Practice patterns among ophthalmic surgeons in treating concomitant oculoplastic conditions in preoperative period:A questionnaire-based study

BACKGROUND Addressing oculoplastic conditions in the preoperative period ensures both the safety and functional success of any ophthalmic procedure.Some oculoplastic conditions,like nasolacrimal duct obstruction,have been extensively studied,whereas others,like eyelid malposition and thyroid eye disease,have received minimal or no research.AIM To investigate the current practice patterns among ophthalmologists while treating concomitant oculoplastic conditions before any subspecialty ophthalmic intervention.METHODS A cross-sectional survey was disseminated among ophthalmologists all over India.The survey included questions related to pre-operative evaluation,anaesthetic and surgical techniques preferred,post-operative care,the use of adjunctive therapies,and patient follow-up patterns.RESULTS A total of 180 ophthalmologists responded to the survey.Most practitioners(89%)felt that the ROPLAS test was sufficient during pre-operative evaluation before any subspecialty surgery was advised.The most common surgical techniques employed were lacrimal drainage procedures(Dacryocystorhinostomy)(63.3%),eyelid malposition repair(36.9%),and ptosis repair(58.7%).Post-operatively,47.7%of respondents emphasized that at least a 4-week gap should be maintained after lacrimal drainage procedures and eyelid surgeries.Sixty-seven percent of ophthalmologists felt that topical anaesthetic procedures should be preferred while performing ocular surgeries in thyroid eye disease patients.CONCLUSION Approximately 50%of ophthalmologists handle prevalent oculoplastic issues themselves,seeking the expertise of an oculoplastic surgeon under particular conditions.Many ophthalmologists still favor using ROPLAS as a preliminary screening method before proceeding with cataract surgery.Eyelid conditions and thyroid eye disease are not as commonly addressed before subspecialty procedures compared to issues like nasolacrimal duct obstruction and periocular infections. 展开更多

Methods: Use the KTA knowledge translation model to guide evidence-based practice in nutrition management, and compare the nutritional status, cardiac function status, quality of life, and quality review indicators of chronic heart failure patients before and after the application of evidence. Objectives To investigate the barriers and facilitators influencing the sustainable implementation of evidence-based practice(EBP)for Peristomal Irritant Contact Dermatitis(PICD)based on the Consolidated Framework for Implementation Research(CFIR).Methods The sample consisted of 17 nurses from three urology wards at a tertiary hospital in Beijing,China,who had participated in the EBP.Guided by the CFIR,we identified constructs influencing the sustainability of implementation by conducting a directed content analysis of the 17 individual in-depth interviews.By rating,the valence and magnitude of each construct were determined.Results This study identified 19 factors across the five domains of the CFIR.Among these determinants,16 were identified as barriers,while three were recognized as facilitators.Common barriers across different contexts were identified,such as delayed evidence updating,the complexity of intervention components and steps,and incompatibility between the implementation process and clinical practices.In contrast,facilitators of intervention implementation were regular communication and feedback,support and drive from principals and a positive cultural atmosphere.In addition,we identified“regular evidence updating”as an independent construct outside of the CFIR,a finding that will provide key information for updating the CFIR framework.Conclusions Regular evidence updating is crucial for the sustainability of EBP implementation.Researchers should receive consistent feedback from practitioners and adjust or modify the EBP as necessary.Additionally,researchers must consider the compatibility or adaptability of the EBP’s content with clinical practice in formulating it rather than adding additional complexity to the original workflow. 展开更多

This study investigates and explores the teaching of higher vocational English courses for non-English majors from the perspective of core competencies.First,it outlines the current research status of core competencies and analyzes the national-level requirements for English core competencies.Subsequently,based on professional training objectives and curriculum standards,a questionnaire on the core competencies of English courses has been designed to understand students’perceptions and experiences regarding the development of their competencies in areas such as workplace communication,cross-cultural exchange,language thinking enhancement,and autonomous learning improvement.The survey results showed that the majority of students had a positive attitude towards the effectiveness of English courses in cultivating the aforementioned core competencies.However,some students also suggest improvements,such as increasing practical opportunities and strengthening cultural exchange.Based on this,the study further explores how to integrate the cultivation of core competencies into classroom teaching,including the use of a combination of formative and summative assessments,the design of teaching content and activities related to the workplace and cross-cultural communication,and the guidance of students to actively explore and apply their knowledge,thereby enhancing their English language application abilities.This study has certain reference value for the optimization of the higher vocational English curriculum system and the reform of teaching methods. 展开更多

With the iteration and upgrading of medical technology and the continuous growth of public health demands,the quality of nursing services has become a core indicator for measuring the effectiveness of the medical system.The clinical practice ability of nursing staff is directly related to the safety of patient diagnosis and treatment and the rehabilitation process.However,the current clinical nursing talent training model is facing bottlenecks such as limited practical scenarios and fragmented case cognition.This study focuses on the teaching application of augmented reality(AR)technology in hospital Settings and systematically reviews the research progress on the improvement of clinical practice ability of trainee nurses based on the AR immersive teaching model.By constructing a clinical teaching scenario that integrates virtual and real,AR technology can dynamically simulate complex case handling processes and enhance nursing students’three-dimensional cognition of condition assessment,operation norms,and emergency plans.Hospitals,as the core base for practical teaching,can effectively shorten the connection cycle between theoretical teaching and clinical practice by integrating AR technology,improve the clinical practice level of trainee nurses,and provide an innovative model for optimizing the path of clinical nursing talent cultivation. 展开更多

Application of Ideological and Political Theories Teaching in Clinical Practice Course for Nursing Interns Based on Nightingale Spirit

[Objectives]To explore the effectiveness of integrating ideological and political education based on the Nightingale spirit into the clinical practice curriculum for nursing interns.[Methods]A total of 90 questionnaires were distributed and analyzed among nursing interns and patients to evaluate multiple dimensions,including the interns satisfaction with the internship,mastery of clinical knowledge,ideological and political literacy,professional ethics,and social responsibility.[Results]The overall internship satisfaction of nursing interns reached 98.9%,indicating that the course design and teaching model were widely recognized by students.Secondly,the interns demonstrated excellent mastery of clinical knowledge,with 94.0%of students stating they had fully grasped the clinical knowledge taught during the internship,an increase of 18.6%compared to the previous cohort.In terms of ideological and political literacy,98%of students had a better understanding and application of the Nightingale spirit,an increase of 16.2%compared to the previous cohort.Additionally,the sense of professional identity was significantly enhanced,with 95.8%of students showing a stronger sense of identity and responsibility towards the nursing profession,an increase of 14.0%compared to the previous cohort.Patients also gave high evaluations of the interns performance,with 98.8%expressing satisfaction with the students attitude and performance in nursing care.[Conclusions]The curriculum model based on the integration of Nightingale spirit has effectively improved the professional quality,ideological and political quality and professional identity of nursing interns,and has been highly recognized by patients.It not only provides strong support for the practice of combining ideological and political work with professional courses in nursing education,but also provides a reference for the optimization of clinical practice teaching mode of nursing interns in the future. 展开更多

Establishing and Sustaining Academic and Local Health Collaborations: A Case Study of Practice-Based Teaching and Workforce Development

These gaps required innovative probl... The pandemic highlighted significant gaps in the public health infrastructure impacted by shortages of public health workers, an undertrained workforce, and years of disinvestment. These gaps required innovative problem-solving by public health agencies (PHAs), including local health departments (LHDs), to respond to rapidly changing community conditions during and after the pandemic. Many schools and programs of public health (SPPH) worked with PHAs to mobilize public health (PH) students through practice-based teaching (PBT). Current research indicates PBT benefits all stakeholders—PHAs, students, faculty, SPPH, and ultimately the community served. However, more research is needed on the utility of PBT in addressing a community’s systemic public health issues, the extent to which the academic-community collaboration enhances a PHA’s capacity, and the impact of the pedagogy on preparing the workforce for an evolving PH landscape. This paper examines the process of a semester-long PBT course, guided by the PBT STEPS framework, which includes five steps from collaboration to implementation to evaluation of a PBT course. The collaborating PHA and its student group addressed community trauma and resilience issues during the semester. Additionally, it examines the longer-term impacts after the semester for the PHA, community, and the workforce by 1) conducting a formative evaluation to understand needs and gaps in the community;2) redesigning an intervention that merged the results of the formative evaluation with the intervention developed during the semester;and 3) securing funding and resources for intervention sustainability. Through the documentation of a post-course partnership between an LHD and faculty at a large school of public health, this case study illustrates the potential for PBT to lay the foundation for ongoing research that supports more impactful interventions for PHAs while bolstering the workforce abilities of students as future practitioners. 展开更多

Reversible data hiding(RDH)enables secret data embedding while preserving complete cover image recovery,making it crucial for applications requiring image integrity.The pixel value ordering(PVO)technique used in multi-stego images provides good image quality but often results in low embedding capability.To address these challenges,this paper proposes a high-capacity RDH scheme based on PVO that generates three stego images from a single cover image.The cover image is partitioned into non-overlapping blocks with pixels sorted in ascending order.Four secret bits are embedded into each block’s maximum pixel value,while three additional bits are embedded into the second-largest value when the pixel difference exceeds a predefined threshold.A similar embedding strategy is also applied to the minimum side of the block,including the second-smallest pixel value.This design enables each block to embed up to 14 bits of secret data.Experimental results demonstrate that the proposed method achieves significantly higher embedding capacity and improved visual quality compared to existing triple-stego RDH approaches,advancing the field of reversible steganography. 展开更多

BACKGROUND System based practice(SBP) milestones require trainees to effectively navigate the larger health care system for optimal patient care. In gastroenterology training programs, the assessment of SBP is difficult due to high volume, high acuity inpatient care, as well as inconsistent direct supervision. Nevertheless,structured assessment is required for training programs. We hypothesized that objective structured clinical examination(OSCE) would be an effective tool for assessment of SBP.AIM To develop a novel method for SBP milestone assessment of gastroenterology fellows using the OSCE.METHODS For this observational study, we created 4 OSCE stations: Counseling an impaired colleague, handoff after overnight call, a feeding tube placement discussion, and giving feedback to a medical student on a progress note. Twentysix first year fellows from 7 programs participated. All fellows encountered identical case presentations. Checklists were completed by trained standardized patients who interacted with each fellow participant. A report with individual and composite scores was generated and forwarded to program directors to utilize in formative assessment. Fellows also received immediate feedback from a faculty observer and completed a post-session program evaluation survey.RESULTS Survey response rate was 100%. The average composite score across SBP milestones for all cases were 6.22(SBP1), 4.34(SBP2), 3.35(SBP3), and 6.42(SBP4)out of 9. The lowest composite score was in SBP 3, which asks fellows to advocate for cost effective care. This highest score was in patient care 2, which asks fellows to develop comprehensive management plans. Discrepancies were identified between the fellows’ perceived performance in their self-assessments and Standardized Patient checklist evaluations for each case. Eighty-seven percent of fellows agreed that OSCEs are an important component of their clinical training,and 83% stated that the cases were similar to actual clinical encounters. All participating fellows stated that the immediate feedback was "very useful." One hundred percent of the fellows stated they would incorporate OSCE learning into their clinical practice.CONCLUSION OSCEs may be used for standardized evaluation of SBP milestones. Trainees scored lower on SBP milestones than other more concrete milestones. Training programs should consider OSCEs for assessment of SBP. 展开更多

Objectives Even though guidelines are available to guide dysphagia identification and management practice,there is still a gap between evidence and practice,which requires improvement.The purpose of this study was to determine the effect of using tailored,multifaceted strategies to improve evidence-based post-stroke dysphagia identification and management practice in a community hospital.Methods Guided by the Knowledge to Action framework,the tailored,multifaceted strategies were developed and implemented for 5 months in a community hospital using a before-and-after study design.These strategies consisted of training intervention,policy intervention,and audit and feedback intervention.Nurses’level of knowledge and adherence,were collected in March 2019 and again in January 2020.Patients’quality of life and satisfaction were evaluated during the pre-intervention period(between February 2019 and April 2019)and the post-intervention period(between November 2019 and January 2020).Results A total of 55 patients with post-stroke dysphagia(28 in the pre-intervention period and 27 in the post-intervention period)and 17 registered nurses were recruited.Following implementation,there were statistically significant improvements in patients’outcomes(quality of life and satisfaction)and nurses’outcomes(level of knowledge and adherence).Conclusions This study assists in closing the research-practice gap by using tailored,multifaceted strategies to increase the use of evidence-based nursing care for dysphagia identification and management practices. 展开更多

IBP and its practice in health care promote also many advantages, such as improvements of practices based on the attitudes and cognitive ideas. This study aims to assess the inquiry based on evidence (IBP) and its practices in two Health Care Facilities (HCFs) of Bujumbura to help the practitioners to understand its importance. <strong>Methods:</strong> A cross-sectional study design was used to analyze the importance of IBP and its practice in these two hospitals. The probability-sampling technique was used also to select 104 nurses from the Military Hospital of Kamenge and 55 nurses from the Van Norman Clinic. A questionnaire was used to collect data with two mains components, demographic data and knowledge and attitudes addressing the following parameters: evidence practice during inquiry, nursing theory, current analysis in nursing care oriented the evidence, prioritization of care, rational diagnostic, monitoring and assessment. <strong>Results:</strong> The findings from this study revealed a poor knowledge and attitude among participants towards Inquiry Based Practice. In all variables, participants were scoring less than 10%. However, majority of participants (76.5%) know the indicators of patients’ satisfaction with nursing interventions through survey-based practice and 74.1% argued to analyze their information collected. <strong>Conclusion:</strong> This study revealed a weak awareness on IBP and its importance during nursing practice among participants as for almost all variables, participants were scoring less than 10%, except for the indicators of patients’ satisfaction with nursing interventions through survey-based practice (76.5%). Therefore, in-service training and curriculum revision had been highlighted and recommended another to provide the best rational diagnosis and achieve the patient’s outcomes. 展开更多

Objective: Reflection is viewed as the most significant skill and should be advocated for lifelong learning. In order to grasp the process of reflection and embrace the reflective skill, one of the authors of this paper reflects on a practice-based issue in relation to the communication between doctors and nurses, to gain new understanding and thus improve the authors' clinical practice.Methods: Smyth's framework with four stages is utilized as an ideal framework to guide the author's reflection on the practice-based issue to free the author from her entrenched assumptions and the oppressive forces that limit her practice.Results: The espoused and enacted theory, together with the critical reflection theory, are used to explore the values and beliefs that essentially govern the author's practice and how these are distinct from the author's worldviews. Following this, critical emancipatory reflection is undertaken to explore the dominant power structures within the author's workplace. Furthermore, given the hegemonic and chaotic working context of this issue, hegemony and symbolic interaction theory are applied to unearth the various hidden constraining and oppressive forces. Additionally, socialization theory is utilized to help the author achieve professional identity.Conclusions: Reconstructing the practice-based issue empowers the author to realize that in the future, she should act as a reflective practitioner, creating a daily habit, staying alert to practice, seeing things freshly, finding support systems, improving communication skills, conducting reflective research, and reifying reflective practice. Ultimately, the author will be sufficiently equipped to be able to transform her practice and change its outcomes. 展开更多
