Effects of acupuncture-moxibustion combined with rehabilitation training on limb function and surface electromyography in patients with hemiplegia after stroke

Objective:To observe the effects of acupuncture-moxibustion combined with rehabilitation training on limb function and surface electromyography(sEMG)in patients with hemiplegia after stroke(HAS).Methods:Eighty-four HAS patients were divided into a control group and a combined group using the random number table method,with 42 cases in each group.The control group received dynamic-static balance rehabilitation training,and the combined group additionally received Fu Zheng Bu Tu(strengthening the healthy Qi and invigorating the spleen)acupuncture-moxibustion therapy.After 4 weeks of treatment,their clinical efficacy was compared,and they were also observed for changes in limb motor function,muscle spasticity level,balance ability,sEMG indices,and hemorheological parameters,as well as adverse reactions.Results:After treatment,the combined group had a higher total effective rate than the control group(P<0.05).After the intervention,both groups presented significant increases in the upper-and lower-limb Fugl-Meyer assessment(FMA)score,Berg balance scale(BBS)score,maximum voluntary contraction(MVC),root mean square(RMS),and integrated electromyography(iEMG);the combined group had superior improvements compared to the control group(P<0.05).After treatment,the modified Ashworth scale(MAS)scores of the elbow,wrist,knee,and ankle joints,and the levels of plasma viscosity(PV),whole blood viscosity(WBV),and hematocrit(HCT)dropped in both groups and were lower in the combined group than in the control group(P<0.05).Neither group showed significant adverse reactions during the treatment period.Conclusion:Fu Zheng Bu Tu acupuncture-moxibustion plus dynamic-static balance rehabilitation training can effectively alleviate muscle spasticity,improve balance ability,enhance sEMG signal levels,modulate hemodynamics,and promote the recovery of limb motor function in HAS patients. 展开更多

Clinical Investigation of Shoulder Joint Function and Pain Alleviation in Patients with Rotator Cuff Injuries via Structured Phased Rehabilitation Training

[Objectives]To investigate the clinical effects of implementing structured phased rehabilitation training,in addition to conventional rehabilitation,on shoulder joint function and pain alleviation in patients with rotator cuff injuries managed conservatively.[Methods]Eighty patients diagnosed with rotator cuff injury were selected and randomly assigned to either the control group or the experimental group,each comprising 40 individuals.The control group received conventional rehabilitation treatment,whereas the experimental group underwent phased rehabilitation training in addition to the conventional treatment for 6 weeks.Assessments were conducted prior to treatment,6 weeks following treatment,and 8 weeks after the completion of treatment(follow-up period).The visual analogue scale(VAS)was employed to evaluate pain intensity,the Constant-Murley score was utilized to assess shoulder joint function,and the shoulder joint range of motion was measured.[Results]Prior to treatment,no statistically significant differences were observed between the two patient groups across all measured indicators(P>0.05).Following 6 weeks of treatment and throughout the follow-up period,both groups exhibited significant reductions in VAS scores compared to baseline measurements,alongside improvements in Constant-Murley scores and shoulder joint range of motion(P<0.05).Furthermore,the magnitude of improvement in the experimental group was significantly greater than that in the control group(P<0.05).[Conclusions]Phased rehabilitation training can enhance shoulder joint function and alleviate pain in patients with rotator cuff injuries beyond the effects of conventional rehabilitation treatment,demonstrating notable clinical application value. 展开更多

Objective:To analyze the improvement effect of early postoperative rehabilitation training on balance ability and quality of life in elderly patients with hip fracture.Methods:A total of 50 elderly patients with hip fracture admitted to our hospital from January 2023 to January 2024 were selected and divided into the observation group(25 cases)and the control group(25 cases)by random number table method.The control group received routine nursing,while the observation group received early rehabilitation training on the basis of routine nursing.The balance ability(Berg Balance Scale,BBS)and quality of life(SF-36)of the two groups were compared.Results:The BBS scores of the observation group at all postoperative time points were significantly higher than those of the control group(p<0.05),and the quality-of-life scores of the observation group were also significantly higher than those of the control group(p<0.05).Conclusion:Early postoperative rehabilitation training for elderly patients with hip fracture can improve their balance ability,enhance their quality of life,and reduce the incidence of postoperative complications,which is worthy of clinical promotion. 展开更多

Objective To compare the influences of acupuncture combined with rehabilitative training and simple rehabilitative training on neural functional recovery of stroke patients during recovery phase.Methods Sixty-seven stroke patients during recovery phase were randomly divided into an acupuncture combined with rehabilitative training group(group A) and a simple rehabilitative training group(group B) through random number table method,with 34 cases and 33 cases respectively;33 cases and 31 cases were respectively completed actually other than cases dropped out during the observation.Rehabilitation methods such as motion control and functional exercise training were primarily used for rehabilitative training,once daily for 45 min at a time,and 6 times a week;Baihui(百会 GV 20) and Fengchi(风池 GB 20) on the head and relevant acupoints on limbs were selected for acupuncture,once every other day,and 3 times a week.Both groups were treated for 8 weeks,National Institnte of Health Stroke Scale(NIHSS) was used to assess neurologic impairment,Fugl-Meyer scale was used to assess motor function,Barthel was used to assess activity of daily living,and comprehensive assessment was finally made to patients.Results NIH score,Fugl-Meyer scale scores of upper and lower limbs,Barthel score,and comprehensive assessment of group A after treatment were respectively:12.78±2.99,37.55±2.09 and25.67±3.93,63.76±4.65,11.33±1.98,superior to 16.06±4.57,28.71±4.07 and20.41±2.98,53.8214.35,15.18±1.72 of group B.Indices of both groups were significantly improved compared with those before treatment(P〈0.01,P〈0.05),but improvement of indices of group A was more obvious that that of group B after treatment subject to comparison(all P〈0.05).Conclusions Acupuncture combined with rehabilitative training has definite therapeutic effect on recovery of various functions such as neural functional recovery of stroke patients during recovery phase,obviously superior to simple rehabilitative training. 展开更多

Objective:To explore the rehabilitation effect of upper limb rehabilitation robots combined with isokinetic muscle strength training on hemiplegic patients in the recovery phase of stroke,and to provide a more effective intervention plan for clinical rehabilitation treatment.Methods:One hundred and sixty type 2 stroke patients with hemiplegia in recovery phase treated at Jintan First People’s Hospital from May 2020 to May 2025 were selected as the research subjects.They were divided into a control group(n=80)and an observation group(n=80)using a random number table method.The control group received isokinetic muscle strength training,while the observation group received additional upper limb rehabilitation robot-assisted intervention based on the control group’s treatment.The general information(age,gender,course of disease),Brunnstrum staging,Barthel index,FMA score,and facial expression pain score were compared between the two groups.Results:After treatment,the indicators of both groups improved compared to before treatment.The observation group showed significant advantages in improving various indicators.In terms of Brunnstrum staging,the number of patients in stage IV and above was significantly higher than that in the control group;in terms of the Barthel index,the increase was more significant than that of the control group(P=0.000<0.001).In terms of the FMA score,the increase was more pronounced(P=0.000<0.001);and in terms of facial expression pain assessment,the decrease was more evident(P=0.000<0.01).Conclusion:The combination of upper limb rehabilitation robots and isokinetic muscle strength training can significantly improve the recovery of limb function and activities of daily living in hemiplegic patients during the recovery phase of stroke,and reduce pain.Compared with simple isokinetic muscle strength training,the rehabilitation effect is better,and it is worthy of clinical application. 展开更多

Bronchiectasis is a chronic inflammatory airway disease,and patients often suffer from recurrent airway infections leading to decreased lung function and impaired quality of life.In this study,the effects of supervised pulmonary rehabilitation training on pulmonary function,training compliance,and quality of life in patients with bronchiectasis under home rehabilitation mode are investigated.Ninety stable patients were selected,and the observation group adopted the home-supervised mode of pulmonary rehabilitation training.The results showed that the observation group’s pulmonary function indexes,quality of life,and training adherence were better than those of the control group.The differences were statistically significant(P<0.05).The supervised pulmonary rehabilitation training in home rehabilitation mode can effectively improve patients’pulmonary function and quality of life,and improve training compliance,which has good clinical application value. 展开更多

Objective:To analyze the necessity and effectiveness of vestibular rehabilitation training instruction in the treatment of sudden deafness accompanied by vertigo.Methods:Sixty medical staff who learned the treatment methods for sudden deafness accompanied by vertigo(vestibular rehabilitation training)from January 2024 to February 2025 were selected as the research subjects.They were divided into two groups according to the time of study:the control group,who participated in the training teaching first,and the observation group,who participated later.Each group had 30 people.The control group received conventional training,while the observation group underwent continuous quality improvement.The effects and assessment results of different teaching methods were compared.Results:The theoretical and practical assessment scores of medical staff in the observation group were higher than those in the control group(p<0.05);the satisfaction of medical staff in the observation group with the teaching mode was higher than that in the control group(p<0.05).Conclusion:Vestibular rehabilitation training has significant effects on sudden deafness accompanied by vertigo.Training medical staff not only enhances their understanding of training methods but also enables them to provide specialized guidance to patients,improving their vertigo condition. 展开更多

Rehabilitation training is believed to be an effectual strategy that canreduce the risk of dysfunction caused by spasticity.However,achieving visualizationrehabilitation training for patients remains clinically challenging.Herein,wepropose visual rehabilitation training system including iontronic meta-fabrics withskin-friendly and large matrix features,as well as high-resolution image modules fordistribution of human muscle tension.Attributed to the dynamic connection and dissociationof the meta-fabric,the fabric exhibits outstanding tactile sensing properties,such as wide tactile sensing range(0~300 kPa)and high-resolution tactile perception(50 Pa or 0.058%).Meanwhile,thanks to the differential capillary effect,the meta-fabric exhibits a“hitting three birds with one stone”property(dryness wearing experience,long working time and cooling sensing).Based on this,the fabrics can be integrated with garmentsand advanced data analysis systems to manufacture a series of large matrix structure(40×40,1600 sensing units)training devices.Significantly,the tunability of piezo-ionic dynamics of the meta-fabric and the programmability of high-resolution imaging modules allowthis visualization training strategy extendable to various common disease monitoring.Therefore,we believe that our study overcomes theconstraint of standard spasticity rehabilitation training devices in terms of visual display and paves the way for future smart healthcare. 展开更多

Effect of stratified management and stepwise training on pulmonary rehabilitation in older patients with liver cancer

BACKGROUND Older patients with liver cancer often experience impaired pulmonary function post-surgery,increasing complications and recovery challenges.AIM To investigate the effects of evidence-based stratified management and stepwise training in the perioperative pulmonary rehabilitation of older patients with liver cancer,providing a basis for clinical application.METHODS In total,120 older patients with liver cancer who underwent surgery at our hospital between February 2023 and February 2025 were selected and randomly divided into study and control groups,with 60 patients in each group.All the patients underwent radical hepatectomy.Postoperatively,the control group received routine nursing management and rehabilitation training,while the study group received evidence-based stratified management combined with stepwise training for a continuous intervention period of one week.Time to first ambulation,length of hospital stays,and average hospitalization costs were recorded.Oxygen saturation(SPO_(2))was measured on postoperative day 1 and day 3.The 6-minute walk distance and Borg scale scores were assessed on postoperative day 1 and day 7,respectively.The postoperative complication rates were recorded.RESULTS The study group had a significantly shorter time to first ambulation,shorter hospital stays,and lower average hospitalization costs than the control group(P<0.05).On postoperative day 1,there was no significant difference in SPO_(2)between the groups(P>0.05);however,on postoperative day 3,the study group had significantly higher SPO_(2)(P<0.05).On postoperative day 7,the study group showed a significantly longer 6-minute walk distance and lower Borg scores than the control group(P<0.05).The incidence of postoperative complications in the study group was 3.33%,which was significantly lower than that in the control group(13.33%;P<0.05).CONCLUSION Implementing evidence-based stratified management combined with stepwise training in the perioperative pulmonary rehabilitation of older patients with liver cancer is improves lung function,reduces complications,and promotes effective recovery,demonstrating significant clinical value. 展开更多

The Impact of Narrative Nursing Combined with Exercise Training on Health Beliefs and Cardiac Rehabilitation in Patients with Acute Myocardial Infarction After Intervention Therapy

Objective:To explore the effect of narrative nursing combined with exercise training on health beliefs and cardiac rehabilitation in patients with acute myocardial infarction(AMI)after interventional treatment.Methods:A total of 111 patients with acute myocardial infarction(AMI)who received interventional therapy in Department of Cardiology of Hefei First People’s Hospital from January 2022 to September 2023 were selected as the study subjects.They were randomly divided into a control group(n=55)and a study group(n=56)using a random table method.Both groups received routine nursing care,while the control group received exercise training on top of it.The study group received narrative nursing care on top of the control group.Both groups were intervened until the patients were discharged.The health beliefs,heart function,psychological status,and quality of life after intervention were compared between two groups using a 6-minute walk test(6MWT).Results:After intervention,the scores of the TSK-SV Heart in all dimensions(fear of movement,functional disorders,risk perception,and avoidance of movement)decreased in both groups,and the study group was lower than the control group,with a statistical significant difference(P<0.05).After intervention,the scores of the Self Rating Anxiety Scale(SAS)and Self Rating Depression Scale(SDS)in both groups decreased,and the study group was lower than the control group,with a statistical significant difference(P<0.05).After intervention,both groups showed an increase in left ventricular ejection fraction(LVEF)and left ventricular early diastolic peak flow velocity/left ventricular late diastolic peak flow velocity(E/A),with the study group being higher than the control group.The left ventricular end diastolic diameter(LVEDD)and left atrial volume index(LAVI)decreased,and the study group was lower than the control group,with statistical significant differences(P<0.05).The walking distance of the study group at 6MWT(488.8±31.4)m was greater than that of the control group(425.54±30.7)m,with statistical significant differences(P<0.05).The quality of life measurement scale(CROQ-PTCA-Post)of the study group after coronary intervention treatment had higher scores in all dimensions(physical function,satisfaction,psychosocial function,adverse reactions,symptoms,cognitive function)than the control group,with statistical significant differences(P<0.05).Conclusion:Narrative nursing combined with exercise training can enhance the health beliefs of AMI patients towards exercise training after intervention therapy,which is beneficial for cardiac rehabilitation and can improve psychological status and quality of life. 展开更多

Objective:To explore the rehabilitation effect of pulmonary rehabilitation training in elderly patients with chronic obstructive pulmonary disease(COPD).Methods:Elderly COPD patients hospitalized from June 2024 to December 2024 were selected as subjects for a quasi-experimental study.The study randomly assigned patients into two equal groups of 50 each.One group served as the control and received standard treatment and nursing care,while the other group,the intervention group,was given additional pulmonary rehabilitation exercises alongside the conventional care.The lung function indexes[forced expiratory volume in the first second(FEV1),forced vital capacity(FVC),FEV1/FVC],6-minute walk distance(6MWD),Borg’s Category Scale for perceived exertion(BCSS),COPD Assessment Test(CAT),Barthel Index for activities of daily living,blood oxygen saturation,and Modified Medical Research Council(MMRC)dyspnea scale were compared between the two groups before and after intervention.Results:Following the intervention,the intervention group demonstrated improved lung function compared to the control group(P<0.05).However,no significant difference was observed in the 6-minute walk distance(6MWD)(P>0.05).The intervention group had lower Bronchitis Severity Scale(BCSS)scores(P<0.05)and better quality of life scores(P<0.05)than the control group.No notable differences were found in activities of daily living scores or blood oxygen saturation(P>0.05).Additionally,the intervention group exhibited a lower proportion of residual severe dyspnea(P<0.05).Conclusion:Pulmonary rehabilitation can enhance respiratory function in individuals with COPD,leading to beneficial therapeutic outcomes and ultimately improving their overall quality of life. 展开更多

The recovery of motor function in spastic hemiplegia following stroke is closely associated with cortical reorganisation.This study aimed to investigate the effects of warm acupuncture combined with rehabilitation training on cortical activity and upper limb function using functional near-infrared spectroscopy.Ninety patients were randomly assigned to a warm acupuncture group,a rehabilitation training group,or a combined intervention group.After four weeks of intervention,clinical scores and changes in oxyhaemoglobin concentration in the affected motor cortex were assessed.Results demonstrated that the combined intervention group showed significantly greater improvement in Fugl-Meyer scores,reduced muscle tone,and enhanced activation in the affected primary motor cortex compared to the single intervention groups.Furthermore,functional improvement was significantly correlated with cortical activation changes.These findings indicate a synergistic effect between warm acupuncture and rehabilitation training,potentially mediated through promoting functional reorganisation of the affected cerebral cortex.This provides neuroimaging evidence supporting integrated Chinese and Western medicine rehabilitation protocols. 展开更多

Virtual reality(VR)technology revitalises rehabilitation training by creating rich,interactive virtual rehabilitation scenes and tasks that deeply engage patients.Robotics with immersive VR environments have the potential to significantly enhance the sense of immersion for patients during training.This paper proposes a rehabilitation robot system.The system integrates a VR environment,the exoskeleton entity,and research on rehabilitation assessment metrics derived from surface electromyographic signal(sEMG).Employing more realistic and engaging virtual stimuli,this method guides patients to actively participate,thereby enhancing the effectiveness of neural connection reconstruction—an essential aspect of rehabilitation.Furthermore,this study introduces a muscle activation model that merges linear and non-linear states of muscle,avoiding the impact of non-linear shape factors on model accuracy present in traditional models.A muscle strength assessment model based on optimised generalised regression(WOAGRNN)is also proposed,with a root mean square error of 0.017,347 and a mean absolute percentage error of 1.2461%,serving as critical assessment indicators for the effectiveness of rehabilitation.Finally,the system is preliminarily applied in human movement experiments,validating the practicality and potential effectiveness of VRcentred rehabilitation strategies in medical recovery. 展开更多

Application of Phased Nursing Guidance Program in Early Rehabilitation Training for Patients after Minimally Invasive Spinal Surgery

Objective:To explore the application effect of a phased nursing guidance program in early rehabilitation training for patients after minimally invasive spinal surgery,providing references for clinical rehabilitation nursing.Methods:A total of 148 patients who underwent minimally invasive spinal surgery in our hospital from July 2023 to June 2025 were selected as the study subjects.They were randomly divided into an observation group and a control group using a random number table method,with 74 cases in each group.The control group received conventional nursing guidance after minimally invasive spinal surgery,while the observation group implemented a phased nursing guidance program.The pain levels(VAS scores),spinal function(JOA scores),quality of life(SF-36 scores),rehabilitation training compliance,and complication rates of the two groups of patients at different postoperative time points were compared.Results:At 7,14,and 30 days postoperatively,the Visual Analogue Scale(VAS)scores in the observation group were significantly lower than those in the control group(all p<0.001).At 7,14,and 30 days postoperatively,the Japanese Orthopaedic Association(JOA)scores in the observation group were significantly higher than those in the control group(all p<0.0001).Moreover,the JOA scores in both groups gradually increased over time,with a more pronounced increase observed in the observation group.At 30 days postoperatively,the scores in all dimensions of the Short Form-36(SF-36)scale in the observation group were significantly higher than those in the control group(all p<0.001).The compliance rate with rehabilitation training in the observation group was 95.95%,significantly higher than the 82.43%in the control group(χ^(2)=7.008,p<0.05).The complication rate in the observation group was 4.05%,significantly lower than the 14.86%in the control group(χ^(2)=5.049,p<0.05).Conclusion:The phased nursing guidance program can effectively alleviate pain,improve spinal function and quality of life,enhance compliance with rehabilitation training,and reduce the incidence of complications in patients after minimally invasive spinal surgery,making it worthy of clinical promotion and application. 展开更多

[Objectives]To explore the effects of electroacupuncture combined with rehabilitation training on knee joint function,three-dimensional(3D)gait,and inflammatory markers in patients with knee osteoarthritis(KOA)of cold-dampness obstruction syndrome.[Methods]A total of 162 KOA patients admitted to Huadong Hospital Affiliated to Fudan University from January 2021 to May 2023 were enrolled and randomly divided into an electroacupuncture group and a control group,with 81 patients in each group.The control group received routine rehabilitation training,while the electroacupuncture group received electroacupuncture treatment in addition to the same rehabilitation training,both for 4 weeks.The efficacy,syndrome scores,Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC),Lysholm Knee Score(LKS),3D gait parameters,and levels of inflammatory markers were compared between the two groups.[Results]Following treatment,the total effective rate in the electroacupuncture group was 92.59%,which was significantly higher than 77.78%observed in the control group(P<0.05).Additionally,the electroacupuncture group exhibited lower TCM syndrome scores(P<0.05),reduced WOMAC scores,and elevated LKS scores compared to the control group(P<0.05).Gait parameters,including step frequency,step speed,stride length,initial ground contact flexion angle,maximum swing phase extension angle,and support phase extension angle,were all higher in the electroacupuncture group.Additionally,the sagittal plane maximum abduction moment was lower in the electroacupuncture group.Inflammatory markers showed that interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were lower in the electroacupuncture group,while transforming growth factor-β1(TGF-β1)was higher(P<0.05).[Conclusions]Electroacupuncture combined with rehabilitation training effectively enhances clinical efficacy,alleviates symptoms,improves knee joint mobility and walking ability,enhances knee function scores,and reduces inflammatory levels,contributing to the rapid recovery of knee joints in KOA patients. 展开更多

Stroke causes long-term disability, and rehabilitative training is commonly used to improve the consecutive functional recovery. Following brain damage, surviving neurons undergo morphological alterations to reconstruct the remaining neural network. In the motor system, such neural network remodeling is observed as a motor map reorganization. Because of its significant correlation with functional recovery, motor map reorganization has been regarded as a key phenomenon for functional recovery after stroke. Although the mechanism underlying motor map reorganization remains unclear, increasing evidence has shown a critical role for axonal remodeling in the corticospinal tract. In this study, we review previous studies investigating axonal remodeling in the corticospinal tract after stroke and discuss which mechanisms may underlie the stimulatory effect of rehabilitative training. Axonal remodeling in the corticospinal tract can be classified into three types based on the location and the original targets of corticospinal neurons, and it seems that all the surviving corticospinal neurons in both ipsilesional and contralesional hemisphere can participate in axonal remodeling and motor map reorganization. Through axonal remodeling, corticospinal neurons alter their output selectivity from a single to multiple areas to compensate for the lost function. The remodeling of the corticospinal axon is influenced by the extent of tissue destruction and promoted by various therapeutic interventions, including rehabilitative training. Although the precise molecular mechanism underlying rehabilitation-promoted axonal remodeling remains elusive, previous data suggest that rehabilitative training promotes axonal remodeling by upregulating growth-promoting and downregulating growth-inhibiting signals. 展开更多

In order to mitigate the effects of space adaptation syndrome(SAS) and improve the training efficiency of the astronauts, a novel astronaut rehabilitative training robot(ART) was proposed. ART can help the astronauts to carry out the bench press training in the microgravity environment. Firstly, a dynamic model of cable driven unit(CDU) was established whose accuracy was verified through the model identification. Secondly, to improve the accuracy and the speed of the active loading, an active loading hybrid force controller was proposed on the basis of the dynamic model of the CDU. Finally, the actual effect of the hybrid force controller was tested by simulations and experiments. The results suggest that the hybrid force controller can significantly improve the precision and the dynamic performance of the active loading with the maximum phase lag of the active loading being 9° and the maximum amplitude error being 2% at the frequency range of 10 Hz. BACKGROUND:Botulinum toxin type A(BTX-A)is mostly to be used to treat various diseases of motor disorders,whereas its effect on muscle spasm after stroke and brain injury needs further observation.OBJECTIVE:To observe the effect of BTX-A plus rehabilitative training on treating muscle spasm after stroke and brain injury.DESIGN:A randomized controlled observation.SETTINGS:Department of Rehabilitation,Department of Neurology and Department of Neurosurgery,the Second Hospital of Hebei Medical University.PARTICIPANTS:Sixty inpatients with brain injury and stroke were selected from the Department of Rehabilitation,Department of Neurology and Department of Neurosurgery,the Second Hospital of Hebei Medical University from January 2001 to August 2006.They were all confirmed by CT and MRI,and had obvious increase of spastic muscle strength in upper limbs,their Ashworth grades were grade 2 or above.The patients were randomly divided into treatment group(n=30)and control group(n=30).METHODS:①Patients in the treatment group undertook comprehensive rehabilitative trainings,and they were administrated with domestic BTX-A,which was provided by Lanzhou Institute of Biological Products,Ministry of Health(S10970037),and the muscles of flexion spasm were selected for upper limbs,20-25 IU for each site.②Patients in the treatment group were assessed before injection and at 1 and 2 weeks,1 and 3 months after injection respectively,and those in the control group were assessed at corresponding time points.The recovery of muscle spasm was assessed by modified Ashworth scale(MAS,grade 0-Ⅳ;Grade 0 for without increase of muscle strength;GradeⅣfor rigidity at passive flexion and extension);The recovery of motor function of the upper limbs was evaluated with Fugl-Meyer Assessment(FMA,total score was 226 points,including 100 for exercise,14 for balance,24 for sense,44 for joint motion,44 for pain and 66 for upper limb);The ADL were evaluated with Barthel index,the total score was 100 points,60 for mild dysfunction,60-41 for moderate dysfunction,<40 for severe dysfunction).MAIN OUTCOME MEASURES:Changes of MAS grade,FMA scores and Barthel index before and after BTX-A injection.RESULTS:All the 60 patients with brain injury and stroke were involved in the analysis of results.①FMA scores of upper limbs:The FMA score in the treatment group at 2 weeks after treatment was higher than that before treatment[(14.98±10.14),(13.10±9.28)points,P<0.05],whereas there was no significant difference at corresponding time point in the control group.The FMA scores at 1 and 3 months in the treatment group[(23.36±10.69),(35.36±11.36)points]were higher than those in the control group[(20.55±10.22),(30.33±10.96)points,P<0.01].②MAS grades of upper limbs:There were obviously fewer cases of gradeⅢin MAS at 2 weeks after treatment than before treatment in the treatment group(0,9 cases,P<0.05),whereas there was no obvious difference in the control group.There were obviously fewer cases of gradeⅢin MAS at 2 weeks and 1 month after treatment in the treatment group(0,0 case)than the control group(5,2 cases,P<0.01).③Barthel index of upper limbs:The Barthel index at 2 weeks after treatment was higher than that before treatment in the treatment group[(30.36±22.25),(28.22±26.21)points,P<0.05],whereas there was no significant difference in the control group.The Barthel indexes at 1 and 3 months after treatment in the treatment group were obviously higher than those in the control group[(20.55±10.22),(30.33±10.96)points,P<0.01].CONCLUSION:BTX-A has obvious efficacy on decreasing muscle tension after stroke and brain injury,and relieving muscle spasm;Meanwhile,the combination with rehabilitative training can effectively ameliorate the motor function of upper limbs and ADL of the patients. 展开更多
