不同剂量托法替布对中重度类风湿关节炎患者DAS28评分及血清学指标的影响

目的分析不同剂量托法替布对中重度类风湿关节炎(rheumatoid arthritis,RA)患者28个关节的肿胀数和压痛数表(Disease Activity Score 28,DAS28)评分及血清学指标的影响。方法选取于2021年8月至2024年3月收治的136例中重度RA患者作为研... 目的分析不同剂量托法替布对中重度类风湿关节炎(rheumatoid arthritis,RA)患者28个关节的肿胀数和压痛数表(Disease Activity Score 28,DAS28)评分及血清学指标的影响。方法选取于2021年8月至2024年3月收治的136例中重度RA患者作为研究对象。用随机数表法将患者分为低剂量组(68例)和高剂量组(68例)。2组均给予托法替布联合甲氨蝶呤治疗,低剂量组患者每日服用5 mg托法替布,高剂量组每日服用20 mg托法替布,2组服用甲氨蝶呤的剂量一致。2组患者均遵循既定流程,完成3个月连续治疗。于治疗前后,系统统计关节疼痛、肿胀评分及DAS28评分,记录晨僵时间,并监测C反应蛋白(Creactive protein,CRP)、红细胞沉降率(erythrocyte sedimentation rate,ESR)及血清细胞因子白细胞介素-35(interleukin-35,IL-35)、白细胞介素-17(interleukin-17,IL-17)、白细胞介素-6(interleukin-6,IL-6)、干扰素-γ(interferon-gamma,IFN-γ)水平,同时记录不良反应的发生情况。结果治疗后,2组患者的关节疼痛评分、关节肿胀评分、DAS28评分、晨僵持续时间均降低(缩短),且高剂量组的关节疼痛评分、关节肿胀评分、DAS28评分、晨僵持续时间显著低(短)于低剂量组(P<0.05);治疗后,高剂量组和低剂量组患者的ESR、CRP、IL-17、IL-6、IFN-γ水平均下降,IL-35水平均上升,且高剂量组患者的ESR、CRP、IL-17、IL-6、IFN-γ水平均显著低于低剂量组,IL-35水平显著高于低剂量组(P<0.05);治疗后,高剂量组和低剂量组患者的血常规、血脂、肝肾功能与治疗前比较差异均无统计学意义,且2组间血常规、血脂、肝肾功能比较差异均无统计学意义(P>0.05),未观察到其他不良反应。结论采用更高剂量的托法替布能够有效缓解RA患者的临床症状,并降低CRP及ESR水平。高剂量治疗还能够促进抗炎细胞因子IL-35的表达上调,同时有效抑制促炎细胞因子IL-17、IL-6及IFN-γ的过度产生,尽管剂量升高,但并不影响患者的血常规、血脂、肝肾功能,且未观察到其他不良反应。 展开更多

Background:Insufficient physical activity and prolonged sedentary behavior have emerged as major global public health challenges.Short bouts(≤10 min)of accumulated exercise(SBAE)throughout the day may be a promising strategy to mitigate the adverse effects of prolonged sitting and promote physical activity,ultimately promoting overall health.However,previous ambiguity in defining this concept has resulted in a fragmented and inconsistent evidence base,impeding practical applications,the development of guidelines,and policymaking.The purpose of this study is to establish an operational definition of SBAE by synthesizing systematic reviews and research trials alongside an expert consensus.Additionally,it seeks to evaluate acute and long-term efficacy and feasibility,providing evidence-based recommendations for practice and future research directions.Methods:A literature search was performed across PubMed and Web of Science,followed by systematic screening and summarization of eligible studies based on predefined inclusion criteria.Inclusion criteria encompassed various modes/types of SBAE(bouts lasting≤10 min,performed multiple times daily with≥30 min intervals);both aerobic and resistance exercise were considered.Relevant systematic reviews and research trials were included.Methodological quality,risk of bias,and evidence certainty were assessed.Expert consensus was obtained through a survey to evaluate recommendations and agreement levels on findings.Results:After analyzing 27 systematic reviews,135 research studies,and an expert consensus involving 48 researchers from 11 countries,SBAE is defined as any exercise mode of activity,regardless of intensity,that is accumulated in either continuous or intermittent bouts lasting≤10 min per session(including multiple intermittent sets)that are performed multiple times(≥2 sessions/day)per day,with intervals of≥30 min between bouts or otherwise sufficient time for recovery.When used to interrupt prolonged periods of sedentary time,SBAE mitigates the acute adverse effects of sedentary behavior on more than 10 clinical biomarkers of endocrine,cardiovascular,and brain health/function among adults of diverse ages and conditions.Moreover,SBAE was superior for improving acute glycemic control compared to a single continuous exercise session.As a long-term intervention(average of 11 weeks),SBAE can improve over 20 health outcomes,including peak oxygen uptake,resting blood pressure,and metabolic health.Additionally,SBAE might be more effective than continuous exercise for improving longer-term glycemic control and body composition.Long-term completion rates for SBAE interventions are generally high(95%),with low dropout rates(12%)and high adherence rates even without supervision(85%),and its safety has been preliminarily validated.Conclusion:An operational definition of SBAE is provided along with its classification and acute and long-term efficacy.Practical exercise prescription recommendations and evidence-based strategies for various populations and contexts are provided.Future research should focus on generating high-quality evidence for SBAE in 5 key areas:quantification and monitoring,population-specific responses,optimization of exercise prescriptions,intervention efficacy,and practical implementation.Additionally,addressing policy,environmental,and promotional barriers is crucial for transitioning from expert consensus to public consensus,and for facilitating the application of this strategy in real-world environments. 展开更多

分布式光纤测井DAS低频数据处理模型研究

分布式光纤生产测井DAS(Distributed Acoustic Sensing,DAS)信号定量计算井筒内流体速度一直是研究焦点,本文提出一种CNN-FIR(Convolutional Neural Network-finite Impulse Response,CNN-FIR)滤波方法,实现对分布式光纤测井DAS低频数... 分布式光纤生产测井DAS(Distributed Acoustic Sensing,DAS)信号定量计算井筒内流体速度一直是研究焦点,本文提出一种CNN-FIR(Convolutional Neural Network-finite Impulse Response,CNN-FIR)滤波方法,实现对分布式光纤测井DAS低频数据滤波。在数据处理阶段,将原始DAS数据转换到频率-波数域,并将零频率和零波数分量移动到中心;在特征提取模块,首次引入粒子群算法(Particle Swarm Optimization,PSO)优化单类支持向量机(One-class Support Vector Machine,OCSVM)超参数,利用其超平面完成特征提取;在特征增强模块,通过分析特征上噪声信息的规律,创建模式化的规则来完成初步特征强化,再使用样本训练的支持向量机分类器(Support Vector Classifier,SVC)完成进一步的特征增强;最终得到了分布式光纤测井DAS低频数据处理模型。并根据模型,设计了一种计算流体速度的计算方法,准确计算出了井筒内流体速度值,精度达到95%以上。通过实例,论证了该模型计算DAS低频数据的科学性和有效性。 展开更多

为提高煤矿有毒有害气体CH_(4)和CO的实时动态监测能力,基于直接激光吸收光谱(DAS)与变分模态分解法(VMD),研究CH_(4)和CO双组份气体同步在线监测系统,并进行试验验证。针对CH_(4)和CO痕量气体,选用中心波长为1653.4 nm和2325.2 nm的2... 为提高煤矿有毒有害气体CH_(4)和CO的实时动态监测能力,基于直接激光吸收光谱(DAS)与变分模态分解法(VMD),研究CH_(4)和CO双组份气体同步在线监测系统,并进行试验验证。针对CH_(4)和CO痕量气体,选用中心波长为1653.4 nm和2325.2 nm的2台分布式反馈激光器,采用时分复用(TDM)技术,构建双组份痕量气体同步在线监测系统,克服双激光器工作时的相互干扰;优化VMD方法,实现信号分解和噪声抑制,提高检测系统的信噪比;搭建煤自燃在线监测实验平台,开展煤自燃长时间的在线监测试验。研究结果表明:降噪后CH_(4)和CO的探测极限分别为9.4×10^(-6)%与9.9×10^(-6)%,CH_(4)和CO检测极限降幅为38.4%,39.2%;所构建系统在煤自燃过程中对CH_(4)和CO体积分数变化具有良好的跟踪能力与检测可靠性。研究结果可为煤矿灾害气体的高精度、高稳定性实时监测提供可靠的技术手段,提高煤自燃早期预警能力。 展开更多

目的:探讨28关节疾病活动评分(DAS28)对应关节的高频超声评分与类风湿性关节炎(RA)病情活动程度的关系及临床意义。方法:选取2021年9月—2024年1月就诊于高州市人民医院的RA患者123例,根据DAS28将患者分为缓解组(25例)、活动组(98例)。... 目的:探讨28关节疾病活动评分(DAS28)对应关节的高频超声评分与类风湿性关节炎(RA)病情活动程度的关系及临床意义。方法:选取2021年9月—2024年1月就诊于高州市人民医院的RA患者123例,根据DAS28将患者分为缓解组(25例)、活动组(98例)。比较两组的一般资料、临床资料和高频超声评分,分析高频超声评分与RA病情活动程度的关系及临床意义。结果:两组性别、体质量指数、发病关节比较,差异无统计学意义(P>0.05);活动组年龄、C反应蛋白(CRP)、血沉(ESR)、骨侵蚀评分、关节积液评分、滑膜内血流信号评分、滑膜厚度评分均高于缓解组,病程长于缓解组,差异有统计学意义(P<0.05);经Pearson相关性分析,骨侵蚀、关节积液、滑膜内血流信号、滑膜厚度评分与RA病情活动程度呈正相关(P<0.05);骨侵蚀、关节积液、滑膜内血流信号、滑膜厚度评分联合诊断RA的AUC为0.896,灵敏度为92.54%,优于各指标单独预测,差异有统计学意义(P<0.05)。结论:高频超声评分与RA病情活动程度呈正相关,且高频超声评分联合诊断RA患者病情活动程度的灵敏度好,临床应用价值也较高。 展开更多

地铁隧道沿线入侵事件造成地铁线路变形破坏,地下管道沿线人为破坏、野蛮施工引发安全事故与财产损失,有必要在隧道沿线与地下管道沿线进行监测与预警,及时发现入侵破坏处,进行阻止或者及时开展补救维修降低事故损失。分布式光纤声波传... 地铁隧道沿线入侵事件造成地铁线路变形破坏,地下管道沿线人为破坏、野蛮施工引发安全事故与财产损失,有必要在隧道沿线与地下管道沿线进行监测与预警,及时发现入侵破坏处,进行阻止或者及时开展补救维修降低事故损失。分布式光纤声波传感(DAS)技术,具有动态在线监测、大范围密集测量、方便布设免维护等独特优势。通过在地下轨道交通、地下管道沿线布设分布式光纤,采用DAS技术对光纤沿线外界扰动进行监测,针对传统的时频分析算法不适用分析DAS信号,笔者采用基于优化S变换的时频分析方法,将DAS信号变换到时间-频率域,根据不同振动信号的时频特征差异,在时间频率域对不同振动信号进行分类识别、精确定位,并对超过阈值的侵入信号进行预警。通过数值模拟分析以及实际地铁线路区间DAS监测实验,结果表明基于优化S变换的DAS监测方法技术,对不同主频振动信号具有较强识别监测能力,并且具有监测距离长、响应及时等特点,可以实现长距离、大范围密集测量、动态、实时监测预警,为城市地下管道、地下轨道交通沿线监测提供了一种快速准确的解决方案。 展开更多

复杂黄土塬对地震波能量的吸收衰减一直是地球物理学者关注的焦点之一,其地震资料分辨率和信噪比提升困难,难以满足老油区油藏精细描述和剩余油挖潜的需求。为此,在应用常规井中地震和地面地震资料解释技术的基础上,综合利用DAS 3D-VSP... 复杂黄土塬对地震波能量的吸收衰减一直是地球物理学者关注的焦点之一,其地震资料分辨率和信噪比提升困难,难以满足老油区油藏精细描述和剩余油挖潜的需求。为此,在应用常规井中地震和地面地震资料解释技术的基础上,综合利用DAS 3D-VSP成像数据,开展面向油藏精细描述的配套解释技术研究,形成了基于井地联采数据的解释性预处理、储层预测、缝网刻画等技术系列。在鄂尔多斯盆地东部某区块,应用基于井地联采数据的叠后自适应频谱拓宽高分辨率处理技术,地震资料主频可提高5 Hz,频带拓宽25 Hz;基于DAS 3DVSP数据的波形指示薄储层反演技术能够准确识别厚度3~5 m单砂体,各层系验证井综合符合率达到86.32%;基于倾角方位扫描约束的缝网识别技术实现了低序级断裂—缝网的有效预测;基于DAS井地联采数据,形成了一套井地+井震油藏精细描述技术,储层刻画精度提升15%。应用结果表明,基于DAS井地联合勘探的配套技术具有推广价值。 展开更多

某储气库注采井投入生产后经历多周期注采,长期承受交变载荷变化影响,其油管与油套环空、油套与技套环空、技套与表套环空可能会出现起压现象;同时,各级环空长期承受高压,易破坏井筒密封完整性,造成注入天然气窜漏,影响井控安全和正常... 某储气库注采井投入生产后经历多周期注采,长期承受交变载荷变化影响,其油管与油套环空、油套与技套环空、技套与表套环空可能会出现起压现象;同时,各级环空长期承受高压,易破坏井筒密封完整性,造成注入天然气窜漏,影响井控安全和正常生产。造成各级环空起压的原因多达十几种,该储气库目前仅依靠注采井压力、温度,以及环空保护液液面监测数据无法准确判断泄漏点和具体泄漏途径。为了精准治理和有效管控环空起压情况,引入基于DTS与DAS技术的光纤测试方法。文章介绍了DTS与DAS光纤测试技术原理,以及该技术在几口典型井中的应用情况。应用结果显示,利用该技术可有效判断漏点位置,为下一步对症治理环空起压现象提供支持,对其他储气库环空起压井的治理也有一定的参考意义。 展开更多

目前为了解决市场主流基于声发射、次声波、低频电磁波等原理的地面定点标记方法无法满足内检测器分布式实时跟踪需求的问题,利用管道同沟敷设光纤以及基于分布式光纤传感的光纤传感解调设备,结合“端、边、云”物联网技术架构,研发了... 目前为了解决市场主流基于声发射、次声波、低频电磁波等原理的地面定点标记方法无法满足内检测器分布式实时跟踪需求的问题,利用管道同沟敷设光纤以及基于分布式光纤传感的光纤传感解调设备,结合“端、边、云”物联网技术架构,研发了一套管道内检测器跟踪定位系统。同时,制定了一套管道内检测器跟踪定位的现场操作技术流程,解决了由于同沟光纤缠绕导致的光纤皮长与管道里程不一致,从而引发定位精度低的问题。在西南地区某天然气集输管道内检测作业现场进行的应用试验表明,该系统能够实现内检测器的分布式、高精度、实时跟踪定位。试验结果显示,系统推断内检测器通过的时间与MARK盒记录的时间差值小于3 s,根据各区间内检测器的运行速度,定位误差范围为9.84 m以内。 展开更多

水平井油水两相流入DAS响应特征实验研究

为明确水平井生产过程中不同地层流体流入井筒时的声波响应特征,解决分布式光纤声波传感(Distributed-fiber Acoustic Sensing,DAS)数据在产出剖面解释中面临的信号复杂、定量识别困难等问题,通过设计并开展室内物理模拟实验,系统研究... 为明确水平井生产过程中不同地层流体流入井筒时的声波响应特征,解决分布式光纤声波传感(Distributed-fiber Acoustic Sensing,DAS)数据在产出剖面解释中面临的信号复杂、定量识别困难等问题,通过设计并开展室内物理模拟实验,系统研究了不同产量、含水率及流体黏度条件下径向流入水平井筒过程中产生的声波信号特征。实验模拟了油、水两相流在多种工况下的流动状态,采用DAS系统采集相应声波数据,并综合运用快速傅里叶变换(Fast Fourier Transform,FFT)与数字滤波技术对原始信号进行处理,以提取不同模拟条件下的有效频幅特征参数。在此基础上,分析了水平井两相流入过程中DAS信号的响应规律,绘制了声波能量与流体径向流入速度之间的关系图版,并通过数据回归方法,首次构建了综合考虑黏度、流速与含水率这3种因素协同作用的DAS响应模型库,实现了不同模拟工况下两相流入速度与DAS响应之间的定量化表征。研究结果表明:①DAS信号的能量强度与流体径向流入速度呈显著正相关,且在特定频带范围内对流速变化尤为敏感;②含水率的变化对声波频谱结构具有明显影响,不同含水条件下主频特征存在可识别的差异;③流体黏度的增加会改变声波信号的衰减特性,进而影响DAS响应幅值;④所建立的响应模型库能够有效反映多因素耦合作用下DAS响应与流入速度之间的量化关系,回归模型的决定系数R^(2)均大于0.9。该研究建立的考虑多因素协同的DAS响应模型库,为基于DAS数据的水平井油水两相流产出剖面定量反演提供了可靠的模型基础与方法支撑,对推动DAS技术在水平井生产动态监测中的精准应用具有重要价值。 展开更多

OBJECTIVE:In recent years,the number of clinical research reports on acupuncture and manipulation for the treatment of greater occipital neuralgia has gradually increased,but the quality is uneven.There is currently no literature evaluating the quality of published reports,which is not conducive to the promotion of clinical use of these therapies.Therefore,this article assessed the reporting quality of randomized controlled trials on acupuncture and manipulation for greater occipital neuralgia.METHODS:Cochrane Library,PubMed,Web of Science,Embase,China National Knowledge Infrastructure(CNKI),VIP,WanFang Data,and Chinese BioMedical Literature Database(CBM)from inception to May 20,2024 were searched.The reporting quality of included randomized controlled trials was independently evaluated by two investigators using the CONSORT statement,STRICTA checklist,and Cochrane bias of risk assessment tool.A third investigator resolved any disagreement.RESULTS:A total of 62 articles were included.Based on the CONSORT statement,59.46%(22/37)of all entries had a reporting rate of less than 50%,mainly including“Identification as a randomized trial in the title(1/62,1.61%),”“How sample size was determined(7/62,11.29%),”“Implementation(1/62,1.61%),”“Blinding(1/62,1.61%),”and“Reports of Funding(4/62,6.45%).”According to the STRICTA checklist,29.41%(5/17)of all entries had a reporting rate of less than 50%,mainly including“Details of other interventions(7/58,12.07%),”“Setting and context of treatment(0/58,0%),”and“Description of participating acupuncturists(0/58,0%).”CONCLUSION:The reporting quality of randomized controlled trials on acupuncture and manipulation therapy for greater occipital neuralgia remains low.Future researchers need to make greater efforts to strictly adhere to the CONSORT statement and STRICTA checklist during trial design,implementation,and reporting.This will facilitate the standardization of research in this field and enhance the reliability and reproducibility of the research results. 展开更多

An EDCC-EMD analysis-based network for DAS VSP data denoising in frequency domain

Distributed acoustic sensing(DAS)has rapidly emerged as a transformative technology in seismic exploration,particularly in vertical seismic profiles(vsP).However,the acquired vsP data suffer from strong coherent DAs coupling noise and random noise.Current deep learning denoising methods,dependent on noise labels derived from conventional denoising techniques,fall short in addressing the unique noise properties inherent in DAS data.To address this challenge,we propose an exponential decay curve-constrained empirical mode decomposition(EDcc-EMD)analysis-based supervised denoising network.Our method begins with extracting the initial noise from the field DAs vsP data through the traditional EMD method.Despite containing some signal leakage,this noise is further processed through EMD to derive intrinsic mode functions(IMFs).We,then,analyze the correlation coefficients between these IMFs and the initial noise,applying an exponential decay curve(EDC)law to isolate pure noise.This refined noise data serves as accurate labels,enhancing the denoising network's precision.Meanwhile,most of the methods usually consider the t-x domain features and ignore the important frequency-domain features.Consequently,we train our network with frequency-domain data instead of time domain data,capitalizing on the more distinct separation of noise and signal characteristics,thereby facilitating more effective noise-signal discrimination.The experimental results demonstrate that our method significantly enhances the denoising performance and successfully recovers weak signals. 展开更多

Internal audit plays a strategic role in ensuring the reliability of government financial statements,which are essential tools for achieving public financial accountability and transparency.This study aims to analyze the role of internal audit in improving the quality of government financial reporting using a literature review approach based on agency theory.The method employed is a systematic literature review of 12 scholarly articles(7 national and 5 international),as well as relevant books and official reports published within the last five years(2019-2024).The findings indicate that an effective and independent internal audit can reduce information asymmetry between the government(agent)and the public(principal),enhance compliance with Government Accounting Standards(SAP),and strengthen internal controls and financial governance.This study concludes that a competent internal audit function is a critical prerequisite for producing reliable,relevant,and accountable financial statements.The findings support agency theory and offer key recommendations for strengthening internal audit practices in the public sector. 展开更多
