In traditional Chinese medicine(TCM),based on various pathogenic symptoms and the‘golden chamber’medical text,Huangdi Neijing,diabetes mellitus falls under the category‘collateral disease’.TCM,with its wealth of e...In traditional Chinese medicine(TCM),based on various pathogenic symptoms and the‘golden chamber’medical text,Huangdi Neijing,diabetes mellitus falls under the category‘collateral disease’.TCM,with its wealth of experience,has been treating diabetes for over two millennia.Different antidiabetic Chinese herbal medicines re-duce blood sugar,with their effective ingredients exerting unique advantages.As well as a glucose lowering effect,TCM also regulates bodily functions to prevent diabetes associated complications,with reduced side effects compared to western synthetic drugs.Chinese herbal medicine is usually composed of polysaccharides,saponins,al-kaloids,flavonoids,and terpenoids.These active ingredients reduce blood sugar via various mechanism of actions that include boosting endogenous insulin secretion,enhancing insulin sensitivity and adjusting key enzyme activity and scavenging free radicals.These actions regulate glycolipid metabolism in the body,eventually achiev-ing the goal of normalizing blood glucose.Using different animal models,a number of molecular markers are available for the detection of diabetes induction and the molecular pathology of the disease is becoming clearer.Nonetheless,there is a dearth of scientific data about the pharmacology,dose-effect relationship,and structure-activity relationship of TCM and its constituents.Further research into the efficacy,toxicity and mode of action of TCM,using different metabolic and molecular markers,is key to developing novel TCM antidiabetic formulations.展开更多
Background/Aims:Traditional Chinese medicine(TCM)combined with antiviral therapy has been proven to be effective for liver fibrosis due to chronic hepatitis B(CHB)in clinical practice in China.However,the robust evide...Background/Aims:Traditional Chinese medicine(TCM)combined with antiviral therapy has been proven to be effective for liver fibrosis due to chronic hepatitis B(CHB)in clinical practice in China.However,the robust evidence is limited,and the validity of results has been controversial in the past.The current study is to evaluate the efficacy and safety of the combination therapy of TCM plus entecavir(ETV)in the management of HBV-associated liver fibrosis or cirrhosis.Methods:Seven electronic databases were searched from inception to 10 August 2021.Primary outcome of this study was the regression of liver fibrosis;the secondary outcomes were a necro-inflammatory improvement,alanine aminotransferase(ALT),HBV DNA undetectable rate,HBeAg loss and HBeAg seroconversion.All the trials included were assessed by the Cochrane risk-of-bias tool.Results:It showed that TCM plus ETV attenuated liver fibrosis or cirrhosis in chronic hepatitis B patients as compared to ETV monotherapy(OR=1.65;95%CI:1.29~2.11;P<0.000,1).There is no statistical difference between TCM plus ETV and ETV in histological activity index,HBV DNA undetectable rate,HBeAg loss,HBeAg seroconversion and adverse events(P<0.05).Conclusion:The comprehensive evaluation showed that TCM combined with ETV treatment was safe for the patients with CHB,and better promoted the regression of liver fibrosis than ETV monotherapy.However,the standardized,rigorously designed,and large-scale randomized controlled trials(RCTs)were needed for further validation.展开更多
BACKGROUND Esophageal stricture ranks among the most significant complications following endoscopic submucosal dissection(ESD).Excessive fibrotic repair is a typical pathological feature leading to stenosis after ESD....BACKGROUND Esophageal stricture ranks among the most significant complications following endoscopic submucosal dissection(ESD).Excessive fibrotic repair is a typical pathological feature leading to stenosis after ESD.AIM To examine the effectiveness and underlying mechanism of Kangfuxin solution(KFX)in mitigating excessive fibrotic repair of the esophagus post-ESD.METHODS Pigs received KFX at 0.74 mL/kg/d for 21 days after esophageal full circumferential ESD.Endoscopic examinations occurred on days 7 and 21 post-ESD.In vitro,recombinant transforming growth factor(TGF)-β1(5 ng/mL)induced a fibrotic microenvironment in primary esophageal fibroblasts(pEsF).After 24 hours of KFX treatment(at 1.5%,1%,and 0.5%),expression ofα-smooth muscle actin-2(ACTA2),fibronectin(FN),and type collagen I was assessed.Profibrotic signaling was analyzed,including TGF-β1,Smad2/3,and phosphor-smad2/3(p-Smad2/3).RESULTS Compared to the Control group,the groups treated with KFX and prednisolone exhibited reduced esophageal stenosis,lower weight loss rates,and improved food tolerance 21 d after ESD.After treatment,Masson staining revealed thinner and less dense collagen fibers in the submucosal layer.Additionally,the expression of fibrotic effector molecules was notably inhibited.Mechanistically,KFX downregulated the transduction levels of fibrotic functional molecules such as TGF-β1,Smad2/3,and p-Smad2/3.In vitro,pEsF exposed to TGF-β1-induced fibrotic microenvironment displayed increased fibrotic activity,which was reversed by KFX treatment,leading to reduced activation of ACTA2,FN,and collagen I.The 1.5%KFX treatment group showed decreased expression of p-Smad 2/3 in TGF-β1-activated pEsF.CONCLUSION KFX showed promise as a therapeutic option for post-full circumferential esophageal ESD strictures,potentially by suppressing fibroblast fibrotic activity through modulation of the TGF-β1/Smads signaling pathway.展开更多

Intracerebral hemorrhage(ICH) is a significant and growing threat to human health,with increasing incidence.Promoting blood circulation and removing blood stasis therapy(PBCRBS),a Traditional Chinese Medicine therapy,...Intracerebral hemorrhage(ICH) is a significant and growing threat to human health,with increasing incidence.Promoting blood circulation and removing blood stasis therapy(PBCRBS),a Traditional Chinese Medicine therapy,can be an adjuvant therapy to benefit patients with ICH by improving clinical efficacy.However,in theory,using PBCRBS to treat ICH carries the risk of hematoma enlargement and rebleeding,which has led to controversy over its application in ICH treatment.To demonstrate the effectiveness and safety of PBCRBS in treating ICH,this review first analyzes the pathological and physiological basis of ICH and secondly,the cascade of response after ICH and the involvement of cytokines and signaling pathways in this process.Finally,experimental and clinical studies on the treatment of ICH with PBCRBS over the past decade were retrieved from the Pub Med and China National Knowledge Infrastructure databases,and the content of these studies was used to summarize commonly used herbs with PBCRBS effects and their mechanisms of action.Through analysis,hypertension has been identified as the most common cause of ICH.Heme,interleukin,reactive oxygen species,coagulation promoting particles and other induced mass effects,inflammation,oxidative stress,and coagulation cascade reactions lead to brain damage following ICH.This review includes 56 experimental studies and 83 clinical studies summarizing 28 commonly used herbs,demonstrating the positive impact of PBCRBS as an adjuvant therapy for ICH.In summary,PBCRBS appears effective and safe for treating ICH.展开更多
OBJECTIVE:To analyze the mechanism of Tongqiao Yizhi decoction(TQYZD,通窍益智颗粒)in treating vascular dementia(VaD)rats using brain tissue untargeted metabonomics and fecal 16S rRNA gene sequencing.METHODS:The chemic...OBJECTIVE:To analyze the mechanism of Tongqiao Yizhi decoction(TQYZD,通窍益智颗粒)in treating vascular dementia(VaD)rats using brain tissue untargeted metabonomics and fecal 16S rRNA gene sequencing.METHODS:The chemical composition of TQYZD was analyzed by ultra-high performance liquid chromatography-high resolution mass spectrometry.The effects of TQYZD on VaD rats were evaluated by water maze test,hematoxylin-eosin staining,enzyme-linked immunosorbent assay and Western blot.Untargeted metabolomics and 16S rRNA sequencing were utilized to explore the relationship between metabolic profiles and the structure and function of intestinal flora.Fecal microbiota transplantation(FMT)was used to validate the role of gut microbiota in VaD.Spearman correlation analysis of differential metabolites and gut microbiota was performed.RESULTS:The results showed that TQYZD improved cognitive function and neuronal damage in VaD rats,and reduced inflammatory response and repaired the intestinal barrier.In addition,TQYZD had the effect of modulating gut microbes in VaD rats,and FMT further confirmed that gut flora plays an important role in TQYZD treatment of VaD.Untargeted metabolomics revealed that VaD could lead to metabolic disorders in brain tissues,and TQYZD significantly altered the metabolites of brain tissues in Middle Cerebral Artery Occlusion rats.The results of spearman correlation analysis showed that there was a significant correlation between intestinal flora and the metabolites of brain tissues.CONCLUSION:In this study,we demonstrated that TQYZD can improve metabolic disorders in vascular dementia rats by acting on intestinal flora.展开更多

Objective:To investigate the clinical and molecular genetic characteristics of Chinese adolescents with maturity-onset diabetes of the young type 2(MODY 2)and the safety and efficacy of recombinant human growth hormon...Objective:To investigate the clinical and molecular genetic characteristics of Chinese adolescents with maturity-onset diabetes of the young type 2(MODY 2)and the safety and efficacy of recombinant human growth hormone(r-hGH).Methods:The clinical features and laboratory data of a family with MODY 2 combined with partial growth hormone deficiency(pGHD),diagnosed at the Fourth Clinical Medical College of Xinjiang Medical University,were analyzed.DNA was extracted from peripheral blood using the column method,and Sanger sequencing was conducted to analyze the glucokinase(GCK),hepatocyte nuclear factor 1α(HNF1α),and hepatocyte nuclear factor 4α(HNF4α)in the proband and relevant family members.Results:A heterozygous mutation in GCK(Reference sequence:NM_000162,location:Exon 10)c.1340G>A(p.R447Q)was detected in three family members(the proband,the proband’s younger brother,and their mother).The proband also had pGHD.Conclusion:GCK mutations causing MODY 2 exist in the Chinese population,and the combined treatment with r-hGH is safe and effective.展开更多
BACKGROUND The combination of sorafenib with transarterial chemoembolization(TACE)is being investigated for its potential to improve outcomes in advanced hepatocellular carcinoma(HCC).AIM To evaluate the efficacy of t...BACKGROUND The combination of sorafenib with transarterial chemoembolization(TACE)is being investigated for its potential to improve outcomes in advanced hepatocellular carcinoma(HCC).AIM To evaluate the efficacy of this combined treatment strategy in enhancing overall survival(OS)and progression-free survival(PFS)compared to monotherapies.METHODS A systematic review was conducted following the PRISMA guidelines.A comprehensive search was performed across PubMed,EMBASE,Web of Science,and the Cochrane Library up to May 8,2024.Studies were included if they compared sorafenib plus TACE to sorafenib alone or TACE alone in adults with advanced HCC.Primary outcomes were OS,PFS,response rates,and safety profiles.Data extraction and quality assessment were independently performed by two reviewers.Heterogeneity was assessed using the I^(2)statistic,and a random-effects model was applied for pooling data.Sensitivity analysis and publication bias assessment were also conducted.RESULTS A total of twelve studies involving 1174 patients met the inclusion criteria.Significant heterogeneity was observed for both OS(I^(2)=72.6%,P<0.001)and PFS(I^(2)=83.7%,P<0.001).The combined treatment of sorafenib with TACE significantly improved OS[hazard ratio(HR)=0.60,95%confidence interval(CI):0.44-0.76]and PFS(HR=0.54,95%CI:0.38-0.69).Sensitivity analysis confirmed the robustness of these findings.Funnel plots and Egger's test indicated no significant publication bias.CONCLUSION Sorafenib combined with TACE significantly enhances both OS and PFS in patients with advanced HCC compared to monotherapy.This combination therapy represents a promising approach to improving clinical outcomes in advanced liver cancer.展开更多

Background:Renshen Guben oral liquid(RGSB)has the ability to consolidate constitution,restore vitality,and delay the ageing process and the onset of geriatric diseases.RGSB also has potential therapeutic effects on in...Background:Renshen Guben oral liquid(RGSB)has the ability to consolidate constitution,restore vitality,and delay the ageing process and the onset of geriatric diseases.RGSB also has potential therapeutic effects on intervertebral disc degeneration(IVDD).This study investigated the therapeutic effects of RGSB on IVDD and explored the underlying mechanisms.Methods:Single-cell sequencing and network pharmacology analyses were conducted in combination to determine the mechanisms of IVDD.Thirty-two C57BL/6J mice were randomly assigned to four groups:the sham operation group,IVDD group,Ibuprofen(IB)group,and RGSB group.All groups except for the sham operation group underwent needle puncture of the caudal IVDs.Behavioral tests,ELISA,PCR,TUNEL staining,and immunohistochemical staining were performed to investigate the therapeutic mechanisms associated with IVDD.Results:Bioinformatics analysis revealed that apoptosis is involved in the development of IVDD and that RSGB might alleviate IVDD by inhibiting apoptosis.After needle puncture,the mice exhibited from thermal allodynia,a decrease in disc height,an increase in the Pfirrmann grade,and destruction of IVDs.Surgery to induce IVDD increased the levels of Matrix Metalloproteinase-3(MMP3),Caspase-6,and Cyt-c in nucleus pulposus cells and the periphery;serum levels of interleukin-1β(IL‐1β)and TNF‐α;and the mRNA levels of IL-6,TNF-α,and IL-1β.Moreover,IVDD decreased the IL-10 mRNA level and the expression of Col-I and BCL2.RSGB reversed these changes.Conclusion:RSGB alleviated IVDD by relieving inflammation and apoptosis and alleviating neuroinflammation.展开更多

However, the changes in BDNF expression and associated factors following hypoxia-ischemia induced neonatal brain damage, and the significance of these changes are not fully understood. In the present study, a rat model of hypoxic-ischemic brain damage was established through the occlusion of the right common carotid artery, followed by 2 hours in a hypoxic-ischemic environment. Rats with hypoxic-ischemic brain damage presented deficits in both sensory and motor functions, and obvious pathological changes could be detected in brain tissues. The m RNA expression levels of BDNF and its processing enzymes and receptors(Furin, matrix metallopeptidase 9, tissuetype plasminogen activator, tyrosine Kinase receptor B, plasminogen activator inhibitor-1, and Sortilin) were upregulated in the ipsilateral hippocampus and cerebral cortex 6 hours after injury;however, the expression levels of these m RNAs were found to be downregulated in the contralateral hippocampus and cerebral cortex. These findings suggest that BDNF and its processing enzymes and receptors may play important roles in the pathogenesis and recovery from neonatal hypoxic-ischemic brain damage. OBJECTIVE:To investigate the efficacy of Chaihu Ji Sang Decoction(CJSD),(a modification of Xiao Chaihu Tang(Minor Decoction of Bupleurum) was evaluated in patients with post-infection cough(PIC).Its effect on cough re...OBJECTIVE:To investigate the efficacy of Chaihu Ji Sang Decoction(CJSD),(a modification of Xiao Chaihu Tang(Minor Decoction of Bupleurum) was evaluated in patients with post-infection cough(PIC).Its effect on cough reflex sensitivity(CRS) was also described.METHODS:One hundred and eighteen patients with PIC were enrolled and randomly divided into an experimental group and control group(54 patients in each group).The experimental group was given CJSD orally,and the control group was given cetirizine hydrochloride for 7 d.Cough symptom scores and TCM symptom scores were compared before and after treatment.Efficacy,cough onset time,recurrence,and adverse drug reactions were additionally evaluated.Finally,CRS was evaluated before and after treatment of PIC patients in the experimental group.RESULTS:Fifty-two patients in the experimental group and fifty control patients completed the study.Cough symptom scores and the TCM symptom scores of the two groups were significantly improved after treatment,however cough symptoms in the experimental group 1(0,2) were significantly lower than the control group 2(1,2) after treatment(P < 0.05).Similarly,TCM symptom scores 4(0,7) in the experimental group were significantly lower than the control group 6(3,12)(P < 0.01).Overall,the effective rate of the experimental treatment(84.60%) was significantly higher than that of the control treatment(68.00%)(P < 0.01).The onset time of efficacy of the experimental treatment was one day faster than the control treatment [2(2,3) d vs 3(2,3) d,P < 0.01].The relapse rate in the experimental group(9.09%) was lower than that of the control group(29.41%)(P < 0.05).39 PIC patients in the experimental group completed a capsaicin cough challenge test before treatment.The lgC2 and lg C5 of the experimental group prior to treatment was 1.19(0.59,1.49) and 1.80(1.49,2.10),respectively.The lgC2,lg C5 of the healthy control group was 1.49(0.89,2.40) and 2.70(2.40,3.00),respectively.The differences in lgC2 and lg C5 between experimental group and healthy groups were significant at this time point(both P < 0.01).After treatment,34 patients in the experimental group received re-examination.lg C5 increased from 1.80(1.49,2.10) to 2.10(1.49,3.00).CRS after treatment/re-examination was significantly lower than those of baseline in the experimental group(P < 0.05).CONCLUSION:CJSD could effectively improve the clinical symptoms of PIC patients,with demonstrable low recurrence and safety.CJSD also significantly decreased the CRS in patients with PIC.展开更多
Background:Diabetes mellitus (DM) remains a major health problem worldwide.Several clinical trials have shown the superiority of the Traditional Chinese Medicine in delaying or reversing the development and progres...Background:Diabetes mellitus (DM) remains a major health problem worldwide.Several clinical trials have shown the superiority of the Traditional Chinese Medicine in delaying or reversing the development and progression of DM.This study aimed to evaluate the efficacy of Jinlida (JLD) granule,a Chinese herbal recipe,in the treatment of impaired glucose tolerance (IGT) and its effect on the prevention of DM.Methods:Sixty-five IGT patients were randomized to receive one bag of JLD granules three times daily (JLD group,n =34) or no drug intervention (control group,n =31) for 12 weeks.Oral glucose tolerance test,glycated hemoglobin A1c (HbA1c),body mass index,blood lipids levels,fasting insulin,and insulin resistance calculated using homeostatic model assessment (HOMA-IR) of all the patients were observed and compared before and after the treatment.Results:Sixty-one participants completed the trial (32 in JLD group and 29 in the control group).There were statistically significant decreases in HbA1c (P 〈 0.001),2-h plasma glucose (P 〈 0.001),and HOMA-IR (P =0.029) in JLD group compared with the control group after 12 weeks of treatment.After 12 weeks of treatment,two (6.9%) patients returned to normal blood glucose,and five (17.2%) patients turned into DM in control group,while in the JLD group,14 (43.8%) returned to normal blood glucose and 2 (6.2%) turned into DM.There was a significant difference in the number of subjects who had normal glucose at the end of the study between two groups (P =0.001).Conclusions:JLD granule effectively improved glucose control,increased the conversion of IGT to normal glucose,and improved the insulin resistance in patients with IGT.This Chinese herbal medicine may have a clinical value for IGT.展开更多

OBJECTIVE: To evaluate the clinical effects of Xiyan-ping injection intervention in pediatric patients with mild hand foot and mouth disease(HFMD).METHODS: A total of 329 patients were stratified and block-randomized ...OBJECTIVE: To evaluate the clinical effects of Xiyan-ping injection intervention in pediatric patients with mild hand foot and mouth disease(HFMD).METHODS: A total of 329 patients were stratified and block-randomized for symptomatic treatment of HFMD and assigned to one of the following groups: Western Medicine(group A, n = 103), Xiyanping injection(group B, n = 109), or Xiyanping injection and symptomatic treatment using Western Medicine(group C, n = 117). During the trial, fever,rash, ulcers of the mouth were observed among participants in each group before and after treatment, and conversion rates from mild to severe HFMD were measured.RESULTS: After 3-7 days' treatment, no significant differences in the conversion rates from mild to severe HFMD were observed among the three groups(P > 0.05). There was a significantly low number of patients with the onset time of antifebrile effect,vanished time of hand and foot rashes and cumulative time for the ulcers in the mouth vanished,among the three groups(P < 0.05, P < 0.01). The onset time of antifebrile effect of patients in groups B and C were markedly shorter compared with those in group A(P < 0.05, P < 0.01); there was no statically significant difference observed between groups B and C(P > 0.05). No significant differences were observed in the time for body temperature recovery among the three groups(P > 0.05). There were no significant differences in adverse effects among the three groups(P > 0.05).CONCLUSION: Xiyanping injection together with use of Western Medicine is most effective for symptomatic treatment of mild HFMD. 文摘This editorial provides a commentary on the recent article.The paper reviews current literature and explores innovative treatment strategies for lower extremity arteriosclerosis obliterans(LEASO)through an integrative approach.It highlights the effectiveness of combination therapies that merge traditional Chinese medicine(TCM)with Western medical practices,suggesting that such integrative methods may improve patient compliance and outcomes through personalized care.This paper stresses the importance of rigorous clinical trials to evaluate the efficacy and safety of TCM interventions within LEASO treatment protocols,advocating for evidence-based validation of these combined therapies.Our recommendations emphasize accurate diagnosis,appropriate pharmacological in-terventions,the use of advanced surgical and endovascular techniques,and the inclusion of TCM to address underlying dysfunctions.Additionally,continuous monitoring,patient education,and lifestyle modifications are essential to slow disease progression and achieve optimal patient outcomes.The paper concludes by calling for further research to develop standardized treatment protocols that effectively integrate both Western and Chinese medical approaches in managing LEASO.

文摘In traditional Chinese medicine(TCM),based on various pathogenic symptoms and the‘golden chamber’medical text,Huangdi Neijing,diabetes mellitus falls under the category‘collateral disease’.TCM,with its wealth of experience,has been treating diabetes for over two millennia.Different antidiabetic Chinese herbal medicines re-duce blood sugar,with their effective ingredients exerting unique advantages.As well as a glucose lowering effect,TCM also regulates bodily functions to prevent diabetes associated complications,with reduced side effects compared to western synthetic drugs.Chinese herbal medicine is usually composed of polysaccharides,saponins,al-kaloids,flavonoids,and terpenoids.These active ingredients reduce blood sugar via various mechanism of actions that include boosting endogenous insulin secretion,enhancing insulin sensitivity and adjusting key enzyme activity and scavenging free radicals.These actions regulate glycolipid metabolism in the body,eventually achiev-ing the goal of normalizing blood glucose.Using different animal models,a number of molecular markers are available for the detection of diabetes induction and the molecular pathology of the disease is becoming clearer.Nonetheless,there is a dearth of scientific data about the pharmacology,dose-effect relationship,and structure-activity relationship of TCM and its constituents.Further research into the efficacy,toxicity and mode of action of TCM,using different metabolic and molecular markers,is key to developing novel TCM antidiabetic formulations.

文摘Background/Aims:Traditional Chinese medicine(TCM)combined with antiviral therapy has been proven to be effective for liver fibrosis due to chronic hepatitis B(CHB)in clinical practice in China.However,the robust evidence is limited,and the validity of results has been controversial in the past.The current study is to evaluate the efficacy and safety of the combination therapy of TCM plus entecavir(ETV)in the management of HBV-associated liver fibrosis or cirrhosis.Methods:Seven electronic databases were searched from inception to 10 August 2021.Primary outcome of this study was the regression of liver fibrosis;the secondary outcomes were a necro-inflammatory improvement,alanine aminotransferase(ALT),HBV DNA undetectable rate,HBeAg loss and HBeAg seroconversion.All the trials included were assessed by the Cochrane risk-of-bias tool.Results:It showed that TCM plus ETV attenuated liver fibrosis or cirrhosis in chronic hepatitis B patients as compared to ETV monotherapy(OR=1.65;95%CI:1.29~2.11;P<0.000,1).There is no statistical difference between TCM plus ETV and ETV in histological activity index,HBV DNA undetectable rate,HBeAg loss,HBeAg seroconversion and adverse events(P<0.05).Conclusion:The comprehensive evaluation showed that TCM combined with ETV treatment was safe for the patients with CHB,and better promoted the regression of liver fibrosis than ETV monotherapy.However,the standardized,rigorously designed,and large-scale randomized controlled trials(RCTs)were needed for further validation.

文摘BACKGROUND Esophageal stricture ranks among the most significant complications following endoscopic submucosal dissection(ESD).Excessive fibrotic repair is a typical pathological feature leading to stenosis after ESD.AIM To examine the effectiveness and underlying mechanism of Kangfuxin solution(KFX)in mitigating excessive fibrotic repair of the esophagus post-ESD.METHODS Pigs received KFX at 0.74 mL/kg/d for 21 days after esophageal full circumferential ESD.Endoscopic examinations occurred on days 7 and 21 post-ESD.In vitro,recombinant transforming growth factor(TGF)-β1(5 ng/mL)induced a fibrotic microenvironment in primary esophageal fibroblasts(pEsF).After 24 hours of KFX treatment(at 1.5%,1%,and 0.5%),expression ofα-smooth muscle actin-2(ACTA2),fibronectin(FN),and type collagen I was assessed.Profibrotic signaling was analyzed,including TGF-β1,Smad2/3,and phosphor-smad2/3(p-Smad2/3).RESULTS Compared to the Control group,the groups treated with KFX and prednisolone exhibited reduced esophageal stenosis,lower weight loss rates,and improved food tolerance 21 d after ESD.After treatment,Masson staining revealed thinner and less dense collagen fibers in the submucosal layer.Additionally,the expression of fibrotic effector molecules was notably inhibited.Mechanistically,KFX downregulated the transduction levels of fibrotic functional molecules such as TGF-β1,Smad2/3,and p-Smad2/3.In vitro,pEsF exposed to TGF-β1-induced fibrotic microenvironment displayed increased fibrotic activity,which was reversed by KFX treatment,leading to reduced activation of ACTA2,FN,and collagen I.The 1.5%KFX treatment group showed decreased expression of p-Smad 2/3 in TGF-β1-activated pEsF.CONCLUSION KFX showed promise as a therapeutic option for post-full circumferential esophageal ESD strictures,potentially by suppressing fibroblast fibrotic activity through modulation of the TGF-β1/Smads signaling pathway.

文摘Intracerebral hemorrhage(ICH) is a significant and growing threat to human health,with increasing incidence.Promoting blood circulation and removing blood stasis therapy(PBCRBS),a Traditional Chinese Medicine therapy,can be an adjuvant therapy to benefit patients with ICH by improving clinical efficacy.However,in theory,using PBCRBS to treat ICH carries the risk of hematoma enlargement and rebleeding,which has led to controversy over its application in ICH treatment.To demonstrate the effectiveness and safety of PBCRBS in treating ICH,this review first analyzes the pathological and physiological basis of ICH and secondly,the cascade of response after ICH and the involvement of cytokines and signaling pathways in this process.Finally,experimental and clinical studies on the treatment of ICH with PBCRBS over the past decade were retrieved from the Pub Med and China National Knowledge Infrastructure databases,and the content of these studies was used to summarize commonly used herbs with PBCRBS effects and their mechanisms of action.Through analysis,hypertension has been identified as the most common cause of ICH.Heme,interleukin,reactive oxygen species,coagulation promoting particles and other induced mass effects,inflammation,oxidative stress,and coagulation cascade reactions lead to brain damage following ICH.This review includes 56 experimental studies and 83 clinical studies summarizing 28 commonly used herbs,demonstrating the positive impact of PBCRBS as an adjuvant therapy for ICH.In summary,PBCRBS appears effective and safe for treating ICH.

文摘OBJECTIVE:To analyze the mechanism of Tongqiao Yizhi decoction(TQYZD,通窍益智颗粒)in treating vascular dementia(VaD)rats using brain tissue untargeted metabonomics and fecal 16S rRNA gene sequencing.METHODS:The chemical composition of TQYZD was analyzed by ultra-high performance liquid chromatography-high resolution mass spectrometry.The effects of TQYZD on VaD rats were evaluated by water maze test,hematoxylin-eosin staining,enzyme-linked immunosorbent assay and Western blot.Untargeted metabolomics and 16S rRNA sequencing were utilized to explore the relationship between metabolic profiles and the structure and function of intestinal flora.Fecal microbiota transplantation(FMT)was used to validate the role of gut microbiota in VaD.Spearman correlation analysis of differential metabolites and gut microbiota was performed.RESULTS:The results showed that TQYZD improved cognitive function and neuronal damage in VaD rats,and reduced inflammatory response and repaired the intestinal barrier.In addition,TQYZD had the effect of modulating gut microbes in VaD rats,and FMT further confirmed that gut flora plays an important role in TQYZD treatment of VaD.Untargeted metabolomics revealed that VaD could lead to metabolic disorders in brain tissues,and TQYZD significantly altered the metabolites of brain tissues in Middle Cerebral Artery Occlusion rats.The results of spearman correlation analysis showed that there was a significant correlation between intestinal flora and the metabolites of brain tissues.CONCLUSION:In this study,we demonstrated that TQYZD can improve metabolic disorders in vascular dementia rats by acting on intestinal flora.

文摘Eight new diterpenoids,Isodons A-H(1-8),comprising seco-abietane and abietane-type structures,together with 13 known analogues(9-21),were isolated from Isodon lophanthoides(Buch.-Ham.ex D.Don)Hara.The compounds(+)-3/(-)-3,(+)-4/(-)-4,and(+)-5/(-)-5 were identified as three enantiomeric pairs.The planar structures and absolute configurations of 1-8 were determined through high-resolution electrospray ionization mass spectrometry(HR-ESI-MS),1D&2D nuclear magnetic resonance(NMR)spectroscopy,electronic circular dichroism(ECD)calculations,and X-ray diffraction crystallography.A cholesterol 7α-hydroxylase(Cyp7a1)luciferase reporter assay revealed significant anti-cholestatic activities for compounds 1,(+)-4,6,7,12-14,and 16.Additionally,compound 6 demonstrated anti-cholestatic effects through the farnesoid X receptor(FXR)-associated signaling pathways in vitro and in vivo.These findings suggest potential applications for I.Lophanthoides in pharmaceutical development.
文摘Objective:To investigate the clinical and molecular genetic characteristics of Chinese adolescents with maturity-onset diabetes of the young type 2(MODY 2)and the safety and efficacy of recombinant human growth hormone(r-hGH).Methods:The clinical features and laboratory data of a family with MODY 2 combined with partial growth hormone deficiency(pGHD),diagnosed at the Fourth Clinical Medical College of Xinjiang Medical University,were analyzed.DNA was extracted from peripheral blood using the column method,and Sanger sequencing was conducted to analyze the glucokinase(GCK),hepatocyte nuclear factor 1α(HNF1α),and hepatocyte nuclear factor 4α(HNF4α)in the proband and relevant family members.Results:A heterozygous mutation in GCK(Reference sequence:NM_000162,location:Exon 10)c.1340G>A(p.R447Q)was detected in three family members(the proband,the proband’s younger brother,and their mother).The proband also had pGHD.Conclusion:GCK mutations causing MODY 2 exist in the Chinese population,and the combined treatment with r-hGH is safe and effective.

文摘BACKGROUND The combination of sorafenib with transarterial chemoembolization(TACE)is being investigated for its potential to improve outcomes in advanced hepatocellular carcinoma(HCC).AIM To evaluate the efficacy of this combined treatment strategy in enhancing overall survival(OS)and progression-free survival(PFS)compared to monotherapies.METHODS A systematic review was conducted following the PRISMA guidelines.A comprehensive search was performed across PubMed,EMBASE,Web of Science,and the Cochrane Library up to May 8,2024.Studies were included if they compared sorafenib plus TACE to sorafenib alone or TACE alone in adults with advanced HCC.Primary outcomes were OS,PFS,response rates,and safety profiles.Data extraction and quality assessment were independently performed by two reviewers.Heterogeneity was assessed using the I^(2)statistic,and a random-effects model was applied for pooling data.Sensitivity analysis and publication bias assessment were also conducted.RESULTS A total of twelve studies involving 1174 patients met the inclusion criteria.Significant heterogeneity was observed for both OS(I^(2)=72.6%,P<0.001)and PFS(I^(2)=83.7%,P<0.001).The combined treatment of sorafenib with TACE significantly improved OS[hazard ratio(HR)=0.60,95%confidence interval(CI):0.44-0.76]and PFS(HR=0.54,95%CI:0.38-0.69).Sensitivity analysis confirmed the robustness of these findings.Funnel plots and Egger's test indicated no significant publication bias.CONCLUSION Sorafenib combined with TACE significantly enhances both OS and PFS in patients with advanced HCC compared to monotherapy.This combination therapy represents a promising approach to improving clinical outcomes in advanced liver cancer.

These oligonucleotide fragments, selected through the Systematic Evolution ofLigands by Exponential Enrichment process, exhibit high affinity and specificity toward their targets, such as DNA, RNA,proteins, and other biomolecules. Nucleic acid aptamers offer significant advantages over traditional therapeutic agents,including superior biological stability, minimal immunogenicity, and the capacity for universal chemical modifications thatenhance their in vivo performance and targeting precision. In the realm of osseous tissue repair and regeneration, a complexphysiological process essential for maintaining skeletal integrity, aptamers have shown remarkable potential in influencingmolecular pathways crucial for bone regeneration, promoting osteogenic differentiation and supporting osteoblast survival. Byengineering aptamers to regulate inflammatory responses and facilitate the proliferation and differentiation of fibroblasts,these oligonucleotides can be integrated into advanced drug delivery systems, significantly improving bone repair efficacywhile minimizing adverse effects. Aptamer-mediated strategies, including the use of siRNA and miRNA mimics or inhibitors,have shown efficacy in enhancing bone mass and microstructure. These approaches hold transformative potential for treatinga range of orthopedic conditions like osteoporosis, osteosarcoma, and osteoarthritis. This review synthesizes the molecularmechanisms and biological roles of aptamers in orthopedic diseases, emphasizing their potential to drive innovative andeffective therapeutic interventions.

文摘Background:Renshen Guben oral liquid(RGSB)has the ability to consolidate constitution,restore vitality,and delay the ageing process and the onset of geriatric diseases.RGSB also has potential therapeutic effects on intervertebral disc degeneration(IVDD).This study investigated the therapeutic effects of RGSB on IVDD and explored the underlying mechanisms.Methods:Single-cell sequencing and network pharmacology analyses were conducted in combination to determine the mechanisms of IVDD.Thirty-two C57BL/6J mice were randomly assigned to four groups:the sham operation group,IVDD group,Ibuprofen(IB)group,and RGSB group.All groups except for the sham operation group underwent needle puncture of the caudal IVDs.Behavioral tests,ELISA,PCR,TUNEL staining,and immunohistochemical staining were performed to investigate the therapeutic mechanisms associated with IVDD.Results:Bioinformatics analysis revealed that apoptosis is involved in the development of IVDD and that RSGB might alleviate IVDD by inhibiting apoptosis.After needle puncture,the mice exhibited from thermal allodynia,a decrease in disc height,an increase in the Pfirrmann grade,and destruction of IVDs.Surgery to induce IVDD increased the levels of Matrix Metalloproteinase-3(MMP3),Caspase-6,and Cyt-c in nucleus pulposus cells and the periphery;serum levels of interleukin-1β(IL‐1β)and TNF‐α;and the mRNA levels of IL-6,TNF-α,and IL-1β.Moreover,IVDD decreased the IL-10 mRNA level and the expression of Col-I and BCL2.RSGB reversed these changes.Conclusion:RSGB alleviated IVDD by relieving inflammation and apoptosis and alleviating neuroinflammation.

文摘Brain-derived neurotrophic factor(BDNF) regulates many neurological functions and plays a vital role during the recovery from central nervous system injuries. However, the changes in BDNF expression and associated factors following hypoxia-ischemia induced neonatal brain damage, and the significance of these changes are not fully understood. In the present study, a rat model of hypoxic-ischemic brain damage was established through the occlusion of the right common carotid artery, followed by 2 hours in a hypoxic-ischemic environment. Rats with hypoxic-ischemic brain damage presented deficits in both sensory and motor functions, and obvious pathological changes could be detected in brain tissues. The m RNA expression levels of BDNF and its processing enzymes and receptors(Furin, matrix metallopeptidase 9, tissuetype plasminogen activator, tyrosine Kinase receptor B, plasminogen activator inhibitor-1, and Sortilin) were upregulated in the ipsilateral hippocampus and cerebral cortex 6 hours after injury;however, the expression levels of these m RNAs were found to be downregulated in the contralateral hippocampus and cerebral cortex. These findings suggest that BDNF and its processing enzymes and receptors may play important roles in the pathogenesis and recovery from neonatal hypoxic-ischemic brain damage. 文摘Ulcerative colitis(UC)is one of types of inflammatory bowel disease with high recurrence.Recent studies have highlighted that microbial dysbiosis as well as abnormal gut immunity are crucial factors that initiate a series of inflammatory responses in the UC.Modulating the gut microbiota-intestinal immunity loop has been suggested as one of key strategies for relieving UC.Many Chinese herbal medicines including some of single herb,herbal formulas and the derived constituents have been reported with protective effect against UC through modulating gut microbiome and intestinal immunity.Some clinical trials have shown promising results.This review thus focused on the current knowledge on using Chinese herbal medicines for treating UC from the mechanism aspects of regulating intestinal homeostasis involving microbiota and gut immunity.The existing clinical trials are also summarized.

文摘OBJECTIVE:To investigate the efficacy of Chaihu Ji Sang Decoction(CJSD),(a modification of Xiao Chaihu Tang(Minor Decoction of Bupleurum) was evaluated in patients with post-infection cough(PIC).Its effect on cough reflex sensitivity(CRS) was also described.METHODS:One hundred and eighteen patients with PIC were enrolled and randomly divided into an experimental group and control group(54 patients in each group).The experimental group was given CJSD orally,and the control group was given cetirizine hydrochloride for 7 d.Cough symptom scores and TCM symptom scores were compared before and after treatment.Efficacy,cough onset time,recurrence,and adverse drug reactions were additionally evaluated.Finally,CRS was evaluated before and after treatment of PIC patients in the experimental group.RESULTS:Fifty-two patients in the experimental group and fifty control patients completed the study.Cough symptom scores and the TCM symptom scores of the two groups were significantly improved after treatment,however cough symptoms in the experimental group 1(0,2) were significantly lower than the control group 2(1,2) after treatment(P < 0.05).Similarly,TCM symptom scores 4(0,7) in the experimental group were significantly lower than the control group 6(3,12)(P < 0.01).Overall,the effective rate of the experimental treatment(84.60%) was significantly higher than that of the control treatment(68.00%)(P < 0.01).The onset time of efficacy of the experimental treatment was one day faster than the control treatment [2(2,3) d vs 3(2,3) d,P < 0.01].The relapse rate in the experimental group(9.09%) was lower than that of the control group(29.41%)(P < 0.05).39 PIC patients in the experimental group completed a capsaicin cough challenge test before treatment.The lgC2 and lg C5 of the experimental group prior to treatment was 1.19(0.59,1.49) and 1.80(1.49,2.10),respectively.The lgC2,lg C5 of the healthy control group was 1.49(0.89,2.40) and 2.70(2.40,3.00),respectively.The differences in lgC2 and lg C5 between experimental group and healthy groups were significant at this time point(both P < 0.01).After treatment,34 patients in the experimental group received re-examination.lg C5 increased from 1.80(1.49,2.10) to 2.10(1.49,3.00).CRS after treatment/re-examination was significantly lower than those of baseline in the experimental group(P < 0.05).CONCLUSION:CJSD could effectively improve the clinical symptoms of PIC patients,with demonstrable low recurrence and safety.CJSD also significantly decreased the CRS in patients with PIC.
文摘Background:Diabetes mellitus (DM) remains a major health problem worldwide.Several clinical trials have shown the superiority of the Traditional Chinese Medicine in delaying or reversing the development and progression of DM.This study aimed to evaluate the efficacy of Jinlida (JLD) granule,a Chinese herbal recipe,in the treatment of impaired glucose tolerance (IGT) and its effect on the prevention of DM.Methods:Sixty-five IGT patients were randomized to receive one bag of JLD granules three times daily (JLD group,n =34) or no drug intervention (control group,n =31) for 12 weeks.Oral glucose tolerance test,glycated hemoglobin A1c (HbA1c),body mass index,blood lipids levels,fasting insulin,and insulin resistance calculated using homeostatic model assessment (HOMA-IR) of all the patients were observed and compared before and after the treatment.Results:Sixty-one participants completed the trial (32 in JLD group and 29 in the control group).There were statistically significant decreases in HbA1c (P 〈 0.001),2-h plasma glucose (P 〈 0.001),and HOMA-IR (P =0.029) in JLD group compared with the control group after 12 weeks of treatment.After 12 weeks of treatment,two (6.9%) patients returned to normal blood glucose,and five (17.2%) patients turned into DM in control group,while in the JLD group,14 (43.8%) returned to normal blood glucose and 2 (6.2%) turned into DM.There was a significant difference in the number of subjects who had normal glucose at the end of the study between two groups (P =0.001).Conclusions:JLD granule effectively improved glucose control,increased the conversion of IGT to normal glucose,and improved the insulin resistance in patients with IGT.This Chinese herbal medicine may have a clinical value for IGT.

文摘With the proliferation of synthetic drugs,research on the mechanism of action of addictive drugs and treatment methods is of great significance.Among them,methamphetamine(METH)is the most representative amphetamine synthetic drug,and the treatment of METH addiction has become an urgent medical and social problem.In recent years,the therapeutic effects of Chinese herbal medicines on METH addiction have gained widespread attention because of their non-addictiveness,multiple targets,low side effects,low cost,and other characteristics.Previous studies have identified a variety of Chinese herbal medicines with effects on METH addiction.Based on the research on METH in recent years,this article summarizes the mechanism of action of METH as the starting point and briefly reviews the Chinese herbal medicine-based treatment of METH.

文摘OBJECTIVE: To evaluate the clinical effects of Xiyan-ping injection intervention in pediatric patients with mild hand foot and mouth disease(HFMD).METHODS: A total of 329 patients were stratified and block-randomized for symptomatic treatment of HFMD and assigned to one of the following groups: Western Medicine(group A, n = 103), Xiyanping injection(group B, n = 109), or Xiyanping injection and symptomatic treatment using Western Medicine(group C, n = 117). During the trial, fever,rash, ulcers of the mouth were observed among participants in each group before and after treatment, and conversion rates from mild to severe HFMD were measured.RESULTS: After 3-7 days' treatment, no significant differences in the conversion rates from mild to severe HFMD were observed among the three groups(P > 0.05). There was a significantly low number of patients with the onset time of antifebrile effect,vanished time of hand and foot rashes and cumulative time for the ulcers in the mouth vanished,among the three groups(P < 0.05, P < 0.01). The onset time of antifebrile effect of patients in groups B and C were markedly shorter compared with those in group A(P < 0.05, P < 0.01); there was no statically significant difference observed between groups B and C(P > 0.05). No significant differences were observed in the time for body temperature recovery among the three groups(P > 0.05). There were no significant differences in adverse effects among the three groups(P > 0.05).CONCLUSION: Xiyanping injection together with use of Western Medicine is most effective for symptomatic treatment of mild HFMD. No severe adverse reactions were observed.

Herein, a clickable rhodamine spirolactam probe, Atto565-Tet, is rationally constructed for spontaneously blinking after biorthogonal labelling and successfully applied to super resolution imaging of mitochondria and lysosomes.
