文摘目的分析病种分值付费(diagnosis-intervention packet,DIP)背景下福建医科大学附属龙岩第一医院住院病案首页填写的问题,并提出针对性对策,以提升病案首页数据质量,确保DIP分组的准确性和医保支付的合理性。方法采用便利抽样法,抽取2022年1月—2023年12月福建医科大学附属龙岩第一医院300份住院病案,对病案首页的主要诊断、手术操作、国际疾病分类(international classification of diseases,ICD)编码、患者基本信息等项目进行逐项核查。统计病案首页填写问题的分布情况、科室分布情况及问题原因,并结合DIP背景提出改进对策。结果300份住院病案中有52份存在问题,问题项目87项,存在较多的问题是ICD编码缺失、患者基本信息错填或漏填、主要诊断不全或不符,分别占比31.03%、24.14%、17.24%。手术科室中填写存在问题较多的是普外科、骨科、泌尿外科,分别占比13.79%、10.34%、9.20%;非手术科室中填写存在问题较多的是呼吸科、血液科、神经内科,分别占比17.24%、11.49%、9.20%。据调查,填写问题的主要原因有培训不到位、重视程度不够、信息缺失,分别占比58.62%、9.20%、32.18%。结论福建医科大学附属龙岩第一医院住院病案首页填写的主要问题为ICD编码缺失、患者信息缺陷或错漏、主要诊断补全或不符等方面,通过加强医护人员培训、提升信息化水平、优化电子病历系统逻辑校验功能,以及建立病案首页质控反馈机制,可有效提升病案首页填写质量,确保DIP分组的准确性和医保支付的合理性。
