Objective:To investigate the impact of partial gastrectomy on gastrointestinal function in the treatment of patients with early gastric cancer.Methods:A sample of 20 patients with early-stage gastric cancer treated from January 2022 to January 2023 was randomly divided into two groups.Group A underwent partial gastrectomy,while Group B underwent distal subtotal gastrectomy.Surgical outcomes,complication rates,BMI indices,and quality of life were compared.Result:All surgical outcomes of patients with early gastric cancer in group A were better than those in group B(P<0.05);the postoperative complication rate for early gastric cancer in group A was lower than that in group B(P<0.05);the BMI index for patients with early gastric cancer in group A was higher than that in group B at different times(P<0.05);the postoperative quality of life(SF-36)score of group A for early gastric cancer was higher than that of group B(P<0.05).Conclusion:Partial gastrectomy for patients with early gastric cancer can increase BMI,optimize surgical outcomes,improve gastric function,and enhance the quality of life for gastric cancer patients. 展开更多

BACKGROUND Gastric ulcer perforation is a critical condition that can lead to significant morbidity and mortality if not promptly addressed.It is often the result of chronic peptic ulcer disease,which is characterized by a breach in the gastric wall due to ulceration.Surgical intervention is essential for managing this life-threatening complication.However,the optimal surgical technique remains debatable among clinicians.Various methods have been employed,including simple closure,omental patch repair,and partial gastrectomy,each with distinct advantages and disadvantages.Understanding the comparative efficacy and postoperative outcomes of these techniques is crucial for improving patient care and surgical decision-making.This study addresses the need for a comprehensive analysis in this area.AIM To compare the efficacy and postoperative complications of different surgical methods for the treatment of gastric ulcer perforation.METHODS A retrospective analysis was conducted on 120 patients who underwent surgery for gastric ulcer perforation between September 2020 and June 2023.The patients were divided into three groups based on the surgical method:Simple closure,omental patch repair,and partial gastrectomy.The primary outcomes were the operative success rate and incidence of postoperative complications.Secondary outcomes included the length of hospital stay,recovery time,and long-term quality of life.RESULTS The operative success rates for simple closure,omental patch repair,and partial gastrectomy were 92.5%,95%,and 97.5%,respectively.Postoperative complications occurred in 20%,15%,and 17.5%of patients in each group,respectively.The partial gastrectomy group showed a significantly longer operative time(P<0.001)but the lowest rate of ulcer recurrence(2.5%,P<0.05).The omental patch repair group demonstrated the shortest hospital stay(mean 7.2 days,P<0.05)and fastest recovery time.CONCLUSION While all three surgical methods showed high success rates,omental patch repair demonstrated the best overall outcomes,with a balance of high efficacy,low complication rates,and shorter recovery time.However,the choice of the surgical method should be tailored to individual patient factors and the surgeon’s expertise. 展开更多

Objective:To summarize the perioperative nursing strategies of a patient undergoing laparoscopic retrieval and partial gastrectomy because of the gastric migration of an intrauterine device.Methods:The following individualized care strategies were implemented:preoperative psychological optimization using cognitive-behavioral counseling,modified bowel preparation,accelerated postoperative recovery through early ambulation and stepwise nutritional advancement,strict condition monitoring of the patient,multimodal step analgesia,early tube removal,integration of medical and nursing checkups,and nursing measures using traditional Chinese medicine.Results:The patient was discharged from the hospital smoothly after 8 days.A follow-up visit was conducted one-week post-discharge,during which the patient’s general condition was assessed as good,and no complications were reported.Conclusion:The use of individualized nursing strategies has aided in the smooth postoperative recovery of patients with ectopic intrauterine devices while improving the quality and safety of nursing. 展开更多

Objective:To observe the clinicopathological characteristics of gastric stump cancer(GSC)and evaluate the benefitsof radical surgery of GSC.Methods:The clinicopathological characteristics and postoperative survival time of 37 GSC patientswho underwent surgery were investigated retrospectively.The survival time was compared according to the type of surgicaloperation(radical resection vs palliative operation).Twenty-one cases that received radical resection were analyzed based on thepTMN stage.Survival curves were traced by using Kaplan-Meier methods.Results:Most GSC(32/37)was detected in patientswho had received Billroth Ⅱ reconstruction after partial gastrectomy for benign gastric disease.The lesser curvature side and thesuture line of anastomosis were the most frequent sites where GSC occurred(27/37).Differentiated adenocarcinoma was thedominant histopathological type(24/37).The postoperative 5-year survival rate of early stage GSC patients(n=9)was significantlyhigher than advanced stage GSC(n=12)(55.6% vs 16.5%,x_L^2=11.48,P<0.01).Five-year survival rate of 21 GSC patients withradical resection were 75%(3/4)for stage Ⅰ,60%(3/5)for stage Ⅱ,14.2%(1/7)for stage Ⅲ,and 0%(0/5)for stage Ⅳ respectively.The median survival time of 21 GSC patients who underwent radical resection was longer than those undergoing palliative op-eration(43.0 m vs 13.0 m,X_L^2=36.31,P<0.01),the median survival time of stage Ⅳ patients with radical resection was 23.8months.Conclusions:Without remote metastasis,radical resection for GSC is possible,and is an effective way to improve theprognosis of GSC.Even in stage Ⅳ GSC,radical resection can still prolong the survival time.It is necessary for the patients withbenign gastric diseases who received partial gastrectomy to carry out the endoscopy follow-up,especially in patients with BillrothⅡ reconstruction procedure at 15-20 years. 展开更多
