The workup of these patients includes blood chemistry and biomarkers, imaging and functional testing of the heart and lungs. A diagnosis of diastolic heart failure is inferred through the exclusion of systolic abnormalities, a normal pulmonary function test and normal hemoglobin, coupled with diastolic abnormalities on echocardiography. Differentiating confounders such as obesity or deconditioning in a patient with diastolic abnormalities is difficult. While the most recent guidelines provide more avenues for diagnosis, such as incorporating the left atrial size, little emphasis is given to understanding left atrial function, which contributes to at least 25% of diastolic left ventricular filling; additionally, exercise stress testing to elicit symptoms and test the dynamics of diastolic parameters, especially when access to the "gold standard" invasive tests is lacking, presents clinical translational gaps. It is thus important in diastolic heart failure work up to understand left atrial mechanics and the role of exercise testing to build a comprehensive argument for the diagnosis of diastolic heart failure in a patient presenting with SOB.展开更多

Mandatory testing is implemented in some Australian jurisdictions and not others, allowing for a comprehensive comparison of the outcomes. It is apparent that mandatory testing of HIV and STIs among sex workers in Australia has proven to be a barrier to otherwise successful HIV and STI peer education, prevention and free and anonymous testing and treatment. The outcomes of mandatory testing are counterproductive to reducing HIV and STI rates, do not reach the intended target group, are costly and inefficient, and mandatory testing has proven to be a very difficult policy to repeal once in place. Scarlet Alliance, the Australian Sex Workers Association, as well as numerous academics and policy leaders in Australia recommend against mandatory testing of HIV and STIs among sex workers.展开更多

The plastic flow instability map is established based on Gegel B and Murthy instability criteria because the deformed compression samples suggest that the combination of the above two instability criteria has more comprehensive crack prediction ability. And the processing map based on Dynamic Mate- rial Model (DMM) of as-cast 7075 aluminum alloy has been developed through a superposition of the established instability map and power dissipation map. In terms of microstructure of the deformed samples and whether plastic flow is stable or not, the processing map can be divided into five areas: stable area with as-cast grain, stable area with homogeneous grain resulting from dynamic recovery, instability area with as-cast grain, instability area with the second phase and instability area with mixed grains. In consideration of microstructure characteristics in the above five areas of the processing map, the stable area with homogeneous grain resulting from dynamic recovery, namely the temperatures at 425465 ℃ and the strain rates at 0.01^-1 s^-1, is suggested to be suitable processing window for the as-cast 7075 aluminum alloy.展开更多

The estimated interfacial fracture energies G are independent of the film thickness, the peel angle and the residual stress. The value of G for Ni films on a stainless steel substrate is about 5.47 ~ 6.08?N/m for various peel angles θ , while 5.33 ~ 6.72?N/m for Ni films on titanium substrate with various film thickness h . The effect of the residual stress on the peel strength P/b was also discussed.展开更多

The test is usually time-consuming and energy-consu- ming. And the accurate characteristic curves of the pump were hardly obtained due to a limited a- mount and discreteness of the test data. In order to simplify the test procedure and improve the test accuracy, a novel method for measuring hydraulic pump operating characteristic based on multi-ele- ment nonlinear regression (NLMR) modeling is proposed in this paper. The main idea of this model- ing method is establishing a mathematical model to predict the performance parameters of the hy- draulic pump, only a small amount of test data is needed. Consequently, the pump operating charac- teristics in any working conditions are obtained. And the test results of the pump are easily charac- terized in the graphs, charts, tables and so on. The evaluations of the model are carried out and dis- cussed in this paper. The result shows that the test error of the novel method can be controlled to be about 0. 1%. Compared with the traditional test method, the proposed method reduces greatly the test time and the random error of the test data, and improves the efficiency and accuracy of the pump test.展开更多

However, high discharge in river is rare. Physical model tests at high discharges will probably lead biased hydrological relationship. To improve hydrological relationship at low discharges, in this study, we considered the diversion rating curve of the Yuanshanzi Diversion Work. The 1/100-scaled physical model tests at low and high discharges (90 - 1620 m3/s) were performed and coupled the diversion discharges of 5 flood events (2009-2010) in field. The official diversion rating curve was built only based on physical model tests at high discharges (837 - 1620 m3/s). The results of physical model tests in this study suggested the official diversion rating curve should be modified considering all tests. The modifications showed the official diversion rating curve was over-estimated. A complete series of physical model tests and considering field situations, in this study, indicated expanded physical model tests and constantly field measurements were therefore necessary for hydraulic structure, which provided information to modify used hydrological relationship to fit real situations.展开更多

In this paper, a smartphone-based warning system was proposed to enhance workers’ safety in work zones. Three different warning message systems, including sound, male voice and female voice, were designed to alert drivers. Twenty-four subjects were recruited for a total of ninety-six rounds driving simulator test in a work zone to investigate the impacts of smartphone-based warning messages on subjects’ driving performance, in terms of driving speed, speed variation, acceleration, and brake reaction distance. The outcome showed that, with the assistance of the sound and voice (either female or male) warning messages, drivers could effectively reduce their accelerations and speeds. Meanwhile, such a warning system can induce subjects to shorten their brake reaction distances for worker crossings. All participants found that this warning system is applicable to enhance their defensive driving behaviors while driving through a work zone.展开更多
