BACKGROUND The burden of cannabis use disorder(CUD)in the context of its prevalence and subsequent cardiopulmonary outcomes among cancer patients with severe sepsis is unclear.AIM To address this knowledge gap,especia...BACKGROUND The burden of cannabis use disorder(CUD)in the context of its prevalence and subsequent cardiopulmonary outcomes among cancer patients with severe sepsis is unclear.AIM To address this knowledge gap,especially due to rising patterns of cannabis use and its emerging pharmacological role in cancer.METHODS By applying relevant International Classification of Diseases,Ninth and Tenth Revision,Clinical Modification codes to the National Inpatient Sample database between 2016-2020,we identified CUD(+)and CUD(-)arms among adult cancer admissions with severe sepsis.Comparing the two cohorts,we examined baseline demographic characteristics,epidemiological trends,major adverse cardiac and cerebrovascular events,respiratory failure,hospital cost,and length of stay.We used the Pearsonχ^(2) d test for categorical variables and the Mann-Whitney U test for continuous,non-normally distributed variables.Multivariable regression analysis was used to control for potential confounders.A P value≤0.05 was considered for statistical significance.RESULTS We identified a total of 743520 cancer patients admitted with severe sepsis,of which 4945 had CUD.Demographically,the CUD(+)cohort was more likely to be younger(median age=58 vs 69,P<0.001),male(67.9%vs 57.2%,P<0.001),black(23.7%vs 14.4%,P<0.001),Medicaid enrollees(35.2%vs 10.7%,P<0.001),in whom higher rates of substance use and depression were observed.CUD(+)patients also exhibited a higher prevalence of chronic pulmonary disease but lower rates of cardiovascular comorbidities.There was no significant difference in major adverse cardiac and cerebrovascular events between CUD(+)and CUD(-)cohorts on multivariable regression analysis.However,the CUD(+)cohort had lower all-cause mortality(adjusted odds ratio=0.83,95%confidence interval:0.7-0.97,P<0.001)and respiratory failure(adjusted odds ratio=0.8,95%confidence interval:0.69-0.92,P=0.002).Both groups had similar median length of stay,though CUD(+)patients were more likely to have higher hospital cost compared to CUD(-)patients(median=94574 dollars vs 86615 dollars,P<0.001).CONCLUSION CUD(+)cancer patients with severe sepsis,who tended to be younger,black,males with higher rates of substance use and depression had paradoxically significantly lower odds of all-cause in-hospital mortality and respiratory failure.Future research should aim to better elucidate the underlying mechanisms for these observations.展开更多

Research suggests that teacher made instructional design decisions can lead to enhanced motor skill learning. After presenting a model of evidence-based research this paper presents information that will help teachers plan and execute lessons designed to improve students' motor skills. Variables that impact motor skill learning in physical education including time, type of practice, content, presentation and organizational strategies, and student skill level are presented and discussed. A brief section on student attitudes, their relation to motor skill learning and to physical literacy is included. Motor skills are needed for physically literate people to enjoy lifelong physical activity. Here, the role of the aPKC isoform, PKC iota (PKCι), in the early neuronal differentiation of PC12 cells, was investigated. NGF-treated PC12 cells with stably expressed exogenous wild-type PKCι showed decreased expression of a neuroendocrine marker, increased expression of a neuronal marker and increased neurite formation. Stable expression of a kinase-inactive PKCι, but not constitutively active PKCι lacking a regulatory domain, had similar though less potent effects. Pharmacological inhibition of endogenous aPKC kinase activity in parental PC12 cells did not inhibit neurite formation, suggesting that some of the observed effects of PKCι expression on neuronal differentiation are kinase-independent. Interestingly, exogenous expression of wild-type and kinase-inactive PKCι had little effect on overall PKCι activity, but caused a decrease in PKC zeta (PKCζ) kinase activity, suggesting an interplay between the two isoforms that may underlie the observed results. This affected the language, and, in tu...The purpose of this study is to shed light on the southern part of Italy where Catalonians ruled. Great numbers of Spaniards, principally the Catalonians, headed to that country. This affected the language, and, in turn, the history of people's last names. At first, some of the Spanish last names were used as nicknames for Italians. The method 1 used to get the data from primary sources was from spending time in Italy for four consecutive summers and visiting towns in the Naples area while I collected surnames found on houses. The Catalonians came to rule Sardinia, and their language and subsequently Spanish were official on the island. The linguistic influence of Spanish does not stop with surnames. A list of Spanish and Basque surnames which is redolent of the history of southern Italy and Sicily is appended. The geolinguistics interest lies in the way that the study of language, both ordinary words and proper nouns, offers important clues to the lives and movements of people of ages past, reflects political and economic aspects and also explains the ethnic origin of people who live in Sicily and Italy today or are descendants of Italians who have been important immigrants in the Americas, in Australia, and indeed around the world.展开更多

The reason the author believes dual language programs could benefit all is due to the fact that if done properly, it should promote bilingualism, respect, and equity for all students in the school. Dual language programs are relatively new in the United States. These programs aim to create bilingual, bicultural students without sacrificing these students' success in school or beyond. The goals of dual language are to provide high-quality instruction for language-minority students and to provide instruction in a second language for English-speaking students. Schools teach children through content, with teachers adapting their instruction to ensure children's comprehension and using content lessons to convey vocabulary and language structure. Striving for a balance of half language minority students and half English speaking students in each classroom, dual language programs also aim to teach cross-cultural awareness. Programs vary in terms of the amount of time they devote to each language, which grade levels they serve, how much structure they impose for the division of language and curriculum, and what populations they serve.展开更多

The 12 teachers' stories highl...Teachers of CLD (culturally and linguistically diverse) public school students New York and Rome, Italy were interviewed about their work with this unique, growing subset of learners. The 12 teachers' stories highlight myriad issues facing educators across the globe. Each teacher had recently participated in a research-based and government-sponsored professional development program focused on second language acquisition and culturally responsive pedagogy. Themes emerged through the analysis of individual interviews and classroom observations. These findings highlight teachers' perceptions of effective professional development, along with identification of institutional barriers impeding the delivery of best practices to CLD students. Cross-cultural comparisons are made which suggest an inconsistency in educational services for CLD students, along with an urgent need for professional development courses for teachers and administrators. The educators' voices shed light on both the challenges and rewards associated with this important work.展开更多

Several years ago, it was proven that the common practice of mean centering in moderated regression cannot alleviate multicollinearity among variables comprising an interaction, but merely masks it. Residual centering (orthogonalizing) is unacceptable because it biases parameters for predictors from which the interaction derives, thus precluding interpretation of moderator effects. I propose and validate residual centering in sequential re-estimations of a moderated regression—sequential residual centering (SRC)—by revealing unbiased multicollinearity conditioning across the interaction and its related terms. Across simulations, SRC reduces variance inflation factors (VIF) regardless of distribution shape or pattern of regression coefficients across predictors. For any predictor, the reduced VIF is used to derive a lower standard error of its regression coefficient. A cancer sample illustrates SRC, which allows unbiased interpretations of symptom clusters. SRC can be applied efficiently to alleviate multicollinearity after data collection and shows promise for advancing synergistic frontiers of research.展开更多

Limitations to successful NIV and CPAP therapy were evaluated to determine how prolonged initiation of treatment may lead to hypoxemia (decreased oxygen in the blood) and hypercapnia (increased carbon dioxide in the blood) resulting in poor outcomes. Method: Reviews of literature from nursing and allied health data bases (CINAHL and ProQuest) with terms pulmonary edema, positive pressure device and non-invasive ventilation from 2010 to 2014 were used. Studies were conducted in the hospital and prehospital settings. Results: The literature search located 7 articles from CINAHL and 25 articles from ProQuest. A total of 6 of these articles were analyzed. Additional sources of data were obtained from Ignatavicius and Workman (2013) Medical-Surgical Nursing Patient-Centered Collaborative Care 7th edition and American Journal of Nursing (02/2013) Volume 113: 2. Conclusion: All of the articles concluded that early initiation of continuous positive airway pressure ventilations in the short-term was beneficial;however, late initiation of therapy required additional interventions. The studies indicated that early use of positive airway pressure in acute respiratory distress improved breath rate, heart rate and blood pressure. The use of positive airway pressure for respiratory distress may decrease the need for endotracheal intubation.展开更多

Dream research has often been impaired because researchers were examining dreams during REM ~Rapid Eye Movement) sleep, while many nightmares occur during non-REM sleep. The Beradt's study showed the advantage of retrospective accounts. Dreams of other trauma victims include the discovery of dead bodies and being killed oneself and often replicas of actual events. Such nightmares preclude the restorative function of sleep. The Catalonians came to rule Sardinia, and their language and subsequently Spanish were official on the island. The linguistic influence of Spanish does not stop with surnames. A list of Spanish and Basque surnames which is redolent of the history of southern Italy and Sicily is appended. The geolinguistics interest lies in the way that the study of language, both ordinary words and proper nouns, offers important clues to the lives and movements of people of ages past, reflects political and economic aspects and also explains the ethnic origin of people who live in Sicily and Italy today or are descendants of Italians who have been important immigrants in the Americas, in Australia, and indeed around the world.
文摘This paper makes the case that education aimed at Hispanic or Latino children in the United States needs to recognize both their linguistic and cultural roots (raices) as well as the educational wings (alas). The reason the author believes dual language programs could benefit all is due to the fact that if done properly, it should promote bilingualism, respect, and equity for all students in the school. Dual language programs are relatively new in the United States. These programs aim to create bilingual, bicultural students without sacrificing these students' success in school or beyond. The goals of dual language are to provide high-quality instruction for language-minority students and to provide instruction in a second language for English-speaking students. Schools teach children through content, with teachers adapting their instruction to ensure children's comprehension and using content lessons to convey vocabulary and language structure. Striving for a balance of half language minority students and half English speaking students in each classroom, dual language programs also aim to teach cross-cultural awareness. Programs vary in terms of the amount of time they devote to each language, which grade levels they serve, how much structure they impose for the division of language and curriculum, and what populations they serve.
文摘Teachers of CLD (culturally and linguistically diverse) public school students New York and Rome, Italy were interviewed about their work with this unique, growing subset of learners. The 12 teachers' stories highlight myriad issues facing educators across the globe. Each teacher had recently participated in a research-based and government-sponsored professional development program focused on second language acquisition and culturally responsive pedagogy. Themes emerged through the analysis of individual interviews and classroom observations. These findings highlight teachers' perceptions of effective professional development, along with identification of institutional barriers impeding the delivery of best practices to CLD students. Cross-cultural comparisons are made which suggest an inconsistency in educational services for CLD students, along with an urgent need for professional development courses for teachers and administrators. The educators' voices shed light on both the challenges and rewards associated with this important work.
文摘Multicollinearity constitutes shared variation among predictors that inflates standard errors of regression coefficients. Several years ago, it was proven that the common practice of mean centering in moderated regression cannot alleviate multicollinearity among variables comprising an interaction, but merely masks it. Residual centering (orthogonalizing) is unacceptable because it biases parameters for predictors from which the interaction derives, thus precluding interpretation of moderator effects. I propose and validate residual centering in sequential re-estimations of a moderated regression—sequential residual centering (SRC)—by revealing unbiased multicollinearity conditioning across the interaction and its related terms. Across simulations, SRC reduces variance inflation factors (VIF) regardless of distribution shape or pattern of regression coefficients across predictors. For any predictor, the reduced VIF is used to derive a lower standard error of its regression coefficient. A cancer sample illustrates SRC, which allows unbiased interpretations of symptom clusters. SRC can be applied efficiently to alleviate multicollinearity after data collection and shows promise for advancing synergistic frontiers of research.
文摘Objective: Research was conducted to examine benefits to using non-invasive ventilation (NIV) or continuous positive airway pressure (CPAP) early in the treatment of respiratory distress caused by pulmonary edema, chronic obstructive pulmonary disease (COPD) and asthma. Limitations to successful NIV and CPAP therapy were evaluated to determine how prolonged initiation of treatment may lead to hypoxemia (decreased oxygen in the blood) and hypercapnia (increased carbon dioxide in the blood) resulting in poor outcomes. Method: Reviews of literature from nursing and allied health data bases (CINAHL and ProQuest) with terms pulmonary edema, positive pressure device and non-invasive ventilation from 2010 to 2014 were used. Studies were conducted in the hospital and prehospital settings. Results: The literature search located 7 articles from CINAHL and 25 articles from ProQuest. A total of 6 of these articles were analyzed. Additional sources of data were obtained from Ignatavicius and Workman (2013) Medical-Surgical Nursing Patient-Centered Collaborative Care 7th edition and American Journal of Nursing (02/2013) Volume 113: 2. Conclusion: All of the articles concluded that early initiation of continuous positive airway pressure ventilations in the short-term was beneficial;however, late initiation of therapy required additional interventions. The studies indicated that early use of positive airway pressure in acute respiratory distress improved breath rate, heart rate and blood pressure. The use of positive airway pressure for respiratory distress may decrease the need for endotracheal intubation.
文摘Charlotte Beradt collected hundreds of dreams she gathered in Germany between 1933 and 1939, hid them, and then sent them abroad. According to her The Third Reich of Dreams, people living under the totalitarian government of Nazi Germany had fears of listening devices picking up their own thoughts even when they were asleep, of having no walls, and of being condemned for talking in their sleep. The dreams reveal fears, insecurities, confusion, guilt, and lack of trust. Dream research has often been impaired because researchers were examining dreams during REM ~Rapid Eye Movement) sleep, while many nightmares occur during non-REM sleep. The Beradt's study showed the advantage of retrospective accounts. Dreams of other trauma victims include the discovery of dead bodies and being killed oneself and often replicas of actual events. Such nightmares preclude the restorative function of sleep. Psychotherapy, however, using dreams from traumas when conducted in a safe environment, can be helpful and relieve patients from suffering.

文摘Cardiac arrest(CA)is a life-threatening condition with complex pathophysiology and limited treatment options.To gain deeper insights into the pathological state of vital organs,we employed a proteomics analysis in rodents to assess proteome alterations in the brain,heart,kidney,and liver using a rat model of CA.The brain displayed severe protein alterations in essential cellular pathways,including three major energy-generating pathways after CA,which worsened after resuscitation,resulting in the most significant overall protein changes among the organs.Conversely,the liver,experiencing the most substantial protein alterations post-CA,demonstrated significant recovery,presenting the least protein changes post-resuscitation.

文摘Coronavirus disease 2019 (COVID-19) is one of the worst deadly infections that is currently causing devastating effects and damages to humanity across the world. The global pandemic which outbroke in Wuhan in China has equally affected almost every country in the world. Over the past several months of the coronavirus crisis, cities went into lockdown, countries issued stay-at-home orders, and an increasing number of people had been practicing social distancing on a societal scale that has never been witnessed. However, nurses continue to provide care, comfort, and information to patients at all levels while risking their personal safety and well-being. This executive report provides an insight in Nigerian nurses’ perspectives and challenges in combating COVID-19 during the surge. Capturing nursing practice and nurses’ experiences during COVID-19 surge provide real-time guidance to mitigating challenges and improving well-being of nurses.
