Android Smartphones are proliferating extensively in the digital world due to their widespread applications in a myriad offields.The increased popularity of the android platform entices malware developers to design ma...Android Smartphones are proliferating extensively in the digital world due to their widespread applications in a myriad offields.The increased popularity of the android platform entices malware developers to design malicious apps to achieve their malevolent intents.Also,static analysis approaches fail to detect run-time behaviors of malicious apps.To address these issues,an optimal unification of static and dynamic features for smartphone security analysis is proposed.The proposed solution exploits both static and dynamic features for generating a highly distinct unified feature vector using graph based cross-diffusion strategy.Further,a unified feature is subjected to the fuzzy-based classification model to distinguish benign and malicious applications.The suggested framework is extensively experimentally validated through both qualitative and quantitative analysis and results are compared with the existing solutions.Performance evaluation over benchmarked datasets from Google Play Store,Drebin,Androzoo,AMD,and CICMalDroid2020 revealed that the suggested solution outperforms state-of-the-art methods.We achieve average detection accuracy of 98.62%and F1 Score of 0.9916.展开更多

Since the outbreak of the Covid-19 pandemic, thousands of people have expressed their concerns and worries about the outbreak via Weibo, showing the existence of public panic. Methods: This paper comes up with a sentiment analysis approach to discover public panic. First, we used Octoparse to obtain Weibo posts about the hot topic Covid-19 Pandemic. Second, we break down those sentences into independent words and clean the data by removing stop words. Then, we use the sentiment score function that deals with negative words, adverbs, and sentiment words to get the sentiment score of each Weibo post. Results: We observe the distribution of sentiment scores and get the benchmark to evaluate public panic. Also, we apply the same process to test the mass sentiment under other topics to test the efficiency of the sentiment function, which shows that our function works well.展开更多
The present article aims to examine the heat and mass distribution in a free convection flow of electrically conducted,generalized Jeffrey nanofluid in a heated rotatory system.The flow analysis is considered in the p...The present article aims to examine the heat and mass distribution in a free convection flow of electrically conducted,generalized Jeffrey nanofluid in a heated rotatory system.The flow analysis is considered in the presence of thermal radiation and the transverse magnetic field of strength B0.The medium is porous accepting generalized Darcy’s law.The motion of the fluid is due to the cosine oscillations of the plate.Nanofluid has been formed by the uniform dispersing of the Silver nanoparticles in regular engine oil.The problem has been modeled in the form of classical partial differential equations and then generalized by replacing time derivative with Atangana–Baleanu(AB)time-fractional derivative.Upon taking the Laplace transform technique(LTT)and using physical boundary conditions,exact expressions have been obtained for momentum,energy,and concentration distributions.The impact of a number of parameters on fluid flow is shown graphically.The numerical tables have been computed for variation in the rate of heat and mass transfer with respect to rooted parameters.Finally,the classical solution is recovered by taking the fractional parameter approaching unity.It is worth noting that by adding silver nanoparticles in regular engine oil,its heat transfer rate increased by 14.59%,which will improve the life and workability of the engine.展开更多
It is a very difficult task for the researchers to find the exact solutions to mathematical problems that contain non-linear terms in the equation.Therefore,this article aims to investigate the viscous dissipation(VD)...It is a very difficult task for the researchers to find the exact solutions to mathematical problems that contain non-linear terms in the equation.Therefore,this article aims to investigate the viscous dissipation(VD)effect on the fractional model of Jeffrey fluid over a heated vertical flat plate that suddenly moves in its own plane.Based on the Atangana-Baleanu operator,the fractional model is developed from the fractional constitutive equations.VD is responsible for the non-linear behavior in the problem.Upon taking the Laplace and Fourier sine transforms,exact expressions have been obtained for momentum and energy equations.The influence of relative parameters on fluid flow and temperature distribution is shown graphically.As special cases,and for the sake of correctness,the corresponding results for second-grade fluid and Newtonian viscous fluid are also obtained.It is interesting to note that fractional parameterαprovides more than one line as compared to the classical model.This effect represents the memory effect in the fluid which is not possible to elaborate by the classical model.It is also worth noting that the temperature profile of the generalized Jeffrey fluid rises for higher values of Eckert number which is due to the enthalpy difference of the boundary layer.展开更多

The targeted devices are the following: PHTS (primary heat transport system), SHTS (secondary heat transport system), WS (water and steam system), PPS (plant protection system) and PCS (plant control system). In this paper, the frequency of automatic reactor trips was estimated by extracting and analyzing the important intermediate events and equipment failures covering all the derived fault trees of these systems. The analyses predicted 1.2/RY (reactor year) the value of unplanned shut down frequency by the internal factor of the system. The largest contributed event was function failure of SHTS accounting for 42.6% of total events followed by PHTS with 40.1%. The contribution factor of WS was only 4.4%.展开更多
In this paper,we analyze the article published by El Labban et al,which explores the impact of cirrhosis on patients with necrotizing fasciitis.The authors conclude that cirrhosis is a significant risk factor for incr...In this paper,we analyze the article published by El Labban et al,which explores the impact of cirrhosis on patients with necrotizing fasciitis.The authors conclude that cirrhosis is a significant risk factor for increased in-hospital morbidity and mortality in this patient population.Building upon their final observation regarding the importance of understanding this association,we will delve into the topic of infections in patients with liver cirrhosis.These patients exhibit intrinsic characteristics that make them particularly susceptible to infections,both bacterial and fungal.This heightened risk not only increases the likelihood of severe infections but also makes them a common trigger for acute decompensations,including the development of acute-on-chronic liver failure,which markedly worsens prognosis and mortality.Infections in patients with cirrhosis often require a more aggressive and rapid diagnostic and therapeutic approach due to the higher risk of nosocomial infections,multidrug-resistant organisms,and atypical clinical presentations.Delayed or inadequate management can lead to unfavorable outcomes,further complicating the course of their underlying liver disease.The aim of this article is to emphasize the importance of early and appropriate management in patients with cirrhosis with infections.Evidence supports that timely and tailored interventions not only improve clinical outcomes but also reduce mortality.By raising awareness among clinicians about the complexity of these cases,we hope to contribute to optimizing the care of this high-risk population.展开更多

Here, we provide an overview of the most notable extreme events of the year, including extreme precipitation and floods, tropical cyclones, and droughts. The characteristics and impacts of these extreme events are summarized, followed by discussion on the physical drivers and the role of global warming.Finally, we also discuss the future prospects in extreme event studies, including impact-based perspectives, challenges in attribution of precipitation extremes, and the existing gap to minimize impacts from climate extremes.展开更多

The glove employs custom-designed flexible bend sensors with carbon-impregnated film for in-situ joint angle tracking, simplifying the sensing system and enhancing portability. A multi-modal haptic feedback approach includes an innovative pneumatically actuated tactile feedback technique and a motor-tendon-driven kinaesthetic feedback system, providing exceptional realism in virtual object manipulation. The glove’s kinaesthetic feedback lets users perceive virtual objects’ size, shape, and stiffness characteristics. Psychophysical investigations demonstrate how readily the users acclimate to this hardware and prove each module’s effectiveness and synergistic operation. Lynch syndrome(LS),also known as hereditary non-polyposis colorectal cancer(HNPCC),is an inherited condition associated with a higher risk of colorectal cancer(CRC)and other cancers.It is caused by germline mutations ...Lynch syndrome(LS),also known as hereditary non-polyposis colorectal cancer(HNPCC),is an inherited condition associated with a higher risk of colorectal cancer(CRC)and other cancers.It is caused by germline mutations in DNA mismatch repair(MMR)genes,including MLH1,MSH2,MSH6 and PMS2.These mutations lead to microsatellite instability(MSI)and defective DNA repair mechanisms,resulting in increased cancer risk.Early detection of LS is crucial for effective management and cancer prevention.Endoscopic surveillance,particularly regular colonoscopy,is recommended for individuals with LS to detect CRC at early stages.Additionally,universal screening of CRC for MMR deficiency can help identify at-risk individuals.Genetic counseling plays a valuable role in LS by guiding patients and their families in understanding the genetic basis,making informed decisions regarding surveillance and prevention,and offering reproductive options to reduce the transmission of pathogenic variants of the offspring.The aim of this review is to outline current strategies for the diagnosis,surveillance,and management of LS,with a focus on the role of genetic counseling,endoscopic screening,and emerging therapeutic approaches to mitigate cancer risk in affected individuals.展开更多
BACKGROUND Post-polypectomy syndrome(PPS)is a rare but relevant complication of endo-scopic colorectal polyp removal.Although its course is usually benign,it may conceal more severe conditions.This case report present...BACKGROUND Post-polypectomy syndrome(PPS)is a rare but relevant complication of endo-scopic colorectal polyp removal.Although its course is usually benign,it may conceal more severe conditions.This case report presents several complications resulting from a colorectal polypectomy.CASE SUMMARY We report the case of a 67-year-old man who presented with asthenia and abdo-minal discomfort after a scheduled polypectomy.The patient was initially diagnosed with PPS and managed conservatively.However,progressive clinical deterioration led to hospital admission.Imaging revealed a right-sided intra-abdominal abscess secondary to colonic perforation.The initial polyp was iden-tified as a subepithelial lipoma.The patient underwent successful percutaneous drainage,antibiotic therapy,and recovered without the need for surgical inter-vention.CONCLUSION This case highlights the importance of accurate endoscopic diagnosis and te-chnique selection.PPS should not preclude the consideration of concurrent comp-lications.Early identification and a tailored therapeutic approach can prevent Core Tip:Although therapeutic colonoscopy is generally safe,it may be associated with significant complications.This case illustrates how post-polypectomy syndrome can mask a colonic perforation with subsequent abscess formation.It em-phasizes the importance of an accurate initial endoscopic assessment,the use of advanced resection and closure techniques,and close clinical follow-up.Recognizing this spectrum of complications and intervening early can help prevent major surgical procedures and improve patient outcomes.展开更多

BACKGROUND Acute pancreatitis(AP)is commonly encountered in gastroenterology,with biliary and alcohol-related causes being predominant.Among less frequent etiologies,metabolic and drug-induced origin are the most prev...BACKGROUND Acute pancreatitis(AP)is commonly encountered in gastroenterology,with biliary and alcohol-related causes being predominant.Among less frequent etiologies,metabolic and drug-induced origin are the most prevalent.As an exception,non-pancreatobiliary malignancies may trigger AP,representing less than 1%of cases.We present a case of AP secondary to an uncommon oncologic etiology.CASE SUMMARY We describe the case of a 50-year-old male recently diagnosed with small-cell lung cancer(SCLC).The patient was admitted to the emergency department with acute abdominal pain and subsequently diagnosed with AP.He was hospitalized under the care of the gastroenterology service.During the etiological workup,metastatic pancreatic lesions were identified on imaging,which had not been observed on the initial cancer staging.Following resolution of the initial episode,oral intake was introduced,but the patient experienced recurrent abdominal pain and laboratory abnormalities consistent with new episodes of AP.This pattern repeated over the following days.After several episodes,and with suspicion of a metastatic origin,the case was discussed in multidisciplinary meeting.In agreement with the patient and given the need to initiate treatment for the primary tumor as well,chemotherapy was started.This treatment successfully alleviated symptoms,allowing dietary progression without complications and discharge from the hospital.CONCLUSION Oncologic treatment can be considered as part of the therapeutic approach in AP secondary to SCLC metastasis,especially chemotherapy.展开更多
BACKGROUND Cytomegalovirus(CMV)prophylaxis with valganciclovir and ganciclovir is associated with elevated neutropenia and leukopenia risk in kidney transplant recipients,although the impact of these events on healthc...BACKGROUND Cytomegalovirus(CMV)prophylaxis with valganciclovir and ganciclovir is associated with elevated neutropenia and leukopenia risk in kidney transplant recipients,although the impact of these events on healthcare resource utilization(HCRU)and clinical outcomes is unclear.AIM To quantify clinical events and HCRU associated with neutropenia and leukope-nia among adults receiving valganciclovir and/or ganciclovir post-kidney trans-plantation.METHODS Adult kidney transplant recipients receiving valganciclovir and/or ganciclovir prophylaxis were identified in the TriNetX database from 2012 to 2021.Patient characteristics were evaluated in the 1-year period pre-first transplant.HCRU and adjusted event rates per person-year were evaluated in follow-up year 1 and years 2-5 after first kidney transplantation among cohorts with vs without neutropenia and/or leukopenia.RESULTS Of 15398 identified patients,the average age was 52.39 years and 58.70%were male.Patients with neutropenia and/or leukopenia had greater risk of clinical events for CMV-related events,opportunistic infections,use of granulocyte colony stimulating factor,and hospitalizations(relative risk>1 in year 1 and years 2-5).Patients with vs without neutropenia and/or leukopenia had higher HCRU in year 1 and years 2-5 post kidney transplantation,including the mean number of inpatient admissions(year 1:3.47 vs 2.76;years 2-5:2.70 vs 2.29)and outpatient visits(48.97 vs 34.42;31.73 vs 15.59,respectively),as well as the mean number of labs(1654.55 vs 1182.27;622.37 vs 327.89).CONCLUSION Adults receiving valganciclovir and/or ganciclovir prophylaxis post-kidney transplantation had greater risk of neutropenia and/or leukopenia,which were associated with higher clinical event rates and HCRU up to 5 years post-transplantation.These findings suggest the need for alternative prophylaxis options with lower myelosup-pressive effects to improve patient outcomes.展开更多

In this study, we examined the impact of conservation measures(changes in grassland utilization patterns) on the provision of selected ecosystem services in three types of grasslands(meadow steppe in Hulun Buir, typical steppe in Xilin Gol, and semi-desert steppe in Ordos) in Inner Mongolia. We examined five utilization patterns: no use(natural grasslands), light use, moderate use, intensive use, and recovery sites(degraded sites protected from further use). Through household surveys and vegetation and soil surveys, we measured the differences in ecosystem services among the different grassland utilization patterns. We also identified spatial factors that confounded the quantification of ecosystem services in different types of grasslands. We found that light use generally provided high levels of ecosystem services in meadow steppe and typical steppe, with the main differences in the supporting ecosystem services. Surprisingly, we found no consistently positive impacts of strict conservation activities across the sites, since the results varied spatially and with respect to differences in the land-use patterns. Our study suggests that appropriate grassland utilization patterns can enhance the supply of ecosystem services and reduce negative effects on both household livelihoods and the environment.展开更多

Na-acetate, K-acetate and Nb-pentaethoxide were used as metal precursors an...(Na0.5K0.5)NbO3 (NKN) lead free thin films were synthesized by means of an acetic acid based sol-gel process on Pt/Ti/SiO2/Si substrates. Na-acetate, K-acetate and Nb-pentaethoxide were used as metal precursors and acetic acid as the solvent. The effect of different calcination temperatures on the properties of the NKN films was investigated by X-ray diffraction, scanning electron microscopy, leakage current and hysteresis measurements. Low calcination temperatures led to low currents at high electric fields whereas high calcination temperatures led to low currents at low electric fields. Based on these findings calcination at low temperature was combined with a post annealing treatment. Low leakage currents of 4×10^-4 A/cm2 at 150 kV/cm and 2Pr and 2Ec values of 28 μC/cm2 and 150 kV/cm, respectively, could be obtained. All films were single phase NKN with random crystal orientations and no crack or pore formation was visible on the surface.展开更多

METHODS: RAW 264.7 macrophages were used as a target cell line. Cell viability after treatment with quercetin at different time points was detected by Carboxyfluorescein diacetate, succinimidyl ester. Phagocytic function of macrophages was deter- mined by a fluorometric assay. Cytokine tumor ne- crosis factor a (TNF-a) production was measured by enzyme-linked immunosorbent assay. F-actin cy- toskeleton of L929 cells was stained by Alexa Fluor 488-phalloidin. RESULTS: Pretreatment with quercetin decreasedcell viability only at the highest concentration of 37 μg/mL 2, 24, and 48 h after the treatment. The phagocytic efficiency of macrophages pretreated with quercetin was significantly decreased in a time- and dose-dependent manner. F-actin label- ing showed that the actin cytoskeleton of the cells started to break down 2 h after treatment. More- over, it notably inhibited cytokine TNF-a produc- tion after Candida albicans infection. CONCLUSION: Pretreatment with quercetin in- duced an anti-inflammatory effect against Candida albicans infection in macrophages.展开更多

文摘Android Smartphones are proliferating extensively in the digital world due to their widespread applications in a myriad offields.The increased popularity of the android platform entices malware developers to design malicious apps to achieve their malevolent intents.Also,static analysis approaches fail to detect run-time behaviors of malicious apps.To address these issues,an optimal unification of static and dynamic features for smartphone security analysis is proposed.The proposed solution exploits both static and dynamic features for generating a highly distinct unified feature vector using graph based cross-diffusion strategy.Further,a unified feature is subjected to the fuzzy-based classification model to distinguish benign and malicious applications.The suggested framework is extensively experimentally validated through both qualitative and quantitative analysis and results are compared with the existing solutions.Performance evaluation over benchmarked datasets from Google Play Store,Drebin,Androzoo,AMD,and CICMalDroid2020 revealed that the suggested solution outperforms state-of-the-art methods.We achieve average detection accuracy of 98.62%and F1 Score of 0.9916.
文摘Background: Weibo is a Twitter-like micro-blog platform in China where people post their real-life events as well as express their feelings in short texts. Since the outbreak of the Covid-19 pandemic, thousands of people have expressed their concerns and worries about the outbreak via Weibo, showing the existence of public panic. Methods: This paper comes up with a sentiment analysis approach to discover public panic. First, we used Octoparse to obtain Weibo posts about the hot topic Covid-19 Pandemic. Second, we break down those sentences into independent words and clean the data by removing stop words. Then, we use the sentiment score function that deals with negative words, adverbs, and sentiment words to get the sentiment score of each Weibo post. Results: We observe the distribution of sentiment scores and get the benchmark to evaluate public panic. Also, we apply the same process to test the mass sentiment under other topics to test the efficiency of the sentiment function, which shows that our function works well.
文摘The present article aims to examine the heat and mass distribution in a free convection flow of electrically conducted,generalized Jeffrey nanofluid in a heated rotatory system.The flow analysis is considered in the presence of thermal radiation and the transverse magnetic field of strength B0.The medium is porous accepting generalized Darcy’s law.The motion of the fluid is due to the cosine oscillations of the plate.Nanofluid has been formed by the uniform dispersing of the Silver nanoparticles in regular engine oil.The problem has been modeled in the form of classical partial differential equations and then generalized by replacing time derivative with Atangana–Baleanu(AB)time-fractional derivative.Upon taking the Laplace transform technique(LTT)and using physical boundary conditions,exact expressions have been obtained for momentum,energy,and concentration distributions.The impact of a number of parameters on fluid flow is shown graphically.The numerical tables have been computed for variation in the rate of heat and mass transfer with respect to rooted parameters.Finally,the classical solution is recovered by taking the fractional parameter approaching unity.It is worth noting that by adding silver nanoparticles in regular engine oil,its heat transfer rate increased by 14.59%,which will improve the life and workability of the engine.
文摘It is a very difficult task for the researchers to find the exact solutions to mathematical problems that contain non-linear terms in the equation.Therefore,this article aims to investigate the viscous dissipation(VD)effect on the fractional model of Jeffrey fluid over a heated vertical flat plate that suddenly moves in its own plane.Based on the Atangana-Baleanu operator,the fractional model is developed from the fractional constitutive equations.VD is responsible for the non-linear behavior in the problem.Upon taking the Laplace and Fourier sine transforms,exact expressions have been obtained for momentum and energy equations.The influence of relative parameters on fluid flow and temperature distribution is shown graphically.As special cases,and for the sake of correctness,the corresponding results for second-grade fluid and Newtonian viscous fluid are also obtained.It is interesting to note that fractional parameterαprovides more than one line as compared to the classical model.This effect represents the memory effect in the fluid which is not possible to elaborate by the classical model.It is also worth noting that the temperature profile of the generalized Jeffrey fluid rises for higher values of Eckert number which is due to the enthalpy difference of the boundary layer.
文摘In order to evaluate the operational reliability of Japanese FBR (fast breeder reactor) MONJU, frequencies of important intermediate events and equipment failures resulting during reactor automatic trip are predicted using FTA (fault tree analysis) technique for the plant system model. The targeted devices are the following: PHTS (primary heat transport system), SHTS (secondary heat transport system), WS (water and steam system), PPS (plant protection system) and PCS (plant control system). In this paper, the frequency of automatic reactor trips was estimated by extracting and analyzing the important intermediate events and equipment failures covering all the derived fault trees of these systems. The analyses predicted 1.2/RY (reactor year) the value of unplanned shut down frequency by the internal factor of the system. The largest contributed event was function failure of SHTS accounting for 42.6% of total events followed by PHTS with 40.1%. The contribution factor of WS was only 4.4%.
文摘In this paper,we analyze the article published by El Labban et al,which explores the impact of cirrhosis on patients with necrotizing fasciitis.The authors conclude that cirrhosis is a significant risk factor for increased in-hospital morbidity and mortality in this patient population.Building upon their final observation regarding the importance of understanding this association,we will delve into the topic of infections in patients with liver cirrhosis.These patients exhibit intrinsic characteristics that make them particularly susceptible to infections,both bacterial and fungal.This heightened risk not only increases the likelihood of severe infections but also makes them a common trigger for acute decompensations,including the development of acute-on-chronic liver failure,which markedly worsens prognosis and mortality.Infections in patients with cirrhosis often require a more aggressive and rapid diagnostic and therapeutic approach due to the higher risk of nosocomial infections,multidrug-resistant organisms,and atypical clinical presentations.Delayed or inadequate management can lead to unfavorable outcomes,further complicating the course of their underlying liver disease.The aim of this article is to emphasize the importance of early and appropriate management in patients with cirrhosis with infections.Evidence supports that timely and tailored interventions not only improve clinical outcomes but also reduce mortality.By raising awareness among clinicians about the complexity of these cases,we hope to contribute to optimizing the care of this high-risk population.

文摘This past year, 2024, is on track to be the warmest year, joining 2023 as the two hottest years on record. With the exceptional heat, weather and climate extremes were common across the world. In particular, 2024 has seen a remarkable run of extreme precipitation events and resulting impacts. Here, we provide an overview of the most notable extreme events of the year, including extreme precipitation and floods, tropical cyclones, and droughts. The characteristics and impacts of these extreme events are summarized, followed by discussion on the physical drivers and the role of global warming.Finally, we also discuss the future prospects in extreme event studies, including impact-based perspectives, challenges in attribution of precipitation extremes, and the existing gap to minimize impacts from climate extremes.

文摘This research paper introduces a soft VR glove that enhances how users interact with virtual objects. It seamlessly integrates discrete modules for sensing and providing haptic feedback, encompassing tactile and kinaesthetic aspects while prioritizing wearability and natural finger movements. The glove employs custom-designed flexible bend sensors with carbon-impregnated film for in-situ joint angle tracking, simplifying the sensing system and enhancing portability. A multi-modal haptic feedback approach includes an innovative pneumatically actuated tactile feedback technique and a motor-tendon-driven kinaesthetic feedback system, providing exceptional realism in virtual object manipulation. The glove’s kinaesthetic feedback lets users perceive virtual objects’ size, shape, and stiffness characteristics. Psychophysical investigations demonstrate how readily the users acclimate to this hardware and prove each module’s effectiveness and synergistic operation. This soft VR glove represents a minimalist, lightweight, and comprehensive solution for authentic haptic interaction in virtual environments, opening new possibilities for applications in various fields.
文摘Lynch syndrome(LS),also known as hereditary non-polyposis colorectal cancer(HNPCC),is an inherited condition associated with a higher risk of colorectal cancer(CRC)and other cancers.It is caused by germline mutations in DNA mismatch repair(MMR)genes,including MLH1,MSH2,MSH6 and PMS2.These mutations lead to microsatellite instability(MSI)and defective DNA repair mechanisms,resulting in increased cancer risk.Early detection of LS is crucial for effective management and cancer prevention.Endoscopic surveillance,particularly regular colonoscopy,is recommended for individuals with LS to detect CRC at early stages.Additionally,universal screening of CRC for MMR deficiency can help identify at-risk individuals.Genetic counseling plays a valuable role in LS by guiding patients and their families in understanding the genetic basis,making informed decisions regarding surveillance and prevention,and offering reproductive options to reduce the transmission of pathogenic variants of the offspring.The aim of this review is to outline current strategies for the diagnosis,surveillance,and management of LS,with a focus on the role of genetic counseling,endoscopic screening,and emerging therapeutic approaches to mitigate cancer risk in affected individuals.
文摘BACKGROUND Post-polypectomy syndrome(PPS)is a rare but relevant complication of endo-scopic colorectal polyp removal.Although its course is usually benign,it may conceal more severe conditions.This case report presents several complications resulting from a colorectal polypectomy.CASE SUMMARY We report the case of a 67-year-old man who presented with asthenia and abdo-minal discomfort after a scheduled polypectomy.The patient was initially diagnosed with PPS and managed conservatively.However,progressive clinical deterioration led to hospital admission.Imaging revealed a right-sided intra-abdominal abscess secondary to colonic perforation.The initial polyp was iden-tified as a subepithelial lipoma.The patient underwent successful percutaneous drainage,antibiotic therapy,and recovered without the need for surgical inter-vention.CONCLUSION This case highlights the importance of accurate endoscopic diagnosis and te-chnique selection.PPS should not preclude the consideration of concurrent comp-lications.Early identification and a tailored therapeutic approach can prevent Core Tip:Although therapeutic colonoscopy is generally safe,it may be associated with significant complications.This case illustrates how post-polypectomy syndrome can mask a colonic perforation with subsequent abscess formation.It em-phasizes the importance of an accurate initial endoscopic assessment,the use of advanced resection and closure techniques,and close clinical follow-up.Recognizing this spectrum of complications and intervening early can help prevent major surgical procedures and improve patient outcomes.
文摘The treatment of locally advanced rectal cancer(LARC)has evolved significantly over the past century,driven by a deeper understanding of tumor biology,technological advancements,and multidisciplinary approaches.This article reviews the historical progression of LARC management,emphasizing the latest breakthroughs that are reshaping treatment paradigms.Key developments include the watch and wait strategy for patients achieving a complete clinical response after neoadjuvant therapy,the emergence of total neoadjuvant therapy as a standard approach,and the adoption of minimally invasive surgical techniques,such as transanal endoscopic microsurgery.Watch and wait may reduce treatment-related morbidity and help preserve anorectal function,but it requires rigorous patient selection and close long-term surveillance to ensure oncologic safety.Additionally,the role of targeted therapies and immunotherapy is gaining prominence,offering new opportunities for personalized treatment.These innovations aim to improve oncological outcomes while minimizing morbidity and preserving organ function,ultimately enhancing patients’quality of life.Despite these advancements,challenges remain in optimizing patient selection,refining treatment strategies,and ensuring long-term safety and efficacy.A multidisciplinary approach involving surgeons,oncologists,and radiation specialists is crucial to tailoring therapies to individual patient profiles.As research continues,integrating novel therapeutic strategies will be key to further improving survival rates and reducing treatment-related morbidity in LARC patients.
文摘BACKGROUND Acute pancreatitis(AP)is commonly encountered in gastroenterology,with biliary and alcohol-related causes being predominant.Among less frequent etiologies,metabolic and drug-induced origin are the most prevalent.As an exception,non-pancreatobiliary malignancies may trigger AP,representing less than 1%of cases.We present a case of AP secondary to an uncommon oncologic etiology.CASE SUMMARY We describe the case of a 50-year-old male recently diagnosed with small-cell lung cancer(SCLC).The patient was admitted to the emergency department with acute abdominal pain and subsequently diagnosed with AP.He was hospitalized under the care of the gastroenterology service.During the etiological workup,metastatic pancreatic lesions were identified on imaging,which had not been observed on the initial cancer staging.Following resolution of the initial episode,oral intake was introduced,but the patient experienced recurrent abdominal pain and laboratory abnormalities consistent with new episodes of AP.This pattern repeated over the following days.After several episodes,and with suspicion of a metastatic origin,the case was discussed in multidisciplinary meeting.In agreement with the patient and given the need to initiate treatment for the primary tumor as well,chemotherapy was started.This treatment successfully alleviated symptoms,allowing dietary progression without complications and discharge from the hospital.CONCLUSION Oncologic treatment can be considered as part of the therapeutic approach in AP secondary to SCLC metastasis,especially chemotherapy.
文摘BACKGROUND Cytomegalovirus(CMV)prophylaxis with valganciclovir and ganciclovir is associated with elevated neutropenia and leukopenia risk in kidney transplant recipients,although the impact of these events on healthcare resource utilization(HCRU)and clinical outcomes is unclear.AIM To quantify clinical events and HCRU associated with neutropenia and leukope-nia among adults receiving valganciclovir and/or ganciclovir post-kidney trans-plantation.METHODS Adult kidney transplant recipients receiving valganciclovir and/or ganciclovir prophylaxis were identified in the TriNetX database from 2012 to 2021.Patient characteristics were evaluated in the 1-year period pre-first transplant.HCRU and adjusted event rates per person-year were evaluated in follow-up year 1 and years 2-5 after first kidney transplantation among cohorts with vs without neutropenia and/or leukopenia.RESULTS Of 15398 identified patients,the average age was 52.39 years and 58.70%were male.Patients with neutropenia and/or leukopenia had greater risk of clinical events for CMV-related events,opportunistic infections,use of granulocyte colony stimulating factor,and hospitalizations(relative risk>1 in year 1 and years 2-5).Patients with vs without neutropenia and/or leukopenia had higher HCRU in year 1 and years 2-5 post kidney transplantation,including the mean number of inpatient admissions(year 1:3.47 vs 2.76;years 2-5:2.70 vs 2.29)and outpatient visits(48.97 vs 34.42;31.73 vs 15.59,respectively),as well as the mean number of labs(1654.55 vs 1182.27;622.37 vs 327.89).CONCLUSION Adults receiving valganciclovir and/or ganciclovir prophylaxis post-kidney transplantation had greater risk of neutropenia and/or leukopenia,which were associated with higher clinical event rates and HCRU up to 5 years post-transplantation.These findings suggest the need for alternative prophylaxis options with lower myelosup-pressive effects to improve patient outcomes.

文摘Although several previous studies in Inner Mongolia examined the effects of ecological conservation on the delivery of ecosystem services, they were often limited in scope(few ecosystem services were assessed) and often suffered from confounding by spatial variation. In this study, we examined the impact of conservation measures(changes in grassland utilization patterns) on the provision of selected ecosystem services in three types of grasslands(meadow steppe in Hulun Buir, typical steppe in Xilin Gol, and semi-desert steppe in Ordos) in Inner Mongolia. We examined five utilization patterns: no use(natural grasslands), light use, moderate use, intensive use, and recovery sites(degraded sites protected from further use). Through household surveys and vegetation and soil surveys, we measured the differences in ecosystem services among the different grassland utilization patterns. We also identified spatial factors that confounded the quantification of ecosystem services in different types of grasslands. We found that light use generally provided high levels of ecosystem services in meadow steppe and typical steppe, with the main differences in the supporting ecosystem services. Surprisingly, we found no consistently positive impacts of strict conservation activities across the sites, since the results varied spatially and with respect to differences in the land-use patterns. Our study suggests that appropriate grassland utilization patterns can enhance the supply of ecosystem services and reduce negative effects on both household livelihoods and the environment.

文摘Green development emphasizes co-development between economic and environmental dimensions,and is a peoplecentered sustainable development approach.Western China demands green development,and international experience could provide necessary,unique and important help and support for Western China to achieve its green development goals.This paper has made a comprehensive overall review and analysis of international experience in green development policy and its implementation,in particular,OECD countries'(mostly Australia and Canada) experience have been analyzed following the major policy foci defined by the Task Force on Strategy and Policies on Environment and Development in Western China initiated by China Council for International Cooperation on Environment and Development(CCICED).Data and information were gathered from the field surveys and investigations,expert meetings,as well as literature review.The main sessions include policy framework and road map establishment,implementation and performance assessment,co-development between economic development and environmental protection,as well as green employment and poverty alleviation.The paper has addressed five policy considerations for the future promotion of green development in Western China.
文摘(Na0.5K0.5)NbO3 (NKN) lead free thin films were synthesized by means of an acetic acid based sol-gel process on Pt/Ti/SiO2/Si substrates. Na-acetate, K-acetate and Nb-pentaethoxide were used as metal precursors and acetic acid as the solvent. The effect of different calcination temperatures on the properties of the NKN films was investigated by X-ray diffraction, scanning electron microscopy, leakage current and hysteresis measurements. Low calcination temperatures led to low currents at high electric fields whereas high calcination temperatures led to low currents at low electric fields. Based on these findings calcination at low temperature was combined with a post annealing treatment. Low leakage currents of 4×10^-4 A/cm2 at 150 kV/cm and 2Pr and 2Ec values of 28 μC/cm2 and 150 kV/cm, respectively, could be obtained. All films were single phase NKN with random crystal orientations and no crack or pore formation was visible on the surface.

文摘OBJECTIVE: To investigate the anti-inflammatory effect of pretreatment with quercetin on macro- phages after Candida albicans infection. METHODS: RAW 264.7 macrophages were used as a target cell line. Cell viability after treatment with quercetin at different time points was detected by Carboxyfluorescein diacetate, succinimidyl ester. Phagocytic function of macrophages was deter- mined by a fluorometric assay. Cytokine tumor ne- crosis factor a (TNF-a) production was measured by enzyme-linked immunosorbent assay. F-actin cy- toskeleton of L929 cells was stained by Alexa Fluor 488-phalloidin. RESULTS: Pretreatment with quercetin decreasedcell viability only at the highest concentration of 37 μg/mL 2, 24, and 48 h after the treatment. The phagocytic efficiency of macrophages pretreated with quercetin was significantly decreased in a time- and dose-dependent manner. F-actin label- ing showed that the actin cytoskeleton of the cells started to break down 2 h after treatment. More- over, it notably inhibited cytokine TNF-a produc- tion after Candida albicans infection. CONCLUSION: Pretreatment with quercetin in- duced an anti-inflammatory effect against Candida albicans infection in macrophages.

文摘This paper deals with a general class of weighted multilinear Hardy-Cesaro op- erators that acts on the product of Lebesgue spaces and central Morrey spaces. Their sharp bounds are also obtained. In addition, we obtain sufficient and necessary conditions on weight functions so that the commutators of these weighted multilinear Hardy-Cesaro oper- ators (with symbols in central BMO spaces) are bounded on the product of central Morrey spaces. These results extends known results on multilinear Hardy operators.
