Our ability to capitalize on the strengths of our specific institutional environments and respond to information needs is ever more important if we are to remain central to the work of our academic enterprise. This article describes how the University of North Carolina at Chapel Hill ' s Health Sciences Library(HSL) has sought opportunities and established successful partnerships that integrate library expertise aligning with the two core strategies of Carolina's new strategic framework Blueprint for Next: Of the Public,For the Public,and Innovation Made Fundamental.These efforts have resulted in initiatives that improve access to quality health care and health care information for North Carolinians,accelerate the campus' research enterprise,and demonstrate the importance of evidence-based care at UNC and globally. By understanding and identifying the needs of our various stakeholders,we have been able to progress with our understanding of what key problems need to be solved,what interventions need to be developed,and in what ways librarians can integrate expertise around information synthesis and critical thinking so that we are seen as valued partners in our complex academic environment.展开更多

This new technology exposes students t...Academic dishonesty is a disturbing issue in higher education that has been worsening over the years, especially with the appearance of the internet and the e-learning education. This new technology exposes students to the opportunity of using online bank exams and term papers and increases their tendency to cheat. This study investigates student academic dishonesty in the context of traditional and distance-learning courses in higher education. Data from 1,365 students enrolled in academic institutes in the U.S.A and Israel were surveyed to assess their personality and their willingness to commit various acts of academic misconduct. The findings indicate that in both countries dishonest behaviors are greater in face-to-face courses than in online courses. In addition, both American and Israeli students identified with the personality trait of Agreeableness showed a negative correlation with academic dishonesty. Furthermore, Israeli students identified with the personality traits of Conscientiousness and Emotional Stability demonstrated a negative correlation with academic dishonesty. In contrast, the personality trait of Extraversion among American students was found in a positive correlation with academic misconduct. Implications for further research are discussed.展开更多

With the aim of analyzing the frequency of visualization of alcoholic content in the Facebook, Instagram and Snapchat, patterns of consumption, reasons that lead them to drink, most used social network and to correlate how often the users are exposed to alcoholic publications, determining if there was an important link between individual drinking practice during the beginning period and six months later. The information was obtained through the application of questionnaires for incoming students of the freshmen semester, in two moments: a period from August to December 2016/2, and, asking the same students again, from March to June 2017/1. 52.8% were males in 2016/2 and 57, 9% of females in 2017/1 with age ranging between 18 and 20 years in both periods. Facebook has proven to be the most used platform among college students. In 2016/2, 63.9% reported seeing alcohol content at Facebook and consuming that substance, comparing to Instagram with 70.4%, and to Snapchat with 74.1%. In 2017/2, academics reporting viewing alcoholic content on Facebook and consuming those alcoholic beverages accounted for 65.8%, compared to Instagram, 74.1% and Snapchat 74.4%. “Celebrating a special occasion with friends” is the most cited reason of why claim to consume alcohol. Therelation between exposure to the alcohol content of the virtual media and the network of friends was found imperceptible to the majority of the students, as they mostly do not consider themselves exposed. Many university students start the graduation already consuming alcoholic substances, which explains why there is no significant change in alcohol consumption between the observed moments, although it was found a relationship between the visualization of alcohol content in social networks and consumption, with greater impact on Snapchat.展开更多

Represented by abscesses and empyemas, their management has been improved by the combined contributions of new diagnostic and therapeutic methods. Objective: Appreciate the management of intracranial suppurations banal germ in our service. Material and methods: A retrospective study of 41 cases of intra-cranial suppurations was conducted at Brazzaville University Hospital, from January 2007 to June 2019. Diagnostic, therapeutic and evolution aspects have been studied. Results: They are divided into 24 empyemas, 8 abscesses and 9 associations abscess-empyema. The average age was 20.2 years old in general. The male predominance is noted in all groups. The sex ratio is 3.1 for all of these intracranial suppurations (ICS). They frequently complicated Oto-Rhino-Laryngology (ORL) infections (41.4%). The clinic was mostly represented by the Bergman’s triad 51.2%, followed by headache 14.6% and seizures 12.2%. The lesions were predominant in supratentorial 97.5%. Frontal location was found in 67.4%. Germs were isolated in 29.4% of samples. There were 5 Streptococci, 3 Staphylococci and 2 Gram-negative Bacilli. The medico-surgical treatment indicated in 80.5% combined often probabilistic tri-antibiotherapy with the evacuation of the pus by the trepano-puncture. Evolution under this treatment was marked by healing in 90.2%. The lethality was 9.7%. Conclusion: The pursuit of the improvement of these infections requires the adequate treatment of their gateways, most of them loco-regional.展开更多

The emergence of new data has always been a major force driving the advance of informetrics. The magnitude of data usage for academic literature, given by the Web of Science(Wo S), can be expected to provide a supplem...The emergence of new data has always been a major force driving the advance of informetrics. The magnitude of data usage for academic literature, given by the Web of Science(Wo S), can be expected to provide a supplementary perspective for citation analysis in three theoretical aspects, which are influence dimension, discipline difference and behavioral motive. Based on 166,767 articles in physics, computer science, economics, and Library and Information Science included by SCI and SSCI in 2013 as research samples, this paper conducts an exploratory investigation into the measurement features of the usage data. According to the results, compared with citation data, usage is more discriminative and sensitive. In terms of distribution, usage presents an approximate and positively skewed distribution at high-frequency. An approximate power-law distribution is observed in the cumulative integral. The evaluation result of usage is independent to a certain degree and poses no essential opposition to the result of citation. Although the usage of the Wo S platform still has the limitations of generality, falsifiability and isolation, usage may be accepted as one of the possible options that supplement citation data and provide a richer field of vision for influence evaluation. Quantitative detection of sleeve grouting compactness is a technical challenge in civil engineering testing.This study explores a novel quantitative detection method based on ultrasonic time-frequency dual-domain anal...Quantitative detection of sleeve grouting compactness is a technical challenge in civil engineering testing.This study explores a novel quantitative detection method based on ultrasonic time-frequency dual-domain analysis.It establishes a mapping relationship between sleeve grouting compactness and characteristic parameters.First,this study made samples with gradient defects for two types of grouting sleeves,G18 and G20.These included four cases:2D,4D,6D defects(where D is the diameter of the grouting sleeve),and no-defect.Then,an ultrasonic input/output data acquisition system was established.Three-dimensional sound field distribution data were obtained through an orthogonal detection layout and pulse reflection principles.Finally,a novel quantification detection with a comprehensive defect index(DI)was established by comprehensively considering eight feature parameters,such as time-frequency domain Kurtosis factor(KU),Skewness factor(SK),Formfactor(FF),Crest factor(CF),Impulse factor(IF),Clearance factor(CLF),Wavelet packet energy entropy(WPEE),and Hilbert energy peak(HEP).Construct a DI index by quantifying the difference between defect signals and defect free signals in the time-frequency domain.Experimental results show that,under no-defect conditions,the values of feature parameters are significantly lower than those under defect conditions.Among these,the KU,FF,CF,WPEE and HEP exhibit strong correlations with grout sleeve compactness.The proposed DI index in both types of grout sleeves showed good universality with a linear fit goodness of 0.847–0.962.However,G20 the larger inner diameter and length of the sleeve result in a more complex medium effect during ultrasonic propagation,making its DI index more sensitive to defects than the G18 sleeve.Therefore,the presented method is effective for quantitative detection and analysis of the compactness of grouting sleeves.展开更多
Objective Post tuberculosis lung disease(PTLD)manifests in various forms,including tuberculosisassociated chronic obstructive pulmonary disease(TB-COPD),yet the clinical features of PTLD remain undercharacterized.This...Objective Post tuberculosis lung disease(PTLD)manifests in various forms,including tuberculosisassociated chronic obstructive pulmonary disease(TB-COPD),yet the clinical features of PTLD remain undercharacterized.This study aimed to assess longitudinal changes in lung function over a 5-year period and to identify predictors of airflow obstruction in a cohort of patients treated for active pulmonary TB.Methods Patients with active pulmonary TB were enrolled in this study and were followed during treatment,at treatment completion and five years post-treatment.Assessments included lung function and chest CT,analyzing longitudinal trends and airflow obstruction risk factors.Results Among 53 patients(mean age 36.9±13.9 years;64.2%male),7 patients(13.2%)exhibited airflow obstruction.At the 5-year follow-up,the mean FEV_(1)/FVC declined significantly(76.27%±12.04%vs.80.23%±11.02%,P<0.001)and 9 patients(17.0%)exhibited airflow obstruction.Seven of these patients predominantly showed air trapping consistent with small airway disease on chest CT,aligning with TB-COPD phenotype.Notably,four young-to-middle-aged patients(<60 years old)had persistent obstruction over the five years.Conclusion The initial test revealed that 13.2%of patients presented with airflow obstruction.By the 5-year follow-up,this proportion had increased to 17.0%,with most cases demonstrating imaging findings aligning with TB-COPD,even among younger,non-smoking individuals.These findings emphasize the importance of long-term follow-up and routine lung function assessments in TB survivors.展开更多
Objective To determine whether frailty is a risk factor for hospitalization and mortality in older adults enrolled in the "Chronic Heart Failure Program" at a hospital in Lima, Peru, between 2018–2021.Metho...Objective To determine whether frailty is a risk factor for hospitalization and mortality in older adults enrolled in the "Chronic Heart Failure Program" at a hospital in Lima, Peru, between 2018–2021.Methods This was an ambispective cohort study. A total of 85 older adults participating in the Chronic Heart Failure Program at Guillermo Almenara National Hospital were included. Each had an initial frailty assessment, forming two cohorts: frail and non-frail older adults. Medical records were reviewed, and patients were followed for one year to track events of interest(hospitalization and mortality). Overall survival and risk factors for hospitalization and death were determined.Results During follow-up, 15.3% of the older adults died, and frailty was identified in 58.8% of the patients. Overall survival using the Kaplan-Meier method was 96.5% at 3 months after entering the Chronic Heart Failure Program;92.9% at 6 months;and85.9% at one year. Multivariate analysis using Poisson regression found that frailty was not a risk factor for hospitalization(a RR =0.92;95% CI: 0.42–2.03). Survival analysis using the Cox proportional hazards model showed that frailty was also not a risk factor for mortality after one year of follow-up(a HR = 1.32;95% CI: 0.27–6.53).Conclusions Our research does not confirm frailty as a risk factor for hospitalization or mortality in older adults enrolled in the“Chronic Heart Failure Program” after one year of follow-up.展开更多
Flexible materials with perovskite quantum dots(PQDs)are widely used in the field of photonics and opto-electronics due to their unique properties.Development of new materials based on these nanoparticles,incorporated...Flexible materials with perovskite quantum dots(PQDs)are widely used in the field of photonics and opto-electronics due to their unique properties.Development of new materials based on these nanoparticles,incorporated into flexible and lightweight nonwoven fabrics,demonstrated high photoconductivity and efficient light energy conversion.In this work,we propose a method for creating a stable luminescent nonwoven material using electrospinning,in which inorganic salt precursors are used without the need for additional stabilizers.Equimolar solutions of cesium and lead(Ⅱ)bromide were mixed with a fluoroplast,resulting in a series of samples.Luminescent materials were obtained containing PQDs with a composition of CsPbBr_(3),with emission peaks ranging from 507 to 517 nm under 365-nm excitation.We have experimentally established and theoretically confirmed that the peak position is related to the size of the particles formed in the fiber during electrospinning and depends on processing time.Developed materials exhibited stable luminescent properties for up to 2.5 years,making them a promising candidate for the development of new flexible optoelectronic devices based on PQDs.展开更多
Objectives:The PACIFIC trial established the benefit of durvalumab following chemo-radiotherapy for stage III non-small cell lung cancer(NSCLC).However,the concurrent use of radiotherapy(RT)and durvalumab(PACIFIC-2 tr...Objectives:The PACIFIC trial established the benefit of durvalumab following chemo-radiotherapy for stage III non-small cell lung cancer(NSCLC).However,the concurrent use of radiotherapy(RT)and durvalumab(PACIFIC-2 trial)showed no additional advantage.The PD-RAD study was set up to understand the immunological effects of RT on the tumor microenvironment(TME)to aid in optimizing sequencing of combination therapies.Methods:The PD-RAD trial(ClinicalTrials.gov identifier:NCT03258788)aimed to enroll thirty NSCLC patients receiving radical-intent RT.Tumor biopsies and blood samples were collected pre-RT and at week 2 during RT and analyzed using multiplex immunohistochemistry(mIHC)and high-dimensional mass cytometry(CyTOF),respectively.Results:Paired biopsies were collected from only three patients(Pts 1,3&4)and blood from four patients(Pts 1-4)before the study was closed early during the COVID-19 pandemic.Programmed Death-Ligand 1(PD-L1)expression in the TME was raised in Patient 1,who responded well to treatment,and unaltered in two patients with progressive disease.CyTOF analysis revealed elevated circulating classical monocytes,highest in the patient with a good response.Conclusions:This study underscores the challenges of integrating advanced immune monitoring during RT delivery and did not meet its primary endpoint.The hypothesis-generating findings highlight PD-L1+macrophages in the TME and classical monocytes in the blood as potential immune biomarkers of RT response,but larger studies are needed to validate these observations and characterize the immune changes following curative-intent RT in patients with NSCLC.展开更多
Clostridioides difficile(C.difficile)is one of the major causes of nosocomial infections.Pregnant women,who are generally considered at low risk for C.difficile infection(CDI),have attracted attention because of an in...Clostridioides difficile(C.difficile)is one of the major causes of nosocomial infections.Pregnant women,who are generally considered at low risk for C.difficile infection(CDI),have attracted attention because of an increasing number of reports.Oral vancomycin,the only first-line treatment for pregnant women infected with C.difficile,has been associated with increasing strain resistance,leading to decreased efficacy.Fecal microbiota transplantation(FMT)is recommended for severe,fulminant,and recurrent CDI;however,it is generally avoided in pregnant women because of safety concerns.We report a case of a pregnant woman with a primary ribotype 027 CDI who experienced a successful outcome with washed microbiota transplantation(WMT),an improved form of FMT,via enema.The specific strain of ribotype 027 is related to severe outcomes but has not previously been reported in pregnant women.The follow-up lasted for two years,during which the patient's diarrhea was fully alleviated without recurrence.The baby showed normal growth and development,and no adverse events were recorded for either.This case provides evidence for the efficacy and safety of WMT in pregnant women infected with C.difficile,indicating that WMT via enema may be a viable therapeutic strategy for this population for treating CDI.展开更多

Complex genetic relationships between neurodegenerative disorders and neuropsychiatric symptoms have been shown, suggesting shared pathogenic mechanisms and emphasizing the potential for developing common therapeutic ...Complex genetic relationships between neurodegenerative disorders and neuropsychiatric symptoms have been shown, suggesting shared pathogenic mechanisms and emphasizing the potential for developing common therapeutic targets. Apolipoprotein E(APOE) genotypes and their corresponding protein(Apo E) isoforms may influence the biophysical properties of the cell membrane lipid bilayer. However, the role of APOE in central nervous system pathophysiology extended beyond its lipid transport function. In the present review article, we analyzed the links existing between APOE genotypes and the neurobiology of neuropsychiatric symptoms in neurodegenerative and vascular diseases. APOE genotypes(APOE ε2, APOE ε3, and APOE ε4) were implicated in common mechanisms underlying a wide spectrum of neurodegenerative diseases, including sporadic Alzheimer's disease, synucleinopathies such as Parkinson's disease and Lewy body disease, stroke, and traumatic brain injury. These shared pathways often involved neuroinflammation, abnormal protein accumulation, or responses to acute detrimental events. Across these conditions, APOE variants are believed to contribute to the modulation of inflammatory responses, the regulation of amyloid and tau pathology, as well as the clearance of proteins such as α-synuclein. The bidirectional interactions among Apo E, amyloid and mitochondrial metabolism, immunomodulatory effects, neuronal repair, and remodeling underscored the complexity of Apo E's role in neuropsychiatric symptoms associated with these conditions since from early phases of cognitive impairment such as mild cognitive impairment and mild behavioral impairment. Besides Apo E-specific isoforms' link to increased neuropsychiatric symptoms in Alzheimer's disease(depression, psychosis, aberrant motor behaviors, and anxiety, not apathy), the APOE ε4 genotype was also considered a significant genetic risk factor for Lewy body disease and its worse cognitive outcomes. Conversely, the APOE ε2 variant has been observed not to exert a protective effect equally in all neurodegenerative diseases. Specifically, in Lewy body disease, this variant may delay disease onset, paralleling its protective role in Alzheimer's disease, although its role in frontotemporal dementia is uncertain. The APOE ε4 genotype has been associated with adverse cognitive outcomes across other various neurodegenerative conditions. In Parkinson's disease, the APOE ε4 allele significantly impacted cognitive performance, increasing the risk of developing dementia, even in cases of pure synucleinopathies with minimal co-pathology from Alzheimer's disease. Similarly, in traumatic brain injury, recovery rates varied, with APOE ε4 carriers demonstrating a greater risk of poor long-term cognitive outcomes and elevated levels of neuropsychiatric symptoms. Furthermore, APOE ε4 influenced the age of onset and severity of stroke, as well as the likelihood of developing stroke-associated dementia, potentially due to its role in compromising endothelial integrity and promoting blood–brain barrier dysfunction.展开更多

By understanding and identifying the needs of our various stakeholders,we have been able to progress with our understanding of what key problems need to be solved,what interventions need to be developed,and in what ways librarians can integrate expertise around information synthesis and critical thinking so that we are seen as valued partners in our complex academic environment.
文摘Academic dishonesty is a disturbing issue in higher education that has been worsening over the years, especially with the appearance of the internet and the e-learning education. This new technology exposes students to the opportunity of using online bank exams and term papers and increases their tendency to cheat. This study investigates student academic dishonesty in the context of traditional and distance-learning courses in higher education. Data from 1,365 students enrolled in academic institutes in the U.S.A and Israel were surveyed to assess their personality and their willingness to commit various acts of academic misconduct. The findings indicate that in both countries dishonest behaviors are greater in face-to-face courses than in online courses. In addition, both American and Israeli students identified with the personality trait of Agreeableness showed a negative correlation with academic dishonesty. Furthermore, Israeli students identified with the personality traits of Conscientiousness and Emotional Stability demonstrated a negative correlation with academic dishonesty. In contrast, the personality trait of Extraversion among American students was found in a positive correlation with academic misconduct. Implications for further research are discussed.

文摘This is a descriptive study of quantitative approach, developed at the Federal University of Rond?nia located in the Western Amazon. With the aim of analyzing the frequency of visualization of alcoholic content in the Facebook, Instagram and Snapchat, patterns of consumption, reasons that lead them to drink, most used social network and to correlate how often the users are exposed to alcoholic publications, determining if there was an important link between individual drinking practice during the beginning period and six months later. The information was obtained through the application of questionnaires for incoming students of the freshmen semester, in two moments: a period from August to December 2016/2, and, asking the same students again, from March to June 2017/1. 52.8% were males in 2016/2 and 57, 9% of females in 2017/1 with age ranging between 18 and 20 years in both periods. Facebook has proven to be the most used platform among college students. In 2016/2, 63.9% reported seeing alcohol content at Facebook and consuming that substance, comparing to Instagram with 70.4%, and to Snapchat with 74.1%. In 2017/2, academics reporting viewing alcoholic content on Facebook and consuming those alcoholic beverages accounted for 65.8%, compared to Instagram, 74.1% and Snapchat 74.4%. “Celebrating a special occasion with friends” is the most cited reason of why claim to consume alcohol. Therelation between exposure to the alcohol content of the virtual media and the network of friends was found imperceptible to the majority of the students, as they mostly do not consider themselves exposed. Many university students start the graduation already consuming alcoholic substances, which explains why there is no significant change in alcohol consumption between the observed moments, although it was found a relationship between the visualization of alcohol content in social networks and consumption, with greater impact on Snapchat.
文摘Background: Intracranial suppurations (ICS) due to banal germs are medical-surgical emergencies, despite their little removed frequency. Represented by abscesses and empyemas, their management has been improved by the combined contributions of new diagnostic and therapeutic methods. Objective: Appreciate the management of intracranial suppurations banal germ in our service. Material and methods: A retrospective study of 41 cases of intra-cranial suppurations was conducted at Brazzaville University Hospital, from January 2007 to June 2019. Diagnostic, therapeutic and evolution aspects have been studied. Results: They are divided into 24 empyemas, 8 abscesses and 9 associations abscess-empyema. The average age was 20.2 years old in general. The male predominance is noted in all groups. The sex ratio is 3.1 for all of these intracranial suppurations (ICS). They frequently complicated Oto-Rhino-Laryngology (ORL) infections (41.4%). The clinic was mostly represented by the Bergman’s triad 51.2%, followed by headache 14.6% and seizures 12.2%. The lesions were predominant in supratentorial 97.5%. Frontal location was found in 67.4%. Germs were isolated in 29.4% of samples. There were 5 Streptococci, 3 Staphylococci and 2 Gram-negative Bacilli. The medico-surgical treatment indicated in 80.5% combined often probabilistic tri-antibiotherapy with the evacuation of the pus by the trepano-puncture. Evolution under this treatment was marked by healing in 90.2%. The lethality was 9.7%. Conclusion: The pursuit of the improvement of these infections requires the adequate treatment of their gateways, most of them loco-regional.

文摘The emergence of new data has always been a major force driving the advance of informetrics. The magnitude of data usage for academic literature, given by the Web of Science(Wo S), can be expected to provide a supplementary perspective for citation analysis in three theoretical aspects, which are influence dimension, discipline difference and behavioral motive. Based on 166,767 articles in physics, computer science, economics, and Library and Information Science included by SCI and SSCI in 2013 as research samples, this paper conducts an exploratory investigation into the measurement features of the usage data. According to the results, compared with citation data, usage is more discriminative and sensitive. In terms of distribution, usage presents an approximate and positively skewed distribution at high-frequency. An approximate power-law distribution is observed in the cumulative integral. The evaluation result of usage is independent to a certain degree and poses no essential opposition to the result of citation. Although the usage of the Wo S platform still has the limitations of generality, falsifiability and isolation, usage may be accepted as one of the possible options that supplement citation data and provide a richer field of vision for influence evaluation. 文摘Across different eras and cultures,allusions have been one of the most subtle and at the same time effective artistic devices for conveying an author’s message through subtext.The Soviet era in Russian history was one of those socio-cultural contexts that gave birth to so many artifacts of this kind that it could easily be considered a classic example of the use of this technique in the modern times.The article analyzes various forms of allusion,including quotation as a type of contextual metaphor,in the works of the leading and at the same time most targeted Soviet composer Dmitri Shostakovich.Quotations from famous classical works in Shostakovich’s music,whether or not they are considered to have a predetermined meaning,are addressed along with references to classical,popular,or folk music,as well as messages conveyed through Jewish idiom,whatever it may be,which in itself seems truly unique to the Russian composer’s toolkit.
文摘The purpose of this article is to contextualize the world’s oldest art song directly associated with the railway-Travelling Song by the Russian composer Mikhail Glinka(1804-1857).The composer’s personal circumstances surrounding the creation of the song are examined,along with the historical and technical background of that era.While railways have always been associated with technological progress,new solutions for high-power engines,and the accuracy of arrival and departure timetables,the poetic and romantic side of train travel and railways in general has not yet received sufficient academic coverage.This essay is intended to partially fill this gap,though it rather outlines directions for possible development of its topical range.Each of the suggested reference points and stories is in itself an example of how the arts reflected the inspiration that cutting-edge technological advances of the period were exerting on artists,musicians,and other creative individuals.

文摘Quantitative detection of sleeve grouting compactness is a technical challenge in civil engineering testing.This study explores a novel quantitative detection method based on ultrasonic time-frequency dual-domain analysis.It establishes a mapping relationship between sleeve grouting compactness and characteristic parameters.First,this study made samples with gradient defects for two types of grouting sleeves,G18 and G20.These included four cases:2D,4D,6D defects(where D is the diameter of the grouting sleeve),and no-defect.Then,an ultrasonic input/output data acquisition system was established.Three-dimensional sound field distribution data were obtained through an orthogonal detection layout and pulse reflection principles.Finally,a novel quantification detection with a comprehensive defect index(DI)was established by comprehensively considering eight feature parameters,such as time-frequency domain Kurtosis factor(KU),Skewness factor(SK),Formfactor(FF),Crest factor(CF),Impulse factor(IF),Clearance factor(CLF),Wavelet packet energy entropy(WPEE),and Hilbert energy peak(HEP).Construct a DI index by quantifying the difference between defect signals and defect free signals in the time-frequency domain.Experimental results show that,under no-defect conditions,the values of feature parameters are significantly lower than those under defect conditions.Among these,the KU,FF,CF,WPEE and HEP exhibit strong correlations with grout sleeve compactness.The proposed DI index in both types of grout sleeves showed good universality with a linear fit goodness of 0.847–0.962.However,G20 the larger inner diameter and length of the sleeve result in a more complex medium effect during ultrasonic propagation,making its DI index more sensitive to defects than the G18 sleeve.Therefore,the presented method is effective for quantitative detection and analysis of the compactness of grouting sleeves.

文摘Objective Post tuberculosis lung disease(PTLD)manifests in various forms,including tuberculosisassociated chronic obstructive pulmonary disease(TB-COPD),yet the clinical features of PTLD remain undercharacterized.This study aimed to assess longitudinal changes in lung function over a 5-year period and to identify predictors of airflow obstruction in a cohort of patients treated for active pulmonary TB.Methods Patients with active pulmonary TB were enrolled in this study and were followed during treatment,at treatment completion and five years post-treatment.Assessments included lung function and chest CT,analyzing longitudinal trends and airflow obstruction risk factors.Results Among 53 patients(mean age 36.9±13.9 years;64.2%male),7 patients(13.2%)exhibited airflow obstruction.At the 5-year follow-up,the mean FEV_(1)/FVC declined significantly(76.27%±12.04%vs.80.23%±11.02%,P<0.001)and 9 patients(17.0%)exhibited airflow obstruction.Seven of these patients predominantly showed air trapping consistent with small airway disease on chest CT,aligning with TB-COPD phenotype.Notably,four young-to-middle-aged patients(<60 years old)had persistent obstruction over the five years.Conclusion The initial test revealed that 13.2%of patients presented with airflow obstruction.By the 5-year follow-up,this proportion had increased to 17.0%,with most cases demonstrating imaging findings aligning with TB-COPD,even among younger,non-smoking individuals.These findings emphasize the importance of long-term follow-up and routine lung function assessments in TB survivors.
文摘Objective To determine whether frailty is a risk factor for hospitalization and mortality in older adults enrolled in the "Chronic Heart Failure Program" at a hospital in Lima, Peru, between 2018–2021.Methods This was an ambispective cohort study. A total of 85 older adults participating in the Chronic Heart Failure Program at Guillermo Almenara National Hospital were included. Each had an initial frailty assessment, forming two cohorts: frail and non-frail older adults. Medical records were reviewed, and patients were followed for one year to track events of interest(hospitalization and mortality). Overall survival and risk factors for hospitalization and death were determined.Results During follow-up, 15.3% of the older adults died, and frailty was identified in 58.8% of the patients. Overall survival using the Kaplan-Meier method was 96.5% at 3 months after entering the Chronic Heart Failure Program;92.9% at 6 months;and85.9% at one year. Multivariate analysis using Poisson regression found that frailty was not a risk factor for hospitalization(a RR =0.92;95% CI: 0.42–2.03). Survival analysis using the Cox proportional hazards model showed that frailty was also not a risk factor for mortality after one year of follow-up(a HR = 1.32;95% CI: 0.27–6.53).Conclusions Our research does not confirm frailty as a risk factor for hospitalization or mortality in older adults enrolled in the“Chronic Heart Failure Program” after one year of follow-up.

文摘Flexible materials with perovskite quantum dots(PQDs)are widely used in the field of photonics and opto-electronics due to their unique properties.Development of new materials based on these nanoparticles,incorporated into flexible and lightweight nonwoven fabrics,demonstrated high photoconductivity and efficient light energy conversion.In this work,we propose a method for creating a stable luminescent nonwoven material using electrospinning,in which inorganic salt precursors are used without the need for additional stabilizers.Equimolar solutions of cesium and lead(Ⅱ)bromide were mixed with a fluoroplast,resulting in a series of samples.Luminescent materials were obtained containing PQDs with a composition of CsPbBr_(3),with emission peaks ranging from 507 to 517 nm under 365-nm excitation.We have experimentally established and theoretically confirmed that the peak position is related to the size of the particles formed in the fiber during electrospinning and depends on processing time.Developed materials exhibited stable luminescent properties for up to 2.5 years,making them a promising candidate for the development of new flexible optoelectronic devices based on PQDs.

文摘Objectives:The PACIFIC trial established the benefit of durvalumab following chemo-radiotherapy for stage III non-small cell lung cancer(NSCLC).However,the concurrent use of radiotherapy(RT)and durvalumab(PACIFIC-2 trial)showed no additional advantage.The PD-RAD study was set up to understand the immunological effects of RT on the tumor microenvironment(TME)to aid in optimizing sequencing of combination therapies.Methods:The PD-RAD trial(ClinicalTrials.gov identifier:NCT03258788)aimed to enroll thirty NSCLC patients receiving radical-intent RT.Tumor biopsies and blood samples were collected pre-RT and at week 2 during RT and analyzed using multiplex immunohistochemistry(mIHC)and high-dimensional mass cytometry(CyTOF),respectively.Results:Paired biopsies were collected from only three patients(Pts 1,3&4)and blood from four patients(Pts 1-4)before the study was closed early during the COVID-19 pandemic.Programmed Death-Ligand 1(PD-L1)expression in the TME was raised in Patient 1,who responded well to treatment,and unaltered in two patients with progressive disease.CyTOF analysis revealed elevated circulating classical monocytes,highest in the patient with a good response.Conclusions:This study underscores the challenges of integrating advanced immune monitoring during RT delivery and did not meet its primary endpoint.The hypothesis-generating findings highlight PD-L1+macrophages in the TME and classical monocytes in the blood as potential immune biomarkers of RT response,but larger studies are needed to validate these observations and characterize the immune changes following curative-intent RT in patients with NSCLC.

文摘Clostridioides difficile(C.difficile)is one of the major causes of nosocomial infections.Pregnant women,who are generally considered at low risk for C.difficile infection(CDI),have attracted attention because of an increasing number of reports.Oral vancomycin,the only first-line treatment for pregnant women infected with C.difficile,has been associated with increasing strain resistance,leading to decreased efficacy.Fecal microbiota transplantation(FMT)is recommended for severe,fulminant,and recurrent CDI;however,it is generally avoided in pregnant women because of safety concerns.We report a case of a pregnant woman with a primary ribotype 027 CDI who experienced a successful outcome with washed microbiota transplantation(WMT),an improved form of FMT,via enema.The specific strain of ribotype 027 is related to severe outcomes but has not previously been reported in pregnant women.The follow-up lasted for two years,during which the patient's diarrhea was fully alleviated without recurrence.The baby showed normal growth and development,and no adverse events were recorded for either.This case provides evidence for the efficacy and safety of WMT in pregnant women infected with C.difficile,indicating that WMT via enema may be a viable therapeutic strategy for this population for treating CDI.
文摘The advent of quantum computing poses a significant challenge to traditional cryptographic protocols,particularly those used in SecureMultiparty Computation(MPC),a fundamental cryptographic primitive for privacypreserving computation.Classical MPC relies on cryptographic techniques such as homomorphic encryption,secret sharing,and oblivious transfer,which may become vulnerable in the post-quantum era due to the computational power of quantum adversaries.This study presents a review of 140 peer-reviewed articles published between 2000 and 2025 that used different databases like MDPI,IEEE Explore,Springer,and Elsevier,examining the applications,types,and security issues with the solution of Quantum computing in different fields.This review explores the impact of quantum computing on MPC security,assesses emerging quantum-resistant MPC protocols,and examines hybrid classicalquantum approaches aimed at mitigating quantum threats.We analyze the role of Quantum Key Distribution(QKD),post-quantum cryptography(PQC),and quantum homomorphic encryption in securing multiparty computations.Additionally,we discuss the challenges of scalability,computational efficiency,and practical deployment of quantumsecure MPC frameworks in real-world applications such as privacy-preserving AI,secure blockchain transactions,and confidential data analysis.This review provides insights into the future research directions and open challenges in ensuring secure,scalable,and quantum-resistant multiparty computation.
文摘Complex genetic relationships between neurodegenerative disorders and neuropsychiatric symptoms have been shown, suggesting shared pathogenic mechanisms and emphasizing the potential for developing common therapeutic targets. Apolipoprotein E(APOE) genotypes and their corresponding protein(Apo E) isoforms may influence the biophysical properties of the cell membrane lipid bilayer. However, the role of APOE in central nervous system pathophysiology extended beyond its lipid transport function. In the present review article, we analyzed the links existing between APOE genotypes and the neurobiology of neuropsychiatric symptoms in neurodegenerative and vascular diseases. APOE genotypes(APOE ε2, APOE ε3, and APOE ε4) were implicated in common mechanisms underlying a wide spectrum of neurodegenerative diseases, including sporadic Alzheimer's disease, synucleinopathies such as Parkinson's disease and Lewy body disease, stroke, and traumatic brain injury. These shared pathways often involved neuroinflammation, abnormal protein accumulation, or responses to acute detrimental events. Across these conditions, APOE variants are believed to contribute to the modulation of inflammatory responses, the regulation of amyloid and tau pathology, as well as the clearance of proteins such as α-synuclein. The bidirectional interactions among Apo E, amyloid and mitochondrial metabolism, immunomodulatory effects, neuronal repair, and remodeling underscored the complexity of Apo E's role in neuropsychiatric symptoms associated with these conditions since from early phases of cognitive impairment such as mild cognitive impairment and mild behavioral impairment. Besides Apo E-specific isoforms' link to increased neuropsychiatric symptoms in Alzheimer's disease(depression, psychosis, aberrant motor behaviors, and anxiety, not apathy), the APOE ε4 genotype was also considered a significant genetic risk factor for Lewy body disease and its worse cognitive outcomes. Conversely, the APOE ε2 variant has been observed not to exert a protective effect equally in all neurodegenerative diseases. Specifically, in Lewy body disease, this variant may delay disease onset, paralleling its protective role in Alzheimer's disease, although its role in frontotemporal dementia is uncertain. The APOE ε4 genotype has been associated with adverse cognitive outcomes across other various neurodegenerative conditions. In Parkinson's disease, the APOE ε4 allele significantly impacted cognitive performance, increasing the risk of developing dementia, even in cases of pure synucleinopathies with minimal co-pathology from Alzheimer's disease. Similarly, in traumatic brain injury, recovery rates varied, with APOE ε4 carriers demonstrating a greater risk of poor long-term cognitive outcomes and elevated levels of neuropsychiatric symptoms. Furthermore, APOE ε4 influenced the age of onset and severity of stroke, as well as the likelihood of developing stroke-associated dementia, potentially due to its role in compromising endothelial integrity and promoting blood–brain barrier dysfunction.
