BACKGROUND Dry eye disease(DED)is a multifactorial ocular surface disorder with rising prevalence.It is closely related to systemic health and psychological factors,such as sleep and mood disorders,which significantly...BACKGROUND Dry eye disease(DED)is a multifactorial ocular surface disorder with rising prevalence.It is closely related to systemic health and psychological factors,such as sleep and mood disorders,which significantly impact the quality of life of patients.AIM To explore the correlations between ocular surface function,sleep quality,and anxiety/depression in patients with DED.METHODS This was a cross-sectional investigative study that included 358 patients with DED between January 2022 and January 2025.Ocular surface was assessed using the ocular surface disease index(OSDI),tear film break-up time,fluorescein staining score,and Schirmer I test.The Pittsburgh Sleep Quality Index(PSQI),Self-Rating Anxiety Scale(SAS),and Self-Rating Depression Scale(SDS)were used to evaluate sleep quality and anxiety/depression levels.Correlation and linear regression analyses were used to explore the relationships.RESULTS The mean PSQI score of the patients was 9.94±2.18;the mean SAS score was 47.30±4.90,and the mean SDS score was 50.08±5.52.These suggested a prevalence of sleep and psychological abnormalities.There was a significant correlation between the indicators of ocular surface function(OSDI,tear film break-up time,fluorescein staining,and Schirmer I test)and PSQI,SAS,and SDS scores(P<0.05).Moreover,multiple regression revealed that age≥50 years(β=1.55,P=0.029),PSQI scores(β=0.58,P<0.001),SAS scores(β=0.17,P=0.017),and SDS scores(β=0.15,P=0.019)were independent predictors of the OSDI scores.CONCLUSION Ocular surface function in patients with DED is closely related to sleep quality and anxiety/depression,emphasizing the need for holistic clinical management.展开更多
BACKGROUND Approximately 30%of patients with head and neck cancer experience adverse effects caused by anxiety and depression.Considering the high prevalence,implementing customized interventions to ease adverse emoti...BACKGROUND Approximately 30%of patients with head and neck cancer experience adverse effects caused by anxiety and depression.Considering the high prevalence,implementing customized interventions to ease adverse emotional states is imperative.AIM To evaluate the efficacy of cognitive behavioral therapy(CBT)-based psychological interventions in improving the psychological well-being and quality of life(QoL)of patients with laryngeal carcinoma.METHODS This study enrolled 120 patients admitted from February 2022 to February 2024.The control group,comprising 50 participants,received standard supportive psychological care,while the research group,consisting 70 participants,underwent CBT-based interventions.Several clinical outcomes were systematically assessed that included postoperative recovery metrics(duration of tracheostomy and nasogastric tube dependence and length of hospitalization),psychological status(Self-Rating Anxiety Scale and Self-Rating Depression Scale),nutritional markers(serum albumin and hemoglobin levels),sleep quality(Self-Rating Scale of Sleep and Athens Insomnia Scale),and QoL(Functional Assessment of Cancer Therapy-Head and Neck).RESULTS The results demonstrated that the research group experienced superior outcomes,with significantly reduced durations of tracheostomy and nasogastric tube dependence,as well as shorter hospital stays,compared with the control group.Additionally,the research group exhibited markedly lower post-intervention Self-Rating Anxiety Scale,Self-Rating Depression Scale,Self-Rating Scale of Sleep,and Athens Insomnia Scale scores,along with minimal but higher change in serum albumin and hemoglobin levels compared with the control group.All five domains of Functional Assessment of Cancer Therapy-Head and Neck showed notable improvements in the research group,exceeding those observed in the control group.CONCLUSION CBT-based psychological support positively affects the mental well-being and QoL of patients with laryngeal carcinoma,highlighting its potential for broader clinical application.展开更多

AIM:To assess the visual acuity and visual quality of cataract patients who received femtosecond laser-assisted cataract surgery(FLACS)and multifocal intraocular lens(MIOL)implantation with an improved polishing techn...AIM:To assess the visual acuity and visual quality of cataract patients who received femtosecond laser-assisted cataract surgery(FLACS)and multifocal intraocular lens(MIOL)implantation with an improved polishing technique during a 1-year follow-up period.METHODS:This study included 74 eyes from 37 patients,comprising 17 males and 20 females,with a mean age of 51.74±7.80 years.Using a coin toss method,one eye per patient was randomly selected for improved anterior capsular polishing,while the other eye received standard irrigation/aspiration(I/A)polishing.The polishing group consisted of 37 eyes,including 21 right and 16 left eyes,while the control group comprised the contralateral fellow eyes of the same individuals in the polishing group.Visual acuity and quality of the patients were evaluated before surgery and at 1 wk,1,6,and 12 mo after surgery.The OPD-Scan III was utilized to assess high-order aberrations,while the optical quality analysis system(OQAS)was employed to evaluate modulation transfer function(MTF cutoff),Strehl ratio(SR),and objective scatter index(OSI)for the purpose of visual quality assessment.Paired t-tests and repeated measures analysis of variance(ANOVA)were utilized to compare the results,and the SNK-q post hoc test was applied to identify significant differences.RESULTS:The polishing group’s uncorrected distant visual acuity(UDVA)and uncorrected near visual acuity(UNVA)significantly improved 1-week post-surgery(all P<0.05).At 12-months,UDVA,UNVA,and corrected distant visual acuity(CDVA)were better than the control group(P<0.05).The MTF cutoff,SR,OSI,and high-order aberrations in the polishing group also showed significant improvements compared to the control group at 12 mo(P<0.05).CONCLUSION:The improved capsular polishing method has been demonstrated to effectively maintain visual acuity and visual quality in patients with MIOL after FLACS within 1 a.展开更多
To investigate the efficacy of a structured continuous nursing intervention program on clinical outcomes,self-efficacy,and sleep quality in patients diagnosed with aplastic anemia(AA).Methods:This study was a single-c...To investigate the efficacy of a structured continuous nursing intervention program on clinical outcomes,self-efficacy,and sleep quality in patients diagnosed with aplastic anemia(AA).Methods:This study was a single-center,parallel-group,randomized controlled trial.A total of 64 patients with AA,treated at our hospital’s hematology department from May 2022 to May 2025,were enrolled.Participants were randomly allocated in a 1:1 ratio to either the control group(n=32),receiving routine nursing care,or the intervention group(n=32),receiving a six-month continuous nursing intervention program in addition to routine care.The intervention program consisted of structured health education,individualized psychological support,regular telephone follow-ups,and a 24/7 online communication platform.The primary outcome was the change in the Pittsburgh Sleep Quality Index(PSQI)score from baseline to six months.Secondary outcomes included hematological parameters(hemoglobin[Hb],absolute neutrophil count[ANC],platelet count[PLT]),incidence of adverse events(infections,bleeding episodes),quality of life(assessed by the Functional Assessment of Cancer Therapy–Anemia[FACT-An]),and self-efficacy(assessed by the General Self-Efficacy Scale[GSE]).Statistical analyses were performed using independent t-tests,and chi-square tests or Fisher’s exact tests as appropriate.Results:A total of 55 patients(28 in the intervention group and 27 in the control group)completed the study.At baseline,there were no statistically significant differences in demographic data,clinical characteristics,or outcome measures between the two groups(p>0.05).After six months,the mean PSQI score in the intervention group was significantly lower than that in the control group(7.12±2.05 vs.13.49±2.88;t=−13.450;p<0.001).The intervention group also demonstrated a significantly lower incidence of infections(4/28,14.29%vs.10/27,37.04%;p=0.048)and bleeding episodes requiring intervention(3/28,10.71%vs.9/27,33.33%;p=0.042).Furthermore,patients in the intervention group reported significantly higher scores on the FACT-An(125.70±10.31 vs.109.44±12.10;t=7.934;p<0.001)and the GSE scale(29.82±4.11 vs.23.51±4.80;t=7.311;p<0.001).While hematological parameters showed a trend toward improvement in the intervention group,the differences were not statistically significant compared to the control group(p>0.05).Conclusion:The implementation of a structured continuous nursing intervention program can significantly improve sleep quality,reduce the incidence of complications,and enhance quality of life and self-efficacy in patients with aplastic anemia.This evidence-based model of care should be considered for integration into standard clinical practice for the long-term management of this patient population.展开更多

The high labor demand during rice seedling cultivation and transplantation poses a significant challenge in advancing machine-transplanted rice cultivation.This problem may be solved by increasing the seeding rate dur...The high labor demand during rice seedling cultivation and transplantation poses a significant challenge in advancing machine-transplanted rice cultivation.This problem may be solved by increasing the seeding rate during seedling production while reducing the number of seedling trays.This study conducted field experiments from 2021 to 2022,using transplanting seedling ages of 10 and 15 days to explore the effects of 250,300,and 350 g/tray on the seedling quality,mechanical transplantation quality,yields,and economic benefits of rice.The commonly used combination of 150 g/tray with a 20-day seedling age in rice production was used as CK.The cultivation of seedlings under a high seeding rate and short seedling age significantly affected seedling characteristics,but there was no significant difference in seedling vitality compared to CK.The minimum number of rice trays used in the experiment was observed in the treatment of 350-10(300 g/tray and 10-day seedling age),only 152-155 trays ha^(-1),resulting in a 62%reduction in the number of trays needed.By increasing the seeding rate of rice,missed holes during mechanical transplantation decreased by 2.8 to 4%.The treatment of 300-15(300 g/tray and 15-day seedling age)achieved the highest yields and economic gains.These results indicated that using crop straw boards can reduce the application of seedling trays.On that basis,rice yields can be increased by raising the seeding rate and shortening the seedling age of rice without compromising seedling quality.展开更多
Knee osteoarthritis(KOA),characterized by heterogeneous arthritic manifestations and complex peripheral joint disorder,is one of the leading causes of disability worldwide,which has become a high burden due to the mul...Knee osteoarthritis(KOA),characterized by heterogeneous arthritic manifestations and complex peripheral joint disorder,is one of the leading causes of disability worldwide,which has become a high burden due to the multifactorial nature and the deficiency of available disease-modifying treatments.The application of mesenchymal stem/stromal cells(MSCs)as therapeutic drugs has provided novel treatment options for diverse degenerative and chronic diseases including KOA.However,the complexity and specificity of the“live”cells have posed challenges for MSC-based drug development and the concomitant scale-up preparation from laboratory to industrialization.For instance,despite the considerable progress in ex vivo cell culture technology for fulfilling the robust development of drug conversion and clinical trials,yet significant challenges remain in obtaining regulatory approvals.Thus,there’s an urgent need for the research and development of MSC drugs for KOA.In this review,we provide alternative solution strategies for the preparation of MSC drugs on the basis of the principle of quality by design,including designing the cell production processes,quality control,and clinical applications.In detail,we mainly focus on the quality by design method for MSC manufacturing in standard cell-culturing factories for the treatment of KOA by using the Quality Target Product Profile as a starting point to determine potential critical quality attributes and to establish relationships between critical material attributes and critical process parameters.Collectively,this review aims to meet product performance and robust process design,and should help to reduce the gap between compliant products and the production of compliant good manufacturing practice.展开更多

BACKGROUND The coronavirus disease 2019(COVID-19)pandemic has had profound physical,psychological,and social consequences,with lasting effects on health-related quality of life(HRQoL)among people with a history of COV...BACKGROUND The coronavirus disease 2019(COVID-19)pandemic has had profound physical,psychological,and social consequences,with lasting effects on health-related quality of life(HRQoL)among people with a history of COVID-19.AIM To synthesize the current evidence on HRQoL and long-term health outcomes among people with a history of COVID-19 in India.METHODS We incorporated studies from India reporting post-COVID HRQoL outcomes using validated instruments,including the 5-level EuroQol 5-Dimensional questionnaire,the EuroQol Visual Analogue Scale,the Short Form-36 Health Survey,the World Health Organization Quality of Life-Brief Version,and the European Health Interview Survey-Quality of Life.Pooled mean 5-level EuroQol 5-Dimensional questionnaire scores with 95%CIs were calculated for HRQoL.Adjusted odds ratios were pooled for comorbidity,disease severity,intensive care unit admission,age,sex,and vaccination status using random-effects models.RESULTS Three studies(n=1526)reported EuroQol instruments,with the 5-level EuroQol 5-Dimensional questionnaire utility scores suitable for quantitative pooling.The pooled mean utility was 0.83(95%CI:0.75-0.92),although heterogeneity was high because the included studies represented clinically distinct populations.Across all studies,several determinants were consistently associated with impaired HRQoL.Older adults(≥60 years)had higher odds of poor HRQoL[pooled odds ratio(OR)=1.83,95%CI:1.43-2.35],and females were more likely to experience impaired HRQoL(pooled OR=1.74,95%CI:1.44-2.10),whereas males had a lower risk(pooled OR=0.58,95%CI:0.48-0.70).Being unvaccinated increased the likelihood of persistent symptoms or reduced HRQoL(pooled OR=1.60,95%CI:1.21-2.14).Comorbidity(pooled OR=1.94,95%CI:1.43-2.63)and severe acute COVID-19 or intensive care unit admission(pooled OR=2.77,95%CI:2.13-3.59)were also strongly associated with poorer HRQoL Six additional studies utilizing disparate instruments(EuroQol Visual Analogue Scale,Short Form-36 Health Survey,World Health Organization Quality of Life-Brief Version,European Health Interview Survey-Quality of Life)were excluded from quantitative synthesis due to measurement heterogeneity.CONCLUSION Post-COVID HRQoL in people with a history of COVID-19 in India is suboptimal,with greater impairment observed among older adults,females,patients with comorbidities or severe disease,and unvaccinated individuals.These findings highlight the need for targeted rehabilitation and preventive strategies.展开更多
BACKGROUND Colonoscopy quality relies heavily on adequate bowel preparation,yet traditional methods often result in suboptimal compliance.Emerging network-based monitoring systems offer promise for improving both prep...BACKGROUND Colonoscopy quality relies heavily on adequate bowel preparation,yet traditional methods often result in suboptimal compliance.Emerging network-based monitoring systems offer promise for improving both preparation quality and patient cooperation,potentially enhancing clinical outcomes.AIM To evaluate the effectiveness of an intestinal network monitoring system in enhancing the quality of bowel preparation for colonoscopy and its impact on patient psychological and physiological responses,compliance,and adverse event rates.METHODS Between July 2019 and July 2020,800 enteroscopy patients who met the inclusion criteria in the outpatient clinic of the gastroenterology department of our hospital were randomly divided into 400 cases each in the experimental group(network monitoring group)and the control group(verbal+written preaching group),and the psychological and physiological stress response situation,colon Boston Bowel Preparation Scale,enteroscopy to blindness,arrival time to blindness,and polyp detection rate of the patients were compared before and after the intervention,compliance and adverse reactions were compared.RESULTS There was no difference in anxiety and depression scores,heart rate and systolic blood pressure between the groups before the intervention(P>0.05),and after the intervention,the patients’anxiety and depression scores were lower and lower in the study group(P<0.05);heart rate and systolic blood pressure were elevated,but lower in the test group(P<0.05).The left hemicolon,right hemicolon,transverse colon and total Boston Bowel Preparation Scale scores were lower in the test group than in the control group(P<0.05),the colonoscopy arrival rate and polyp detection rate were higher than those in the control group,and the time to arrival and time to exit the scope were shorter than those in the control group(P<0.05),and the dietary preparations,the preparations for taking medications and the total adherence scores were higher than those in the control group(P<0.05).The incidence of adverse reactions in the experimental group was 11.00%,which was lower than that in the control group(P<0.05).CONCLUSION The Bowel Network Monitoring System has potential clinical promotion value in improving the quality of colonoscopic bowel preparation,which can effectively alleviate patient anxiety and depression,improve the quality of colonoscopic bowel preparation and patient compliance,and has a high degree of safety.展开更多

This may be related to differences in healthcareorganizations among countries but also to the scarcity of data on this topic so far.In the present review, we assessed available evidence on quality metrics in solidorgan procurement in the United States and in Europe by means of a PubMedsearch. Evidence was summarized according to countries, considering that thedonation and transplantation systems differ from country to country. In UnitedStates, the assessment of these indicators is periodically performed by the nationalnetwork for organ sharing to evaluate the performance of each Organ ProcurementOrganization (OPO). Quality metrics consider several factors, in primispopulation characteristics (i.e. race/ethnicity, age, socio-economic status). That iswhy the assessment of each OPO performance relies on several quality metrics,not only one single indicator. In Europe, quality improvement programs representa structural element of organ and transplant system in several countries, but fewpapers have to date addressed the results obtained by a quality improvementprogram based on indicators. In Poland, the use of quality indicators and improvementprocedures were associated with better results in those hospitals whichimplemented these programs in respect to hospitals who did not. In TuscanyRegion (Italy) the implementation of a monitoring and reporting approach basedon indicators by the Regional Transplant Center was associated with an increasedin transplant and donation activity (especially in cDCD donors). BACKGROUND People living with human immunodeficiency virus(HIV)are aging as a result the benefits of combination antiretroviral therapy.AIM To provide descriptions of eligible existing studies on demographics,methodol...BACKGROUND People living with human immunodeficiency virus(HIV)are aging as a result the benefits of combination antiretroviral therapy.AIM To provide descriptions of eligible existing studies on demographics,methodologies,and outcome measures related to health-related quality of life(HRQoL)in the context of HIV and aging.METHODS The MEDLINE,CINAHL,Scopus,and PsycINFO databases were systematically searched using the terms HIV,age,and HRQoL to find studies published between January 1995 and June 2022.Key variables of the eligible studies were identified and categorized into demographics(e.g.,study sites,study year),methodologies(e.g.,use of conceptual frameworks,measures used),and outcome measures[e.g.,HRQoL,quality of life(QoL)].The PRISMA 2009 checklist was followed.RESULTS A total of 68 published studies involving 53504 participants were included.The majority of the studies(55.88%)were conducted in North America,with relatively few studies conducted in Africa.The median age of participants was 51.0 years(IQR=11.0).Over one-third(32.56%)of all participants were older people living with HIV aged 50 years and older Four studies included only older female participants,and six studies included only men who have sex with men.Outcome mea-sures were assessed as HRQoL(26.47%)or QoL(36.76%).Overall,data from African studies,older women living with HIV,socioeconomic status(e.g.,employment,income,education),sexual behavioral risks,theoretical frameworks used,and follow-up studies were limited.CONCLUSION This narrative review highlights imbalances and gaps in research on HRQoL in the context of HIV and aging,providing direction for future studies in this area.展开更多
Objective:The objective of this study is to investigate the effect of empowerment strategies on quality of life(QOL),sleep quality,level of perceived stress,and activity of daily living(ADL)among perimenopausal women....Objective:The objective of this study is to investigate the effect of empowerment strategies on quality of life(QOL),sleep quality,level of perceived stress,and activity of daily living(ADL)among perimenopausal women.Materials and Methods:A single-blinded randomized,controlled community trial was adapted.The sample consisted of 70(35 in each group)perimenopausal women in Tapoban community,Bhubaneswar.A purposive sampling technique was used.Tools used were structured sociodemographic questions,the Menopause-Specific Quality of Life Questionnaire tool,ADL scale,the Pittsburgh Sleep Quality Index(PSQI)scale,and the Perceived Stress scale.The perimenopausal empowerment strategy was introduced to the intervention arm,and the control arm received standard care provided by the government.Descriptive statistics and analytical statistics(paired t-test,Chi-square)were employed to compare groups and examine relationships.Statistical analysis was likely conducted using SPSS version 21 software.The significance level was set at<0.05.Results:The study found a significant improvement in the QOL among perimenopausal women in the experimental group after intervention(t=16.764,P<0.00001).However,there were no significant differences between the experimental and control groups in terms of IADL scores(P=0.323)and PSQI scores(P=0.323)after intervention.The control group had significantly higher perceived stress scores compared to the experimental group(P=0.003).Age and employment status showed significant association with sociodemographic factors associated with QOL.Working women had a poorer QOL compared to homemakers,which was statistically significant(P=0.023).Conclusion:Empowerment strategies,such as training on improving sleep patterns,QOL,self-care activities and reducing perceived stress,were found to be effective interventions for perimenopausal women.展开更多
Objective To develop a quality appraisal tool for case reports in traditional Chinese medicine(TCM)based on their characteristics.Methods An extensive literature search was conducted in Chinese Biomedical Literature D...Objective To develop a quality appraisal tool for case reports in traditional Chinese medicine(TCM)based on their characteristics.Methods An extensive literature search was conducted in Chinese Biomedical Literature Database(CBM),China National Knowledge Infrastructure(CNKI),and China Science and Technology Journal Database(CSTJ),focusing on expert consensus statements and check-lists for TCM case reports.Relevant items were extracted,and a Delphi method involving 34 experts was used in two rounds to rate each item on a 5-point Likert scale.Items were screened based on measures of central tendency and coordination(including total score,mean score,percentage of items rated as unimportant,and coefficient of variation).The weighted average method was used to determine item weights and construct the appraisal tool.Internal consistency was assessed using Cronbach’sαcoefficient.The finalized tool was pilot-tested by two reviewers independently appraising 20 case reports,with an additional four reviewers evaluating 5 of these cases to compare inter-rater consistency.Results A total of 9513 articles were retrieved,and 96 items from 25 articles were extracted.After two rounds of the Delphi method,27 items across 10 domains were retained.The Cron-bach’sαcoefficient was 0.72 in the first round(acceptable range),and 0.96 in the second round,indicating strong internal consistency.The tool was piloted by six reviewers,achieving a kappa value of 0.663 and a Kendall’s coefficient of concordance of 0.845,demonstrating high consistency among reviewers.Conclusion The developed TCM case report quality appraisal tool,consisting of 27 items in 10 domains,offers a scientific and reliable means of assessing the quality of TCM case reports.The tool showed high consistency and practical utility,and its application is expected to en-hance the standardization,scientific rigor,and evidence quality of TCM case reports,facilitat-ing the integration of traditional medical knowledge with modern evidence-based standards.展开更多

文摘Background:Cyperi Rhizoma,derived from Cyperus rotundus L.,is a widely used medicinal herb in traditional Chinese medicine(TCM),with Shandong Province recognized as its geo-authentic habitat.However,the quality of Cyperi Rhizoma varies significantly across different regions,potentially influencing its therapeutic efficacy.This study investigates the influence of geographic origin on the chemical composition and overall quality of Cyperi Rhizoma.Methods:A comprehensive approach,including traditional quality assessment,GC-MS(g as c hromatography-m ass s pectrometry),RP-HPLC(r everse p hase h igh-p erformance l iquid c hromatography),and FT-IR(f ourier t ransform i nfrared s pectroscopy)techniques,was employed to analyze Cyperi Rhizoma samples from Shandong Province.These methods examined the physical appearance,chemical profile,and content variations,particularly focusing onα-cyperone.Results:Traditional quality assessments revealed noticeable differences in the external characteristics of the samples.GC-MS analysis identified a variety of unique chemical constituents,while RP-HPLC and FT-IR showed significant variations inα-cyperone content,with higher levels found in Shandong samples.Conclusion:These results demonstrate that geographic origin is a critical determinant of Cyperi Rhizoma quality,with Shandong specimens exhibiting superiorα-cyperone levels and characteristic phytochemical profiles.This validates the geo-authenticity concept in TCM and provides actionable data for developing evidence-based quality standards,suggesting that provenance should be prioritized in medicinal material selection and pharmacopeial specifications.
文摘BACKGROUND Dry eye disease(DED)is a multifactorial ocular surface disorder with rising prevalence.It is closely related to systemic health and psychological factors,such as sleep and mood disorders,which significantly impact the quality of life of patients.AIM To explore the correlations between ocular surface function,sleep quality,and anxiety/depression in patients with DED.METHODS This was a cross-sectional investigative study that included 358 patients with DED between January 2022 and January 2025.Ocular surface was assessed using the ocular surface disease index(OSDI),tear film break-up time,fluorescein staining score,and Schirmer I test.The Pittsburgh Sleep Quality Index(PSQI),Self-Rating Anxiety Scale(SAS),and Self-Rating Depression Scale(SDS)were used to evaluate sleep quality and anxiety/depression levels.Correlation and linear regression analyses were used to explore the relationships.RESULTS The mean PSQI score of the patients was 9.94±2.18;the mean SAS score was 47.30±4.90,and the mean SDS score was 50.08±5.52.These suggested a prevalence of sleep and psychological abnormalities.There was a significant correlation between the indicators of ocular surface function(OSDI,tear film break-up time,fluorescein staining,and Schirmer I test)and PSQI,SAS,and SDS scores(P<0.05).Moreover,multiple regression revealed that age≥50 years(β=1.55,P=0.029),PSQI scores(β=0.58,P<0.001),SAS scores(β=0.17,P=0.017),and SDS scores(β=0.15,P=0.019)were independent predictors of the OSDI scores.CONCLUSION Ocular surface function in patients with DED is closely related to sleep quality and anxiety/depression,emphasizing the need for holistic clinical management.
文摘BACKGROUND Approximately 30%of patients with head and neck cancer experience adverse effects caused by anxiety and depression.Considering the high prevalence,implementing customized interventions to ease adverse emotional states is imperative.AIM To evaluate the efficacy of cognitive behavioral therapy(CBT)-based psychological interventions in improving the psychological well-being and quality of life(QoL)of patients with laryngeal carcinoma.METHODS This study enrolled 120 patients admitted from February 2022 to February 2024.The control group,comprising 50 participants,received standard supportive psychological care,while the research group,consisting 70 participants,underwent CBT-based interventions.Several clinical outcomes were systematically assessed that included postoperative recovery metrics(duration of tracheostomy and nasogastric tube dependence and length of hospitalization),psychological status(Self-Rating Anxiety Scale and Self-Rating Depression Scale),nutritional markers(serum albumin and hemoglobin levels),sleep quality(Self-Rating Scale of Sleep and Athens Insomnia Scale),and QoL(Functional Assessment of Cancer Therapy-Head and Neck).RESULTS The results demonstrated that the research group experienced superior outcomes,with significantly reduced durations of tracheostomy and nasogastric tube dependence,as well as shorter hospital stays,compared with the control group.Additionally,the research group exhibited markedly lower post-intervention Self-Rating Anxiety Scale,Self-Rating Depression Scale,Self-Rating Scale of Sleep,and Athens Insomnia Scale scores,along with minimal but higher change in serum albumin and hemoglobin levels compared with the control group.All five domains of Functional Assessment of Cancer Therapy-Head and Neck showed notable improvements in the research group,exceeding those observed in the control group.CONCLUSION CBT-based psychological support positively affects the mental well-being and QoL of patients with laryngeal carcinoma,highlighting its potential for broader clinical application.

文摘AIM:To assess the visual acuity and visual quality of cataract patients who received femtosecond laser-assisted cataract surgery(FLACS)and multifocal intraocular lens(MIOL)implantation with an improved polishing technique during a 1-year follow-up period.METHODS:This study included 74 eyes from 37 patients,comprising 17 males and 20 females,with a mean age of 51.74±7.80 years.Using a coin toss method,one eye per patient was randomly selected for improved anterior capsular polishing,while the other eye received standard irrigation/aspiration(I/A)polishing.The polishing group consisted of 37 eyes,including 21 right and 16 left eyes,while the control group comprised the contralateral fellow eyes of the same individuals in the polishing group.Visual acuity and quality of the patients were evaluated before surgery and at 1 wk,1,6,and 12 mo after surgery.The OPD-Scan III was utilized to assess high-order aberrations,while the optical quality analysis system(OQAS)was employed to evaluate modulation transfer function(MTF cutoff),Strehl ratio(SR),and objective scatter index(OSI)for the purpose of visual quality assessment.Paired t-tests and repeated measures analysis of variance(ANOVA)were utilized to compare the results,and the SNK-q post hoc test was applied to identify significant differences.RESULTS:The polishing group’s uncorrected distant visual acuity(UDVA)and uncorrected near visual acuity(UNVA)significantly improved 1-week post-surgery(all P<0.05).At 12-months,UDVA,UNVA,and corrected distant visual acuity(CDVA)were better than the control group(P<0.05).The MTF cutoff,SR,OSI,and high-order aberrations in the polishing group also showed significant improvements compared to the control group at 12 mo(P<0.05).CONCLUSION:The improved capsular polishing method has been demonstrated to effectively maintain visual acuity and visual quality in patients with MIOL after FLACS within 1 a.
文摘To investigate the efficacy of a structured continuous nursing intervention program on clinical outcomes,self-efficacy,and sleep quality in patients diagnosed with aplastic anemia(AA).Methods:This study was a single-center,parallel-group,randomized controlled trial.A total of 64 patients with AA,treated at our hospital’s hematology department from May 2022 to May 2025,were enrolled.Participants were randomly allocated in a 1:1 ratio to either the control group(n=32),receiving routine nursing care,or the intervention group(n=32),receiving a six-month continuous nursing intervention program in addition to routine care.The intervention program consisted of structured health education,individualized psychological support,regular telephone follow-ups,and a 24/7 online communication platform.The primary outcome was the change in the Pittsburgh Sleep Quality Index(PSQI)score from baseline to six months.Secondary outcomes included hematological parameters(hemoglobin[Hb],absolute neutrophil count[ANC],platelet count[PLT]),incidence of adverse events(infections,bleeding episodes),quality of life(assessed by the Functional Assessment of Cancer Therapy–Anemia[FACT-An]),and self-efficacy(assessed by the General Self-Efficacy Scale[GSE]).Statistical analyses were performed using independent t-tests,and chi-square tests or Fisher’s exact tests as appropriate.Results:A total of 55 patients(28 in the intervention group and 27 in the control group)completed the study.At baseline,there were no statistically significant differences in demographic data,clinical characteristics,or outcome measures between the two groups(p>0.05).After six months,the mean PSQI score in the intervention group was significantly lower than that in the control group(7.12±2.05 vs.13.49±2.88;t=−13.450;p<0.001).The intervention group also demonstrated a significantly lower incidence of infections(4/28,14.29%vs.10/27,37.04%;p=0.048)and bleeding episodes requiring intervention(3/28,10.71%vs.9/27,33.33%;p=0.042).Furthermore,patients in the intervention group reported significantly higher scores on the FACT-An(125.70±10.31 vs.109.44±12.10;t=7.934;p<0.001)and the GSE scale(29.82±4.11 vs.23.51±4.80;t=7.311;p<0.001).While hematological parameters showed a trend toward improvement in the intervention group,the differences were not statistically significant compared to the control group(p>0.05).Conclusion:The implementation of a structured continuous nursing intervention program can significantly improve sleep quality,reduce the incidence of complications,and enhance quality of life and self-efficacy in patients with aplastic anemia.This evidence-based model of care should be considered for integration into standard clinical practice for the long-term management of this patient population.

文摘reshwater essential for civilization faces risk from untreated effluents discharged by industries,agriculture,urban areas,and other sources.Increasing demand and abstraction of freshwater deteriorate the pollution scenario more.Hence,water quality analysis(WQA)is an important task for researchers and policymakers to maintain sustainability and public health.This study aims to gather and discuss the methods used for WQA by the researchers,focusing on their advantages and limitations.Simultaneously,this study compares different WQA methods,discussing their trends and future directions.Publications from the past decade on WQA are reviewed,and insights are explored to aggregate them in particular categories.Three major approaches,namely—water quality indexing,water quality modeling(WQM)and artificial intelligence-based WQM,are recognized.Different methodologies adopted to execute these three approaches are presented in this study,which leads to formulate a comparative discussion.Using statistical operations and soft computing techniques have been done by researchers to combat the subjectivity error in indexing.To achieve better results,WQMs are being modified to incorporate the physical processes influencing water quality more robustly.The utilization of artificial intelligence was primarily restricted to conventional networks,but in the last 5 years,implications of deep learning have increased rapidly and exhibited good results with the hybridization of feature extracting and time series modeling.Overall,this study is a valuable resource for researchers dedicated to WQA.

文摘The high labor demand during rice seedling cultivation and transplantation poses a significant challenge in advancing machine-transplanted rice cultivation.This problem may be solved by increasing the seeding rate during seedling production while reducing the number of seedling trays.This study conducted field experiments from 2021 to 2022,using transplanting seedling ages of 10 and 15 days to explore the effects of 250,300,and 350 g/tray on the seedling quality,mechanical transplantation quality,yields,and economic benefits of rice.The commonly used combination of 150 g/tray with a 20-day seedling age in rice production was used as CK.The cultivation of seedlings under a high seeding rate and short seedling age significantly affected seedling characteristics,but there was no significant difference in seedling vitality compared to CK.The minimum number of rice trays used in the experiment was observed in the treatment of 350-10(300 g/tray and 10-day seedling age),only 152-155 trays ha^(-1),resulting in a 62%reduction in the number of trays needed.By increasing the seeding rate of rice,missed holes during mechanical transplantation decreased by 2.8 to 4%.The treatment of 300-15(300 g/tray and 15-day seedling age)achieved the highest yields and economic gains.These results indicated that using crop straw boards can reduce the application of seedling trays.On that basis,rice yields can be increased by raising the seeding rate and shortening the seedling age of rice without compromising seedling quality.

文摘Knee osteoarthritis(KOA),characterized by heterogeneous arthritic manifestations and complex peripheral joint disorder,is one of the leading causes of disability worldwide,which has become a high burden due to the multifactorial nature and the deficiency of available disease-modifying treatments.The application of mesenchymal stem/stromal cells(MSCs)as therapeutic drugs has provided novel treatment options for diverse degenerative and chronic diseases including KOA.However,the complexity and specificity of the“live”cells have posed challenges for MSC-based drug development and the concomitant scale-up preparation from laboratory to industrialization.For instance,despite the considerable progress in ex vivo cell culture technology for fulfilling the robust development of drug conversion and clinical trials,yet significant challenges remain in obtaining regulatory approvals.Thus,there’s an urgent need for the research and development of MSC drugs for KOA.In this review,we provide alternative solution strategies for the preparation of MSC drugs on the basis of the principle of quality by design,including designing the cell production processes,quality control,and clinical applications.In detail,we mainly focus on the quality by design method for MSC manufacturing in standard cell-culturing factories for the treatment of KOA by using the Quality Target Product Profile as a starting point to determine potential critical quality attributes and to establish relationships between critical material attributes and critical process parameters.Collectively,this review aims to meet product performance and robust process design,and should help to reduce the gap between compliant products and the production of compliant good manufacturing practice.

文摘Laser directed energy deposition(LDED)is an emerging branch of metal-based additive manufacturing(AM)processes,offering unprecedented capabilities for high-performance fabrication with complex geometries and near-net shapes.This technology is gathering increasing attention from industries such as biomedical,automotive,and aerospace.However,achieving consistent part quality and desired material properties is challenging due to intricate processing parameters and potential process defects such as dynamic melt-pool behavior and localized heat accumulation.This paper reviews recent advances in on-line quality control,focusing on in-situ measurement and closed-loop control for efficient assurance of LDED-fabricated parts.The quality principles,encompassing accuracy and material performance,are summarized to lay a foundation for understanding the mechanisms of quality defects and influencing factors.This review explores and thoroughly compares advancements in indirect process measurements,such as optical,thermal,and acoustic monitoring with direct quality measurements,including laser-line scanning and operando synchrotron X-ray imaging.Depending on the sensing techniques,this paper highlights a hierarchical control strategy for adaptive parameter regulation on intra-layer and inter-layer scales.The requirements and performance of various state-of-the-art controllers are critically compared to indicate their suitable applications.The importance of machine learning in detecting process anomalies and predicting build quality based on sensory signals is also outlined.Future directions are proposed towards adaptive,automated,and intelligent quality control,with a focus on multi-modal monitoring,physics-informed neural networks for interpretable analysis,and multi-objective control applications.
文摘BACKGROUND The coronavirus disease 2019(COVID-19)pandemic has had profound physical,psychological,and social consequences,with lasting effects on health-related quality of life(HRQoL)among people with a history of COVID-19.AIM To synthesize the current evidence on HRQoL and long-term health outcomes among people with a history of COVID-19 in India.METHODS We incorporated studies from India reporting post-COVID HRQoL outcomes using validated instruments,including the 5-level EuroQol 5-Dimensional questionnaire,the EuroQol Visual Analogue Scale,the Short Form-36 Health Survey,the World Health Organization Quality of Life-Brief Version,and the European Health Interview Survey-Quality of Life.Pooled mean 5-level EuroQol 5-Dimensional questionnaire scores with 95%CIs were calculated for HRQoL.Adjusted odds ratios were pooled for comorbidity,disease severity,intensive care unit admission,age,sex,and vaccination status using random-effects models.RESULTS Three studies(n=1526)reported EuroQol instruments,with the 5-level EuroQol 5-Dimensional questionnaire utility scores suitable for quantitative pooling.The pooled mean utility was 0.83(95%CI:0.75-0.92),although heterogeneity was high because the included studies represented clinically distinct populations.Across all studies,several determinants were consistently associated with impaired HRQoL.Older adults(≥60 years)had higher odds of poor HRQoL[pooled odds ratio(OR)=1.83,95%CI:1.43-2.35],and females were more likely to experience impaired HRQoL(pooled OR=1.74,95%CI:1.44-2.10),whereas males had a lower risk(pooled OR=0.58,95%CI:0.48-0.70).Being unvaccinated increased the likelihood of persistent symptoms or reduced HRQoL(pooled OR=1.60,95%CI:1.21-2.14).Comorbidity(pooled OR=1.94,95%CI:1.43-2.63)and severe acute COVID-19 or intensive care unit admission(pooled OR=2.77,95%CI:2.13-3.59)were also strongly associated with poorer HRQoL Six additional studies utilizing disparate instruments(EuroQol Visual Analogue Scale,Short Form-36 Health Survey,World Health Organization Quality of Life-Brief Version,European Health Interview Survey-Quality of Life)were excluded from quantitative synthesis due to measurement heterogeneity.CONCLUSION Post-COVID HRQoL in people with a history of COVID-19 in India is suboptimal,with greater impairment observed among older adults,females,patients with comorbidities or severe disease,and unvaccinated individuals.These findings highlight the need for targeted rehabilitation and preventive strategies.

文摘BACKGROUND Colonoscopy quality relies heavily on adequate bowel preparation,yet traditional methods often result in suboptimal compliance.Emerging network-based monitoring systems offer promise for improving both preparation quality and patient cooperation,potentially enhancing clinical outcomes.AIM To evaluate the effectiveness of an intestinal network monitoring system in enhancing the quality of bowel preparation for colonoscopy and its impact on patient psychological and physiological responses,compliance,and adverse event rates.METHODS Between July 2019 and July 2020,800 enteroscopy patients who met the inclusion criteria in the outpatient clinic of the gastroenterology department of our hospital were randomly divided into 400 cases each in the experimental group(network monitoring group)and the control group(verbal+written preaching group),and the psychological and physiological stress response situation,colon Boston Bowel Preparation Scale,enteroscopy to blindness,arrival time to blindness,and polyp detection rate of the patients were compared before and after the intervention,compliance and adverse reactions were compared.RESULTS There was no difference in anxiety and depression scores,heart rate and systolic blood pressure between the groups before the intervention(P>0.05),and after the intervention,the patients’anxiety and depression scores were lower and lower in the study group(P<0.05);heart rate and systolic blood pressure were elevated,but lower in the test group(P<0.05).The left hemicolon,right hemicolon,transverse colon and total Boston Bowel Preparation Scale scores were lower in the test group than in the control group(P<0.05),the colonoscopy arrival rate and polyp detection rate were higher than those in the control group,and the time to arrival and time to exit the scope were shorter than those in the control group(P<0.05),and the dietary preparations,the preparations for taking medications and the total adherence scores were higher than those in the control group(P<0.05).The incidence of adverse reactions in the experimental group was 11.00%,which was lower than that in the control group(P<0.05).CONCLUSION The Bowel Network Monitoring System has potential clinical promotion value in improving the quality of colonoscopic bowel preparation,which can effectively alleviate patient anxiety and depression,improve the quality of colonoscopic bowel preparation and patient compliance,and has a high degree of safety.
文摘Evidence is not homogeneous on indicators able to monitor and assess qualityperformance for organ donation. This may be related to differences in healthcareorganizations among countries but also to the scarcity of data on this topic so far.In the present review, we assessed available evidence on quality metrics in solidorgan procurement in the United States and in Europe by means of a PubMedsearch. Evidence was summarized according to countries, considering that thedonation and transplantation systems differ from country to country. In UnitedStates, the assessment of these indicators is periodically performed by the nationalnetwork for organ sharing to evaluate the performance of each Organ ProcurementOrganization (OPO). Quality metrics consider several factors, in primispopulation characteristics (i.e. race/ethnicity, age, socio-economic status). That iswhy the assessment of each OPO performance relies on several quality metrics,not only one single indicator. In Europe, quality improvement programs representa structural element of organ and transplant system in several countries, but fewpapers have to date addressed the results obtained by a quality improvementprogram based on indicators. In Poland, the use of quality indicators and improvementprocedures were associated with better results in those hospitals whichimplemented these programs in respect to hospitals who did not. In TuscanyRegion (Italy) the implementation of a monitoring and reporting approach basedon indicators by the Regional Transplant Center was associated with an increasedin transplant and donation activity (especially in cDCD donors). According toavailable evidence, the development of a method for quality assessment andquality improvement has been recognized as pivotal for donation and transplantauthorities to identify key interventions either at national and/or hospital levels.
文摘This study aims to analyze and discuss the current situation of quality control and quality management in medical imaging.Firstly,it defines the concepts of quality management and image quality,emphasizing the crucial role of the department's Quality and Safety Team in imaging quality management[1].Subsequently,it details the composition and responsibilities of the Quality and Safety Management Group,as well as the specific divisions of labor and tasks of the Quality Management Team.The effective implementation of comprehensive quality control in the imaging department is ensured through the"Seven-Star Sword"method,which encompasses total process control,organization-wide participation,and total quality control[2].Furthermore,the application of the PDCA(Plan-Do-Check-Act)cycle in quality management is explored,along with strategies to improve patient satisfaction by reducing the waiting time for imaging reports[3].Finally,the management of a key departmental medical quality control indicator—critical values—is analyzed,and improvement measures are proposed[4].This study underscores the importance of quality management in enhancing medical service quality and ensuring patient safety,and puts forward specific implementation strategies and improvement methods.
文摘BACKGROUND People living with human immunodeficiency virus(HIV)are aging as a result the benefits of combination antiretroviral therapy.AIM To provide descriptions of eligible existing studies on demographics,methodologies,and outcome measures related to health-related quality of life(HRQoL)in the context of HIV and aging.METHODS The MEDLINE,CINAHL,Scopus,and PsycINFO databases were systematically searched using the terms HIV,age,and HRQoL to find studies published between January 1995 and June 2022.Key variables of the eligible studies were identified and categorized into demographics(e.g.,study sites,study year),methodologies(e.g.,use of conceptual frameworks,measures used),and outcome measures[e.g.,HRQoL,quality of life(QoL)].The PRISMA 2009 checklist was followed.RESULTS A total of 68 published studies involving 53504 participants were included.The majority of the studies(55.88%)were conducted in North America,with relatively few studies conducted in Africa.The median age of participants was 51.0 years(IQR=11.0).Over one-third(32.56%)of all participants were older people living with HIV aged 50 years and older Four studies included only older female participants,and six studies included only men who have sex with men.Outcome mea-sures were assessed as HRQoL(26.47%)or QoL(36.76%).Overall,data from African studies,older women living with HIV,socioeconomic status(e.g.,employment,income,education),sexual behavioral risks,theoretical frameworks used,and follow-up studies were limited.CONCLUSION This narrative review highlights imbalances and gaps in research on HRQoL in the context of HIV and aging,providing direction for future studies in this area.
文摘Objective:The objective of this study is to investigate the effect of empowerment strategies on quality of life(QOL),sleep quality,level of perceived stress,and activity of daily living(ADL)among perimenopausal women.Materials and Methods:A single-blinded randomized,controlled community trial was adapted.The sample consisted of 70(35 in each group)perimenopausal women in Tapoban community,Bhubaneswar.A purposive sampling technique was used.Tools used were structured sociodemographic questions,the Menopause-Specific Quality of Life Questionnaire tool,ADL scale,the Pittsburgh Sleep Quality Index(PSQI)scale,and the Perceived Stress scale.The perimenopausal empowerment strategy was introduced to the intervention arm,and the control arm received standard care provided by the government.Descriptive statistics and analytical statistics(paired t-test,Chi-square)were employed to compare groups and examine relationships.Statistical analysis was likely conducted using SPSS version 21 software.The significance level was set at<0.05.Results:The study found a significant improvement in the QOL among perimenopausal women in the experimental group after intervention(t=16.764,P<0.00001).However,there were no significant differences between the experimental and control groups in terms of IADL scores(P=0.323)and PSQI scores(P=0.323)after intervention.The control group had significantly higher perceived stress scores compared to the experimental group(P=0.003).Age and employment status showed significant association with sociodemographic factors associated with QOL.Working women had a poorer QOL compared to homemakers,which was statistically significant(P=0.023).Conclusion:Empowerment strategies,such as training on improving sleep patterns,QOL,self-care activities and reducing perceived stress,were found to be effective interventions for perimenopausal women.

文摘Objective To develop a quality appraisal tool for case reports in traditional Chinese medicine(TCM)based on their characteristics.Methods An extensive literature search was conducted in Chinese Biomedical Literature Database(CBM),China National Knowledge Infrastructure(CNKI),and China Science and Technology Journal Database(CSTJ),focusing on expert consensus statements and check-lists for TCM case reports.Relevant items were extracted,and a Delphi method involving 34 experts was used in two rounds to rate each item on a 5-point Likert scale.Items were screened based on measures of central tendency and coordination(including total score,mean score,percentage of items rated as unimportant,and coefficient of variation).The weighted average method was used to determine item weights and construct the appraisal tool.Internal consistency was assessed using Cronbach’sαcoefficient.The finalized tool was pilot-tested by two reviewers independently appraising 20 case reports,with an additional four reviewers evaluating 5 of these cases to compare inter-rater consistency.Results A total of 9513 articles were retrieved,and 96 items from 25 articles were extracted.After two rounds of the Delphi method,27 items across 10 domains were retained.The Cron-bach’sαcoefficient was 0.72 in the first round(acceptable range),and 0.96 in the second round,indicating strong internal consistency.The tool was piloted by six reviewers,achieving a kappa value of 0.663 and a Kendall’s coefficient of concordance of 0.845,demonstrating high consistency among reviewers.Conclusion The developed TCM case report quality appraisal tool,consisting of 27 items in 10 domains,offers a scientific and reliable means of assessing the quality of TCM case reports.The tool showed high consistency and practical utility,and its application is expected to en-hance the standardization,scientific rigor,and evidence quality of TCM case reports,facilitat-ing the integration of traditional medical knowledge with modern evidence-based standards.

文摘Energy is the bedrock of national economic development and a vital pillar for achieving sustainable growth.High-quality energy cooperation is critical to ensuring sustained,stable national economic development and the transformation of the energy sector.Based on the achievements of high-quality energy cooperation within the Belt and Road Initiative,this paper systematically analyzes the current state of energy cooperation under the Belt and Road Initiative.In accordance with the intrinsic requirements for high-quality energy cooperation,and building upon the analysis and summary of prior research,this paper proposes strategies to promote high-quality energy cooperation within the Belt and Road Initiative:(1)Innovating cooperative concepts and optimizing the cooperative environment;(2)Innovating cooperation modes and expanding the scope of cooperation;(3)Improving the cooperation mechanism and building a multi-level cooperation platform;(4)Strengthening cultural exchanges and promoting dialogue.This paper offers insights and support for fostering high-quality collaboration within the Belt and Road Initiative and enhancing international energy cooperation.
