Methods Between 1975 to 1997,285 patients with thymoma were surgically treated. 168 cases of thymus tumor (TT,Group Ⅰ ) were compared to 90 cases of thymus tumor with myasthenia gravis (TTMG, Group Ⅱ ). Results In group Ⅰ , the average age was 38. 4 years, the youngest was 4 years old. In 93. 4%, the diameter of tumors was 5 cm. 61. 2% were in stages Ⅲ and Ⅳ of clinical pathology. In group Ⅱ ,the average age was 46.2 with the youngest of 20 years old,the diameter of tumor under 3 cm and 5 cm was 34.6 % and 65.6 % . 55.2 % was in stage Ⅰ of clinical pathology. Conclusion Many cases of thymoma in early stage received early treatment. The diagnostic standard of thymoma in early stage is: (1) the diameter of tumor is below 3 cm. Methods One hundred and twenty-three recipients who had received liver transplantation between April 199...Objective To study the prevention, diagnosis and treatment of biliary complications after liver transplantation. Methods One hundred and twenty-three recipients who had received liver transplantation between April 1993 and November 2001 were retrospectively reviewed. Results Biliary complication was diagnosed cholangiographyically in 11 patients. Nine patients were cured, 1 patient was unproved and 1 patient died. The incidence for biliary complications was 8.9% (11/123),and the mortality was 0.8% (1/123). The incidence of biliary complicatins due to T tube was 4.2% (5/ 119). The incidence of biliary complications due to hepatic artery was 1. 6% (2/123). In recipients with WIT】3 min and CIT】8 h, the incidence of biliary complications elevated significantly. Conclusion The most important reason for biliary complications was preservative and ischemic injury. While repairing donor liver, damage to the blood supply system of donor liver bile ducts should be avoided. Meticulous T tube placement may reduce展开更多

The incerased muscular tension of the former constitutes the rationale of neurotomy. Methods Highly selective peripheral neurotomy in 38 children who suffered from lower limb deformity secondary to unequal nuscular tension. Twenty-one of the children underwent obturator neurotomy, 27 sciatic neurotomy, and 26 tibial neurotomy. Using electric stimulation, the fasciculi involved were decided and severed. The proportion of fasciculi severed depended on the degree of the muscular tension. Results All patients function improved postoperatiely. 80 % of the patients experienced significant improvement. Conclusion Selective peripheral neurotomy is not only more effective and it also has less complication than selective posterior rhizotomy (SPR) in treating cerebral palsy spasm.展开更多

Methods 18 patients (11 with tuberculum sellae and ...Objective To make an investigation of the effect of microsurgical operation by unifrontal interhemispheric approach for tuberculum and diaphragma sellae meningiomas. Methods 18 patients (11 with tuberculum sellae and 7 with diaphragma sellae meningiomas) have been treated microsurgically via unifrontal interhemispheric approach Among these cases 4 patients were with tumor maximum diameter less than 3 cm, 13 cases 3 - 6 cm, and only one was more than 6 cm. Results Total resection (Simpson Ⅰ or Ⅱ ) has been performed in 16 patients, the other 2 patients were processed by subtotal resection. There was no operative mortality. After operations, 11 patients gained improvement with visual acuity and visual field, 5 had no obviously improvement, and 2 became worse. 7 patients suffered diabetes insipidus. Conclusion The tuberculum and diaphragma sellae meningiomas have a good prognosis by unifrontal interhemispheric approach which allows a wider operative field and better removal. 6 refs.展开更多

Methods Sixty ASA Ⅰ - Ⅱ patients aged 24 - 62 yr, weighing 50 - 76 kg, scheduled for elective gynecological surgery under epidural anesthesia were randomly assigned to one of four equal groups of 15 patients each, according to the dose of clonidine added to ropivacaine solution: group 1 (R) received no clonidine and served as control;group 2 - 4 in which clonidine 50,100 or 150 μg was added to 0. 75% ropivacaine 25 ml (R - C 50, R - C 100, R - C 150). Epidural anesthesia was performed at L2-3. Epidural catheter was inserted into epidural space and advanced in cephalad direction for 3 : 5 cm. Ropivacaine solution was prepared and provided by a specially designated person. A test dose of 5 ml of ropivacaine was given. 5 min later when epidural placement of catheter was confirmed, the rest 20 ml of ropivacaine was given in fractions of 5 ml every 2-3 min.
