Objective:To investigate the differential effects of different rivaroxaban dosing regimens on symptom relief,fluctuations in laboratory parameters,and medication safety in patients with stable pulmonary embolism(PE).M...Objective:To investigate the differential effects of different rivaroxaban dosing regimens on symptom relief,fluctuations in laboratory parameters,and medication safety in patients with stable pulmonary embolism(PE).Methods:This study enrolled 100 patients in the maintenance phase of PE who were treated at our hospital between January 2022 and December 2023.They were randomly divided into a control group and an observation group using a random number table,with 50 subjects in each group.The treatment period was uniformly set at 6 months.The control group received oral rivaroxaban 10 mg once daily,while the observation group received oral rivaroxaban 5 mg once daily.The study focused on comparing the two groups regarding the degree of clinical symptom relief,coagulation function parameters(including D-dimer levels,PT,and APTT),cardiac function markers(NT-proBNP),and drug-related adverse events.All data were processed using SPSS 26.0 statistical software.Measurement data are presented as mean±standard deviation,intergroup differences were verified by t-test,categorical variables were analyzed by chi-square test,and the statistical significance level was set at P<0.05.Results:After six months of treatment intervention,there was no significant difference in the overall relief of core clinical symptoms such as dyspnea and chest pain between the two groups.Regarding laboratory indicators,post-treatment D-dimer levels,prothrombin time,activated partial thromboplastin time,and NT-proBNP values were significantly optimized compared to baseline in both groups(P<0.05),but intergroup comparisons did not reach statistical significance.Notably,the overall incidence of bleeding events in the observation group was significantly lower than that in the control group(P<0.05),while there were no significant differences in the incidence rates of other adverse events between the two groups.Conclusion:In the maintenance phase treatment of pulmonary embolism,rivaroxaban 5 mg and 10 mg doses are equivalent in efficacy regarding improvement of clinical symptoms and blood indicators.However,the 5 mg dose significantly reduces the risk of bleeding,offers better safety,and is more suitable for long-term anticoagulation management in some high-risk populations.展开更多

However, MWS setting is a very complicate...The maintenance window scheme(MWS) is one of the most important railway transportation organizational plans and plays an important role in ensuring railway operational safety. However, MWS setting is a very complicated process, and most countries currently do so with the help of a computer-aided decision system. In general, a decision system can generate multiple alternatives for MWS within an acceptable time. Therefore, how to choose the best option from the alternatives is a vital decision. This paper presents a novel framework for MWS evaluation based on the Chinese railway system. Specifically, the requirements of each department related to MWS setting are analysed, and we construct an evaluation indicator system for MWS based on the preferences of different departments. Then, we apply the fuzzy soft set theory to MWS evaluation, a method that not only effectively deals with evaluation of uncertain information, but also gives flexibility for experts to input their subjective judgment.Additionally, using the ‘‘AND’’ operation of soft set theory allows combing evaluation information from multiple evaluators to give comprehensive results. Finally, a case study illustrates the proposed framework, showing that the proposed evaluation indicator system and evaluation method are effective and practical.展开更多
The amalgamation procedure is actual when using structural methods of reliability.This may apply as the case of the action of a complex of damaging processes on a structural element and the case of the action of a cer...The amalgamation procedure is actual when using structural methods of reliability.This may apply as the case of the action of a complex of damaging processes on a structural element and the case of the action of a certain damaging process on a system of elements.Classical methods of reliability are poorly adapted for powertrain mechanical systems with a series structural scheme of elements subjected to the gradual influence of several degradation processes.The problem of amalgamation of individual indicators is exacerbated at the stage of operation when diagnosing the technical state of the mechanical system.The application of the classical rule of amalgamation by multiplying the probabilities of survival leads to the effect of over-maintenance.New rules of amalgamation have been developed,which deprive the assessment of the reliability of excessive conservatism.The complex index of the technical condition is offered—the resource safety index(RSI).Its use determines the remaining lifetime.The algorithm for searching the RSI contains an assessment of the criticality of the failure.The search of RSI under the influence of the complex of damaging processes on the element of the powertrain system is demonstrated.The efficiency of the RSI method is shown by the example of the reliability assessment of aircraft bolts.Application of the RSI method increases the guaranteed lifetime by 4-10 times compared with traditional methods.展开更多
Purpose-Key performance indicators(KPIs)of maintenance systems serve as benchmarks to workers and organizations to compare their goals for decision-making purposes.Unfortunately,the effects of one KPI on the other are...Purpose-Key performance indicators(KPIs)of maintenance systems serve as benchmarks to workers and organizations to compare their goals for decision-making purposes.Unfortunately,the effects of one KPI on the other are least known,restraining decisions on prioritization of KPIs.This article examines and prioritizes the KPIs of the maintenance system in a food processing industry using the novel Taguchi(T)scheme-decisionmaking trial and evaluation laboratory(DEMATEL)method,Taguchi-Pareto(TP)scheme-DEMATEL method and the DEMATEL method.Design/methodology/approach-The causal association of maintenance process parameters(frequency of failure,downtime,MTTR,MTBF,availability and MTTF)was studied.Besides,the optimized maintenance parameters were infused into the DEMATEL method that translates the optimized values into cause and effect responses and keeping in view the result of analysis.Data collection was done from a food processing plant in Nigeria.Findings-The results indicated that downtime and availability have the most causal effects on other criteria when DEMATEL and T-DEMATEL methods were respectively applied to the problem.Furthermore,the frequency of failure is mostly affected by other criteria in the key performance indication selection using the two methods.The combined Taguchi scheme and DEMATEL method is appropriate to optimize and establish the causal relationships of factors.Originality/value-Hardly any studies have reported the joint optimization and causal relationship of maintenance system parameters.However,the current study achieves this goal using the T-DEMATEL,TP-DEMATEL and DEMATEL methods for the first time.The applied methods effectively ease decisions on prioritization of KPIs for enhancement.展开更多
