The CIDARS plays an important role in and has been integrated into the routine outbreak monitoring efforts of the Center for Disease Control(CDC) at all levels in China. In the CIDARS, thresholds are determined using the ?Mean+2SD? in the early stage which have limitations. This study compared the performance of optimized thresholds defined using the ?Mean +2SD? method to the performance of 5 novel algorithms to select optimal ?Outbreak Gold Standard(OGS)? and corresponding thresholds for outbreak detection. Data for infectious disease were organized by calendar week and year. The ?Mean+2 SD?, C1, C2, moving average(MA), seasonal model(SM), and cumulative sum(CUSUM) algorithms were applied. Outbreak signals for the predicted value(Px) were calculated using a percentile-based moving window. When the outbreak signals generated by an algorithm were in line with a Px generated outbreak signal for each week, this Px was then defined as the optimized threshold for that algorithm. In this study, six infectious diseases were selected and classified into TYPE A(chickenpox and mumps), TYPE B(influenza and rubella) and TYPE C [hand foot and mouth disease(HFMD) and scarlet fever]. Optimized thresholds for chickenpox(P_(55)), mumps(P_(50)), influenza(P_(40), P_(55), and P_(75)), rubella(P_(45) and P_(75)), HFMD(P_(65) and P_(70)), and scarlet fever(P_(75) and P_(80)) were identified. The C1, C2, CUSUM, SM, and MA algorithms were appropriate for TYPE A. All 6 algorithms were appropriate for TYPE B. C1 and CUSUM algorithms were appropriate for TYPE C. It is critical to incorporate more flexible algorithms as OGS into the CIDRAS and to identify the proper OGS and corresponding recommended optimized threshold by different infectious disease types.展开更多

Accidents threaten the water quality of the 1085 km 2 surface area of the TGRA and millions of local people if oil and chemical leakage were to occur. A water pollution management system for emergency response (WPMS ER) was therefore designed for the management of pollution in this area. An integrated geographic information system (GIS)-based water pollution management information system for the TGRA, called WPMS ER TGRA, was developed in this study. ArcGIS engine was used as the system development platform, and Visual Basic as the programming language. The models for hydraulic and water quality simulation and the generation of body-fitted coordinates were developed and programmed as a dynamically linked library file using Visual Basic, and they can be launched by other computer programs. Subsequently, the GIS-based information system was applied to the emergency water pollution management of a shipwreck releasing 10 tons of phenol into the Yangtze River during two hours. The results showed that WPMS ER TGRA can assist with emergency water pollution management and simulate the transfer and diffusion of accidental pollutants in the river. Furthermore, it can quickly identify the affected area and how it will change over time within a few minutes of an accident occurring.展开更多

Based on the construction of a monitoring system for HSR earthquake emergency response, the technical operational procedures for HSR seismic emergency response are proposed. The quantity, scale, and location of HSR earthquake emergency response mechanism are defined, and the corresponding emergency response system is built. In particular, the earthquake emergency response system can conduct real-time continuous dynamic monitoring of seismic activity along the railway. When earthquake occurs, the intensity of the ground motion is detected by the system. When the earthquake monitoring value reaches the earthquake alarm threshold, it will send an alarm signal to the dispatch center, and the emergency power supply will be forced to cut off. The earthquake emergency response system will continue to monitor the follow-up ground motion acceleration. The system provides the operation scheduling center with a basis for train operation control to resume operation after stopping. The monitoring result of the system reduces the disaster, and the secondary disaster is caused by the earthquake. This paper improves the HSR response mechanism in detecting earthquake disasters. The result improves the ability of HSR to deal with earthquake disasters, and reduces casualties and economic and property loss caused by earthquake disasters.展开更多

文摘Objectives:This study aimed to study the characteristics of in-hospital deterioration in patients with congenital heart disease who required rapid response system activation and identify risk factors associated with 1-month mortality. Methods:We retrospectively analysed data from a Japanese rapid response system registry with 35 participating hospitals. We included consecutive patients with congenital heart disease who required rapid response system activation between January 2014 and March 2018.Logistic regression analyses were performed to examine the associations between 1-month mortality and other patient-specific variables. Results:Among 9,607 patients for whom the rapid response system was activated, only 82(0.9%)had congenital heart disease.Only few patients with congenital heart disease were being treated at the cardiology and cardiovascular surgery departments(12.3%and 9.9%,respectively).Moreover,the incidences of rapid-response events after intensive care unit discharge or surgery were low(6.8% and 12.2%,respectively).The most common reason for rapid response system activation was respiratory dysfunction(desaturation:35.4%,tachypnoea:25.6%,and new dyspnoea:19.5%).Rapid response system interventions and intensive care unit transfers were required for 65.9% and 20.7%of patients, respectively. The mortality rate was 1.2% at the end of the rapid response system intervention and 11.0% after 1 month. Moreover, decreased respiratory rate and decreased heart rate at rapid response system activation were associated with increased 1-month mortality. The adjusted odds ratio was 1.10(95% confidence interval 1.02–1.19)and 1.02(95% confidence interval,1.00–1.04) for respiratory rate and heart rate, respectively. Conclusions:Rapid response systems were rarely activated after cardiac surgery and intensive care unit discharge, which were situations with a high risk of sudden deterioration in patients with congenital heart disease. Therefore, encouraging the use of the rapid response system in these departments will enable intervention by a third, specialised team for in-hospital emergencies and help provide comprehensive medical care to patients.Furthermore,1-month mortality was associated with vital signs at rapid response system activation. These findings may guide treatment selection for patients with congenital heart disease showing deterioration.

文摘This paper studies the Least Square Method to define high-speed railway(HSR) earthquake risk and solve the problem of its emergency response mechanism. The result improves the ability of HSR to deal with earthquake disasters, and reduces casualties and economic and property loss caused by earthquake disasters.

文摘Background:Acupuncture has shown potential therapeutic benefits for individuals with simple obesity. However, some researchers argue that some of the effectiveness of acupuncture may be due to the placebo response. Objective:To understand the placebo response of acupuncture treatment in simple obesity,a systematic review and meta-analysis was designed based on the comparison between sham acupuncture before and after treatment.Search strategy:Eight databases(Pub Med,Web of Science,Cochrane Library,Embase,China National Knowledge Infrastructure,Wanfang Database,China Biology Medicine Database, and Chinese Scientific Journals Database)were searched from inception to August 1,2023.The Me SH search terms comprised obesity and acupuncture.Inclusion criteria:Randomized controlled trials(RCTs)using sham or placebo acupuncture as a control in treating obesity were enrolled.Data extraction and analysis:Two researchers independently extracted data, and the results were crosschecked after completion. Each RCT’s detailed sham/placebo acupuncture treatment protocol was assessed according to the SHam Acupuncture REporting guidelines. The revised Cochrane risk-of-bias tool for randomized trials and the Grading of Recommendations Assessment,Development, and Evaluation(GRADE)system were used to determine the risk of bias and quality of evidence, respectively.Body mass index(BMI)was defined as the primary outcome.Anthropometric parameters and laboratory test parameters related to obesity were defined as secondary outcomes. We used standardized mean difference(SMD)with 95%confidence interval(CI)to calculate treatment effects of outcomes. Results:Fifteen RCTs with a total of 1250 patients were included. The BMI significantly decreased after treatment in the sham acupuncture group compared to baseline(SMD 0.37,95%CI 0.09–0.66;I2=81%,random model;P<0.01).Treatment duration(P=0.02)and other interventions significantly impacted the placebo response rate(P=0.00).Conclusion:The placebo response of sham acupuncture was strong in the RCTs for simple obesity, and the effect sizes differed between various outcomes. The treatment duration and other interventions emerged as potential influencing factors for the placebo response of sham acupuncture.

文摘Urbanization is one of the most extreme forms of land-use alteration that is advancing across the world with unprecedented speed. As the largest developing country,China has developed a unique path through its high speed and large scale of urbanization, offering valuable research opportunities for avian ecology. However,a comprehensive review on how birds respond to urbanization in China is still lacking. Here,we systematically reviewed 274 studies published from 1962 to 2024 to determine the research trends, current insights, and future directions of avian response to urbanization in China. We synthesized research trends across four core avian response dimensions to urbanization—diversity, behavior, physiology, and life-history—and their applications in conservation strategy design. The number of publications in avian response to urbanization in China increased annually, and it is influenced by China's developing policies of urbanization. The results also showed an unbalanced geographical pattern of the publications,as the research preferences are relatively prevalent in the developed areas of eastern China. In contrast, there are insufficient studies in the emerging urbanizing areas in the western and northeastern China.Regarding the research contents, most existing studies are focusing on the patterns of bird diversity, while there are few studies on the underlying mechanisms, such as physiological adjustments and life-history strategies. In addition, passerines are the most frequent ones among the studied species.Integrating multidimensional urbanization indices and citizen science data are gradually becoming a new trend in recent years. Our study emphasizes that future studies should pay more attention to the response mechanism of birds in urbanizing processes, multidimensional and interdisciplinary studies, and the transformation of the research results into conservation practices.
文摘The penetration rate of new wind and photovoltaic energy in the power system has increased significantly, and the dramatic fluctuation of the net load of the grid has led to a severe lack of flexibility in the regional grid. This paper proposes a hierarchical optimal dispatch strategy for a high proportion of new energy power systems that considers the balanced response of grid flexibility.Firstly, various flexibility resource regulation capabilities on the source-load side are analyzed, and then flexibility demand and flexibility response are matched, and flexibility demand response assessment is proposed;then,a hierarchical optimal dispatch model of the grid taking flexibility adjustment capability into account is established, and the upper model optimizes the net load curve with the objectives of minimizing the fluctuation of the net load, maximizing the benefits of energy storage and controllable loads, and optimizing the flexibility adjustment capability. The upper layer model optimizes the net load curve by minimizing net load fluctuation, maximizing energy storage and controllable load revenue, and optimizing flexibility adjustment capability. In contrast, the lower layer model optimizes the power allocation of thermal power units and regulates the lost load of wind and solar power generation by minimizing the total system operating cost. The results show that the proposed strategy improves the flexibility of the grid by 15.2%,gives full play to the regulation capability of each flexibility resource, and reduces the fluctuation of the net load by 15.6%to achieve optimal coordination between different types of flexibility resources.

文摘Biomechanical study of the visual system by ocular response analyzer investigates the inter-structural biological relationships, mechanics, and function of the visual system. This review aimed to investigate the changes in corneal biomechanical parameters with age and sex.The articles published in PubMed between 2000 and 2021 were investigated and critiqued, and valid scientific evidence was collected, reviewed and concluded according to the inclusion and exclusion criteria. Most studies showed that corneal biomechanical changes occur infrequently in children up to the age of 20y,and with increasing age and wider age range, there was a significant decrease in corneal biomechanical indices, especially corneal hysteresis. In children and adults, most studies have shown that these biomechanical indicators, especially corneal resistance factor, were higher in females.Although hormonal changes may contribute to this finding, the role of axial length and other biometric indicators should not be ignored. The axial length, the intraocular pressure, and the corneal thickness are other factors associated with biomechanical parameters that should be taken into account in clinical diagnosis and management especially for patients undergoing refractive surgery as well as keratoconus patients.

文摘The demand response(DR)market,as a vital complement to the electricity spot market, plays a key role in evoking user-side regulation capability to mitigate system-level supply‒demand imbalances during extreme events. While the DR market offers the load aggregator(LA)additional profitable opportunities beyond the electricity spot market,it also introduces new trading risks due to the significant uncertainty in users’behaviors.Dispatching energy storage systems(ESSs)is an effective means to enhance the risk management capabilities of LAs;however, coordinating ESS operations with dual-market trading strategies remains an urgent challenge. To this end, this paper proposes a novel systematic risk-aware coordinated trading model for the LA in concurrently participating in the day-ahead electricity spot market and DR market, which incorporates the capacity allocation mechanism of ESS based on market clearing rules to jointly formulate bidding and pricing decisions for the dual market.First, the intrinsic coupling characteristics of the LA participating in the dual market are analyzed, and a joint optimization framework for formulating bidding and pricing strategies that integrates ESS facilities is proposed.Second,an uncertain user response model is developed based on price‒response mechanisms, and actual market settlement rules accounting for under-and over-responses are employed to calculate trading revenues, where possible revenue losses are quantified via conditional value at risk.Third,by imposing these terms and the capacity allocation mechanism of ESS,the risk-aware stochastic coordinated trading model of the LA is built, where the bidding and pricing strategies in the dual model that trade off risk and profit are derived. The simulation results of a case study validate the effectiveness of the proposed trading strategy in controlling trading risk and improving the trading income of the LA.
