment, the incidence of new and recurrent coronary artery disease remains elevated and constitutes the leading cause of death in the developed countries... Although the observed progress in the cardiovascular disease treatment, the incidence of new and recurrent coronary artery disease remains elevated and constitutes the leading cause of death in the developed countries. Three-quarters of deaths due to cardiovascular diseases could be prevented with adequate changes in lifestyle, including increased daily physical activity. New evidence confirms that there is an inverse dose-response relationship between physical activity and cardiovascular disease and mortality risk. However, participation in moderate to vigorous physical activity may not fully attenuate the independent effect of sedentary activities on increased risk for cardiovascular diseases. Physical activity also plays an important role in secondary prevention of cardiovascular diseases by reducing the impact of the disease, slowing its progress and preventing recurrence. Nonetheless, most of eligible cardiovascular patients still do not benefit from secondary prevention/cardiac rehabilitation programs. The present review draws attention to the importance of physical activity in the primary and secondary prevention of cardiovascular diseases. It also addresses the mechanisms by which physical activity and regular exercise can improve cardiovascular health and reduce the burden of the disease. 展开更多

Purpose:The aim of the present study was to analyze the associations of youths' sedentary behavior(SB) with parents' and siblings' SB and physical activity(PA),as well as the associations of youths' coparticipation with parents,siblings,and friends in PA and SB with youths' SB.Methods:The sample consisted of 1543 youths(12.02±2.51 years;788 boys) enrolled in the baseline cohort of the UP&DOWN study.SB was assessed by accelerometry and questionnaire.Participants reported the time spent by their parents and siblings watching television,playing videogames,surfing the Internet,sitting/resting,and doing PA.Further,participants reported coparticipation with parents,siblings,and friends in these activities.Linear mixed models,including school and city as random effects,were performed.Results:Parents' television time was positively associated with youths' screen-based SB.Coparticipation with friends in playing videogames(in boys) and in surfing the Internet(in girls) showed a positive association with screen-based SB and a negative association with educational-based SB.Moreover,coparticipation with siblings and friends in PA was inversely associated with accelerometer-based SB in boys and girls.Conclusion:Our results emphasize the important role of social modeling in the development of sedentary lifestyles in youths.Interventions aimed at reducing health risk behaviors in youths could be more effective if they are oriented from a social perspective that involves their families and networks of their closest friends. 展开更多

Background:Children and adolescents can be distinguished by different typologies(clusters) of physical activity and sedentary behavior.How physical activity and sedentary behaviors change over time within different typologies is not known.This study examined longitudinal changes in physical activity and sedentary time among children and adolescents with different baseline typologies of activity-related behavior.Methods:In this longitudinal study(3 annual time points) of children(n=600,age=9.2±0.4 years(mean±SD),50.3% girls) and adolescents(n=1037,age=13.6±1.7 years,48.4% girls),participants were recruited in Spain in 2011-2012.Latent class analyses identified typologies based on self-reported screen,educational,social and relaxing sedentary behaviors,active travel,muscle strengthening activity,and sport at baseline.Within each typology,linear mixed growth models explored longitudinal changes in accelerometer-derived moderate-to-vigorous physical activity and sedentary time,as well as time by class interactions.Results:Three typologies were identified among children( "social screenies",12.8%;"exercisers",61.5%;and "non-sporty active commuters",25.7%) and among adolescents "active screenies",43.5%;"active academics",35.0%;and "non-sporty active commuters",21.5%) at baseline.Sedentary time increased within each typology among children and adolescents,with no significant differences between typologies.No changes in physical activity were found in any typology among children.In adolescents,physical activity declined within all typologies,with "non-sporty active commuters" declining significantly more than "active screenies" over 3 years.Conclusion:These results support the need for interveation to promote physical activity and prevent increases in sedentary time during childhood and adolescence.Adolescents characterized as "non-sporty active commuters" may require specific interventions to maintain their physical activity over time. 展开更多

Purpose:Current Direct Observation(DO)methods are burdensome to the observer and relevant limitations can be identified on the outcome information.This study aims to characterize DO from the observers’point of view and to analyze the feasibility and usefulness of visual information in the form of video.Method:data collection was done with ten adult males,observed while performing different physical activities in an indoor court.Sessions were video recorded by two cameras.Three observers performed systematic observation,using SOPLAY,with different sampling rates.Inter observer’s agreement and with reference data was analyzed by Cohen’s Kappa statistic.Results:SOPLAY highest agreement between observers was 0.611 on vigorous category and walking category had the lowest 0.188.It doubles the time needed to annotate the video with pauses,but half of player’s behavior is preserved.Conclusion(s):using video to support DO permits to collect more accurate data and a more detailed behavior categorization is warranted.Developments in computer vision are expected to reduce the human efforts in DO methods. 展开更多

Nowadays,is well established that the benefits induced by exercise training(ET)affects not only skeletal muscle,but also other non-contractile organs over time.One potential mechanism underlying this crosstalk is the synthesis and secretion of several biological active factors,such as irisin,by muscle contractile activity.This hormone has been described to be able to induce a brown adipocyte-like phenotype in white adipose(WAT),increase whole-body metabolic rate,and therefore prevent and/or treat obesity-related metabolic diseases.Thus,the modulatory impact of ET on WAT may also occur through skeletal muscle-adipose organ axis.In this review,we summarize the acute and chronic adaptations to ET-induced irisin synthesis and secretion on the development of browning of white fat and,thus,providing an overview of the potential preventive and therapeutic role of ET on the obesity-related underlying pathways. 展开更多

Chronic hemodynamic adaptations induced by resistance training with and without blood flow restriction in adults:A systematic review and meta-analysis

The purposes of this systematic review and meta-analysis of peer-reviewed literature were to examine the chronic effects of resistance training with blood flow restriction(RT-BFR)on hemodynamics,and to compare these adaptations to those induced by traditional resistance training(TRT)programs in adults(PROSPERO:Registry:CRD42022339510).A literature search was conducted across PubMed,Sports Discus,Scielo,and Web of Science databases.Two independent reviewers extracted study characteristics and blood pressure measures.Risk of bias(The Cochrane risk of bias tool for randomized controlled trials[RoB-2]),and the certainty of the evidence(Grading of Recommendations,Assessment,Development,and Evaluation[GRADE])were used.A total of eight studies met the inclusion criteria for systolic(SBP),diastolic(DBP),and mean arterial pressure(MAP).Regarding the comparison of RT-BFR vs.non-exercise,no significant differences favoring the exercise group were observed(p>0.05).However,when compared to TRT,RT-BFR elicited additional improvements on DBP(-3.35;95%CI-6.00 to-0.71;I^(2)=14%;z=-2.48,p=0.01),and on MAP(-3.96;95%CI-7.94 to 0.02;I^(2)=43%;z=-1.95,p=0.05).Results indicate that RT-BFR may elicit a decrease in DBP in comparison with TRT,but the lack of data addressing this topic makes any conclusion speculative.Future research on this topic is warranted. 展开更多
