TIPS联合肠系膜下静脉弹簧圈栓塞治疗肝硬化患者复发性痔出血:1例报道

痔疮是目前最常见的疾病之一,常伴出血、脱垂、疼痛和瘙痒。目前,痔疮治疗以非手术或保守治疗为主,例如套扎、硬化治疗等。然而,肝硬化合并内痔出血尚无标准的治疗方案。1例68岁的肝硬化门静脉高压症女性患者在接受两次内镜硬化治疗后,... 痔疮是目前最常见的疾病之一,常伴出血、脱垂、疼痛和瘙痒。目前,痔疮治疗以非手术或保守治疗为主,例如套扎、硬化治疗等。然而,肝硬化合并内痔出血尚无标准的治疗方案。1例68岁的肝硬化门静脉高压症女性患者在接受两次内镜硬化治疗后,仍因“反复便血4个月”入院。通过腹盆的增强计算机断层扫描(computed tomography, CT)和结肠镜检查,我们发现这位肝硬化患者的内痔静脉曲张与门脉高压之间存在相关性。因此,我们考虑选择经颈静脉肝内门体分流术(transjugular intrahepatic portosystemic shunt, TIPS)作为该患者的治疗手段。术中造影发现肠系膜上静脉内为完全的远肝血流。术中完成TIPS+门静脉血栓抽吸+弹簧圈肠系膜下静脉栓塞,门静脉压力从27 mmHg降至11 mmHg。在后续3年的随访期间,患者未出现任何消化道出血,血红蛋白水平也显著回升。本病例报告表明,如果内镜治疗失败,并且排除了其他出血原因,TIPS可以作为肝硬化门脉高压患者内痔出血的替代治疗措施。 展开更多

Impact of multiscale flow structures on mixing and losses in turbine blade tip region

Multiscale mixing of the turbine blade tip leakage and mainstream flows causes considerable aerodynamic loss.Understanding it is crucial to correctly estimating the mixing loss and thus improving the turbine's performance.The multiscale mixing phenomenon in a typical high-pressure turbine rotor flow was studied in this work.The contributions of various scale flows to entropy production and mixing properties were identified.The corresponding physical mechanisms at different scales were explored.It is shown that the large-scale and time-averaged flow contributions to mixing are significant,accounting for approximately 37.1% and 25% of the total.Time-averaged and large-scale flows cause the majority of the fluid deformation of the material surface,while mesoand small-scale flows just generate finer deformations.It raises the area stretch coefficient and the virtual concentration gradient.Thus,mixing is enhanced.Furthermore,time-averaged and large-scale flows account for the majority of the losses in the upstream and downstream regions of the blade tip respectively,accounting for approximately 53.8%and 33.5%of the total.The sheet-like structures—rather than the tip leaking vortex—are the primary source of the loss.High-dissipation regions are produced by the sheet-like structures via the pressure Hessian term and the self-amplification terms. 展开更多

Diplodia tip blight,caused by Diplodia sapinea,is a global pine necrotic disease causing heavy economic losses to the pine industry.Chemical control,its main current management,easily induces pathogen resistance and environmental pollution,which biological control avoids.This study investigated juniper essential oil’s efficacy against the disease on Mongolian Scots pine(Pinus sylvestris var.mongolica)and its induced resistance mechanisms via pot experiments,physiological assays(defense enzyme activities,resistant substances)and metabolomic sequencing(secondary metabolites).Results showed varied efficacy:three foliar sprays of 10μL mL^(-1) oil achieved the best control phenylalanine ammonia-lyase(PAL),polyphenol oxidase(82.9%).The 20μL mL^(-1) treatment significantly increased(PPO)activities,and contents of lignin,flavonoids and total phenolics.Metabolomic analysis showed 326 upregulated and 527 downregulated different metabolites in essential oilinduced and pathogen-inoculated pines,compared to 483 upregulated and 277 downregulated metabolites in noninduced but inoculated pines.The differentially expressed metabolites in treated pines were primarily enriched in pathways related to amino acid metabolism and plant secondary metabolite biosynthesis,with notably increased expression levels of ferulic acid,scopoletin,pipecolic acid,D-proline,and DL-arginine.Therefore,juniper essential oil protects against D.sapinea by inducing systemic acquired resistance in Mongolian Scots pine.In conclusion,juniper essential oil controls D.sapinea by inducing systemic acquired resistance(SAR)in Mongolian Scots pine,clarifying the molecular mechanism and supporting biological control of the disease. 展开更多

Validation of time-space collocation method for simulating asymmetric unsteady flows in eccentric compressors

Circumferentially non-uniform tip clearances induced by rotor eccentricity significantly affect the overall performance of axial compressors,particularly the stability margin.Currently,Computational Fluid Dynamics(CFD)plays a crucial role in the aerodynamic analysis of eccentric compressors.However,conventional full-annulus Unsteady Reynolds-Averaged Navier-Stokes(URANS)simulations are prohibitively expensive for routine design and analysis purposes.To address this issue,the paper presents a novel Fourier-based method,called the Time-Space Collocation(TSC)method,for efficient simulations of eccentric compressors.This method coherently treats temporal and spatial harmonics,making it well-suited to tackle the rotor eccentricity problem,as the perturbation waves induced by eccentricity are time-periodic with respect to the rotor and space-periodic with respect to the stator.Three numerical cases,including NASA Rotor 67,original Stage 67,and Stage 67 with a reduced rotor-stator axial gap,were conducted to verify the effectiveness of the TSC method.The results indicate that,for the rotor eccentricity levels studied in this paper,the influence of weak rotor-stator interactions can be disregarded in the original Stage 67.In this situation,applying three harmonics can accurately capture both the performance variations and the non-uniformly distributed flowfields of eccentric compressors,while achieving a reduction in run time by two orders of magnitude compared to full-annulus URANS simulations.However,in Stage 67 with a reduced rotor-stator axial gap,the results that include rotor-stator interactions align much more closely with the URANS results.Nevertheless,the TSC simulations can still achieve speed-ups of several dozen times.Overall,the TSC method shows promising potential for application within the engineering community. 展开更多

目的:探讨改良尿道板纵切卷管尿道成形术(TIP)治疗尿道下裂的临床价值。方法:选取尿道下裂患儿24例。根据不同手术方式分为观察组(13例)和对照组(11例)。观察组采用改良TIP术;对照组行传统TIP术。比较两组一般资料及围手术期指标。对患... 目的:探讨改良尿道板纵切卷管尿道成形术(TIP)治疗尿道下裂的临床价值。方法:选取尿道下裂患儿24例。根据不同手术方式分为观察组(13例)和对照组(11例)。观察组采用改良TIP术;对照组行传统TIP术。比较两组一般资料及围手术期指标。对患者术后进行随访,比较两组术后并发症发生情况、手术成功率、术后排尿功能及阴茎外观评分。结果:两组一般资料及围手术期指标比较差异无统计学意义(均P>0.05)。对照组尿瘘、尿道狭窄、阴茎头裂开发生率高于观察组,手术成功率低于观察组(均P<0.05)。与对照组比较,观察组术后最大尿流率升高,排尿时间缩短(均P<0.05)。观察组术后尿道口位置、阴茎整体外观评分高于对照组(均P>0.05)。结论:改良TIP术式治疗尿道下裂患儿并发症发生率低,排尿功能较好,且手术成功率及外观满意度较高。 展开更多

目的评估患者和手术相关因素对经颈静脉肝内门体分流术(TIPS)的累积透视时间,手术时长,总剂量面积乘积(DAP),累积剂量的影响。方法回顾性的收集2022年11月至2024年5月西安交通大学第一附属医院的125例经颈静脉肝内门体分流术(TIPS)病例... 目的评估患者和手术相关因素对经颈静脉肝内门体分流术(TIPS)的累积透视时间,手术时长,总剂量面积乘积(DAP),累积剂量的影响。方法回顾性的收集2022年11月至2024年5月西安交通大学第一附属医院的125例经颈静脉肝内门体分流术(TIPS)病例。收集透视时间,手术时长,总剂量面积乘积(DAP)和累积剂量的数据资料,同时包括人口统计学,肝硬化相关症状,门脉情况及曲张静脉情况,以及术中程序的资料作为自变量,所有统计学均在SPSS统计软件中完成,P值小于0.05为有意义。结果共收集125例患者的信息,其中男性82名,女性43名,平均年龄52.44岁,平均BMI 22.69 kg/m^(2),平均手术时长80.32 min,平均透视时间25.77 min,平均总DAP 306.81 mGy·cm^(2),平均累积剂量775.67 mGy。66岁及以上(P=0.021),BMI>24.9(P=0.049),有先天性分流道(P<0.001),栓塞2条曲张静脉(P=0.002)是透视时间的独立影响因素。女性(P=0.048),有先天性分流道(P<0.001),栓塞2条曲张静脉(P=0.022)和栓塞4条曲张静脉(P=0.047),上午手术(P=0.003)及使用弹簧圈栓塞数量(2~3个)(P=0.025)是手术时长的独立影响因素。女性(P=0.002),66岁及以上(P=0.049),有先天性分流道(P=0.038),栓塞2条曲张静脉(P=0.004),门静脉及肠系膜上静脉血栓(P=0.012)是总DAP的独立影响因素。女性(P=0.002),66岁及以上(P=0.021),有先天性分流道(P=0.028),栓塞2条曲张静脉(P=0.004),门静脉及肠系膜上静脉血栓(P=0.008)是累积剂量的独立影响因素。结论TIPS术中辐射暴露受多种因素共同影响,其中年龄、性别、BMI、先天性分流道、曲张静脉数量及血栓情况为关键变量。透视时间与辐射剂量高度相关,可作为评估和监控暴露强度的简明指标。优化术前评估和术中操作,有助于有效控制患者和术者的辐射风险。 展开更多

CD8^(+)T细胞耗竭是导致HIV持续感染的重要因素,研究其对HIV感染的影响具有重要意义.研究构建了一个包含CD8^(+)T细胞耗竭动态演变过程的HIV病毒-免疫相互作用的时变动力学模型.首先分析了对应自治系统平衡点的存在性与稳定性,并通过数值分析揭示了系统的鞍结分岔与双稳态现象.随后,引入函数s(t)描述耗竭的非平稳发展,并对时变系统进行了数值模拟研究.结果表明:(1)CD8^(+)T细胞耗竭的发展是驱动HIV感染的疾病进展的关键因素;(2)更快的耗竭发展速率会显著加速HIV感染的疾病进展;(3)耗竭的发展通过诱发B-tipping现象这一状态突变机制,导致病毒载量急剧上升,促使疾病从低病毒载量的无症状期进展到高病毒载量的艾滋病期. 展开更多

Glucose molecules are of great significance being one of the most important molecules in metabolic chain.However,due to the small Raman scattering cross-section and weak/non-adsorption on bare metals,accurately obtaining their"fingerprint information"remains a huge obstacle.Herein,we developed a tip-enhanced Raman scattering(TERS)technique to address this challenge.Adopting an optical fiber radial vector mode internally illuminates the plasmonic fiber tip to effectively suppress the background noise while generating a strong electric-field enhanced tip hotspot.Furthermore,the tip hotspot approaching the glucose molecules was manipulated via the shear-force feedback to provide more freedom for selecting substrates.Consequently,our TERS technique achieves the visualization of all Raman modes of glucose molecules within spectral window of 400-3200 cm^(-1),which is not achievable through the far-field/surface-enhanced Raman,or the existing TERS techniques.Our TERS technique offers a powerful tool for accurately identifying Raman scattering of molecules,paving the way for biomolecular analysis. 展开更多

Low-electrode capacitive deionization(FCDI)is an emerging desalination technology with great potential for removal and/or recycling ions from a range of waters.However,it still suffers from inefficient charge transfer and ion transport kinetics due to weak turbulence and low electric intensity in flow electrodes,both restricted by the current collectors.Herein,a new tip-array current collector(designated as T-CC)was developed to replace the conventional planar current collectors,which intensifies both the charge transfer and ion transport significantly.The effects of tip arrays on flow and electric fields were studied by both computational simulations and electrochemical impedance spectroscopy,which revealed the reduction of ion transport barrier,charge transport barrier and internal resistance.With the voltage increased from 1.0 to 1.5 and 2.0 V,the T-CC-based FCDI system(T-FCDI)exhibited average salt removal rates(ASRR)of 0.18,0.50,and 0.89μmol cm^(-2) min^(-1),respectively,which are 1.82,2.65,and 2.48 folds higher than that of the conventional serpentine current collectors,and 1.48,1.67,and 1.49 folds higher than that of the planar current collectors.Meanwhile,with the solid content in flow electrodes increased from 1 to 5 wt%,the ASRR for T-FCDI increased from 0.29 to 0.50μmol cm^(-2) min^(-1),which are 1.70 and 1.67 folds higher than that of the planar current collectors.Additionally,a salt removal efficiency of 99.89%was achieved with T-FCDI and the charge efficiency remained above 95%after 24 h of operation,thus showing its superior long-term stability. 展开更多

针对IOTA网络中的节点恶意攻击,提出了一种基于节点信誉值的共识机制RV_IOTA。在RV_IOTA中引入时间衰减的动态信誉值系统,即根据历史交易表现调整节点的信誉值,若交易有效则提升信誉值,若交易冲突(如双花攻击)则降低信誉值,从而限制恶... 针对IOTA网络中的节点恶意攻击,提出了一种基于节点信誉值的共识机制RV_IOTA。在RV_IOTA中引入时间衰减的动态信誉值系统,即根据历史交易表现调整节点的信誉值,若交易有效则提升信誉值,若交易冲突(如双花攻击)则降低信誉值,从而限制恶意节点的影响力。RV_IOTA根据节点信誉值优化选择Tips算法,根据信誉值和交易累积权重调整Tips的被引用概率,使高信誉节点发布的交易更容易被验证。本文提出的机制有效抑制了网络初期的双花攻击,降低了攻击的成功率,并限制了恶意节点的交易投放能力,促使诚实节点主导共识过程,保证了网络的稳健性和安全性。实验结果表明,在500节点规模下,RV_IOTA实现39 TPS的吞吐量,较传统IOTA提升了15%,同时高信誉节点的交易确认时延缩短至1.2 s。通过将Tips选择验证范围从全局缩减至邻域,算法复杂度也得到了降低,在仅增加25%内存开销的情况下,所提机制为物联网应用提供了高效可靠的去中心化解决方案。 展开更多

全溶液法制备PAA/SU-8双层电介质OFET及其性能

有机场效应晶体管(OFET)因其低成本和优异的可兼容特性,是柔性电子领域的重要器件。通过构建聚丙烯酸(PAA)/SU-8双层介电结构,结合全溶液法制备了基于6,13-双(三异丙基硅烷基乙炔基)(TIPS)-并五苯半导体层的OFET。通过原子力显微镜(AFM... 有机场效应晶体管(OFET)因其低成本和优异的可兼容特性,是柔性电子领域的重要器件。通过构建聚丙烯酸(PAA)/SU-8双层介电结构,结合全溶液法制备了基于6,13-双(三异丙基硅烷基乙炔基)(TIPS)-并五苯半导体层的OFET。通过原子力显微镜(AFM)和接触角测量仪分析表面特性,PAA/SU-8双层薄膜的均方根(RMS)粗糙度(0.284 nm)较PAA单层薄膜(2.77 nm)显著降低,表面接触角(83°)接近SU-8单层薄膜(85°)且明显高于PAA单层薄膜(60°)。采用傅里叶变换红外(FTIR)光谱和LCR测试仪分析不同介电层的界面极性和介电性能,揭示了双介电层通过高电容与低极性界面的协同作用,有效改善了器件的电荷传输效率与稳定性。对PAA/SU-8双层结构OFET的性能进行测试,结果表明,PAA/SU-8双层器件表现出低阈值电压(1.0 V)、高电流开关比(>10^(4))、高迁移率(0.244 cm^(2)·V^(-1)·s^(-1))和良好的亚阈值摆幅(2.5 V/dec),显著优于SU-8单层器件。 展开更多

Climatic and edaphic controls of root-tip production and mortality in five temperate tree species

Root tips are the main components of absorptive fine roots,but their seasonal dynamics and relationship to environmental factors remain unclear due to the difficulties in methodology.In this study,we explored the temporal patterns of root-tip production and mortality in monoculture plantations of five temperate tree species at a common site in northeastern China,and identified the general environmental controls on such processes.We made monthly in-situ assessments of root tip length(RTL)production and mortality in two hardwood and three coniferous species with a minirhizotron(MR)method during the growing seasons of 2008 and 2009.Air temperature,rainfall,soil temperature and water content at 10 cm depth were determined concurrently.RTL production in all species exhibited consistent peaks in summer(June–August)in two growing seasons.RTL mortality showed substantial interannual and interspecific variability,with peaks in autumn and winter in 2008,but various patterns in 2009.RTL production positively correlated with monthly soil and air temperature across all species,and with monthly rainfall in three coniferous species.However,there was no significant correlation between RTL production and soil water content.By contrast,RTL mortality was weakly related to environmental factors,showing positive correlations with soil temperature in Korean spruce,and with rainfall in Korean pine and Korean spruce.Our findings suggest that the seasonal patterns of RTL production are convergent across the five temperate tree species due to the overlapped distribution of heat and rainfall,which can conduce roots to maximizing the acquisition of nutrient resources in the soil. 展开更多
