In Mali, from March, 2007 to February, 2008, in National Center for Disease Control and Prevention, 19.2% of hospitalized patients (24/125) received long-term corticosteroid therapy. Until now, the department of internal medicine of the university hospital of Point G hasn’t done any research on the corticosteroid therapy specially. Therefore, we initiated this work to evaluate our practice of prescription of corticosteroid. The target of the study was to evaluate practical activity of prescription of corticosteroid of internal medicine. Methods: Based on all hospitalized patient materials of the department of internal medicine of Point G regional university central hospital during January 2009-December 2013, we initiated this retrospective and descriptive study. This study was based on all hospitalized patients receiving corticosteroid in the time of researching, without distinguishing their sex or age. Results: During the period of study, the department of internal medicine of point G regional central hospital had received 2155 patients;64 were eligible: prevalence of 2.97%. Most patients were female, accounting for 87.5% with a sex-ratio of 0.14. The average age of our patients was 39.11 ± 16.92 years, ranging from 14 to 97 years. Physicians enrolled in the study Specialized Diploma (DES) and Internal represented 79.7% of prescribers. As pretreatment assessment, only 2 patients (3.12%) had carried out all of the standard balance (NFS, electrolytes, calcium, blood glucose and serum protein). The most common glucocorticoide was prednisone. Solid tumors represented 31.25% (20 cases) of the diseases treated, followed by systemic lupus erythematosus with 15.62% (10 cases). In addition, there are two cases of adrenals insufficiency (3.1%). In Point G internal medicine, pre-therapy evaluation and methods about corticosteroids therapy were insufficient and different.展开更多

There are no specific treatment-recommendations for this cancer. We report a 59-year-old man, a former smoker, with this condition. He presented with a cough and computed tomography revealed a pulmonary nodule (50 × 47 × 48 mm) with a right pleural effusion, together with a cerebral right temporal-lobe lesion and a right adrenal lesion. The serum alpha-fetoprotein value was normal. Histological examination via bronchoscopic biopsy revealed an adenocarcinoma. Positive CK7, AE1/AE2, and TTF1 with granular cytoplasmic staining on immunohistochemical study confirmed the diagnosis of hepatoid adenocarcinoma of the lung, stage IV-B. The patient had received chemotherapies (carboplatin-pemetrexed, gemcitabine) and cerebral radiotherapy, but he died at eight months of the treatment as a result of tumor progression.展开更多
OBJECTIVE:To observe the clinical efficacy of Bushenjiangya(reinforcing kidney and lowering blood pressure)decoction in curing hypertension of kidney Qi deficiency type.METHODS:First 60 patients with hypertension of k...OBJECTIVE:To observe the clinical efficacy of Bushenjiangya(reinforcing kidney and lowering blood pressure)decoction in curing hypertension of kidney Qi deficiency type.METHODS:First 60 patients with hypertension of kidney Qi deficiency type were divided into control group and treatment group randomly;the conventional western medicine was given to patients of the control group for antihypertensive therapy,while the traditional Chinese medicine Bushenjiangya decoction was added for patients in the treatment group based on the therapy for those in control group;after 2 months,the blood pressure,clinical efficacy and urine microalbumin changes of patients in the two groups before and after the treatment were observed.RESULTS:a.The systolic pressure of patients in the two groups was all reduced obviously(P<0.05);b.The total effective rate of experiment group in antihypertensive effect was 76.7%,while that of control group was 83.3%(P>0.05);c.The total effective rate of experiment group in TCM syndrome effect was 86.7%,while that of control group was 63.3%,showing that the effect of experiment group was better than that of control group(P<0.05);d.The 24 h urine microalbumin reduction of experiment group was better than that of control group(P<0.05).CONCLUSIONS:The curative effect of Bushenjiangya decoction in combination with the western medicine is better than that of simple western medicine for hypertension of kidney Qi deficiency type,and also the urine microalbumin can be obviously reduced to protect the kidney function.展开更多

In order to achieve this purpose, an integrated system for integrated design of CAD/CAPP/FTL with box-type parts is realized. In this paper, firstly the theory of the conceptual design of FTL based on CAPP is established and the software architecture is developed. The global-domain pr oduct model is put forward to meet with data representation. After the manufactu ring feature is analyzed and decomposed for machining operations, the feature-b ased part unified data model object-oriented is built. Secondly the knowledge- based CAPP system data model is established. The process planning unit model is formed by work element (WE), which is tool-cell based function abstraction meth od, and has unified layers with respect to designing and manufacturing. With WEs through dynamic response and simultaneous structure in process planning, the pr inciple solution for CAPP function system can be implemented. Lastly the mapping from process domain to module design domain is built. The unidirectional select ion process based on fuzzy matching technology is easily dealt with computer. Th e algorithm of decision with fuzzy synthetic evaluation method and technology is applied to estimate the feasibility of the results in local working procedure o f modular NC machine tools and make decision of designs in overall FTL, which co nsists of many different factors. The example shows that the rapid response desi gn in bidding-based FTL scheme design is realized.展开更多
