In recent years,large vision-language models(VLMs)have achieved significant breakthroughs in cross-modal understanding and generation.However,the safety issues arising from their multimodal interactions become promine...In recent years,large vision-language models(VLMs)have achieved significant breakthroughs in cross-modal understanding and generation.However,the safety issues arising from their multimodal interactions become prominent.VLMs are vulnerable to jailbreak attacks,where attackers craft carefully designed prompts to bypass safety mechanisms,leading them to generate harmful content.To address this,we investigate the alignment between visual inputs and task execution,uncovering locality defects and attention biases in VLMs.Based on these findings,we propose VOTI,a novel jailbreak framework leveraging visual obfuscation and task induction.VOTI subtly embeds malicious keywords within neutral image layouts to evade detection,and breaks down harmful queries into a sequence of subtasks.This approach disperses malicious intent across modalities,exploiting VLMs’over-reliance on local visual cues and their fragility in multi-step reasoning to bypass global safety mechanisms.Implemented as an automated framework,VOTI integrates large language models as red-team assistants to generate and iteratively optimize jailbreak strategies.Extensive experiments across seven mainstream VLMs demonstrate VOTI’s effectiveness,achieving a 73.46%attack success rate on GPT-4o-mini.These results reveal critical vulnerabilities in VLMs,highlighting the urgent need for improving robust defenses and multimodal alignment.展开更多

Methods: A total of 200 patients who used intravenous indwelling needles during hosp...Objective: To explore the effectiveness of applying an infusion project team in the safety risk management of indwelling needle use. Methods: A total of 200 patients who used intravenous indwelling needles during hospital treatment from July to October 2022 were selected and randomly divided into an observation group and a control group, with 100 patients in each group. Patients in the control group received routine nursing methods, while those in the observation group were managed using the safety risk management method of the infusion project team. The tube blocking rate and tube removal rate were compared between the two groups. Results: The one-time puncture success rate and the standard implementation rate of intravenous indwelling needle use in the observation group were significantly higher than those in the control group. The total incidence of complications in the observation group was significantly lower than that in the control group, and patient satisfaction in the observation group was notably higher, with statistically significant differences (P < 0.05). Conclusion: The application of an infusion project team in the safety risk management of indwelling needle use is effective and has positive impacts on improving the one-time puncture success rate, the standard implementation rate of intravenous indwelling needle use, and patient satisfaction. It is a practice worth promoting in clinical settings.展开更多

Pulmonary embolism(PE)represents the third leading cause of cardiovascular death,despite the implementation of European Society of Cardiology guidelines,the establishment of PE response teams and advances in diagnosis...Pulmonary embolism(PE)represents the third leading cause of cardiovascular death,despite the implementation of European Society of Cardiology guidelines,the establishment of PE response teams and advances in diagnosis and treatment modalities.Unfavorable prognosis may be attributed to the increasing incidence of the disease and pitfalls in risk stratification using the established risk stratification tools that fail to recognize patients with intermediate-high risk PE at normotensive shock in order to prevent further deterioration.In this light,research has been focused to identify novel risk stratification tools,based on the hemodynamic impact of PE on right ventricular function.Furthermore,a growing body of evidence has demonstrated that novel interventional treatments for PE,including catheter directed thrombolysis,mechanical thrombectomy and computer-assisted aspiration,are promising solutions in terms of efficacy and safety,when targeted at specific populations of the intermediate-high-and high-risk spectrum.Various therapeutic protocols have been suggested worldwide,regarding the indications and proper timing for interventional strategies.A STelevation myocardial infarction-like timing approach has been suggested in highrisk PE with contraindications for fibrinolysis,while optimal timing of the procedure in intermediate-high risk patients is still a matter of debate;however,early interventions,within 24-48 hours of presentation,are associated with more favorable outcomes.展开更多

Objectives:In this study,we examined the psychological impact of hierarchical and authoritarian structures in elite sports teams in South Korea on the ego formation and mental health of athletes.We aimed to analyze ho...Objectives:In this study,we examined the psychological impact of hierarchical and authoritarian structures in elite sports teams in South Korea on the ego formation and mental health of athletes.We aimed to analyze how these environments shape psychological well-being in athletes,drawing on Freud’s group psychology theory,while integrating perspectives from the Self-DeterminationTheory(SDT)and Social Identity Theory(SIT).Methods:We applied a qualitative case-study approach,with data collected through in-depth interviews with eight retired elite table tennis players from South Korea.These athletes shared their experiences with psychological mechanisms in their teams and how those mechanisms impacted their mental health throughout their careers.We analyzed the collected data using thematic analysis.Results:The investigated psychological mechanisms significantly influenced the ego development and psychological well-being in athletes.In hierarchical and authoritarian environments,identification and ego idealization suppressed the autonomy of the athletes.Hierarchical order and obedience to authority figures exert significant pressure on athletes to conform,impeding the development of an independent ego.Furthermore,group pressure exacerbates ego erosion and psychological stress,while corporal punishment reinforces psychological pressure and hinders ego integrity.The integration of the SDT highlights the need for autonomy-supportive environments to mitigate such risks,while the SIT emphasizes balancing individual identity with group cohesion to address identity conflicts and psychological tension.Conclusion:Hierarchical structures and authoritarian dynamics in South Korean elite sports teams critically suppress autonomy in athletes,leading to psychological distress and identity conflicts.This study highlights the urgent need for systemic interventions to reform coaching practices and foster autonomysupportive environments.These results contribute to the global discourse on the mental health of athletes in elite sports and offer actionable strategies for improving long-term well-being.Finally,this study provides practical recommendations adaptable to diverse cultural and sporting contexts,enhancing athlete mental health worldwide.展开更多

Methods: A total of 200 patients who used intravenous indwelling needles during hospital treatment from July to October 2022 were selected and randomly divided into an observation group and a control group, with 100 patients in each group. Patients in the control group received routine nursing methods, while those in the observation group were managed using the safety risk management method of the infusion project team. The tube blocking rate and tube removal rate were compared between the two groups. Results: The one-time puncture success rate and the standard implementation rate of intravenous indwelling needle use in the observation group were significantly higher than those in the control group. The total incidence of complications in the observation group was significantly lower than that in the control group, and patient satisfaction in the observation group was notably higher, with statistically significant differences (P < 0.05). Conclusion: The application of an infusion project team in the safety risk management of indwelling needle use is effective and has positive impacts on improving the one-time puncture success rate, the standard implementation rate of intravenous indwelling needle use, and patient satisfaction. It is a practice worth promoting in clinical settings.
文摘Objective:To explore the experiences of palliative care multidisciplinary teams from the perspective of pluralistic synergy theory,aiming to enhance team collaboration.Method:Utilizing a qualitative research methodology,we conducted in-depth interviews with 15 palliative care team members to collect data.Results:The operational framework of palliative care multidisciplinary teams under pluralistic synergy theory involves:enhancing collaboration among team members;developing a comprehensive management model for the team;rationalizing the allocation of work tasks and durations;improving team communication and interaction through effective communication mechanisms and platforms;cultivating skilled professionals;and establishing a robust health record management system.Conclusion:The operational mechanism,informed by pluralistic synergy theory,advances the development of palliative care.
文摘Pulmonary embolism(PE)represents the third leading cause of cardiovascular death,despite the implementation of European Society of Cardiology guidelines,the establishment of PE response teams and advances in diagnosis and treatment modalities.Unfavorable prognosis may be attributed to the increasing incidence of the disease and pitfalls in risk stratification using the established risk stratification tools that fail to recognize patients with intermediate-high risk PE at normotensive shock in order to prevent further deterioration.In this light,research has been focused to identify novel risk stratification tools,based on the hemodynamic impact of PE on right ventricular function.Furthermore,a growing body of evidence has demonstrated that novel interventional treatments for PE,including catheter directed thrombolysis,mechanical thrombectomy and computer-assisted aspiration,are promising solutions in terms of efficacy and safety,when targeted at specific populations of the intermediate-high-and high-risk spectrum.Various therapeutic protocols have been suggested worldwide,regarding the indications and proper timing for interventional strategies.A STelevation myocardial infarction-like timing approach has been suggested in highrisk PE with contraindications for fibrinolysis,while optimal timing of the procedure in intermediate-high risk patients is still a matter of debate;however,early interventions,within 24-48 hours of presentation,are associated with more favorable outcomes.

文摘Objectives:In this study,we examined the psychological impact of hierarchical and authoritarian structures in elite sports teams in South Korea on the ego formation and mental health of athletes.We aimed to analyze how these environments shape psychological well-being in athletes,drawing on Freud’s group psychology theory,while integrating perspectives from the Self-DeterminationTheory(SDT)and Social Identity Theory(SIT).Methods:We applied a qualitative case-study approach,with data collected through in-depth interviews with eight retired elite table tennis players from South Korea.These athletes shared their experiences with psychological mechanisms in their teams and how those mechanisms impacted their mental health throughout their careers.We analyzed the collected data using thematic analysis.Results:The investigated psychological mechanisms significantly influenced the ego development and psychological well-being in athletes.In hierarchical and authoritarian environments,identification and ego idealization suppressed the autonomy of the athletes.Hierarchical order and obedience to authority figures exert significant pressure on athletes to conform,impeding the development of an independent ego.Furthermore,group pressure exacerbates ego erosion and psychological stress,while corporal punishment reinforces psychological pressure and hinders ego integrity.The integration of the SDT highlights the need for autonomy-supportive environments to mitigate such risks,while the SIT emphasizes balancing individual identity with group cohesion to address identity conflicts and psychological tension.Conclusion:Hierarchical structures and authoritarian dynamics in South Korean elite sports teams critically suppress autonomy in athletes,leading to psychological distress and identity conflicts.This study highlights the urgent need for systemic interventions to reform coaching practices and foster autonomysupportive environments.These results contribute to the global discourse on the mental health of athletes in elite sports and offer actionable strategies for improving long-term well-being.Finally,this study provides practical recommendations adaptable to diverse cultural and sporting contexts,enhancing athlete mental health worldwide.
