文摘BACKGROUND Return to work(RTW)serves as an indication for young and middle-aged colorectal cancer(CRC)survivors to resume their normal social lives. However, these survivors encounter significant challenges during their RTW process.Hence, scientific research is necessary to explore the barriers and facilitating factors of returning to work for young and middle-aged CRC survivors.AIM To examine the current RTW status among young and middle-aged CRC survivors and to analyze the impact of RTW self-efficacy(RTW-SE),fear of progression(FoP),eHealth literacy(eHL),family resilience(FR),and financial toxicity(FT)on their RTW outcomes.METHODS A cross-sectional investigation was adopted in this study. From September 2022 to February 2023,a total of 209 participants were recruited through a convenience sampling method from the gastrointestinal surgery department of a class A tertiary hospital in Chongqing. The investigation utilized a general information questionnaire alongside scales assessing RTW-SE,FoP,eHL,FR,and FT.To analyze the factors that influence RTW outcomes among young and middle-aged CRC survivors,Cox regression modeling and Kaplan-Meier survival analysis were used.RESULTS A total of 43.54%of the participants successfully returned to work, with an average RTW time of 100 days.Cox regression univariate analysis revealed that RTW-SE,FoP,eHL,FR,and FT were significantly different between the non-RTW and RTW groups(P<0.05).Furthermore,Cox regression multivariate analysis identified per capita family monthly income, job type,RTW-SE,and FR as independent influencing factors for RTW(P<0.05).CONCLUSION The RTW rate requires further improvement.Elevated levels of RTW-SE and FR were found to significantly increase RTW among young and middle-aged CRC survivors.Health professionals should focus on modifiable factors, such as RTW-SE and FR,to design targeted RTW support programs, thereby facilitating their timely reintegration into mainstream society.
文摘Background:Hepatocellular carcinoma(HCC)is the most common cause of cancer-related death in Saudi Arabia. Our study aimed to investigate the patterns of HCC and the effect of TNM staging,Alfa-fetoprotein(AFP),and Child-Turcotte Pugh(CTP)on patients’overall survival(OS).Methods:A retrospective analysis was conducted on 43 HCC patients at a single oncology center in Saudi Arabia from 2015 to 2020.All patients had to fulfill one of the following criteria:(a)a liver lesion reported as definitive HCC on dynamic imaging and/or(b)a biopsy-confirmed diagnosis. Results:The mean patient age of all HCC cases was 66.8 with a male-to-female ratio of 3.3:1.All patients were stratified into two groups:viral HCC(n=22,51%)and non-viral HCC(n=21,49%).Among viral-HCC patients,55%were due to HBV and 45%due to HCV.Cirrhosis was diagnosed in 79%of cases.Age and sex did not significantly statistically differ in OS among viral and non-viral HCC patients(p-value>0.05).About 65%of patients had tumor size>5 cm during the diagnosis, with a significant statistical difference in OS(p-value=0.027).AFP was>400 ng/ml in 45%of the patients. There was a statistically significant difference in the OS in terms of AFP levels(p-value=0.021).A statistically significant difference was also observed between the CTP score and OS(p-value=0.02).CTP class B had the longest survival.BSC was the most common treatment provided to HCC patients followed by sorafenib therapy. There was a significant statistical difference in OS among viral and non-viral HCC patients(p-value=0.008).Conclusions:The most common predictors for OS were the underlying cause of HCC,AFP,and tumor size.Being having non-viral etiology,a tumor size>5 cm,an AFP>400 ng/mL,and a CTP score class C were all negatively associated with OS.
文摘Let a_(1),a_(2),a_(3)be nonzero integers with gcd(a_(1),a_(2),a_(3))=1,and let k be any positive integer,K=max[3,|a_(1)|,|a_(2)|,|a_(3)|,k].Suppose that l_(1),l_(2),l_(3)are integers each coprime to k.Suppose further that b is any integer satisfying some necessary congruent conditions. The solvability of linear equation a_(1)p_(1)+a_(2)p_(2)+a_(3)p_(3)=b(p_(j)=l_(j)(mod k),1≤j≤3)with prime variables pi,p_(2),ps is investigated. It is proved that if ai,a_(2),a_(3)are all positive, then the above equation is solvable whenever b≥K^(25);if a,a_(2),a_(3)are not all of the same sign, then the above equation has a solution p_(1),p_(2),p_(3)satisfying max(p_(1),p_(2),p_(3))≤3|b|+K^(25).

文摘Ovarian cancer(OC),a highly lethal gynaecological malignancy,is often diagnosed at advanced stages, resulting in a poor prognosis.Sialylation,an important form of glycosylation, significantly contributes to the progression of various solid tumours, including OC.Aberrant sialylation promotes tumour progression and metastasis by altering the structure and function of glycoproteins.Although its role in several solid tumours is well documented, the role of abnormal sialylation in OC and its potential as a therapeutic target remain poorly understood. This review highlights sialylation as a key regulator of the progression, metastasis, and drug resistance of OC.A deeper understanding of altered sialylation can contribute to the identification of novel therapeutic strategies for OC.
文摘Objective We examined the associations between obesity-related indices and the risk of diabetes progression from prediabetes in older adults, comparing the differences in using the American Diabetes Association(ADA)and World Health Organization(WHO)criteria. Methods Data were obtained from the Healthy Aging Evaluation Longitudinal Study conducted in China. At baseline, prediabetes(in participants without diabetes)was classified based on fasting plasma glucose(FPG)levels using both criteria.Body mass index(BMI)and waist circumference(WC)were categorized according to data distribution and diagnostic cut-off values, respectively.Cox proportional hazards regression models were used to estimate the adjusted hazard ratios(aHRs)with 95%confidence intervals(CIs)for obesity-related indices and diabetes progression from prediabetes. Results Among the 1,127 participants classified as prediabetic according to the ADA criteria,474 met the WHO criteria.Under ADA-defined prediabetes, the highest WC quartile(≥93 cm)was significantly associated with an increased diabetes risk(aHR 1.93[1.06,3.53,P<0.05]),whereas BMI-related and cut-off-based abdominal obesity demonstrated no significant associations(P>0.05).Under WHOdefined prediabetes, both the high tertile of WC(≥90 cm)and general obesity(BMI≥28.0 kg/m^(2))were significantly associated with progression to diabetes(P<0.05),with aHR 2.13(1.06,4.27)and 2.44(1.19,5.01),respectively.However,cut-off-based abdominal obesity and the high BMI tertile(≥25.75 kg/m^(2))were not significantly associated with diabetes progression(P>0.05).Conclusion Elevated WC,rather than BMI-based indices or cut-off-based abdominal obesity, was significantly associated with diabetes progression according to the ADA-defined prediabetes criteria. However, both the evaluated WC and general obesity predicted progression to diabetes according to the WHO criteria.

文摘Epidermal growth factor receptor(EGFR)mutations are among the most prevalent driver gene alterations in non-small cell lung cancer(NSCLC).Osimertinib, with or without chemotherapy, the first-line standard treatment for patients with advanced NSCLC bearing sensitive EGFR mutations, significantly prolongs the progression-free survival(PFS)to 25.5 months1.Despite great breakthroughs in survival data, patients inevitably experience disease progression.A large meta-analysis has indicated that, compared with chemother-apy,immuno-based therapies achieve longer PFS in patients with EGFR mutation who progressed on third-generation epidermal growth factor receptor tyrosine kinase inhibitors(EGFR-TKIs)2.Therefore,immunotherapies are often used after EGFR-TKI resistance is observed.

文摘BACKGROUND Emerging evidence has demonstrated that ANXA13 is closely related to the occurrence and development of malignant tumors. However, the functions and underlying molecular mechanisms of ANXA13 in hepatocellular carcinoma(HCC)have not been defined.AIM To examine the expression of ANXA13 in HCC,investigate its correlation with clinicopathological features.METHODS Quantitative real-time PCR and western blotting were performed to detect the ANXA13 expression in HCC tissues and cell lines at the mRNA and protein levels, respectively.Transwell and cell counting kit-8 assays were performed to assess the effects of ANXA13 overexpression on the proliferation and migration of Huh7 cells.RESULTS ANXA13 mRNA was significantly downregulated in HCC tissues, while protein levels were elevated.ANXA13 expression correlated positively with tumor diameter and tumor node metastasis stage. In HCC cell lines(Hep3B and Huh7),ANXA13 expression was higher.Overexpression of ANXA13 enhanced the proliferation and migration capabilities of Huh7 cells.CONCLUSION ANXA13 was upregulated in HCC.Its overexpression promotes tumor progression and is associated with advanced clinicopathological features, suggesting ANXA13 as a potential biomarker and therapeutic target for HCC.
文摘Gastric cancer(GC)is one of the most aggressive malignancies worldwide and is characterized by its poor prognosis and resistance to conventional therapies.Autophagy and long non-coding RNAs(lncRNAs)play critical yet complex roles in GC,functioning as both tumor suppressors and promoters depending on the disease stage and context.Autophagy influences cellular homeostasis and metabolism, whereas lncRNAs regulate gene expression through epigenetic modifications,RNA sponging, and protein interactions.Notably, the interplay between lncRNAs and autophagy modulates tumor progression, metastasis, chemoresistance, and the tumor microenvironment. This study explored the intricate relationship between lncRNAs and autophagy in GC,highlighting their roles in pathogenesis and treatment resistance.By addressing current knowledge gaps and proposing innovative therapeutic strategies,we have emphasized the potential of targeting this dynamic interplay for improved diagnostic and therapeutic outcomes.
文摘Hepatitis B virus(HBV)infection is still a major worldwide health concern, contributing to chronic liver disorders like hepatocellular carcinoma(HCC).This review comprehensively analyzes HBV genotype distribution, mutation patterns, and their clinical implications, focusing on diagnostic and therapeutic strategies for HBV-positive patients. The discussion begins with HBV virology, emphasizing its capacity for chronic hepatitis and its association with severe liver complications, notably HCC.Understanding HBV genotypes(A-J)and their distinct geographic distributions is crucial,as genotype variations influence disease progression and treatment responses.Genotypes like C are particularly linked to heightened HCC risk, highlighting the need for genotype-specific management strategies. The genomic structure of HBV,consisting of four open reading frames(ORFs)encoding essential viral proteins,is detailed, with emphasis on mutations within these ORFs influenced by host immune responses and antiviral therapies. These mutations contribute to viral resistance and virulence, impacting treatment outcomes through alterations in viral replication dynamics.Clinical implications are explored through genotype-specific impacts on disease outcomes and treatment approaches.Genotype and mutation analysis guide personalized treatment regimens, optimizing therapeutic efficacy while minimizing adverse effects and preventing drug resistance.Diagnostic molecular techniques such as polymerase chain reaction and sequencing are pivotal in genotype and mutation detection, facilitating tailored treatment decisions.

文摘Biotite content critically influences rock mechanical behavior and threatens underground engineering stability.Uniaxial compression tests with acoustic emission(AE)monitoring were conducted on granite pegmatite samples having varying biotite content.Peak frequency distribution analysis, rise angleaverage frequency(RA-AF)analysis,multifractal theory, and a dynamic multifractal algorithm were applied to explore the relationship between damage evolution and AE characteristics. Results indicate that increased biotite content reduces uniaxial compressive strength and elastic modulus, enhances plastic deformation, and increases the proportion of shear cracks. The segmented evolution of the dynamic multifractal parameter Δα_(m) is biotite-dependent.Oscillations during the elastic phase signify localized shear crack initiation and propagation;their attenuation in the plastic phase reflects frictional closure along biotite cleavage planes, promoting elastic energy storage and delaying release.AE-based damage models and time-varying signals characterize rock damage progression.Stress concentrations around biotite minerals foster localized shear band formation, leading to concentrated shear failure at lower damage levels.Higher biotite content accelerates crack propagation, while smooth cleavage planes lower the fracture energy threshold, reducing strength and stiffness. These findings enhance understanding of biotite-influenced progressive rock damage and underpin stability monitoring and early-warning systems for underground engineering.
