Mitochondrial dysfunction and oxidative stress are widely regarded as primary drivers of aging and are associated with several neurodegenerative diseases. The degeneration of motor neurons during aging is a critical pathological factor contributing to the progression of sarcopenia. However, the morphological and functional changes in mitochondria and their interplay in the degeneration of the neuromuscular junction during aging remain poorly understood.A defined systematic search of the Pub Med,Web of Science and Embase databases(last accessed on October 30,2024)was conducted with search terms including'mitochondria','aging'and'NMJ'.Clinical and preclinical studies of mitochondrial dysfunction and neuromuscular junction degeneration during aging.Twentyseven studies were included in this systematic review. This systematic review provides a summary of morphological, functional and biological changes in neuromuscular junction, mitochondrial morphology, biosynthesis, respiratory chain function, and mitophagy during aging. We focus on the interactions and mechanisms underlying the relationship between mitochondria and neuromuscular junctions during aging.Aging is characterized by significant reductions in mitochondrial fusion/fission cycles, biosynthesis, and mitochondrial quality control, which may lead to neuromuscular junction dysfunction, denervation and poor physical performance.Motor nerve terminals that exhibit redox sensitivity are among the first to exhibit abnormalities, ultimately leading to an early decline in muscle strength through impaired neuromuscular junction transmission function.Parg coactivator 1 alpha is a crucial molecule that regulates mitochondrial biogenesis and modulates various pathways, including the mitochondrial respiratory chain, energy deficiency, oxidative stress, and inflammation.Mitochondrial dysfunction is correlated with neuromuscular junction denervation and acetylcholine receptor fragmentation, resulting in muscle atrophy and a decrease in strength during aging.Physical therapy, pharmacotherapy, and gene therapy can alleviate the structural degeneration and functional deterioration of neuromuscular junction by restoring mitochondrial function. Therefore, mitochondria are considered potential targets for preserving neuromuscular junction morphology and function during aging to treat sarcopenia. 展开更多

BACKGROUND Aseptic loosening remains the leading cause of revision in primary total hip arthroplasty(pTHA).However, the literature demonstrates significant variability regarding the relative contributions of different factors.AIM To investigate the key determinants of aseptic loosening,we performed a systematic review and meta-analysis.METHODS A comprehensive search of PubMed,Web of Science,EMBASE,and the Cochrane Library was conducted, encompassing studies from database inception to January 1,2025.Meta-analyses were performed to evaluate factors associated with aseptic loosening following pTHA.Inclusion and exclusion criteria were systematically applied at each stage to ensure methodological transparency and reproducibility.Study quality was assessed using standardized categories.Pooled odds ratio(OR)with corresponding 95%confidence interval were calculated with random-or fixed-effects models to generate reliability estimates, and study heterogeneity was visualized using forest plots.Ten factors, categorized into patient-,surgeon-,and device-related domains, were reviewed and meta-analyzed.Funnel plot analysis demonstrated a relatively symmetrical distribution, suggesting minimal publication bias.RESULTS A meta-analysis of 20 studies(520789 participants)found a pooled prevalence of 1.96%.Significant risk factors for aseptic loosening after pTHA included elevated body mass index(OR=1.116,P<0.001),higher Charlson comorbidity index(OR=1.378,P<0.001),prosthesis-related factors(OR=1.497,P<0.001),and adverse lifestyles(OR=2.198,P=0.037).Protective factors were non-white race(OR=0.445,P<0.001)and favorable genetics(OR=0.723,P<0.001).Male sex increased risk(OR=1.232,P=0.016),while age and anatomy were not significant.Surgical expertise showed a slight protective effect(OR=1.048,P<0.001).A comprehensive understanding of the modifiable and non-modifiable factors contributing to aseptic loosening after pTHA requires consideration of patient-related factors, surgical expertise, and prosthesis characteristics.CONCLUSION The identification of these factors is critical for risk mitigation.High-risk patients should receive targeted counseling regarding individualized profiles.Further studies are warranted to establish clearer causal relationships and identify additional contributing factors. 展开更多

Stroke remains a leading cause of death and disability worldwide, and electroacupuncture has a long history of use in stroke treatment. This meta-analysis and systematic review aimed to evaluate the efficacy of electroacupuncture and explore its potential mechanisms in animal models of ischemic stroke. The PubMed,EMBASE,Web of Science,CENTRAL,and CINAHL databases were comprehensively searched up to May 1,2024.This review included articles on preclinical investigations of the efficacy and mechanisms of electroacupuncture in treating ischemic stroke.Data from 70 eligible studies were analyzed in Stata 18.0,using a random-effects model to calculate the standardized mean difference(Hedge’s g).The risk of bias was assessed using RevMan 5.4 software, and the quality of evidence was rated according to the Grading of Recommendations,Assessment,Development, and Evaluation(GRADE)system.Subgroup analyses were conducted to test the consistency of the results and sensitivity analyses were used to assess their robustness. The quality assessment revealed that most studies adequately handled incomplete data and selective reporting. However, several methodological limitations were identified:only 4 studies demonstrated a low risk of allocation concealment,26 achieved a low risk of outcome assessment bias, and 9 had a high risk of randomization bias.Additionally, there was an unclear risk regarding participant blinding and other methodological aspects. The GRADE assessment rated 12 outcomes as moderate quality and 6 as low quality. The mechanisms of electroacupuncture treatment for ischemic stroke can be categorized as five primary pathways:(1)Electroacupuncture significantly reduced infarct volume and apoptotic cell death(P<0.01)in ischemic stroke models;(2)electroacupuncture significantly decreased the levels of pro-inflammatory factors(P<0.01)while increasing the levels of anti-inflammatory factors(P=0.02);(3)electroacupuncture reduced the levels of oxidative stress indicators(P<0.01)and enhanced the expression of antioxidant enzymes(P<0.01);(4)electroacupuncture significantly promoted nerve regeneration(P<0.01);and(5)electroacupuncture influenced blood flow remodeling(P<0.01)and angiogenesis(P<0.01).Subgroup analyses indicated that electroacupuncture was most effective in the transient middle cerebral artery occlusion model(P<0.01)and in post-middle cerebral artery occlusion intervention(P<0.01).Dispersive waves were found to outperform continuous waves with respect to neuroprotection and anti-inflammatory effects(P<0.01),while scalp acupoints demonstrated greater efficacy than body acupoints(P<0.01).The heterogeneity among the included studies was minimal, and sensitivity analyses indicated stable results.Their methodological quality was generally satisfactory. In conclusion, electroacupuncture is effective in treating cerebral ischemia by modulating cell apoptosis, oxidative stress,inflammation,stroke-induced nerve regeneration, blood flow remodeling, and angiogenesis. The efficacy of electroacupuncture may be influenced by factors such as the middle cerebral artery occlusion model, the timing of intervention onset, waveform, and acupoint selection.Despite the moderate to low quality of evidence, these findings suggest that electroacupuncture has clinical potential for improving outcomes in ischemic stroke. 展开更多

In the vast field of medical education, systematic anatomy serves as a solid cornerstone, firmly supporting medical students in their journey towards clinical professional knowledge. It is not just a course but also a key to unlocking the mysteries of medicine. This course is as rich as a vast treasure trove of knowledge, covering the fine structures and functions of various human body systems. Each knowledge point is like a bright pearl in the treasure trove. It is the foundation of clinical professional courses. Many subsequent clinical courses, such as diagnostics and surgery, rely on the basic understanding provided by systematic anatomy. Its professional vocabulary is also an important source of medical field terminologies, acting as the root of medical language and constructing the entire edifice of medical communication.Although the vocabulary of this course is rich,it is not complex. It is like a set of orderly-arranged jigsaw puzzles. As long as the correct methods are mastered,it is easy to piece them together. However,in the current teaching environment, the traditional cramming-style teaching method is like an old-fashioned carriage, showing obvious disadvantages on the road of the times. The pre-study link is like a rehearsal for a wonderful performance, having a direct and crucial impact on students’listening efficiency in class.Nevertheless, such an important teaching link is often overlooked, like a precious treasure forgotten in the corner.High-quality pre-study,for lecturers,is like giving wings to classroom teaching, which can greatly improve the efficiency of classroom teaching and make the teaching process smoother and more efficient. For students,it is like a magnifying glass that can deepen their understanding of knowledge points and their impression of the text, making the knowledge more clearly engraved in their minds. This article will focus on exploring the methods to achieve effective pre-study for systematic anatomy, hoping to contribute to the development of medical education. 展开更多

Objective:To evaluate the dosing, efficacy and safety of the main antileishmanial agents amphotericin B(conventional or liposomal),pentavalent antimonials, miltefosine and paromomycin recommended for the treatment of visceral leishmaniasis in children. Methods:The efficacy and safety of visceral leishmaniasis treatments in children were systematically reviewed using literature from PubMed,Cochrane,clinicaltrials.gov,and Google Scholar, focusing on randomised trials with separate pediatric data(published from 2000-2024).The risk of bias of selected trials was assessed using the revised Cochrane risk-of-bias tool for randomised trials(RoB 2).Reporting was done per the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 checklist. Results:Of 1186 records, only 7 were eligible for qualitative synthesis.Three trials exclusively included children. The treatment regimens studied showed high heterogeneity and lacked sufficient data for a meta-analysis.Most trial arms reported efficacies over 94%for children across different regimens.Miltefosine monotherapy showed the highest rate of late treatment failures, highlighting that allometric dosing is crucial to ensure proper drug exposure in children.Safety data for children were available in only three studies with varied reporting systems of adverse events.Although regimens in this review were generally considered to be safe in children,antimonial-related cardiac toxicity remains a threat. Conclusions:This review highlights the need for pediatric-specific trials, clear presentation of pediatric data, and systematic documentation of adverse events to enhance evidence for policy-making and pediatric guideline development. 展开更多

OBJECTIVE:To update the current best evidence on the effectiveness and safety of cupping therapy in pain management.METHODS:The protocol of this systematic review was registered at PROSPERO(CRD42021261308).An updated literature searching in 7 databases was conducted from January 2014 to January 2023.Two authors extracted data and assessed the risk of bias independently.Statistical analysis was performed using RevMan 5.4.1 software(Cochrane Collaboration,London,UK).Meta-analysis with a random effect model was conducted when there was no serious statistical heterogeneity among trials(I^(2) ≤ 75%).Grading of Recommendations Assessment,Development, and Evaluation was also conducted to assess the quality of evidence.RESULTS:Seventy-two trials with 5720 participants were included. All included trials were assessed as having high risk of bias. The majority of the included trials reported the benefit of cupping plus other therapy or cupping alone on improving cure rate(average risk ratio more than 1.15) and reducing visual analogue scale [average mean difference(MD) reduction 0.16 to 7.0 cm],improving quality of life, quality of sleep or other symptoms related to pain condition. And there was low/very low quality evidence that the incidence of adverse events in the cupping groups were lower than that in the control groups.Although the heterogeneity between studies and the methodological quality of the study itself lead to the low evidence strength of the current conclusions, the results of this study are a valuable supplement to the founding of previous review.CONCLUSION:Cupping therapy alone or combined with other therapy was considered benefit in relieving pain, improving the quality of life, and increasing the cure rate of patients with pain conditions, though supported by the low quality of evidence.According to the limited evidence, cupping therapy seems to have less harm than drugs when treating pain conditions. 展开更多

BACKGROUND Diabetes and hypertension are metabolic disorders that are becoming more prevalent.Breastfeeding is recommended by the World Health Organization for women who have given birth and have been reported to reduce the risk of chronic diseases potentially. However, there is no systematic review to explore the relationship between breastfeeding and diabetes/hypertension by consolidating all available evidence on the topic.AIM To investigate the relationship between breastfeeding and maternal diabetes and hypertension.METHODS This review included comparative studies investigating the association between breastfeeding and the risk of type 2 diabetes or hypertension in parous women.Non-English-language articles, reviews, editorials, letters, and protocols were excluded. The Medline,EMBASE,PubMed,Web of Science, and Cochrane databases were searched until May 6,2024.Risk of bias in non-randomized studies-of exposure was used to assess bias in all the included studies.A meta-analysis was conducted to determine the risk of two chronic lactation-related diseases.RESULTS Eighteen studies were included.Five studies discussed hypertension,12 discussed diabetes, and one discussed both.According to a meta-analysis,lactation significantly reduced the risk of postpartum diabetes in women with gestational diabetes mellitus[hazard ratio=0.64(95%CI:0.47-0.89)].In addition, the meta-analysis produced significant results for postpartum diabetes and hypertension in non-gestational diabetes women, both favoring breastfeeding. However, the risk of bias in most included studies was low or unclear.CONCLUSION Breastfeeding has consistently been shown to be associated with a lower incidence of maternal hypertension and diabetes later in life, even among mothers who experienced gestational diabetes during pregnancy. 展开更多

Background and Objective Patients with glioma experience a high symptom burden and have diverse palliative care needs. However, the assessment scales used in palliative care remain non-standardized and highly heterogeneous. To evaluate the application patterns of the current scales used in palliative care for glioma,we aim to identify gaps and assess the need for disease-specific scales in glioma palliative care. Methods We conducted a systematic search of five databases including PubMed,Web of Science,Medline,EMBASE,and CINAHL for quantitative studies that reported scale-based assessments in glioma palliative care.We extracted data on scale characteristics, domains, frequency, and psychometric properties.Quality assessments were performed using the Cochrane ROB 2.0 and ROBINS-I tools. Results Of the 3,405 records initially identified,72 studies were included. These studies contained 75 distinct scales that were used 193 times.Mood(21.7%),quality of life(24.4%),and supportive care needs(5.2%)assessments were the most frequently assessed items, exceeding half of all scale applications.Among the various assessment dimensions, the Distress Thermometer(DT)was the most frequently used tool for assessing mood, while the Short Form-36 Health Survey Questionnaire(SF-36)was the most frequently used tool for assessing quality of life. The Mini Mental Status Examination(MMSE)was the most common tool for cognitive assessment.Performance status(5.2%)and social support(6.8%)were underrepresented.Only three brain tumor-specific scales were identified.Caregiver-focused scales were limited and predominantly burden-oriented.Conclusions There are significant heterogeneity, domain imbalances, and validation gaps in the current use of assessment scales for patients with glioma receiving palliative care. The scale selected for use should be comprehensive and user-friendly. 展开更多
