St...Objectives/Hypothesis: To present a novel technique based on the combination of aesthetic and reconstructive techniques for the radical and safe excision of preauricular sinus along with minimal cosmetic impairing. Study Design: Case report, description of surgical method and brief overview of the literature. Methods: In a patient with bilateral preauricular sinus, we used a modified face-lift incision and SMAS-fixation instead of solely excising the sinus with skin ellipse. Results: Total removal of the bilateral fistulous systems could be achieved, along with a very aesthetically pleasing result. Conclusions: The surgical approach shown here allows a radical excision with only minimal aesthetic impairment using a shortened facelift-incision and SMAS-fixation.展开更多
BACKGROUND Pyoderma gangrenosum resulting from or associated with congenital preauricular fistula is rarely reported.CASE SUMMARY We report a rare case of pyoderma gangrenosum misdiagnosed as preauricular fistula infe...BACKGROUND Pyoderma gangrenosum resulting from or associated with congenital preauricular fistula is rarely reported.CASE SUMMARY We report a rare case of pyoderma gangrenosum misdiagnosed as preauricular fistula infection.To our knowledge,this is the first report to describe pyoderma gangrenosum originating from the site of preauricular fistula.The lesion continued expanding even after combined treatment of systemic antibiotics and thorough debridement.Taking into account the possibility of pyoderma gangrenosum,we applied soft care with normal saline and Vaseline gauze dressing.Systemic corticosteroids were not used until intestinal Clostridium difficile was controlled.No local recurrence was noted at the 12-mo follow-up.CONCLUSION This case highlights the necessity of considering rare diseases,such as pyoderma gangrenosum,when the preauricular sinus deteriorates with general management.The treatment strategy is mutually conflicting between pyoderma gangrenosum and infection of the preauricular sinus.展开更多

Unusual presentations can be subtle with some difficulty in obtaining diagnosis. A 27-year-old woman with 6 months history of recurrent right postauricular swelling which was initially thought to be a recurrent mastoid abscess presented. She had repeated incision and drainage with recurrent post auricular abscess. An incidental extravasation of fluid from an asymptomatic preauricular sinus during postauricular abscess cavity irrigation and a positive fistulogram revealed the diagnosis. Combined excision of the preauricular sinus tract and postauricular abscess cavity achieved a good outcome with patient been asymptomatic over a period of one-year follow-up. There should be a high index of suspicion of recurrent abscesses in close proximity with an ipsilateral preauricular sinus in the head and neck region.展开更多

A 13 year-old boy present...The preauricular sinus is a common finding in the pediatric population. An unusual case of a preauricular sinus that had as only manifestations recurrent postauricular abscesses is presented. A 13 year-old boy presented to the outpatients department with an infected cystic lesion and a hypertrophic scar below the left earlobe. He was operated 7 times in the past by different surgeons;however the postauricular abscess always recurred. Thorough clinical examination revealed a preauricular pit. The combined excision of the preauricular sinus and the remnants of the postauricular cyst resolved the problem and the patient has been asymptomatic ever since during a two years follow-up. Recurrent postauricular infected cysts may, albeit uncommonly, indicate an aberrant preauricular sinus, and the clinician needs to be aware of this possibility.展开更多
