BACKGROUND In pediatric age group patients(<18 years old)treated operatively for distal radius/both bone fractures extending imaging beyond the initial postoperative period-particularly in uncomplicated cases-appears to provide limited additional benefit.AIM To determine the necessary number of follow-up X-rays to use resources efficiently.METHODS Participants included in this study are pediatric age group patients who were treated operatively for distal radius/both bone fractures and were identified from a prospected collected data from the operating room database between the years 2009 and 2017.The data in the study included patients who had distal radius fractures and underwent fixation surgery(n=88).RESULTS When assessing the difference in the odds of conducting 1 or less X-ray compared to 2 or more X-rays in regard to the type of fixation,the only significant difference is the closed reduction fixation method.Patients who underwent closed reduction method procedure have significantly lower odds of having 2 more X-rays compared to those who didn’t have closed reduction method.Open reduction,internal fixation,and other fixation methods(close reduction and internal fixation,debridement,or epiphysiodesis)have higher odds of having two or more X-rays compared to patients who did not receive these methods;however,these odds are not statistically significant.CONCLUSION The findings of this study reveal notable absence of a statistically significant association between the frequency of postoperative X-rays and the outcome of children with distal radius fractures. 展开更多

Osteosynthesis of the Displaced Fractures of the Distal Radius Treated with New-Clip® Plate: Case Study in the Basse Terre Hospital

Its frequency is constantly increasing. The locked anterior plate with its well-established biomechanical properties, offers a reliable alternative. The aim of this study was to evaluate the radiological, the functional results and to determine the factors of poor postoperative prognosis of DRF treated with Newclip radial plates®. Methodology: This prospective cohort study evaluates the radiological and functional outcames of displaced radius fractures (DRFs) in patients ≥50 years old treated with Newclip® (locked anterior plates) at the Basse-Terre Hospital in Guadeloupe from 2022 to 2024. The patients were categorized into those with epiphyseal involvement (E1 - E4) and without epiphyseal involvement (E0) based on Laulan’s MEU classification. Radiological parameters (distal radio-ulnar index (DRUI), radial inclination frant view (IRF), radial inclination sagittal view (IRS) were assessed pre and post-operatively. Functional recovery was evaluated at 12 months using the QuickDash questionnaire. Results: Falls were the most common cause of fracture. Post-operatively, SRI was the least restored parameter. Poor prognostic factor for SRI improvement included posterior commimution and unstable fractures. Factors associated with higher QuickDash scores included unstable factures, unrestored DRUI, low plate position, metaphyseal features, and ulnar features. Conclusion: The anterior locking plate osteosynthesis is reliable treatment option with excellent functional outcomes. 展开更多

Cold-flow experiments on planar Expansion Deflection(ED)nozzle flows are conducted under a simulated startup-shutdown process of rocket motors.The purpose is to investigate the flow and performance characteristics in ED nozzles,capture the behavior of shock flapping,and explore asymmetric flow dynamics utilizing a symmetric nozzle.A total pressure condition,characterized by rapid rise followed by a slow fall,is employed to simulate the continuous startup and shutdown processes.The schlieren imaging technique and high-frequency pressure transducers are employed to obtain the flow information.The experimental results indicate that the flow characteristics differ between the startup and shutdown processes with a hysteresis observed in the nozzle wake mode transition.During the startup process,the shock waves are pushed outward of the nozzle,while during the shutdown process,the flow propagates inward dominated by Mach stems.Counterintuitive results are demonstrated,namely,the mode transition is not the cause of the sudden thrust decrease,and the moment of maximum thrust does not coincide with the moment of maximum total pressure.During the operation of the nozzle,two stages of shock wave flapping occur,accompanied by significant wall pressure oscillations.These oscillation frequencies are demonstrated to be related to the inherent acoustic frequencies of the test chamber.An improved pressure ratio method is proposed to predict the position of the shock oscillation separation point.The prediction results revealed the shock behavior during the flapping process. 展开更多

The challenge of solving nonlinear problems in multi-connected domains with high accuracy has garnered significant interest.In this paper,we propose a unified wavelet solution method for accurately solving nonlinear boundary value problems on a two-dimensional(2D)arbitrary multi-connected domain.We apply this method to solve large deflection bending problems of complex plates with holes.Our solution method simplifies the treatment of the 2D multi-connected domain by utilizing a natural discretization approach that divides it into a series of one-dimensional(1D)intervals.This approach establishes a fundamental relationship between the highest-order derivative in the governing equation of the problem and the remaining lower-order derivatives.By combining a wavelet high accuracy integral approximation format on 1D intervals,where the convergence order remains constant regardless of the number of integration folds,with the collocation method,we obtain a system of algebraic equations that only includes discrete point values of the highest order derivative.In this process,the boundary conditions are automatically replaced using integration constants,eliminating the need for additional processing.Error estimation and numerical results demonstrate that the accuracy of this method is unaffected by the degree of nonlinearity of the equations.When solving the bending problem of multi-perforated complex-shaped plates under consideration,it is evident that directly using higher-order derivatives as unknown functions significantly improves the accuracy of stress calculation,even when the stress exhibits large gradient variations.Moreover,compared to the finite element method,the wavelet method requires significantly fewer nodes to achieve the same level of accuracy.Ultimately,the method achieves a sixth-order accuracy and resembles the treatment of one-dimensional problems during the solution process,effectively avoiding the need for the complex 2D meshing process typically required by conventional methods when solving problems with multi-connected domains. 展开更多

Minimally invasive plate osteosynthesis for distal radius fractures using a 3-point positioning technique

BACKGROUND The volar approach with plate fixation is the gold standard for treating distal radius fractures,often requiring incision of the pronator quadratus(PQ)muscle.Preserving the PQ during surgery may facilitate early postoperative recovery.However,conventional minimally invasive plate osteosynthesis(MIPO)techniques frequently necessitate multiple(3-4)intraoperative fluoroscopic adjustments to achieve optimal plate positioning,which can inadvertently damage the PQ muscle.Based on our clinical observations,we developed a novel 3-point positioning technique to minimize PQ injury while ensuring accurate plate placement.Preliminary results demonstrate promising early clinical outcomes.AIM To retrospectively analyze distal radius fractures treated using the 3-point positioning-assisted MIPO technique with preservation of the PQ.METHODS The 3-point positioning technique was applied:The Kirschner wire was inserted after fluoroscopy and was correctly adjusted the position of the plate above the PQ.With the aid of Kirschner wires positioning the PQ stripping was performed only once,and the plate then placed in a correct and satisfactory position.Operation time,incision length,wrist pain score,upper extremity function disabilities of the arm,shoulder and hand(DASH)score,wrist Gartland-Werley score,wrist grip strength,and range of motion were among the quantitative variables recorded.Qualitative variables including AO fracture classification,intraoperative and postoperative complications were evaluated.RESULTS At a mean follow-up of 6.9±0.8 months,the mean scar length was 25.4±1.5 mm,the pain score was 0.7±0.6,the DASH score for the upper limb was 4.7±1.3,and the Gartland-Werley score for wrist function was 4.1±1.1 at the last follow-up.Mean flexion was 97.3%,extension was 97.0%,pronation was 98.9%,supination was 98.9%,and grip strength was 86.6%compared to contralateral values.No unfavorable intraoperative or postoperative complications occurred.CONCLUSION The 3-point positioning technique may reduce the damage to the PQ muscle and is a safe and effective method for MIPO for distal radius fractures. 展开更多

To improve the drill pipe fracture failure phenomenon in deflecting drills with ultra-short radius flexible drill pipes,this study establishes a mechanical model of a deflecting drill with an ultra-short radius flexible drill pipe,numerically simulates the mechanical characteristics of the flexible drill pipe under the conventional drilling process using the finite element analysis method,performs mechanical performance tests on the flexible drill pipe,and optimizes and improves its structure.The results show that,with the gradual increase in the bending angle of the flexible drill pipe unit section,the upper drilling pressure of the flexible drill pipe unit section decreases rapidly and its friction increases approximately linearly.The tensile strength of the optimized ultra-short radius flexible drill pipe is increased by 24.7%,and the minimum threshold of its torsional strength is doubled.This effectively improves the overall strength of the ultra-short radius flexible drill pipe and provides a theoretical basis for downhole drilling stability studies of flexible drill pipes. 展开更多

Imbalance is a distinctive feature of many datasets,and how to make the dataset balanced become a hot topic in the machine learning field.The Synthetic Minority Oversampling Technique(SMOTE)is the classical method to solve this problem.Although much research has been conducted on SMOTE,there is still the problem of synthetic sample singularity.To solve the issues of class imbalance and diversity of generated samples,this paper proposes a hybrid resampling method for binary imbalanced data sets,RE-SMOTE,which is designed based on the improvements of two oversampling methods parameter-free SMOTE(PF-SMOTE)and SMOTE-Weighted Ensemble Nearest Neighbor(SMOTE-WENN).Initially,minority class samples are divided into safe and boundary minority categories.Boundary minority samples are regenerated through linear interpolation with the nearest majority class samples.In contrast,safe minority samples are randomly generated within a circular range centered on the initial safe minority samples with a radius determined by the distance to the nearest majority class samples.Furthermore,we use Weighted Edited Nearest Neighbor(WENN)and relative density methods to clean the generated samples and remove the low-quality samples.Relative density is calculated based on the ratio of majority to minority samples among the reverse k-nearest neighbor samples.To verify the effectiveness and robustness of the proposed model,we conducted a comprehensive experimental study on 40 datasets selected from real applications.The experimental results show the superiority of radius estimation-SMOTE(RE-SMOTE)over other state-of-the-art methods.Code is available at:https://github.com/blue9792/RE-SMOTE(accessed on 30 September 2024). 展开更多

Data-driven approaches such as neural networks are increasingly used for deep excavations due to the growing amount of available monitoring data in practical projects.However,most neural network models only use the data from a single monitoring point and neglect the spatial relationships between multiple monitoring points.Besides,most models lack flexibility in providing predictions for multiple days after monitoring activity.This study proposes a sequence-to-sequence(seq2seq)two-dimensional(2D)convolutional long short-term memory neural network(S2SCL2D)for predicting the spatiotemporal wall deflections induced by deep excavations.The model utilizes the data from all monitoring points on the entire wall and extracts spatiotemporal features from data by combining the 2D convolutional layers and long short-term memory(LSTM)layers.The S2SCL2D model achieves a long-term prediction of wall deflections through a recursive seq2seq structure.The excavation depth,which has a significant impact on wall deflections,is also considered using a feature fusion method.An excavation project in Hangzhou,China,is used to illustrate the proposed model.The results demonstrate that the S2SCL2D model has superior prediction accuracy and robustness than that of the LSTM and S2SCL1D(one-dimensional)models.The prediction model demonstrates a strong generalizability when applied to an adjacent excavation.Based on the long-term prediction results,practitioners can plan and allocate resources in advance to address the potential engineering issues. 展开更多

It is of great practical value to explore the correlation between the vertical curve radius of desert highway and the increase of sand accumulation in local lines,and to select the appropriate vertical curve radius for reducing the risk of sand accumulation.In this study,three-dimensional models of desert highway embankments with different vertical curve radii were constructed,and Fluent software was used to simulate the wind-sand flow field and sand accumulation distribution of vertical curve embankments.The results show that:(1)Along the direction of the road,the concave and the convex vertical curve embankments have the effect of collecting and diverging the wind-sand flow,respectively.When the radius of the concave vertical curve is 3000 m,5000 m,8000 m,10000 m and 20000 m,the wind velocity in the middle of the vertical curve is 31.76%,22.58%,10.78%,10.53%and 10.44%,higher than that at both ends.When the radius of the convex vertical curve is 6500 m,8000 m,10000 m,20000 m and 30000 m,the wind velocity at both ends of the vertical curve is 14.06%,9.99%,6.14%,3.22%and 2.41%,higher than that in the middle.The diversion effect also decreases with the increase of the radius.(2)The conductivity of the concave and convex vertical curve embankments with different radii is greater than 1,which is the sediment transport roadbed.The conductivity increases with the increase of radius and gradually tends to be stable.When the radius of the concave and convex vertical curves reaches 8000 m and 20000 m respectively,the phenomenon of sand accumulation is no longer serious.Under the same radius condition,the concave vertical curve embankment is more prone to sand accumulation than the convex one.(3)Considering the strength of the collection and diversion of the vertical curve embankment with different radii,and the sand accumulation of the vertical curve embankment in the desert section of Wuma Expressway,the radius of the concave vertical curve is not less than 8000 m,and the radius of the convex vertical curve is not less than 20000 m,which can effectively reduce the sand accumulation of the vertical curve embankment.In the desert highway area,the research results of this paper can provide reference for the design of vertical curve to ensure the safe operation of desert highway. 展开更多

The extended kernel ridge regression(EKRR)method with odd-even effects was adopted to improve the description of the nuclear charge radius using five commonly used nuclear models.These are:(i)the isospin-dependent A^(1∕3) formula,(ii)relativistic continuum Hartree-Bogoliubov(RCHB)theory,(iii)Hartree-Fock-Bogoliubov(HFB)model HFB25,(iv)the Weizsacker-Skyrme(WS)model WS*,and(v)HFB25*model.In the last two models,the charge radii were calculated using a five-parameter formula with the nuclear shell corrections and deformations obtained from the WS and HFB25 models,respectively.For each model,the resultant root-mean-square deviation for the 1014 nuclei with proton number Z≥8 can be significantly reduced to 0.009-0.013 fm after considering the modification with the EKRR method.The best among them was the RCHB model,with a root-mean-square deviation of 0.0092 fm.The extrapolation abilities of the KRR and EKRR methods for the neutron-rich region were examined,and it was found that after considering the odd-even effects,the extrapolation power was improved compared with that of the original KRR method.The strong odd-even staggering of nuclear charge radii of Ca and Cu isotopes and the abrupt kinks across the neutron N=126 and 82 shell closures were also calculated and could be reproduced quite well by calculations using the EKRR method. 展开更多

Due to the network planning of subways and their surrounding structures,increasingly more overlapping shields with a small curve radius have been constructed. A newly constructed upper tunnel partly overlaps a lower one, leading to the extremely complex uplift of the lower tunnel caused by the construction of a new tunnel. Based on the shield-driven project that runs from the Qinghe Xiaoyingqiao Station to the Qinghe Station in Beijing, which adopts the reinforcement measures of interlayer soil grouting and steel supports on site, in this study, the uplift pattern of the lower tunnel and the stress characteristics of steel supports were investigated through numerical simulations and on-site monitoring.The study results show that among all tunnel segments, the first segment of the shield witnesses a maximum uplift displacement that increases with the horizontal space between tunnels. On using either interlayer soil grouting or steel-ring bracing reinforcement, the uplift of the tunnel lining exceeds the control value;by contrast,when these two measures are jointly applied, the uplift of the tunnel lining does not exceed a maximum value of 4.87 mm, which can satisfy the requirements of deformation control. Under these two joint measures, the soil strength between two stacked shield tunnels can be enhanced and the uplift deformation can be restricted with the interlayer soil grouting. Also, the segmental deformation and overall stability of the existing tunnel can be controlled with the temporary steel supports.The deformation of circumferential supports and segments is closely related to each other, and the segmental uplift is controlled by H-shaped steel supports. With the increase in the horizontal space between twin shields, the effect of the construction would gradually weaken, accompanied by a gradual reduction of the stresses of steel supports. These findings provide a valuable reference for the engineering design and safe construction of overlapping shield tunnels with a small curve radius. 展开更多

In this study,we examined the thermal fluctuations,deflection angle,and greybody factor of a high-dimensional Schwarzschild black hole in scalar-tensor-vector gravity(STVG).We calculated some thermodynamic quantities related to the correction of the black hole entropy caused by thermal fluctuations and discussed the effect of the correction parameters on these quantities.By analyzing the changes in the corrected specific heat,we found that thermal fluctuations made the small black hole more stable.It is worth noting that the STVG parameter did not affect the thermodynamic stability of this black hole.Additionally,by utilizing the Gauss-Bonnet theorem,the deflection angle was obtained in the weak field limit,and the effects of the two parameters on the results were visualized.Finally,we calculated the bounds on the greybody factor of a massless scalar field.We observed that as the STVG parameter around the black hole increased,the weak deflection angle became larger,and more scalar particles can reach infinity.However,the spacetime dimension has the opposite effect on the STVG parameter on the weak deflection angle and greybody factor. 展开更多

Introduction: Forearm fractures involving both the radius and ulna, present distinctive challenges in orthopedic trauma management. This case report explores the complexities of a rare case of bilateral fractures, emphasizing the importance of a comprehensive approach for optimal outcomes. Case Presentation: This report presents the case of a 40-year-old motorcyclist who was involved in a road accident and arrived at the emergency department shortly after the incident. Clinical examination revealed closed fractures in both forearms with mild swelling and severe tenderness. Fortunately, no neurovascular issues or compartment syndrome were detected. The patient was administered intravenous fluids and effective pain relief. Forearm splints were applied and the arms were elevated using pillows. Radiographs showed fractures in the right distal radius and ulna, as well as the left mid-shaft radius and ulna. Under general anesthesia, open reduction and internal fixation were per-formed using plates and screws for all four fractures. Postoperatively, the patient’s arms were immobilized and elevated, with gradual recovery during follow-up appointments over six months, eventually achieving full function without complaints. Conclusion: This article highlights the possibility, although infrequent, of bilateral shaft fractures of the ulna and radius. With thorough clinical assessment and radiological investigation, such fractures can be well-defined. The required definitive treatment, which usually involves open reduction and internal fixation, can be performed. 展开更多

A novel compliant spinal fixation designed based on the concept of compliant mechanisms can reduce the stress-shielding effect and adjacent segment degeneration(ASD)effectively,but propose higher requirements for the properties of the used materials.Bulk metallic glasses(BMGs),as a kind of young biomaterials,exhibiting excellent comprehensive properties,which are attractive for compliant spinal fixation.Here,according to the practical service condition of the basic elements in compliant spinal fixation,large deflection deformation behaviors of Zr_(61)Ti_(2)Cu_(25)Al_(12)(at.%,ZT1)BMG beam,including elastic,yielding and plastic were investigated systematically.It was shown that the theoretical nonlinear analytical solution curve as the benchmark not only with the capacity to predict the nonlinear load-deflection relation within the elastic deformation regime,but also assists to capture the yielding event roughly,which can be used as a powerful design tool for engineers.To capture the beginning of the yielding event exactly,bending proof strength(σ_(p),0.05%)accompanied with tiny permanent strain of 0.05% was proposed and determined for BMGs in biomedical implant applications,which is of significance for setting the allowable operating limits of the basic flexible elements.By approach of interrupted loading-unloading cycles,plastic deformation driven by the bending moment can be classified into two typical stages:the initial stage which mainly characterized by the nucleation and intense interaction of abundant shear bands when the plastic strain below the critical value,and the second stage which dominated by the progressive propagation of shear bands and coupled with the emergence of shear offsets on tensile side.The plasticity of BMG beam structures depends on the BMG's inherent plastic zone size(rp).When the half beam thickness less than that of the rp,the plastic deformation of BMGs will behave in a stable manner,which can be acted as the margin of safety effectively. 展开更多
