Seco...AIM To study tumor response, and tolerability of arterially directed embolic therapy(ADET) with polyethylene glycol embolics loaded with irinotecan for the treatment of colorectal cancer liver metastases(CRC-LM). Secondary objectives were to monitor quality of life, time to progression and survival of patients.METHODS Patients were included in the study if they were affected by CRC-LM, refractory to systemic chemotherapy, treated with ADET using polyethylene glycol embolics, and had liver involvement < 50%. Tumor response, performance status(PS), tumor marker antigens, and quality of life(QoL) were monitored at 1, 3 and 6 mo after ADET. QoL was assessed with the Palliative Performance Scale(PPS).RESULTS We treated 50 consecutive CRC-LM patients with ADET using polyethylene glycol embolics. Their tumor response one month after ADET was: 28% of complete response(CR), 48% of partial response(PR), 8% stable disease(SD), and 16% of progression. Tumor response 3 mo after ADET was CR 24%, PR 38%, SD 19% and progression disease(PD) 19%. Tumor response 6 mo after ADET was CR 18%, PR 44%, SD 21% and PD 18%. QoL was 90% PPS at each time point. Median time to progression for patients who progressed was 2.5 mo(range 0.8-6). Median follow-up was 14 mo(0.8-25 range). ADETs were performed with no complications. Observed side effects(mild or moderate intensity) were: Pain in 32% of patients, increase of transaminase levels in 20% and fever in 14%, whereas 30% of patients did not complain any adverse event. CONCLUSION The treatment of unresectable CRC-LM with ADET using polyethylene glycol microspheres loaded with irinotecan was effective in tumor response and resulted in mild toxicity, and good QoL.展开更多

Background:Sleep disorders after heart surgery lead to increased heart rate,myocardial oxygen demand,and cause dysrhythmia that worsens heart ischemia.The purpose of this study was to determine the effect of head and ...Background:Sleep disorders after heart surgery lead to increased heart rate,myocardial oxygen demand,and cause dysrhythmia that worsens heart ischemia.The purpose of this study was to determine the effect of head and facial massage on sleep conditions following coronary artery bypass graft surgery.Materials and methods:A randomized controlled trial was performed on 72 patients.They were randomly divided into interventional(n=36)and control groups(n=36).On the third to fifth day after the operation,head and the facial massage were done for 15 min in the intervention group.The patients in the control group received only routine care.Richard Campbell’s Sleep Questionnaire was completed for four consecutive days for each group.Data were analyzed in SPSS V26.Results:Mean scores for sleep conditions before the intervention no statistically significant difference seen between the two groups(P>0.05).After the intervention,there was a statistically significant difference between mean scores for sleep conditions of the interventional and control groups(P<0.001).Also with general estimated equation test compared mean scores for sleep conditions between two groups.That revealed sleep condition total score in the intervention group was more than the control group and the trend of changes over time was statistically significant(P<0.001).Conclusion:Head and facial massage is an effective nursing intervention in improving the sleep condition of patients after coronary artery bypass graft.Due to the fact that head and face massage is an easy method and brings patient satisfaction,it is recommended to use this method as a suitable supplement for drug therapy and postoperative interventions in these patients.展开更多

They allow the healthcare team to monitor patient ...Background: Patient-reported outcome measures (PROMS) are essential tools in clinical practice and research to assess patients’ needs from their unique perspectives. They allow the healthcare team to monitor patient status and concerns outside the clinical setting. However, the real innovation in this field is its digitization: electronic patient-reported outcome measures (ePROMs). Aims: This review aimed to get an overview of whether these new technologies are being used to aid palliative care teams in their daily struggle to provide comfort to their patients. Methods: We conducted a systematic review of articles retrieved from PubMed and Web of Science, up to November 2021. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were followed. The search strategy yielded 242 records, of which 13 met the predefined inclusion and exclusion criteria. In addition, relevant information related to ePROMs was extracted from each study. Results: Outcomes were grouped into the quality of life assessment, symptom burden and simple assessments, and the decision to introduce Palliative Cures (PC). In 61.5% of cases, ePROMs positively impacted patients’ quality of life. Furthermore, in 46.15% of cases, ePROMs led Primary Care (PC) teams to make an ethical decision;the same relative value as in the circumstances did not define the direction in ethical terms. Conclusion: Remind professionals and patients that these tools exist and can be applied in many situations. If used correctly, they can provide patients with a better quality of life and more complete information for professionals.展开更多

With the focus of urban regeneration gradually shifting to the community level, community regeneration has become an important path for improving residents' quality of life and the governance of grassroots society...With the focus of urban regeneration gradually shifting to the community level, community regeneration has become an important path for improving residents' quality of life and the governance of grassroots society. How to innovate the governance model and how to improve the community public space utilization efficiency become vital questions. From the perspective of property rights regime, this paper puts forward the theoretical framework of “property rights regime-governance model-space utilization,” and argues that the property rights regime can be adjusted through the subdivision of property rights, which can change the governance model of public space and affect the space utilization efficiency. Through an in-depth investigation of the community regeneration of Shanghai KIC Garden, this study finds that different property rights regimes affect the process and results of collective action in public space governance, and the monocentric governance model formed by the concentration of governmental power can easily lead to the governance dilemmas such as rent-seeking, ineffective supervision, and information asymmetry. By subdividing and decentralizing property rights, social organizations, and community residents can be introduced to participate in the process of spatial governance, so as to facilitate the sinking of governance centers to the grassroots level and form a polycentric governance model. Moreover, multiple stakeholders can be motivated to make full use of public space through the property rights regime adjustment, and the public space utilization efficiency can be enhanced as a result.展开更多

The digital transformation of tax administrations has ceased to be a distant aspiration and become,arguably,the principal vehicle for transparency.The capacity to translate rules and procedures into auditable digital ...The digital transformation of tax administrations has ceased to be a distant aspiration and become,arguably,the principal vehicle for transparency.The capacity to translate rules and procedures into auditable digital flows enhances taxpayers’understanding of how tax institutions operate and,in turn,improves their public image and legitimacy.The Angolan General Tax Administration(AGT)has,since 2014,pursued a comprehensive modernisation agenda anchored in the Integrated Tax Management System(SIGT)and the ASYCUDA World platform,incorporating key modules such as e-invoicing,automated settlement,risk-based VAT refund processing,digital tax enforcement,and taxpayer portals.In parallel,the AGT has strengthened its analytical and intelligence layers through APEX/BI systems,developed Tax Compliance Certificates(CCT)as tools of digital trust,and implemented a Tax Intelligence System(SIT)that supports the Annual Audit Plan(PAF)through data-driven risk analysis.This article demonstrates how these components-governed by detailed blueprints,reinforced by cybersecurity and interoperability frameworks-give practical form to the core principles of taxation,elevating the governance of public revenues to a level of predictability,traceability,and proportionality.It concludes that in Angola,transparency has evolved from rhetoric to practice-from a mere normative principle to an operational reality-producing measurable,verifiable tax actions that strengthen both institutional legitimacy and citizens’trust.展开更多
The rapid growth of mobile Internet technologies has induced a dramatic increase in mobile payments as well as concomitant mobile transaction fraud. As the first step of mobile transactions, bankcard enrollment on mob...The rapid growth of mobile Internet technologies has induced a dramatic increase in mobile payments as well as concomitant mobile transaction fraud. As the first step of mobile transactions, bankcard enrollment on mobile devices has become the primary target of fraud attempts. Although no immediate financial loss is incurred after a fraud attempt, subsequent fraudulent transactions can be quickly executed and could easily deceive the fraud detection systems if the fraud attempt succeeds at the bankcard enrollment step. In recent years, financial institutions and service providers have implemented rule-based expert systems and adopted short message service(SMS) user authentication to address this problem. However, the above solution is inadequate to face the challenges of data loss and social engineering. In this study, we introduce several traditional machine learning algorithms and finally choose the improved gradient boosting decision tree(GBDT) algorithm software library for use in a real system, namely, XGBoost. We further expand multiple features based on analysis of the enrollment behavior and plan to add historical transactions in future studies. Subsequently, we use a real card enrollment dataset covering the year 2017, provided by a worldwide payment processor. The results and framework are adopted and absorbed into a new design for a mobile payment fraud detection system within the Chinese payment processor.展开更多
