BACKGROUND Renal cysts,especially the cortical type,are a prevalent renal pathology.Most cases are asymptomatic and detected incidentally during abdominal imaging examination for unrelated complaints.They are often be...BACKGROUND Renal cysts,especially the cortical type,are a prevalent renal pathology.Most cases are asymptomatic and detected incidentally during abdominal imaging examination for unrelated complaints.They are often benign,but they can rarely transform into cystic renal malignancies.When huge or complicated,especially with an abscess,it may become symptomatic from the renal capsular stretch or inflammation.The open surgical,endoscopic,laparoscopic,and robotic appro-aches are available for symptomatic renal cyst treatment.This paper aims to report our experience in the management of a case of a giant renal cyst abscess.CASE SUMMARY This is a 26-year-old housewife who was referred to the urology outpatient clinic with a history of left flank pain and swelling for 5 months,with the transabdo-minal ultrasound scan and computerized tomography scan findings of a huge left renal cortical cyst(Bosniak I).She had associated anorexia,weight loss,nausea,and intermittent fever,which on one occasion was severe and high-grade with chills and rigors.This warranted hospital admission,analgesic and antibiotic therapy.The physical examination was unremarkable at presentation except for a ballotable and mildly tender left lumbar cystic mass.Her vital signs were stable.There was leukocytosis with relative neutrophilia.Further review of the imaging films confirmed the diagnosis of a left giant renal cortical cyst abscess.She was counselled and had open surgical exploration,drainage of 300 mL of pus,cyst unroofing with marsupialization,and was discharged home on the 10th postoperative day.CONCLUSION Treatment of giant simple renal cortical cyst abscesses should be individualized depending on the pathology in question,the surgeon’s experience,patient preference and availability of facilities for endoscopic,laparoscopic or robotic modalities.Notwithstanding open surgical exploration,cyst unroofing and marsupialization are useful modalities in giant renal cortical cyst abscesses and are associated with patient satisfaction.展开更多
Immunization has long played essential roles in preventing diseases.However,the desire for precision delivery of vaccines to boost a robust immune response remains largely unmet.Here,we describe the use of acupoint de...Immunization has long played essential roles in preventing diseases.However,the desire for precision delivery of vaccines to boost a robust immune response remains largely unmet.Here,we describe the use of acupoint delivery of nanovaccines(ADN)to elicit dual-niche immunological priming.ADN can simultaneously stimulate mast cell-assisted maturation of dendritic cells at the acupoint and enable direct delivery of nanovaccines into the draining lymph nodes.We demonstrate that ADN not only provokes antigen presentation by lymph node-resident CD8α^(+)dendritic cells,but also induces the accumulation of nanovaccines in B-cell zones,amplifying antigen-specific cytotoxic T lymphocyte responses and immunoglobulin G antibody expression in draining lymph nodes.ADN also generates systemic immune responses by causing immune memory and preventing T-cell anergy in the spleen.Further supported by evoking effective antitumor responses and high-level antiviral antibodies in mice,ADN provides a simple yet versatile platform for advanced nanovaccination.展开更多
Mycobacterium tuberculosis(Mtb)employs multiple mechanisms,such as phagocytosis and autophagy,to evade innate immune clearance and establish infection.In the present study,we identified the ESX-1 secretion-associated ...Mycobacterium tuberculosis(Mtb)employs multiple mechanisms,such as phagocytosis and autophagy,to evade innate immune clearance and establish infection.In the present study,we identified the ESX-1 secretion-associated protein EspL,which promotes Mtb survival by inhibiting phagosome maturation and autophagy initiation.EspL knockout decreased Mtb intracellular survival,while EspL overexpression increased bacterial survival by interfering with phagocytosis and autophagy.EspL interacts with ULK1 and promotes its phosphorylation at Ser^(757),leading to the inhibition of autophagy initiation.Additionally,overexpression of EspL reduced antigen presentation and T-cell responses both in vitro and in vivo.Our findings revealed that EspL interferes with autophagy and antigen presenta-tion by suppressing ULK1 activation.These insights provide a novel understanding of Mtb pathogenicity.展开更多
BACKGROUND Groove pancreatitis(GP)is a rare focal chronic pancreatitis of the pancreaticoduodenal groove that is usually diagnosed in chronic alcoholics.However,it is often difficult to differentiate from pancreatic o...BACKGROUND Groove pancreatitis(GP)is a rare focal chronic pancreatitis of the pancreaticoduodenal groove that is usually diagnosed in chronic alcoholics.However,it is often difficult to differentiate from pancreatic or periampullary cancers.In this study,we report a case of GP with the rare symptom of severe gastric outlet obstruction.CASE SUMMARY A 68-year-old man presented to our institution with severe gastric outlet obstruction accompanied by vomiting persisted for 3 days,severe electrolyte imbalance,and altered mental status with cardiac arrest.Differential diagnosis of cancers of the ampulla of Vater or duodenum was difficult due to discordant findings from preoperative imaging and an endoscopic biopsy of the lesion.In addition,the gastric outlet obstruction did not improve with conservative treatment;therefore,pancreatoduodenectomy was performed.Postoperatively,the histological findings revealed multiple cystic lesions in the duodenal wall that were collectively diagnosed as GP.The patient underwent a follow-up 3 years postoperatively and was found to have no postoperative sequelae.CONCLUSION In this case of GP,pancreatoduodenectomy safely treated severe gastric outlet obstruction when a cancer diagnosis could not be excluded.展开更多

While diarrhea, abdominal pain, and weight loss are hallmarks, atypical manifestations can obscure the diagnosis. This report highlights an unusual presentation of CD to emphasize the need for comprehensive diagnostic strategies. Case Report: A 25-year-old male presented with peripheral edema, anorexia, and abdominal distension but lacked classic gastrointestinal (GI) symptoms. Laboratory findings included microcytic anemia and hypoalbuminemia, while imaging revealed ascites and bowel wall thickening. Elevated fecal calprotectin and positive Anti-Saccharomyces cerevisiae antibodies (ASCA) supported the diagnosis. Endoscopy confirmed ileocolic Crohn’s Disease (L3 + L4). Infliximab therapy resulted in marked clinical improvement. Discussion: This case underscores the complexity of atypical CD presentations. Early use of serological markers, imaging, and endoscopy guided the diagnosis. Recognition of CD’s diverse manifestations is critical for timely intervention. Conclusion: Atypical CD presentations require heightened clinical suspicion and a multidisciplinary approach to reduce diagnostic delays and improve patient outcomes.展开更多

This research investigates the design and optimization of a photovoltaic(PV)water pumping system to address seasonal water demands across five locations with varying elevation heads.The systemdraws water froma deep we...This research investigates the design and optimization of a photovoltaic(PV)water pumping system to address seasonal water demands across five locations with varying elevation heads.The systemdraws water froma deep well with a static water level of 30mand a dynamic level of 50m,serving agricultural and livestock needs.The objective of this study is to accurately size a PV system that balances energy generation and demand while minimizing grid dependency.Meanwhile,the study presents a comprehensivemethodology to calculate flowrates,pumping power,daily energy consumption,and system capacity.Therefore,the PV system rating,energy output,and economic performance were evaluated using metrics such as discounted payback period(DPP),net present value(NPV),and sensitivity analysis.The results show that a 2.74 kWp PV system is optimal,producing 4767 kWh/year to meet the system’s annual energy demand of 4686 kWh.In summer,energy demand peaks at 1532.7 kWh,while in winter,it drops to 692.1 kWh.Meanwhile,flow rates range from 11.71 m^(3)/h at 57 m head to 10.49 m^(3)/h at 70 m head,demonstrating the system’s adaptability to diverse hydraulic conditions.Economic analysis reveals that at a 5%interest rate and an electricity price of$0.15/kWh,the NPV is$6981.82 with a DPP of 3.76 years.However,a 30%increase in electricity prices improves the NPV to$10,005.18 and shortens the DPP to 2.76 years,whereas a 20%interest rate reduces the NPV to$1038.79 and extends the DPP to 6.08 years.Nevertheless,the annual PV energy generation exceeds total energy demand by 81 kWh,reducing grid dependency and lowering electricity costs.Additionally,the PV system avoids approximately 3956.6 kg of CO_(2) emissions annually,underscoring its environmental benefits over traditional pumping systems.As a result,this study highlights the economic and environmental viability of PV-powered water pumping systems,offering actionable insights for sustainable energy solutions in agriculture.展开更多

The expansion of renewable energy sources(RESs)in European Union countries has given rise to the development of Renewable Energy Communities(RECs),which aremade up of locally generated energy by these RESs controlled ...The expansion of renewable energy sources(RESs)in European Union countries has given rise to the development of Renewable Energy Communities(RECs),which aremade up of locally generated energy by these RESs controlled by individuals,businesses,enterprises,and public administrations.There are several advantages for creating these RECs and participating in them,which include social,environmental,and financial.Nonetheless,according to the Renewable Energy Directive(RED II),the idea of RECs has given opportunities for researchers to investigate the behavior from all aspects.These RECs are characterized by energy fluxes corresponding to self-consumption,energy sales,and energy sharing.Our work focuses on amathematical time-dependentmodel on an hourly basis that considers the optimization of photovoltaic-based RECs tomaximize profit based on the number of prosumers and consumers,as well as the impact of load profiles on the community’s technical and financial aspects usingMATLAB software.In this work,REC’s users can install their plant and become prosumers or vice versa,and users could change their consumption habits until the optimumconfiguration of REC is obtained.Moreover,this work also focuses on the financial analysis of the plant by comparing the Net Present Value(NPV)as a function of plant size,highlighting the advantage of creating a REC.Numerical results have been obtained investigating the case studies of RECs as per the Italian framework,which shows an optimal distribution of prosumers and consumers and an optimal load profile in which the maximum profitability is obtained.Optimization has been performed by considering different load profiles.Moreover,starting from the optimized configurations,an analysis based on the plant size is also made to maximize the NPV.This work has shown positive outcomes and would be helpful for the researchers and stakeholders while designing the RECs.展开更多

文摘This research investigates the design and optimization of a photovoltaic(PV)water pumping system to address seasonal water demands across five locations with varying elevation heads.The systemdraws water froma deep well with a static water level of 30mand a dynamic level of 50m,serving agricultural and livestock needs.The objective of this study is to accurately size a PV system that balances energy generation and demand while minimizing grid dependency.Meanwhile,the study presents a comprehensivemethodology to calculate flowrates,pumping power,daily energy consumption,and system capacity.Therefore,the PV system rating,energy output,and economic performance were evaluated using metrics such as discounted payback period(DPP),net present value(NPV),and sensitivity analysis.The results show that a 2.74 kWp PV system is optimal,producing 4767 kWh/year to meet the system’s annual energy demand of 4686 kWh.In summer,energy demand peaks at 1532.7 kWh,while in winter,it drops to 692.1 kWh.Meanwhile,flow rates range from 11.71 m^(3)/h at 57 m head to 10.49 m^(3)/h at 70 m head,demonstrating the system’s adaptability to diverse hydraulic conditions.Economic analysis reveals that at a 5%interest rate and an electricity price of$0.15/kWh,the NPV is$6981.82 with a DPP of 3.76 years.However,a 30%increase in electricity prices improves the NPV to$10,005.18 and shortens the DPP to 2.76 years,whereas a 20%interest rate reduces the NPV to$1038.79 and extends the DPP to 6.08 years.Nevertheless,the annual PV energy generation exceeds total energy demand by 81 kWh,reducing grid dependency and lowering electricity costs.Additionally,the PV system avoids approximately 3956.6 kg of CO_(2) emissions annually,underscoring its environmental benefits over traditional pumping systems.As a result,this study highlights the economic and environmental viability of PV-powered water pumping systems,offering actionable insights for sustainable energy solutions in agriculture.

文摘This paper investigated and analyzed the conservation and utilization of four local livestock breeds in Binzhou City:Wadi Sheep,Bohai Black Cattle,Wudi Donkey,and Lubei White Goat.Shortcomings in the protection and utilization of local germplasm resources were pointed out,and strategies and recommendations were proposed to promote high-quality development of livestock and poultry genetic resources in Binzhou,including building a solid germplasm foundation,standardizing production,and driving innovation.This paper provides references for the conservation,development,and utilization of local genetic resources in Binzhou City.
文摘Animal husbandry is an essential pillar sector in China.However,the wastewater including a mixture of feces,urine,and flushing water from livestock and poultry farming poses serious environmental risks if not properly managed or over-applied.This paper analyzes the existing challenges in the utilization of livestock manure,focusing on source control,process management,and end-use treatment.To address these issues,it proposes establishing a sustainable long-term mechanism.Key recommendations include enhancing source control,strengthening policy support to alleviate the financial burden on enterprises,aligning with market demands,intensifying the promotion of technologies and equipment development,and improving manure quality.In addition,advocating for grain-efficient animal husbandry and promoting diversified utilization through bio-chain approaches are essential.
文摘The expansion of renewable energy sources(RESs)in European Union countries has given rise to the development of Renewable Energy Communities(RECs),which aremade up of locally generated energy by these RESs controlled by individuals,businesses,enterprises,and public administrations.There are several advantages for creating these RECs and participating in them,which include social,environmental,and financial.Nonetheless,according to the Renewable Energy Directive(RED II),the idea of RECs has given opportunities for researchers to investigate the behavior from all aspects.These RECs are characterized by energy fluxes corresponding to self-consumption,energy sales,and energy sharing.Our work focuses on amathematical time-dependentmodel on an hourly basis that considers the optimization of photovoltaic-based RECs tomaximize profit based on the number of prosumers and consumers,as well as the impact of load profiles on the community’s technical and financial aspects usingMATLAB software.In this work,REC’s users can install their plant and become prosumers or vice versa,and users could change their consumption habits until the optimumconfiguration of REC is obtained.Moreover,this work also focuses on the financial analysis of the plant by comparing the Net Present Value(NPV)as a function of plant size,highlighting the advantage of creating a REC.Numerical results have been obtained investigating the case studies of RECs as per the Italian framework,which shows an optimal distribution of prosumers and consumers and an optimal load profile in which the maximum profitability is obtained.Optimization has been performed by considering different load profiles.Moreover,starting from the optimized configurations,an analysis based on the plant size is also made to maximize the NPV.This work has shown positive outcomes and would be helpful for the researchers and stakeholders while designing the RECs.
