文摘Rheumatoid arthritis(RA)patients face significant psychological challenges alongside physical symptoms, necessitating a comprehensive understanding of how psychological vulnerability and adaptation patterns evolve throughout the disease course. This review examined 95 studies(2000-2025)from PubMed,Web of Science, and CNKI databases including longitudinal cohorts, randomized controlled trials, and mixed-methods research,to characterize the complex interplay between biological, psychological, and social factors affecting RA patients’mental health.Findings revealed three distinct vulnerability trajectories(45%persistently low,30%fluctuating improvement,25%persistently high)and four adaptation stages, with critical intervention periods occurring 3-6 months postdiagnosis and during disease flares.Multiple factors significantly influence psychological outcomes, including gender(females showing 1.8-fold increased risk),age(younger patients experiencing 42%higher vulnerability),pain intensity, inflammatory markers, and neuroendocrine dysregulation(48%showing cortisol rhythm disruption).Early psychological intervention(within 3 months of diagnosis)demonstrated robust benefits, reducing depression incidence by 42%with effects persisting 24-36 months, while different modalities showed complementary advantages:Cognitive behavioral therapy for depression(Cohen’s d=0.68),mindfulness for pain acceptance(38%improvement),and peer support for meaning reconstruction(25.6%increase).These findings underscore the importance of integrating routine psychological assessment into standard RA care, developing stage-appropriate interventions, and advancing research toward personalized biopsychosocial approaches that address the dynamic psychological dimensions of the disease.
文摘BACKGROUND Approximately 30%of patients with head and neck cancer experience adverse effects caused by anxiety and depression.Considering the high prevalence, implementing customized interventions to ease adverse emotional states is imperative.AIM To evaluate the efficacy of cognitive behavioral therapy(CBT)-based psychological interventions in improving the psychological well-being and quality of life(QoL)of patients with laryngeal carcinoma.METHODS This study enrolled 120 patients admitted from February 2022 to February 2024.The control group, comprising 50 participants, received standard supportive psychological care, while the research group, consisting 70 participants, underwent CBT-based interventions.Several clinical outcomes were systematically assessed that included postoperative recovery metrics(duration of tracheostomy and nasogastric tube dependence and length of hospitalization),psychological status(Self-Rating Anxiety Scale and Self-Rating Depression Scale),nutritional markers(serum albumin and hemoglobin levels),sleep quality(Self-Rating Scale of Sleep and Athens Insomnia Scale),and QoL(Functional Assessment of Cancer Therapy-Head and Neck).RESULTS The results demonstrated that the research group experienced superior outcomes, with significantly reduced durations of tracheostomy and nasogastric tube dependence,as well as shorter hospital stays, compared with the control group.Additionally, the research group exhibited markedly lower post-intervention Self-Rating Anxiety Scale,Self-Rating Depression Scale,Self-Rating Scale of Sleep, and Athens Insomnia Scale scores, along with minimal but higher change in serum albumin and hemoglobin levels compared with the control group. All five domains of Functional Assessment of Cancer Therapy-Head and Neck showed notable improvements in the research group, exceeding those observed in the control group.CONCLUSION CBT-based psychological support positively affects the mental well-being and QoL of patients with laryngeal carcinoma, highlighting its potential for broader clinical application.
文摘This review explored the application and effectiveness of and research gaps in integrative psychological support in patients undergoing cardiac surgery during the perioperative period. These patients often encounter psychological issues such as anxiety and depression, which affect their postoperative recovery and quality of life.Previous research indicated that preoperative psychological assessments and interventions significantly enhance patients’psychological states and posto-perative outcomes.Interventions, including preoperative education,mindfulness-based stress reduction, and cognitive training, have proven effective in reducing the incidence of postoperative delirium and anxiety.Intraoperative environment optimization(e.g.,music therapy)and postoperative psychological support(e.g.,family visits and psychological interventions in intensive care unit psychological interventions)positively influence patient recovery.Furthermore,theory-based nursing interventions and digital health tools(e.g.,remote monitoring applica-tions)offer new directions for psychological support.However,challenges,inclu-ding fragmented psychological support, insufficient cross-stage integration, and inadequate training, remain in current nursing practices.Future research should focus on developing standardized psychological support pathways, enhancing multidisciplinary collaboration, and verifying the long-term effects of digital the-rapeutics.
文摘BACKGROUND Ovarian cancer patients often face complex treatment processes and psychological challenges, with different treatment modalities potentially affecting patients’psychological adjustment abilities.AIM To explore the differences in psychological adjustment patterns among ovarian cancer patients receiving surgery, chemotherapy, targeted therapy, and combined therapy, and to analyze their relationship with clinical outcomes.METHODS A retrospective analysis was conducted on the clinical data of 286 ovarian cancer patients who received different treatment modalities from January 2020 to December 2023.Patients were divided into surgery group(n=78),chemotherapy group(n=65),targeted therapy group(n=61),and combined therapy group(n=82).The Self-Rating Anxiety Scale,Self-Rating Depression Scale, and Psychological Adjustment to Cancer Scale were used to assess psychological status, while quality of life, treatment adherence, and two-year survival rate data were collected. Some patients(n=76)received systematic psychological intervention, and the intervention effects were evaluated.RESULTS Patients in the combined therapy group had significantly higher Self-Rating Anxiety Scale(56.3±7.2)and Self-Rating Depression Scale(58.4±6.9)scores than other groups, with the highest incidence of anxiety(58.5%)and depression(62.2%);the targeted therapy group scored highest in the positive coping dimension(28.5±3.6)and had the lowest incidence of anxiety and depression(29.5%/31.1%).Logistic regression analysis showed that positive coping(odds ratio=2.86,95%confidence interval:1.75-4.68)and utilization of social support(odds ratio=2.13,95%confidence interval:1.42-3.56)were protective factors for good treatment adherence.Longitudinal assessment showed that although all patients experienced increased anxiety and depression symptoms at 3 months of treatment, the targeted therapy group and surgery group showed significant improvement at 6 months(P<0.05),while the combined therapy group showed no significant improvement.Psychological intervention effectively improved patients’treatment adherence(by 22.7%)and quality of life(by 15.6 points),with the best effect in the combined therapy group(anxiety incidence decreased by 30.5%,P<0.001).CONCLUSION Different treatment modalities significantly affect the psychological adjustment abilities of ovarian cancer patients, with combined therapy patients facing greater psychological challenges, while targeted therapy patients exhibit healthier psychological adjustment patterns.

文摘Objective Accurately identifying the key influencing factors of psychological birth trauma in primiparous women is crucial for implementing effective preventive and intervention measures. This study aimed to develop and validate an interpretable machine learning prediction model for identifying the key influencing factors of psychological birth trauma in primiparous women. Methods A multicenter cross-sectional study was conducted on primiparous women in four tertiary hospitals in Sichuan Province, southwestern China, from December 2023 to March 2024.The Childbirth Trauma Index was used in assessing psychological birth trauma in primiparous women.Data were collected and randomly divided into a training set(80%,n=289)and a testing set(20%,n=73).Six different machine learning models were trained and tested.Training and prediction were conducted using six machine learning models included Linear Regression,Support Vector Regression,Multilayer Perceptron Regression,eXtreme Gradient Boosting Regression,Random Forest Regression, and Adaptive Boosting Regression. The optimal model was selected based on various performance metrics, and its predictive results were interpreted using SHapley Additive exPlanations(SHAP)and accumulated local effects(ALE).Results Among the six machine learning models, the Multilayer Perceptron Regression model exhibited the best overall performance in the testing set(MAE=3.977,MSE=24.832,R2=0.507,EVS=0.524,RMSE=4.983).In the testing set,the R2 and EVS of the Multilayer Perceptron Regression model increased by 8.3%and 1.2%,respectively,compared to the traditional linear regression model.Meanwhile, the MAE,MSE,and RMSE decreased by 0.4%,7.3%,and 3.7%,respectively,compared to the traditional linear regression model. The SHAP analysis indicated that intrapartum pain, anxiety, postpartum pain, resilience, and planned pregnancy are the most critical influencing factors of psychological birth trauma in primiparous women. The ALE analysis indicated that higher intrapartum pain, anxiety, and postpartum pain scores are risk factors, while higher resilience scores are protective factors. Conclusions Interpretable machine learning prediction models can identify the key influencing factors of psychological birth trauma in primiparous women.SHAP and ALE analyses based on the Multilayer Perceptron Regression model can help healthcare providers understand the complex decision-making logic within a prediction model. This study provides a scientific basis for the early prevention and personalized intervention of psychological birth trauma in primiparous women.
文摘BACKGROUND Long-term chemotherapy for patients with gastric cancer(GC),facilitated by peripherally inserted central catheter(PICC)catheterization, reduces vascular damage and enhances drug delivery efficiency but carries risks of catheter-related complications.A combination of group psychological nursing and physical mo-vement care significantly mitigates the risk of venous thrombosis and improves psychological well-being,and enhances motor function, underscoring its clinical importance.AIM To assess group psychological and physical movement nursing in preventing venous thrombosis in patients with PICC GC.METHODS Sixty-five GC patients with PICC,admitted from January 2022 to January 2023,were randomly divided into two groups using the lottery method:A control group(n=35,routine nursing)and an observation group(n=30,routine nursing plus psychological nursing and physical movement nursing). Both groups re-ceived continuous care for 2 weeks.Pre-nursing and post-nursing data on psycho-logical state, physical function,chemotherapy-related thrombosis incidence, and cancer-related fatigue were analyzed using SPSS 26.0 and GraphPad Prism 8.0.RESULTS After nursing, both groups showed reduced Hamilton Anxiety Scale scores and increased General Perceived Self-Efficacy Scale scores, with the observation group performing better(P<0.05).The Functional Comprehensive Assessment score for the observation group after nursing was(65.42±2.35)points,lower than the control group’s(62.19±4.33)points(P<0.05).Although no significant difference was observed in the incidence of venous thrombosis between the two groups(χ2=0.815,P=0.367),the observation group had lower incidence. Both groups showed decreased Revised Piper Fatigue Scale scores, with the observation group scoring lower(P<0.05).CONCLUSION Group psychological and physical movement nursing for patients with PICC reduces venous thrombosis risk, improves psychological well-being,cancer-related fatigue, and physical function, making it highly promotable.
文摘BACKGROUND Endoscopic submucosal dissection(ESD)has become a widely accepted, minimally invasive treatment for gastrointestinal submucosal tumors. It has been reported that humanistic nursing care with graded psychological interventions can effectively enhance patients’physical activity in patients, reduce postoperative complications, and improve their postoperative quality of life.AIM To investigate the effects of combining humanistic care with graded psychological support on nursing satisfaction and quality of life in patients undergoing ESD for gastrointestinal submucosal tumors.METHODS A retrospective analysis was conducted on the clinical data of 180 patients who underwent ESD surgery for gastrointestinal submucosal tumors at our hospital between March 2021 and February 2023.Patients were allocated into groups based on the nursing care they received:The control group, which received routine care(n=90),and the observation group, which was subjected to humanistic nursing care in combination with graded psychological support(n=90).Patient anxiety and depression were assessed using the self-rating anxiety scale(SAS)and self-rating depression scale(SDS).Quality of life was evaluated using the shortform 36 health survey, and additional indications such as time to first food intake, surgery duration, length of hospital stay, nursing satisfaction, and adverse reactions were also recorded.Data was analyzed using SPSS22.0,with t-tests employed for continuous variables andχ2 tests for categorical data.RESULTS Patients in the observation group experienced significantly shorter times to first postoperative meal, surgery, and hospital stay compared to the control group. After the intervention, the SAS score of the observation group was 43.17±5.68,and the SDS score was 41.57±6.52,both significantly lower than those of the control group, with SAS score of 52.38±5.21 and SDS score of 51.23±8.25.In addition, the observation group scored significantly higher in daily living, physical function, psychological well-being,and social functioning(80.01±6.39,83.59±6.89,81.69±5.34,and 85.23±6.05,respectively).Moreover,the observation group also exhibited higher satisfaction and selfefficacy scores and a lower incidence of adverse reactions compared to the control group(P<0.05).CONCLUSION For patients undergoing ESD for gastrointestinal submucosal tumors, humanistic nursing care in combination with graded psychological nursing care significantly shorten the times to first postoperative meal, surgery, and hospital stay, effectively alleviates anxiety and depression, improves quality of life and nursing satisfaction, and mitigate the incidence of adverse reactions.

文摘BACKGROUND Frailty has become a significant public health issue. The recent increase in the number of frail older adults has led to increased attention being paid to psycho-logical care services in communities. The social isolation of pre-frail older adults can impact their psychological distress.AIM To explore the mediating effect of health literacy between social isolation and psychological distress among communitydwelling older adults with pre-frailty.METHODS This descriptive cross-sectional study was conducted with 254 pre-frail older individuals aged 60 years and over.Social isolation, health literacy, and psycholo-gical distress were comprehensively measured using the Lubben Social Network Scale-6,12-item Short-Form Health Literacy Questionnaire, and the Kessler Psy-chological Distress Scale-10.Data were evaluated using the SPSS 27.0 package program and the PROCESS macro tool.Descriptive statistical analyses, correlation analyses, and bootstrap mediation tests were used to assess associations between the variables.RESULTS The results showed that social isolation had an effect on health literacy among pre-frail older adults(β=0.240,P<0.001),social isolation impact on psycho-logical distress pre-frail older adults(β=-0.415,P<0.001);health literacy was identified effect on psychological distress among pre-frail older persons(β=-0.307,P<0.001).Health literacy partially mediated the relationship between social isolation and psychological distress among community-dwelling older adults with pre-frailty,with a mediation effect of-0.074,accounting for 17.83%of the total effect.
