The prognosis of hepatocellular carcinoma(HCC) depends on tumor extension as well as hepatic function.Hepatic functional reserve is recognized as a factor affecting survival in the treatment of HCC;the ChildPugh classif ication system is the most extensively used method for assessing hepatic functional reserve in patients with chronic liver disease,using serum albumin level to achieve accurate assessment of the status of protein metabolism.However,insuff icient attention has been given to the status of amino acid(AA) metabolism in chronic liver disease and HCC.Fischer's ratio is the molar ratio of branched-chain AAs(BCAAs:leucine,valine,isoleucine) to aromatic AAs(phenylalanine,tyrosine) and is important for assessing liver metabolism,hepatic functional reserve and the severity of liver dysfunction.Although this ratio is diff icult to determine in clinical situations,BCAAs/tyrosine molar concentration ratio(BTR) has been proposed as a simpler substitute.BTR correlates with various liver function examinations,including markers of hepatic f ibrosis,hepatic blood flow and hepatocyte function,and can thus be considered as reflecting the degree of hepatic impairment.This manuscript examines the literature to clarify whether BTR can serve as a prognostic factor for treatment of HCC. 展开更多

A model of focal cerebral ischemic infarction was established in dogs through middle cerebral artery occlusion of the right side.Thirty minutes after occlusion,models were injected with nerve growth factor adjacent to the infarct locus.The therapeutic effect of nerve growth factor against cerebral infarction was assessed using the hemisphere anomalous volume ratio,a quantitative index of diffusion-weighted MRI.At 6 hours,24 hours,7 days and 3 months after modeling,the hemisphere anomalous volume ratio was significantly reduced after treatment with nerve growth factor. Hematoxylin-eosin staining,immunohistochemistry,electron microscopy and neurological function scores showed that infarct defects were slightly reduced and neurological function significantly improved after nerve growth factor treatment.This result was consistent with diffusion-weighted MRI measurements.Experimental findings indicate that nerve growth factor can protect against cerebral infarction,and that the hemisphere anomalous volume ratio of diffusion-weighted MRI can be used to evaluate the therapeutic effect. 展开更多

Adopting the load and resistance factor design format, the design method for steel jaeket platform structures is developed. Firstly, the limit state equations and design format for steel jacket platform structures are introduced. Then, the ratio of live load effect to dead load effect is estimated. The target reliabilities for design of offshore structures in China offshore area are calibrated by past practice in API RP2A-WSD code. The load and resistance factors are optimized by minimizing the difference within the target reliability and the resulting reliability over the range of load effect ratios. Considering the concurrence of different loads, load combination factors are obtained through an optimization process, and the relation between the load combination factor and load correlation coefficient is established. Finally, the design formulae for steel jacket structures in China offshore area are recommended. 展开更多

The attenuation factor or quality factor(Q-factor or Q) has been used to measure the energy attenuation of seismic waves propagating in underground media. Many methods are used to estimate the Q-factor. We propose a method to calculate the Q-factor based on the prestack Q-factor inversion and the generalized S-transform. The proposed method specifies a standard primary wavelet and calculates the cumulative Q-factors; then, it finds the interlaminar Q-factors using the relation between Q and offset(QVO) and the Dix formula. The proposed method is alternative to methods that calculate interlaminar Q-factors after horizon picking. Because the frequency spectrum of each horizon can be extracted continuously on a 2D time–frequency spectrum, the method is called the continuous spectral ratio slope(CSRS) method. Compared with the other Q-inversion methods, the method offers nearly effortless computations and stability, and has mathematical and physical significance. We use numerical modeling to verify the feasibility of the method and apply it to real data from an oilfield in Ahdeb, Iraq. The results suggest that the resolution and spatial stability of the Q-profile are optimal and contain abundant interlaminar information that is extremely helpful in making lithology and fluid predictions. 展开更多

目的:分析肝母细胞瘤(hepatoblastoma,HB)患儿的临床特征、治疗效果及预后影响因素。方法:回顾性分析2019年1月至2023年11月就诊于湖南省儿童医院和湖南中医药大学第一附属医院的132例HB患儿的临床资料,采用Kaplan-Meier法绘制生存曲线,... 目的:分析肝母细胞瘤(hepatoblastoma,HB)患儿的临床特征、治疗效果及预后影响因素。方法:回顾性分析2019年1月至2023年11月就诊于湖南省儿童医院和湖南中医药大学第一附属医院的132例HB患儿的临床资料,采用Kaplan-Meier法绘制生存曲线,Log-rank检验进行单因素生存分析,将单因素分析中差异具有统计学意义的因素纳入COX回归模型进行多因素分析,确定影响HB患儿总生存率(overall survival,OS)预后的独立危险因素。结果:入组患儿中位年龄18.5月,男性占60.61%;PRETEXTⅡ/Ⅲ期占80.30%,高危组65.15%,上皮型病理86.36%,单发肿瘤86.36%,V/P/EF/R阳性率31.82%。中位随访31个月;1、3年的OS分别为98.5%、77.4%,1、3年的无事件生存率(event-free survival,EFS)为95.0%、77.8%。HB患儿完全缓解88例,部分缓解12例,治疗有效率75.76%(100/132),进展14例,死亡18例。PRETEXT分期、病理分型、肿瘤个数、V/P/EF/R是否阳性、甲胎蛋白(alpha-fetoprotein,AFP)水平、中性粒细胞与淋巴细胞比值(neutrophil-to-lymphocyte ratio,NLR)、血小板与淋巴细胞比值(platelet-to-lymphocyte ratio,PLR)及危险度分层均与HB预后有关(P<0.05);病理分型为混合型、AFP水平>300000μg/L、NLR≥2是HB预后的独立危险因素(P<0.05)。结论:病理分型为混合型、极高AFP水平及NLR升高是影响HB的独立预测指标。 展开更多

Stability analysis of strain-softening slopes is carried out using the shear strength reduction method and Mohr-Coulomb model with degrading cohesion and friction angle.The e ffect of strain-softening behavior on the slope factor of safety is investigated by performing a series of analyses for various slope geometries and strength properties.Stability charts and equations are developed to estimate the factor of safety of strain-softe ning slopes from the results of traditional stability analysis based on perfectly-plastic behavior.Two example applications including an open pit mine in weak rock and clay shale slope with daylighting bedding planes are presented.The results of limit equilibrium analysis and shear strength reduction method with perfectly-plastic models were in close agreement.Using perfectly-plastic models with peak strength properties led to overly optimistic results while adopting residual strength properties gave excessively conservative outcomes.The shear strength reduction method with a strain-softening model gave realistic factors of safety while accounting for the process of strength degradation. 展开更多

Key components of large structures in aeronautics industry are required to be made light and have long enough fatigue lives.It is of vital importance to estimate the fatigue life of these structures accurately.Since the FCG process is affected by various factors,no universal model exists due to the complexity of the mechanisms.Most of the existing models are obtained by fitting the experimental data and could hardly describe the integrative effect of most existing factors simultaneously.In order to account for the integrative effect of specimen parameters,material property and loading conditions on FCG process,a new model named integrative influence factor model(IIF) is proposed based on the plasticity-induced crack closure theory.Accordingly to the predictions of crack opening ratio(γ) and effective stress intensity factor range ratio(U) with different material under various loading conditions,predictions of γ and U by the IIF model are completely identical to the theoretical results from the plane stress state to the plane strain state when Poisson's ratio equals 1/3.When Poisson's ratio equals 0.3,predictions of γ and U by the IIF model are larger than the predictions by the existing model,and more close to the theoretical results.In addition,it describes the influence of R ratios on γ and U effectively in the whole region from-1.0 to 1.0.Moreover,several sets of test data of FCG rates in 5 kinds of aluminum alloys with various specimen thicknesses under different loading conditions are used to validate the IIF model,most of the test data are situated on the predicted curves or between the two curves that represent the specimen with different thicknesses under the same stress ratio.Some of the test data slightly departure from the predictions by the IIF model due to the surface roughness and errors in measurement.Besides,based on the analysis of the physical rule of crack opening ratios,a relative thickness of specimen is defined to describe the influence of material property,specimen thickness and so forth on FCG characteristics conveniently.In conclusion,the relative thickness of specimen simplifies the expression of FCG characteristic and provides a general parameter to analyze the fatigue characteristics of different materials with various thicknesses under different loading conditions.The IIF model describes the integrative effect of existing influence factors explicitly and quantitatively,and provides a helpful tool for fatigue property estimation of practical component and experiment design. 展开更多

Domestic coal stove is widely used in China, especially for countryside during heating period of winter, and polycyclic aromatic hydrocarbons (PAHs) are important in flue gas of the stove. By using dilution tunnel system, samples of both gaseous and particulate phases from domestic coal combustion were collected and 18 PAH species were analyzed by GC-MS. The average emission factors of total 18 PAH species was 171.73 mg/kg, ranging from 140.75 to 229.11 mg/kg for bituminous coals, while was 93.98 mg/kg, ranging from 58.48 to 129.47 mg/kg for anthracite coals. PAHs in gaseous phases occupied 95% of the total of PAHs emission of coal combustion. In particulate phase, 3-ring and 4- ring PAHs were the main components, accounting for 80% of the total particulate PAHs. The total toxicity potency evaluated by benzo[a]pyrene-equivalent carcinogenic power, sum of 7 carcinogenic PAH components and 2,3,7,8-tetrachlorodibenzodioxin had a similar tendency. And as a result, the toxic potential of bituminous coal was higher than that of anthracite coal. Efficient emission control should be conducted to reduce PAH emissions in order to protect ecosystem and human health. 展开更多

Several parameters for predicting survival in patients with colorectal cancer have been identified, including the performance status, age, gender and tumor-nodemetastasis(TNM) stage. Although the TNM stage is important and useful for predicting the prognosis and determining the appropriate treatment, it is well known that the survival time varies widely, even in patients with the same stage of disease. Therefore, the identification of new parameters capable of more precisely predicting patient survival is needed to help select the optimal treatment, especially in patients in the advanced stage of disease. Although the TNM stage reflects the tumor characteristics, cancer progression and survival are not determined solely based on the local characteristics of the tumor, but also the host systemic immune/inflammatory response. Therefore, using a combination of parameters that reflect both tumor characteristics and the host systemic inflammatory status is thought to be important for accurately predicting patient survival. 展开更多

Ratanasampil (RNSP) is a traditional Tibetan medicine used for the treatment of stroke and cerebrovascular diseases. Previous discoveries that RNSP can reduce β-amyloid protein levels and increase learning and memory in Alzheimer’s mouse models (Tg2576) led us to investigate whether RNSP can improve cognitive functions in Alzheimer’s patients. In this study, 146 AD patients living in Qinghai province received either one gram or 0.33 gram daily of RNSP for 16 weeks. Placebo patients received Piracetam. Serum Aβ40 and Aβ42 levels were measured at the beginning of the study and after 4 and 16 weeks of treatment. Compared to the same group before treatment, MMSE scores, ADAS-cog scores and ADL scores were significantly improved (p 0.05, p > 0.05). After 16-week treatment, serum TNF-α, IL-1β, IL-6 and Aβ42 levels were significantly decreased (p < 0. 01) in the high-dose RNSP group, whereas no significant differences were found in the low-dose and placebo groups. The Aβ42/Aβ40 ratio was significantly decreased after 4-week and 16-week treatment in the high-dose RNSP group (p < 0. 05, p < 0.01). Furthermore, serum Aβ42 concentrations had a strong positive correlation with TNF-α, IL-1β and IL-6 levels. There were no observable adverse effects in either treatment or control groups. We conclude that further clinical trials of RNSP in Alzheimer disease are warranted. 展开更多

Thus, the appropriate fillerbitumen ratio directly affects the asphalt mortar's performance. This paper tested the physical indexes of the No. 70 matrix asphalt mortar modified by additive Sasobit (SB) and Sasowam (SW) through dynamic shear rheometer and bending beam rheometer under different temperature conditions, and comprehensively analyzed the high-temperature anti-rutting and fatigue performance, low-temperature crack resistance performance, and ductility of asphalt mortar. The results show that ore powder not only can increase the antirutting factor but also can increase the aging resistance of asphalt. SB has better performances than SW at high temperatures. As for the filler-bitumen ratio of asphalt mortar with additive SB, the recommended value is between 0.8 and 1.2, and the value may be a little larger for that with SW. 展开更多

The results showed that the distribution ratio and separation coefficient both increased with decreasing of feed acidity and increasing of the lactic acid concentration,and the extraction capacity increased with increasing of lactic acid concentration in D2EHPA-HCl-LA system. When the pH value of the feed was 3.5 and lactic acid concentration was 0.6 mol/L,the max separation coefficient was 1.57,and the extraction capacity was 27.87 g/L. 展开更多

The influences of parameters such as filling expansion ratio n, plasticity factor kl, deformation parameter k2, Poisson ratio of concrete v, diameter of reinforced bar d and protective layer thickness c on the critical corrosive ratio are researched by theory analysis and experiments. The experimental results validate the accuracy of the model. According to the experimental study, the least squares solution is calculated as n = 1.8 ,k1 =0.61 ,k2 =0.5. 展开更多

Factors Related to Functional Capacity and Quality of Life for Elderly People in a Middle-Sized City of São Paulo, Brazil

Introduction: It is a fact that the elderly population has been increasing all around the world and also in Brazil. This has been a challenge for governments and managers in the development of public policies to promote healthy and quality aging. And one of the conditions for healthy aging is to assess the functional capacity and quality of life of elderly people. Objective: The aim of the present study was to evaluate the factors that influence the loss of functional capacity and the worsening of the quality of life of a representative sample of elderly people from a medium-sized city in the inner of S<span style="white-space:nowrap;">&#227;o Paulo State, Brazil. Methodology: Questionnaires about activities of daily living (ADL), instrumental activities of daily living (IADL), Flanagan quality of life scale (FQOLS), sociodemographic and morbidities aspects were applied to the elderly people. In order to assess associated factors to functional capacity and quality of life, chi-square tests and logistic regression models were fitted including variables: gender, age group, marital status, income, education level, body mass index, abdominal circumference and some morbidities reported by the elderly people. The level of significance adopted for the statistical tests was 5%. Results: Elderly with age over 75 years old and not married had more risk to be dependent on ADL, while elderly with more than 75 years old, not married, income below one minimal wage, cardiovascular disease and be diabetic had more risk to be dependent by IADL. Already, people with excess of weight had smallest risk to be dependent by IADL, paradoxically. The elderly not married and with an income below a minimum wage had the worst quality of life. Conclusions: Population aging in Brazil is an evident factor, so it is hoped that this study will contribute to the greatest diligence on the issues of the elderly and related social problems. 展开更多

Colorectal cancer(CRC) is one of the most common malignant diseases in the world.Presently,the most widely used staging system for CRC is the tumor nodes metastasis classification system,which classifies patients into prognostic groups according to the depth of the primary tumor,presence of regional lymph node(LN) metastases,and evidence of distant metastatic spread.The number of LNs with confirmed metastasis is related to the severity of the disease,but this number depends on the number of LNs retrieved,which varies depending on patient age,tumor grade,surgical extent,and tumor site.Numerous studies and a recent structured review have demonstrated associated improvements in the survival of CRC patients with increasing numbers of LNs retrieved for examination.Hence,the impact of lymph node ratio(LNR),defined as the number of metastatic LNs divided by the number of LNs retrieved,has been investigated in various malignancies,including CRC.In this editorial,we review the literature demonstrating the clinicopathological significance of LNR in CRC pati-ents.Some reports have indicated the advantage of considering the LNR compared to the number of LNs retrieved and/or LN status.When the LNR is taken into consideration for survival analysis,the number of LNs retrieved and/or the LN status is not always found to be a prognostic factor.The cut-off points for LNRs were proposed in numerous studies.However,optimal thresholds for LNRs have not yet received consensus.It is still unclear whether the LNR has more prognostic validity than N stage.For all these reasons,the potential advantages of LNRs in the staging system should be investigated in large prospective data sets. 展开更多
