Fatal bleeding in patients with tracheostomy is an uncommon incident, in the order of 0.1% to 1% of cases. It is secondary in 70% of cases to the innominate artery fistula in the tracheal lumen. It is secondary in 70% of cases to the innominate artery fistula in the tracheal lumen. The fistula is generally created after the necrosis of tracheal rings. The incriminated factors are the type of cannula used, the site of tracheal opening below the 3rd tracheal ring, tracheal infections and the proximity of the innominate artery to the tracheal axis. The outcome of this incident is fatal in most cases. We report the case of a 59-year-old patient with a tracheostomy on the 11th day of his admission to intensive care for severe head trauma secondary to a road accident. On the 22nd day of his admission, the occurrence of a cataclysmic and fatal haemorrhage through the tracheostomy tube evoked an arterio-tracheal fistula of the innominate artery.展开更多
Background: Unsafe abortion (USA) is defined as the termination of a non-desired pregnancy, performed by an unqualified person or in an environment without minimum medical standards, or both. It can impact negatively ...Background: Unsafe abortion (USA) is defined as the termination of a non-desired pregnancy, performed by an unqualified person or in an environment without minimum medical standards, or both. It can impact negatively the fertility in the future by tubo-peritoneal lesions. The current study aimed to highlight the infertility factors associated with USA. Material and Methods: From January 1st 2000 to December 31st 2021, a total of 3876 patients that sought care for infertility in the University Clinic of Kinshasa and the Edith Medical Center were enrolled. The socio-demographic, clinical and paraclinical variables were recorded and analyzed in relation to USA history. Results: The patients’ average age was 33.4 ± 5.7 years. Half of them were housewives;about sixty percent of the patients were nulliparous (59.31%). Seventy percent had a secondary infertility with 43.33% having a history of unsafe abortion. The main diagnosis was genital infections and uterus myoma. The chronic endometritis, bilateral tubal occlusion and Uterine synechiae were more associated with USA group. The spermogram anomalies and other partners’ lesions seemed of infectious origin. Conclusion: The USA remains a more frequent phenomenon in our setting. The chronic endometritis, bilateral tubal occlusion, uterine synechiae and indirectly uterus myoma were the infertility factors related to USA. The genital infection of any origin seems to be the main denominator underlying the female and the male infertility in our setting.展开更多
Minimal cut sets (or prime implicants: minimal combinations of basic event conditions leading to system failure) are important information for reliability/safety analysis and design. To obtain minimal cut sets for ...Minimal cut sets (or prime implicants: minimal combinations of basic event conditions leading to system failure) are important information for reliability/safety analysis and design. To obtain minimal cut sets for general non-coherent fault trees, including negative basic events or multi-valued basic events, a special procedure such as the consensus rule must be applied to the results obtained by logical operations for coherent fault trees, which will require more steps and time. This paper proposes a simple method for a non-coherent fault tree, whose top event is represented as an AND combination of monotonic sub-trees. A "monotonic" sub-tree means that it does not have both positive and negative representations for each basic event. It is proven that minimal cut sets can be obtained by a conventional method for coherent fault trees. An illustrative example of a simple event tree analysis shows the detail and characteristics of the proposed method.展开更多
Geochemical data of fifteen Cretaceous sediment samples from Kumba area in the Douala sub-basin are presented to determine the provenance, source rock weathering, tectonic setting and paleo-oxidation conditions of the...Geochemical data of fifteen Cretaceous sediment samples from Kumba area in the Douala sub-basin are presented to determine the provenance, source rock weathering, tectonic setting and paleo-oxidation conditions of the depositional setting of these rocks. For this purpose, the whole-rocks were analyzed for their major and trace element, including rare earth elements (REEs), contents by ICP-AES and ICP-MS methods respectively. On the basis of their major element composition, the rocks have been classified mainly as Fe-shale, shale, arkose and Fe-sandstone. For the provenance, the plot of Zr vs. TiO2, Y/Ni vs. Cr/V, TiO2 vs. Al2O3 diagrams, high LREE/HREE ratios (5.84 to 20.91) and negative and positive Eu anomalies (Eu/Eu*= 0.87 to 1.62) suggest that the studied rocks were mainly derived from felsic igneous rocks with lesser contribution of mafic components. The higher values of paleo-weathering indices such as Plagioclase Index of Alteration (PIA) and Chemical Index of Weathering (CIW), ranging from 79.63 to 99.90 and 87.57 to 99.92 respectively, suggest that the sediments and their potential source rocks experienced intense weathering. Whereas the variable Chemical Index of Alteration (CIA) values (63.01 to 99.50) coupled with high content of K2O (up to 7.5) in most samples could be indicative of K-addition diagenetic processes. The A-CN-K plot also suggests a possible K-addition. The redox conditions during the sedimentation were suboxic to oxic as evidenced by the Ce anomalies of 0.88 to 1.03. This is also confirmed by the values of Ni/Co (less than 7), U/Th (less than 1.25) and Cu/Zn (mostly less than 1). Tectonic discrimination diagrams (e.g., SiO2-K2O/Na2O and Th-Sc-Zr/10) show that these rocks were deposited mainly in an active continental margin setting, and in various tectonic environments. This reflects probably the recycling effect experienced by the samples studied.展开更多

They impose on practitioners optimal diagnostic methods such as telemedicine in order to be able to quickly detect anomalies for daily care and...Cardiovascular diseases are the origin of many causes of death worldwide. They impose on practitioners optimal diagnostic methods such as telemedicine in order to be able to quickly detect anomalies for daily care and monitoring of patients. The Electrocardiogram (ECG) is an examination that can detect abnormal functioning of the heart and generates a large number of digital data which can be stored or transmitted for further analysis. For storage or transmission purposes, one of the challenges is to reduce the space occupied by ECG signal and for that, it is important to offer more and more efficient algorithms capable of achieving high compression rates, while offering a good quality of reconstruction in a relatively short time. We propose in this paper a new ECG compression scheme that is based on a subset of signal splitting and 2D processing, the wavelet transform (DWT) and SPIHT coding which has proved their worth in the field of signal processing and compression. They are exploited for decorrelation and coding of the signal. The results obtained are significant and offer many perspectives.展开更多
