Comparative Study on the Efficacy of Closed Reduction and Kirschner Wire Fixation versus Open Reduction and Plate Fixation in the Treatment of Hand Surgery Fractures

Objective: To investigate the therapeutic advantages of closed reduction and Kirschner wire fixation versus open reduction and plate fixation in patients with hand surgery fractures. Methods: The sample was collected from May 2021 to May 2025, consisting of 80 patients with hand surgery fractures. These patients were randomly divided into two groups using the red and blue ball method: the plate fixation group (40 cases, treated with open reduction and plate fixation) and the Kirschner wire fixation group (40 cases, treated with closed reduction and Kirschner wire fixation). The therapeutic effects between the two groups were randomly compared. Results: The Kirschner wire fixation group outperformed the plate fixation group in all indicators except for hand function scores (p < 0.05). There was no statistically significant difference in hand function scores between the two groups (p > 0.05). Conclusion: Compared with open reduction and plate fixation, closed reduction and Kirschner wire fixation for patients with hand surgery fractures achieves a more pronounced therapeutic effect, with advantages such as less trauma, shorter operation time, less bleeding, and a lower incidence of complications. It is suitable for hand surgery fractures with good stability. Open reduction and plate internal fixation have greater advantages in complex fractures and cases requiring high stability, and are worthy of promotion and application. 展开更多

With the wide application of information technologi es , Internet-based remote diagnosis (IRD) of plant will surely become the main se rvice mode of corporations in the future. Therefore, it has received a great dea l recognition from academia and the industry. The IRD technology, which is based upon database, computer, and network technologies is the focus of correlative r esearch all over the world. Although some scientific institutions have developed primary IRD systems, their functions are quite narrow with many shortcomings. I n short, the standard of remote diagnosis has not been created, in particula r the network manufacture and diagnostic resources cannot be shared extensiv ely, so the systemic open character has not been developed enough to create the efficiency and the scope of the existing remote diagnostic systems. Aimed at improving the limitations mentioned above, in this paper we try to develop Web -CORBA open remote diagnosis architecture over the Internet, which is based on the existing troubleshooting software. We have designed and realized the general -purpose data interface of the software and some Java applets functional module s. Thereby the software’s open character and application field get widened. Our current research is focused on building the remote intelligent diagnosis center on the basis of the Web-CORBA architecture. Some basic functions have been app lied. Further, we put forward the project of combining the enterprise interior d iagnostic system to the remote diagnosis center, and make a study of building th e remote diagnosis object network architecture. The open remote diagnostic syste m based on Internet offers many advantages. Also, with the CORBA distributed obj ect technology, the distributed diagnostic resources can be connected as a coope rative diagnosis object system, in order that all the resources can be shared to the full extent, and the advanced diagnostic technique can be applied as soon a s possible. A remote open-path laser-induced breakdown spectroscopy(LIBS) system was designed and studied in the present work for the purpose of combining the LIBS technique with the steel production line. In this system, the relatively simple configuration and optics were employed to measure the steel samples at a remote distance and a hot sample temperature. The system has obtained a robustness for the deviation of the sample position because of the open-path and alloptical structure. The measurement was carried out at different sample temperatures by placing the samples in a muffle furnace with a window in the front door. The results show that the intensity of the spectral lines increased as the sample temperature increased. The influence of the sample temperature on the quantitative analysis of manganese in the steel samples was investigated by measuring ten standard steel samples at different temperatures. Three samples were selected as the test sample for the simulation measurement. The results show that, at the sample temperature of 500 ℃, the average relative error of prediction is 3.1% and the average relative standard deviation is 7.7%, respectively. 展开更多

Tracing Escaping Structure in the Northern Indo-China Peninsula by Openness and Remote Sensing

It facilitates the interpretation of detailed landforms on the Earth's surface. Compared with the layer stacking of ETM+ with less three-dimensionality and visualizability and with indefinite details of linear images in the deep cutting or deep covered region, the Openness is used for accurate interpretation of tectonic geomorphic units and linear structures. In this paper, the ETM+ images(741 RGB) and RRIM based on Openness combined with the field geological investigation are used to trace the escaping structure in SE Asia. The east boundary is Ailaoshan shear zone and the west boundary is Uttaradit-Dien Bien Phu fault, which together form the southwards extruding wedge block. The arc boundary surface of the southern Khorat Plateau is jutted to the north. The NW and NE sides of Khorat Plateau are traversed by Uttaradit-Dien Bien Phu fault and Thakhek-Da Nang fault, respectively, resulting in a blocked escaping structure. The SE margins of Truong Son structure belt and Song Ma structure belt are both arcs jutting to SE. These arc structures clamped by faults or related to the fault on one side indicating the material flow direction obviously, are the most specific manifestation of escaping structures. Moreover, these push units are extruded from south to north successively. 展开更多

There were 35 men and 28 women with a sex ratio of 1.25. The mean age was 35.36 ± 10.16 years with extremes ranging from 19 to 62 years. The typical presentation was pain on defecation. The majority of fissures were located at the posterior commissure. The average duration of the procedure was longer in patients in group 1 (15.34 minutes) compared to 5.22 minutes in patients in group 2. We found 3.12% of surgical wound infections in patients in group 1 and neither patient in group 2. An open-plus-closed-loop (OPCL) control problem for the chaotic motion of a 3D rigid pendulum subjected to a constant gravitationM force is studied. The 3D rigid pendulum is assumed to be consist of a rigid body supported by a fixed and frictionless pivot with three rotational degrees. In order to avoid the singular phenomenon of Euler's angular velocity equation, the quaternion kinematic equation is used to describe the motion of the 3D rigid pendulum. An OPCL controller for chaotic motion of a 3D rigid pendulum at equilibrium position is designed. This OPCL controller contains two parts: the open-loop part to construct an ideal trajectory and the closed-loop part to stabilize the 3D rigid pendulum. We prove that the interior stresses within both a non-parabolic open inhomogeneity and another interacting non-elliptical closed inhomogeneity can still remain constant when the matrix is simultaneously under the action of a screw dislocation and uniform remote anti-plane stresses.The constancy of interior stresses is realized through the construction of a conformal mapping function for the doubly connected domain occupied by the surrounding matrix.The mapping function is endowed with the information describing the screw dislocation via the incorporation of two specifically defined logarithmic terms.The constant interior stress fields are observed to be independent of the specific open and closed shapes of the two inhomogeneities and the existence of the screw dislocation.In contrast,the existence of the neighboring screw dislocation significantly affects the open and closed shapes of the two inhomogeneities. 展开更多

Free torsion of thin-walled structures of open- and closed-sections is a classical elastic mechanics problem, which, in literature, is often solved by the method of membrane analogy. The method of membrane analogy, however, can be only applied to structures of a single material. If the structure consists of both open- and closed-sections, the method of membrane analogy is difficult to be applied. In this paper, a new method is presented for solving the free torsion of thin-walled structures of open- and/or closed- sections with multiple materials. By utilizing a simple statically indeterminate concept, torsional equations are derived based on the equilibrium and compatibility conditions. The method presented here not only is very simple and easy to understand but also can be applied to thin-walled structures of combined open- and closed-sections with multiple materials. 展开更多

露天煤矿开采和生态修复共同驱动矿山生态系统发生显著变化,新时期矿山土地复垦与生态修复监测评价将生态系统退化和恢复状况作为必选指标,但缺少有效监测方法。利用哨兵卫星数据反演矿山生态系统植被和土壤参数,并基于生态位原理开发... 露天煤矿开采和生态修复共同驱动矿山生态系统发生显著变化,新时期矿山土地复垦与生态修复监测评价将生态系统退化和恢复状况作为必选指标,但缺少有效监测方法。利用哨兵卫星数据反演矿山生态系统植被和土壤参数,并基于生态位原理开发露天煤矿生态系统多维超体积分割方法,将其应用到准能集团露天煤矿2018—2024年生态系统状态变化监测中。结果表明:(1)综合利用哨兵卫星的极化、光谱和纹理特征变量,可以准确有效估测关键植被和土壤参数;(2)随着生态系统参数数量的增加,不同生态系统状态空间的Jaccard相似系数降低,在超体积维数为3时,各生态系统状态空间的重叠度小,可分性高,植被覆盖度、群落结构指数、地面粗糙度、土壤湿度和土壤有机质是超体积分割的关键参数;(3)基于多维超体积分割,可以有效识别露天煤矿生态系统状态变化区域及幅度,生态系统状态跃迁幅度越大,监测精度越高,总体精度超过80%;(4)研究区2018—2024年剥离区和开采区生态系统状态以负向跃迁为主,排土区和复垦区生态系统状态以正向跃迁为主,表明准能集团露天煤矿有效实施了边采边复对策。总体来看,基于哨兵卫星数据和多维超体积分割方法可以实现对露天煤矿开采前、中、后生态系统退化和恢复状况的全过程监测,今后应加强多源多时相遥感数据的融合与协同分析,实现露天煤矿生态系统状态、结构和功能的一体化监测。 展开更多

龙砂开阖枢针法联合来曲唑、人绒毛膜促性腺激素治疗多囊卵巢综合征不孕临床研究

目的研究龙砂开阖枢针法联合来曲唑(letrozole,LE)、人绒毛膜促性腺激素(human chorionic gonadotropin,HCG)治疗多囊卵巢综合征(polycystic ovary syndrome,PCOS)不孕的效果。方法纳入2021年6月—2022年10月无锡市中医医院收治的PCOS患... 目的研究龙砂开阖枢针法联合来曲唑(letrozole,LE)、人绒毛膜促性腺激素(human chorionic gonadotropin,HCG)治疗多囊卵巢综合征(polycystic ovary syndrome,PCOS)不孕的效果。方法纳入2021年6月—2022年10月无锡市中医医院收治的PCOS患者84例,通过随机数字表法分为对照组(42例)与针灸组(42例)。对照组予以LE、HCG治疗,针灸组在对照组基础上接受龙砂开阖六气针法治疗,均治疗6个月经周期。比较两组治疗前、治疗6个月经周期后子宫内膜指标、性激素、代谢指标,治疗6个月经周期后疗效,治疗期间不良反应及妊娠情况。结果较治疗前,治疗6个月经周期后两组子宫内膜厚度(endometrial thickness,ET)、子宫内膜分型A型比例,血清雌二醇(estradiol,E2)水平升高,且针灸组更高(P<0.05)。较治疗前,治疗6个月经周期后,两组搏动指数(pulsing index,PI)、阻力指数(resistance index,RI)、血清睾酮(testosterone,T)、促黄体素(luteinizing hormone,LH)、卵泡刺激素(follicle stimulating hormone,FSH)、空腹血糖(fasting blood glucose,FPG)、胰岛素(insulin,INS)、胰岛素抵抗指数(insulin resistance index,HOMA-IR)、甘油三酯(triglyceride,TG)、总胆固醇(total cholesterol,TC)降低,且针灸组更低(P<0.05)。与对照组治疗6个月经周期后的疗效比较,针灸组临床疗效优于对照组(P<0.05)。治疗期间,两组均未因治疗出现明显不良反应而终止治疗,两组不良反应发生率比较,14.29%(6/42)vs 9.52%(4/42),差异无统计学意义(P>0.05);针灸组生化妊娠率、临床妊娠率发生率更高(P<0.05)。结论龙砂开阖枢针法联合LE、HCG治疗PCOS不孕,可改善子宫内膜指标,调节性激素、代谢指标,最终改善妊娠情况,疗效确切,安全性良好。 展开更多

超深层碳酸盐岩储层参数三维表征及裂缝启闭规律研究

超深层碳酸盐岩储层所处地质环境复杂,非均质性极强,导致储层物性参数及裂缝启闭规律精确表征极其困难,直接影响碳酸盐岩油气高效开发效果。选取塔里木盆地富满油田M井区为研究对象,在断层约束的结构框架下,以地震解释层面作为空间约束... 超深层碳酸盐岩储层所处地质环境复杂,非均质性极强,导致储层物性参数及裂缝启闭规律精确表征极其困难,直接影响碳酸盐岩油气高效开发效果。选取塔里木盆地富满油田M井区为研究对象,在断层约束的结构框架下,以地震解释层面作为空间约束构建三维构造模型;采用变差函数分析与序贯高斯模拟(SGS)建立孔隙度、渗透率及弹性参数三维模型,基于修正的弹簧模型研究地应力场;以裂缝法向应力与孔隙压力关系定义裂缝启闭系数Kc,实现超深层碳酸盐岩储层物性参数及裂缝启闭规律三维量化研究。研究结果表明:研究区构造起伏大,且(O_(3)t)上奥陶统吐木休克组顶界面构造起伏明显,走滑断层主要发育于中部构造隆起区;储层孔隙度为0~5.29%,渗透率为0~15.13mD;随着深度的增加,储层岩石的泊松比逐渐减小,而弹性模量逐渐增大且分布差异较大;最大和最小水平主应力随着深度的增加而增大,且地应力变化幅度较大;储层裂缝法向应力与裂缝启闭系数Kc均随着深度的增加而增大,O_(3)t—Z_(1)层启闭系数小、裂缝更易开启;Z_(2)—O_(2)y(中奥陶统一间房组)层启闭系数相对较大、裂缝易闭合,开启程度相对低。 展开更多
