Objective: to study the methods and countermeasures of rational utilization of emergency medical resources based on severity grading. Methods: data of 4134 patients treated in the emergency department of our hospital from May 2019 to May 2020 were retrospectively analyzed. According to severity grading, statistics of patient treatment, diagnosis, costs, etc. Results: the difference of emergency diagnosis items among each group was statistically significant (P < 0.05), the severity of the disease was directly proportional to the number of emergency diagnosis items, and the proportion of drugs for non-emergency patients in emergency treatment cost was relatively high, the difference was statistically significant (P < 0.05). Conclusion: non-emergency patients occupy a certain amount of emergency medical resources. Hospitals should make rational use of the severity grading system and carry out differentiated diagnosis, treatment, examination to ensure the rational use of emergency medical resources. 展开更多

Objective:To assess whether there is a correlation between CT scan severity score and pulmonary function in Interstitial Lung Disease, and provide a more reliable basis for clinical diagnosis and treatment.Materials and Methods:Sixty patients with clinical diagnosis of Interstitial Lung Disease(ILD)were collected, and chest CT and pulmonary function tests were performed at the same time. Here DLCO%and FEV1%were used as the pulmonary function indexes. The severity of the patients was assessed by CT scores as mild(range,1~10),moderate(range 11~20),and severe(range 21~30).Correlation analysis was carried out between CT score and pulmonary function index, and the lung function parameters of patients at three grades were compared between groups and pairwise among the means. Results:As per the CT severity classification, there were 13 mild cases,31 moderate cases, and 16 severe cases.CT score was negatively correlated with DLCO%and FEV1%.R value was-0.814 and-0.797,respectively;The comparison of the mean value of DLCO%and FEV1%among the three groups and the pairings of the mean value of DLCO%and FEV1%were statistically significant. Conclusion:There was good correlation between CT score of ILD and the pulmonary function index. The pulmonary function index of patients with different CT severity grading was statistically significant, which can provide a new basis for the clinical evaluation and diagnosis of the disease. 展开更多

Objective To determine the correlation between traditional Chinese medicine(TCM)inspec-tion of spirit classification and the severity grade of depression based on facial features,offer-ing insights for intelligent intergrated TCM and western medicine diagnosis of depression. Methods Using the Audio-Visual Emotion Challenge and Workshop(AVEC 2014)public dataset on depression, which conclude 150 interview videos, the samples were classified ac-cording to the TCM inspection of spirit classification:Deshen(得神,presence of spirit),Shaoshen(少神,insufficiency of spirit),and Shenluan(神乱,confusion of spirit).Meanwhile, based on Beck Depression Inventory-II(BDI-II)score for the severity grade of depression, the samples were divided into minimal(0-13,Q1),mild(14-19,Q2),moderate(20-28,Q3),and severe(29-63,Q4).Sixty-eight landmarks were extracted with a ResNet-50 network, and the feature extracion mode was stadardized.Random forest and support vectior machine(SVM)classifiers were used to predict TCM inspection of spirit classification and the severity grade of depression, respectively.A Chi-square test and Apriori association rule mining were then applied to quantify and explore the relationships. Results The analysis revealed a statistically significant and moderately strong association be-tween TCM spirit classification and the severity grade of depression,as confirmed by a Chi-square test(χ^(2)=14.04,P=0.029)with a Cramer’s V effect size of 0.243.Further exploration us-ing association rule mining identified the most compelling rule:“moderate depression(Q3)→Shenluan”.This rule demonstrated a support level of 5%,indicating this specific co-occur-rence was present in 5%of the cohort.Crucially,it achieved a high Confidence of 86%,mean-ing that among patients diagnosed with Q3,86%exhibited the Shenluan pattern according to TCM assessment. The substantial Lift of 2.37 signifies that the observed likelihood of Shenlu-an manifesting in Q3 patients is 2.37 times higher than would be expected by chance if these states were independent-compelling evidence of a highly non-random association.Conse-quently,Shenluan emerges as a distinct and core TCM diagnostic manifestation strongly linked to Q3,forming a clinically significant phenotype within this patient subgroup. 展开更多

Repeated blast impacts on personnel in explosive environments can exacerbate craniocerebral trauma. Most existing studies focus on the injury effects of a single blast, lacking in-depth analysis on the injury effects and cumulative effects of repeated blasts. Therefore, rats were used as the experimental samples to suffer from explosion blasts with different peak air overpressures(167 kPa~482 kPa)and varying number of repeated blasts. The cumulative effect of craniocerebral trauma was most pronounced for moderate repeated blast, showing approximately 95%increase of trauma severity with penta blast, and an approximately 85%increase of trauma severity with penta minor blast. The cumulative effect of craniocerebral trauma from severe, repeated blast has a smaller rate of change compared to the other two conditions. The severity of trauma from penta blast increased by approximately 69%compared to a single blast.Comprehensive physiological, pathological and biochemical analysis show that the degree of neurological trauma caused by repeated blasts is higher than that of single blasts, and the pathological trauma to brain tissue is more extensive and severe. The trauma degree remains unchanged after double blast, increases by one grade after triple or quadruple blast, and increases by two grades after penta blast. 展开更多

AIM:To evaluate chronic ocular sequelae in patients with symblepharon caused by ocular burns and propose an objective grading system.METHODS:This was a retrospective,single-center clinical study.Patients with symblepharon caused by ocular burns at least six months later were assessed.Chronic ocular sequelae were classified into 3 categories(eyelid, conjunctiva, and cornea)and 9 chronic ocular sequelae[friction factors, exposure factors, conjunctival hyperemia, length of symblepharon, scope of adhesion, lacrimal area adhesion, loss of the palisades of Vogt(POV),corneal neovascularization, and corneal opacification].Each ocular sequela was graded from 0 to 3,depending on the increasing severity. The 9 ocular sequelae were evaluated to obtain the total severity score for each eye.The total severity score was defined as Grade I(1-9),Grade II(10-18),and Grade III(19-27).Moreover,the correlation between the severity of chronic ocular sequelae and visual acuity, surgical strategy, and the prognosis was analyzed, respectively.RESULTS:Cases of 79 eyes with symblepharon caused by ocular burns were included in this study.Of these,20(25.32%)were defined as Grade I,43(54.43%)as Grade II,and 16(20.25%)as Grade III.Eyes with a high total severity score had reduced visual acuity, required complicated surger y strategies, and poor prognosis(P<0.001).Multivariate regression analysis showed that the scope of adhesion, corneal opacification, and corneal neovascularization significantly affected visual acuity, surgical strategy, and prognosis(all P<0.001).CONCLUSION:The evaluation of chronic ocular sequelae enabled the development of an objective grading system for patients with symblepharon caused by ocular burns. This grading system can be applied to guide the treatment and predict the prognosis. 展开更多
