Impact of depression on functional status in elderly patients undergoing total knee arthroplasty

BACKGROUND The prevalence of depressive symptoms in patients undergoing total knee arthroplasty(TKA)ranges from 22%to 26%.The impact of depression on functional status post-TKA remains controversial.AIM To be the first study in Latin American population to evaluate the association between depression and functional status one year after TKA,hypothesizing that elderly patients with depression will demonstrate lower rates of functional improvement.METHODS We conducted an observational,descriptive and analytic,retrospective cohort study involving patients over 65 years old who were indicated for TKA.Assessments were made via the Program for Determination and Management of Risks for Practices and Procedures of the Division of Geriatric Medicine at the Italian Hospital of Buenos Aires,between June 2015 and July 2019.Depression screening was conducted using Yesavage’s abbreviated score and Patient Health Questionnaire-9,while functional ability was evaluated using the Knee Society Score(KSS).RESULTS Of the 100 patients analyzed,22(22%)screened positive for depression.The mean age was 80 years±6.3 years,with an average of 77.6 years±6 years in the depressed group and 80.6 years±6.3 years in the non-depressed group(P=0.05).Depressed patients showed significantly greater cognitive impairment[clock-face drawing test median:5(3-6)vs 6(5-7),P=0.06]and more risk factors for confusional syndrome(mean:8±2 vs 6.5±2.2,P=0.006).Frailty was also more prevalent in depressed patients[Edmonton:15(68%)vs 33(42%),P=0.05;Fried:17(77%)vs 42(54%),P=0.05].Postoperative Functional KSS were similar between groups(depressed:65±22.1 vs non-depressed:66.3±20.3,P=0.8).Linear regression analysis revealed no association between depression and changes in KSS.Spearman’s rank correlation coefficients were-0.0304(P=0.8)for Functional KSS variation and-0.1(P=0.3)for KSS variation.CONCLUSION Depression in patients with osteoarthritis should not hinder surgical planning.Identifying and treating depression preoperatively may enhance outcomes such as pain relief and reduce risks of acute confusional syndrome,cognitive impairment,and frailty. 展开更多

The introduction of Buddhism to Tibet started the Sinicization of Buddhism forming a Buddhist doctrine with distinct Tibetan characteristics of China,during which the spread and development of Buddhist arts,especially the statue making promoted.Buddhist statues are not only an important part of Tibetan Buddhist culture but also reflect the religious and social contexts of the time.A numerous Buddhist statues were carved across Tibet during the Qing Dynasty,which exemplify the model of combining religious and secular affairs and enriches the artistic forms of Tibetan Buddhist statues.Referencing to the Autobiography of the 5th Dalai Lama:The Silk Cloth and Biography of 6th Dalai Lama Tsangyang Gyatso:Gold Ear of Grain written by Disi Sangye Gyatso,this article conducts a comparative study on the rock curving Buddha statues and stupas constructed in the time,concludes that the carving statues during 5th Dalai Lam and 6th Panchen Lama aimed to achieve social stability and well-being of the people.This paper also explores the initial period of this phenomenon based on the carving records,provides references in the academic research on the history of carving Buddha statues across Tibet in the 17th century. 展开更多

L-arabinose is a newly developed low-caloric monosaccharide, which has many biomedical and health effects, especially intestinal sucrase inhibition effect. L-arabinose is mainly produced by chemical or enzymatic hydrolysis of hemicellulose. The taste of L-arabinose is quite similar to that of sucrose, with approximately 50% of the sweetness. As a functional additive, L-arabinose can be used in medical and pharmaceutical applications for the treatment of diseases such as diabetes, chronic constipation, mineral absorption disorder and secondary bile acid formation disorder. However, L-arabinose has not been widely used in functional foods due to high price and lack of publicity and guidance. A comprehensive review of L-arabinose physicochemical properties, production, applications field, market statue and development direction is presented in this paper. 展开更多

The way monuments hold sway seem stable and predictable,yet their‘meanings’migrate and their traditionally accepted roles shift.In South Africa there are monuments that commemorate achievements once considered laudable,that have become symbols of oppressive systems.The challenge facing the current generation is to make sense of these monuments in a way that will dignify the lived situation of all stakeholders.One such contested monument is the statue of President M.T.Steyn that used to stand in front of the Main Building of the University of the Free State.The statue divided the campus community.It provoked some students to violent unrest,while others saw it as an important heritage marker.How could the seemingly irreconcilable conflict between these stakeholders be resolved while adhering to heritage laws and without alienating a significant portion of the student body?The complex process that followed relied on creative leaps to provoke public participation and stimulate new forms of appreciation amid shifting regions of concern.One important facet of this process was the construction of a reflective triangular prism installation which provoked new visual,experiential and spatial relationships with the monument,including the visual removal of the statue from the‘axis of power’,the celebration of oblique views,and opportunities to face the statue in a distorted mirror.This article will critically reflect on the poetic process of strange-making that was followed in the relocation of the President M.T.Steyn statue in order to illustrate how traditional conflict resolution strategies can be reinterpreted in terms of German philosopher,Martin Heidegger’s notion of inceptual thinking.The resulting approach opens new possibilities for creatively rescripting contested monuments;a way of thinking which engages the inceptual regionings impacting the way people live as beings of emplaced care. 展开更多
