BACKGROUND Metabolic dysfunction-associated steatotic liver disease(MASLD),hepatic fibrosis,and cirrhosis are major risk factors for hepatocellular carcinoma(HCC),yet current blood-based diagnostic assays lack suffici...BACKGROUND Metabolic dysfunction-associated steatotic liver disease(MASLD),hepatic fibrosis,and cirrhosis are major risk factors for hepatocellular carcinoma(HCC),yet current blood-based diagnostic assays lack sufficient accuracy for routine clinical use.Identifying a non-invasive molecular signature that accurately detects liver disease could improve early diagnosis and monitoring.We hypothesized that the Liver FibraChek Dx■serum assay could discriminate MASLD and HCC from healthy controls using a multiplex biomarker-based algorithm.AIM To evaluate the diagnostic performance of the Liver FibraChek Dx■assay for detecting MASLD and HCC.METHODS This was a prospective,single-center study conducted in a United States tertiary care setting.Serum samples were collected from 45 participants(14 MASLD,19 HCC,12 healthy controls)with liver histology confirmed by biopsy.The Liver FibraChek Dx■algorithm integrates weighted values of aspartate aminotransferase,alanine aminotransferase,taurocholic acid,L-tyrosine,platelet count,and patient age to generate a risk score.Wilcoxon rank sum tests were used to assess associations with histologic diagnosis,and receiver operating characteristic(ROC)curves quantified diagnostic performance.RESULTS Liver FibraChek Dx■risk scores were significantly elevated in MASLD and HCC compared to controls(median:6.92±3.86 vs 3.61±1.67,P<0.001).The area under the ROC curve was 0.890(95%CI:0.776-1.000)for distinguishing diseased from healthy individuals.Sensitivity was 93.9%,specificity 75.0%,positive predictive value 91.1%,negative predictive value 81.8%,and overall accuracy 88.9%.CONCLUSION The Liver FibraChek Dx■assay accurately detects liver disease and shows promise as a non-invasive tool for diagnosing and monitoring MASLD and HCC.展开更多

Chemical hydrogen storage in organic materials is a promising method thanks to its high storage density,reversibility,and safety.However,the dehydrogenation process of organic materials requires high temperatures due ...Chemical hydrogen storage in organic materials is a promising method thanks to its high storage density,reversibility,and safety.However,the dehydrogenation process of organic materials requires high temperatures due to their unfavorable thermodynamic properties.This study proposes a strategy to design a new type of hydrogen storage materials,i.e.,alkali metal pyridinolate/piperidinolate pairs,by combining the effects of a heteroatom and an alkali metal in one molecule to achieve suitable dehydrogenation thermodynamics along with high hydrogen storage capacities.These air-stable compounds can be synthesized using low-cost reactants and water as a green solvent.Thermodynamic predictions indicate that enthalpy changes of dehydrogenation(ΔH_(d))can be significantly reduced to the optimal range for efficient hydrogen release,exemplified by lithium 2-piperidinolate with a 5.6 wt%hydrogen capacity and a suitableΔH_(d)of 32.2 kJ/mol-H_(2).Experimental results obtained using sodium systems validate the computational predictions,demonstrating reversible hydrogen storage even below 100℃.The superior hydrogen desorption performance of alkali metal piperidinolates could be attributed to their suitableΔH_(d)induced by the combined effect of ring nitrogen and metal substitution on their structures.This study not only reports new low-cost hydrogen storage materials but also provides a rational design strategy for developing metalorganic compounds possessing high hydrogen capacities and suitable thermodynamics for efficient hydrogen storage.展开更多
Magnesium(Mg)-based materials are promising for lightweight structural applications.However,their widespread adoption is significantly constrained by inherent limitations in mechanical properties.To address this chall...Magnesium(Mg)-based materials are promising for lightweight structural applications.However,their widespread adoption is significantly constrained by inherent limitations in mechanical properties.To address this challenge,this study introduces a novel Mg-based interpenetratingphase composite reinforced with a nickel-titanium(NiTi)scaffold featuring a triply periodic minimal surface(TPMS)configuration.By combining experimental investigations with finite element simulations,we systematically elucidate the dual impact of the scaffold’s unit cell size(a)on manufacturing viability and mechanical enhancement.To compensate for compromised infiltration dynamics induced by decreasing a,a critical permeability threshold of 1×10^(-8) m^(2) is proposed for achieving successful composite fabrication.Mechanically,reducing a strengthens the interaction between the scaffold and matrix:the TPMS-configured NiTi scaffolds improve stress transfer,deflect crack propagation,and facilitate damage delocalization,whereas the Mg matrix preserves structural integrity and enables load redistribution.Consequently,the composites significantly outperform pure Mg,and lowering a leads to more substantial enhancements in compressive strength,energy dissipation,and deformation recoverability.This study offers valuable insight into the design and fabrication of highperformance Mg-based materials for structural and biomedical applications.展开更多
The mechanical properties of minerals in planetary materials are not only interesting from a fundamental point of view but also critical to the development of future space missions.Here we present nanoindentation expe...The mechanical properties of minerals in planetary materials are not only interesting from a fundamental point of view but also critical to the development of future space missions.Here we present nanoindentation experiments to evaluate the hardness and reduced elastic modulus of olivine,(Mg,Fe)_(2)SiO_(4),in meteorite NWA 12008,a lunar basalt.Our experiments suggest that the olivine grains in this lunaite are softer and more elastic than their terrestrial counterparts.Also,we have performed synchrotron-based high-pressure X-ray diffraction(HP-XRD)measurements to probe the compressibility properties of olivine in this meteorite and,for comparison purposes,of three ordinary chondrites.The HP-XRD results suggest that the axial compressibility of the orthorhombic b lattice parameter of olivine relative to terrestrial olivine is higher in NWA 12008 and also in the highly-shocked Chelyabinsk meteorite.The origin of the observed differences is discussed.A simple model combining the results of both our nanoindentation and HP-XRD measurements allows us to describe the contribution of macroscopic and chemical-bond related effects,both of which are necessary to reproduce the observed elastic modulus softening.Such joint analysis of the mechanical and elastic properties of meteorites and returned samples opens up a new avenue for characterizing these highly interesting materials.展开更多
Amid the scarcity of lunar meteorites and the imperative to preserve their scientific value,nondestructive testing methods are essential.This translates into the application of microscale rock mechanics experiments an...Amid the scarcity of lunar meteorites and the imperative to preserve their scientific value,nondestructive testing methods are essential.This translates into the application of microscale rock mechanics experiments and scanning electron microscopy for surface composition analysis.This study explores the application of Machine Learning algorithms in predicting the mineralogical and mechanical properties of DHOFAR 1084,JAH 838,and NWA 11444 lunar meteorites based solely on their atomic percentage compositions.Leveraging a prior-data fitted network model,we achieved near-perfect classification scores for meteorites,mineral groups,and individual minerals.The regressor models,notably the KNeighbor model,provided an outstanding estimate of the mechanical properties—previously measured by nanoindentation tests—such as hardness,reduced Young’s modulus,and elastic recovery.Further considerations on the nature and physical properties of the minerals forming these meteorites,including porosity,crystal orientation,or shock degree,are essential for refining predictions.Our findings underscore the potential of Machine Learning in enhancing mineral identification and mechanical property estimation in lunar exploration,which pave the way for new advancements and quick assessments in extraterrestrial mineral mining,processing,and research.展开更多

The technique used in this paper is handling the inadequate Information for citation;it removes the problem of cold start, which is encountered by very many other recommender systems. In this paper, abstracts, the titles, and the Microsoft academic graphs have been used in coming up with the recommendation list for every document, which is used to combine the content-based approaches and the co-citations. Prioritization and the blending of every technique have been allowed by the tuning system parameters, allowing for the authority in results of recommendation versus the paper novelty. In the end, we do observe that there is a direct correlation between the similarity rankings that have been produced by the system and the scores of the participant. The results coming from the associated scrips of analysis and the user survey have been made available through the recommendation system. Managers must gain the required expertise to fully utilize the benefits that come with business intelligence systems [1]. Data mining has become an important tool for managers that provides insights about their daily operations and leverage the information provided by decision support systems to improve customer relationships [2]. Additionally, managers require business intelligence systems that can rank the output in the order of priority. AIM To investigate the epidemiology,treatment and outcomes of acute appendicitis(AA)in a large population study.METHODS This is a retrospective cohort study derived from the administrative dataset of the Bergamo distr...AIM To investigate the epidemiology,treatment and outcomes of acute appendicitis(AA)in a large population study.METHODS This is a retrospective cohort study derived from the administrative dataset of the Bergamo district healthcare system(more than 1 million inhabitants)from 1997 to 2013.Data about treatment,surgery,length of stay were collected.Moreover for each patients were registered data about relapse of appendicitis and hospital admission due to intestinal obstruction.RESULTS From 1997 to 2013 in the Bergamo district we collected 16544 cases of AA,with a crude incidence rate of 89/100000 inhabitants per year;mean age was 24.51±16.17,54.7%were male and the mean Charlson's comorbidity index was 0.32±0.92.Mortality was<0.0001%.Appendectomy was performed in 94.7%of the patients and the mean length of stay was 5.08±2.88 d;the cumulative hospital stay was 5.19±3.36 d and 1.2%of patients had at least one further hospitalization due intestinal occlusion.Laparoscopic appendectomy was performed in 48%of cases.Percent of 5.34 the patients were treated conservatively with a mean length of stay of 3.98±3.96 d;the relapse rate was 23.1%and the cumulative hospital stay during the study period was 5.46±6.05 d.CONCLUSION The treatment of acute appendicitis in Northern Italy is slowly changing,with the large diffusion of laparoscopic approach;conservative treatment of non-complicated appendicitis is still a neglected option,but rich of promising results.展开更多
Bimetallic additively manufactured structures(BAMSs)can replace traditionally-fabricated functionallygraded-components through fusion welding processes and can eliminate locally-deteriorated mechanical properties aris...Bimetallic additively manufactured structures(BAMSs)can replace traditionally-fabricated functionallygraded-components through fusion welding processes and can eliminate locally-deteriorated mechanical properties arising from post-processing.The present work fabricates a BAMS by sequentially depositing the austenitic stainless-steel and Inconel625 using a gas-metal-arc-welding(GMAW)-based wire+arc additive manufacturing(WAAM)system.Elemental mapping shows a smooth compositional transition at the interface without any segregation.Both materials being the face-center-cubic(FCC)austenite,the electron backscattered diffraction(EBSD)analysis of the interface shows the smooth and cross-interfacecrystallographic growth of long-elongated grains in the<001>direction.The hardness values were within the range of 220-240 HV for both materials without a large deviation at the interface.Due to the controlled thermal history,mechanical testing yielded a consistent result with the ultimate tensile strength and elongation of 600 MPa and 40%,respectively,with the failure location on the stainless-steel side.This study demonstrates that WAAM has the potential to fabricate BAMS with controlled properties.展开更多

BACKGROUND While colorectal polyps are not cancerous,some types of polyps,known as adenomas,can develop into colorectal cancer over time.Polyps can often be found and removed by colonoscopy;however,this is an invasive...BACKGROUND While colorectal polyps are not cancerous,some types of polyps,known as adenomas,can develop into colorectal cancer over time.Polyps can often be found and removed by colonoscopy;however,this is an invasive and expensive test.Thus,there is a need for new methods of screening patients at high risk of developing polyps.AIM To identify a potential association between colorectal polyps and small intestine bacteria overgrowth(SIBO)or other relevant factors in a patient cohort with lactulose breath test(LBT)results.METHODS A total of 382 patients who had received an LBT were classified into polyp and non-polyp groups that were confirmed by colonoscopy and pathology.SIBO was diagnosed by measuring LBTderived hydrogen(H)and methane(M)levels according to 2017 North American Consensus recommendations.Logistic regression was used to assess the ability of LBT to predict colorectal polyps.Intestinal barrier function damage(IBFD)was determined by blood assays.RESULTS H and M levels revealed that the prevalence of SIBO was significantly higher in the polyp group than in the non-polyp group(41%vs 23%,P<0.01;71%vs 59%,P<0.05,respectively).Within 90 min of lactulose ingestion,the peak H values in the adenomatous and inflammatory/hyperplastic polyp patients were significantly higher than those in the non-polyp group(P<0.01,and P=0.03,respectively).In 227 patients with SIBO defined by combining H and M values,the rate of IBFD determined by blood lipopolysaccharide levels was significantly higher among patients with polyps than those without(15%vs 5%,P<0.05).In regression analysis with age and gender adjustment,colorectal polyps were most accurately predicted with models using M peak values or combined H and M values limited by North American Consensus recommendations for SIBO.These models had a sensitivity of≥0.67,a specificity of≥0.64,and an accuracy of≥0.66.CONCLUSION The current study made key associations among colorectal polyps,SIBO,and IBFD and demonstrated that LBT has moderate potential as an alternative noninvasive screening tool for colorectal polyps.展开更多
The disease coronavirus disease 2019(COVID-19)is a severe respiratory illness that has emerged as a devastating health problem worldwide.The disease outcome is heterogeneous,and severity is likely dependent on the imm...The disease coronavirus disease 2019(COVID-19)is a severe respiratory illness that has emerged as a devastating health problem worldwide.The disease outcome is heterogeneous,and severity is likely dependent on the immunity of infected individuals and comorbidities.Although symptoms of the disease are primarily associated with respiratory problems,additional infection or failure of other vital organs are being reported.Emerging reports suggest a quite common co-existence of gastrointestinal(GI)tract symptoms in addition to respiratory symptoms in many COVID-19 patients,and some patients show just the GI symptoms.The possible cause of the GI symptoms could be due to direct infection of the epithelial cells of the gut,which is supported by the fact that(1)The intestinal epithelium expresses a high level of angiotensin-converting enzyme-2 and transmembrane protease serine 2 protein that are required for the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)entry into the cells;(2)About half of the severe COVID-19 patients show viral RNA in their feces and various parts of the GI tract;and(3)SARS-CoV-2 can directly infect gut epithelial cells in vitro(gut epithelial cells and organoids)and in vivo(rhesus monkey).The GI tract seems to be a site of active innate and adaptive immune responses to SARS-CoV-2 as clinically,stool samples of COVID-19 patients possess proinflammatory cytokines(interleukin 8),calprotectin(neutrophils activity),and immunoglobulin A antibodies.In addition to direct immune activation by the virus,impairment of GI epithelium integrity can evoke immune response under the influence of systemic cytokines,hypoxia,and changes in gut microbiota(dysbiosis)due to infection of the respiratory system,which is confirmed by the observation that not all of the GI symptomatic patients are viral RNA positive.This review comprehensively summarizes the possible GI immunomodulation by SARS-CoV-2 that could lead to GI symptoms,their association with disease severity,and potential therapeutic interventions.展开更多

Methods: EUS TCB was performed in patients presenting with obstructive jaundice who were suspected of having autoimmune pancreatitis based on their clinical, la boratory and imaging studies. The charts were retrospectively reviewed to determ ine the feasibility of TCB. Results: Between August 2002 and June 2004, 3 patien ts with obstructive jaundice and suspected autoimmune pancreatitis (AIP) underwe nt EUS TCB. In each case, a diagnosis of pancreatic cancer also was considered, and surgical resection was the planned therapy before the patient underwent EUS TCB. Histologic review of the TCB specimens established the diagnosis of AIP in two patients and identified nonspecific changes of chronic pancreatitis in the t hird patient. EUS-guided FNA was performed in two of the 3 patients and failed to establish the diagnosis in either patient. Other than mild transient abdomina l pain (n = 1), no complications were identified. Conclusions: This preliminary study suggests that EUS TCB can safely establish the diagnosis of AIP. Doing so helps guide management and may help to avoid unnecessary surgery. Prosp- ective studies are needed to verify these findings and to more clearly define the role of EUS TCB in these patients.展开更多

These faults may induce inherent failure mode,thus bringing about unexpected output, and the anticipated goal of the test is attained. The fault injection con-sists of voltage signal generator, current signal generator and rear drive circuit which are specially developed,and the ATS can work regularly by means of software simulation. The experimental results indicate that the faultinjection system can find the deficiency of the automatic testing software, and identify the preference of fault re-dundancy. The brain pericyte is a unique and indispensable part of the blood-brain barrier(BBB),and contributes to several pathological processes in traumatic brain injury(TBI).However,the cellular and molecular mechanisms by w...The brain pericyte is a unique and indispensable part of the blood-brain barrier(BBB),and contributes to several pathological processes in traumatic brain injury(TBI).However,the cellular and molecular mechanisms by which pericytes are regulated in the damaged brain are largely unknown.Here,we show that the formation of neutrophil extracellular traps(NETs)induces the appearance of CD11b^(+)pericytes after TBI.These CD11b^(+)pericyte subsets are characterized by increased permeability and pro-inflammatory profiles compared to CD11b-pericytes.Moreover,histones from NETs by Dectin-1 facilitate CD11b induction in brain pericytes in PKC-c-Jun dependent manner,resulting in neuroinflammation and BBB dysfunction after TBI.These data indicate that neutrophil-NET-pericyte and histone-Dectin-1-CD11b are possible mechanisms for the activation and dysfunction of pericytes.Targeting NETs formation and Dectin-1 are promising means of treating TBI.展开更多

Objective: To test the hypothesis that the incidence of heart failure has declined and survival after heart failure diagnosis has improved over time but that secular trends have diverged by sex. Design, Setting, and Participants: Population-based cohort study using the resources of the Rochester Epidemiology Project conducted in Olmsted County , Minnesota. Patients were 4537 Olmsted County residents (57%women; mean <<SD>> a ge, 74 <<14>> years) with a diagnosis of heart failure between 1979 and 2000. Fram ingham criteria and clinical criteria were used to validate the diagnosis Main O utcome Measures: Incidence of heart failure and survival after heart failure dia gnosis. Results: The incidence of heart failure was higher among men (378/100000 persons; 95%confidence interval <<CI>>, 361-395 for men; 289/100000 persons; 95 %CI, 277-300 for women) and did not change over time among men or women. After a mean follow-up of 4.2 years (range, 0-23.8 years), 3347 deaths occurred, in cluding 1930 among women and 1417 among men. Survival after heart failure diagno sis was worse among men than women (relative risk, 1.33; 95%CI, 1.24-1.43) but overall improved over time (5-year age-adjusted survival, 43%in 1979-1984 v s 52%in 1996-2000, P< .001). However, men and younger persons experienced larg er survival gains, contrasting with less or no improvement for women and elderly persons. Conclusion: In this community-based cohort, the incidence of heart fa ilure has not declined during 2 decades, but survival after onset of heart failu re has increased overall, with less improvement among women and elderly persons.展开更多

An analysis (APA Automatic Patent Analysis) has bee...The recent increase in patent granted in China prompts for an analysis of the various strategies which are certainly developed by Chinese applicants in that field. An analysis (APA Automatic Patent Analysis) has been done in several fields which have different impacts on the Chinese economy. We chose four different subjects: solar panels; photovoltaic panels; fireworks; and honeysuckle. These areas go from deep international concern to only Chinese interest. We analyzed in the different ratios between Chinese priority patents, Chinese patents extended to other countries, Patent extended from other countries to China and Chinese utility models. The results show clearly that various directions are followed depending on the economic importance of the field and also the pressure of the competition inside and outside China. In this context the strategic dependence (number of patents extended to China by other countries) was examined for each of the fields. We also show that in the domain where the man power is important such as fireworks the number of utility models is important. This suggests that utility models are used to spread in that field minor innovations. In the fields such as honeysuckle many applicants are the same that the inventors and the involvement of universities larger than in other fields. These two fields more or less centered on Chinese domestic aspects only a few patents (not for Honeysuckle) are extended to other countries. The impact of foreign patents in these two fields is quasi nil. In other domains such as photovoltaic panel the triadic patents (extension to US JP EP) are examined its show that if some Chinese patents are extended to other countries there are also an important numbers of foreign patents extended to China. This also suggests that in these most critical subjects, China develops a sort of technological protection barrier from domestic patents. Parkinson's disease,the second most prevalent neurodegenerative disorder worldwide,is characterized by a progressive loss of dopaminergic neurons in substantia nigra pars compacta,causing motor symptoms.This disor...Parkinson's disease,the second most prevalent neurodegenerative disorder worldwide,is characterized by a progressive loss of dopaminergic neurons in substantia nigra pars compacta,causing motor symptoms.This disorder's main hallmark is the formation of intraneuronal protein inclusions,named Lewy bodies and neurites.The major component of these arrangements is α-synuclein,an intrinsically disordered and soluble protein that,in pathological conditions,can form toxic and cell-to-cell transmissible amyloid structures.Preventing α-synuclein aggregation has attracted significant effort in the search for a disease-modifying therapy for Parkinson's disease.Small molecules like Synu Clean-D,epigallocatechin gallate,trodusquemine,or anle138 b exemplify this therapeutic potential.Here,we describe a subset of compounds containing a single aromatic ring,like dopamine,ZPDm,gallic acid,or entacapone,which act as molecular chaperones against α-synuclein aggregation.The simplicity of their structures contrasts with the complexity of the aggregation process,yet the block efficiently α-synuclein assembly into amyloid fibrils,in many cases,redirecting the reaction towards the formation of non-toxic off-pathway oligomers.Moreover,some of these compounds can disentangle mature α-synuclein amyloid fibrils.Their simple structures allow structure-activity relationship analysis to elucidate the role of different functional groups in the inhibition of α-synuclein aggregation and fibril dismantling,making them informative lead scaffolds for the rational development of efficient drugs.展开更多

This study quantifies, with the help of density functional(DFT) calculations and a novel topological approach, atomic ordering and surface segregation effects in Pd-Rh particles with compositions 1:3, 1:1 and 3:1 containing up to 201 atoms(ca. 1.7 nm). The obtained data are used to reliably optimise energetically preferred atomic orderings in inaccessible by DFT Pd-Rh particles containing thousands of atoms and exhibiting sizes exceeding 5 nm, which are typical for catalytic metal particles. It is outlined, how segregation effects on the surface arrangement of Pd-Rh nanoalloy catalysts induced by adsorbates can be evaluated in a simple way within the present modelling setup.展开更多
Monkeypox(mpox),is a disease from the Poxviridae family that can cause several serious medical issues.This mini-review sought to analyze the existing literature regarding the current mpox outbreak with a focus on the ...Monkeypox(mpox),is a disease from the Poxviridae family that can cause several serious medical issues.This mini-review sought to analyze the existing literature regarding the current mpox outbreak with a focus on the prevalence,diagnostics,and containment measures.Mpox cases have been reported to World Health Organization(WHO)from 85 Member States in all six WHO regions during the period of January 1,2022,through August 3,2022.Standardized or optimized guidelines for the clinical care of patients with mpox are limited,particularly in low-resource settings.In an effort to achieve guidance and meet standards,special attention should be paid to this outbreak in order to eradicate such a rare infectious disease by analyzing prevention and control measures.Patient outcomes may also be poor,and their illnesses may last for a long time.The spectrum of clinical symptoms,including complications and sequelae,as well as aspects of the illness may be indicators of sickness severity and complications;therefore,its clinical presentation must be better understood to improve containment measures.In addition,it is important to create and evaluate a standard of care that takes a variety of parameters into account,including antiviral,immune therapies,and clinical metrics that are particular to mpox.The global emergence of mpox has presented new challenges for public health and has called for further investigation into its epidemiological profile across international contexts.展开更多
文摘BACKGROUND Metabolic dysfunction-associated steatotic liver disease(MASLD),hepatic fibrosis,and cirrhosis are major risk factors for hepatocellular carcinoma(HCC),yet current blood-based diagnostic assays lack sufficient accuracy for routine clinical use.Identifying a non-invasive molecular signature that accurately detects liver disease could improve early diagnosis and monitoring.We hypothesized that the Liver FibraChek Dx■serum assay could discriminate MASLD and HCC from healthy controls using a multiplex biomarker-based algorithm.AIM To evaluate the diagnostic performance of the Liver FibraChek Dx■assay for detecting MASLD and HCC.METHODS This was a prospective,single-center study conducted in a United States tertiary care setting.Serum samples were collected from 45 participants(14 MASLD,19 HCC,12 healthy controls)with liver histology confirmed by biopsy.The Liver FibraChek Dx■algorithm integrates weighted values of aspartate aminotransferase,alanine aminotransferase,taurocholic acid,L-tyrosine,platelet count,and patient age to generate a risk score.Wilcoxon rank sum tests were used to assess associations with histologic diagnosis,and receiver operating characteristic(ROC)curves quantified diagnostic performance.RESULTS Liver FibraChek Dx■risk scores were significantly elevated in MASLD and HCC compared to controls(median:6.92±3.86 vs 3.61±1.67,P<0.001).The area under the ROC curve was 0.890(95%CI:0.776-1.000)for distinguishing diseased from healthy individuals.Sensitivity was 93.9%,specificity 75.0%,positive predictive value 91.1%,negative predictive value 81.8%,and overall accuracy 88.9%.CONCLUSION The Liver FibraChek Dx■assay accurately detects liver disease and shows promise as a non-invasive tool for diagnosing and monitoring MASLD and HCC.

文摘The additive manufacturing(AM)landscape has significantly transformed in alignment with Industry 4.0 principles,primarily driven by the integration of artificial intelligence(AI)and digital twins(DT).However,current intelligent AM(IAM)systems face limitations such as fragmented AI tool usage and suboptimal human-machine interaction.This paper reviews existing IAM solutions,emphasizing control,monitoring,process autonomy,and end-to-end integration,and identifies key limitations,such as the absence of a high-level controller for global decision-making.To address these gaps,we propose a transition from IAM to autonomous AM,featuring a hierarchical framework with four integrated layers:knowledge,generative solution,operational,and cognitive.In the cognitive layer,AI agents notably enable machines to independently observe,analyze,plan,and execute operations that traditionally require human intervention.These capabilities streamline production processes and expand the possibilities for innovation,particularly in sectors like in-space manufacturing.Additionally,this paper discusses the role of AI in self-optimization and lifelong learning,positing that the future of AM will be characterized by a symbiotic relationship between human expertise and advanced autonomy,fostering a more adaptive,resilient manufacturing ecosystem.

文摘Chemical hydrogen storage in organic materials is a promising method thanks to its high storage density,reversibility,and safety.However,the dehydrogenation process of organic materials requires high temperatures due to their unfavorable thermodynamic properties.This study proposes a strategy to design a new type of hydrogen storage materials,i.e.,alkali metal pyridinolate/piperidinolate pairs,by combining the effects of a heteroatom and an alkali metal in one molecule to achieve suitable dehydrogenation thermodynamics along with high hydrogen storage capacities.These air-stable compounds can be synthesized using low-cost reactants and water as a green solvent.Thermodynamic predictions indicate that enthalpy changes of dehydrogenation(ΔH_(d))can be significantly reduced to the optimal range for efficient hydrogen release,exemplified by lithium 2-piperidinolate with a 5.6 wt%hydrogen capacity and a suitableΔH_(d)of 32.2 kJ/mol-H_(2).Experimental results obtained using sodium systems validate the computational predictions,demonstrating reversible hydrogen storage even below 100℃.The superior hydrogen desorption performance of alkali metal piperidinolates could be attributed to their suitableΔH_(d)induced by the combined effect of ring nitrogen and metal substitution on their structures.This study not only reports new low-cost hydrogen storage materials but also provides a rational design strategy for developing metalorganic compounds possessing high hydrogen capacities and suitable thermodynamics for efficient hydrogen storage.

文摘Magnesium(Mg)-based materials are promising for lightweight structural applications.However,their widespread adoption is significantly constrained by inherent limitations in mechanical properties.To address this challenge,this study introduces a novel Mg-based interpenetratingphase composite reinforced with a nickel-titanium(NiTi)scaffold featuring a triply periodic minimal surface(TPMS)configuration.By combining experimental investigations with finite element simulations,we systematically elucidate the dual impact of the scaffold’s unit cell size(a)on manufacturing viability and mechanical enhancement.To compensate for compromised infiltration dynamics induced by decreasing a,a critical permeability threshold of 1×10^(-8) m^(2) is proposed for achieving successful composite fabrication.Mechanically,reducing a strengthens the interaction between the scaffold and matrix:the TPMS-configured NiTi scaffolds improve stress transfer,deflect crack propagation,and facilitate damage delocalization,whereas the Mg matrix preserves structural integrity and enables load redistribution.Consequently,the composites significantly outperform pure Mg,and lowering a leads to more substantial enhancements in compressive strength,energy dissipation,and deformation recoverability.This study offers valuable insight into the design and fabrication of highperformance Mg-based materials for structural and biomedical applications.

文摘The mechanical properties of minerals in planetary materials are not only interesting from a fundamental point of view but also critical to the development of future space missions.Here we present nanoindentation experiments to evaluate the hardness and reduced elastic modulus of olivine,(Mg,Fe)_(2)SiO_(4),in meteorite NWA 12008,a lunar basalt.Our experiments suggest that the olivine grains in this lunaite are softer and more elastic than their terrestrial counterparts.Also,we have performed synchrotron-based high-pressure X-ray diffraction(HP-XRD)measurements to probe the compressibility properties of olivine in this meteorite and,for comparison purposes,of three ordinary chondrites.The HP-XRD results suggest that the axial compressibility of the orthorhombic b lattice parameter of olivine relative to terrestrial olivine is higher in NWA 12008 and also in the highly-shocked Chelyabinsk meteorite.The origin of the observed differences is discussed.A simple model combining the results of both our nanoindentation and HP-XRD measurements allows us to describe the contribution of macroscopic and chemical-bond related effects,both of which are necessary to reproduce the observed elastic modulus softening.Such joint analysis of the mechanical and elastic properties of meteorites and returned samples opens up a new avenue for characterizing these highly interesting materials.

文摘Amid the scarcity of lunar meteorites and the imperative to preserve their scientific value,nondestructive testing methods are essential.This translates into the application of microscale rock mechanics experiments and scanning electron microscopy for surface composition analysis.This study explores the application of Machine Learning algorithms in predicting the mineralogical and mechanical properties of DHOFAR 1084,JAH 838,and NWA 11444 lunar meteorites based solely on their atomic percentage compositions.Leveraging a prior-data fitted network model,we achieved near-perfect classification scores for meteorites,mineral groups,and individual minerals.The regressor models,notably the KNeighbor model,provided an outstanding estimate of the mechanical properties—previously measured by nanoindentation tests—such as hardness,reduced Young’s modulus,and elastic recovery.Further considerations on the nature and physical properties of the minerals forming these meteorites,including porosity,crystal orientation,or shock degree,are essential for refining predictions.Our findings underscore the potential of Machine Learning in enhancing mineral identification and mechanical property estimation in lunar exploration,which pave the way for new advancements and quick assessments in extraterrestrial mineral mining,processing,and research.
文摘This research paper has provided the methodology and design for implementing the hybrid author recommender system using Azure Data Lake Analytics and Power BI. It offers a recommendation for the top 1000 Authors of computer science in different fields of study. The technique used in this paper is handling the inadequate Information for citation;it removes the problem of cold start, which is encountered by very many other recommender systems. In this paper, abstracts, the titles, and the Microsoft academic graphs have been used in coming up with the recommendation list for every document, which is used to combine the content-based approaches and the co-citations. Prioritization and the blending of every technique have been allowed by the tuning system parameters, allowing for the authority in results of recommendation versus the paper novelty. In the end, we do observe that there is a direct correlation between the similarity rankings that have been produced by the system and the scores of the participant. The results coming from the associated scrips of analysis and the user survey have been made available through the recommendation system. Managers must gain the required expertise to fully utilize the benefits that come with business intelligence systems [1]. Data mining has become an important tool for managers that provides insights about their daily operations and leverage the information provided by decision support systems to improve customer relationships [2]. Additionally, managers require business intelligence systems that can rank the output in the order of priority. Ranking algorithm can replace the traditional data mining algorithms that will be discussed in-depth in the literature review [3].
文摘AIM To investigate the epidemiology,treatment and outcomes of acute appendicitis(AA)in a large population study.METHODS This is a retrospective cohort study derived from the administrative dataset of the Bergamo district healthcare system(more than 1 million inhabitants)from 1997 to 2013.Data about treatment,surgery,length of stay were collected.Moreover for each patients were registered data about relapse of appendicitis and hospital admission due to intestinal obstruction.RESULTS From 1997 to 2013 in the Bergamo district we collected 16544 cases of AA,with a crude incidence rate of 89/100000 inhabitants per year;mean age was 24.51±16.17,54.7%were male and the mean Charlson's comorbidity index was 0.32±0.92.Mortality was<0.0001%.Appendectomy was performed in 94.7%of the patients and the mean length of stay was 5.08±2.88 d;the cumulative hospital stay was 5.19±3.36 d and 1.2%of patients had at least one further hospitalization due intestinal occlusion.Laparoscopic appendectomy was performed in 48%of cases.Percent of 5.34 the patients were treated conservatively with a mean length of stay of 3.98±3.96 d;the relapse rate was 23.1%and the cumulative hospital stay during the study period was 5.46±6.05 d.CONCLUSION The treatment of acute appendicitis in Northern Italy is slowly changing,with the large diffusion of laparoscopic approach;conservative treatment of non-complicated appendicitis is still a neglected option,but rich of promising results.

文摘Bimetallic additively manufactured structures(BAMSs)can replace traditionally-fabricated functionallygraded-components through fusion welding processes and can eliminate locally-deteriorated mechanical properties arising from post-processing.The present work fabricates a BAMS by sequentially depositing the austenitic stainless-steel and Inconel625 using a gas-metal-arc-welding(GMAW)-based wire+arc additive manufacturing(WAAM)system.Elemental mapping shows a smooth compositional transition at the interface without any segregation.Both materials being the face-center-cubic(FCC)austenite,the electron backscattered diffraction(EBSD)analysis of the interface shows the smooth and cross-interfacecrystallographic growth of long-elongated grains in the<001>direction.The hardness values were within the range of 220-240 HV for both materials without a large deviation at the interface.Due to the controlled thermal history,mechanical testing yielded a consistent result with the ultimate tensile strength and elongation of 600 MPa and 40%,respectively,with the failure location on the stainless-steel side.This study demonstrates that WAAM has the potential to fabricate BAMS with controlled properties.

文摘One kind of steepest descent incremental projection learning algorithm for improving the training of radial basis function(RBF)neural network is proposed,which is applied to analog circuit fault isolation.This algorithm simplified the structure of network through optimum output layer coefficient with incremental projection learning(IPL)algorithm,and adjusted the parameters of the neural activation function to control the network scale and improve the network approximation ability.Compared to the traditional algorithm,the improved algorithm has quicker convergence rate and higher isolation precision.Simulation results show that this improved RBF network has much better performance,which can be used in analog circuit fault isolation field.

文摘BACKGROUND While colorectal polyps are not cancerous,some types of polyps,known as adenomas,can develop into colorectal cancer over time.Polyps can often be found and removed by colonoscopy;however,this is an invasive and expensive test.Thus,there is a need for new methods of screening patients at high risk of developing polyps.AIM To identify a potential association between colorectal polyps and small intestine bacteria overgrowth(SIBO)or other relevant factors in a patient cohort with lactulose breath test(LBT)results.METHODS A total of 382 patients who had received an LBT were classified into polyp and non-polyp groups that were confirmed by colonoscopy and pathology.SIBO was diagnosed by measuring LBTderived hydrogen(H)and methane(M)levels according to 2017 North American Consensus recommendations.Logistic regression was used to assess the ability of LBT to predict colorectal polyps.Intestinal barrier function damage(IBFD)was determined by blood assays.RESULTS H and M levels revealed that the prevalence of SIBO was significantly higher in the polyp group than in the non-polyp group(41%vs 23%,P<0.01;71%vs 59%,P<0.05,respectively).Within 90 min of lactulose ingestion,the peak H values in the adenomatous and inflammatory/hyperplastic polyp patients were significantly higher than those in the non-polyp group(P<0.01,and P=0.03,respectively).In 227 patients with SIBO defined by combining H and M values,the rate of IBFD determined by blood lipopolysaccharide levels was significantly higher among patients with polyps than those without(15%vs 5%,P<0.05).In regression analysis with age and gender adjustment,colorectal polyps were most accurately predicted with models using M peak values or combined H and M values limited by North American Consensus recommendations for SIBO.These models had a sensitivity of≥0.67,a specificity of≥0.64,and an accuracy of≥0.66.CONCLUSION The current study made key associations among colorectal polyps,SIBO,and IBFD and demonstrated that LBT has moderate potential as an alternative noninvasive screening tool for colorectal polyps.
文摘The disease coronavirus disease 2019(COVID-19)is a severe respiratory illness that has emerged as a devastating health problem worldwide.The disease outcome is heterogeneous,and severity is likely dependent on the immunity of infected individuals and comorbidities.Although symptoms of the disease are primarily associated with respiratory problems,additional infection or failure of other vital organs are being reported.Emerging reports suggest a quite common co-existence of gastrointestinal(GI)tract symptoms in addition to respiratory symptoms in many COVID-19 patients,and some patients show just the GI symptoms.The possible cause of the GI symptoms could be due to direct infection of the epithelial cells of the gut,which is supported by the fact that(1)The intestinal epithelium expresses a high level of angiotensin-converting enzyme-2 and transmembrane protease serine 2 protein that are required for the severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)entry into the cells;(2)About half of the severe COVID-19 patients show viral RNA in their feces and various parts of the GI tract;and(3)SARS-CoV-2 can directly infect gut epithelial cells in vitro(gut epithelial cells and organoids)and in vivo(rhesus monkey).The GI tract seems to be a site of active innate and adaptive immune responses to SARS-CoV-2 as clinically,stool samples of COVID-19 patients possess proinflammatory cytokines(interleukin 8),calprotectin(neutrophils activity),and immunoglobulin A antibodies.In addition to direct immune activation by the virus,impairment of GI epithelium integrity can evoke immune response under the influence of systemic cytokines,hypoxia,and changes in gut microbiota(dysbiosis)due to infection of the respiratory system,which is confirmed by the observation that not all of the GI symptomatic patients are viral RNA positive.This review comprehensively summarizes the possible GI immunomodulation by SARS-CoV-2 that could lead to GI symptoms,their association with disease severity,and potential therapeutic interventions.
文摘Background: The diagnosis of autoimmune pancreatitis can be difficult and ofte n requires a larger specimen than can be provided by FNA alone to determine if t he tissue sample obtained with EUS trucut biopsy (TCB) is sufficient to allow ad equate histologic review to establish the diagnosis of autoimmune pancreatitis. Methods: EUS TCB was performed in patients presenting with obstructive jaundice who were suspected of having autoimmune pancreatitis based on their clinical, la boratory and imaging studies. The charts were retrospectively reviewed to determ ine the feasibility of TCB. Results: Between August 2002 and June 2004, 3 patien ts with obstructive jaundice and suspected autoimmune pancreatitis (AIP) underwe nt EUS TCB. In each case, a diagnosis of pancreatic cancer also was considered, and surgical resection was the planned therapy before the patient underwent EUS TCB. Histologic review of the TCB specimens established the diagnosis of AIP in two patients and identified nonspecific changes of chronic pancreatitis in the t hird patient. EUS-guided FNA was performed in two of the 3 patients and failed to establish the diagnosis in either patient. Other than mild transient abdomina l pain (n = 1), no complications were identified. Conclusions: This preliminary study suggests that EUS TCB can safely establish the diagnosis of AIP. Doing so helps guide management and may help to avoid unnecessary surgery. Prosp- ective studies are needed to verify these findings and to more clearly define the role of EUS TCB in these patients.

文摘Considering the deficiency of the means for confirming the attribution of fault redundancy in the re-search of Automatic Testing System(ATS) , a fault-injection system has been proposed to study fault redundancyof automatic testing system through compurison. By means of a fault-imbeded environmental simulation, thefaults injected at the input level of the software are under test. These faults may induce inherent failure mode,thus bringing about unexpected output, and the anticipated goal of the test is attained. The fault injection con-sists of voltage signal generator, current signal generator and rear drive circuit which are specially developed,and the ATS can work regularly by means of software simulation. The experimental results indicate that the faultinjection system can find the deficiency of the automatic testing software, and identify the preference of fault re-dundancy. On the other hand, some soft deficiency never exposed before can be identified by analyzing the tes-ting results.

文摘The brain pericyte is a unique and indispensable part of the blood-brain barrier(BBB),and contributes to several pathological processes in traumatic brain injury(TBI).However,the cellular and molecular mechanisms by which pericytes are regulated in the damaged brain are largely unknown.Here,we show that the formation of neutrophil extracellular traps(NETs)induces the appearance of CD11b^(+)pericytes after TBI.These CD11b^(+)pericyte subsets are characterized by increased permeability and pro-inflammatory profiles compared to CD11b-pericytes.Moreover,histones from NETs by Dectin-1 facilitate CD11b induction in brain pericytes in PKC-c-Jun dependent manner,resulting in neuroinflammation and BBB dysfunction after TBI.These data indicate that neutrophil-NET-pericyte and histone-Dectin-1-CD11b are possible mechanisms for the activation and dysfunction of pericytes.Targeting NETs formation and Dectin-1 are promising means of treating TBI.
文摘Context: The epidemic of heart failure has yet to be fully investigated , and d ata on incidence, survival, and sexspecific temporal trends in community-based populations are limited. Objective: To test the hypothesis that the incidence of heart failure has declined and survival after heart failure diagnosis has improved over time but that secular trends have diverged by sex. Design, Setting, and Participants: Population-based cohort study using the resources of the Rochester Epidemiology Project conducted in Olmsted County , Minnesota. Patients were 4537 Olmsted County residents (57%women; mean <<SD>> a ge, 74 <<14>> years) with a diagnosis of heart failure between 1979 and 2000. Fram ingham criteria and clinical criteria were used to validate the diagnosis Main O utcome Measures: Incidence of heart failure and survival after heart failure dia gnosis. Results: The incidence of heart failure was higher among men (378/100000 persons; 95%confidence interval <<CI>>, 361-395 for men; 289/100000 persons; 95 %CI, 277-300 for women) and did not change over time among men or women. After a mean follow-up of 4.2 years (range, 0-23.8 years), 3347 deaths occurred, in cluding 1930 among women and 1417 among men. Survival after heart failure diagno sis was worse among men than women (relative risk, 1.33; 95%CI, 1.24-1.43) but overall improved over time (5-year age-adjusted survival, 43%in 1979-1984 v s 52%in 1996-2000, P< .001). However, men and younger persons experienced larg er survival gains, contrasting with less or no improvement for women and elderly persons. Conclusion: In this community-based cohort, the incidence of heart fa ilure has not declined during 2 decades, but survival after onset of heart failu re has increased overall, with less improvement among women and elderly persons.
文摘The recent increase in patent granted in China prompts for an analysis of the various strategies which are certainly developed by Chinese applicants in that field. An analysis (APA Automatic Patent Analysis) has been done in several fields which have different impacts on the Chinese economy. We chose four different subjects: solar panels; photovoltaic panels; fireworks; and honeysuckle. These areas go from deep international concern to only Chinese interest. We analyzed in the different ratios between Chinese priority patents, Chinese patents extended to other countries, Patent extended from other countries to China and Chinese utility models. The results show clearly that various directions are followed depending on the economic importance of the field and also the pressure of the competition inside and outside China. In this context the strategic dependence (number of patents extended to China by other countries) was examined for each of the fields. We also show that in the domain where the man power is important such as fireworks the number of utility models is important. This suggests that utility models are used to spread in that field minor innovations. In the fields such as honeysuckle many applicants are the same that the inventors and the involvement of universities larger than in other fields. These two fields more or less centered on Chinese domestic aspects only a few patents (not for Honeysuckle) are extended to other countries. The impact of foreign patents in these two fields is quasi nil. In other domains such as photovoltaic panel the triadic patents (extension to US JP EP) are examined its show that if some Chinese patents are extended to other countries there are also an important numbers of foreign patents extended to China. This also suggests that in these most critical subjects, China develops a sort of technological protection barrier from domestic patents. This underlines a strategy on attack and defense in that field.
文摘Parkinson's disease,the second most prevalent neurodegenerative disorder worldwide,is characterized by a progressive loss of dopaminergic neurons in substantia nigra pars compacta,causing motor symptoms.This disorder's main hallmark is the formation of intraneuronal protein inclusions,named Lewy bodies and neurites.The major component of these arrangements is α-synuclein,an intrinsically disordered and soluble protein that,in pathological conditions,can form toxic and cell-to-cell transmissible amyloid structures.Preventing α-synuclein aggregation has attracted significant effort in the search for a disease-modifying therapy for Parkinson's disease.Small molecules like Synu Clean-D,epigallocatechin gallate,trodusquemine,or anle138 b exemplify this therapeutic potential.Here,we describe a subset of compounds containing a single aromatic ring,like dopamine,ZPDm,gallic acid,or entacapone,which act as molecular chaperones against α-synuclein aggregation.The simplicity of their structures contrasts with the complexity of the aggregation process,yet the block efficiently α-synuclein assembly into amyloid fibrils,in many cases,redirecting the reaction towards the formation of non-toxic off-pathway oligomers.Moreover,some of these compounds can disentangle mature α-synuclein amyloid fibrils.Their simple structures allow structure-activity relationship analysis to elucidate the role of different functional groups in the inhibition of α-synuclein aggregation and fibril dismantling,making them informative lead scaffolds for the rational development of efficient drugs.

文摘Pd-Rh nanoparticles are known to easily undergo surface restructuring in reactive environment. This study quantifies, with the help of density functional(DFT) calculations and a novel topological approach, atomic ordering and surface segregation effects in Pd-Rh particles with compositions 1:3, 1:1 and 3:1 containing up to 201 atoms(ca. 1.7 nm). The obtained data are used to reliably optimise energetically preferred atomic orderings in inaccessible by DFT Pd-Rh particles containing thousands of atoms and exhibiting sizes exceeding 5 nm, which are typical for catalytic metal particles. It is outlined, how segregation effects on the surface arrangement of Pd-Rh nanoalloy catalysts induced by adsorbates can be evaluated in a simple way within the present modelling setup.
文摘Monkeypox(mpox),is a disease from the Poxviridae family that can cause several serious medical issues.This mini-review sought to analyze the existing literature regarding the current mpox outbreak with a focus on the prevalence,diagnostics,and containment measures.Mpox cases have been reported to World Health Organization(WHO)from 85 Member States in all six WHO regions during the period of January 1,2022,through August 3,2022.Standardized or optimized guidelines for the clinical care of patients with mpox are limited,particularly in low-resource settings.In an effort to achieve guidance and meet standards,special attention should be paid to this outbreak in order to eradicate such a rare infectious disease by analyzing prevention and control measures.Patient outcomes may also be poor,and their illnesses may last for a long time.The spectrum of clinical symptoms,including complications and sequelae,as well as aspects of the illness may be indicators of sickness severity and complications;therefore,its clinical presentation must be better understood to improve containment measures.In addition,it is important to create and evaluate a standard of care that takes a variety of parameters into account,including antiviral,immune therapies,and clinical metrics that are particular to mpox.The global emergence of mpox has presented new challenges for public health and has called for further investigation into its epidemiological profile across international contexts.
