文摘OBJECTIVE:To evaluate the efficacy and safety of Yunpiqiangshen gel(运脾强肾浸膏)which composed of eleven herbs on quality of life(QOL)improvement.METHODS:We enrolled 180 patients lasted from January 2020 to December 2021.Seventy-eight patients received standards of care(control group)and 76 patients received Yunpiqiangshen gel[Traditional Chinese Medicine,(TCM)group]for 6 months in statistical analysis. The primary outcome assessed using Kidney Disease Quality of Life Short Form Questionnaire version 1.3(KDQOL-SFTM 1.3),including Short Form Health Survey(SF-36)and Kidney Disease Targeted Areas(KDTA)scores. The second outcomes included the TCM syndrome score,Fatigue Assessment Instrument(FAI)score,Modified Quantitative Subjective Global Assessment(MQSGA)score, anthropometric indicators, and blood chemical testing.Adverse events, including decompensated gastrointestinal symptoms, were evaluated.RESULTS:The results showed that the mean change of SF-36(75±13 vs 64±16,F=6.070,P=0.015)and KDTA(76±7 vs 70±9,F=4.118,P=0.044)scores in the TCM group had an improvement(increase)from baseline,as compared with the control group.Yunpiqiangshen gel also resulted in a significant improvement in almost dimensions of QOL.At the end of follow-up,the imputed percentage of patients in the response rate of TCM syndrome was greater in the TCM group than in the control group(76.32%vs 20.51%,χ^(2)=48.02,P=0.000).The fatigue, soreness of waist, anorexia, abdominal distension, loose stool, and constipation were alleviated after Yunpiqiangshen gel therapy. The FAI(98.58±25.08 vs 131.21±31.85,F=8.745,P=0.004)and MQSGA(10.13±2.84 vs 12.83±3.85,F=11.396,P=0.001)scores in the TCM group had an improvement(reduce)from baseline compared with the control group.A higher level of albumin of patients in the TCM group compared with the control group.Diarrhea, vomit, and loose stool were more common in the TCM group, but generally mild in severity.CONCLUSION:Compared with the standards of care, added Yunpiqiangshen gel was a safe and effective therapy for improving QOL in dialysis patients.
文摘Haemochromatosis is the most common genetic condition among people of European descent, resulting in iron overload and multi-organ dysfunction.Despite early detection and treatment advances, affected individuals experience significant morbidity impacting their quality of life(QoL).To scope the literature for QoL issues and rank them in order of relevance by professional bodies.A literature search was conducted using PubMed,EMBASE,and MEDLINE in addition to a grey literature search against the eligibility criteria up to July 2023.Inclusion cri-teria included original articles with data concerning symptoms and QoL in patients with haemochromatosis.Nineteen issues were identified from 47 articles and scored by a haemochromatosis special interest group using a scale of 1 to 10(10=highest importance).Mean scores were then calculated for each issue.Fatigue, joint pain and sexual issues were key factors associated with impaired QoL.The least relevant were weight changes and abdominal pain.Other issues raised were anxiety, the development of diabetes, and concerns about genetics and family. This is the first scoping review examining common symptoms affecting QoL of patients with hereditary haemochromatosis.Further studies, including patient interviews and a randomised controlled trial, will inform a validated QoL questionnaire.
文摘BACKGROUND Endoscopic bariatric therapies(EBT)have been developed as an adjunctive therapy to treat persistent obesity and recurrent weight gain and dumping symptoms after metabolic and bariatric surgery(MBS).While the efficacy of revisional EBTs has been documented,no studies to date have examined how these procedures impact quality of life(QOL)and internalized weight bias(IWB).AIM To understand how endoscopic revisional therapies impact QOL,IWB,and dumping syndrome.METHODS Analysis included 19 participants treated for post-MBS recurrent weight gain and/or dumping syndrome. The short-form 36 survey, weight bias internalization scale, and the Sigstad scoring system were used to measure QOL,IWB,and dumping syndrome pre-and≥4 weeks post-EBT.RESULTS At 6-months,total body weight loss was 8.6%±7.15%(n=16),at 12-months was 13.4%±7.46%(n=16).Short-form 36 scores for all 8 scales significantly increased from pre-to post-procedure with the greatest improvement in the scales related to emotional(mean improvement=59.6,P<0.05)and physical health(mean improvement=31.1,P<0.05).Both total Sigstad score(mean difference=4.947,P<0.05)and total weight bias internalization score(mean difference=0.761,P<0.05)significantly decreased from pre-to post-procedure.CONCLUSION Findings suggest that revisional EBT may improve post-MBS QOL across a broad spectrum of outcomes beyond optimizing body weight. As early EBT results indicate positive mental and physical health outcomes, further research is needed to evaluate the relationship between these improvements, body weight and interdisciplinary post-MBS care.
文摘Objective:This study aimed to summarize the quality of life experiences of individuals with physical frailty undergoing hemodialysis or peritoneal dialysis and to identify the difficultiesand resources that enable better person-centered health care. Methods:The study described is a qualitative meta-synthesis literature review. The search was performed in databases such as CINAHL,Scopus,PubMed,Web of Science, the Cochrane Library, and Cuiden Plus through Medical Subject Headings and free terms.Qualitative and mixed studies were included on individuals undergoing hemodialysis or peritoneal dialysis,18 years of age or older, published in English or Spanish, between January 2013 and January 2024.The Mixed Methods Appraisal Tool was used to assess the methodological quality. The information was analyzed and coded through a socioecological model and the social determinants of health. Results:Fourteen qualitative and two mixed articles were selected.A total of 256 individuals participated in the study.Seventeen themes and 25 sub-themes were identifiedand grouped into two blocks(difficulties and resources). The difficultiesthat stood out were a low tolerance for activities of daily living and physical activity, loss of self-control over life and social roles, and lack of community and public resources. As for the resources, the following was found:the positive meaning of dialysis, the safety offered by close individuals and the healthcare team, the activation of specificprograms, and person-centered policies. Conclusions:The analysis and interpretation of the identifieddifficultiesand resources revealed key elements to consider when designing and implementing health programs for individuals undergoing dialysis.Future research should explore these dimensions in diverse cultural and geographical contexts to enhance generalizability and support health equity.
文摘BACKGROUND First-generation antipsychotics demonstrate certain therapeutic benefits in schizophrenia;however, they often fail to significantly address negative symptoms. Thus, continued exploration is essential to refine these treatments.AIM To examine lurasidone plus sulpiride influence on treatment efficacy, psychiatric symptoms, and quality of life in patients with schizophrenia.METHODS A total of 110 patients with schizophrenia, admitted between October 2021 and October 2024,were recruited for this study. The control group(n=50)received sulpiride alone.Conversely, the observation group(n=60)was treated with a combination of lurasidone and sulpiride.A series of assessments were conducted to compare the two groups. These included evaluating treatment efficacy;recording the incidence of adverse events such as fatigue, xerostomia, insomnia, anorexia, and headache;assessing psychiatric symptoms using the positive and negative syndrome scale(PANSS);evaluating cognitive and social functions using the repeatable battery for the assessment of neuropsychological status(RBANS),and the personal and social performance scale(PSP);measuring quality of life using the schizophrenia quality of life scale(SQLS);and analyzing serum markers including interleukin 6(IL-6),IL-17,and prolactin(PRL).RESULTS Overall treatment efficacy was significantly higher in the observation group than in the control group. The total incidence of adverse events was comparable between the two groups. After treatment, the scores for positive symptoms, negative symptoms, and general psychopathological symptoms on the PANSS in the observation group were significantly reduced compared to pretreatment levels, and were also lower than those in the control group.Additionally,RBANS and PSP scores in the observation group significantly increased post-treatment and were notably higher than in the control group.Regarding the quality of life,SQLS scores in the psychosocial, symptoms, and side effects and motivation and energy dimensions in the observation group were significantly lower after treatment than both baseline levels and those in the control group.Furthermore,post-treatment levels of IL-6 and IL-17 in the observation group were significantly reduced and lower than those in the control group, whereas the PRL level was significantly elevated.CONCLUSION The combination of lurasidone and sulpiride can effectively enhance treatment efficacy, alleviate psychiatric symptoms, and improve quality of life in patients with schizophrenia, supporting its broader clinical use.
文摘BACKGROUND Chronic kidney disease(CKD)is an incapacitating illness associated with distressing symptoms(DS)that have negative impact on patients’health-related quality of life(HRQOL).AIM To assess the severity of DS and their relationships with HRQOL among patients with CKD in Jordan.METHODS A descriptive cross-sectional design was used.A convenience sampling approach was used to recruit the participants.Patients with CKD(n=140)who visited the outpatient clinics in four hospitals in Amman between November 2021 and December 2021 were included.RESULTS The Edmonton Symptom Assessment System was used to measure the severity of the DS while the Short Form-36 tool was used to measure the HRQOL.Participants’mean age was 50.9(SD=15.14).Most of them were males(n=92,65.7%),married(n=95,67.9%),and unemployed(n=93,66.4%).The highest DS were tiredness(mean=4.68,SD=2.98)and worse well-being(mean=3.69,SD=2.43).The highest HRQOL mean score was for the bodily pain scale with a mean score of 68.50 out of 100(SD=32.02)followed by the emotional well-being scale with mean score of 67.60(SD=18.57).CONCLUSION Patients with CKD had suboptimal HRQOL,physically and mentally. They suffer from multiple DS that have a strong association with diminished HRQOL such as tiredness and depression. Therefore, healthcare providers should be equipped with the essential knowledge and skills to promote individualized strategies that focusing on symptom management.
