A clinical nomogram for predicting major adverse cardiovascular and cerebrovascular events in elderly Chinese patients with acute coronary syndrome undergoing percutaneous coronary intervention:development and validation in a real-world cohort

BACKGROUND As the global population ages,the number of elderly patients with acute coronary syndrome(ACS)rises.However,prognostic assessment tools for elderly patients with ACS remain lacking,particularly in the Chinese population.This study aimed to develop and validate a nomogram to predict 2-year major adverse cardiovascular and cerebrovascular events(MACCE)in elderly Chinese patients with ACS.METHODS A retrospective analysis was conducted using two independent cohorts of ACS patients aged≥65 years who underwent percutaneous coronary intervention:the derivation cohort(n=1674)and the validation cohort(n=2333).Candidate predictors were selected using multivariable Cox proportional hazards regression and the Akaike information criterion.A final nomogram incorporating ten variables was constructed.Model performance was evaluated in terms of discrimination[concordance index(C-index)and area under the receiver operating characteristic curve(AUC)]and calibration(calibration plots).RESULTS The 2-year incidence of MACCE was 12.5%(n=210)in the derivation cohort and 15.6%(n=364)in the validation cohort.The nomogram demonstrated good discrimination,with C-index values of 0.727 and 0.661 and AUCs of 0.723 and 0.699 in the derivation cohort and the validation cohort,respectively;significantly outperforming the GRACE risk score(P<0.001).Calibration plots showed good agreement between the predicted and observed outcomes.Patients classified as the high-risk group by the nomogram had a significantly higher MACCE incidence compared to that of the low-risk group(log-rank P<0.001).CONCLUSIONS This newly developed nomogram provides a reliable tool for individualized prediction of the 2-year MACCE risk in elderly Chinese patients with ACS who underwent percutaneous coronary intervention.It outperformed the GRACE score in both discrimmination and calibration and may help improve clinical decision-making and personalized risk stratification in this vulnerable population. 展开更多

Through a comprehensive review of the development status of workover technology of PetroChina Company Limited(PetroChina), this paper presents the connotation of workover operation under the background of the new era, the latest progress of workover operation in the respects of equipment, tools, technology and the construction of information and standardization. The gaps between PetroChina and foreign counterpart in workover technology are as follows: the level of automation and intellectualization of tools and equipment is relatively low, the snubbing operation in gas wells characterized by HT/HP and high H2 S is lagged behind;water plugging in the long horizontal wellbore needs to be further developed, coiled tubing and its relevant equipment for ultra-deep well operation has to be optimized;informationization, standardization and big data application of workover operation need to be started. Based on this as well as the development status of workover technology in China and the technical difficulties faced in the future, eight suggestions for future development are put forward:(1) strengthen the dynamic understanding of reservoir and improve the pertinence of workover schemes;(2) develop the general overhaul technology in a systematical way to tackle issues of seriously problematic wells;(3) put more efforts into the research of horizontal well workover operation and develop relevant technology for long horizontal section operation;(4) improve the snubbing technology and extend its applications;(5) expand the capacity of coiled tubing operation and improve the level of special operations;(6) develop automatic workover technology into the field of artificial intelligence;(7) promote clean operation in an all-round way and build green oil and gas fields;(8) perfect the informationization construction to realize the sharing of workover resources. 展开更多

A two streams system seems to exist for tactile perception.We asked kindergarten children to identify(semantic)and describe the function of grasped unseen instruments of their daily life(fork,knife,spoon,brush,toothbrush,pencil,and scissor),and then,to simulate its use(pragmatic).The capacity to describe the function of the set of instruments was significantly superior to that of identifying them by name,although there was a significant direct association between identification and description of the function in the responses to the set of instruments.The simulation of the instrument function was of two types:(i)use―the child simulated how the instrument is grasped and used;and(ii)incorporation―the child simulated the function of the instrument with her/his own hand.The results support the hypothesis of a two streams tactile system(semantic and pragmatic),and that kindergarten children have the ability to incorporate functional properties of instruments of their daily life,probably as a support for language development. 展开更多

In this paper, we identify a set of factors that may be used to forecast software productivity and software development time. Software productivity was measured in function points per person hours, and software development time was measured in number of elapsed days. Using field data on over 130 field software projects from various industries, we empirically test the impact of team size, integrated computer aided software engineering (ICASE) tools, software development type, software development platform, and programming language type on the software development productivity and development time. Our results indicate that team size, software development type, software development platform, and programming language type significantly impact software development productivity. However, only team size significantly impacts software development time. Our results indicate that effective management of software development teams, and using different management strategies for different software development type environments may improve software development productivity. 展开更多

Healthcare waste management (HCWM) is an important aspect of healthcare delivery globally because of its hazardous and infectious components that have potential for adverse health and environmental impacts. The paper introduces a set of indicators for assessing HCWM systems in hospitals. These indicators are: HCWM policies and standard operating procedures, management and oversight, logistics and budget support, training and occupational health and safety, and treatment, disposal and waste treatment equipment housing. By plotting a mark on a continuum which is defined as good and poor on the extremes and is connected with all other marks in a spoke arrangement, it’s possible to describe a baseline for HCWM in any specific hospital. This baseline can be used to improve awareness of the actors and policy-makers, compare the same hospital at a different point in time, to compare observations by different evaluators and to track improvements. Results suggest that in Kenya, the application of such indicators is useful for evaluating which priorities should be addressed to improve outcomes in HCWM systems. Systematic sampling technique was used to identify and collect data by use of observational checklist, interviews, visual verification and review of documents and a HCWM assessment tool. The objective is to suggest an integrated management tool as a method to identify prevailing problems with a HCWM system. The method can be replicated in other contexts worldwide, with a focus on the developing world. The integrated indicators focus on management of HCW and not its potential impact on human health and environment, an area recognized to be critical for future research. 展开更多

目的:通过量化分析国家层面发布的公立医院高质量发展的政策文件,了解政策工具的使用情况与分布特点,为优化政策工具提供依据。方法:基于ROTHWELL与ZEGVELD的政策工具理论,构建“基本政策工具类型”与“公立医院高质量发展任务”二维分... 目的:通过量化分析国家层面发布的公立医院高质量发展的政策文件,了解政策工具的使用情况与分布特点,为优化政策工具提供依据。方法:基于ROTHWELL与ZEGVELD的政策工具理论,构建“基本政策工具类型”与“公立医院高质量发展任务”二维分析框架,采用内容分析法和统计分析法,对2021-2024年相关国家政策文本进行分析。结果:2021年政策工具使用频次最高,达115次(83.3%),2022年和2023年逐年下降,2024年未出台新政策。环境型政策工具的使用最为广泛,共99次(约71.7%),供给型工具次之,需求型工具使用频次最少。不同公立医院高质量发展任务的政策工具使用情况差异具有统计学意义(P<0.01)。结论:对照公立医院高质量发展任务,环境型政策工具仍是主力,而公立医院实际所需的人才、技术、资金等供给型和需求型政策工具支持相对较少。研究结果提示,国家和地方政策制定者要制定多层级的政策体系,差异化使用3类基本政策工具,提高政策连续性、协同性、均衡性和适配性,助力公立医院实现高质量发展的各项任务和目标。 展开更多

钻井工程作为油气开发的核心环节,正面临深水、超深井及非常规资源开发带来的多工况耦合与高风险挑战。智能钻完井技术的发展推动钻井装备与人工智能、数字孪生、实时测控和自主决策工控系统的深度融合,加速了钻井作业向自动化与自治化... 钻井工程作为油气开发的核心环节,正面临深水、超深井及非常规资源开发带来的多工况耦合与高风险挑战。智能钻完井技术的发展推动钻井装备与人工智能、数字孪生、实时测控和自主决策工控系统的深度融合,加速了钻井作业向自动化与自治化演进。本文系统综述了智能钻机及其配套装备的关键进展,包括井口自动化系统、智能钻头与钻杆、随钻测传导工具/仪器以及智能工控与决策系统等模块,阐述其工作原理、工程应用与发展趋势。研究表明,智能钻完井技术通过实时感知、参数自适应优化与闭环控制实现了地面装备与井下工具的协同运行,显著提高了机械钻速轨迹控制精度和风险预警能力。然而,当前领域仍面临协议标准分散、井下-地面链路稳定性不足、执行机构响应受限、模型可解释性不足以及系统集成度不高等深层瓶颈。未来研究需在协议统一、链路强化、装备与工具性能提升、可信作业文化构建以及可解释小模型与跨设备数据融合等方向协同突破,形成覆盖钻进、导向与风险防控的全流程自动驾驶钻井体系,推动智能钻完井技术向规模化与高等级自主化阶段发展。 展开更多
