Preliminary experiments are carried out to characterize the droplets generated in two aerosol-generating health care procedures,i.e.taking nasopharyngeal aspirates(NPA)and nebulizer therapy.Glass slides and water-se...Preliminary experiments are carried out to characterize the droplets generated in two aerosol-generating health care procedures,i.e.taking nasopharyngeal aspirates(NPA)and nebulizer therapy.Glass slides and water-sensitive paper(WSP)are used to collect large droplets when taking NPA.Droplet stain-marks on glass slides are counted under a microscope,and then a size analysis is performed.During nebulizer therapy dust monitors are used to detect small droplets and droplet nuclei at different positions around the nebulizer and in the room.From the preliminary results it is found that taking NPA can stimulate coughing and generate large droplets.Nebulizers can generate more than tens of millions fine droplets ranging from 0.3 to 20 μm per minute,a large volume of which can escape from the holes on the nebulizer's facemask and disperse in the whole room.Droplets coagulate on the inner surface of the mask and the volunteer's face,suggesting a great possibility of drug solution contamination by patients' secretion during nebulizer therapy.展开更多
BACKGROUND Healthcare professionals need to be prepared to promote healthy lifestyles and care for patients.By focusing on what students should be able to perform one day as clinicians,we can bridge the gap between me...BACKGROUND Healthcare professionals need to be prepared to promote healthy lifestyles and care for patients.By focusing on what students should be able to perform one day as clinicians,we can bridge the gap between mere theoretical knowledge and its practical application.Gender aspects in clinical medicine also have to be considered when speaking of personalized medicine and learning curricula.AIM To determine sets of intellectual,personal,social,and emotional abilities that comprise core qualifications in medicine for performing well in anamnesis-taking,in order to identify training needs.METHODS An analysis of training clinicians’conceptions with respect to optimal medical history taking was performed.The chosen study design also aimed to assess gender effects.Structured interviews with supervising clinicians were carried out in a descriptive study at the Medical University of Vienna.Results were analyzed by conducting a qualitative computer-assisted content analysis of the interviews.Inductive category formation was applied.The main questions posed to the supervisors dealt with(1)Observed competencies of students in medical history taking;and(2)The supervisor’s own conceptions of"ideal medical history taking".RESULTS A total of 33 training clinicians(n=33),engaged in supervising medical students according to the MedUni Vienna’s curriculum standards,agreed to be enrolled in the study and met inclusion criteria.The qualitative content analysis revealed the following themes relevant to taking an anamnesis:(1)Knowledge;(2)Soft skills(relationship-building abilities,trust,and attitude);(3)Methodical skills(structuring,precision,and completeness of information gathering);and(4)Environmental/contextual factors(language barrier,time pressure,interruptions).Overall,health care professionals consider empathy and attitude as critical features concerning the quality of medical history taking.When looking at physicians’theoretical conceptions,more general practitioners and psychiatrists mentioned attitude and empathy in the context of"ideal medical history taking",with a higher percentage of females.With respect to observations of students’history taking,a positive impact from attitude and empathy was mainly described by male health care professionals,whereas no predominance of specialty was found.Representatives of general medicine and internal medicine,when observing medical students,more often emphasized a negative impact on history taking when students lacked attitude or showed non-empathetic behavior;no gender-specific difference was detected for this finding.CONCLUSION The analysis reveals that for clinicians engaged in medical student education,only a combination of skills,including adequate knowledge and methodical implementations,is supposed to guarantee acceptable performance.This study’s findings support the importance of concepts like relationship building,attitude,and empathy.However,there may be contextual factors in play as well,and transference of theoretical concepts into the clinical setting might prove challenging.展开更多

Here, we experimentally tested this hypothesis by providing wild passerine birds supplemental food (on a raised feeding platform) at either 1 or 4 m from the edge of forest cover (potential refuge), in either the presence or absence of a nearby simulated predation threat (a sharp-shinned hawk Accipiter striatus model). Compared with the control treatment, we observed proportionally fewer bird visits to the food patch, and the birds took longer to re-emerge from forest refuge and return to feed at the food patch, after the hawk presentation than before it. The observed threat-sensitive latency-to-return response was stronger when the food patch was further away from the nearest refuge. Overall, our results are consistent with the predictions of the threat-sensitive predator avoidance hypothesis in that wild passerine birds (primarily black-capped chickadees Poecile atricapillus) exhibited more intense antipre- dator behavioural responses with increasing level of apparent threat. Met...Objective: To evaluate the effectiveness of an educational intervention to improve knowledge and skills in taking Pap smear conducted by health staff implementing the program on Early Detection of Cervical Cancer. Methods: A quasiexperimental, longitudinal and prospective study, held from October 25 2010 to April 11, 2011 at a hospital in San Luis Potosi, Mexico. To select groups applied a non-probabilistic convenience sampling that was composed of 28 people, 15 for the study group and 13 to the control group. The study group received training that was divided into three phases in which assessed the knowledge and skills taking the smear, the control group received conventional training offered by your workplace. Papanicolaou quality was measured before and after the intervention. The reference used international classification system Bethesda 2001. For data analysis we used the Student t test. Results: The study group increased knowledge (t = 8.768, p = 0.000) in the management of official standards, in the anatomy of the lower female genital tract and in the form of testing. Technical skills (t = 8.639, p = 0.000) also increased in the study group after the intervention. The control group showed no significant changes. The quality report of the samples in the study group increased from 60% to 86.7% and in the control group decreased from 92.3% to 84.6%. Conclusions: The training is effective in improving the knowledge and skills in taking Pap. Health staff who received the training improved their performance and showed more interest in providing high quality service to women.展开更多
Background: Acquisition of family medical history (FMH) is emphasized as a part of obtaining a complete medical history, but whether FMH is consistently documented and utilized in primary care, as well as how it can a...Background: Acquisition of family medical history (FMH) is emphasized as a part of obtaining a complete medical history, but whether FMH is consistently documented and utilized in primary care, as well as how it can affect patient care in this context, remains unclear. Thus, the objectives of this study were to determine: 1) if FMH is regularly acquired in a representative primary care practice (the Queen’s Family Health Team, QFHT);2) what is included in the FMH obtained;3) what the utility of FMH is with regards to patient management in primary care;and 4) to utilize healthcare practitioners’ perspectives in order to elucidate any findings regarding the acquisition and utility of FMH at the QFHT. Methods: Patients were interviewed in order to obtain their FMH. For each patient, the FMH obtained was compared to the FMH documented in the patient’s record to determine the record’s completeness. Each patient’s FMH was analyzed for significant history of coronary artery disease (CAD), diabetes mellitus type II (DMII), substance abuse (SA) and colorectal cancer (CRC). Participants were patients scheduled for appointments at the QFHT between May and July 2011. Any patient of the QFHT older than 25 years was eligible to participate. Clinical staff of the QFHT completed an online questionnaire to determine healthcare practitioners’ perspectives regarding the acquisition and utility of FMH. Results: 83 patients participated in the study. Participants ranged in age from 25 - 86 years (median: 63 years);69% were female. FMH present in patients’ records was often either incomplete (42% of charts reviewed) or not documented at all (51% of charts reviewed). Knowledge of FMH can affect patient management in primary care for the diseases assessed (CAD, DMII, SA and CRC). HCP do consider FMH to be important in clinical practice and 86% of respondents stated that they regularly inquired about patients’ FMH. Interpretation: Despite the belief by HCP that FMH is important, there is a disparity between this belief and their practices regarding its documentation and utilization. Finally, analysis of the FMH of the representative population studied shows that information commonly missing in patients’ FMH can affect patient management at a primary care level.展开更多

This study examined the factors influencing the behavior of taking medicine in elderly patients undergoing HD. Subjects and Methods: The subjects comprised 70 outpatients > 65 years undergoing HD (48 male and 22 female). The mean age of the patients was 72.5 ± 4.4 years. The mean duration of dialysis history was 6.1 ± 5.2 years. We performed a questionnaire survey using Medication Assessment Tool, The Kidney Disease Quality of Life (QOL)-Short Form and Acceptance for Dialysis Instrument. Result: Two factors related to poor behavior of taking medicine: dialysis history < 2 years and poor acceptance of dialysis. The multiple regression analysis showed patient satisfaction (β = –0.329, p < 0.01) and symptoms/problems (β = –0.273, p<0.05) as significant independent variables relating to behavior of taking medicine. Conclusion: Poor behavior of taking medicine is related to a short dialysis period, poor acceptance and poor satisfaction of dialysis therapy in elderly patients.展开更多

文摘Objective: To evaluate the effectiveness of an educational intervention to improve knowledge and skills in taking Pap smear conducted by health staff implementing the program on Early Detection of Cervical Cancer. Methods: A quasiexperimental, longitudinal and prospective study, held from October 25 2010 to April 11, 2011 at a hospital in San Luis Potosi, Mexico. To select groups applied a non-probabilistic convenience sampling that was composed of 28 people, 15 for the study group and 13 to the control group. The study group received training that was divided into three phases in which assessed the knowledge and skills taking the smear, the control group received conventional training offered by your workplace. Papanicolaou quality was measured before and after the intervention. The reference used international classification system Bethesda 2001. For data analysis we used the Student t test. Results: The study group increased knowledge (t = 8.768, p = 0.000) in the management of official standards, in the anatomy of the lower female genital tract and in the form of testing. Technical skills (t = 8.639, p = 0.000) also increased in the study group after the intervention. The control group showed no significant changes. The quality report of the samples in the study group increased from 60% to 86.7% and in the control group decreased from 92.3% to 84.6%. Conclusions: The training is effective in improving the knowledge and skills in taking Pap. Health staff who received the training improved their performance and showed more interest in providing high quality service to women.
文摘Background: Acquisition of family medical history (FMH) is emphasized as a part of obtaining a complete medical history, but whether FMH is consistently documented and utilized in primary care, as well as how it can affect patient care in this context, remains unclear. Thus, the objectives of this study were to determine: 1) if FMH is regularly acquired in a representative primary care practice (the Queen’s Family Health Team, QFHT);2) what is included in the FMH obtained;3) what the utility of FMH is with regards to patient management in primary care;and 4) to utilize healthcare practitioners’ perspectives in order to elucidate any findings regarding the acquisition and utility of FMH at the QFHT. Methods: Patients were interviewed in order to obtain their FMH. For each patient, the FMH obtained was compared to the FMH documented in the patient’s record to determine the record’s completeness. Each patient’s FMH was analyzed for significant history of coronary artery disease (CAD), diabetes mellitus type II (DMII), substance abuse (SA) and colorectal cancer (CRC). Participants were patients scheduled for appointments at the QFHT between May and July 2011. Any patient of the QFHT older than 25 years was eligible to participate. Clinical staff of the QFHT completed an online questionnaire to determine healthcare practitioners’ perspectives regarding the acquisition and utility of FMH. Results: 83 patients participated in the study. Participants ranged in age from 25 - 86 years (median: 63 years);69% were female. FMH present in patients’ records was often either incomplete (42% of charts reviewed) or not documented at all (51% of charts reviewed). Knowledge of FMH can affect patient management in primary care for the diseases assessed (CAD, DMII, SA and CRC). HCP do consider FMH to be important in clinical practice and 86% of respondents stated that they regularly inquired about patients’ FMH. Interpretation: Despite the belief by HCP that FMH is important, there is a disparity between this belief and their practices regarding its documentation and utilization. Finally, analysis of the FMH of the representative population studied shows that information commonly missing in patients’ FMH can affect patient management at a primary care level.
文摘Purpose: The elderly are often faced with multiple diseases, in particular hemodialysis (HD) which requires many kinds of medication. This study examined the factors influencing the behavior of taking medicine in elderly patients undergoing HD. Subjects and Methods: The subjects comprised 70 outpatients > 65 years undergoing HD (48 male and 22 female). The mean age of the patients was 72.5 ± 4.4 years. The mean duration of dialysis history was 6.1 ± 5.2 years. We performed a questionnaire survey using Medication Assessment Tool, The Kidney Disease Quality of Life (QOL)-Short Form and Acceptance for Dialysis Instrument. Result: Two factors related to poor behavior of taking medicine: dialysis history < 2 years and poor acceptance of dialysis. The multiple regression analysis showed patient satisfaction (β = –0.329, p < 0.01) and symptoms/problems (β = –0.273, p<0.05) as significant independent variables relating to behavior of taking medicine. Conclusion: Poor behavior of taking medicine is related to a short dialysis period, poor acceptance and poor satisfaction of dialysis therapy in elderly patients.

It’s developed for Windows Mobile? for use in mobile devices such as PDA’s and Smartphone’s. It enables the creation of four types of records: -Reception;-Temperature;-Cleaning;-Chlorine level in water. It’s possible to export data to common file formats like .pdf or .word. An Android OS version is currently under development.

The homeland security issue that various sovereign states are confronting has become prominent. These changes re-challenge the traditional security concept.
文摘No property may be taken for public use without just compensation.The issue is what is“just compensation”and how it is determined in a particular circumstance.Regardless of the type of valuation,the Greek Constitution(Article 17,paragraphs 2-5)clearly forbids estimation based on possible future projections.In this way,future externalities,positive or negative,deriving from the change of use are not incorporated.This contravenes the Pareto optimal resource distribution and even challenges the Caldor-Hicks compensation criterion,especially in cases of partial takings.We suggest valuation to include a best-case and a worst-case scenario for possible future values.A combination of them,e.g.,the average,will be used to determine benefits and costs and the final level of compensation.The most important externality under consideration,especially in cases of land properties condemned for public large-scale constructions,such as environmental public utilities,should be the environmental effect.The internalisation of possible negative externalities should be internalised by the construction benefactor.
