Psoriasis is a hereditary,autoimmune,chronic illness that influences the immune system and can have both cutaneous and systemic symptoms.It can seriously impair a patient’s quality of life.Psoriasis affects 2.3 perce...Psoriasis is a hereditary,autoimmune,chronic illness that influences the immune system and can have both cutaneous and systemic symptoms.It can seriously impair a patient’s quality of life.Psoriasis affects 2.3 percent of people globally and has a significant financial cost for those who suffer from it.Genes and environmental factors are the primary etiological factors.Dendritic cells,T cells,human neutrophilic peptides,lipoprotein-2,galactosin-3,fractalkine,vaspin,and familial predispositions,among other factors,are characteristics of the pathophysiology of psoriasis.Conventional psoriasis treatments for patients include corticosteroids,biological agents,vitamin D3 analogs,acitretin,calcineurin inhibitors,cyclosporine,methotrexate,and phototherapy.Growing in popularity as a multidisciplinary field of study,nano dermatology is being used to treat psoriasis.Over the years,major advancements have been made in understanding its complex pathogenesis and developing more effective,targeted treatments.Medication delivery methods utilizing nanocarriers demonstrate promise in treating psoriasis because they improve medication penetration,reduce side effects,and provide targeted action at the afflicted areas.Because of their biological compatibility,adaptability,and capacity for carrying a variety of therapeutic substances,lipid-based and polymer-based nanocarriers have demonstrated exceptional promise among them.This article summarizes the pathogenesis,epidemiology,clinical diagnosis,and conventional psoriasis treatments.Furthermore,the review includes an overview of various nanotechnology-based psoriasis treatments.展开更多

Purpose:The study examines the synergy and hysteresis in the evolution of funding and its supported literature,depicts their temporal correlation mechanism,which aids in improving trend predictions.Design/methodology/...Purpose:The study examines the synergy and hysteresis in the evolution of funding and its supported literature,depicts their temporal correlation mechanism,which aids in improving trend predictions.Design/methodology/approach:The study uses the LDA model to identify topics in funding texts and supported papers.A cosine similarity algorithm was employed to estimate the nexus between topics and construct the topic evolution time series.Similarly,the hysteresis effect in topic evolution is analyzed based on topic popularity and content,leading to insights into their temporal correlation mechanism.Findings:The study finds that fund and sponsored paper topics exhibit strong collaboration with a noticeable lag in evolution.The fund topics significantly influence sponsored paper topics after a two-year lag.Moreover,the lag effect is inversely proportional to the topic’s similarity.Research limitations:We use the LDA model to determine the hysteresis effect in topic evolution despite its limitations in handling long-tail words and domain-specific vocabulary.Furthermore,the timing of the emergence of the focal topic in funds is undermined,affecting the findings.Practical implications:These findings enhance the accuracy and scientific validity of trend prediction.Estimating and identifying patterns can help technology managers anticipate future research hotspots,supporting informed decision-making and technology management.Originality/value:This study introduces a research framework to quantitatively and visually analyze the hysteresis effect,revealing the correlation and evolutionary patterns between fund research topics and their funded papers.展开更多
BACKGROUND Eosinophilic esophagitis(EoE)is a chronic inflammatory disorder presenting as symptoms of dysphagia,esophageal food impaction,chest pain,and heartburn.After an initial trial of proton pump inhibitor(PPI)the...BACKGROUND Eosinophilic esophagitis(EoE)is a chronic inflammatory disorder presenting as symptoms of dysphagia,esophageal food impaction,chest pain,and heartburn.After an initial trial of proton pump inhibitor(PPI)therapy,swallowed topical corticosteroids(STC)are effective as induction therapy for EoE.However,out-come data for STC as a maintenance strategy is limited.RESULTS Three randomized control trials and one observational study were included,involving 303 patients(189 in the STC group,114 in the placebo-controlled group).Analysis showed that histologic recurrence was significantly lower with STC(OR:0.04,95%CI:0.01-0.28,P<0.00001,I^(2)=78%).Overall symptom recurrence was similar between groups(OR:0.23,95%CI:0.02-3.54,P=0.29,I^(2)=92%).On sensitivity analysis,symptom recurrence was significantly lower in the STC group(OR:0.05,95%CI:0.02-0.17,P=0.00001,I^(2)=39%).Odds of repeat dilation were significantly lower in the STC group(OR:0.14,95%CI:0.02-0.91,P=0.04,I^(2)=0%).Candida infection rates were similar between groups(OR:6.13,95%CI:0.85-44.26,P=0.07,I^(2)=24%).Proportion of concomitant PPI use was similar between groups(OR:1.64,95%CI:0.83-3.21,P=0.15,I^(2)=0%).CONCLUSION For patients who successfully achieved remission of EoE with STC induction therapy,maintaining treatment is effective in sustaining histologic remission,while newer regimens may be effective in preventing symptom recurrence compared to placebo.We found no significant difference for oropharyngeal/esophageal candidiasis with STC maintenance therapy.Future studies with longer follow-up periods are needed.展开更多
BACKGROUND Erythrodermic psoriasis(EP)is a rare and life-threatening form of psoriasis associated with significant morbidity and mortality.Systemic immunosuppre-ssive therapies are often required but may predispose to...BACKGROUND Erythrodermic psoriasis(EP)is a rare and life-threatening form of psoriasis associated with significant morbidity and mortality.Systemic immunosuppre-ssive therapies are often required but may predispose to opportunistic infections.Disseminated herpes simplex virus type-1(HSV-1)is an unusual complication in otherwise immunocompetent patients and has not been reported in association with ixekizumab therapy for EP.CASE SUMMARY We describe a 49-year-old man with longstanding severe plaque psoriasis,liver cirrhosis,and bipolar disorder who developed EP involving>90%of body surface area[Psoriasis Area and Severity Index(PASI)45].Following initial stabil-ization,he was admitted to the intensive care unit(ICU)with hemodynamic instability,leukocytosis with eosinophilia,and diffuse desquamation.Ixekizumab was initiated with high-dose topical clobetasol.During his ICU stay,he developed recurrent bacteremias and neurologic decline(Glasgow Coma Scale 7/15),fo-llowed by the appearance of widespread vesicles and hemorrhagic crusts.HSV-1 infection was confirmed by polymerase chain reaction(PCR).Immunosuppressive therapy was withheld,and intravenous acyclovir was started,leading to progre-ssive improvement.After ten days,ixekizumab was reintroduced with careful monitoring,resulting in marked clinical improvement(PASI 9.7 at six weeks).The patient remained stable on long-term follow-up with oral acyclovir prophylaxis.CONCLUSION This case highlights the diagnostic and therapeutic challenges of managing EP in the setting of biologic therapy.Disseminated cutaneous HSV-1 should be considered in immunosuppressed patients presenting with new vesicular eruptions,and prompt PCR testing with early antiviral therapy is essential.A multidisciplinary approach is critical to balance immunosuppression for disease control with infection risk.展开更多

This study investigates the application of large language models in analyzing sentiment features within the exchange rate markets.Traditional natural language processing methods,such as LDA and BERT,are effective in e...This study investigates the application of large language models in analyzing sentiment features within the exchange rate markets.Traditional natural language processing methods,such as LDA and BERT,are effective in extracting topics from text;however,they fail to assess the relative importance of these topics in relation to target exchange rates.To bridge this gap,this paper employs ChatGPT to extract topics from texts and evaluate their importance scores,further enhancing exchange rate forecasting by integrating topic importance into the sentiment analysis framework.Through empirical analysis,the superiority of ChatGPT over LDA and BERT in both topic extraction and importance assessment is demonstrated.Furthermore,this study utilizes the topic importance scores generated by ChatGPT to develop a novel interval-valued sentiment index(TIS index).This index not only accounts for the relative importance of various events influencing exchange rate fluctuations but also captures the dynamic evolution of market sentiment within an interval.Empirical results highlight that the TIS Index significantly enhances the forecasting accuracy of interval models such as TARI and IMLP for exchange rates.These findings further demonstrate the advantages of ChatGPT in sentiment analysis within the foreign exchange market.These findings offer new insights into the application of ChatGPT in financial text research.展开更多

Background:Corneal scarring following bacterial keratitis,particularly from Pseudomonas infections,poses significant challenges in ophthalmic care.Current treatments often fall short in effectively reducing corneal ha...Background:Corneal scarring following bacterial keratitis,particularly from Pseudomonas infections,poses significant challenges in ophthalmic care.Current treatments often fall short in effectively reducing corneal haze and restoring vision.To our knowledge,this is the first report documenting the use of topical losartan,an angiotensin II receptor antagonist known to inhibit the transforming growth factor-β(TGF-β)pathway,for treating corneal haze resulting from bacterial keratitis.Case Description:A 30-year-old male presented with a persistent corneal scar in his right eye,178 days post-Pseudomonas keratitis.Despite a prolonged course of topical corticosteroids,his best-corrected visual acuity(BCVA)stabilized at 20/40 with a hybrid contact lens over a 2-month period.Given the lack of improvement,we initiated treatment with topical losartan at a concentration of 0.8 mg/mL,administered six times daily.After 4 months of therapy,the patient’s BCVA improved to 20/25.Slit-lamp examination and corneal tomography revealed a significant reduction in corneal haze,indicating a positive response to the treatment.Conclusions:This case suggests that topical losartan may be a promising therapeutic option for reducing corneal opacity following bacterial keratitis by inhibiting the TGF-βpathway.However,further clinical studies are necessary to confirm its efficacy and safety in broader patient populations.展开更多

Modules enable students to engage with content at their own pace,fostering autonomy and deeper understanding.The modular approach ensures clarity in presenting objectives,instructions,and concepts,while having illustr...Modules enable students to engage with content at their own pace,fostering autonomy and deeper understanding.The modular approach ensures clarity in presenting objectives,instructions,and concepts,while having illustrations,activities,and assessments could enhance comprehension and retention.This paper was a developmental study on STS module for college students using the ADDIE Model(Analysis,Design,Development,Implementation,and Evaluation).Sampled 673 first-year students from Northwest Samar State University participated in the study,with 299 participating in a test try-out and 374 in the students’performance evaluation.Three expert evaluators with backgrounds in science,English,and psychology,each with over four years of experience,assessed the modules to ensure alignment with the study’s constructivist learning goals and instructional integrity.The findings revealed that both students and experts had rated the instructional module positively,indicating its effectiveness in facilitating learning and completing lessons.Key aspects such as the style of illustrations and written expressions,the usefulness of learning activities,and the guidance provided by illustrations and captions were especially well-received.The module was praised for its clear objectives,understandable instructions,and engaging tasks like trivia and puzzles.Expert evaluations highlighted relevance,simplicity,and balanced emphasis on topics in the module content.Furthermore,students in test group demonstrated significant improvement in performance,with post-test scores notably higher than pre-test scores,confirming the module’s effectiveness in enhancing learning outcomes.Consequently,this paper provides an opportunity to integrate science learning with initiatives aimed at promoting environmental preservation and driving social change.展开更多

文摘Purpose:The study examines the synergy and hysteresis in the evolution of funding and its supported literature,depicts their temporal correlation mechanism,which aids in improving trend predictions.Design/methodology/approach:The study uses the LDA model to identify topics in funding texts and supported papers.A cosine similarity algorithm was employed to estimate the nexus between topics and construct the topic evolution time series.Similarly,the hysteresis effect in topic evolution is analyzed based on topic popularity and content,leading to insights into their temporal correlation mechanism.Findings:The study finds that fund and sponsored paper topics exhibit strong collaboration with a noticeable lag in evolution.The fund topics significantly influence sponsored paper topics after a two-year lag.Moreover,the lag effect is inversely proportional to the topic’s similarity.Research limitations:We use the LDA model to determine the hysteresis effect in topic evolution despite its limitations in handling long-tail words and domain-specific vocabulary.Furthermore,the timing of the emergence of the focal topic in funds is undermined,affecting the findings.Practical implications:These findings enhance the accuracy and scientific validity of trend prediction.Estimating and identifying patterns can help technology managers anticipate future research hotspots,supporting informed decision-making and technology management.Originality/value:This study introduces a research framework to quantitatively and visually analyze the hysteresis effect,revealing the correlation and evolutionary patterns between fund research topics and their funded papers.
文摘BACKGROUND Eosinophilic esophagitis(EoE)is a chronic inflammatory disorder presenting as symptoms of dysphagia,esophageal food impaction,chest pain,and heartburn.After an initial trial of proton pump inhibitor(PPI)therapy,swallowed topical corticosteroids(STC)are effective as induction therapy for EoE.However,out-come data for STC as a maintenance strategy is limited.RESULTS Three randomized control trials and one observational study were included,involving 303 patients(189 in the STC group,114 in the placebo-controlled group).Analysis showed that histologic recurrence was significantly lower with STC(OR:0.04,95%CI:0.01-0.28,P<0.00001,I^(2)=78%).Overall symptom recurrence was similar between groups(OR:0.23,95%CI:0.02-3.54,P=0.29,I^(2)=92%).On sensitivity analysis,symptom recurrence was significantly lower in the STC group(OR:0.05,95%CI:0.02-0.17,P=0.00001,I^(2)=39%).Odds of repeat dilation were significantly lower in the STC group(OR:0.14,95%CI:0.02-0.91,P=0.04,I^(2)=0%).Candida infection rates were similar between groups(OR:6.13,95%CI:0.85-44.26,P=0.07,I^(2)=24%).Proportion of concomitant PPI use was similar between groups(OR:1.64,95%CI:0.83-3.21,P=0.15,I^(2)=0%).CONCLUSION For patients who successfully achieved remission of EoE with STC induction therapy,maintaining treatment is effective in sustaining histologic remission,while newer regimens may be effective in preventing symptom recurrence compared to placebo.We found no significant difference for oropharyngeal/esophageal candidiasis with STC maintenance therapy.Future studies with longer follow-up periods are needed.
文摘BACKGROUND Erythrodermic psoriasis(EP)is a rare and life-threatening form of psoriasis associated with significant morbidity and mortality.Systemic immunosuppre-ssive therapies are often required but may predispose to opportunistic infections.Disseminated herpes simplex virus type-1(HSV-1)is an unusual complication in otherwise immunocompetent patients and has not been reported in association with ixekizumab therapy for EP.CASE SUMMARY We describe a 49-year-old man with longstanding severe plaque psoriasis,liver cirrhosis,and bipolar disorder who developed EP involving>90%of body surface area[Psoriasis Area and Severity Index(PASI)45].Following initial stabil-ization,he was admitted to the intensive care unit(ICU)with hemodynamic instability,leukocytosis with eosinophilia,and diffuse desquamation.Ixekizumab was initiated with high-dose topical clobetasol.During his ICU stay,he developed recurrent bacteremias and neurologic decline(Glasgow Coma Scale 7/15),fo-llowed by the appearance of widespread vesicles and hemorrhagic crusts.HSV-1 infection was confirmed by polymerase chain reaction(PCR).Immunosuppressive therapy was withheld,and intravenous acyclovir was started,leading to progre-ssive improvement.After ten days,ixekizumab was reintroduced with careful monitoring,resulting in marked clinical improvement(PASI 9.7 at six weeks).The patient remained stable on long-term follow-up with oral acyclovir prophylaxis.CONCLUSION This case highlights the diagnostic and therapeutic challenges of managing EP in the setting of biologic therapy.Disseminated cutaneous HSV-1 should be considered in immunosuppressed patients presenting with new vesicular eruptions,and prompt PCR testing with early antiviral therapy is essential.A multidisciplinary approach is critical to balance immunosuppression for disease control with infection risk.

文摘As blockchain technology advances,non-fungible tokens(NFTs)are emerging as unconventional assets in the commercial market.However,it is necessary to establish a comprehensive NFT ecosystem that addresses the prevailing public concerns.This study aimed to bridge this gap by analyzing user-generated content on prominent social media platforms such as Twitter,Weibo,and Reddit.Employing text clustering and topic modeling techniques,such as Latent Dirichlet Allocation,we constructed an analytical framework to delve into the intricacies of the NFT ecosystem.Our investigation revealed seven distinct topics from Twitter and Reddit data and eight topics from Weibo data.Weibo users predominantly engaged in reviews and critiques,whereas Twitter and Reddit users emphasized personal experiences and perceptions.The NFT ecosystem encompasses several crucial elements,including transactions,customers,infrastructure,products,environments,and perceptions.By identifying the prevailing trends and common issues,this study offers valuable guidance for the development of NFT ecosystems.

文摘This study investigates the application of large language models in analyzing sentiment features within the exchange rate markets.Traditional natural language processing methods,such as LDA and BERT,are effective in extracting topics from text;however,they fail to assess the relative importance of these topics in relation to target exchange rates.To bridge this gap,this paper employs ChatGPT to extract topics from texts and evaluate their importance scores,further enhancing exchange rate forecasting by integrating topic importance into the sentiment analysis framework.Through empirical analysis,the superiority of ChatGPT over LDA and BERT in both topic extraction and importance assessment is demonstrated.Furthermore,this study utilizes the topic importance scores generated by ChatGPT to develop a novel interval-valued sentiment index(TIS index).This index not only accounts for the relative importance of various events influencing exchange rate fluctuations but also captures the dynamic evolution of market sentiment within an interval.Empirical results highlight that the TIS Index significantly enhances the forecasting accuracy of interval models such as TARI and IMLP for exchange rates.These findings further demonstrate the advantages of ChatGPT in sentiment analysis within the foreign exchange market.These findings offer new insights into the application of ChatGPT in financial text research.

文摘Background:Corneal scarring following bacterial keratitis,particularly from Pseudomonas infections,poses significant challenges in ophthalmic care.Current treatments often fall short in effectively reducing corneal haze and restoring vision.To our knowledge,this is the first report documenting the use of topical losartan,an angiotensin II receptor antagonist known to inhibit the transforming growth factor-β(TGF-β)pathway,for treating corneal haze resulting from bacterial keratitis.Case Description:A 30-year-old male presented with a persistent corneal scar in his right eye,178 days post-Pseudomonas keratitis.Despite a prolonged course of topical corticosteroids,his best-corrected visual acuity(BCVA)stabilized at 20/40 with a hybrid contact lens over a 2-month period.Given the lack of improvement,we initiated treatment with topical losartan at a concentration of 0.8 mg/mL,administered six times daily.After 4 months of therapy,the patient’s BCVA improved to 20/25.Slit-lamp examination and corneal tomography revealed a significant reduction in corneal haze,indicating a positive response to the treatment.Conclusions:This case suggests that topical losartan may be a promising therapeutic option for reducing corneal opacity following bacterial keratitis by inhibiting the TGF-βpathway.However,further clinical studies are necessary to confirm its efficacy and safety in broader patient populations.

文摘Modules enable students to engage with content at their own pace,fostering autonomy and deeper understanding.The modular approach ensures clarity in presenting objectives,instructions,and concepts,while having illustrations,activities,and assessments could enhance comprehension and retention.This paper was a developmental study on STS module for college students using the ADDIE Model(Analysis,Design,Development,Implementation,and Evaluation).Sampled 673 first-year students from Northwest Samar State University participated in the study,with 299 participating in a test try-out and 374 in the students’performance evaluation.Three expert evaluators with backgrounds in science,English,and psychology,each with over four years of experience,assessed the modules to ensure alignment with the study’s constructivist learning goals and instructional integrity.The findings revealed that both students and experts had rated the instructional module positively,indicating its effectiveness in facilitating learning and completing lessons.Key aspects such as the style of illustrations and written expressions,the usefulness of learning activities,and the guidance provided by illustrations and captions were especially well-received.The module was praised for its clear objectives,understandable instructions,and engaging tasks like trivia and puzzles.Expert evaluations highlighted relevance,simplicity,and balanced emphasis on topics in the module content.Furthermore,students in test group demonstrated significant improvement in performance,with post-test scores notably higher than pre-test scores,confirming the module’s effectiveness in enhancing learning outcomes.Consequently,this paper provides an opportunity to integrate science learning with initiatives aimed at promoting environmental preservation and driving social change.

文摘With the popularization of social media,public opi-nion information on emergencies spreads rapidly on the Internet,the impact of negative public opinions on an event has become more significant.Based on the organizational form of public opinion information,the knowledge graph is used to construct the knowledge base of public opinion risk cases on the emer-gency network.The emotion recognition model of negative pub-lic opinion information based on the bi-directional long short-term memory(BiLSTM)network is studied in the model layer design,and a linear discriminant analysis(LDA)topic extraction method combined with association rules is proposed to extract and mine the semantics of negative public opinion topics to real-ize further in-depth analysis of information topics.Focusing on public health emergencies,knowledge acquisition and knowl-edge processing of public opinion information are conducted,and the experimental results show that the knowledge graph framework based on the construction can facilitate in-depth theme evolution analysis of public opinion events,thus demon-strating important research significance for reducing online pub-lic opinion risks.
文摘This paper conducts topic mining and analysis of research literature in the domestic smart library field based on the BERTopic model,aiming to reveal its topic development context and evolution trends.Journal literature in the smart library field collected by CNKI(China National Knowledge Infrastructure)from 2015 to 2024 was analyzed using the BERTopic model and dynamic topic modeling for topic mining and evolution trend analysis.The study found that the domestic smart library field involves multiple core topics,identifying a diversified topic structure centered around“data”,“user”,“5g”,etc.The research results provide data support and practical reference for libraries to accurately identify key points of technology integration during smart transformation and to optimize smart service models.
文摘This study investigates how universities are developing normativeframeworks to regulate the use of generative AI tools in higher education, with aparticular focus on balancing empowerment and discipline. Drawing on theoreticallenses such as Foucault’s discipline theory and contemporary AI ethics, the paperanalyzes policy documents from institutions including Harvard, Oxford, and severaltop Chinese universities. Using Latent Dirichlet Allocation (LDA) topic modeling,the study reveals five dominant governance themes—ranging from academicintegrity enforcement to pedagogical empowerment. The findings highlight a globalshift from restrictive to balanced, ethics-informed AI governance, with significantdisciplinary variations. The paper concludes by proposing a “principledpermissiveness” model that combines transparency, accountability, andpedagogical innovation in future AI governance.
